技术领域
本发明涉及一种小腿假肢,它具有一个坚硬的、壳形的、带有一个封闭的末端的外筒壁和一个安装在其外筒壁内并可从中取出的柔韧的内筒壁,该内筒壁同样具有一个封闭的末端和一个可通过一个阀门充气的空气室。
背景技术
一种这样的实施形式例如可从美国专利US 5,156,629A中查到。带有由一种高分子材料组成的内筒壁的残肢从上面插入仅仅其上方开口的外筒壁的空腔内。对于携带舒适性而言,最重要的问题是残肢体积的变化,这种体积变化是由假肢携带者体内的动态的液体体积引起的,并且每天都可能改变。出于这种原因,外筒壁,或者更确切地说空腔,不必绝对与残肢的外轮廓相适配,而相反,尤其在残肢末端区域内应保留一个使残肢能够膨胀的空腔。由此,如果由于残肢萎缩或体积波动而使外筒壁显得过大时,假肢携带者可能会掉出其假肢。这将由于强烈的泵激作用形成一个差的配合形状。行走将会困难,且常伴随有难听的噪声和疼痛。这一问题在上述实施形式中通过向内筒壁充入空气仅能部分地解决。因此在内外筒壁之间尤其缺乏一种可靠的摩擦力或刚性闭锁。
发明内容
本发明要解决的技术问题在于提出一种小腿假肢,它具有较好的携带舒适性。
上述技术问题按照本发明是通过这样一种小腿假肢来解决的,它具有一个坚硬的、壳形的、带有一个封闭的末端的外筒壁和一个可取出地安装在该外筒壁内的柔韧的内筒壁,该内筒壁同样具有一个封闭的末端和一个借助一个阀可充气的空气室,其中内筒壁设计成软的、可在假肢携带者的残肢上展开的、从各方向包围该残肢并与之相毗连的硅树脂筒壁;并且该内筒壁的空气室包围残肢末端并仅为内筒壁的一小部分长度;其特点在于:
a)在内筒壁的末端固定着一个插销,它通过一个贯通孔与空气室形成空气交换连通;
b)在外筒壁的末端内设有一个与插销适配的并可拆卸地将其容纳的插孔;在该插孔上连有一个软管,它的通向外面的软管端头与所述阀和一个泵相连,其中,该阀还可排气。
上述解决方案一方面使得残肢和外筒壁之间保持一种调节的体积平衡,在此,所获得的作用于残肢上的力的宽面分布提高了其舒适性并增强了假肢携带者的活动能力。通过空气室充气而产生的压力分布在软的部分上,此外该部分包在骨头上,由此提高了软的部分的气垫效应。残肢和假肢之间的接触将变为最佳,这会提高协调性(假肢控制)并减少创伤。按照本发明的空气室作为内筒壁不可缺少的组成部分全圆周地与残肢末端紧贴并且适用于任何残肢形状。由于硅树脂是软的、皮肤相容的,因此不会产生皮肤角质化,剪切力大部分由硅树脂吸收,因此,内筒壁可直接支撑在皮肤上。附着(悬浮)一部分通过摩擦力,一部分通过刚性闭锁来实现。为避免由于空气室充气而把残肢挤压出外筒壁,在外筒壁内的残肢末端的区域内,相宜地提供一个超过空气室高度或长度的足够的膨胀空间,而其中在其上方区域内的外壁空腔至少部分地近似于圆筒形结构是适合的。残肢和外筒壁之间的体积差则可由空气室完全填满。在残肢内将达到全接触和端载荷及由此形成的一种近似理想的流体静压力。
不过,本发明最基本的是,通过内筒壁末端,即空气室末端与外筒壁末端的刚性闭锁的摩擦连接,而使内筒壁在外筒壁内产生附加的固定。按照本发明而设置的插销连接保证了一种内外筒壁之间的可靠连接以及内筒壁在外筒壁内的位置不变,并由此产生这样的可能性,即用来使空气室充气的带有软管的泵可与内筒壁相分隔地直接设在外筒壁内,那么,内筒壁不必再具有这种部件,因此可通过不同的硅树脂筒壁组成一个模型组系。借助尺寸上不同的模型测量患者的残肢尺寸,以便从硅树脂内筒壁组系中选出一个确切适合该尺寸的硅树脂筒壁。
内筒壁与外筒壁的结合在已带有硅树脂筒壁的残肢进入外筒壁时自动实现。在此,如果插销在其整个长度上配备有橡胶环,相宜地在插孔内配置环形凹槽。可根据患者需要,通过更换橡胶环和/或通过改变橡胶环数量来调节插销连接的摩擦。此外,空气环也供用于空气系统的密封。

具体实施方式
图1中所示的小腿假肢具有一个坚硬的、壳状外筒壁和一个可取出地安装在其外筒壁内的柔韧的内筒壁。该外筒壁具有一个封闭的,带有一个为模件系列而设的连接头2的末端1a,该内筒壁同样具有一个封闭的末端3a。内筒壁3被设计成软的、可在假肢携带者的残肢上面展开的、从各方面包绕该残肢并与之毗连的硅树脂筒壁,它在自身的末端区域内被设计成双层的,并在此构成一个封闭的空气室4,该空气室从各方面包围残肢末端并且仅为内筒壁3的一小段长度L。
在内筒壁3的末端3a固定着一个插销5(见图2和3),它借助一个贯通孔6与空气室4形成空气交换连通。在外筒壁1的末端1a设有一个与插销5适配的并可拆卸地将其容纳的插孔7,一个软管8与其相连(见图1),其通向外面的软管端头8a与一个排气阀9和一个泵10相连。其中,软管端头8a穿过外筒壁1被引向外面;排气阀9和泵10固定在外筒壁1的外侧。不过,软管可按照点划线8b在外筒壁1外面延伸。
从图3可以看出,插销5具有6个在其长度上分布设置的环槽11;它们各自可更换地容纳一个橡胶环12(见图2)。这六个橡胶环12在装入时略突出于插销5的外壁面,在插孔7内为这六个橡胶环配有环形凹槽13。
为固定插销5,在内筒壁3的末端3a设有一个插入件14,它具有一个布置在空气室4内的半球形夹持片15,空气室的末端壁由一个连接套筒16穿过,该套筒具有一个外螺纹17。最好为铝质的插销5压入或也可螺旋拧入连接套筒16内。一个从外面对着内筒壁3的末端3a放置的夹紧件被推到连接套筒16上,并可借助一个拧到连接套筒16上的夹紧螺母19(见图2)与夹持片15对置夹紧。为了确保空气室4的末端壁达到一种防滑式的夹紧,夹持片15具有两个环形凸起20,与之相配地在夹紧件18内设置有两个环形槽21(见图4)。
插孔7为一个容纳体22的中心纵向孔(见图5),并与一个设在容纳体22末端内的孔23相连,软管8与该孔相连。
容纳体22由一个设计成罐形的可更换式容纳体24包围,该容纳体24被插入一个胶粘在外筒壁1的末端1a内的浇注件25内(见图7)。可更换式容纳体24具有一个环状的、设计成锥形或球形的支座面26,夹紧螺母19的倒圆角头支撑其上。支座面26与浇注件25的一个包围自己的环形面27相平齐。
为了敷设软管8,在可更换式容纳体24的内壁内设有一个平行于轴的、向内腔开放的槽28,它通向一个设在支座面26内的横槽29,该横槽与浇注件25的环形面27内对应设计的横槽30对准。
带有连接套筒16的夹持片15、夹紧件18、例如可设计双孔螺母形式的夹紧螺母19、容纳体22、可更换式容纳体24及浇注件25可由塑料制成。
病人在戴上假肢的外筒壁1之前,将内筒壁3戴在残肢上,通过将插销5插入插孔7产生一个自动的锁定并因此使内筒壁3与外筒壁1相连接。其中,可根据病人需要通过插销5的橡胶环12调节摩擦。为获得较大的摩擦,选择具有较高的肖氏硬度的橡胶环,由此产生一种牢固的摩擦连接。相反,要获得较小的摩擦,橡胶环就选择较低的肖氏硬度,或减少橡胶环数量,由此可使病人更简单地戴上或取下假肢。橡胶环12同时也用作空气密封。
在把内筒壁3装入外筒壁1之后,病人可通过空气室4的充气来自行确定内筒壁3末端的体积。空气接通将由泵10通过软管8、孔23、插孔7和插销5内的贯通孔6来实现。

为避免由于空气室4充气而把内筒壁3并进而把残肢挤压出外筒壁1,在外筒壁1内的残肢末端区域内,设置一个超过空气室4高度或长度L的足够的膨胀空间31,与该段向上相连着一段至少部分近似于圆筒形的段1b。其中,在上述优选实施形式中的空气室长度L约为7厘米。
Claims (9)
1、一种小腿假肢,它具有一个坚硬的、壳形的、带有一个封闭的末端(1a)的外筒壁(1)和一个可取出地安装在该外筒壁内的柔韧的内筒壁(3),该内筒壁同样具有一个封闭的末端(3a)和一个借助一个阀可充气的空气室(4),其中,内筒壁(3)设计成软的、可在假肢携带者的残肢上展开的、从各方向包围该残肢并与之相毗连的硅树脂筒壁;并且内筒壁(3)的空气室(4)包围残肢末端并仅为内筒壁(3)的一小部分长度(L);其特征在于:
a)在内筒壁(3)的末端(3a)固定着一个插销(5),它通过一个贯通孔(6)与空气室(4)形成空气交换连通;
b)在外筒壁(1)的末端(1a)内设有一个与插销(5)适配的并可拆卸地将其容纳的插孔(7);在该插孔(7)上连有一个软管(8),它的通向外面的软管端头(8a)与所述阀和一个泵(10)相连,其中,该阀还可排气。
2、按照权利要求1所述的小腿假肢,其特征在于,插销(5)在其整个长度上配备有橡胶环(12),在插孔(7)内为橡胶环配有环形凹槽(13)。
3、按照权利要求2所述的小腿假肢,其特征在于,橡胶环(12)设置成可更换的。
4、按照权利要求1、2或3所述的小腿假肢,其特征在于,插销(5)与一个插入件(14)相联结,该插入件设有一个布置在空气室(4)内的半球形夹持片(15),而空气室的末端壁由一个用于固定插销(5)的连接套筒(16)穿过,并且一个夹紧件(18)被推移到具有外螺纹(17)的连接套筒(16)上,它从外朝向内筒壁(3)的末端(3a)并可利用一个拧在连接套筒(16)上的夹紧螺母(19)与夹持片(15)对置夹紧。
5、按照权利要求4所述的小腿假肢,其特征在于,夹持片(15)或夹紧件(18)具有至少一个环状凸起(20),相应地在夹紧件(18)或夹持片(15)内配有一个环形槽(21)。
6、按照权利要求5所述的小腿假肢,其特征在于,插孔(7)是在容纳体(22)内的中心纵向孔并与一个设在容纳体(22)末端的孔(23)相连通,软管(8)与该孔相连。
7、按照权利要求6所述的小腿假肢,其特征在于,容纳体(22)设置在一个设计成罐形的可更换式容纳体(24)内,该容纳体(24)插入一个胶粘在外筒壁(1)的末端(1a)内的浇注件(25)内。
8、按照权利要求7所述的小腿假肢,其特征在于,可更换式容纳体(24)具有一个环状的、设计成锥形或球弧形的支座面,夹紧螺母的倒圆角头支撑其上。
9、按照权利要求1至3中任一项所述的小腿假肢,其特征在于,软管端头(8a)穿过外筒壁(1)通向外面并且所述阀及泵(10)固定在外筒壁(1)的外侧上。
