背景技术
在缺血中,器官或组织的含血量减少。缺血可以是全身性贫血的局部表现或是局部血液循环障碍的结果。缺血类型包括:1):可以由例如肿瘤、绷带过紧和积液对动脉血管的压迫导致血管腔变狭窄或闭塞所致的压迫性缺血。临床上,由于卧侧血管受到压迫,长期躺卧形成的痔疮或溃疡是缺血造成的组织坏死的适例,这可以引起肌肉损伤。2)因动脉内血栓形成或栓塞的阻塞性缺血可以造成血管阻塞,导致供给至例如肢体或心脏的血液受阻。3)可以由大量血液急剧流入腹腔脏器而造成其它器官和组织缺血造成侧肢性缺血。
患有下肢周边动脉疾病的患者年龄大多超过60岁,且这些患者中约一半患有糖尿病。目前,针对缺血性下肢进行血管新生治疗已有药物上市。例如,AutoloGel system是通过抽取患者自体的高浓度富含血小板的血浆(PRP),并添加促进伤口愈合的生长因子和细胞因子以形成胶状物质制备的伤口敷料。然而,这类治疗方法仅仅用于治疗慢性伤口,但它们不能治疗潜在的缺血。其它治疗方法如血管绕道手术(bypass grafting)、血管扩张术和放置血管支架对于解决血管堵塞是必要的。
对于缺血性下肢目前也有许多研究正积极研发血管新生的疗法,如与血管新生信号传递相关的重组生长因子,如刺激血管新生的VEGF和FGF。已经发现血小板衍生生长因子(PDGF)可在发育组织或成体组织中刺激间叶细胞增生、迁移和分化,并且应用于促进患者骨髓释放内皮衍生细胞,达到血管增生功效。也可以由与血管生成信号间接相关的物质刺激人类脐静脉内皮细胞(HUVEC)以刺激血管生成。利用组织纤溶酶原激活物(tPA)和HUVEC给予治疗,以增加内皮前趋细胞从骨髓迁移至血管的数量,以促进血管内皮新生,达到治疗效果。
除了上述对血管生成的直接促进外,研究还发现可以用聚(乳酸-共-乙醇酸)(PLGA)纳米颗粒处理肝素,以实现治疗性血管生成。它的作用是通过延迟释放药物来刺激血管生成相关生长因子的表达。肝素(也称为普通肝素、分级肝素(也称为低分子量肝素)及其低分子量衍生物(例如,依诺肝素、达肝素和亭扎肝素)可有效预防深静脉血栓形成和肺栓塞。它们通常通过静脉内给药或皮下给药。
糖尿病造成的高血糖的生理情况会使血管内皮生长因子的分泌减少,使得在血管病变严重时可能导致截肢。目前大多数的治疗方式与血管生长因子相关,其在临床使用上或医学伦理上都面临着种种困难。如今,仍然没有有效的治疗方法来再生缺血性组织,以及从截肢中拯救肢体。因此,发展出适合大多数患者缺血组织的治疗性组合物是需要解决的重要问题。
发明内容
在一方面,本文描述了用于治疗缺血性组织的药物组合物。该药物组合物包含核心成分和基质成分,其中核心成分包括肝素或其衍生物肝素,并且基质成分包括透明质酸或其衍生物,该药物组合物具有大于10mPa·s的黏度。在一些实施方案中,黏度为10至10000mPa·s。在一些实施方案中,黏度在3至10mg/ml的平均分子量为700至2000kDa的透明质酸的黏度范围内。在一些实施方案中,黏度与5mg/ml的平均分子量为1560kDa的透明质酸的黏度相同。
在一些实施方案中,透明质酸的平均分子量为100kDa至5000kDa(例如,700kDa至2000kDa)。在一些实施方案中,药物组合物含有1mg/ml至100mg/ml的透明质酸。透明质酸的平均分子量可以为700至2000kDa,并且透明质酸的浓度可以为3至10mg/ml。在一些实施方案中,透明质酸的平均分子量为1560kDa,并且透明质酸的浓度为5mg/ml。
药物组合物中的肝素或其衍生物可以是选自由普通肝素(UFH,unfractionatedheparin)、分级肝素(fractionated heparin,低分子量肝素)、类肝素、依诺肝素、达肝素或亭扎肝素所组成的组。在一些实施方案中,药物组合物含有0.00001U至20U/100μl药物组合物的肝素或其衍生物。
在一些实施方案中,基质成分还包含胶原蛋白、细胞外基质因子、蛋白质或多糖。
在一些实施方案中,核心成分还包含溶栓药物或血管生成物。溶栓药物或血管生成物可选自由噻氯匹定(ticlopidine)、华法林(warfarin)、组织纤溶酶原激活物(tissueplasminogen activator)、阿尼普酶(eminase)、瑞替普酶(retavase)、链激酶(streptase)、组织纤溶酶原激活物、替奈普酶(tenecteplase)、阿伯激酶(abbokinase)、尿激酶(kinlytic)、尿激酶(urokinase)、尿激酶原(prourokinase)、茴香酰化纯化链激酶活化剂复合物(APSAC),纤维蛋白、纤溶酶和血管内皮生长因子(VEGF)组成的组。
另一方面,本文提供了治疗缺血性组织的方法。该方法包括将本文所述的药物组合物直接给予至受试者的缺血性组织,条件是该药物组合物是非经由静脉内给予。在一些实施方案中,药物组合物是经由肌肉给药。组织性缺血可以是溃疡,或在受试者的心脏或肢体中。
在一些实施方案中,缺血性组织是溃疡,或在受试者的心脏或肢体中。缺血性组织可以是肌肉。在一些实施方案中,受试者患有糖尿病。
在附图和以下描述中阐述了一个或多个实施例的细节。根据说明书和附图以及权利要求,实施例的其它特征、目的和优点将显而易见。

具体实施方式
于意外中发现含有具有一定黏度的透明质酸和肝素的药物组合物对治疗缺血性组织是有效的。
药物组合物
因此,本文描述了用于治疗缺血性组织的药物组合物。药物组合物包含核心成分和基质成分,核心成分包括肝素并且基质成分包括透明质酸或其衍生物。药物组合物的黏度大于10mPa·s。根据选择用于测量黏度的参数(例如,转子和旋转速度),组合物的黏度可以在10至10000mPa·s的范围内(例如,10-100、50-150、100-200、150-250、250-500、500-1000、1000-1500、1500-2000、2000-2500、2500-3000、3000-3500、3500-5000、5000-6000、6000-7000、7000-8000、8000-9000、或9000-10000)。
药物组合物的黏度可以落入在3至10mg/ml(例如,3、3.5、4、4.5、5、5.5、6、6.5、7、7.5、8、8.5、9、9.5或10mg/ml)的平均分子量为700至2000kDa(例如,700、800、900、1000、1500、1600、1700、1800、1900或2000)的透明质酸的黏度范围内。见下表2至表5。在一些实施方案中,组合物的黏度与5mg/ml的平均分子量为1560kDa的透明质酸的黏度相同。例如,下面描述的数据显示4mg/ml的2000kDa透明质酸、5mg/ml的1,560kDa透明质酸和6.5mg/ml的700kDa透明质酸具有大约相同的黏度。
药物组合物中透明质酸的分子量可以在4kDa至5000kDa的范围内(例如,4至20、20至100、100至500、500至1000、1000至2000、2000至2500、2500至5000、5、10、50、100、200、300、400、500、750、1000、1500、1800、2000、2500、3000、3500、4000、4500或5000kDa)。药物组合物中透明质酸的浓度可以是1至100mg/ml(例如,1、1.5、2、2.5、3、3.5、4、4.5、5、5.5、6、6.5、7、7.5、8、8.5、9、9.5、10、20、30、40、50、60、70、80、90或100mg/ml)。具体地,如果使用平均分子量为700至2000kDa的透明质酸,药物组合物中透明质酸的浓度可以是3至10mg/ml(例如,3、3.5、4、4.5、5、5.5、6、6.5、7、7.5、8、8.5、9、9.5、或10mg/ml)。熟练的从业者将能够选择分子量和浓度的适当组合以获得具有所需黏度的组合物。熟练的从业者将还能够使用本领域已知的方法和市售仪器测定组合物的黏度。
药物组合物中的肝素可以是肝素或其衍生物。肝素可选自由普通肝素(UFH)、分级肝素(低分子量肝素)、类肝素、依诺肝素、达肝素和亭扎肝素组成的组。在一些实施方案中,药物组合物含有浓度为每100μL的0.00001U至20U(例如,0.0001至10、0.0001、0.0005、0.001、0.005、0.01、0.05、0.5、1、1.5、2、2.5、3、3.5、4、4.5、5、5.5、6、6.5、7、7.5、8、8.5、9、9.5、10、11、12、13、14、15、16、17、18、19或20U)的肝素。一单位(U)的肝素(“豪威尔氏单位”)是大约相当于0.002mg纯肝素的量,这是在0℃下保持1ml猫血流动24小时所需的量。
核心成分还可包括溶栓药物或血管生成物,该溶栓药物或血管生成物是选自由VEGF、噻氯匹定、华法林、组织纤溶酶原激活物(t-PA)、阿尼普酶(eminase,anistreplase)、瑞替普酶(retavase,reteplase)、链激酶(streptase,streptokinase,kabikinase)、活化酶(activase)、替奈普酶(tenecteplase,TNKase)、阿伯激酶(abbokinase)、尿激酶(kinlytic,rokinase)、尿激酶(urokinase)、尿激酶原(prourokinase)、茴香酰化纤溶酶原链激酶活化复合物(APSAC)、纤维蛋白和纤溶酶组成的组。药物组合物可包括一种或多种溶栓药物或血管生成物。药物组合物可以含有类似于或低于推荐临床剂量的溶栓药物和血管生成物。
术语“透明质酸”是指天然存在阴离子型的非硫酸化糖胺聚糖,包括N-乙酰葡糖胺和D-葡糖醛酸的重复性二糖单元及其衍生物。天然存在的透明质酸(hyaluronan)(也称为透明质酸(hyaluronic acid)或透明质酸盐(hyaluronate))可以通过常规方法从其天然来源例如链球菌(Streptococci)孢蒴、公鸡鸡冠、软骨、滑膜关节液、脐带、皮肤组织和眼玻璃体中分离。参照文献例如,Guillermo Lago等人,Carbohydrate Polymers 62(4):321-326,2005;和Ichika Amagai等人,Fisheries Science 75(3):805-810,2009。或者,它可以从商业供货商处(例如Genzyme Corporation、Lifecore Biomedical、LLC和Hyaluron ContractManufacturing)购买。天然存在的透明质酸的衍生物包括但不限于透明质酸酯、己二酸二酰肼改性的透明质酸、透明质酸酰胺产物、交联的透明质酸、琥珀酸半酯或其重金属盐透明质酸、透明质酸的部分或全部酯类、硫酸化透明质酸、N-硫酸化透明质酸和胺或二胺改性的透明质酸。它们可以通过化学改性透明质酸的一个或多个官能团(例如,羧酸基团、羟基、还原性端基、N-乙酰基)来获得。羧基可以通过酯化反应或由碳二亚胺和双酰肼介导的反应进行改性。羟基改性包括但不限于硫酸化、酯化、异脲偶联、溴化氰活化和高碘酸盐氧化。还原性端基可以通过还原胺化来改性。它还可以与磷脂、染料(例如荧光团或发色团)或适于制备亲和基质的试剂连接。天然存在的透明质酸的衍生物也可以通过使用交联剂(例如,双环氧化物、二乙烯砜、双碳二亚胺、小的同双功能连接分子、甲醛、环己基异氰化物和赖氨酸乙酯、金属阳离子、酰肼或其混合物)交联,或者通过内部酯化、光交联或表面等离子体处理来获得。为了制备透明质酸溶液,可以将透明质酸溶解在磷酸盐缓冲溶液(例如,pH 7±1,≤0.05M)和/或NaCl(例如,≤0.9%)中。
基质成分可含有一种或多种其它基质分子,只要组合物的黏度保持在所需范围内。基质分子可包括明胶、胶原蛋白、透明质酸、纤连蛋白、弹性蛋白、细胞粘合素、层粘连蛋白、玻连蛋白、多肽、硫酸乙酰肝素、软骨素、硫酸软骨素、角质素、硫酸角质素、硫酸皮肤素、卡拉胶、肝素、甲壳素、几丁聚糖、海藻酸盐、琼脂糖、琼脂、纤维素、甲基纤维素、羧甲基纤维素、肝糖及其衍生物。此外,基质成分可包括纤维蛋白、纤维蛋白原、凝血酶、聚谷氨酸、合成聚合物(例如,丙烯酸酯、聚乳酸、聚乙醇酸或聚(乳酸-共-乙醇酸))或交联剂(例如,京尼平、戊二醛、甲醛或环氧化物)。
治疗方法
可以将有效量的药物组合物给予至患者以治疗缺血性组织。它可以被直接给予(例如,注射或敷用)到缺血性组织(例如肌肉)或附近。稠度呈凝胶状或黏稠状的组合物是非经静脉内给予的。
可依据需要将组合物给予至受试者,例如每天1至5次、每周1至5次、每月1至5次,持续合适的治疗期,例如1至4周、1至12个月、或1至3年。优选的是在缺血或缺血性损伤发生后(例如,在0至48小时或1-7天内)尽快给药。
给予的药物组合物的量应足以提供治疗性化合物(例如肝素、溶栓药物或血管生成物)的有效剂量。肝素的有效剂量可以是每克体重的0.00001U至20U/100μl(例如,0.0001至10、0.0001、0.0005、0.001、0.005、0.01、0.05、0.5、1、1.5、2、2.5、3、3.5、4、4.5、5、5.5、6、6.5、7、7.5、8、8.5、9、9.5、10、11、12、13、14、15、16、17、18、19或20U)。溶栓药物或血管生成物的有效剂量可以是例如0.00001至10μg/克受试者体重(例如,0.00001至0.001、0.001至0.005、0.005至0.01、0.05至0.1、0.1至0.5、0.5至1、0.00001、0.0001、0.0005、0.001、0.005、0.01、0.02、0.03、0.04、0.05、0.06、0.07、0.08、0.09、0.1、0.2、0.3、0.4、0.5、0.6、0.7、0.8、0.9、1、2、3、4、5、6、7、8、9或10μg),取决于溶栓药物的功效。
“治疗”是指向患有疾病或有患病风险的受试者给予药物组合物,目的是治愈、缓解、减轻、补救、推迟疾病的发作,预防或改善疾病、疾病症状、继发于疾病的疾病状态、或对疾病的倾向。“有效量”是指能够在治疗的受试者中产生医学上期望的结果的组合物的量。治疗方法可以单独进行或与其它药物或疗法联合进行。待治疗的受试者可以是人或实验动物或家畜动物。
以下具体实施例应被解释为仅是说明性的,并且不以任何方式限制本公开的其余部分。无需进一步详细说明,相信本领域技术人员基于本文的描述可以最充分地利用本公开信息。本文引用的所有出版物均通过引用整体并入本文。
实施例1:糖尿病下肢缺血小鼠模型
下述进行的所有实验均由成功大学(NCKU)的实验动物管理委员会(IACUC)预先批准。C57BL/6品系雄鼠来源于NCKU的实验动物中心或乐斯科生物科技公司(BioLASCOTaiwan Co.,Ltd.)。小鼠饲养于成功大学生物科技研究所的动物房至少一周,以便它们适应环境后才进行实验。
建立这种实验动物模型用于研究下肢缺血的治疗方法。将6-8月龄或更大的小鼠以50mg/kg体重的链脲佐菌素(STZ)溶液处理以诱导I型糖尿病,以便展示高龄与受损糖尿病组织不易痊愈的特性。由于小鼠的低血糖水平会干扰结果,因此将血糖水平在400mg/dl至550mg/dl范围内的小鼠用于实验,并可能向小鼠施用微量胰岛素以避免威胁生命的高血糖水平。为了避免自我再生新血管形成的可能性,阻断小鼠下肢的股动脉及其外周血管。该模型不具有在不经治疗下血管再生的可能性,能够更加确切评价测试药物的血管新生能力。
为诱导下肢缺血,将剃毛处理的糖尿病小鼠置于气体麻醉箱中,通气使每分钟每公升气体含有1-3%的异氟烷。待小鼠昏迷后,将其移至手术台上并维持气体麻醉。在利用透气胶带将小鼠四肢固定后,以37℃加热垫维持小鼠体温恒定。对小鼠的下腹部与肢体进行消毒后,将肢体的皮肤从左脚踝处的小开口切割至大腿。以手术缝线将小鼠小腿肌肉背侧的两个侧向血管的上下游都打结,并移除血管以阻断背侧血管血流。接着阻断腹侧股动脉分支与主要血管。利用手术缝线将踝关节附近的动脉末端及其周围血管打结,以确保股动脉和外周血流完全被阻塞。
截断血管后,将待测试的用于测试缺血疗效的待测药物组合物直接施用于组织或注射于腓肠肌的8个位点处,并对开创伤口处进行缝合。同时向小鼠皮下注射1mg/kg体重的酮咯酸止痛剂及盐酸利多卡因局部麻醉剂并且还给予1ml生理盐水,减缓伤痛与补充水分。必要时则给予葡萄糖液以维持体力。

在术后第0、1、2、3、4、5、6、7、14、21和28天,使用表1中所示的评分系统和激光多普勒血流仪(laser Doppler flowmetry)分别评估小鼠下肢的表现外观和血流量。感兴趣区域(ROI)是通过激光多普勒血流仪从大腿到趾甲末端的区域获得。然后ROI被计算为术后左下肢血流信号与未处理的右下肢血流信号的比值,并且根据术前采集的血流信号将该比值以百分比归一化。
实施例2:非糖尿病组织的再生能力
为了比较非糖尿病小鼠和糖尿病小鼠的再生能力,对两组小鼠均进行了如上所述的下肢缺血手术。记录了这两组随时间推移的肢体外观评分及其血流信息。参见图1(A)和(B)。当两组小鼠均接受严重的缺血手术后,在小鼠中诱导糖尿病使得肢体恢复比非糖尿病小鼠差。结果还表明,非糖尿病组的下肢血流的恢复优于糖尿病组。为了筛选并确保药物组合物的功效,在以下实验中采用了糖尿病小鼠,但是结果不应用于限制权利要求的应用领域。
实施例3:透明质酸黏度测试
制备含有不同浓度的分子量不同的透明质酸的组合物。
根据手册采用DV2TRV黏度计(Brookfield,USA)检测药物组合物的黏度。依照黏度选取适当的转子(CPE40或CPE52)。在测量前,对机器进行校正,并且设定为在25℃以20rpm运转1分钟。以黏度吸管将500μl的每个样品转移至样品盘上,按下运转键开始检测样品黏度。
测定5mg/ml的分子量分别为1560kDa、700kDa和2,000kDa的透明质酸的黏度,结果如表2所示。将5mg/ml的平均分子量为1,560kDa的透明质酸的黏度作为参考,并且测量了各种浓度的平均分子量为700kDa和2,000kDa的透明质酸的黏度,如表3和表4所示。然后计算黏度接近于参考黏度的平均分子量为700kDa和2000kDa透明质酸的浓度,并分别调节至6.5和4mg/ml。
如下表5所示,值得注意的是,当测量参数改变时,具有相同浓度和分子量范围的透明质酸的黏度发生变化。

实施例4:透明质酸或肝素单独治疗缺血性组织的疗效
通过肌肉注射将透明质酸施加于糖尿病小鼠的缺血性下肢。如图2所示,单独的透明质酸在第2周时仅具有较小效果,但是在糖尿病小鼠为下肢严重缺血时效果不明显。在第3天或第5天进行第二次注射时,多剂量的治疗效果不比在缺血部位仅接受磷酸盐缓冲液的对照组强。参见图2。
将肝素用于静脉内注射以预防血栓形成。通过将肝素注射到小鼠下肢的缺血部位进行单独测试。总是在小鼠下肢周围观察到组织出血,或者在小鼠缺血性下肢中未观察到恢复效果(数据未显示)。
实施例5:含有透明质酸和肝素的药物组合物
制备各自包含肝素核心成分和透明质酸基质成分的药物组合物,并测试其对缺血的治疗性效果。肝素成分是以下之一:普通肝素(UFH)、分级肝素(低分子量肝素)、类肝素、依诺肝素、达肝素和亭扎肝素。透明质酸基质成分的平均分子量为4至5000kDa。如实施例3中所述,不同分子量的透明质酸在不同浓度下具有不同黏度。此外,肝素(尤其是UFH和类肝素)是胶状物质,并且还具有一定黏度。
根据体重将可在人体中使用的肝素药物的最大化和最小化有效剂量转换为小鼠剂量。如上实施例1所述,将药物组合物注射到下肢缺血术后的糖尿病小鼠的多个位点。将术后仅用缓冲液治疗的糖尿病小鼠用作对照。如上实施例1所述,在术后第0、1、2、3、4、5、6、7、14、21和28天记录外观得分,并在术后第7、14、21和28天测量血流信号。
在一种情况下,3.5mg/ml的平均分子量2500kDa的透明质酸和0.1U低分子量肝素可以预防缺血性肢体断裂。在另一种情况下,6.5mg/ml的平均分子量700kDa的透明质酸和0.05U类肝素可以预防缺血性肢体断裂。
实施例6:含有透明质酸和肝素的药物组合物的功效
制备在磷酸缓冲盐溶液中的各自包含低分子量肝素核心成分和5mg/ml的平均分子量为1,560kDa透明质酸的药物组合物,并研究它们对下肢缺血的治疗性效果。如实施例1所述,将药物组合物给予至下肢缺血术后的糖尿病小鼠的缺血肌肉中的八个位点。将在术后仅用磷酸盐缓冲盐溶液处理的糖尿病小鼠用作对照。如上实施例1中所述,在术后第0、1、2、3、4、5、6、7、14、21和28天记录外观得分,并在术后第7、14、21和28天测量血流信号。外观得分如图3所示。
将处理的小鼠各自注射有100μl药物组合物,该药物组合物包含6U、8U、10U、14U或16U的低分子量肝素和500μg的平均分子量为1560kDa的透明质酸(即,分别为DIB6、DIB8、DIB10、DIB14和DIB 16)。如图3所示,DIB6组(6U的低分子量肝素和透明质酸)对糖尿病小鼠的缺血性下肢坏疽的恢复或预防显示显著的治疗性效果。如果转换成每体重(BW)使用的剂量,则分别将0.20±0.01U/g BW、0.26±0.01U/g BW和0.51±0.01U/g BW的肝素给予至DIB6组、DIB8组和DIB 16组。
实施例7:多剂量的含透明质酸和肝素的药物组合物的功效
将糖尿病小鼠在术后分别注射100μl的药物组合物,该药物组合物包含2U、4U、6U、8U、10U、12U、14U或16U的低分子量肝素和500μg的平均分子量为1,560kDa的透明质酸(即,分别为DIB2、DIB4、DIB6、DIB8、DIB10、DIB12、DIB14和DIB16)。换言之,分别将将药物组合物中0.05~0.08U/g BW、0.12~0.15U/g BW、0.20±0.01U/g BW、0.26±0.01U/g BW和0.51±0.01U/g BW的肝素给予至DIB2组、DIB4组、DIB6组、DIB8组和DIB16组。
除了在术后第3天在受损肌肉的多个位点进行第二次注射外,如实施例6所述的进行研究。下肢的外观评分如图4所示。结果表明6U或更低的肝素与透明质酸组合对缺血产生治疗性效果,并且多剂量能够显著增强治疗效果。
实施例8:具有治疗性效果的药物组合物中肝素的最佳剂量
如表6所示制备各自包含低分子量肝素和透明质酸的多种药物组合物,其中肝素剂量以U/g体重(BW)表示。将组合物给予至缺血术后糖尿病小鼠的缺血肌肉的多个位点,并且如实施例6所述进行观察。

外观得分如上表6所示。结果表明,6U/g BW或更低的肝素与透明质酸具有治疗性效果,并且有效剂量可以在0.1mU/g BW至6U/g BW的范围内,尽管在不同小鼠中可能会看到一些变化。
实施例9:含有不同类型肝素、一种或多种溶栓药物/血管生成物和透明质酸的药物组合物
如表7所示制备包含不同类型肝素、溶栓药物/血管生成物和透明质酸组合的多种药物组合物。将组合物给予至缺血术后糖尿病小鼠的缺血肌肉的多个位点,并且如实施例6所述进行观察。
外观得分如上表7所示。结果表明,药物组合物中的核心成分可以包括多种类型的肝素和多种溶栓药物/血管生成物,并且肝素的有效剂量可以在0.00001至20U/g BW的范围内。同时,药物组合物可包含平均分子量(mean)或平均分子量(average molecularweight)为4至5000kDa(例如,50kDa至2500kDa)范围内的透明质酸。

·肝素核心成分的量为0.00001至20U/g BW肝素、低分子量肝素或类肝素。
尽管个体与个体之间可能会出现一些变化,但是透明质酸和核心成分的组合效果部分是通过黏性透明质酸和核心成分的相互作用来控制有效成分的释放,从而维持治疗性效果而不会造成组织出血。
实施例10:用药物组合物治疗的小鼠的功能性评估
由于在实施例6至9中描述的实验中观察到积极性结果,研究了缺血术后第28天下肢的生理功能以便了解药物组合物的治疗性效果。成像分析显示在DIB治疗组中明显出现新血管形成(数据未显示)。从组织学的不同染色观察到,药物组合物的局部施用不仅增加了新血管形成的功效,而且使受损组织免于坏疽。
图5(A)显示了在术后第35天的DIB治疗组与正常小鼠中的新血管形成之间的定量性比较。结果表明,缺血组织的再生需要一定时间,并且在组织再生的同时,观察到血管在密度和血管尺寸方面的增殖。非常可能的是,在更多时间下新生血管的密度和尺寸会接近于正常值。
为了进一步了解神经功能的再生,将每只小鼠放置在平台上,并且在术后第35天进行步态行走分析。结果示于图5(B)。观察到DIB治疗组的两肢体之间的基础宽度与正常对照组没有差异。尽管DIB组的步幅与对照组相比显著增加,但仍不如正常小鼠。参见图5(B)。由于受损肢体的行走功能需要时间恢复,结果显示,DIB组小鼠的下肢在术后开始萎缩因而其步伐还没有完全恢复正常,但是仍明显优于对照组。
实施例11:药物组合物对梗塞后缺血性心脏组织再生的治疗性效果
根据以上实验经验,制备包含10U/ml肝素和5mg/ml的平均分子量为1500kDa透明质酸的药物组合物用于心脏梗塞实验。通过注射四氧嘧啶诱导12至18月龄的兰屿猪(Lanyupig)的糖尿病症状。糖尿病猪的再生能力明显低于正常对照组。对猪心脏的冠状动脉进行心肌手术,并将左前降支(LAD)的上部对角线分支D2中的血管结扎。在缺血术后,通过肌内注射将如上制备的胶状药物组合物给予至缺血心肌的30至40个位点处。分别采用超声心动图和血管造影术以观察心脏功能和血管反应。每周进行一次超声检查。
图6示出了在结扎下边缘处顶部的横截面。将临近结扎的冠状动脉的心肌放大用于比较纤维化面积(比例尺=1cm)。观察到,在手术后第28天,用药物组合物治疗得到的显著结果为,尽管心脏中仍存在结扎结,但是在心肌中未观察到纤维化组织。另一方面,在假手术对照组和另一实验组(其中通过用磷酸缓冲盐溶液稀释将药物组合物的黏度特定降低到优选药物组合物的三分之一)中,纤维化组织都是明显存在的。此外,处死小鼠后收集心脏用于随后的组织学染色。这些结果还表明一定范围的黏度是最佳的。
其它实施例
本说明书中公开的所有特征可以任何组合进行组合。本说明书中公开的每个特征可以由用于相同,等同或类似目的的替代特征代替。因此,除非另有明确说明,否则所公开的每个特征仅是一系列等效或类似特征的示例。
根据以上描述,本领域技术人员可以容易地确定所描述的实施例的基本特征,并且在不脱离其精神和范围的情况下,可以对实施例进行各种改变和修改以使其适应各种用途和条件。因此,其它实施例也在权利要求内。
Claims (34)
1.一种用于治疗缺血性组织的药物组合物,包含核心成分和基质成分,
其中所述核心成分包括肝素或其衍生物并且所述基质成分包括透明质酸或其衍生物,
所述药物组合物具有大于10mPa·s的黏度。
2.根据权利要求1所述的药物组合物,其中所述黏度为10至10000mPa·s。
3.根据权利要求2所述的药物组合物,其中所述透明质酸的平均分子量为100kDa至5000kDa。
4.根据权利要求3所述的药物组合物,其中所述透明质酸的平均分子量为700kDa至2000kDa。
5.根据权利要求3所述的药物组合物,其中所述药物组合物含有1mg/ml至100mg/ml的所述透明质酸。
6.根据权利要求1所述的药物组合物,其中所述黏度在3至10mg/ml的平均分子量为700至2000kDa的所述透明质酸的黏度范围内。
7.根据权利要求1所述的药物组合物,其中所述黏度与5mg/ml的平均分子量为1560kDa的所述透明质酸的黏度相同。
8.根据权利要求6所述的药物组合物,其中所述透明质酸的平均分子量为700至2000kDa且所述透明质酸的浓度为3至10mg/ml。
9.根据权利要求7所述的药物组合物,其中所述透明质酸的平均分子量为1560kDa且所述透明质酸的浓度为5mg/ml。
10.根据权利要求1所述的药物组合物,其中所述基质成分进一步包括胶原蛋白、细胞外基质因子、蛋白质或多糖。
11.根据权利要求1所述的药物组合物,其中所述肝素或其衍生物选自由普通肝素(UFH)、分级肝素(低分子量肝素)、类肝素、依诺肝素、达肝素或亭扎肝素所组成的组。
12.根据权利要求1所述的药物组合物,其中所述药物组合物含有0.00001U至20U/100μl的所述肝素或其衍生物。
13.根据权利要求1至12中任一项所述的方法,其中所述核心成分还包含溶栓药物或血管生成物。
14.根据权利要求13所述的药物组合物,其中所述溶栓药物或所述血管生成物选自由噻氯匹定、华法林、组织纤溶酶原激活物、阿尼普酶、瑞替普酶、链激酶、组织纤溶酶原激活物、替奈普酶、阿伯激酶、尿激酶(kinlytic)、尿激酶(urokinase)、尿激酶原、茴香酰化纯化链激酶活化复合物(APSAC)、纤维蛋白、纤溶酶和血管内皮生长因子(VEGF)所组成的组。
15.根据权利要求1至14中任一项所述的药物组合物,其中所述药物组合物被直接给予至缺血性组织,但非经由静脉内给予。
16.根据权利要求15所述的药物组合物,其中所述缺血性组织是溃疡,或在受试者的心脏或肢体中。
17.一种治疗受试者中缺血性组织的方法,包括:将药物组合物直接给予至缺血性组织,条件是所述药物组合物非经由静脉内给予;
其中所述药物组合物含有核心成分和基质成分,所述核心成分包括肝素或其衍生物且所述基质成分包括透明质酸或其衍生物,和所述药物组合物具有大于10mPa·s的黏度。
18.根据权利要求17所述的方法,其中所述缺血性组织是溃疡,或在所述受试者的心脏或肢体中。
19.根据权利要求17所述的方法,其中所述缺血性组织是肌肉。
20.根据权利要求17所述的方法,其中所述受试者患有糖尿病。
21.根据权利要求17所述的方法,其中所述黏度为10至10000mPa·s。
22.根据权利要求21所述的方法,其中所述透明质酸的平均分子量为100kDa至5000kDa。
23.根据权利要求22所述的方法,其中所述透明质酸的平均分子量为700kDa至2000kDa。
24.根据权利要求21所述的方法,其中所述药物组合物含有1mg/ml至100mg/ml的所述透明质酸。
25.根据权利要求24所述的方法,其中所述黏度在3至10mg/ml的平均分子量为700至2000kDa的所述透明质酸的黏度范围内。
26.根据权利要求24所述的方法,其中所述黏度与5mg/ml的平均分子量为1560kDa的所述透明质酸的黏度相同。
27.根据权利要求25所述的方法,其中所述透明质酸的平均分子量为700至2000kDa且所述透明质酸的浓度为3至10mg/ml。
28.根据权利要求26所述的方法,其中所述透明质酸的平均分子量是1560kDa且所述透明质酸的浓度是5mg/ml。
29.根据权利要求17所述的方法,其中所述基质成分还包含胶原蛋白、细胞外基质因子、蛋白质或多糖。
30.根据权利要求17所述的方法,其中所述肝素或其衍生物选自由普通肝素(UFH)、分级肝素(低分子量肝素)、类肝素、依诺肝素、达肝素或亭扎肝素所组成的组。
31.根据权利要求17所述的方法,其中所述药物组合物含有0.00001U至20U/100μl的所述肝素或其衍生物。
32.根据权利要求17所述的方法,其中所述肝素以0.00001U至20U/克体重的剂量给药。
33.根据权利要求17至32中任一项所述的方法,其中所述核心成分还包含溶栓药物或血管生成物。
34.根据权利要求33所述的药物组合物,其中所述溶栓药物或所述血管生成物选自由噻氯匹定、华法林、组织纤溶酶原激活物、阿尼普酶、瑞替普酶、链激酶、组织纤溶酶原激活物、替奈普酶、阿伯激酶、尿激酶(kinlytic)、尿激酶(urokinase)、尿激酶原、茴香酰化纯化链激酶活化复合物(APSAC)、纤维蛋白、纤溶酶和血管内皮生长因子(VEGF)所组成的组。
