Abstract

技术领域

背景技术
过继细胞转移
过继细胞转移(ACT)方法是治疗癌症的有力新工具。这些方法将患者自身的细胞重新定向,以识别和控制癌症的生长。重要的是,ACT包括CAR T细胞疗法(也称为“CAR T疗法”)。
CAR T细胞疗法包括基因修饰的自体或异体T细胞以表达靶向肿瘤细胞抗原的嵌合抗原受体(CAR)。CAR是一种抗原受体,通常采用单克隆抗体的单链部分可变区,单链部分可变区被设计用于以不依赖于人类白细胞抗原的方式识别细胞表面抗原。发现于正常B细胞并在特定形式的淋巴瘤上过度表达的针对CD19的CAR最近被发现能显著提高患者的反应率,并且在一些患者中提供持久的反应。CAR也正在被开发用于其他血癌,用于靶向肿瘤表达抗原,所述肿瘤表达抗原包括BCMA、CD33、CD22和CD20。最近,CAR-T被工程以靶向实体瘤上发现的抗原,包括EGFR、EGFRvIII、Erb-B2、CEA、PSMA、MUC1、IL13-Rα2和GD2(D’Aloia等,2018,Cell Death and Disease,9:282-293)。

基因编辑技术
随着特定位点编辑方法的出现,如TALEN、CRISPR/cas9和锌指核酸酶(ZFN)方法,基因编辑技术有了实质性的进步。这些方法对恶性遗传病患者和非恶性遗传病患者具有治疗潜力。
基因编辑精确和永久地改变基因组DNA序列,所述基因组DNA序列仍处于内源性遗传调控下,并且控制修饰后遗传因子的正确地和适当地表达。目前用于人类基因组基因编辑的核酸酶主要有四类:锌指核酸酶(ZFN)、转录激活因子-样效应物核酸酶(TALENs)、大范围核酸酶(MNs)和簇集的规则地间隔的短回文重复(CRISPR/Cas9)。这些中的每一类都能识别并结合特定的DNA靶序列。根据不同的方法,靶DNA可以在一条或两条链上被切割。为了纠正突变,使用一个修正模板对靶向损伤部位的引入的断裂进行同源定向修复。这项技术还可以通过结合突变插入或缺失来沉默或消融特定基因。此外,基因编辑技术还可用于功能性地将一个基因替换为另一个基因,例如在T细胞受体α常数位点(TRAC)内,从而改变T细胞的特异性(Eyquem等,2017,Nature,543:113-117)。
通常的淋巴细胞消减
在给患者施用一定剂量的工程免疫细胞之前,通常会消减患者的淋巴细胞。淋巴细胞消减过程,对ACT方法的成功来说,被认为是重要的,事实上是必不可少的。这一过程在免疫微环境(如骨髓)中创造了足够的空间,使被转移的细胞能够移入。它还通过诱导良好的细胞因子谱,为被转移细胞的成功植入、增殖和持久性创造了良好的免疫稳态环境。它诱导了这种细胞因子谱,特别是在外周免疫领域(如骨髓、脾脏和淋巴结)中,以建立和增殖工程细胞。(见,例如,Maine等,2002,J.Clin.Invest.110:157-159;Muranski等,2006,Nat.Clin.Pract.Oncol.3(12):668-681;Klebanoff等,2005,Trends Immunol.26(2):111-117)
基于化学疗法的淋巴细胞消减
在采用ACT方法,例如CAR T细胞疗法,之前,通常使用高度细胞毒性化疗药物的组合,特别是环磷酰胺和氟达拉滨,来消减患者的淋巴淋巴细胞。这些药物可以减少淋巴细胞的数量。然而,它们是剧毒的。它们不仅以非靶向方式消减免疫系统,而且可能损害其他正常细胞和组织。不是所有的病人都能忍受它们。此外,尤其是在CAR T细胞疗法中,持久应答率通常低于50%。许多患者在接受CAR T细胞疗法后最终复发,需要进一步的治疗干预或干细胞移植(如骨髓移植)。
基于抗体的淋巴细胞消减
抗体具有比化疗药物更高的细胞靶向特异性。因此,抗免疫细胞特异性抗原的抗体作为淋巴消减药物的化疗药物的潜在替代物引起了人们的兴趣。CD45是一种免疫细胞特异性抗原。一般来说,除了成熟的红细胞和血小板外,所有造血源细胞都表达CD45。CD45的高表达也见于大多数急性淋巴细胞白血病和急性骨髓性白血病。例如,CD45在循环白细胞和恶性B细胞上以每个细胞约200000到300000个位点的密度表达。
基于抗-CD45抗体的淋巴细胞消减是已知的(见,例如,Louis等,2009,Blood,113:2442-2450)。然而,这种方法也有缺点。例如,在Louis等的研究中,8例患者输注抗-CD45抗体来消减淋巴细胞后,预期的T细胞外周血频率升高。然而,只有三个病人有临床疗效,只有一个病人有完全应答。
未满足的需要
在基于细胞疗法(如CAR T细胞疗法和TCR细胞疗法)之前,需要更好的方法来消减受试者的淋巴细胞。也就是说,需要一种淋巴细胞消减方法,即(i)使用比化疗药物更特异的药物,(ii)在低剂量下足够有效,并且(iii)使至少某些类型的淋巴细胞免于显著消减。
发明内容
本发明提供了一种消减受试者免疫细胞的方法,包括在以单剂量给受试者施用有效量的放射性标记的抗-CD45抗体,受试者的免疫细胞包括淋巴细胞、调节性T细胞、髓源性抑制细胞、肿瘤相关巨噬细胞、活化巨噬细胞,以及它们的组合。
本发明还提供了一种用于消减人类受试者免疫细胞的方法,包括给受试者施用25mCi~200mCi(例如,50mCi、100mCi或150mCi)131I-BC8。本发明还提供了一种消减人类受试者免疫细胞的方法,包括向受试者施用0.05μCi/kg~5.0μCi/kg受试者重量的225AC-BC8。
本发明还提供了一种治疗患有癌症的受试者的方法,其包括(i)向受试者施用一定量的有效地消减受试者淋巴细胞的放射性标记的抗-CD45抗体,以及(ii)在适当的时间段后,对受试者进行过继细胞疗法以治疗受试者的癌症。

具体实施方式
本发明提供了一种基于放射性标记的抗-CD45抗体的用于消减受试者的淋巴细胞的方法,以及相关的方法和制品。当这些方法先于某些基于细胞疗法使用时,这些方法能够提高基于细胞疗法的结果,同时最小化不良反应。
定义
在本申请中,某些术语的含义如下。
除非上下文另有明确规定,否则单数形式“a”、“an”、“The”等包括复数指称。因此,例如,对“an”抗体的引用包括单个抗体和多个不同抗体。
当在数字名称(例如温度、时间、数量和浓度,包括范围)之前使用时,术语“大约”包含一个范围,表示可能变化±10%、±5%或±1%的近似值。
如本文所使用的,关于抗体的“施用”是指通过任何已知的适合于抗体递送的方法将抗体递送到受试者的身体。具体的施用方式包括但不限于静脉施用、经皮施用、皮下施用、腹腔施用、鞘内施用和瘤内施用。抗体的示例性施用方法可如通过引用并入本文的国际公布号WO 2016/187514进行实质性描述。
此外,在本发明中,可以使用一个或多个常规使用的医药上可接受的载体来制备抗体。本领域技术人员熟知这种载体。例如,可注射药物给药系统包括溶液、悬浮液、凝胶、微球和聚合可注射物,并且可包括赋形剂,例如溶解度改变剂(例如乙醇、丙二醇和蔗糖)和聚合物(例如聚己内酯和PLGA)。

如本文所用,对于受试者的淋巴细胞的“消减”,应指降低受试者的至少一种淋巴细胞(例如,受试者的至少一种外周血淋巴细胞或至少一种骨髓淋巴细胞)的数量。根据本发明的某些优选方面,通过测量受试者的外周血淋巴细胞水平来确定受试者的淋巴细胞减少。因此,举例来说,如果受试者的至少一种外周血淋巴细胞的数量降低不超过99%,则受试者的淋巴细胞数量被消减。例如,如果受试者的外周血T细胞水平降低50%,受试者的外周血NK细胞水平降低40%,和/或受试者的外周血B细胞水平降低30%,则受试者的淋巴细胞被消减。在本例中,即使另一种免疫细胞类型(如中性粒细胞)的水平没有降低,受试者的淋巴细胞也被消减。根据某些方面,受试者淋巴细胞的消减反映在外周血淋巴细胞数量减少至少10%、20%、30%、40%、50%、60%、70%、80%、90%或99%。
外周血淋巴细胞计数方法为常规方法。例如,它们包括对全血样本使用流式细胞术,以通过标记针对特定细胞表面标记物(如CD45、CD4或CD8)的荧光抗体来确定淋巴细胞计数。测量外周血中性粒细胞数量的方法也是常规的。例如,它们包括对全血样本使用流式细胞术,以通过标记针对特定细胞表面标记物(如Ly6G)的荧光抗体来确定中性粒细胞计数。

对于用放射性同位素标记的抗体,给受试者施用的大多数药物通常由非标记抗体组成,少数为标记抗体。标记抗体对非标记抗体的比例可以用已知的方法进行调整。因此,根据本发明的某些方面,抗-CD45抗体的总蛋白含量可以高达60mg(例如5mg至45mg),或0.1mg/kg至1.0mg/kg患者体重(例如0.2mg/kg至0.6mg/kg患者体重)之间的总蛋白量。

过继细胞疗法可包括施用表达嵌合抗原受体(CAR)或T细胞受体(TCR)的细胞,或可包括肿瘤浸润淋巴细胞(TIL)。表达CAR/TCR的细胞群可包括活化的T细胞群或自然杀伤(NK)细胞群或表达识别抗原的CAR/TCR的树突状细胞群。树突状细胞具有抗原递呈和直接杀伤肿瘤的能力。表达CAR/TCR的细胞群可包括基因编辑细胞群。

T细胞基因编辑以生成同种异体的通用的CAR T细胞的例子包括Eyquem及其同事的工作(Eyquem等,2017,Nature,543:113-117)。在这项研究中,重组CAR基因构建体有效地替代内源性T细胞受体α常数位点(TRAC)。通过这种方法,重组的CAR被有效地置于细胞自身TCR调节信号的控制下。通过同样的策略,可通过敲入T细胞受体β常数基因位点(TRBC)或已知在所有T细胞中表达的β-2微球蛋白(B2M)MHC-I-相关基因位点而有效插入CAR或重组TCR。另一个例子包括Ren及其同事的工作(Ren等,2017,Clin.Cancer Res.23:2255-2266)。认识到监测点受体具有免疫抑制作用,可能会抑制外源性自体或同种异体CAR T细胞的刺激,该研究小组利用CRISPR/cas9技术消融内源性TCRα和β位点(TRAC和TRBC)和B2M基因,同时也使内源性PD1基因沉默。通过这种方法,工程细胞不会引发移植物抗宿主病,但确实能够抵抗免疫监测点受体的抑制。

本发明的方面
本发明通过提供一种出乎意料的优越的方法来解决本领域中未满足的需求,该方法理想地在基于细胞疗法(例如CAR T细胞疗法或TCR细胞疗法)之前,对受试者进行淋巴细胞消减。本发明利用放射性标记的抗-CD45抗体,例如131I-BC8,来消减受试者的淋巴细胞。抗体能以惊人的低剂量消减受试者的淋巴细胞。这种方法避免了化疗等非特异性药物引起的某些不良反应。此外,使用这种方法,至少某些类型的CD45+免疫细胞,如中性粒细胞,可以令人惊讶地避免显著的消减。
具体而言,本发明提供一种用于消减受试者淋巴细胞的方法,包括向受试者施用有效量的放射性标记的抗-CD45抗体,所述受试者最好是人类。
例如,这种淋巴细胞消减方法对于改善需要消减淋巴细胞的后续治疗的结果是有用的。根据该方法的某些优选方面,受试者患有癌症,即将接受过继细胞疗法来治疗癌症。过继细胞疗法是已知的,包括,例如,CAR T细胞疗法(例如,自体细胞疗法和异体细胞疗法)。CART细胞疗法优选地用于治疗血液系统恶性肿瘤,如ALL、AML和CLL。经批准的CAR T细胞治疗的例子包括,但不限于,用于治疗NHL和DLBCL的(tisagenlecleucel),和用于治疗NHL的(axicabtagene ciloleucel)。
这些目前公开的方法可改善血液病(包括实体瘤)的治疗结果,和/或可减少与过继细胞疗法(如CAR T细胞疗法和/或)相关的副作用。例如,过继细胞疗法的副作用包括神经毒性、细胞因子释放综合征(CRS)、低丙种球蛋白血症、血细胞减少、毛细血管渗漏综合征(CLS)、巨噬细胞活化综合征(MAS)、肿瘤溶解综合征(TLS)及其组合。此外,当与不施用放射性标记的抗-CD45抗体的方法相比,本公开的方法可延长表达CAR/TCR或TIL的细胞群的持续性。
本发明可以减少CAT-T疗法的一些副作用,和/或可以通过提供免疫抑制细胞(例如调节性T(T-reg)细胞和髓源性抑制细胞(MDSC))的靶向淋巴细胞消减的方法来改善CAR-T疗法的治疗结果。两种细胞类型(即T-reg和MDSC)都能抑制CAR T细胞疗法的活化和疗效。此外,本发明还提供了免疫抑制细胞(如单核细胞和肿瘤相关巨噬细胞(TAM))的靶向淋巴细胞消减的方法,所述免疫抑制细胞与导致毒性的细胞因子释放有关,如细胞因子释放综合征(CRS)和与CAR T细胞相关的神经毒性。
肿瘤,包括实体肿瘤和液体肿瘤均已经进化出了劫持和/或逃避免疫系统的方法以作为一种延续和成长的手段。这被称为恶性肿瘤免疫微环境(TME)。一个经典且相关的例子是与在T细胞表面的PD1结合的在肿瘤细胞表面的配体PD-L1的上调表达导致免疫细胞活化的下调。有趣的是,尽管阻断这种机制已经导致了在几种不同类型的癌症中显著的应答率和持久的生存率,但大多数患者对这种治疗形式(即抗-PD1/PD-L1)没有反应,这意味着肿瘤微环境中的免疫逃避是多方面和复杂的。为此,肿瘤部分地通过致癌表达、信号传导和细胞因子的产生,对免疫系统造成挑战,从而阻碍有效的抗肿瘤应答的产生。这会导致一个氧化应激、营养素缺乏、酸性pH值和缺氧的环境。此外,这些抑制性免疫细胞(T-reg和MDSC)和肿瘤相关巨噬细胞(TAM)的存在可以通过直接接触或释放抑制性可溶性因子和细胞因子有效地抑制免疫细胞的活化。
虽然患者的内源性免疫系统可能会遇到这样的环境并导致缺乏抵抗力的抗肿瘤免疫应答,但过继细胞疗法(如CAR T细胞疗法)也可能易受这些免疫抑制机制的影响,从而限制了这些新细胞疗法对肿瘤产生有效应答的能力。
CAR T疗法和过继细胞疗法(ACT)通常是临床研究中最有希望的抗癌策略之一。这些肿瘤的应答率非常高,达到80%,尽管持久应答只有40-50%左右(见,例如,图6所列的研究)。然而,这些结果表明这些患者的结果有了显著改善。虽然肿瘤免疫微环境可能是调节细胞疗法的应答的一个因素,但尚不清楚为什么有些患者会有应答,而有些患者则没有。
为此,临床前和临床研究表明,调节性T细胞(T-reg)对小鼠和黑色素瘤患者的ACT的应答有影响(Gattinoni等,2005,JEM,202:907;Yao等,2012,Blood,119:5688)。在这些研究中,T-reg的消减,无论是通过有意消减还是通过使用外部射线辐射调节的作用,对ACT的抗肿瘤应答都有良好的影响。有趣的是,这些和其他研究表明,放疗后的T-reg的消减更为持久,这与化疗诱导调节相反,后者的化疗诱导调节后的T-reg迅速反弹,结果较差。
MDSC和TAM是与肿瘤免疫微环境不良有关的其他细胞类型。通过代谢基因表达的上调,如吲哚胺2,3-双加氧酶(IDO)、腺苷A2A受体和CD73,肿瘤可以有效地在肿瘤环境中造成营养缺乏,从而抑制T细胞的活化。例如,通过来自肿瘤的IDO和MDSC的色氨酸代谢导致T细胞无效能和死亡,以及T-reg在肿瘤部位的积聚。此外,这些免疫抑制细胞可能分泌免疫调节细胞因子,如也可能对T细胞活化产生负面影响的TGF-β。
恶性肿瘤免疫微环境对液体肿瘤和实体肿瘤都可能存在负面影响,尽管在实体肿瘤中可能甚至更明显。为此,早期临床结果表明,在液体肿瘤(如淋巴瘤)中的CAR-T疗法的强烈应答并未在实体瘤中观察到,这表明实体瘤中可能存在对产生有效的CAR-T介导的免疫反应造成物理或代谢障碍的因素或条件(Newick等,2016,Mol.Ther.–Oncolytics,3:16006;D’Aloia等,2018,Cell Death and Disease,9:282-293)。
肿瘤免疫微环境也牵涉到与CAR-T施用有关的两个主要不良反应,即细胞因子释放综合征(CRS)和神经毒性。最近的临床前研究表明,导致CRS或神经毒性的细胞因子释放是由于募集至CAR-T和肿瘤细胞的部位后被激活的巨噬细胞所致。小鼠研究结果(Giavadris等,2018,Nat.Med.24:731)证明巨噬细胞被在肿瘤部位的CAR-T细胞募集和激活之后分泌IL-1或IL-6。
研究表明,调理可以改善免疫稳态环境,从而使在输注后能够在体内成功地进行ACT或CAR-T移植和扩张。然而,细胞毒性非特异性化疗的使用可引起脱靶毒性,并且已被确定为在CRS中的危险因素和CAR-T施用之后的神经毒性(Hay等,2016)。有趣的是,大多数CAR-T项目利用氟达拉滨和环磷酰胺(flu/cy)的联合使用作为CAR-T前的调理疗法。这些药物通常在ACT输注前的2-7天施用,使用2-5天的疗程。
本发明的调理的靶向治疗提供了一种利用CAR-T提高结果的显著改进的策略。在本文描述的本发明中,不仅淋巴细胞可以被靶向消减,还有那些参与介导恶性肿瘤免疫微环境的免疫细胞类型,以及那些牵涉到CAR-T不良反应(如CRS和神经毒性)的免疫细胞类型。本发明以正常免疫细胞为靶点,包括T-reg、MDSC、TAM和分泌IL-1和/或IL-6的活化巨噬细胞。这样做,本发明可能在CAR-T的结果和安全性方面有显著改善。
此外,本发明靶向患者癌细胞,主要地靶向造血肿瘤,以减轻肿瘤负荷并增加CAR-T抗肿瘤应答的可能性。更具体地说,本发明提供了一种靶向CD45抗原的治疗策略,所述CD45抗原存在于除红细胞和血小板之外的所有正常有核免疫细胞上。CD45也在大多数淋巴和白血病肿瘤细胞上表达。虽然裸抗体已显示出对减少免疫细胞群的一些影响,但本发明的放射性标记的抗-CD45抗体将对参与调节CAR-T应答的免疫细胞产生更显著和更持续的抑制,这与外部射线辐射一致,但本发明的放射性标记的抗-CD45抗体具有靶向性。这样,放射被靶向CD45细胞群,并对CD45细胞群是有效的,同时使正常组织不受伤。更具体地说,放射标记的抗CD45抗体可提供为单一剂量,单一剂量的水平足以有效地消减脾脏、淋巴结和外周血中的循环免疫细胞,但对骨髓中的造血干细胞的影响有限。重要的是,除淋巴细胞消减外,巨噬细胞、MDSC和T-reg也将被消减,以改善CAR-T疗法的活化和反应并减轻CRS和神经毒性的不良反应。
根据某些方面,可以通过构建与肿瘤细胞上的抗原特异性结合的所需抗原结合域,CAR T细胞可以被工程以靶向感兴趣的肿瘤抗原。在本发明的上下文中,“肿瘤抗原”或“增殖性疾病抗原”或“与增殖障碍有关的抗原”是指特定的增殖性疾病(如癌症)共有的抗原。本文所讨论的抗原仅作为示例而包括,并不意指排他的,并且对于本领域技术人员来说,进一步的示例将是显而易见的。

因此,并参照图1A,本发明提供用于治疗增殖性疾病(例如恶性血液肿瘤)的方法,所述方法包括施用放射性标记的抗-CD45抗体和过继细胞疗法。过继细胞疗法通常可包括自体细胞的分离,其可在经放射性标记的抗-CD45抗体进行淋巴细胞消减后的再输注(过继细胞疗法,如CAR T细胞疗法)之前进行基因编辑。或者,同种异体细胞可以在淋巴细胞消减后再输注,以提供过继细胞疗法。根据本发明的方法,如图1B所示,可在过继细胞疗法前的3至9天(例如6至8天)以单剂量提供放射性标记的抗-CD45抗体。

实施例1–131I-BC8(Iomab-B)
Iomab-B药物产品是放射性碘化的抗-CD45鼠源性单克隆抗体(mAb)(131I-BC8)。它对造血CD45抗原是特异性的。Iomab-B药物产品作为被包含在容器闭合系统中的无菌制剂供给,所述容器闭合系统由去热原的1型50mL玻璃瓶、灭菌的灰色氯丁橡胶塞和顶开型铝封条组成。每个剂量瓶也含有在50mL瓶中的45mL药物产品填充体积。类似地,它提供为在静脉内施用过程中完成输注的单次使用剂量,且含有1mCi~200mCi(例如,100mCi或150mCi)的131I和6.66~45mg的BC8的患者特异性放射性。根据理想体重在0.5mg/kg的水平确定BC8抗体剂量。以1:9的药物产品与盐水溶液的比例,将药物产品与0.9%氯化钠注射USP(生理盐水溶液)一起共同施用给患者。在不同的期间施用大约430~450mL的总药物产品和盐水输注体积,因为输注速率取决于在45mL药物产品填充体积中的BC8抗体的量。

实施例2–225Ac-BC8
抗CD45 BC8与DOTA的缀合及之后用225Ac标记:使用具有50000的分子量截止值的Centricon过滤器或具有50000的分子量截止值的Vivaspin超滤管,通过四次超滤旋转,将抗体BC8(2mg)用缀合缓冲液(含有1mM EDTA的碳酸钠缓冲液,pH=8.5~9.0)平衡。使用1.5ml缀合缓冲液/旋转。对于每次旋转,将抗体在53000RPM和在4℃旋转5~20分钟至100~200μl的残余渗余物体积。在第2次和第3次旋转以后将抗体在4℃温育30分钟以允许平衡。对于DOTA缀合,通过溶解和涡旋来制备在0.15M NH4OAc中的3mg/ml的S-2-(4-异硫氰酸苯甲酰基)-1,4,7,10四氮杂环十二烷四乙酸(p-SCN-Bz-DOTA;MW=687)的溶液。将DOTA-Bz-pSCN和BC8抗体(在>5mg/ml)在Eppendorf试管中以7.5摩尔比(DOTA:抗体)混合在一起并在室温温育15小时。为了纯化DOTA-抗体缀合物,通过用1.5ml 0.15M NH4OAc缓冲液(pH=6.5)超滤七轮至大约100μl的体积,除去未反应的DOTA-Bz-pSCN。最终的洗涤以后,加入0.15M NH4OAc缓冲液以使物质达到大约1mg/ml的终浓度。测量DOTA-BC8缀合物的终浓度,并确定与抗体缀合的DOTA分子的数目为每个抗体1.2~1.5个DOTA。
用225Ac对DOTA-抗体缀合物的放射性标记:为了用225Ac标记DOTA-BC8缀合物,在Eppendorf反应试管中将15μL 0.15M NH4OAc缓冲液(pH=6.5)与2μL(10μg)DOTA-BC8(5mg/ml)混合。随后加入在0.05M HCl中的4μL 225Ac(10μCi),将试管的内容物用移液器尖头混合,并将反应混合物在100rpm摇动下在37℃温育90分钟。在温育时段结束时,将3μL 1mMDTPA溶液加入反应混合物并在室温温育20分钟以结合未络合的225Ac。用10cm硅胶条带和10mM EDTA/生理盐水流动相执行快速薄层色谱法(ITLC)以确定225Ac-BC8的放射化学纯度,将225Ac-标记的BC8与225Ac-DTPA分离,并在配备多通道分析仪的γ计数器中计数切片。确定在几批中的放射性标记效率大于80%。
实施例3–中性粒细胞绝对计数的变化
CD45是一种表达于大多数免疫细胞类型的细胞表面蛋白,包括淋巴细胞和中性粒细胞。Iomab-B(131I-BC8)放射免疫疗法靶向并将其β放射有效载荷传递给CD45阳性细胞。在定量施用131I-BC8后,所有CD45阳性细胞类型都同样容易消减。图2描绘了在使用10mCi剂量的131I-BC8后的在不同时间点的相对绝对中性粒细胞计数,呈现为倍数增加或减少。随着时间的推移,绝对淋巴细胞数显著减少并表现出持续减少,而中值的中性粒细胞绝对数在定量施用131I-BC8后减少最小,而恢复迅速。这是一个令人惊讶的发现。131I-BC8对中性粒细胞的有限影响及其快速反弹有望通过预防可能发生的感染而使患者受益。
实施例4–证明淋巴细胞消减和清除的临床数据
从被给予低剂量水平的131I-BC8的患者获得的临床数据显示,外周血淋巴细胞减少的情况是一致的。药代动力学数据显示131I-BC8的快速清除。这种清除限制了与CAR T产品的相互作用。增强的疾病控制和延长的淋巴细胞消减允许在131I-抗-CD45抗体施用和CAR-T输注之间有一个灵活的时间窗口,从而防止毒性。例如,如图3A-3F,使用替代的CD45抗体(30F11)对131I-抗-CD45抗体靶向淋巴消减后的免疫细胞分析显示,在外周血和脾脏免疫细胞群(图3B)中的白细胞(图3A)的选择性的消减,对骨髓腔室(图3C)的影响最小,并保留红细胞和血小板(分别是图3D和图3E)。图3F提供这些数据的条形图。图12显示了131I-抗CD45抗体从患者循环血液中的快速清除。平均而言,从血液中清除的131I-抗CD45抗体中,59%的生物半衰期为0.65小时(39分钟),41%的生物半衰期为31小时。
实施例5–开发中的各种CART细胞疗法及其淋巴细胞消减方案
有许多CAR T细胞疗法正在开发中,每个疗法都有自己的淋巴细胞消减方案。图4中的图表显示了针对患有B-细胞NHL的病人的抗-CD19CAR T细胞疗法的选定的已发表的试验。这些试验大多采用化疗的淋巴细胞消减方案。

实施例8–抗-CD45放射免疫疗法介导的调理/淋巴细胞消减的临床前模型
概要:在患者接受过继细胞转移(如工程自体或同种异体CAR T细胞)的剂量之前,通常使用高剂量化学疗法来执行淋巴细胞消减步骤。这一过程被认为对创造足够的空间在免疫微环境(如骨髓)是重要的,以允许被转移细胞的植入。此外,它似乎引发了一个有利的用于供体淋巴细胞的建立和增殖的细胞因子谱。抗-CD45放射免疫治疗作为在AML患者的同种异体造血细胞移植之前的一种清髓方案正在进行III期临床试验。这项试验的结果表明,低剂量的131I-抗-CD45放射免疫疗法可能足以消减淋巴细胞,但不是清髓性的。
例如,调查接受了单剂量为50mCi~200mCi的131I-抗-CD45的人类患者在脾脏中吸收的累积辐射剂量的研究已被实施。如表1所示,对脾脏的剩余吸收剂量不超过25cGy的时间随131I-抗CD45的总剂量而变化。此外,图11通过输注施用100mCi 131I-抗-CD45的患者的脾脏的脾脏累积剂量率。根据脾脏累积剂量率和输注后的时间进行绘图。

实施例10–基因-编辑的T细胞
本例涉及癌症患者在施用一剂或多剂含有基因-编辑T细胞的过继细胞疗法之前的淋巴细胞消减。
CAR T细胞在临床上显示出相当大的前景,难治性淋巴瘤患者群体的应答率超过80%,近50%的治疗患者的持久应答持续6个月或更长时间。
然而,这些工程T细胞仍然易受免疫调节控制,例如免疫监测点受体(如PD1、Lag3或TIM3)的上调。这些受体介导一种衰竭状态并限制工程细胞的活化潜能。对于同种异体CAR T细胞,这些外源性施用的细胞具有诱发移植物抗宿主病(GVHD)的风险。这种风险是由于存在于被工程的同种异体T细胞上的自身T细胞受体识别错配的主要组织相容性抗原引起的。
通过基因编辑技术,如CRISPR/cas9,可以有效缓解这些不良结果。例如,切除编码PD1的基因将消除CAR T抗肿瘤应答的检查点调控的可能性。此外,在同种异体CAR T细胞的准备中,用基因编辑去除内源性TCR位点可有效预防GVHD(Ren等)。淋巴细胞消减是使施用的CAR T细胞能够成功植入、增殖和持久的一个重要步骤。然而,需要更安全、更有效的淋巴细胞消减方法(如主题的方法)来代替非特异性化疗和放射治疗的使用。众所周知,非特异性方案可能导致CAR-T相关毒性的出现,如细胞因子释放综合征(CRS),而基因编辑的CAR T细胞也不能免除这种风险。主题的基于CD45的淋巴细胞消减方法是在基因编辑的CAR T之前消减淋巴细胞的一种更安全、靶向的和更有效的方法,无论CAR T细胞是否因为监测点受体或内源性TCR而被消融。
实施例11-Iomab-B的多中心关键3期研究的红骨髓放射量测定和降低活性水平的
时间
进行计算,以评估输注131I-抗-CD45(Iomab-B)后活性水平下降至减少的水平或假设的“安全”水平所需的时间,以使红骨髓中的辐射剂量影响最小化,从而实现并促进后续的细胞骨髓恢复治疗。
施用Iomab-B后,放射性标记抗体的活性从显著摄取的每个主要器官呈指数下降。在不希望受到理论约束的情况下,一种可能的解释是,由于生物清除加上放射性衰变的联合效应,红骨髓的剂量率随时间呈指数下降,这样就可以达到一个时间点,在这个时间点之后,经过无限时间的骨髓的总剩余剂量不超过25cGy。25cGy值代表一个相对“安全”的吸收剂量的估计值,该剂量不应对先前使用大剂量Iomab-B治疗后的红骨髓细胞再生和恢复产生不利影响。
数据回顾:在临床试验中的25例接受Iomab-B治疗的患者中,红骨髓中131I抗-CD45抗体(Iomab-B)的平均初始摄取率为17.4%,以平均有效半衰期为45.1小时被清除。图10显示输注了100mCi的Iomab-B患者的红骨髓的平均放射剂量率(cGy/小时)。根据骨髓的放射剂量率和输注后的时间进行绘图。消失曲线代表一个指数函数,保留半衰期为45.1小时。输注后154小时(约6.5天),曲线下面积显示在所有剩余时间内,不超过25cGy的总吸收剂量被给予红骨髓(约占总剂量271cGy的9%)。
图10还显示红骨髓的平均初始剂量率为4.16cGy/小时。达到25cGy点的时间将随着输注的活度的增加而增加。表2显示了输注50mCi、75mCi、100mCi、150mCi和200mCi 131I-抗-CD45抗体的25cGy时间点。

个体患者差异性:患者在Iomab生物分布和清除动力学方面存在差异。尽管红骨髓的平均初始摄取量占总输注量的17.4%,但一名患者输注后观察到的最高红骨髓摄取量为36%。从骨髓的平均清除半衰期为45.1小时,最长观察到的清除半衰期为71小时。红骨髓的平均吸收剂量为2.71cGy/mCi 131I,而当前方案中观察到的最高值为4.24cgy/mCi。
实施例12-小鼠131I-BC8靶向淋巴细胞消减后的T-reg细胞的免疫表型。
使用131I-抗-CD45抗体定向放射免疫疗法对小鼠靶向淋巴细胞消减的研究表明,所述研究能够确定包括T-Reg在内的淋巴细胞可被靶向消减的剂量,同时有效地避免对骨髓的影响。图4A-4D和5A-5C表明,剂量为50-200μCi的131I-BC8会消减淋巴细胞和T-reg,并且对骨髓细胞没有明显影响。用以指导抗肿瘤作用的B细胞淋巴瘤小鼠肿瘤模型显示,淋巴细胞消减的剂量水平比评估值高1.5-4倍。本发明的更低的淋巴细胞消减剂量也将靶向CD45阳性肿瘤细胞并有助于抗肿瘤作用,减少包括PD1阳性肿瘤在内的肿瘤负荷,并改善ACT或CAR-T结果。综上所述,当用于为ACT作准备时,本发明将靶向免疫抑制肿瘤微环境,从而改善ACT的植入、应答和抗肿瘤结果。

Claims (21)
1.一种用于消减受试者的免疫细胞的方法,包括以单剂量向所述受试者施用有效量的放射性标记的抗-CD45抗体,其中所述受试者免疫细胞包括淋巴细胞、调节性T细胞、髓源性抑制细胞、肿瘤相关巨噬细胞、活化巨噬细胞,及其组合,其中所述放射性标记的抗-CD45抗体是放射性标记的BC8。
2.根据权利要求1所述的方法,其中所述放射性标记的BC8的所述有效量消减至少50%的受试者淋巴细胞,或至少70%的受试者淋巴细胞,或至少80%的受试者淋巴细胞。
3.根据权利要求1所述的方法,其中所述放射性标记的BC8抗体的所述有效量不会消减所述受试者的中性粒细胞。

16.根据权利要求14所述的方法,其中所述过继细胞疗法在施用所述放射性标记的BC8后的6、7或8天执行。
17.根据权利要求1所述的方法,其中所述放射性标记的BC8的所述单剂量包括未标记的BC8,其中标记的的BC8:未标记的BC8的比值为从0.1:10至1:1。
18.根据权利要求1所述的方法,其中所述放射性标记的BC8的所述单剂量包括高达60mg的总蛋白质量,或0.2mg/kg患者重量~0.6mg/kg患者重量之间的总蛋白质量。
