Description

Cream for striae gravidarum repair and preparation method thereof
Technical Field
The invention belongs to the field of pharmaceutical composition preparations, and particularly relates to a cream for striae gravidarum repair and application thereof.
Background
Striae gravidarum, also known as obesity striae, orange peel striae and growth striae, generally refers to striae on the body caused by pregnancy, and belongs to linear skin damage of dermis generated in the process of pregnancy, and is represented by purple red or white wavy stripes with different widths appearing on abdomen, buttocks or other parts during pregnancy. Medically, the generation of striae gravidarum is affected by the change of pregnancy hormones, the function of fibroblasts is inhibited, and as the abdominal skin during pregnancy is subjected to overlarge tension due to the enlargement of the uterus, the main component of the elastic fibrin in the skin is decomposed and deformed, and collagen fibers and elastic fibers are broken to present fracture traces, so that the striae gravidarum is generated. Early striae gravidarum is accompanied by dilation of small blood vessels and is therefore purplish red. The reason for the vasodilatation is that the rupture of skin fibers starts a self-repair mechanism of an organism, the aim is to increase nutrition of a striae gravidarum area and repair broken tissues, but the self-repair function of the organism is limited, although a part of fibers can be finally repaired, the skin cannot be restored to normal skin, and white striae gravidarum is left after the repair is finished.
Striae gravidarum occurs quite commonly in women, and about 60% -90% of pregnant women suffer from striae gravidarum, which begins to appear mostly around 24 weeks of gestation. During pregnancy, the abdomen has the largest shape change and the largest tension force on the skin of the abdomen, so the abdomen is the most common part of striae gravidarum and is most obvious for the primiparous woman. Under the condition of no external force assistance, the striae gravidarum cannot disappear once appearing, and the striae gravidarum is accompanied with skin looseness, breast drop and abdominal fat accumulation, so that the dressing, the beautiful body and the physical and psychological health of the woman after delivery are seriously influenced.
At present, the traditional external medicine treatment mainly comprises the following steps: 1. retinoids, such as tazarotene and apadaline, mainly act on epidermal keratinocytes and dermal fibroblasts to induce epidermal hyperplasia and stimulate the synthesis of skin extracellular matrix proteins, and accelerate the formation of new connective tissue bands on the upper part of the dermis, but these drugs can cause skin irritation symptoms, such as burning sensation, erythema and desquamation, etc., and should not be used by patients in the early stage of pregnancy or patients with dermatitis and eczema. 2. The bitter almond oil can effectively reduce the incidence rate of striae gravidarum by applying the bitter almond oil and massaging, but has no obvious curative effect by applying the bitter almond oil alone without massaging. 3. The anti-striae gravidarum cream comprises the following effective components: the hydroxyproline complex, rose hip oil, centella asiatica and vitamin E should be applied for at least 2 times per day, preferably after bathing and before sleeping on abdomen, buttocks, chest and thigh, to effectively prevent striae gravidarum and reduce its severity. 4. Other drugs such as the leiogong root, hyaluronic acid, vitamin E cream and the like have been proved to be effective by some studies, but are not popularized to the treatment of striae gravidarum.
Therefore, for the treatment of striae gravidarum, the types of the existing medicines are relatively few, or certain adverse stimulation exists, so that further research on the medicines with the specific effect of treating the striae gravidarum is necessary. Therefore, the invention further develops and prepares the pharmaceutical composition with good curative effect, small side effect, safety and effectiveness.

Experimental example 1 Effect of topical application of the drug of the present invention on abdominal skin of pregnant rat
(1) Experiment grouping
90 SPF-grade female SD rats (provided by the Experimental animals center of Guangdong medical school) of 3 months of age were randomly divided into 90 groups of 10 animals each, and divided into a normal control group, a pregnant group, and each of examples 1-7. The rats in the normal control group and the pregnant group are normally bred, except for the normal breeding, the rats in the groups of examples 1-7 are evenly smeared with the cream in the examples 1-7 every day and every other abdomen after successful hybridization for 7 d, 2 times every day, 0.05 g/mouse is averagely smeared every time, and the administration of the drug is stopped after the pregnant rats are born and young.
(2) Experimental methods
All pregnant rats are sacrificed after birth of young animals, the median skin of the abdomen is taken and the rat hair is removed by 15 percent sodium sulfide aqueous solution, the residual sodium sulfide is repeatedly washed by clear water, the skin with the size of 0.5 cm multiplied by 0.5 cm is quickly put into the Bouin solution for fixation, and tissue sections are made for histomorphological observation and quantitative analysis. And (3) slicing a conventional sample by paraffin, wherein the thickness of the paraffin is 4-5 mu m, the skin growth epidermal cells adopt an HE (high affinity electrophoresis) staining method, and the elastic fiber staining adopts a Weigart resorcinol-basic fuchsin staining method.
Average values of skin growth epidermis thickness (length perpendicular to the basal layer of the growth epidermis to the stratum corneum under HE staining) and percent of dermal layer spandex area (percentage of black-blue spandex area in visual field tissue under Weigert resorcinol-basic fuchsin staining) were measured using a Leica MPS30 fluorescence-optical microscope photograph with computer image analysis software in each slice with 5 high power lens fields selected and the results are shown in table 1.

Under normal conditions, the elastic fibers of the skin keep certain elasticity with the abdominal muscles and can freely stretch and contract within certain limits. During pregnancy, as uterus gradually enlarges and adrenocortical hormone secretion increases, skin elastic fibers break when a certain limit is exceeded. After production, the elastic fiber, which is usually broken, is difficult to return to the previous state. The medicine of the present invention can repair the damage of epidermal cells caused by radiation, improve microcirculation, improve the nutrition condition of local tissues, enhance the antibacterial ability of skin and repair damaged skin, particularly repair the elastic fibers of the dermal layer of the damaged skin. As can be seen from Table 1, the thickness of the growth epidermis and the percentage of the area of the elastic fiber of the rat are obviously reduced in the pregnant group compared with the normal control group; the experimental groups 1 to 7 had an increased thickness of the growing epidermis compared to the pregnant group, while the percentage of dermal elastic fibers in the experimental groups 1 to 7 was significantly increased. In the experimental groups 1-7, it can be seen by comparison that the growth epidermis thickness of the experimental groups 1-4 is increased more significantly than that of the experimental groups 5-7, and the percentage increase of the dermal elastic fiber of the experimental groups 1-4 is higher than that of the experimental groups 5-7. Further analysis shows that in experimental groups 1-4, the growth epidermis thickness of the experimental group 1 is increased most, and the percentage growth ratio of the dermal elastic fibers is the highest, which indicates that the cream of example 1 used in the experimental group 1 has the best effect of repairing striae gravidarum. Compared with the experimental group 5-7, the effect of repairing striae gravidarum is best when the radix angelicae extract and the clove extract are mixed with carrot seed oil, jasmine oil, vanilla root oil and rosewood oil and exist at the same time.
Measuring SOD activity and MDA and Hyp content in unit skin, collecting skin of 500 mg at the position where the abdomen has been plucked from each rat, shearing the tissue block with ophthalmologic scissors, pouring into homogenizer, measuring pre-cooled normal saline 9 times the weight of the tissue block, and homogenizing in ice water to obtain 10% skin tissue. The activity of superoxide dismutase SOD and the MDA content are measured. About 0.05g of the abdominal skin is removed and the connective tissue skin is peeled off, precisely weighed and recorded, and the hydroxyproline Hyp content in the skin is calculated, and the results are shown in Table 2.
TABLE 2 Effect of each group of drugs on SOD, MDA, Hyp in rat abdominal unit skin (x. + -.S, n ═ 10)
Figure 886534DEST_PATH_IMAGE002
As can be seen from Table 2, the differences in the superoxide dismutase activity and malondialdehyde content among the groups were not statistically significant. The hydroxyproline content in the skin of the unit of the pregnant group is lower than that of the normal control group, and compared with the pregnant group, the hydroxyproline content in the skin of the unit of the experimental group 1-7 is obviously increased. In the experimental groups 1-7, the comparison shows that the hydroxyproline content in the unit skin of the experimental groups 1-4 is obviously increased more than that of the experimental groups 5-7. Further analysis shows that the hydroxyproline content in the unit skin of the experimental group 1 is increased most in the experimental groups 1 to 4, which indicates that the cream used in the experimental group 1 and used in the example 1 has the best effect of repairing the stretch marks. Compared with the experimental group 5-7, the effect of repairing striae gravidarum is best when the radix angelicae extract and the clove extract are mixed with carrot seed oil, jasmine oil, vanilla root oil and rosewood oil and exist at the same time.
Experimental example 2 Effect of the drug of the present invention on clinical evaluation of striae gravidarum of subjects
(1) Grouping experiments: healthy postpartum female subjects, age 28-36 years, 80 people, with stretch marks appearing for about 1 year, without any treatment, the color of abdominal stretch marks is red. All subjects were randomly divided into experimental group 1 (cream of example 1) and experimental group 5 (cream of example 5), 40 persons in each group, and the age and the course of illness of two groups of patients and the area, color and skin tightness of striae gravidarum were not statistically different, and the patients were used once in the morning and at night, and massaged for 3-5 minutes while being smeared until the cream was sufficiently absorbed.
(2) Grading standard: observing the treatment effect, and comparing the striae gravidarum color scores before and after treatment with the total treatment scores of different ages, fetal times, treatment time and postpartum time. After 12 weeks of treatment, the treatment effect is judged according to 5 dimensions of striae gravidarum color, area, concave-convex feeling, elasticity of the abdomen and tightness of the abdomen. The scoring method for each dimension is as follows: (1) color: the sequence is as follows: mauve-red-pink-skin color, no change of 0 point, and 1 point for each reduction of chroma; (2) area: 0 minute: no change, 1 point: slightly better, 2 points: improvement, 3 points: substantially invisible; (3) concave-convex feeling: 0 minute: no change, 1 point: slightly better, 2 points: improvement, 3 points: the concave-convex is basically eliminated; (4) elasticity of the abdomen: 0 minute: no change, 1 point: slightly better, 2 points: improvement, 3 points: is very satisfactory; (5) tightening the abdomen: 0 minute: no change, 1 point: slightly better, 2 points: improvement, 3 points: is very satisfactory. The total score is 9-15 for basic cure; the total score is 3-8, and the effect is obvious; the total score is 1-2 effective; the total score is less than or equal to 1, and the score is invalid. The total effective rate of treatment = (basic cure + significant effect + effective)/total number of cases x 100%. Striae gravidarum color before and after treatment was assessed according to color scoring.
(3) The experimental results are as follows: the improvement values of striae gravidarum color scores before and after treatment are shown in Table 3.
Table 3 effect of each group of drugs on striae gravidarum color scores and total scores of treatments (x ± S, n ═ 40) for subjects before and after treatment
Figure 331422DEST_PATH_IMAGE003
In the data, the total score of each group is 3-8, which shows that the drugs of each experimental group have certain treatment effect on the striae gravidarum, the scores of the striae gravidarum color, area, concave-convex feeling, elasticity of abdomen and tightness of abdomen of the experimental group 1 after treatment are all higher than the score of the experimental group 5, the total treatment score of the experimental group 1 is higher than the total treatment score of the experimental group 5, and the treatment effect of the experimental group 1 is more obvious and better than the improvement effect of the experimental group 5.
After 12 weeks of treatment, 40 volunteers in the experimental group 1 basically cured 0, 18 significant cases (45.0%), 4 ineffective cases (10.0%) and the total effective rate of treatment is 90.0%. In the experimental group 2, 40 volunteers basically cure 0, 16 significant cases (40.0%), 19 significant cases (47.5%), 5 ineffective cases (12.5%) and the total effective rate of treatment is 87.50%.
In the clinical observation, the whole severity of striae gravidarum, the whole color of striae gravidarum, the target area and the degree of relaxation at the beginning of treatment, 2 weeks of treatment, 4 weeks of treatment, 8 weeks of treatment and 12 weeks of treatment are recorded, the score is calculated by a standard 6-point method (0-5 points), 0 point represents the perfect state, and 5 points represents the severe state. And (3) tracing the target skin damage by adopting a transparent film, and scanning the film by using a scanner to obtain a picture to calculate the traced target skin damage area. Based on the suction and stretching principle, the skin elasticity detector is used, the elasticity of the skin is automatically calculated and reflected through software, and the result is shown in table 4.
Table 4 effect of each group of drugs on subject striae gravidarum color and parameter change (x ± S, n ═ 40)
Figure 278649DEST_PATH_IMAGE004
According to the results, the scores of the overall severity of striae gravidarum after the medicines are taken in each group are all in a descending trend, and the descending trend of the experimental group 1 is higher than that of the experimental group 5; the score of each group on the relaxation degree is also reduced after the medicine is taken, and the reduction degree of the experimental group 1 is higher than that of the experimental group 5; the target skin damage area value of each group shows a descending trend after the medicine is applied, and the descending degree of the experimental group 1 is higher than that of the experimental group 5.
