背景技术
复发性流产(recurrent spontaneous abortion,RSA)是一种复杂的疾病,指两次或两次以上发生在20周之内的妊娠失败(Kim J H,Jeon Y J,Lee B E,et al.Associationof methionine synthase and thymidylate synthase genetic polymorphisms withidiopathic recurrent pregnancy loss[J].Fertil Steril,2013,99(6):1674-1680.)。在育龄期女性中的发生率为1%~5%,RSA女性再次妊娠发生自然流产的可能性高达70%~80%(Wu M,Liu P,Cheng L.Galectin-1reduction and changes in T regulatorycells may playcrucial roles in patients with unexplained recurrentspontaneous abortion[J].Int J Clin ExpPathol,2015,8(2):1973-1978.)。RSA病因复杂多样,除遗传因素、内分泌紊乱、解剖异常、感染、自身免疫、精子质量、生活方式、精神心理及环境等因素,仍有50%的RSA仍病因不明,称为不明原因的复发性流产(unexplainedrecurrent spontaneous abortion,URSA)。URSA的复发风险随着流产次数的增加而上升(Gynaecologists R C O O.The investigation and treatment of couples withrecurrent firsttrimester and second-trimester miscarriage(Green-top GuidelineNo.17)[J].Royal College of Obstetricians and Gynaecologists,2011.),给家庭及社会带来严重的影响,但目前临床上缺乏有效预测URSA发生的指标。
微生物组学是一门研究特定环境下全部微生物的总和的学科。目前普遍认为,人体内有很多共生微生物菌群,其基因组含量总和是人类基因组总和的10倍,微生物培养的方法仅能检测到身体中存在的一小部分微生物的多样性,而绝大多数的微生物无法通过传统的培养的方式进行鉴定,但与人类的健康与疾病息息相关。美国国立卫生研究院将人类微生物组项目纳入到研究内容中,主要的研究内容包括胃肠道微生物,口腔微生物,鼻腔微生物,皮肤微生物和泌尿生殖道的微生物(Nguyen L D N,Viscogliosi E,Delhaes L.Thelung mycobiome:an emerging field of the human respiratory microbiome[J].Frontiers in Microbiology,2015,6:89.)。目前对于复发性流产患者的的微生物组特征的研究非常有限,本申请通过研究复发性流产患者与正常妊娠受试者的微生物组特征,发现呈现显著性差异的微生物菌群,以期实现复发性流产的诊断。
发明内容
针对现有技术的不足及实际的需求,本发明的目的在于提供一种用于评估复发性流产的微生物标志物及其应用,本发明的微生物标志物预测复发性流产风险的灵敏性好,只需要获取微生物标志物的相对含量,作为协助诊断,指导微生物环境的调整,降低复发性流产的风险。
为达上述目的,本发明采用以下技术方案:
第一方面,本发明提供一种复发性流产微生物标志物,所述微生物标志物为Rhodoluna。
第二方面,本发明提供了一种检测第一方面所述的微生物标志物的试剂。
第三方面,本发明提供了一种第一方面所述的微生物标志物或第二方面所述的试剂的用途,所述用途包括用于构建预测复发性流产风险的模型、制备诊断复发性流产的产品。

本发明提供了一种诊断复发性流产的产品,所述产品能在疾病早期进行诊断,实现早期预警,提高患者的生活质量。

具体实施方式
本发明通过将正常妊娠的人群和不明原因复发性流产的人群作为对象,首次查明了细菌与不明原因复发性流产的临床医学指标的相关性,并以此为基础提供了不明原因复发性流产的早期诊断技术。为了评估微生物菌群能否作为不明原因复发性流产的预测因子,本发明通过收集正常人群与不明原因复发性流产的阴道分泌物,综合分析16S rRNA测序、宏基因组测序和针对特定菌群的定量聚合酶链反应结果,发现与复发性流产相关的微生物菌群,本发明通过16S rRNA测序,首次发现了Rhodoluna的丰度在复发性流产和正常人群中呈现显著性差异,说明Rhodoluna可作为复发性流产诊断的生物标志物。
在本发明的实施方式中,本发明通过以下步骤来诊断复发性流产:在来自个体的核酸样本中,检测与复发性流产的诊断相关的菌种所对应的一个或多个核酸片段。在特定的实施方式中,检测对应于Rhodoluna的核酸片段。在实施本文描述的方法中,使用分子生物学、蛋白质生物化学、细胞生物学、免疫学、微生物学和重组DNA方面的很多传统技术,这些技术是公知的。
下文提供了本说明书中使用的一些术语的定义。除非另有说明,本文中使用的所有技术和科学用语通常具有和本发明所属领域的普通技术人员通常理解的意思相同的意思。
本文中术语“OTU”(操作分类单元)是指系统发生树中的末端叶(terminal leaf),由特定的遗传序列以及与该序列在科、属、种或菌株水平上享有序列同一性的全体序列来定义。所述特定遗传序列可以是16S序列或16S序列的一部分,或者可以是在广泛存在于整个真细菌界的功能保守的管家基因。OTU之间具有至少70%、75%、80%、85%、90%、95%、96%、97%、98%或99%的序列同一性。OTU经常通过比较生物体之间的序列来定义,序列同一性低于95%的序列不视为构成同一OTU的一部分,然而在本文所用,一个OTU标识可包含序列同一性为0至100%、25%至100%和50%至100%、优选70%至100%、75%至100%、77%至100%、80%至100%、81%至100%、82%至100%、83%至100%、84%、至100%、更优选85%至100%、86%至100%、87%至100%、88%至100%、89%至100%、90%至100%、91%至100%、92%至100%、93%至100%、94%至100%、95%至100%、96%至100%、97%至100%98%至100%和99%至100%的序列。
本文中,OTU代表此前已经归入或尚未归入属、种和/或株名的细菌,也即OTU或OTU标识等同于细菌的目、科、属、种或株,在进行生物信息学分析的过程中,通过OUT聚类,分析其代表的细菌的含量。
16S rRNA的“V1-V9区”指16S rRNA基因的第一至第九高变区,用于细菌样品的基因分型,这是普通技术人员众所周知的。一些实施方式中,用V1、V2、V3、V4、V5、V6、V7、V8和V9区中至少其一来表征微生物标志物。一些实施方式中,用V3和V4区来表征微生物标志物。
本文中检测微生物标志物的探针是与微生物标志物多核苷酸“特异性杂交”的寡核苷酸,其具有充分互补序列从而允许在本领域常用预定条件下与目标核苷酸序列杂交的寡核苷酸(有时称为“基本互补”)。尤其,该表述包括一寡核苷酸与本文所述单链DNA或RNA分子内所含基本上互补的序列杂交,基本上排除该寡核苷酸与非互补序列的单链核酸杂交。探针和引物的具体长度和序列取决于所需核酸靶标的复杂性以及反应条件(例如温度和离子强度)。总体来说,杂交条件是本领域所知的严格杂交条件。“严格”指核苷酸序列能够结合相关或非特异性序列的条件。例如,高温和低盐提高严格性,使得非特异性结合或低熔融温度的结合发生解离。一些实施方式中,与微生物标志物多核苷酸互补的寡核苷酸与微生物标志物多核苷酸至少95%、96%、97%、98%、99%或100%互补。
作为一种可选择的实施方式,用于诊断对象复发性流产的方法包括:分析对象测试样品的核酸;检测测试样品的核酸中一种或多种微生物和/或OTU的水平;当所述测试样品中一种或多种微生物和/或OTU的水平低于对照样品时诊断所述对象存在发生复发性流产的风险;优选的,所述微生物标志物为Rhodoluna。
另一实施方式中,用于诊断对象复发性流产的方法包括:获取对象的阴道分泌物样品;处理阴道分泌物样品以获取16S rRNA基因序列数据;检测阴道分泌物样品中一种或多种微生物和/或OTU的水平,包括用生物信息学软件分析16S rRNA基因序列数据;并且当阴道分泌物样品中一种或多种微生物和/或OTU的水平低于对照样品时诊断所述对象存在发生复发性流产的风险;优选的,所述微生物标志物为Rhodoluna。
如本文所用,“显著性差异”是指当测试样品中一种或多种微生物或OTU的水平相比对照样品按log2差异倍数计改变至少约1.2倍。“改变”一词包括测试样品中微生物或OTU水性相比对照样品升高或降低。一些实施方式中,测试样品与对照样品之间一种或多种微生物或一个或多个OTU水平的改变可以是按log2差异倍数计相对于对照样品的约1.2倍、1.3倍、1.4倍、1.5倍、1.6倍、1.7倍、1.8倍、1.9倍、2倍、2.1倍、2.2倍、2.3倍、2.4倍、2.5倍、2.6倍、2.7倍、2.8倍、2.9倍、3倍、3.1倍、3.2倍、3.3倍、3.4倍、3.5倍、3.6倍、3.7倍、3.8倍、3.9倍、4倍、4.1倍、4.2倍、4.3倍、4.4倍、4.5倍、4.6倍、4.7倍、4.8倍、4.9倍或5倍及以上。
一些实施方式中,当测试样品中一种或多种微生物或一个或多个OTU的水平按log2差异倍数计相比对照样品高约1.2倍、1.3倍、1.4倍、1.5倍、1.6倍、1.7倍、1.8倍、1.9倍、2倍、2.1倍、2.2倍、2.3倍、2.4倍、2.5倍、2.6倍、2.7倍、2.8倍、2.9倍、3倍、3.1倍、3.2倍、3.3倍、3.4倍、3.5倍、3.6倍、3.7倍、3.8倍、3.9倍、4倍、4.1倍、4.2倍、4.3倍、4.4倍、4.5倍、4.6倍、4.7倍、4.8倍、4.9倍或5倍及以上。
一些实施方式中,当测试样品中一种或多种微生物或一个或多个OTU的水平按log2差异倍数计相比对照样品低约1.2倍、1.3倍、1.4倍、1.5倍、1.6倍、1.7倍、1.8倍、1.9倍、2倍、2.1倍、2.2倍、2.3倍、2.4倍、2.5倍、2.6倍、2.7倍、2.8倍、2.9倍、3倍、3.1倍、3.2倍、3.3倍、3.4倍、3.5倍、3.6倍、3.7倍、3.8倍、3.9倍、4倍、4.1倍、4.2倍、4.3倍、4.4倍、4.5倍、4.6倍、4.7倍、4.8倍、4.9倍或5倍及以上。
本文中使用的术语“样本”或“测试样本”指的是含核酸的任何液体材料或固体材料。在合适的实施方式中,测试样本从生物源获取(即,“生物样本”),比如培养中的细胞,或者是来自动物且最优选地来自人类的组织样本。在示例性实施方式中,所述样本是阴道分泌物。
本发明的方法和产品可以用来利用获取自个体的生物样本来检测与各种细菌有关的核酸。可以根据本领域的技术人员所熟知的任何方法将该核酸(DNA或RNA)从所述样本中分离出来。可以通过标准步骤来获取生物样本并且可以立即使用所述生物样本,或者可以在适于该类型的生物样本的条件下储存该生物样本以备后续使用。
实施例1不明原因复发性流产相关的微生物菌群的检测
1、样本的采集
选取早期妊娠不明原因复发性流产患者40例作为实验组(URSA),选取同期正常妊娠妇女45例作为对照组,后者经过严格匹配。收集实验组和对照组的阴道分泌物。
入组标准:对照组:至少有过一次正常妊娠,且无自然流产病史;URSA组:有2次及2次以上的自然流产病史,符合复发性流产的诊断标准。
排除标准:存在染色体异常、子宫解剖结构异常或男方精液常规异常者,妊娠早期服用致畸药物史;胃肠手术史及严重的肝肾功能不全、消化系统疾病病史;糖尿病、冠心病、恶性肿瘤、甲状腺疾病、免疫系统或血液系统疾病史;生殖道病原微生物感染。研究对象的民族、年龄、BMI及生活方式(吸烟、饮酒)无统计学差异。
2、16S rRNA测序
2.1 DNA的提取
使用DNA提取试剂盒自阴道分泌物中提取细菌DNA,操作步骤按说明书进行。
2.2 DNA样本纯度及浓度测定
利用1%琼脂糖凝胶电泳检测基因组DNA。
2.3 PCR扩增及产物纯化
按指定测序区域,合成带有barcode的特异引物,或合成带有错位碱基的融合引物。
PCR采用TransGen AP221-02(TransStart Fastpfu DNA Polymerase)进行扩增反应,全部样本按照正式实验条件进行,每个样本3个重复,同一样本的PCR产物混合后用2%琼脂糖凝胶电泳检测,使用AxyPrepDNA凝胶回收试剂盒(AXYGEN公司)切胶回收PCR产物,Tris-HCl洗脱;2%琼脂糖电泳检测。
2.4荧光定量
将PCR产物用QuantiFluorTM-ST蓝色荧光定量系统(Promega公司)进行检测定量,之后按照每个样本的测序量要求,进行相应比例的混合。
2.5 Miseq文库构建
使用TruSeqTM DNA Sample Prep Kit进行文库的构建,连接“Y”字形接头,使用磁珠筛选去除接头自连片段,利用PCR扩增进行文库模板的富集,氢氧化钠变性,产生单链DNA片段。具体步骤按照说明书进行。
2.6 Miseq测序
将DNA片段的一端与引物碱基互补,固定在芯片上,另一端随机与附近的另外一个引物互补,也被固定住,形成“桥(bridge)”,PCR扩增,产生DNA簇,将DNA扩增子线性化成为单链,加入改造过的DNA聚合酶和带有4种荧光标记的dNTP,每次循环只合成一个碱基;用激光扫描反应板表面,读取每条模板序列第一轮反应所聚合上去的核苷酸种类;将“荧光基团”和“终止基团”化学切割,恢复3'端粘性,继续聚合第二个核苷酸;统计每轮收集到的荧光信号结果,获知模板DNA片段的序列。
3、数据分析
3.1数据预处理
Miseq测序得到的是Pair-End(PE)双端序列数据,对测得的Fastq数据进行质控处理,最终得到优质的Fasta数据。
使用FLASH、Trimmomatic等软件对Miseq测序得到的PE reads根据overlap关系进行拼接,同时对序列质量进行质控和过滤;使用Usearch软件进行聚类操作,将序列按照彼此的相似性归为许多OUT,采用RDP classifier贝叶斯算法在97%的相似水平下的OTU进行生物信息统计分析,并使用Silva数据库进行比对。
3.2微生物菌群物种差异分析
使用STAMP软件,基于物种分类结果,计算在不同水平上各rank的丰度,比较样本或组间丰度差异,找出样本或组间丰度存在显著差异的物种分类,筛选条件为P<=0.05。
当比较对象为样本时,采用fisher exact test;当比较对象为组时,采用Welch’st-test。最后将检验得到的p value值采用FDR做Multiple test correction得到q value值。
4、结果
结果显示,与对照组相比,Rhodoluna的丰度在不明原因复发性流产的患者中显著下调,差异具有统计学意义(P<0.0001),提示Rhodoluna可以作为生物标志物应用于复发性流产的诊断。
实施例2 Rhodoluna的诊断效能
根据Rhodoluna的相对含量,使用SPSS绘制受试者工作特征曲线(ROC),计算二项精确置信空间,分析Rhodoluna用于复发性流产诊断的灵敏性和特异性。
ROC曲线以及特征参数分别如图2和表1所示,曲线下面积为0.916,最佳临界点的阈值为3.5,该点的敏感性为0.975,特异性为0.733,具有较高的敏感性和特异性。

上述实施例的说明只是用于理解本发明的方法及其核心思想。应当指出,对于本领域的普通技术人员来说,在不脱离本发明原理的前提下,还可以对本发明进行若干改进和修饰,这些改进和修饰也将落入本发明权利要求的保护范围内。

1.一种复发性流产的微生物标志物,其特征在于,所述微生物标志物Rhodoluna。
2.一种检测权利要求1所述的微生物标志物的试剂。
3.一种权利要求1所述的微生物标志物或权利要求2所述的试剂的用途,其特征在于,所述用途包括用于构建预测复发性流产风险的模型、制备诊断复发性流产的产品。
4.根据权利要求3所述的用途,其特征在于,所述产品包括芯片、试剂盒、试纸。
5.根据权利要求3所述的用途,其特征在于,所述模型的输入变量为权利要求1所述的微生物标志物的丰度。
6.根据权利要求5所述的用途,其特征在于,所述微生物标志物丰度的测定方法包括宏基因组测序、16S测序或qPCR定量检测中的任意一种或几种。
7.一种诊断复发性流产的产品,其特征在于,所述产品包括权利要求2所述的试剂。
8.根据权利要求7所述的产品,其特征在于,所述试剂包括检测Rhodoluna的特异性的引物、探针、反义寡核苷酸、适配体或抗体。
9.Rhodoluna在制备预防复发性流产的药物或食品中的用途。
10.根据权利要求9所述的用途,其特征在于,所述药物或食品包括促进Rhodoluna丰度增加的试剂。
