Abstract

Description
技术领域
本发明涉及一种用于再生医疗的细胞制剂。更具体而言,涉及一种含有对大疱性表皮松解症的治疗有效的多能干细胞的细胞制剂和含有从对大疱性表皮松解症等皮肤疾病的治疗有效的多能干细胞分化诱导得到的皮肤细胞的细胞制剂。
背景技术
大疱性表皮松解症(epidermolysis bullosa,EB)是表皮和真皮间的粘附功能因皮肤基底膜区域中的粘附结构控制蛋白质的基因异常而被破坏,表皮在日常生活中的轻微外力下在基底膜水平发生剥离而形成全身烫伤样水疱、溃疡的遗传性水疱性皮肤顽疾(表1)。大疱性表皮松解症根据水疱的形成部位可大致分为单纯型、交界型、营养不良型这三种类型,在肢端、大关节部等容易受到外力的部位,会由于轻微外力而产生水疱、糜烂。在单纯型和显性营养不良型中,水疱、糜烂可以较快地治愈,单纯型在治愈后不会留下疤痕和皮肤萎缩,但显性营养不良型会留下疤痕。在交界型和隐性营养不良型中,水疱和糜烂一般很难治愈,治愈后,在交界型中会留下皮肤萎缩,在隐性营养不良型中会留下疤痕。难以通过出生时的临床表现进行鉴别,需要与电子显微镜、免疫染色、基因诊断、随着生长而变化的临床表现一起进行综合诊断。早期诊断在很好地管理皮肤和全身方面成为有用的信息。由于是罕见的难治性疾病,在诊断治疗中需要专业设施、专科医生进行的检查和建议。

大疱性表皮松解症的治疗方法在现阶段没有根治疗法,只有对症疗法。其对症疗法也因病型而不同,因此,首先进行准确的病型诊断是必不可少的。此外,由于本症会根据病型而产生各种并发症,有时会出现病情恶化,显著限制患者的日常生活,因此还需要针对各种并发症的处置。进而,由于本症是难治的遗传性疾病,因此还需要考虑预防家族中患者的复发。
<局部疗法>
对水疱、糜烂、溃疡等进行流水清洗后,在纱布上涂抹凡士林等,来防止纱布与糜烂发生粘连。此时,预先对水疱内容物进行穿刺排液(不除去水疱皮)。在伴随有指趾间粘连的病例中,在指趾间夹着凡士林纱布等,来预防指趾间粘连。长期使用含抗生素的软膏是导致耐药细菌出现的原因,因此除了特别的情况以外,不需要主动使用含抗生素的软膏。在确认糜烂、溃疡恶化的情况下,由于可能出现真菌、细菌感染的并发,特别是在隐性营养不良型大疱性表皮松解症中可能出现皮肤癌,因此要主动实施皮肤活检、真菌检查、细菌培养检查等。基本上按1天1次实施软膏疗法。
<全身疗法>
营养补给:特别是在隐性营养不良型大疱性表皮松解症中,由于口腔粘膜、食道的病变,无法充分摄取营养,发生慢性营养不良、贫血的情况非常多。因此,Ensure Liquid(商品名)等营养剂的经口摄取是有用的。在难以经口摄取的情况下,有时也通过经鼻导管、点滴来补给营养。在瘙痒剧烈的情况下,有时抗组胺剂也会起效。
<对并发症的治疗>
在隐性营养不良型和交界型中,指(趾)间粘连、皮肤恶性肿瘤、食道狭窄、幽门狭窄、肛门部的糜烂和狭窄、营养不良、结膜糜烂、贫血等成为问题的情况较多。此外,作为严重的并发症之一,存在继发性全身性淀粉样变性。
正因为大疱性表皮松解症的并发症大多使QOL显著降低,所以治疗的必要性较高。与成形术、扩张手术、营养管理等临床各领域的专科医生协作来进行诊断和治疗是很重要的。(非专利文献1)
但是,在尚未确立使基因异常正常化的安全且可靠的方法0论的当前,还没有以大疱性表皮松解症为代表的遗传性疾病的根治疗法。
另一方面,随着近年来再生医疗的研究的进展,正在探索基于骨髓移植、骨髓干细胞移植等的大疱性表皮松解症的治疗。
例如,公开了如下见解。(非专利文献2)
(1)基于骨髓来源细胞的皮肤再生机理:作为反复发生长年大范围的表皮剥离而丧失了大量表皮干细胞的大疱性表皮松解症患者皮肤的表皮再生机理,将骨髓内干细胞经由末梢循环动员到损伤部皮肤,并明确了其有助于水疱部皮肤的再生。(非专利文献3、非专利文献4)
(2)针对大疱性表皮松解症的骨髓移植疗法:美国明尼苏达大学的研究小组在世界上首次实施了针对隐性营养不良型大疱性表皮松解症的骨髓移植,并报告了皮肤症状的改善效果。但是,7例中的2例在经过过程中死亡,更安全的骨髓移植治疗方案的开发是必不可少的。(非专利文献5)
(3)针对大疱性表皮松解症的骨髓间充质干细胞移植疗法:南美洲智利的研究小组将来源于健康者骨髓的培养间充质干细胞皮下移植给重症隐性营养不良型大疱性表皮松解症的2个病例,并明确了其有效性(非专利文献6)。此外,英国、埃及的小组向重症隐性营养不良型大疱性表皮松解症的患者点滴施用来源于健康者骨髓的培养间充质干细胞,并报告了其有效性(非专利文献7、非专利文献8)。但是,也一并示出所移植的间充质干细胞在数月内逐渐减少的可能性。
(4)针对大疱性表皮松解症的利用骨髓间充质干细胞血中动员因子的再生诱导医疗的可能性:发现从剥离表皮释放出的HMGB1经由末梢血液使骨髓间充质干细胞聚集在剥离表皮部的皮肤上,强烈地诱导损伤皮肤的再生。(非专利文献4)
如上所述,以大疱性表皮松解症的根治疗法为目标,正在积极地推进基因治疗、再生治疗的基础和临床研究。但是,目前仍未发现确认了安全性和有效性的、可使大疱性表皮松解症完全治愈的治疗方法,期待实现进一步的根治治疗。
另一方面,根据出泽等人的研究可知,表达表面抗原SSEA-3(Stage-SpecificEmbryonic Antigen-3)的多能性干细胞(Multilineage-differentiating StressEnduring cells;Muse细胞)是间充质细胞级分的多能性的承担者,该细胞存在于间充质细胞级分中,可以不通过基因导入或利用细胞因子等的诱导操作而得到,在以组织再生为目的的疾病治疗方面具有潜在用途(例如,专利文献1;非专利文献9~11)。但是,尚没有公开了将Muse细胞用于大疱性表皮松解症的治疗而获得所期待的治疗效果的例子。

发明要解决的课题
本发明的目的在于提供一种用于治疗大疱性表皮松解症等皮肤疾病的细胞制剂。
用于解决课题的手段
本发明人们发现,通过实验性地对损伤了皮肤的小鼠从血管施用人Muse细胞,在创伤部位表达了人源胶原蛋白,损伤的修复会提前。同样地,发现在遗传性地缺失17型胶原蛋白(COL17)的大疱性表皮松解症模型小鼠中,在表皮上形成水疱后,通过从血管等施用人Muse细胞,能够将人源17型胶原蛋白提供给表皮,从而发现Muse细胞能够用于大疱性表皮松解症的治疗。进而,发现能够从Muse细胞获得对皮肤疾病的治疗有用的角质形成细胞和成纤维细胞等皮肤细胞,从而完成了本发明。
即,本发明如下所述。
[1]一种用于治疗大疱性表皮松解症的细胞制剂,其含有来源于活体的间充质组织或培养的间充质细胞的SSEA-3阳性多能干细胞。
[2]根据[1]所述的细胞制剂,其中,大疱性表皮松解症为单纯型大疱性表皮松解症。
[3]根据[1]所述的细胞制剂,其中,大疱性表皮松解症为交界型大疱性表皮松解症。

发明效果
在本发明中,通过从血管等向患有大疱性表皮松解症的患者施用Muse细胞、或者在对象的形成了水疱和糜烂的皮肤部位及其周围直接施用Muse细胞,能够重建和修复损伤皮肤,改善或恢复皮肤症状。因此,本发明的含有Muse细胞的细胞制剂可用于大疱性表皮松解症的治疗。
Muse细胞能够高效地迁移和植活到形成了水疱和糜烂的皮肤部位,在植活的部位自发地分化为表皮细胞,因此不需要在移植之前进行向待治疗细胞的分化诱导。此外,其为非致瘤性的,安全性也非常好。进而,由于Muse细胞不会受到免疫排斥,因此也可以用于从供体制造的异体制剂的治疗。因此,利用具有以上所示的优异性能的Muse细胞,能够提供治疗大疱性表皮松解症患者的易于实施的方法。
在本发明中,通过向患有大疱性表皮松解症等皮肤疾病的患者的皮肤疾病部位及其周围施用从Muse细胞分化诱导得到的角质形成细胞和成纤维细胞等皮肤细胞,能够重建和修复损伤皮肤,改善或恢复皮肤症状。因此,本发明的含有从Muse细胞分化诱导得到的皮肤细胞的细胞制剂可用于大疱性表皮松解症等皮肤疾病的治疗。

具体实施方式
<1>含有Muse细胞的细胞制剂
本发明涉及用于治疗大疱性表皮松解症的细胞制剂,所述细胞制剂含有SSEA-3阳性多能干细胞(Muse细胞)。另外,治疗包括症状的治愈、缓解、防止复发等。在下文对本发明进行详细说明。
1.适用疾病
本发明的含有SSEA-3阳性多能干细胞(Muse细胞)的细胞制剂用于治疗大疱性表皮松解症。
在本发明中,“大疱性表皮松解症”是指表皮和真皮间的粘附功能因皮肤基底膜区域中的粘附结构控制蛋白质的基因异常等原因而被破坏,表皮在日常生活中的轻微的外力下在基底膜水平发生剥离而形成水疱、溃疡和/或糜烂的遗传性水疱性皮肤顽疾。
在本发明中,大疱性表皮松解症根据水疱的形成部位可大致分为单纯型大疱性表皮松解症、交界型大疱性表皮松解症、营养不良型大疱性表皮松解症(例如显性营养不良型大疱性表皮松解症、隐性营养不良型大疱性表皮松解症等)这三种类型,例如,在肢端、大关节部等容易受到外力的部位,会由于轻微外力而产生水疱、糜烂。
2.细胞制剂
(1)多能干细胞(Muse细胞)
本发明的细胞制剂中所使用的多能干细胞,是由出泽等人在人体内发现其存在并命名为“Muse(Multilineage-differentiating Stress Enduring)细胞”的细胞。已知Muse细胞除了可以从骨髓液、脂肪组织(Ogura,F.,et al.,Stem Cells Dev.,Nov20,2013(Epub)(published on Jan 17(2014))、皮肤的真皮结缔组织等中得到以外,还广泛存在于组织、器官的结缔组织中。此外,该细胞是具有多能干细胞和间充质干细胞双方的性质的细胞,例如,可作为细胞表面标记物即“SSEA-3(Stage-specific embryonicantigen-3)”阳性细胞,优选为SSEA-3阳性且CD-105阳性的双重阳性细胞加以鉴定。因此,例如可以以单独表达SSEA-3、或表达SSEA-3和CD-105为指标,从活体组织分离Muse细胞或含有Muse细胞的细胞群。Muse细胞的分离法、鉴定法和特征等的详细情况已在国际公开第WO2011/007900号中公开。此外,利用Muse细胞对各种外部应激的抗性高这一点,可以通过在蛋白质分解酶处理、低氧条件、低磷酸条件、低血清浓度、低营养条件、暴露于热激、有害物质存在下、活性氧存在下、机械刺激下、压力处理下等各种外部应激条件下进行培养来选择性地浓缩Muse细胞。另外,在本说明书中,作为用于治疗大疱性表皮松解症的细胞制剂,有时将SSEA-3用作指标,将从活体的间充质组织或培养间充质组织制备的多能干细胞(Muse细胞)或含有Muse细胞的细胞群简称为“SSEA-3阳性细胞”。此外,在本说明书中,“非Muse细胞”有时是指活体的间充质组织或培养的间充质细胞中所含的细胞、即指“SSEA-3阳性细胞”以外的细胞。
Muse细胞或含有Muse细胞的细胞群可以以作为细胞表面标记物的SSEA-3或SSEA-3和CD-105为指标,从活体组织(例如间充质组织)制备。此处,“活体”是指哺乳动物的活体。在本发明中,活体不包括受精卵、发育阶段在囊胚期之前的胚胎,但包括囊胚期以后的发育阶段的胚胎,包括胎儿和囊胚。对哺乳动物没有限制,可以为人、猴等灵长类;小鼠、大鼠、兔子、豚鼠等啮齿类;猫、狗、羊、猪、牛、马、驴、山羊、雪貂等。在本发明的细胞制剂中所使用的Muse细胞是携带标记从活体组织直接被分离的,在这一点上,明确区别于胚胎干细胞(ES细胞)、人工多能干(iPS)细胞。此外,“间充质组织”是指存在于骨、滑膜、脂肪、血液、骨髓、骨骼肌、真皮、韧带、肌腱、牙髓、脐带、脐带血、羊膜等组织和各种器官中的组织。例如,Muse细胞能够从骨髓、皮肤、脂肪组织、血液、牙髓、脐带、脐带血、羊膜等组织中获得。例如,优选选取活体的间充质组织,从该组织制备Muse细胞并加以利用。此外,也可以使用上述制备方法,从成纤维细胞、骨髓间充质干细胞等的培养的间充质细胞来制备Muse细胞。
此外,本发明的细胞制剂中所使用的含有Muse细胞的细胞群也可以通过包括以下步骤的方法制备:通过对活体的间充质组织或培养的间充质细胞施加外部应激刺激、使对该外部应激具有抗性的细胞选择性地增殖,并回收提高了此类细胞的存在比率的细胞。
所述外部应激可以是以下任一个或多个的组合:蛋白酶处理、低氧浓度下的培养、低磷酸条件下的培养、低血清浓度下的培养、低营养条件下的培养、暴露于热激下的培养、低温下的培养、冷冻处理、有害物质存在下的培养、活性氧存在下的培养、机械刺激下的培养、振荡处理下的培养、压力处理下的培养或物理冲击。
对所述基于蛋白酶的处理时间而言,为了对细胞施加外部应激,优选进行总计0.5~36小时。此外,蛋白酶浓度只要是将与培养容器粘附的细胞剥下时、将细胞团块分散成单一细胞时或从组织回收单一细胞时使用的浓度即可。
所述蛋白酶优选为:丝氨酸蛋白酶、天门冬氨酸蛋白酶、半胱氨酸蛋白酶、金属蛋白酶、谷氨酸蛋白酶或N末端苏氨酸蛋白酶。所述蛋白酶进一步优选为胰蛋白酶、胶原酶或分散酶。
另外,在本发明的细胞制剂中所使用的Muse细胞相对于接受细胞移植的接受者而言,可以是自体的、也可以是异体的。
如上所述,Muse细胞或含有Muse细胞的细胞群,例如,可以以SSEA-3阳性或SSEA-3和CD105的双重阳性为指标从活体组织制备,但已知在成人皮肤中含有各种类型的干细胞和前体细胞。然而,Muse细胞与这些细胞并不相同。对于这种干细胞和前体细胞而言,可以举出皮肤来源前体细胞(SKP)、神经嵴干细胞(NCSC)、成黑素细胞(MB)、血管周围细胞(PC)、内皮前体细胞(EP)和脂肪来源干细胞(ADSC)的例子。可以以“不表达”这些细胞固有的标记物为指标来制备Muse细胞。更具体而言,Muse细胞可以以不表达选自由CD34(EP和ADSC的标记物)、CD117(c-kit)(MB的标记物)、CD146(PC和ADSC的标记物)、CD271(NGFR)(NCSC的标记物)、NG2(PC的标记物)、vWF因子(血管假性血友病因子)(EP的标记物)、Sox10(NCSC的标记物)、Snai1(SKP的标记物)、Slug(SKP的标记物)、Tyrp1(MB的标记物)和Dct(MB的标记物)组成的组中的、11个标记物中的至少1个、例如不表达2个、3个、4个、5个、6个、7个、8个、9个、10个或者11个标记物为指标进行分离。例如,虽然没有限定,但可以以不表达CD117和CD146为指标进行制备,进而,可以以不表达CD117、CD146、NG2、CD34、vWF和CD271为指标进行制备,进而,可以以不表达上述11个标记物为指标进行制备。
此外,在本发明的细胞制剂中所使用的、具有上述特征的Muse细胞还可以具有选自由以下性质所组成的组中的至少一种性质:
(i)端粒酶活性低或没有端粒酶活性;
(ii)具有分化为三胚层中的任一胚层的细胞的能力;
(iii)不显示肿瘤性增殖;以及
(iv)具有自我更新能力。
优选地,本发明的细胞制剂中所使用的Muse细胞具有上述所有的性质。
此处,关于上述(i),“端粒酶活性低或没有端粒酶活性”是指例如在使用TRAPEZE XLtelomerase detection kit(Millipore公司)检测端粒酶活性的情况下,活性较低或无法检测。端粒酶活性“低”是指例如具有与体细胞即人成纤维细胞同等程度的端粒酶活性,或者与Hela细胞相比具有1/5以下、优选1/10以下的端粒酶活性。
关于上述(ii),Muse细胞在体外和体内具有分化为三胚层(内胚层系、中胚层系和外胚层系)的能力,例如,通过体外诱导培养,可以分化为肝细胞(包括表达肝成纤维细胞或肝细胞标记的细胞)、神经细胞、骨骼肌细胞、平滑肌细胞、骨细胞和脂肪细胞等。此外,在体内移植到睾丸的情况下,有时也显示出分化为三胚层的能力。进而,具有通过静脉注射移植到活体而迁移和植活到受损伤的器官(心脏、皮肤、脊髓、肝和肌肉等)并分化为与组织相对应的细胞的能力。
关于上述(iii),Muse细胞具有这样的性质:以约1.3天的增殖速度增殖,在悬浮培养中从1个细胞开始增殖,形成胚状体样细胞团块,并在达到一定的大小时,到14天左右停止增殖,但是,当将这些胚状体样细胞团块转为贴壁培养时,细胞增殖再次开始,从细胞团块增殖的细胞开始以约1.3天的增殖速度逐渐扩散。进而,在移植到睾丸的情况下,具有至少半年时间不癌变的性质。
此外,关于上述(iv),Muse细胞具有自我更新(自己复制)能力。此处,“自我更新”是指:从1个Muse细胞悬浮培养所得到的胚状体样细胞团块中包含的细胞可以确认向三胚层性细胞的分化,同时,通过将胚状体样细胞团块的细胞再次采取1个细胞悬浮培养,而使其形成下代的胚状体样细胞团块,可确认到从该细胞团块开始再次发生三胚层性的分化以及在悬浮培养下的胚状体样细胞团块。自我更新可以为1次或重复多次循环。
(2)含有Muse细胞的细胞制剂的制备和使用
本发明的含有Muse细胞的细胞制剂没有限制,但能够通过将上述(1)中所得到的Muse细胞或含有Muse细胞的细胞群悬浮于生理盐水、适当的缓冲液(例如磷酸缓冲生理盐水)中获得。在该情况下,在从自体或异体组织分离的Muse细胞数较少的情况下,也可以在细胞移植前培养细胞,并增殖至得到规定的细胞数。另外,如已经报告的那样(国际公开第WO2011/007900号公报),由于Muse细胞不发生肿瘤化,因此即使包含从活体组织回收的未分化的细胞,癌变的可能性也较低,是安全的。此外,回收的Muse细胞的培养没有特别限定,可以在通常的增殖培养基(例如含有10%胎牛血清的α-最小必需培养基(ot-MEM)等)中进行。更详细而言,可以参照上述国际公开第WO2011/007900号公报,在Muse细胞的培养和增殖中,适当地选择培养基、添加物(例如抗生素、血清)等,制备含有规定浓度的Muse细胞的溶液。在对人对象施用本发明的含有Muse细胞的细胞制剂的情况下,从人的髂骨提取骨髓液,例如可以在将骨髓间充质干细胞作为来自骨髓液的贴壁细胞进行培养并使其增加至达到可得到有效治疗量的Muse细胞的细胞量之后,以SSEA-3的抗原标记物为指标分离Muse细胞,制备自体或异体的Muse细胞作为细胞制剂。或者,例如,可以在外部应激条件下培养由骨髓液得到的骨髓间充质干细胞并使Muse细胞增殖,直到达到有效治疗量为止,在浓缩后,制备细胞制剂形式的自体或异体的Muse细胞。
此外,在Muse细胞的细胞制剂的使用中,也可以为了保护该细胞而在细胞制剂中含有二甲基亚砜(DMSO)、血清白蛋白等,还可以为了防止细菌的混入和增殖而在细胞制剂中含有抗生素等。进而,在细胞制剂中也可以含有制药上容许的其他成分(例如,载体、赋形剂、崩解剂、缓冲剂、乳化剂、悬浮剂、安抚剂、稳定剂、保存剂、防腐剂和生理盐水等)。本领域技术人员可以以适当的浓度将这些因子和药剂添加至细胞制剂中。这样,Muse细胞也可以作为含有各种添加物的药物组合物来使用。
上述制备的细胞制剂中含有的Muse细胞数可以考虑对象的性别、年龄、体重、患部的状态和所使用的细胞的状态等来适当地调整,以在大疱性表皮松解症的治疗中得到所希望的效果。另外,作为对象的个体包括但并不限于人等哺乳动物。此外,本发明的含有Muse细胞的细胞制剂可以以适当间隔(例如,1天2次、1天1次、1周2次、1周1次、2周1次、1个月1次、2个月1次、3个月1次以及6个月1次)而多次施用,直到获得所期望的治疗效果。因此,治疗的有效量虽然根据对象的状态而不同,但可优选例如:一个个体每次1×103细胞~1×1010细胞、1年之间1~10次的施用量。作为一个个体的施用总量,没有限定,可以包括1×103细胞~1×1011细胞,优选为1×104细胞~1×1010细胞,进一步优选为1×105细胞~1×109细胞等。
本发明的细胞制剂中所使用的Muse细胞具有向大疱性表皮松解症的损伤皮肤迁移并植活的性质。因此,在细胞制剂的施用中,对细胞制剂的施用部位或施用方法没有限定,可以向患部局部施用,也可以向静脉等施用。
本发明的含有Muse细胞的细胞制剂能够对大疱性表皮松解症患者的损伤皮肤进行修复和再生。损伤皮肤的修复和再生,例如可以通过COL7、COL17等胶原蛋白蛋白质的表达恢复和/或表达上升来确认。即,本发明的含有Muse细胞的细胞制剂具有胶原蛋白蛋白质的表达恢复和/或表达上升的作用。
<2>从Muse细胞分化诱导得到的皮肤细胞和含有其的细胞制剂在本发明中,也可以将从Muse细胞分化诱导得到的角质形成细胞和/或成纤维细胞等皮肤细胞用作细胞制剂。
角质形成细胞是产生角蛋白的细胞,也被称为表皮细胞或角化细胞。从Muse细胞向角质形成细胞的分化诱导可以通过如下培养来进行:例如用含有角质形成细胞增殖因子(KGF)和上皮增殖因子(EGF)的培养基对Muse细胞进行培养,优选在用含有KGF和EGF的培养基对Muse细胞进行培养后、再用含有KGF、EGF、肝细胞增殖因子(HGF)和胰岛素样增殖因子2(IGF2)的培养基进行培养。此处,KGF的优选的浓度范围例如为5~20ng/ml,EGF的优选的浓度范围例如为20~40ng/ml,IGF2的优选的浓度范围例如为40~80ng/ml。优选的培养时间例如为7~28天。
成纤维细胞是指产生胶原蛋白、弹性蛋白等真皮成分的细胞,从Muse细胞向成纤维细胞的分化诱导可以通过如下培养来进行:例如用含有转化生长因子-β2(TGF-β2)和抗坏血酸(AA)的培养基对Muse细胞进行培养,优选在用含有TGF-β2和AA的培养基对Muse细胞进行培养后、再用含有AA的培养基进行培养。此处,TGF-β2的优选的浓度范围例如为30~60μg/ml,AA的优选的浓度范围例如为20~80mmol/l。优选的培养时间例如为7~28天。
含有从Muse细胞分化诱导得到的角质形成细胞和/或成纤维细胞等皮肤细胞的细胞制剂,不仅可用于大疱性表皮松解症的治疗,而且可用于可通过皮肤细胞的补充疗法进行治疗的所有皮肤疾病的治疗。
在含有从Muse细胞分化诱导得到的皮肤细胞的细胞制剂的使用中,也可以为了保护该细胞而在细胞制剂中含有二甲基亚砜(DMSO)、血清白蛋白等,还可以为了防止细菌的混入和增殖而在细胞制剂中含有抗生素等。进而,也可以在细胞制剂中含有制药上容许的其他成分(例如载体、赋形剂、崩解剂、缓冲剂、乳化剂、悬浮剂、安抚剂、稳定剂、保存剂、防腐剂和生理盐水等)。
细胞制剂的施用量可以考虑对象的性别、年龄、体重、患部的状态和使用的细胞的状态等来适当地调整,以在皮肤疾病的治疗中得到所希望的效果。另外,作为对象的个体包括但不限于人等哺乳动物。此外,细胞制剂可以以适当间隔(例如1天2次、1天1次、1周2次、1周1次、2周1次、1个月1次、2个月1次、3个月1次以及6个月1次)而多次施用,直到获得所期望的治疗效果。因此,治疗的有效量虽然根据对象的状态而不同,但可优选例如:一个个体每次1×103细胞~1×1010细胞、1年之间1~10次的施用量。作为一个个体的施用总量,没有限定,可以包括1×103细胞~1×1011细胞,优选为1×104细胞~1×1010细胞,进一步优选为1×105细胞~1×109细胞等。施用方法没有特别限制,优选为对皮肤疾病部位或其周围进行局部施用。皮肤细胞也可以薄片化而应用于患部。
通过以下的实施例,进一步对本发明进行具体的说明,但本发明并不受这些实施例的任何限定。
[实施例]
人Muse细胞的制备
按照与人Muse细胞的分离和鉴定相关的国际公开第WO2011/007900号中记载的方法,得到了Muse细胞。Muse细胞通过在应激条件下培养间充质干细胞而进行了扩增富集培养。此外,购入市售的MSC来用作MSC组。
实施例1:全层创伤模型小鼠的评价
在成体C57BL/6小鼠的背部实施全层创伤而用作模型。在实施了全层创伤起30分钟以内,用上述制备的Muse细胞(3×105/小鼠或3×104/小鼠)、MSC(3×105/小鼠)或HBSS 200μl进行尾静脉注射,检验治疗效果。
另外,上皮化率按照以下方式算出。
用尺子和数码相机拍摄创伤制作时和制作后第3、6、9、11天的背部皮肤,使用Image J软件(版本1.50i)计算出各自的皮肤溃疡面积(mm2)。以创伤制作时的面积为基准值,计算出面积缩小率的百分比(%)。

结果如图1和图2所示。在任一组中,创伤均随着时间的经过而治愈,但在第3天的时间点,在Muse细胞(3×105/小鼠)施用组中,与MSC施用组、HBSS施用组相比发现明显更快的愈合速度和更高的上皮化率,显示出Muse细胞的施用对大疱性表皮松解症的治疗有效。
此外,在第14天分离创伤部位的皮肤组织,制作切片,进行核染色和人COL7染色,结果如图3所示,确认到在Muse细胞施用组中,在表皮和真皮中存在人COL7,并且能够确认所施用的Muse细胞迁移到皮肤上并产生了表皮与真皮的粘附所需的分子。
实施例2.COL17敲除大疱性表皮松解症模型小鼠的评价
在COL17基因敲除小鼠(参考文献Nat Med.2007Mar;13(3):378-83.)(3~4周龄)中,摩擦表皮形成水疱,从水疱形成起30分钟以内通过尾静脉注射Muse细胞(3×105/小鼠)。一个月后观察皮肤的状态的结果是,如图4所示,在施用了HBSS的对照小鼠中,被毛状态较差,广泛地确认到伤口、粘膜糜烂的形成,但在施用了Muse细胞的小鼠中,被毛状态、伤口的形成都较轻。此外,分离施用一个月后的皮肤组织,从其中提取RNA,通过RT-PCR,检验人源COL7基因和COL17基因的表达。结果如5所示。其结果,在施用了Muse细胞的小鼠中确认到人COL7和人COL17的存在,可以确认所施用的Muse细胞提供了粘附因子。人COL7的表达也可以在蛋白质水平确认(图6)。
实施例3.Muse细胞向角质形成细胞的分化诱导
按照以下的步骤培养Muse细胞,由此进行了向角质形成细胞的分化诱导。
第0天Muse细胞的接种
第1天以DMEM low葡萄糖培养基+10%FBS+KGF(10ng/ml)+EGF(20~30ng/ml)培养3天
第4天以DMEM low葡萄糖培养基+10%FBS+KGF(10ng/ml)+EGF(20~30ng/ml)+HGF(10ng/ml)+IGF2(60ng/ml)进行培养,每隔1天更换一次培养基,培养8~14天
结果如图7~9所示。如图7所示,分化第8天的细胞显示出角质形成细胞样的形态。此外,如图8和图9所示,分化诱导得到的细胞在蛋白质水平和mRNA水平下显示出角质形成细胞标记的表达。
实施例4.Muse细胞向成纤维细胞的分化诱导
按照以下的步骤培养Muse细胞,由此进行了向成纤维细胞的分化诱导。
第0天Muse细胞的接种
第1天以10ml的DMEM low葡萄糖培养基+2μl TGF-β2(50μg/ml)+60μl AA(50mM)+100μlITS-A(胰岛素、转铁蛋白、亚硒酸钠)进行培养,每隔1天更换一次培养基,培养6天
第7天以DMEM low葡萄糖培养基+20%FBS+60μl AA(50mM)进行培养,每隔1天更换一次培养基,培养10天
第17天以DMEM low葡萄糖培养基+10%FBS进行培养
结果如图10~12所示。如图10所示,分化第10天的细胞显示出成纤维细胞样的形态。此外,如图11和图12所示,分化诱导得到的细胞在蛋白质水平和mRNA水平下显示出成纤维细胞标记的表达。
工业上的可利用性
本发明的细胞制剂,通过向大疱性表皮松解症患者施用,可以重建和修复损伤皮肤,改善或恢复皮肤症状,可以应用于大疱性表皮松解症的治疗。

1.一种用于治疗大疱性表皮松解症的细胞制剂,其含有来源于活体的间充质组织或培养的间充质细胞的SSEA-3阳性多能干细胞。
2.根据权利要求1所述的细胞制剂,其中,大疱性表皮松解症为单纯型大疱性表皮松解症。
3.根据权利要求1所述的细胞制剂,其中,大疱性表皮松解症为交界型大疱性表皮松解症。
4.根据权利要求1所述的细胞制剂,其中,大疱性表皮松解症为营养不良型大疱性表皮松解症。
5.根据权利要求4所述的细胞制剂,其中,营养不良型大疱性表皮松解症为显性营养不良型大疱性表皮松解症或隐性营养不良型大疱性表皮松解症。
6.根据权利要求1~5中任一项所述的细胞制剂,其中,所述多能干细胞为具有以下所有性质的多能干细胞:
(i)端粒酶活性低或没有端粒酶活性;
(ii)具有分化为三胚层中的任一胚层的细胞的能力;
(iii)不显示肿瘤性增殖;以及
(iv)具有自我更新能力。
7.根据权利要求1~5中任一项所述的细胞制剂,其中,所述多能干细胞为具有以下所有性质的多能干细胞:
(i)SSEA-3阳性;
(ii)CD105阳性;
(iii)端粒酶活性低或没有端粒酶活性;
(iv)具有分化为三胚层中的任一胚层的能力;
(v)不显示肿瘤性增殖;以及
(vi)具有自我更新能力。
8.一种皮肤细胞,其是从来源于活体的间充质组织或培养的间充质细胞的SSEA-3阳性多能干细胞分化诱导得到的。
9.根据权利要求8所述的皮肤细胞,其中,皮肤细胞为角质形成细胞和/或成纤维细胞。
10.一种用于治疗皮肤疾病的细胞制剂,其含有权利要求8或9所述的皮肤细胞。
11.根据权利要求10所述的细胞制剂,其中,皮肤疾病为大疱性表皮松解症。
