Abstract
本发明公开了一种治疗盆腔炎性疾病后遗症的中药制剂,由以下重量份的中药原料制成:柴胡5‑15份、炒枳实5‑15份、败酱草20‑40份、赤芍10‑20份、白芍10‑20份、甘草1‑5份、醋莪术5‑15份、醋三棱5‑15份、丹参10‑30份、大血藤10‑30份、醋香附5‑15份、酒大黄5‑15份、川牛膝5‑15份、蒲公英20‑40份、糊精10‑30份和蔗糖20‑40份。本发明的优点:具有清热利湿,行气活血止痛,以达到标本兼治等优点。

技术领域
本发明涉及中草药制剂技术领域,具体是指一种治疗盆腔炎性疾病后遗症的中药制剂。
背景技术
近年来,随着科技的进步和生活方式的改变,社会的开放,性生活紊乱,宫腔手术操作越来越频繁,饮食偏嗜、喜食辛辣刺激,作息不规律、长期熬夜、抵抗力下降,工作学习压力大、生活节奏快,尤其女性承担着家庭与工作的双重压力,盆腔炎性疾病的发病率明显上升,并且发病年龄趋于年轻化。
盆腔炎性疾病(pelvic inflammatory disease,PID)是病原体感染导致女性上生殖道及其周围组织炎症的总称,包括子宫炎、输卵管炎、卵巢炎、输卵管卵巢炎、盆腔腹膜炎及盆腔结缔组织炎。PID大多发生于性活跃期妇女。PID可引起弥漫性腹膜炎、败血症、感染性休克,严重者可危及生命。慢性盆腔炎实际为PID的后遗症,如盆腔粘连、输卵管阻塞,从而导致不孕、异位妊娠、慢性盆腔疼痛。PID严重影响妇女身体健康,增加家庭及社会经济负担。美国疾病控制中心的近年数据显示:PID年发病率约为4.4%,每年超过100万的盆腔炎性疾病患者需要治疗费用超过40亿美元。
目前,已经上市的治疗盆腔炎性疾病的药物种类繁多。对于PID目前西医主要以抗生素治疗为主,然而,最佳治疗方案和疾病早期治疗的远期结局尚未确定,尤其抗生素需不断更新换代,才能保证对不断耐药的病原体产生杀伤作用,抗生素耐药已成为威胁人类健康的全球性问题。中药成分复杂,作用靶点多,细菌较少对其耐药并有助于提高疗效,缩短病程,减少后遗症发生。然而,中药治疗主要采用清热解毒利湿、凉血化瘀止痛为主,这些中草药组合物多为片剂或胶囊,其疗程长,见效慢,易复发,给患者增加了经济负担和精神压力。
发明内容
本发明为了解决上述的各种问题,提供了一种针对盆腔炎性疾病的发病机理,清热利湿,行气活血止痛,以达到标本兼治的治疗盆腔炎性疾病后遗症的中药制剂。
为解决上述技术问题,本发明提供的技术方案为:一种治疗盆腔炎性疾病后遗症的中药制剂,由以下重量份的中药原料制成:柴胡5-15份、炒枳实5-15份、败酱草20-40份、赤芍10-20份、白芍10-20份、甘草1-5份、醋莪术5-15份、醋三棱5-15份、丹参10-30份、大血藤10-30份、醋香附5-15份、酒大黄5-15份、川牛膝5-15份、蒲公英20-40份、糊精10-30份和蔗糖20-40份。
优选的,具体由以下重量份的中药原料制成:柴胡9g,炒枳实9g,败酱草30g,赤芍15g,白芍15g,甘草3g,醋莪术10g,醋三棱10g,丹参20g,大血藤20g,醋香附10g,酒大黄9g,川牛膝9g,蒲公英30g,糊精20g,蔗糖30g。
本发明还包括一种治疗盆腔炎性疾病后遗症的中药制剂的制备方法,包括以下步骤:
步骤一:取柴胡、炒枳实、败酱草、赤芍、白芍、甘草、醋莪术、醋三棱、丹参、大血藤、醋香附、酒大黄、川牛膝、蒲公英、共十四味药加水煎煮三次,第一次沸后1.5小时,第二次和第三次沸后各1小时,合并,得煎液;
步骤二:取煎液,滤过,减压浓缩至60—80ml,加入两倍量95%乙醇,静置24小时以上,得醇沉液;
步骤三:取醇沉上清液回收乙醇,离心,浓缩至稠膏,相对密度1.3,得稠膏;
步骤四:取稠膏加入糊精,于70℃减压干燥,粉碎,得浸膏粉;
步骤五:取蔗糖于80℃干燥、粉碎,加入浸膏粉混匀,用95%乙醇制粒,在80℃下干燥,整粒,得干颗粒;
步骤六:取干颗粒,分装,得成品。
优选的,所述一种治疗盆腔炎性疾病后遗症的中药制剂为颗粒状,每袋装15g,每盒装3袋。
优选的,所述步骤五中浸膏粉和蔗糖通过U型混料机进行混合,然后用160型摇摆式制粒机制粒。
优选的,所述160型摇摆式制粒机上的制粒筛网为12目,整粒筛网为14目,且不能通过160型摇摆式制粒机的一号筛与能通过160型摇摆式制粒机的五号筛的总和不能超过制剂成品的15%。
本发明与现有技术相比的优点在于:本发明在使用时,中医立法根本不同,治标治本立现:其基本病机湿热瘀结,湿热瘀既是重要病因,又是该病发展变化的病理基础;药物组方不同:以经方四逆散为基,时方红藤败酱散为楯,加减化裁而成柴枳败酱汤。方中有方、方方组合,激发出“1+1>2”的强大效应;剂型创新:本发明采用颗粒剂,它是一种在尽量保留汤剂优点,克服其煎煮缺点的基础上发展起来的中药新剂型,具有服用剂量小、吸收较快、质量稳定、无须患者煎煮、携带和服用方便、易保存等优点。

所述中药的剂型为颗粒剂,颗粒均匀,色泽一致,无吸潮、结块、潮解等现象。每袋装量15g,每盒3袋。
浸膏粉加入糖粉后,在U型混料机混合后,用160型摇摆式制粒机制粒,制粒筛网12目,整粒筛网14目,且不能通过一号筛与能通过五号筛的总和不能超过制剂成品的15%。

动物药效学试验
研究方法:采用临床随机对照研究设计,设立治疗组(妇炎清颗粒)与对照组(金刚藤胶囊),按1:1比例分配病例。
病例来源:选取2010年8月至2011年2月期间在荆州市中医医院门诊和住院治疗的,符合病例筛选标准的70例盆腔炎性疾病(湿热瘀结证)后遗症患者。
治疗方案:(1)试验药物
治疗组用药:妇炎清颗粒,规格:3包/盒,批号:20100818,由荆州市中医院制剂室提供,批准文号:鄂药自字Z20082071。对照组:金刚藤胶囊,规格:24片/盒,批号:20100613,由湖北福人药业股份有限公司生产。
(2)用药方法
治疗组:妇炎清颗粒,1日3次,每次1包,温开水冲服,疗程4周。对照组:金刚藤胶囊,1日3次,每次4粒,温开水送服,疗程4周。
研究结果:临床研究表明,妇炎清颗粒治疗盆腔炎性疾病(湿热瘀结证)后遗症有较好疗效,总有效率和金刚藤胶囊相当,痊愈率明显高于金刚藤胶囊。能很好的改善腹痛、腰痛、带下异常等症状,提高IL-2水平,调节外周血T细胞亚群中CD4+/CD8+,提高机体免疫力。
临床观察试验
患者资料
选取2016年10月至2017年06月就诊于荆州市中医院,湖北省妇幼保健院,武汉市中西医结合医院,荆门市中医院,十堰市中医院的SPID湿热瘀结证患者,将受试者分配至妇炎清颗粒治疗组和金刚藤胶囊对照组进行随机双盲对照研究。两组患者年龄相当,孕产史、人流次数比较差异无统计学意义,治疗前两组患者病情分级情况对比无明显差异。
诊断标准
(一)西医诊断标准
参考《妇产科学》(第八版)相关章节及《临床诊疗指南·妇产科学分册》(2007年)中盆腔炎诊疗方案制定。
(1)病史:有盆腔炎性疾病病史且总病程大于3个月。
(2)症状:下腹疼痛,腰骶部酸胀疼痛,常于劳累、同房、经期加重,可伴月经失调,白带量多,乏力,低热或不孕等症。
(3)体征:子宫多为后位,活动度差甚至粘连固定。子宫体有压痛,提示为子宫肌炎;如在子宫一侧或两侧触及条索状增粗输卵管,并有压痛,提示输卵管炎;如盆腔一侧或两侧触及囊性肿物,活动多受限,可有压痛,提示为输卵管积水或输卵管卵巢囊肿;如在子宫一侧或两侧扪及片状增厚、压痛,或有宫骶韧带增粗、变硬、触痛,提示为盆腔结缔组织炎。
以上体征至少需同时具备下列2项:子宫活动受限(粘连固定)或压痛;一侧附件区压痛。
(4)辅助检查
妇科超声检查:可显示附件区包块,输卵管积液或增粗,或盆腔积液。实验室检查:宫颈分泌物培养可找到致病的病原体。
(二)中医证候诊断标准
参考中华中医药学会《中医妇科常见病临床指南》[3](2012年)中盆腔炎诊疗方案及《中医妇科学》[4]相关章节(新世纪第二版)制定。
湿热瘀结证
主症:①下腹胀痛或刺痛,痛处固定;②腰骶胀痛;③带下量增多,色黄,质粘稠或伴臭味。
次症:①经期腹痛加重;②经期延长或伴有月经量较平素增多;③口腻或纳呆;④小便黄;⑤大便溏而不爽或干结。
舌脉:舌红或黯红,或舌边见瘀点、瘀斑,苔黄腻,脉弦滑或弦数。
以上证候至少具备主症2项、次症2项,结合舌脉即可诊断为湿热瘀结证。
治疗方案
治疗组:妇炎清颗粒,口服,一次1包,一天3次,温开水冲服;金刚藤胶囊模拟剂,口服,一次4粒,一天3次。连续治疗4周为一疗程,治疗一疗程。
对照组:金刚藤胶囊,口服,一次4粒,一天3次,温开水送服;妇炎清颗粒模拟剂,口服,一次1包,一天3次。连续治疗4周为一疗程,治疗一疗程。
观察指标
安全性观察指标
(1)生命体征:体温、呼吸、脉搏、血压(休息十分钟后检查)。
(2)血常规、尿常规、肝功能(ALT/AST)、肾功能(Scr/Bun)、十二导联心电图(心率、心律、ST-T段变化、QT/QTC间期)
(3)不良事件随时记录。
疗效性观察指标
(1)湿热瘀结证症状积分变化情况。
(2)局部体征积分变化情况。
(3)盆腔疼痛评分变化情况。
(4)盆腔彩超检查:盆腔包块及盆腔积液等变化情况。
(5)实验室检查:宫颈分泌物培养变化情况。
注:安全性及疗效性观察指标用药前、治疗第30±3天后各检查一次。
安全性评价
本临床研究期间,两组患者治疗前后生命体征、血常规、尿常规、肝肾功能、心电图等安全性指标无统计学差异(P>0.05);研究过程中未出现影响工作能力,危及生命等不良事件及药物不良反应,说明妇炎清颗粒安全无毒副作用。
疗效评价
①治疗后两组盆腔疼痛疗效比较,差异具有统计学意义(P<0.05),说明治疗组盆腔疼痛疗效较对照组更佳。见表17。

②治疗后,两组综合疗效比较,差异不具有统计学意义(P>0.05),说明两组综合疗效相当。见表20。

上述临床试验观察结果表明,本发明的妇炎清颗粒治疗盆腔炎性疾病后遗症总体疗效与金刚藤胶囊相当(P>0.05),改善患者盆腔疼痛方面作用明显优于金刚藤胶囊(P<0.05)。服用方便,大大减轻了患者的痛苦和经济负担,见效快,疗程短,无毒副作用,标本兼治。
典型病例
陈某,女,26岁,1995年4月16日入院。
主诉:小腹疼痛月余。
现病史:患者近月来小腹持续疼痛,经期亦痛。右侧附件包块已行切除术。近来B超发现左侧附件又有一包块,约5.0cm×3.3cm。子宫内有多个不规则暗区。提示:左侧附件囊性包块、子宫内膜异位症。平时肛坠,倦息,睡眠欠佳,纳差,大便不畅。月经周期可,经期5~7天。末次月经提前10天来潮。舌暗红,苔黄腻,脉沉弦。妇检:子宫后位,略大,轻压痛,左侧附件可触及鸡蛋大小包块,压痛阳性,活动欠佳。
中医诊断:癥瘕。证属血湿热阻滞冲任。
西医诊断:①左侧附件炎性包块;②子宫内膜异位症。
治则:清热利湿、化瘀消癥。
方药:柴枳败酱汤加蒲公英30g,黄连9g,黄芩9g,5剂。煎服,日1剂。服后小腹疼痛、肛坠均减轻。诉矢气、便溏、腹痛。守上方去大黄5剂。三诊诉月经提前4天至,腹痛递减,经量中等,5天经净,净后左少腹略痛,大便略溏,倦怠心慌,舌暗红,苔黄,脉沉弱。守上方加黄芪30g,昆布15g,海藻15g,6剂。浓煎。四诊:腹痛未作,精神好转,略感心慌,舌红略暗,苔黄,脉弦数。守上方加沙参15g,乌药9g,再进6剂。此后B超检查子宫、附件未见异常回声。
按语:患者因金刃所伤,胞脉空虚,经期或房事不节,湿热之邪乘虚而入,与血相合,结于少腹,形成癥,阻滞气机,“不通则痛”。此刻湿热之邪偏重,故于方中加公英、黄连、黄芩以加强清热祛湿解毒之力。药后出现便溏、心慌,说明病久正伤,实中夹虚,宜祛瘀清热兼以扶正。大便溏,故去大黄,佐黄芪、沙参益气扶正而不恋邪、助诸药驱邪之力,再伍昆布、海藻以祛湿软坚、使湿去热清,气行血畅,则腹痛癥瘕自消。
以上对本发明及其实施方式进行了描述,这种描述没有限制性,附图和实施例中所示的也只是本发明的实施方式之一,实际的结构并不局限于此。总而言之如果本领域的普通技术人员受其启示,在不脱离本发明创造宗旨的情况下,不经创造性的设计出与该技术方案相似的结构方式及实施例,均应属于本发明的保护范围。
Claims (6)
1.一种治疗盆腔炎性疾病后遗症的中药制剂,其特征在于:由以下重量份的中药原料制成:柴胡5-15份、炒枳实5-15份、败酱草20-40份、赤芍10-20份、白芍10-20份、甘草1-5份、醋莪术5-15份、醋三棱5-15份、丹参10-30份、大血藤10-30份、醋香附5-15份、酒大黄5-15份、川牛膝5-15份、蒲公英20-40份、糊精10-30份和蔗糖20-40份。
2.根据权利要求1所述的一种治疗盆腔炎性疾病后遗症的中药制剂,其特征在于:由以下重量份的中药原料制成:柴胡9g,炒枳实9g,败酱草30g,赤芍15g,白芍15g,甘草3g,醋莪术10g,醋三棱10g,丹参20g,大血藤20g,醋香附10g,酒大黄9g,川牛膝9g,蒲公英30g,糊精20g,蔗糖30g。
3.根据权利要求1所述的一种治疗盆腔炎性疾病后遗症的中药制剂,其特征在于:其制备方法为:
步骤一:取柴胡、炒枳实、败酱草、赤芍、白芍、甘草、醋莪术、醋三棱、丹参、大血藤、醋香附、酒大黄、川牛膝、蒲公英、共十四味药加水煎煮三次,第一次沸后1.5小时,第二次和第三次沸后各1小时,合并,得煎液;
步骤二:取煎液,滤过,减压浓缩至60—80ml,加入两倍量95%乙醇,静置24小时以上,得醇沉液;
步骤三:取醇沉上清液回收乙醇,离心,浓缩至稠膏,相对密度1.3,得稠膏;
步骤四:取稠膏加入糊精,于70℃减压干燥,粉碎,得浸膏粉;
步骤五:取蔗糖于80℃干燥、粉碎,加入浸膏粉混匀,用95%乙醇制粒,在80℃下干燥,整粒,得干颗粒;
步骤六:取干颗粒,分装,得成品。
4.根据权利要求2所述的一种治疗盆腔炎性疾病后遗症的中药制剂,其特征在于:所述一种治疗盆腔炎性疾病后遗症的中药制剂为颗粒状,每袋装15g,每盒装3袋。
5.根据权利要求2所述的一种治疗盆腔炎性疾病后遗症的中药制剂,其特征在于:所述步骤五中浸膏粉和蔗糖通过U型混料机进行混合,然后用160型摇摆式制粒机制粒。
6.根据权利要求4所述的一种治疗盆腔炎性疾病后遗症的中药制剂,其特征在于:所述160型摇摆式制粒机上的制粒筛网为12目,整粒筛网为14目,且不能通过160型摇摆式制粒机的一号筛与能通过160型摇摆式制粒机的五号筛的总和不能超过制剂成品的15%。
