技术领域

背景技术

CGRP(降钙素基因相关肽)是37个氨基酸的神经肽,其属于包括降钙素、肾上腺髓质素和胰淀素的肽家族。在人类中,存在两种形式的CGRP(α-CGRP和β-CGRP)并且具有相似的活性。它们具有三个氨基酸的差异,并且展现出不同的分布。至少两种CGRP受体亚型也可以解释差异活性。CGRP受体位于疼痛信号传导通路、颅内动脉和肥大细胞内,并且其激活被认为在偏头痛病理生理学中起因果作用。例如,研究和临床研究已显示,在偏头痛发作期间CGRP的血清水平升高,静脉内CGRP输注在偏头痛患病者和非偏头痛患病者中产生持续疼痛,并且用抗偏头痛药物的治疗使CGRP活性正常。

正在用偏头痛药物(例如,生物药物,诸如抗体,例如伽奈珠单抗、弗雷曼单抗、依普奈珠单抗或厄瑞努单抗)治疗的一些患者尽管正在用他们目前的偏头痛药物治疗,但可能经历偏头痛、症状或发作的突破。
因此,仍然存在显著尚未满足的对新型偏头痛特异性药物的医学需求,所述新型偏头痛特异性药物与现有疗法相比具有增强的患者益处。更具体地,希望的是治疗经受偏头痛、症状或发作的突破的正在治疗偏头痛的患者的疗法。
发明内容

预防性药物使用

Claims (68)
1.一种治疗正在经受用偏头痛药物的基础治疗的患者的突破性偏头痛的方法,所述患者已经历导致偏头痛、症状或发作的突破,所述方法包括向所述患者施用包含治疗有效量的突破性CGRP拮抗剂或其药学上可接受的盐的药物组合物。
2.根据权利要求1所述的方法,其中用于所述基础治疗的所述偏头痛药物是生物制剂。
3.根据权利要求2所述的方法,其中所述生物制剂是抗体。
4.根据权利要求3所述的方法,其中所述抗体选自伽奈珠单抗-gnlm、弗雷曼单抗-vfrm、依普奈珠单抗和厄瑞努单抗-aooe。
5.根据权利要求1所述的方法,其中所述突破性CGRP拮抗剂是非生物性CGRP拮抗剂。
6.根据权利要求5所述的方法,其中所述突破性CGRP拮抗剂不包含抗体、抗体片段或肽。
7.根据权利要求1和5-6中任一项所述的方法,其中所述突破性CGRP拮抗剂选自奥塞吉泮、替卡吉泮、乌布罗吉泮、阿托吉泮、瑞美吉泮和vazegepant。
8.根据权利要求1和5-7中任一项所述的方法,其中所述突破性CGRP拮抗剂选自乌布罗吉泮、阿托吉泮、瑞美吉泮和vazegepant。
9.根据权利要求1和5-8中任一项所述的方法,其中所述突破性CGRP拮抗剂是乌布罗吉泮或阿托吉泮。
10.根据权利要求1和5-8中任一项所述的方法,其中所述突破性CGRP拮抗剂是瑞美吉泮。
11.根据权利要求1和5-8中任一项所述的方法,其中所述突破性CGRP拮抗剂是vazegepant。
12.根据权利要求3-4和7中任一项所述的方法,其中所述抗体是伽奈珠单抗-gnlm,并且其中所述突破性CGRP拮抗剂是乌布罗吉泮或阿托吉泮。

16.根据权利要求3-4和7中任一项所述的方法,其中所述抗体是弗雷曼单抗-vfrm,并且其中所述突破性CGRP拮抗剂是瑞美吉泮。
17.根据权利要求3-4和7中任一项所述的方法,其中所述抗体是弗雷曼单抗-vfrm,并且其中所述突破性CGRP拮抗剂是vazegepant。
18.根据权利要求3-4和7中任一项所述的方法,其中所述抗体是依普奈珠单抗,并且其中所述突破性CGRP拮抗剂是乌布罗吉泮或阿托吉泮。
19.根据权利要求3-4和7中任一项所述的方法,其中所述抗体是依普奈珠单抗,并且其中所述突破性CGRP拮抗剂是瑞美吉泮。
20.根据权利要求3-4和7中任一项所述的方法,其中所述抗体是依普奈珠单抗,并且其中所述突破性CGRP拮抗剂是vazegepant。
21.根据权利要求3-4和7中任一项所述的方法,其中所述抗体是厄瑞努单抗-aooe,并且其中所述突破性CGRP拮抗剂是乌布罗吉泮或阿托吉泮。
22.根据权利要求3-4和7中任一项所述的方法,其中所述抗体是厄瑞努单抗-aooe,并且其中所述突破性CGRP拮抗剂是瑞美吉泮。
23.根据权利要求3-4和7中任一项所述的方法,其中所述抗体是厄瑞努单抗-aooe,并且其中所述突破性CGRP拮抗剂是vazegepant。
24.根据权利要求1-23中任一项所述的方法,其中用于所述基础治疗的所述偏头痛药物包含曲坦和抗体。
25.根据权利要求24所述的方法,其中所述曲坦选自利扎曲坦、舒马曲坦、那拉曲坦、依立曲坦、多尼曲坦、阿莫曲坦、夫罗曲坦、阿维曲坦、和佐米曲坦。
26.根据权利要求24所述的方法,其中所述曲坦不是利扎曲坦、舒马曲坦、那拉曲坦、依立曲坦、多尼曲坦、阿莫曲坦、夫罗曲坦、阿维曲坦、或佐米曲坦。
27.根据权利要求1-26中任一项所述的方法,其中所述基础治疗包括用至少一种非曲坦药物的治疗。
28.根据权利要求1-27中任一项所述的方法,其中所述基础治疗进行一周、两周、三周、四周、五周、六周、七周、八周、九周、十周、十一周、或十二周或更多周。
29.根据权利要求1-28中任一项所述的方法,其中在用所述CGRP拮抗剂治疗后,所述患者经历偏头痛频率降低或严重程度降低。
30.根据权利要求1-29中任一项所述的方法,其中所述偏头痛、症状或发作选自窦炎、恶心、鼻咽炎、畏光、食欲改变、认知和注意力困难、肢冷、腹泻或其他肠道变化、兴奋或易激惹、疲劳、尿频、记忆力改变、虚弱、打哈欠、伸展、看到亮点或闪光、视力丧失、看到黑点、刺痛感、言语问题、失语、耳鸣、胃潴留、头部一侧或两侧的脉动性或搏动性疼痛、对光(畏光)、声音(声音恐惧症)或气味极度敏感、在体力活动期间疼痛恶化、以及呕吐、腹痛或胃灼热、食欲不振、头晕、视力模糊、和昏厥。
31.根据权利要求1-30中任一项所述的方法,其中所述偏头痛、症状或发作在用至少一种曲坦药物和至少一种抗体治疗后出现。
32.根据权利要求1-31中任一项所述的方法,其中所述偏头痛、症状或发作在用所述CGRP拮抗剂治疗后降低。
33.根据权利要求1-32中任一项所述的方法,其中以约1-1000mg/天的剂量施用所述CGRP拮抗剂。
34.根据权利要求1-33中任一项所述的方法,其中以约1、5、10、15、20、25、30、40、50、60、70、80、90、100、200、250、300、400、500、750、或1000mg/天的剂量施用所述CGRP拮抗剂。
35.根据权利要求1-34中任一项所述的方法,其中口服施用所述CGRP拮抗剂。
36.根据权利要求1-35中任一项所述的方法,其中鼻内施用所述CGRP拮抗剂。
37.根据权利要求1所述的方法,其中所述药物组合物包含约50mg或100mg乌布罗吉泮或效力等同于约50mg或100mg乌布罗吉泮的其药学上可接受的盐。
38.根据权利要求1所述的方法,其中所述药物组合物包含约75mg瑞美吉泮或效力等同于约75mg瑞美吉泮的其药学上可接受的盐。
39.根据权利要求38所述的方法,其中所述瑞美吉泮呈半硫酸盐倍半水合物盐的形式。
40.根据权利要求1-39中任一项所述的方法,其中所述药物组合物呈片剂的形式。
41.根据权利要求1-39中任一项所述的方法,其中所述药物组合物呈胶囊的形式。
42.根据权利要求1所述的方法,其中所述药物组合物包含约50-60重量%的瑞美吉泮半硫酸盐倍半水合物盐、约30-35重量%的微晶纤维素、约2-7重量%的羟丙基纤维素、约3-7重量%的交联羧甲基纤维素钠、和约0.1-1.0重量%的硬脂酸镁。
43.根据权利要求1所述的方法,其中所述药物组合物包含约57.1重量%的瑞美吉泮半硫酸盐倍半水合物盐、约33.4重量%的微晶纤维素、约4.0重量%的羟丙基纤维素、约5.0重量%的交联羧甲基纤维素钠、和约0.5重量%的硬脂酸镁。
44.根据权利要求1和38-40中任一项所述的方法,其中所述药物组合物呈口服固体模制的快速分散剂型的形式。
45.根据权利要求44所述的方法,其中所述药物组合物包含从约70-80重量%的瑞美吉泮半硫酸盐倍半水合物盐、约10-20重量%的鱼明胶、约10-20重量%的填充剂、和0.1-5.0重量%的调味剂。
46.根据权利要求45所述的药物组合物,其中所述填充剂是甘露醇。
47.根据权利要求1或37所述的方法,其中所述药物组合物是喷雾干燥的组合物。
48.根据权利要求47所述的方法,其中所述喷雾干燥的组合物包含羟丙甲纤维素醋酸琥珀酸酯和治疗有效量的突破性CGRP拮抗剂或其药学上可接受的盐。
49.根据权利要求2所述的方法,其中所述生物制剂是神经毒性蛋白。

54.根据权利要求53所述的方法,其中所述药物组合物包含约50-60重量%的瑞美吉泮半硫酸盐倍半水合物盐、约30-35重量%的微晶纤维素、约2-7重量%的羟丙基纤维素、约3-7重量%的交联羧甲基纤维素钠、和约0.1-1.0重量%的硬脂酸镁。

64.根据权利要求63所述的方法,其中所述药物组合物包含约50-60重量%的瑞美吉泮半硫酸盐倍半水合物盐、约30-35重量%的微晶纤维素、约2-7重量%的羟丙基纤维素、约3-7重量%的交联羧甲基纤维素钠、和约0.1-1.0重量%的硬脂酸镁。
