发明背景
过度反应性Th1应答可以产生器官特异性自身免疫性疾病,如关节炎、多发性硬化或I型糖尿病,而过度反应性Th2/Th17应答可能是变态反应和萎缩的基础。目前认为,Th17细胞在宿主对抗病原体的防御中起主要作用,并且过度的Th17应答可以导致严重的炎症应答和自身免疫性疾病,如银屑病。Th17细胞产生白介素17(IL17)。IL17有六种已知的同等型,从A至F。IL17A和IL17F两者均是促炎性细胞因子。
已知不同的病原体在体外诱导不同的IFN-α亚型,并且IFN-α亚型具有不同的抗病毒、抗增殖和免疫调节活性。已显示经由多种途径的感染诱导不同的亚型概况。IFN-α亚型结合相同的受体,激活共同的信号传导途径,并预期具有类似的免疫学功能。根据定义,所有IFN-α亚型均具有抗病毒活性,尽管它们在此情况下的绝对功效可能有很大差异。另外,已经描述了许多其他生物学特性,但是具有不同的效力,包括免疫调节和抗增殖活性。多效性效应似乎是由于与受体链的差异的相互作用以及通过不同细胞内途径到达一系列效应分子的信号传导所致。I型IFN受体由两条链IFNR1和IFNR2组成。具有不同受体链的12种IFN-α亚型中的每一种都有一定的结合亲和力范围。IFNα-14对两种干扰素受体都具有最高的亲和力之一,这就是为什么其与其他11个亚型相比如此具有活性的原因。IFNα-6对该两种干扰素受体也具有高亲和力。
IFN-α可能在调节Th1和Th17应答中起关键作用。已经表明,IFN-α处理间接地(主要经由IFN-γ)促进Th1细胞分化,但是也通过抑制IL4和IL13基因表达来抑制Th2细胞发育。因此,IFN-α能够在促进Th2细胞失衡的疾病和感染中重新建立Th1/Th2群体平衡。近年来,很明显,除了抗病毒效应外,IFN-α还发挥了几种免疫调节功能。IFN-α可以影响树突状细胞分化并控制各种促炎性细胞因子(如IL8或IL18)的表达,并诱导几种抗炎介质,如IL1受体拮抗剂(IL1Ra)、可溶性TNF受体p55、IL10和IL18结合蛋白。然而,对IFN-α,特别是个别的IFN-α亚型的作用机理仍仅部分了解。
银屑病是一种自身免疫性疾病,其特征在于患者皮肤上的异常斑块。这些异常斑块通常是红色、发痒和鳞状的。斑块通常出现在肘部、膝盖、下背部或头皮上,但可以出现在身体的任何位置。银屑病影响全世界约1.25亿人。银屑病与银屑病关节炎、淋巴瘤、心血管疾病、克罗恩病和抑郁症的风险增加有关。银屑病关节炎影响多达30%的患有银屑病的个体。
银屑病的特征在于皮肤表皮层的异常过度和快速生长。银屑病的皮肤细胞每3-5天更换一次,而不是通常的28-30天。据信,这些变化源自角质形成细胞的过早成熟,其由涉及树突状细胞、巨噬细胞和T细胞的真皮中的炎性级联诱导。这些免疫细胞(Th17淋巴细胞和巨噬细胞)从真皮移动到表皮,并分泌炎性化学信号(细胞因子),尤其是肿瘤坏死因子-alpha(TNF-α)、IL17A、IL17F和IL22。这些分泌的炎性信号刺激角质形成细胞增殖并产生趋化因子-CXCL1、CXCL5和CXCL8(IL8)。这些细胞因子对嗜中性粒细胞、嗜碱性粒细胞和肥大细胞具有化学吸引性,并且它们会迁移到角质形成细胞层中。然后,嗜中性粒细胞、嗜碱性粒细胞和肥大细胞释放其颗粒的内含物,从而导致银屑病的许多特征。
目前尚无银屑病的治愈方法;然而,各种治疗方法可以帮助控制症状。这些治疗方法包括皮质类固醇乳膏、维生素D3类似物乳膏、紫外光和免疫系统抑制药物如甲氨蝶呤。约70-80%的银屑病病例以轻度至中度形式出现,并通常可以用乳膏和/或光疗法来处理。然而,现有的局部药物功效有限,且不适合长期使用。在过去的25年中,还没有新的局部药物治疗银屑病。近年来,研究集中在用于治疗中度至重度银屑病的系统性药剂上,例如抗IL17A、TNF-α单克隆抗体构建体。这些新的系统性生物药是有效的,但可能导致与抗感染能力下降相关的一系列有害副作用。极其迫切需要有效、安全、局部的治疗,其可以用来治疗大多数银屑病患者。
特应性皮炎(也称为特应性湿疹)是一种皮肤炎症的类型,其可以导致皮肤发红、肿胀和破裂。其是与Th2相关的疾病,并涉及IL-3、4、5、13、17、22、31的增加。还存在参与粒细胞的增殖和吸引的趋化因子如G(M)CSF,CXCL1、5和8的显著增加。
发明概述
本发明人认为,需要开发一种改进的免疫治疗方法来治疗和/或防治银屑病和特应性皮炎。由于银屑病是由Th17细胞的过度反应性和相应的某些细胞因子的过度生成引起的,因此能够修饰和平衡错向的(misdirected)Th17应答和相关细胞因子的过度生成的药物对治疗银屑病将是有益的。此类药物将进一步适合于治疗过度的Th17应答发挥作用的疾病和状况,例如在特应性皮炎中。
发明人认为,需要提供一种可以关闭角质形成细胞层中的细胞因子/趋化因子的局部治疗,该细胞因子/趋化因子对于可以引起皮肤中的银屑病和/或特应性皮炎中的嗜中性粒细胞和嗜碱性粒细胞/肥大细胞具有趋化性。
本发明涉及用于预防或治疗银屑病以及其中过度的Th-17应答发挥有害作用的状况如炎症应答和自身免疫的组合物和方法。本发明进一步扩展到本发明的组合物在治疗和/或防治银屑病中的用途。
经过广泛的实验,本发明的发明人惊奇地发现,如本文所述,施用IFN-α14,例如SEQ ID NO:1或其变体或片段导致与银屑病或特应性皮炎中免疫应答有关的各种细胞因子的阻抑或抑制。发明人出乎意料地确定,IFN-α14可以直接相互作用以关闭角质形成细胞层中的细胞因子,该细胞因子对于引起皮肤中银屑病的嗜中性粒细胞和嗜碱性粒细胞/肥大细胞具有趋化性。发明人证明,即使在TNF-α的影响下,IFN-α14也抑制这些趋化因子。令人惊讶的是,当局部施用IFN-α14时证明了这种效果。IFN-α14是17,000道尔顿的大分子,且出乎意料的是该分子会穿过皮肤。更令人惊讶的是,发明人出乎意料地发现,当局部提供时,观察到了IFN-α14对角质形成细胞中趋化因子的作用。发明人认为,与向全血提供IFN-α14时更不加区分的效应——在多效性效应和更多组织与IFN-α14接触的事实两者中相比,IFN-α14的局部效应对银屑病更具选择性且更有用。
发明人还建立了本文中称为杂合物1的重组IFN-杂合分子
(SEQ ID NO:2)对干扰素受体也具有高结合亲和力,并且将证明对与银屑病或特应性皮炎有关的趋化因子的影响,特别是在角质形成细胞层中关闭或抑制趋化因子,该趋化因子对于引起皮肤中的银屑病和/或特应性皮炎的嗜中性粒细胞和嗜碱性粒细胞/肥大细胞具有趋化性。

在实施方案中,杂合物包含氨基酸序列SEQ ID NO:2或其功能活性片段或变体或由之组成。
功能活性是指包含IFN-α10和IFN-α14的主要干扰素结合位点的IL-α10IL-α14杂合物多肽,其中向受试者施用肽或受试者中肽的表达促进Th1介导的免疫应答的增强和Th2/Th17介导的免疫应答的抑制。此外,可以通过杂合肽增强Th1介导的免疫应答和抑制Th2/Th17介导的应答的能力来指示功能活性。
片段可以包含来自SEQ ID NO:1或2的至少50个,优选100个,且更优选150个或更多个连续氨基酸,并且其具有功能活性。合适地,可以使用例如cDNA的C末端连续缺失来确定片段。然后可以将所述缺失构建体克隆到合适的质粒中。然后,可以如本文所述对这些缺失突变体的活性测试生物学活性。
变体是指与SEQ ID NO:1或2至少70%同源,更优选与SEQ ID NO:1或2至少80%同源,更优选与SEQ ID NO:1或2至少90%同源,甚至更优选与SEQ ID NO:1或2至少95%同源,甚至更优选与SEQ ID NO:1或2至少96%同源,甚至更优选与SEQ ID NO:1或2至少97%同源,且最优选与SEQ ID NO:1或2至少98%同源的氨基酸序列。变体涵盖SEQ ID NO:1或2的多肽序列,其包括氨基酸的取代,尤其是众所周知具有很高的概率不导致蛋白质的生物学活性或构型或折叠的任何显著改变的取代。这些取代(通常称为保守取代)是本领域已知的。例如,精氨酸、赖氨酸和组氨酸的组是已知的可互换的碱性氨基酸。合适地,在实施方案中,具有相同电荷、大小或疏水性的氨基酸可以彼此取代。合适地,可以基于对干扰素alpha亚型的氨基酸序列比对的分析来选择任何取代,以提供当序列比对时在相似或相同位置处存在于其他alpha亚型中的氨基酸的氨基酸取代。杂合物及其变体和片段可以使用本领域已知的合适的分子生物学方法来产生。
发明人还认为,使用IFN-α14或杂合物1(SEQ ID NO:2)作为与皮肤的表皮下层有关的局部治疗存在某种关联性。不希望受理论的束缚,认为IFN-α14或杂合物1的一部分可以穿过皮肤到达真皮下层,在真皮下层有许多白细胞,特别是生成IL17A、IL17F和IL22的Th17。IL17A和IL17F刺激巨噬细胞制造TNF-α,其是导致角质形成细胞释放粒细胞吸引趋化因子(如CXCL1、CXCL5和CXCL8)的主要介质。
发明人惊奇地发现,IFN-α14或杂合物1(特别是SEQ ID NO:1或2或其变体或片段)作为局部治疗的施用导致与以前的局部用药相比,角质形成细胞中CXCL1、CXCL8(IL8)、CXCL-5和CCL20的更大的降低或抑制。另外,发明人已经确定可以使用非常低剂量的IFN-α14或杂合物1,例如高达5x103IU/ml或5x104IU/ml的局部乳膏。
发明人还指出,当IFN-α14(特别是SEQ ID NO:1或其变体或片段)向下扩散到真皮下层时,其也可以关闭或抑制TNF-α,这也导致趋化因子(如IL17,例如IL17A、IL17B、IL17F和/或IL22)的抑制。

因此,本发明的第一方面提供了用于治疗和/或防治银屑病或特应性皮炎的方法,所述方法包括以下步骤:
(i)向有此需要的受试者施用治疗有效量的干扰素alpha亚型,其中干扰素alpha亚型是IFN-α14、杂合物1或IFN-α14和杂合物1的组合。

发明详述
本发明的发明人惊奇地发现,如本文所述,施用IFN-α14,例如SEQ ID NO:1或其变体或片段,导致阻抑或抑制与银屑病中与免疫应答有关的多种细胞因子。令人惊讶地,当局部施用IFN-α14时,该效应增强。

CXCL8(IL8)是参与嗜中性粒细胞募集到损伤或感染部位的主要细胞因子;该过程称为趋化。许多变量对于嗜中性粒细胞的成功趋化是必不可少的,包括增加高亲和力粘附分子的表达以将嗜中性粒细胞缚牢在受影响部位附近的内皮上(因此不被冲走进入循环系统),以及嗜中性粒细胞可以通过基底膜和细胞外基质(ECM)消化到达受影响的部位。CXCL8在诱导引起这些变化所必需的细胞信号传导中起关键作用。首先,在感染部位,组胺释放导致受伤区域附近的毛细血管的血管舒张,其减慢该区域中的血流,并促使白细胞(如嗜中性粒细胞)更靠近内皮并远离管腔的中心(血流速度最高的地方)。一旦发生这种情况,在嗜中性粒细胞上表达的选择素与内皮细胞之间就产生弱相互作用(通过CXCL8和其他细胞因子的作用,其表达也增加)。

发明人已惊奇地发现,施用IFN-α14(特别是SEQ ID NO:1或其变体或片段)能够使IL-22抑制α/β和γ/δT淋巴细胞合成达50%、优选60%、优选70%、优选76%、优选大于76%、优选80%、优选90%且更优选95%。同样,认为杂合物1提供了相同的效应。
发明人已惊奇地发现,施用IFN-α14(特别是SEQ ID NO:1或其变体或片段)导致抑制角质形成细胞中的CXCL1、CXCL8(IL-8)、CXCL-5或CCL-20达50%、优选60%、优选70%、优选80%、优选90%、优选91%、优选92%、优选93%、优选94%、优选95%、优选96%、优选97%且更优选98%。
发明人已惊奇地发现,如本文所讨论,以低剂量施用IFN-α14(特别是SEQ ID NO:1或其变体或片段)导致角质形成细胞中CXCL1、CXCL8(IL8)、CXCL5或CCL20的抑制。同样,杂合物1显示出相似的功能作用。

药物组合物
如上所述,本发明扩展到用于治疗银屑病或特应性皮炎的药物组合物。
根据本发明以及根据本发明使用的药物组合物,除了活性成分外,还可以包含药学上可接受的赋形剂、载体、缓冲稳定剂或本领域技术人员众所周知的其他材料。此类材料应是无毒的,并且不应干扰活性成分的功效。载体或其他材料的确切性质将取决于施用途径,其可以是例如口服、静脉内、鼻内或经由口服或鼻吸入。配制剂可以是液体,例如,含有pH 6.8-7.6的非磷酸盐缓冲剂的生理盐溶液,或经冻干或冷冻干燥的粉末。
剂量

实验数据
实验1:IFNα-14对来自正常人皮肤的角质形成细胞中IL-6、CXCL8(IL8)、CXCL1和
CCL2生成的影响
发明人测试了IFNα-14对经TNF-α激活以诱导趋化因子分泌的来自正常人皮肤的角质形成细胞的影响。
图1证明了IFNα-14直接抑制CXCl8(IL8)分泌达>80%。CXCL8(IL-8)是涉及银屑病的主要趋化因子,并且图1显示在存在IFNα-14的情况下对CXCL8(IL8)的强烈抑制。
图2证明了对CXCL1生成的强烈抑制。CXCL1是CXC家族的成员,其在响应组织损伤和微生物感染的嗜中性粒细胞的募集和激活中起着不可或缺的作用。
图3证明了对CXCL5生成的强烈抑制。众所周知,CXCL5主要在急性炎症应答期间对嗜中性粒细胞具有趋化和激活功能。它还可以维持嗜中性粒细胞的稳态。
因此,低浓度的IFNα-14强烈抑制所有3种嗜中性粒细胞的化学引诱剂。
图4证明了在存在IFNα-14的情况下对IL6生成无抑制。Il-6是生长促进物。IFNα-14对IL6没有影响。这表明IFNα-14将允许皮肤继续生长。
图5证明了IFNα-14诱导CCL2。CCL2是M2巨噬细胞趋化因子。M2巨噬细胞通常参与受损组织的修复。这是不寻常的,因为它吸引作为“修复”细胞的M2巨噬细胞修复角质形成细胞的损伤。
图6证明了IFNα-14诱导CCL5。CCL5是T淋巴细胞的化学引诱剂。这表明IFNα-14将响应偏转向Th1偏向(bias)。
图7证明了IFNα-14抑制CCL20。CCL20上调代表在屏障破坏的皮肤和伴随屏障功能受损的炎性皮肤状况中免疫监视增加,以抵消潜在抗原入侵的危险信号。它吸引淋巴细胞和树突状细胞。IFNα-14完全阻止角质形成细胞产生CCL20。由IFNα-14进行的此类抑制是完全出乎意料的。
实验2:IFNα-14对刺激成银屑病状态的皮肤活组织检查中IL17A生成的影响
从健康受试者获得正常皮肤活组织检查,并用细胞因子的混合物诱导为银屑病状态。双相应答是药理学众所周知的,并且由于IFNα-14的抗病毒特性所致。图8证明了IL17A在宽范围的IFNα-14浓度内被显著抑制。图8支持以下假设:IFNα-14可以抑制皮肤中IL17的分泌。给药的期望治疗窗口在102-105的范围内。
实验3:人全血测定法中IFNα-14对IL17、IL17F、IL22、TNF-α、IL6、CXCL8(IL8)和
CXCL1生成的影响
该实验使用正常的人全血。用凝集素PHA(植物血凝素)刺激它以非特异性激活T淋巴细胞。由于应答可存在差异(未存在),使用两剂PHA。因为这是外周血,所以>96%的T淋巴细胞是alpha-beta受体类型的。这高度指示了仅针对系统性治疗的功效。
图9证明了在用100IU/ml IFN-α14(1ng/ml)的情况下从衍生自人全血的αβ淋巴细胞的IL17A合成下降高达80%。这是一个出乎意料且惊人的结果。
图10证明了在存在IFNα-14的情况下,对从衍生自人全血的αβ淋巴细胞的IL17F合成的强烈抑制(三个受试者的平均值)。图10证明了IFNα-14显著抑制IL17F高达87%。
图11证明了在存在IFNα-14的情况下,对IL22生成的强烈抑制。IL22复制IL17的趋化因子刺激活性,因此抑制IL22也将对银屑病具有积极影响。
图12证明了在存在IFNα-14的情况下,对TNF-α生成的强烈抑制。100μg PHA诱导TNF-α的产生。该5天全血测定法显示TNF-α生成的>70%减少。TNF-α对内皮细胞、上皮细胞和角质形成细胞传导信号以产生吸引嗜中性粒细胞的趋化因子。这些脱粒以释放导致银屑病的组织破坏性化学物质和酶。
图13证明了在1天全血测定法中,在存在IFNα-14的情况下,对IL-6生成的强烈抑制。IL-6是一种急性期反应物,其在创伤中升高,并被广泛用作辅助生长因子/刺激物。IL6是通常与应激相关的生长因子。它参与确定Treg与分泌IL17的Th17细胞的比率,并因此其除去使T细胞平衡推离分泌IL17的Th-17细胞。
图14证明了在1天全血测定法中,在存在非常低水平的IFNα-14的情况下,CXCL8(IL8)合成受抑制。CXCL8是银屑病的许多方面的主要贡献者,并且是银屑病的效应分子之一。CXCL8是导致炎症的主要贡献者。它将嗜中性粒细胞和嗜碱性粒细胞/肥大细胞吸引到炎症部位——后者释放出组胺和许多其他有害物质,例如前列腺素、白三烯。
图15证明了在存在IFNα-14的情况下,CXCL-1生成受抑制。CXCL1是重要的角质形成细胞趋化因子,具有强的趋化特性。CXCL1主要将嗜中性粒细胞吸引到作用部位,并由TNF-α诱导。图15表明用LPS或PHA两者刺激均受到抑制。条形表示用不同水平的LPS或PHA刺激产生可能的最高水平。这给出低达1IU/ml的IC50,显示出IFNα-14的效力。此1天CXCL1全血测定法表明了该疗法可以多么有效。
这些结果证明了IFNα-14对趋化因子的抑制是靶向性现象。结果表明IFNα-14在生成IL17之前和之后起作用。与其他银屑病疗法相比,这是该疗法的另一个关键的区分者。
实验4:在未经处理和用50微克/ml植物血凝素(PHA)刺激3天的人类正常单个核细
胞中400个白介素、趋化因子和CD标志物的浓度变化。
将人干扰素-alpha 10和14以105IU/ml的终浓度加入培养物中,并且仅显示浓度的显著变化(倍数)。这是通过人类生物标志物检测阵列(RayBiotech Inc.)进行的。
图18显示了α-14抑制IL-3和G-CSF(髓样细胞和粒细胞)、IL-5(嗜酸性粒细胞)、IL-13(Th2应答和IgE合成)以及IL-17和22(Th17/Th22细胞)的合成,如先前讨论。它还增强IL-12p70,其刺激从Th2向更为Th1的形式的免疫平衡改变。它还抑制嗜碱性粒细胞/肥大细胞上的低亲和力IgE受体CD-23,并抑制趋化因子XCL-1、5和CCL-1、7、16、20,从而抑制任何变应性状态和相关的粒细胞吸引。这强烈暗示不仅在银屑病的控制中而且还在特应性皮炎的控制中也起作用。
实验5:人IFN-α14抑制犬IL-17A。
从雄性比格犬(Beagledog)获得肝素化全血,并在存在浓度增加的IFN-α14的情况下用PHA刺激3天。通过ELISA获得的结果显示在图19中,并显示出对犬IL-17A的显著抑制。该结果与用人血液(图9)和人IL-17A获得的结果非常相似。由于IL-17A被认为是人类银屑病和特应性皮炎的主要靶标,因此犬结果表明使用人IFN-α14在患有相同病症的犬中的响应。
实验6:治疗患有重度特应性皮炎的犬(杂种犬)。
一只患有重度皮炎的5岁龄雄性宠物犬已经用所有可用于该疾病的药物治疗。用这些疗法中任一种均未见益处,并且痛苦的动物不断地抓挠并且很少进食——因此其重量减轻且其总体健康状况不佳。
在2周的时间段内每2天舌下施用0.5ml 0.9%盐水中的人IFN-α14(104 IU/Kg)。在3周后动物从其疾病状态中恢复过来,未抓挠并且正常进食。因此,在所使用的所有其他疗法均未成功的情况下,该治疗起作用。血液、肝或肾功能检查未记录不利数据。
实验7.对PHA诱导的从全人白细胞生成白介素/趋化因子比较杂合物1与干扰素-
alpha14。
干扰素的浓度范围为0至40IU/ml,并且细胞以100微克/ml PHA-P刺激3天。白细胞介素/趋化因子使用商业ELISA进行评估,并测量IL-17A、IL-8、CXCL-1、TNF-α和干扰素-gamma。结果显示在图20至24中,并且在干扰素-alpha-14获得的值与由HYBRID1诱导的值之间在P<0.05(学生T-检验)没有观察到统计学差异。本质上,在采用的5种测定法中,这2个分子被判断为相同的。

Claims (5)

4.权利要求1或2的用途,其中所述银屑病是轻度、轻度至中度、中度、中度至重度或重度银屑病。
