Abstract

相关技术的描述
烧伤是最严重形式的创伤中的一种。烧伤面积越大,后果越严重且恶劣的结局及死亡机会越高。每年烧伤患者的数量超过2百万以上,而治疗的费用每年则超过10亿美元。烧伤和烫伤是1-18岁儿童受伤和死亡的第三位主要原因。过去十年,烧伤死亡数有所下降,主要是因为早期和适当的液体复苏、早期和积极的营养支持、改进了对感染的控制、改善的伤口护理/伤口愈合和体液调节。
烧伤病人康复中最重要的是伤口愈合,及其临床结果。已显示烧伤后早期创伤切除术和组织移植改善了代谢亢进反应和存活。可采用自身的皮肤移植于切除的伤口处(供体-位点),但对烧伤非常大的患者而言这并非是有效的治疗方法。在这些病例中,已使用了合成的皮肤材料或尸体皮肤。
应将伤口闭合与伤口覆盖相区别。伤口闭合物质是烧伤床生物学上接受的,且将永久地掺入到愈合的伤口中。另一方面,伤口覆盖物质依赖于掺入到伤口凝块及生长的肉芽组织而粘附;这种现象是许多伤口覆盖物质的特征。通常伤口覆盖材料不能生物降解,因此只能是表皮的临时替代物。因此必须通过上皮重新形成或皮肤移植以患者的皮肤替代伤口覆盖材料。对临时覆盖而言,伤口上不能有细菌,且应足够浅从而可以预计能在3周内完全愈合。表皮附件的上皮细胞生长并替代受损的上皮,并逐渐遮蔽伤口覆盖物质。因此,用于浅表二度烧伤的伤口覆盖材料的主要目的是限制微生物侵入伤口床(微生物屏障),从而预防感染,并限制空气的进入以最大程度减轻伤痛。
在大面积烧伤患者伤口由自身皮肤闭合之前,也将伤口覆盖物质用于深二度或三度烧伤。已确定了最理想的伤口覆盖材料。但伤口闭合物质需要模拟正常真皮和表皮。具体说,优良的伤口闭合材料的条件是:a)提供针对细菌和其它微生物的无毒、抗菌、无炎症和无抗原性的屏障;b)提供热和水传导的正常速率;c)提供对创伤床的快速、均一和密切的粘附;e)保持弹性和长期的持久性;f)允许皮肤生长;和g)伴随伤口挛缩提供与裂口厚度自体移植物相当的长期机械性和整容功能。
INTEGRATM、ALLODERMTM或BIOBRANETM表现出非常好的生物相容性和愈合特性。但由于这些材料价格昂贵,限制了它们的广泛应用。尸体皮肤是一种相对有效和价廉的伤口覆盖方法。但HIV、CMV、HSV和肝炎传播的危险是其最受关注的问题,并因此限制了尸体皮肤的应用。
胎儿羊膜具有如下多种优点而使该材料可用作伤口覆盖材料,包括:a)免疫原性低;b)无毒、抗菌和无炎症;c)无HIV、HSV或CMV感染;d)无限量(这使胎儿羊膜组织成为目前皮肤替换物的价廉的替代品);e)长度、直径和厚度的可变性;和f)具有内源性机械性成分(如胶原、层粘连蛋白和纤连蛋白)因此能确保机械稳定性、生长支持并与正常人皮肤可能相似。
从生物化学角度来看,高温损伤是一种非常严重的创伤形式,其伴有特征为心排血量高、耗氧增加、免疫应答和蛋白质与脂肪分解代谢遭到损伤的代谢亢进反应[34]。烧伤产生和释放凝血噁烷和前炎症细胞因子支持这种易损的代谢亢进状态[35-37]。所以伤口愈合对烧伤患者的恢复和存活非常重要的[22,38-39]。合成代谢剂(如生长激素和类胰岛素生长因子-I)已显示能减弱这种代谢亢进反应并改善伤口愈合[35,39-41]。
类胰岛素生长因子-I,一种大小为7.5kD的小型多肽,是一种已显示出能在烧伤后改善代谢[35]、肠粘膜功能[42]和蛋白质缺失[43]的合成代谢剂。IGF-1通过缓解脂肪外的体重丧失、免疫应答的损伤、急性期反应和通过提高伤口的愈合,来调节处于代谢亢进状态中的生长激素的作用[35,38,44-47]。IGF-I治疗通过刺激胶原形成和成纤维母细胞与角质形成细胞有丝分裂,而改善伤口愈合[40,41,48]。但存在副作用如低血糖、精神状态的改变、水肿、疲乏和头痛,限制了IGF-I在烧伤治疗中的治疗性应用[49,50]。这些副作用极可能是因为游离IGF-I的超生理剂量(这是生物效力所需的)[49,50]。
对用于基因送递的合适载体的选择是非常重要的[1,2]。病毒尤其是腺病毒,因它们的特异性转染能力,已被广泛用作基因送递载体[1-3]。但病毒具有与病毒感染相关的毒性、免疫损伤和可能的突变或致癌作用,使此法具有潜在的危险性[1]。因此用脂质体作为送递系统成为有吸引力的模型,因为它们无病毒成分、稳定且能与细胞膜相互作用[4]。将阳离子特性物质加到标准脂质体结构中并掺入胆固醇,以及将巨细胞病毒(CMV)启动子掺入到cDNA结构中用于基因转移,增加了转基因表达的效力和水平,达到了相当于采用腺病毒结构所达到的效力和水平[4,5]。
现有技术在提高伤口愈合方法和强化的伤口覆盖材料上存在缺陷。本发明满足了本领域中这一长期存在的需要和愿望。
发明概述
本发明描述了一种采用携带编码生长促进剂基因的脂质体来提高伤口愈合的方法。本发明还描述了一种浸透了携带有表达生长因子基因的脂质体的强化伤口覆盖材料,来改善伤口的愈合。本发明的一个目的是降低代谢亢进反应,并因此改善临床结果和增加创伤尤其是高温损伤后的存活性。
本发明描述了直接将脂质体性基因构建物掺入到伤口和/或掺入覆盖材料来改善伤口修复并增加伤口覆盖物质的功能。本发明还描述了在全层伤口修复中,采用强化的胎儿羊膜联用表达生长因子的脂质体性基因构建物作为暂时的伤口覆盖物。胎儿羊膜与最近所使用的材料(如IntegraTM、BiobraneTM、AlloderrnTM)相比具有优势,是一种能改善临床结果的有效和安全的方法。

发明详述
本发明显示了采用脂质体将表达生长促进剂的载体送递到大鼠创伤(尤其是烧伤伤口)的优点。本发明还进一步比较了各种伤口覆盖材料如胎儿羊膜、人皮肤和各种可购得的合成材料在约克小种猪上的功能。
具体说,本发明描述了将编码生长促进因子(如胰岛素类生长因子-I(IGF-I)或角质形成细胞生长因子(KGF))的脂质体性基因构建物直接引入伤口床或伤口覆盖物中,并与未用脂质体处理的伤口作比较。通过在4个月过程中每周测定吸收率和愈合时间、用组织学方法检验伤口收缩、组织结构学、覆盖物排斥的免疫标记物和代谢亢进反应的程度,来评估脂质体基因治疗的效果。
根据本文所报道的发现,即通过直接将脂质体载体注射入伤口或伤口覆盖材料(随后敷加到伤口)来送递一种或多种表达生长因子的基因的基因治疗法,将能革新和提高现有的对烧伤、皮肤溃疡和皮肤移植的治疗方法。
本发明还涉及提高伤口愈合方法和一种强化的伤口覆盖物。
本发明还涉及提高伤口愈合的方法,包括如下步骤:在伤口中注射入脂质体,该脂质体包含有至少一种编码生长促进剂的基因。
本发明还涉及一种提高伤口愈合的方法,其包括如下步骤:用伤口覆盖材料覆盖伤口,其中此伤口覆盖材料用脂质体浸透,该脂质体包含有至少一种编码生长促进剂的基因。
另一方面,本发明还涉及一种提高伤口愈合的方法,所述方法包括如下步骤:用伤口闭合材料覆盖伤口,其中所述的伤口闭合材料用脂质体浸透,所述的脂质体包含有至少一种编码生长促进剂的基因。
另一方面,本发明还涉及一种强化的伤口敷料,其包括:伤口覆盖材料;和包含有至少一种编码生长促进剂基因的脂质体。通常,通过注射将包含生长促进剂的脂质体送递到伤口覆盖材料中,这可以在将伤口覆盖材料敷加到伤口之前或之后进行。
在本发明的另一方面还涉及,提供一种用于提高伤口愈合的组合物,其包括:脂质体,该脂质体包含有至少一种编码生长促进剂的基因;和药学上可接受的载体。可将此实例的组合物包装,从而使该组合物易于装填入针筒或可直接包装在针筒中。
在这些实施例中,可用本发明组合物和方法治疗的典型伤口包括烧伤、化学损伤、切伤、手术伤口或擦伤。优选的脂质体是含胆固醇的阳离子脂质体。生长促进剂通常是生长激素、类胰岛素生长因子-I、角质形成细胞生长因子、成纤维母细胞生长因子、表皮生长因子、血小板衍生的生长因子或转化生长因子-β。较佳地,当生长促进剂是类胰岛素生长因子-I(IGF-I)时,脂质体中编码IGF-I的基因的浓度约为2.2μg/ml脂质体。
典型的伤口覆盖材料包括人胎儿羊膜、人胎绒毛膜、人尸体皮肤、合成的皮肤和本领域技术人员已知的其它材料。典型的伤口闭合材料包括人胎儿羊膜、人胎绒毛膜、人同基因型(syngeneic)皮肤、人异体移植皮肤和本领域技术人员已知的其他材料。

“DNA分子”指聚合形式的脱氧核糖核苷酸(腺嘌呤、鸟嘌呤、胸腺嘧啶、或胞嘧啶),无论是单链形式还是双链螺旋形式。这一术语只指分子的一级和二级结构,并不将它限制于任何特定的三级形式。所以,本术语其中包括在线性DNA分子(如限制片段),病毒,质粒和染色体中发现的双链DNA。在论述时,结构按常例仅给出DNA非转录链的5’到3’的序列(即,具有与mRNA同源序列的链)。
“载体”指的是一种复制子,如质粒,嗜菌体或粘粒,它们可以与另一DNA片段相连,从而导致所连接片段的复制。“复制子”是一种基因元件(如质粒,染色体,病毒),其功能是作为体内DNA复制的自主单元,即能在其自身控制下复制。“复制起点”是参与DNA合成的DNA序列。“表达控制序列”指控制和调节另一DNA序列转录和翻译的DNA序列。细胞内当RNA聚合酶将编码序列转录为mRNA,然后mRNA被翻译成该编码序列所编码的蛋白时,编码序列与转录和翻译控制序列操作性连接并在它们的控制下。
通常,含有启动子序列的表达载体是与宿主配合使用的,其中启动子序列能促进插入的DNA片段有效转录和翻译。表达载体典型的包含复制起点,启动子,终止子,以及那些能在转化细胞中提供表型选择的特定基因,它们通常被称为“选择标记基因”或“可选择的标记”。被转化的宿主可以按本领域已知的方法发酵和培养,以达到最佳细胞生长。
DNA“编码序列”是指当置于适当的调节序列控制下,能在体内转录和翻译为多肽的DNA双链序列。编码序列的边界由5‘(氨基)端的起始密码子和3’(羧基)端的翻译终止子界定。一个编码序列可以包括(但不限于):原核序列,源自真核mRNA的cDNA,真核(如哺乳动物)DNA的基因组DNA序列和甚至合成的DNA序列。聚腺苷酸化信号和转录终止子序列通常位于编码序列的3’端。“cDNA”指的是复制-DNA或互补-DNA,它是mRNA转录物的反转录反应产物。“外显子”是自基因座转录的表达的序列,而“内显子”是来自基因座的非表达序列。
转录和翻译控制序列是DNA调节序列,如启动子,增强子,聚腺苷酸化信号,终止子等,它们提供编码序列在宿主细胞中的表达。“顺式元件”是一种核苷酸序列,也称为“共有序列”或“基序”,它可与能上调和下调特定基因座表达的其它蛋白质相互作用。“信号序列”也可以包括在编码序列内。这个序列编码编码位于多肽N-端的信号肽,该信号肽与宿主细胞联系并且将该多肽引向合适的细胞部位。已发现,信号序列可与许多原核和真核生物的天然蛋白质相连。
“启动子序列”是一种DNA调节区域,它能在细胞内结合RNA聚合酶并引发下游(3’方向)编码序列的转录。出于限定本发明的目的,启动子序列的边界在其3’端是转录起始位点,并向上游(5’方向)延伸包括引发高于背景的可检测水平转录所必需的最少数目的碱基或元件。在启动子序列中,可找到转录起始位点和负责与RNA聚合酶结合的蛋白结合区域(共有序列)。真核启动子经常,但并非总是,含有“TATA”盒和“CAT”盒。原核启动子除了-10和-35共有序列外,还含有SD(Shine-Dalgarno)序列。
通常,“基因”指包括启动子和操作性连接于编码序列的调节元件。这些启动子序列和调节元件可以是该基因的天然启动子和/或调节元件,或者也可以是异源的。DNA构建物中的“异源”区域是在较大的DNA分子中可看作是相同的一段DNA,这段DNA在天然情况下不与该较大DNA分子相连。因此,当该异源区域编码哺乳动物基因时,该基因侧接的DNA(如启动子序列和/或调节序列)往往是在来源生物基因组中并不侧接于该哺乳动物基因组的DNA。另一例子中,编码序列是其自身在自然中不存在的一种构建物(如,在基因组编码序列包括内含子情况下的cDNA,或包含了与自然基因不同密码子的合成序列)。等位(基因)变异或天然发生的突变事件并不产生这里所说DNA的异源区域。
“DNA重组技术”指合并两个异源DNA分子的技术,通常这是体外连接不同生物体DNA的结果。DNA重组分子通常在基因工程实验中产生。同义的术语包括“基因拼接”,“分子克隆”和“基因工程”。这些操作的产物是“重组体”或“重组分子”。
当这种DNA被引入到某细胞内时,该细胞就被外来或异源DNA“转化”或“转染”了。转化DNA可整合或不整合(共价连接)到细胞的基因组中。例如在原核生物、酵母和哺乳动物细胞中,转化DNA可保持在附加元件(如载体或质粒)上。对于真核细胞来说,稳定转化的细胞是转化DNA已整合到染色体中的细胞,这样通过染色体的复制可遗传给子代细胞。这种稳定性表现为该真核细胞能够建立由含有转化DNA的子代细胞群组成的细胞系和细胞克隆。“克隆”是由一个细胞或祖先细胞通过有丝分裂而衍生出的一群细胞。“细胞系”是能在体外稳定生长许多代的一个原代细胞的克隆。一种生物,如植物或动物,用外源DNA转化后称为“转基因生物”。
本文所用的术语“羊膜”指围绕胚胎和衍生自外胚层和中胚层组织的外胚层性薄膜。
本文所用的术语“绒毛膜”指最终成为胎盘一部分的胚外膜的最外层,除呼吸功能外,它还提供营养并除去废物。
本文所用的术语“脂质体”指脂质双层膜(由磷脂人工制备)构成的小泡。可将DNA、蛋白质和其他物质包裹在脂质体中,其可通过与胞质膜融合引入动物细胞。本文所用的术语“胆固醇-阳离子脂质体”特指含有胆固醇的脂质体。
本文所用的术语“生长促进剂”指能促进生长的化合物。
本文所用的术语“吸收率”指供体组织对宿主组织的“康复率”或“接收受率”。
具体考虑了用本发明所述的脂质体制备药物组合物。此时,该药物组合物含有编码生长因子的基因、本发明的脂质体和药学上可接受的载体。本领域的普通技术人员无过多实验,即能不难确定本发明脂质体的适当剂量和给药途径。当用于治疗的患者时,将本发明治疗有效量(即能有效送递编码宿主生长增强剂适当量的DNA)的所述脂质体载体给予该患者或动物。通常以注射形式给予到伤口或伤口覆盖材料中,但也可用其他适用的方式给药。剂量和给药方案取决于伤口的特质(组织损伤的严重程度)和伤口的大小、患者的病史和其他因素。所给的脂质体的量通常取决于伤口的面积,且脂质体注射部位分开约为4cm。继续治疗从而使效果最优同时抗衡治疗的副作用。参见Remington’sPharmaceutical Science,第17版(1990)Mark Publishing Co.,Easton,Penn;和Goodman和Gilman的The Pharmacological Basis of Therapeutics第8版(1990)Pergamon Press;本文将它们纳入作为参考。对局部给药而言,通常将脂质体与药学上可接受的载体一起制备成单位剂量的注射形式(溶液、悬浮液、乳液)。较佳地,这些载体是无毒和无治疗作用的。这些载体的例子包括水、盐水、Ringer溶液、葡萄糖溶液和5%人血清白蛋白。也可用非水性的载体如固定油和油酸乙酯。载体可含有极少量的添加剂,如能提高等渗性和化学稳定性的物质(如缓冲液和防腐剂)。通常可以用大约1μg-10μg浓度的这些载体制备脂质体。
给出以下的实施例是为了说明本发明的各种具体实施方案,并不意味以任何方式限制本发明。
实施例1
实验动物-大鼠
将成年雄性Sprague-Dawley大鼠(350-375g)置于铁底的笼子中,关养在控温的12小时亮-暗循环的房间内。在开始盲检实验前7天,让动物熟悉它们的环境。在研究的整个过程中,所有动物都接收相同量的Sustacal液体饲料(MeadJohnson Nutritionals,Evansville,Indiana,USA)并任意饮水。每只大鼠接收60%总体表面积(TBSA)的全层皮烧伤。然后如下随机将烧伤的大鼠分组:
a)2组,接收含胆固醇阳离子脂质体(180μl盐水中有20μl脂质体,数量(n)=28)或盐水(对照,200μl,n=28);
(b)2组,每周在烧伤边缘的一个注射部位接收皮下注射含有2.2μgIGF-IcDNA构建物和0.2μg由CMV启动子驱动的β-半乳糖苷酶的报道基因(Lac ZcDNA构建物)的脂质体(180μl盐水中有10μl脂质体)(n=12)(图1),或每周在烧伤边缘的两处注射部位皮下接收含有2.2μg巨细胞病毒驱动的IGF-I cDNA构建物和0.2μgβ-半乳糖苷酶的报道基因(Lac Z cDNA)的脂质体(180μl盐水含10μl)(n=12)(图1);或
(c)3组,每周皮下注射盐水(200μl生理盐水;n=10);每周皮下注射含有0.2μgβ-半乳糖苷酶的报道基因(LacZ cDNA构建物)的脂质体(180μl盐水中有10μl脂质体)(n=10);或每周皮下注射含有2.2μgIGF-I cDNA构建物和0.2μgβ-半乳糖苷酶报道基因Lac Z cDNA构建物的脂质体(180μl盐水中有10μl脂质体)(n=10)。
实施例2
实验动物-猪
约克小种猪的伤口修复机制与人伤口的修复机制最接近。因此约克小种猪是一种已有详细记录的模型,且已运用于许多实验性创伤研究中。在麻醉和镇痛条件下,每只约克小种猪(10只)都接收4处全层皮伤口(标准模型)。各伤口部位的形状为正方形或矩形,尺寸约为8×8cm,部位间距约5cm,并且固定在上侧和背部。必需按特定的流程固定和安置伤口,如此可使动物无痛苦地侧卧,然后从限制性吊床上放松。
在诱发创伤后迅速用人胎儿绒毛膜/羊膜(INTEGRATM(Life Science)、ALLODERMTM(Life-Cell)或BIOBRANETM(Dow-Hickhan))覆盖伤口。将覆盖物固定在未烧伤的伤口上,并用三种抗生素软膏浸透的纱布覆盖,并加压敷压。需要使用缝针和缝线。
手术后,将自麻醉复苏的动物悬置在吊床束缚中,以保护移植部位防止损伤或移动。将动物保持于吊床束缚中不超过24小时。
实施例3
脂质体
所用的脂质体是含胆固醇的阳离子脂质体,用胆固醇、膜过滤的水(LifeTechnologies,Rockville,MD)制备DMRIE-C试剂(1,2-二肉豆蔻基氧代丙基-3-二甲基-羟基乙基溴化铵)。IGF-I cDNA构建物(图2)含有UTMB Sealy Center forMolecular Science Recombinant DNA Core Facility)制备的巨细胞病毒驱动的IGF-I cDNA质粒(IGF-I cDNA是G.Rotwein,MD NIH.Bethesda,的赠品)。使用的剂量为10%脂质体(在DNA转移试验中使用的最高浓度,对DNA溶解性无有害作用,且与基因转移模式相容)。为了确定脂质体作用的时间过程,烧伤后0.5小时将脂质体注射入组(a)大鼠的尾静脉,并检验其变化7天。200μl体积是可以注射入大鼠尾静脉而不造成有害副作用(如心脏停跳)的量。烧伤后,立即将组(b)或(c)的各只大鼠在任一部位(离伤口边缘1cm处)或在2处彼此远离部位注射接收0.2ml溶液(图1)。每周重复此流程一次,共4周。注射前每周制备新鲜的脂质体复合物(lipoplex)。
分析实施例4
流程
每天相同时间测量体重。通常烧伤后前2天体重的变化百分比增加,在随后的2天体重减少。与用盐水处理的对照相比,接收脂质体的大鼠的体重维持得较好,p<0.05(图3)。组(a)大鼠烧伤后1、2、5或7天,或组(b)和(c)的大鼠在末次注射后5天(烧伤后第33天),断头处死大鼠。将血液收集到血清和血浆分离器中,1000g离心15分钟,倾析并在-73℃冷冻。采集肝和腓肠肌,称重、切片并将各样品60℃干燥至恒重。用干重/湿重的比例来估计蛋白质含量。收集三份背部皮肤样品(命名为活检I、II或III),液氮快速冷冻并-73℃储存用于分析(图1)。对于(腓肠肌和肝脏)脂质体处理和对照动物组间的干重/湿重比例没有差异。
用Behring浊度计(Behring,Dearfield,IL.)测定血清胆固醇、游离脂肪酸、急性期血清蛋白质(触珠蛋白和α2-巨球蛋白)、固有肝蛋白质(总蛋白、运铁蛋白和白蛋白)和葡萄糖。用ELISA(Wako Chemicals Inc.,Richmond,VA)测定血清α1-酸性糖蛋白。在对数表上从0到1500pg/ml,大鼠α1-酸性糖蛋白浓度的标准曲线是线性的。用EIISA(Endogen,Woburn,MA.)测定血浆TNF-α水平。在对数表上用于定量大鼠TNF-α的标准曲线从0到833pg/ml是线性的。用ELISA(BiosourceInt.,Camarillo,CA.)测定血清IL-1β水平,且其用于定量大鼠IL-1β的标准曲线在对数表上从0到1500pg/ml是线性的。通过生物试验,即用递增血清浓度处理对数生长期的B9细胞(小鼠杂交瘤)测定血清IL-6。用分光光谱法定量MTS的减少测定了在对加入的血清反应中细胞的增殖。
分析实施例5
转染
通过测定β-半乳糖苷酶是否存在来测定转染。在三个皮肤活检组织中用Bluo-gal作组织化学染色检测β-半乳糖苷酶蛋白质。4℃用含4%多聚甲醛的HEPES缓冲的Hanks溶液(pH 7.6)固定皮肤标本过夜。用缓冲液和磷酸盐缓冲液(PBS)洗涤后,将标本在37℃ pH 7.6的0.1% Bluo-Gal底物溶液(卤化的吲哚基-β-D-半乳糖苷)(LIFE Technologies,Gaithersburg,MD,USA)中培育过夜。大量洗涤后,将组织包埋入石蜡,制备组织切片并用苏木精和曙红或单用曙红染色。
用化学发光报道基因试验(GALACTO-LIGHT PLUSTM,Tropix Inc.Bedford,MA.USA)也检测到皮肤中存在β-半乳糖苷酶。如下制备样品[55]:将100mg组织在200μl裂解缓冲液(40mM Tris(pH 7.5)、1mM EDTA、150mM NaCl)中匀浆约30秒。将样品离心(12,000×g)3分钟。取出上清液,测定体积并用冰储存。用200μl裂解缓冲液漂洗残留的颗粒,微量离心。按制造商说明书用96孔板进行试验。
β-半乳糖苷酶反应的细颗粒蓝-绿色反应产物主要存在于肉芽组织中,由伤口下纺锤形成肌纤维细胞、巨噬细胞和生长的小血管组成。与β-半乳糖苷酶相同染色的细胞是成肌纤维细胞、内皮细胞和巨噬细胞(炎症区域),而且包括多核巨细胞,表明增殖速率较高的细胞优先转染(图4A)。虽然β-半乳糖苷酶的大部分染色位于细胞质中,但细胞边缘的外部也发现一些反应产物,可能是由于死细胞释放出的酶或反应产物的简单扩散。注射部位附近毛囊的基质中也检测到少量反应产物。用盐水处理的动物中没有检测到β-半乳糖苷酶(图4B)。
与用盐水处理的大鼠相比,接收脂质体包裹的LacZ cDNA和IGF-I cDNA构建物的大鼠伤口周围的β-半乳糖苷酶蛋白质表达增加(p<0.05;表1)。各组之间血细胞、肝、脾或肾中β-半乳糖苷酶浓度没有差异。此发现与皮下cDNA注射后体细胞没有被转染的结论一致(表2)。

IGF-I
在接收IGF-I cDNA构建物的大鼠中,有证据表明转染部位附近的皮肤中有IGF-I mRNA,而在对照或假处理的组织中则没有(图5)。
用RIA在三份皮肤活检组织中测定了IGF-I蛋白质的浓度(图1)。在液氮下粉碎大约40mg组织,以1∶7的体积(7ml缓冲液/克组织)加入提取缓冲液(PBS、0.25ml PMSF、50mg亮抑蛋白酶肽、100mg抑蛋白酶肽和50mg抗蛋白酶),并匀浆化此混合物来提取蛋白质。为了回收蛋白质,将样品在-80℃冷冻过夜。解冻后,将50μl匀浆加到150μl的提取溶液中,并13,500rpm离心5分钟。将100μl上清液加到400μl中和溶液中,并如试剂盒的说明书所述进行RIA(Diagnostic System Laboratories,Webster,TX,USA)。
在采集的所有皮肤活检样品中,用IGF-I cDNA构建物处理的动物,与仅用脂质体或用盐水处理的动物相比,伤口周围具有较高的IGF-I蛋白质浓度(p<0.05,表3)。各组动物血清、肝、脾或肾的IGF-I蛋白质浓度没有差异(表4)。

固有的肝蛋白、急性期蛋白质和细胞因子
烧伤后血清总蛋白质浓度减少。接收脂质体的大鼠显示在烧伤后5天血清总蛋白质增加(脂质体:5.6±0.1g/dl与对照:5.2±0.1g/dl),p<0.05。烧伤后血清的运铁蛋白将近减少正常水平的30%以下。用脂质体处理的大鼠显示与对照相比在烧伤后第2和5天血清运铁蛋白增加(p<0.05,图6)。处理组和对照组间的血清白蛋白没有显著性差异。另外,在整个研究过程中可以证明处理动物和对照动物间血清胆固醇、游离脂肪酸和葡萄糖没有显著性差异。
在烧伤后I型急性期蛋白质、血清触珠蛋白和α1-酸性糖蛋白增加。烧伤后第1和5天脂质体减少了血清α1-酸性糖蛋白。脂质体处理动物和对照动物间血清触珠蛋白没有显著性差异。烧伤后II型急性期蛋白质增加近50%。脂质体处理的动物和对照动物间血清α2-巨球蛋白没有差异。
烧伤后所有测定的细胞因子都增加。与对照相比,接收脂质体的大鼠烧伤后第1天血清IL-1β减少,烧伤后第1和2天血清TNF-α减少(p<0.05;图8A、B)。与对照相比,脂质体处理的大鼠中没有观察到血清IL-6的变化。

生物学效力
所有60%TBSA烫伤的大鼠都存活,而且药物注射没有有害的副作用。如下确定伤口的愈合:在前28天让伤口焦痂保持完整,然后通过轻微的牵拉除去,小心不破坏或损伤周边上的愈合边缘。除去焦痂后,将动物置于标准表面上,并将伤口区域在醋酸板上印迹,其很好地标定了表皮的再生和未烧伤的界面和新生上皮的前沿。用计算机化的面积法(Sigma Scan和Sigma Plot软件)计算这些印迹的面积。另外,烧伤后第33天从伤口边缘采集皮肤活检组织,并用已确立的方法进行光学显微镜分析。线性皮肤重新形成上皮的组织测定使用了HE染色法。

实施例9
胎儿羊膜供体组织的征收和羊膜及绒毛膜的制备
在告知同意后,为了收集病史和筛检潜在的危险因素如癌症、传染病、药物滥用和性行为,测试预定供体的乙型肝炎和丙型肝炎、RPR、HIV1、HIV2(这由SBI皮肤储备库常规进行,且如AATB所定义)。该测试是在分娩时和分娩后60-90天定期门诊时进行的。分娩后,立即从合适的供体得到附着有胎膜的胎盘(按照AATB的指导)。在分娩室用Ringer溶液初步洗涤后,将胎盘转移到含有抗菌素和抗真菌剂的组织培养液(如RPMI 1640)中,并在4℃运输到加工地。
用无菌技术进行加工步骤,从而使产物不受细菌和真菌的污染。对各个加工步骤进行微生物测试以监测污染情况。还对组织样品进行PCR以排除病毒(如HIV-1或-2)感染。
在加工地(SBI组织储备所),通过脐血管用Ringer溶液再对胎盘进行洗涤和冲洗。然后机械性地将羊膜和绒毛膜彼此分开并与胎盘分离,并用胰蛋白酶(Boehringer Mannheim,Indianapolis,IN)(20℃ 2小时蒸馏水和胰蛋白酶1∶1稀释液)进行酶消化来除去覆盖的细胞物质,然后再用磷酸盐缓冲盐水漂洗(按WO93/10722)。为保持一致性,用标准冷冻速率控制技术保存制备的羊膜。为了得到不同的组织性状和特性,在20℃用1.5%戊二醛溶液(Sigma,St.Louis,MO)将胎膜结构交联20分钟,然后用1.5%甘氨酸(Sigma,St.Louis,MO)洗涤3次15分钟,如Euro Skin Bank所述的皮肤保存法(61)。
制备最后的存储物,包括用带有双冷冻干燥流程的搁板冷冻干燥系统,使水分低于或等于6%,或20℃在85%的甘油中振荡胎膜2个3小时来保存。后者残留15%水分,并确保对HIV和其他病毒灭活(62)。第二种处理导致不会残留活细胞且降低了抗原性,从而使免疫性应答反应降低(63,64)。将甘油防腐的组织保存在4℃的甘油中。将这些组织转移到铝箔袋中完成上述两个程序,从而可以用γ射线或乙烯气体进行进一步的杀菌。使用前,将组织浸泡在盐水/Ringer溶液中重建。为了比较,制备其它覆盖材料如含I型和II型胶原的合成纤维和人皮肤(美国专利No.5,002,071;Walther等人,1998)。

将含有胆固醇的阳离子脂质体作为创伤基因治疗的送递系统减弱了烧伤大鼠的急性期反应
一个引起与烧伤相关的代谢亢进的主要因素是急性期蛋白质和细胞因子的增加[9,10,11]。本文以显示了与盐水相比,给予含有胆固醇的阳离子脂质体能改善烧伤后的体重和固有肝蛋白质运铁蛋白和总蛋白质的表达。还进一步显示了脂质体减少了血清I型急性期蛋白质和I型急性期蛋白质相应的前炎症细胞因子血清TNF-α和IL-1β。也有可能是IL-1和TNF-α的减少然后导致这些急性期蛋白质的减少。
脂质体的有益作用非常可能是因为脂质体的脂肪分子对受损细胞膜的直接作用或者是提高了对细胞外营养物的摄取和对内源生长因子和细胞因子的原位包入和保护(由急性期蛋白质和细胞因子引发的代谢亢进反应的一部分而局部产生的)。前炎症细胞因子(尤其是TNF-α)抑制了蛋白质的合成并引发体重的下降[16-18]。在烧伤及脓毒症中,TNF-α血清水平和脓毒症引发的肌肉蛋白质水解增加[19-21]。血清TNF-α水平下降即伴有烧伤儿科患者的净蛋白平衡改善和体重丢失缓解[22,23]。所以,如本文所示血清TNF-α的降低保持了体重并增加了血清运铁蛋白的水平。
一些研究试图确定阳离子脂质体对体外细胞因子表达的抑制作用的机制,但至今未明确其确切机理[25-30]。看来似乎是核因子κB(NF-κB)起着重要作用[27]。NF-κB是一种转录信号,是产生细胞免疫和炎症应答决定性因素[27]。由于细胞和阳离子脂质体间的静电相互作用,脂质体能结合氧化的低密度脂蛋白(OxLDL)的受体[28,29]。Sambrand及其合作者显示了这种竞争性接合和随后对OxLDL受体的活化间接抑制了NF-κB的活性和/或NF-κB与其同源DNA位点的结合[28]。另外,Aramaki及其合作者显示阳离子脂质体能抑制p41(MAP激酶转录因子家族的一种蛋白质)的酪氨酸磷酸化,从而随后导致NF-κB的下调[25-27]。对p41和NF-κB的抑制作用抑制了可诱导的一氧化氮合成酶(iNOS)的诱发,从而降低了随后的一氧化氮(NO)的表达[25,26]。已假设在该信号途径中的这些变化导致了对TNF-α表达转录后水平的选择性抑制[25,26,31]。鉴于NO和NOS刺激IL-1β的表达,通过阳离子脂质体对NO或iNOS活性的抑制似乎可能导致IL-1β表达的减少[32,33]。因此,显然证明给予阳离子脂质体对治疗炎性疾病有益,部分是由于它们对前炎症细胞因子释放的抑制[31,34]。
给予的含有胆固醇的阳离子脂质体通过对细胞因子表达的作用,调节了代谢亢进反应,虽然注射的作用持续不可能超过5天。烧伤后7天,脂质体处理动物和对照动物之间血清细胞因子和蛋白质不存在差异。另外,烧伤后5-7天,接收脂质体的大鼠血清运铁蛋白减少,而对照动物显示血清运铁蛋白增加。另外,脂质体处理组在5-7天血清触珠蛋白增加,而相同时期中对照动物血清触珠蛋白减少。因此,胆固醇-阳离子脂质体增加了固有的肝蛋白质,降低了I型急性期蛋白质,并降低了IL-1β和TNF-α水平。这样胆固醇阳离子脂质体看来适合用作基因治疗损伤的送递系统,因为脂质体有利地调节损伤引起的代谢亢进反应而没有体外使用其它阳离子脂质体时通常伴有的细胞毒性[4,7]。

在烧伤后多次注射IGF-I基因构建物的临床效果
各组所有60%TBSA烫伤的大鼠都存活,且药物注射没有显示有害的副作用。接收单次或多次脂质体-IGF-I cDNA构建物注射的动物在烧伤后的前4周,每周总体重增加近2%,且两组间没有差异。与单次注射的相比,接收多次IGF-IcDNA构建物注射的大鼠具有较高的血清蛋白质水平(单次注射5.2±0.05g/dl与多次注射5.5±0.06g/dl)和总肝蛋白质(单次注射0.64±0.002mg/ml与多次注射0.71±0.03mg/dl)(p<0.05)。除去焦痂后,与单次注射相比,注射两次IGF-IcDNA构建物的大鼠在烧伤后第33天烧伤上皮再生的面积百分比明显大(分别为38±2%与31±2%;p<0.05)。线性上皮再生的组织学测定证实了这些结果。与单次注射相比,接收两次IGF-I cDNA注射治疗的大鼠上皮再生更明显(p<0.05,图11)。
通过化学发光指示剂基因试验检测β-半乳糖苷酶所测定,表明与单次注射相比,接收多次注射包裹有Lac Z cDNA和IGF-I cDNA构建物脂质体的动物伤口周边的转染增加(p<0.05;图12A、B)。沿伤口边缘接收单次IGF-I cDNA构建物注射的大鼠皮肤活检点I到皮肤活检点III的IGF-I蛋白质皮肤浓度减少(图13A)。接收两次cDNA构建物注射的动物显示沿整个伤口边缘IGF-I蛋白质浓度一致上升(图13B)。另外,早在皮下注射LacZ基因后的第一天就可检测到转染。注射后转染速度增加并在第5天到达峰值。与在给药后第7天检测不到转染的体外试验相反,在体内注射后第7和14天依旧可检测到皮肤细胞的转染。
皮下注射IGF-I cDNA和报道子Lac Z构建物后,可鉴定到转染的真皮细胞、成肌纤维细胞、内皮细胞和巨噬细胞,包括已知的增生性多核巨细胞。在被IGF-I构建物转染的大鼠的皮肤中IGF-I的mRNA水平增加。该mRNA被翻译成蛋白质,这与皮肤IGF-I蛋白质浓度增加的观察相一致。这种IGF-I蛋白质局部表达的瞬时增加,将导致对IGFBP-3蛋白质合成的并行刺激,并将局部增加生物活性复合物IGF-I/IGFBP-3的水平,而游离IGF-I蛋白质的循环水平则不会有任何伴发性超生理增加,从而没有有害的副作用[49,50]。脂质体转染后表达的少量IGF-I蛋白质是以旁分泌性方式起作用,不会发生全身给药需要较大剂量所引起的副作用。
皮肤中IGF-I蛋白质浓度的增加,通过上皮再生和真皮细胞康复及真皮细胞有丝分裂改善了伤口的愈合。接收IGF-I cDNA的大鼠的体重和血清及肝中的总蛋白质浓度也增加。由于在血液、肝、脾或肾中都未能观察到转染或增加的IGF-I表达,这些有益作用(如保持体重、增加血清和肝的蛋白质浓度)是由于受伤后伤口愈合的增进和真皮细胞康复的改善,而且是较高的局部IGF-I蛋白质水平伴随的旁泌性作用的结果,这与全身转染导致IGF-I蛋白质循环水平的变化相反。IGF-I对角质形成细胞和成纤维细胞起着促有丝分裂作用,并刺激胶原合成以及改善受伤后的细胞复原,从而提高伤口的愈合[40,41,48]。
在一些临床研究[36,37]中证明了早期伤口闭合的益处,包括减低代谢亢进性烧伤反应和降低炎症介质如IL-1、IL-6、IL-8和TNFα[36,37]。另外,烧伤后IGF-I蛋白质减低前炎症细胞因子IL-1β和TNF-α的表达[57]。因此,通过提高皮肤上皮的再生和/或减低前炎症反应(这是烧伤后细胞因子合成释放的主要来源之一),IGF-I发挥了其全身的有益作用[58,59]。
本文证明了转染限制在注射部位周边小范围内的皮肤上。IGF-I表达的性质最可能是由于阳离子脂质体表面正电荷与细胞外膜负电荷的相互作用,这限制了脂质体的迁移[54]。然而,与单次注射部位相比,当使用多部位注射时转染和IGF-I表达明显增加。IGF-I cDNA的多次注射表明IGF-I蛋白质表达一致提高和伤口愈合的改善,而单次注射则表现沿伤口只有较少的IGF-I蛋白质。该发现是临床相关的,因为包裹该基因的脂质体应施加于距伤口部位精确的距离,以提供最佳转染和蛋白质表达。
本文的实验表明皮下给予包裹IGF-I cDNA构建物的脂质体能成功地转染真皮细胞。还表明该cDNA被转录为mRNA并被翻译成IGF-I蛋白质。多次注射IGF-I cDNA增加了转染细胞的数量和蛋白质的表达,从而改善伤口愈合(与单次注射相比)。这种转染、转录和翻译的过程限于皮肤,因为未观察到全身转染或IGF-I蛋白质表达的全身性增加。对增加的皮肤IGF-I的生物学反应是伤口愈合的提高,和对代谢亢进反应的全身性改善。

包裹IGF-I cDNA基因的脂质体送递增加了皮肤细胞中的IGF-I蛋白质以促进伤口的愈合
受伤后采用转基因治疗是一种新型的改善临床结果和死亡率的方法。其主要点是选择适当的基因送递载体[1,2]。无病毒的组合物、无细胞毒性、感染性的增加和抗炎症活性使含有胆固醇的阳离子脂质体成为一种能缓解烧伤引起的代谢亢进反应的有前途的方法[4,51-53]。本文已显示烧伤后含有胆固醇的阳离子脂质体是一种有效的体外送递系统。另外,本文也报告了DAN转染和随后诱导基因表达而改变烧伤反应的机理,以及IGF-I基因转移在烧伤模型中的作用。本文表明皮下给予包裹IGF-I cDNA构建物的脂质体能成功地转染真皮细胞,且该cDNA被转录为mRNA并翻译成IGF-I蛋白质。转染、转录和翻译过程限于皮肤。对增加的皮肤IGF-I的生物学反应是伤口愈合改善,而且随后对代谢亢进反应的全身改善。从这些发现可以得到以下结论:当给予60%TBSA烧伤的大鼠时,包裹IGF-I cDNA表达质粒载体的含有胆固醇的阳离子脂质体能有效地增加IGF-I皮肤蛋白质浓度从而提高伤口愈合。

用于覆盖材料的膜的比较
为了测定不同伤口覆盖材料的生物学效果,每周进行结果测定为期4个月,包括:a)伤口愈合时间;b)通过计算机化的面积法、组织学/电子显微镜测定确定上皮再生;c)吸收率和排斥的组织学及免疫标记物显示的完整性;和d)弹性和压缩率。根据该数据,评估不同覆盖材料的功能和效果,并确定最适合的。
另外,如下测定基因的转移:a)作为对转染的筛检进行报道基因(β-半乳糖苷酶)表达的组织化学和化学发光试验;b)Nothern印迹分析皮肤中作为转录标记的IGF-I mRNA;和c)放射免疫试验检测皮肤中的IGF-I蛋白质。
另外,如下测定急性期反应:a)体重、营养物的摄入、氮平衡和肌肉及肝中的蛋白质浓度;b)用浊度计测定固有肝蛋白质的产生(白蛋白、运铁蛋白、前白蛋白和结合视黄醇的蛋白质);c)通过酶联免疫试验(ELISA)和浊度计测定急性期蛋白质(触珠蛋白、α1-酸性-糖蛋白、α2-巨球蛋白和纤维蛋白原);和e)用ELISA测定细胞因子(白细胞介素-1β、白细胞介素-4、白细胞介素-6、白细胞介素-8、白细胞介素-10、肿瘤坏死因子-α和干扰素-γ)。

烧伤的伤口覆盖物
为了更精确地模拟临床治疗,在麻醉和镇痛下使每只Yorkshire猪(共15只)接收35%总体表面积(TBSA)的全层火焰烧伤(标准模型)。将这些动物限制在吊床中24小时,并接收广泛监护(包括适当的复苏和镇痛)。为了模拟临床情况,在烧伤后24小时,将动物进行伤口切除和移植。这与人类患者在烧伤后进行切除和移植的时间大约相同。在全身麻醉和镇痛下,切除整个烧伤伤口并立即用人羊膜或浸透了脂质体基因复合物的人羊膜覆盖。每个动物只使用一种类型的覆盖材料,反对同一动物的背部使用不同类型的伤口覆盖材料。将覆盖物钉在未烧伤的伤口处,用含有三种抗菌素油膏的纱布覆盖和压覆料。如需要使用肘钉和/或缝线。手术后,在动物从上述麻醉复苏的同时将这些用于研究的动物悬吊在吊床的约束中。
这些实验测定了有或无脂质体表达载体的不同的覆盖材料对肝急性期反应的作用,并且将那些作用与伤口愈合相关连。因此,在烧伤、手术后及处理后立即和第2、4、6、8、10、12、14和16天,从动物采集静脉血样来测定固有肝蛋白质、急性期蛋白质和细胞因子。人羊膜或INTEGRATM提供了脂质体基因构建物的储备。由于细胞/伤口表面之间的静电性相互作用,脂质体可从浸透的覆盖材料中逸出并转染皮肤和烧伤的细胞,从而增加局部IGF-I浓度。IGF-I浓度的增加随之将改善伤口的愈合。

与IGF-I相一致的其它生长因子
联用胎膜或人尸体皮肤(浸透了脂质体基因构建物)将加速和改善受伤后移植的利用、伤口愈合(功能地)和代谢亢进的反应。脂质体基因构建物可编码类胰岛素生长因子-I(IGF-I)、角质形成细胞生长因子(KGF)、生长激素(GH)、成纤维细胞生长因子(FGF)、表皮生长因子(EGF)、血小板衍生的生长因子(PDGF)、转化生长因子-β(TGF-β)或任何上述因子的组合。
结论
含有编码IGF-I的cDNA的脂质体已显示出能保持体重(60%TBSA烧伤后)、防止肌肉蛋白质的消耗、改善伤口愈合并增加皮肤中IGF-I蛋白质的浓度。另外,细胞转染及随后的IGF-I基因表达是限制在注射部位的局部事件。由于血清IGF-I和IGFGBP-3浓度没检测到显著性差异,脂质体基因送递的有益作用是因为其局部性提高伤口愈合而不是循环性IGF-I的全身性改变。
本发明证明编码生长增强剂的cDNA有利地调节了烧伤后代谢亢进反应,和脂质体构建物可有效地用作基因转染的送递系统。通过增加伤口边缘周围注射部位的数目或通过“伤口覆料”(增加被转染的细胞的数量和伴随的基因表达水平),进一步提高了脂质体基因转染的治疗效益。因此,浸透了含有IGF-I基因构建物的脂质体的胎膜或其它伤口覆盖材料改善了伤口愈合并提供了最佳的烧伤治疗方法。脂质体基因治疗的应用可用于浸泡人尸体皮肤、羊膜或其它伤口覆盖和/或闭合材料,并能革新塑料的重建的创伤和烧伤手术,并改善这些患者的临床治疗效果。

含胆固醇的阳离子脂质体,所述脂质体含有至少一种编码类胰岛素生长因子-I的基因构建体,其中类胰岛素生长因子-I的cDNA构建物与所述脂质体的比例是2.2微克:10微升。
2.如权利要求1所述的强化的伤口敷料,其特征在于,所述的伤口覆盖材料选自:人胎儿羊膜、人胎儿绒毛膜、人尸体皮肤、和合成的皮肤。
3.一种用于提高外部伤口的伤口愈合的组合物,其特征在于,所述的组合物包括:
含胆固醇的阳离子脂质体,其中所述脂质体含有至少一种编码类胰岛素生长因子-I的基因构建体,其中类胰岛素生长因子-I的cDNA构建物与所述脂质体的比例是2.2微克:10微升;和
