背景
荧光原位杂交(FISH)是一种细胞遗传学技术,其使用仅与染色体中具有高度序列互补性的部分结合的荧光探针。FISH长期以来用于检测染色体重排,而且由于染色体重排在人类癌症中是十分常见的,FISH通常被用于诊断或评估恶性肿瘤。

本文所用术语“互补的”是指通过非共价键与感兴趣的靶核酸碱基配对的核苷酸序列。在规范的沃森-克里克碱基配对中,在DNA中,腺嘌呤(A)与胸腺嘧啶(T)形成碱基对,鸟嘌呤(G)与胞嘧啶(C)形成碱基对。在RNA中,胸腺嘧啶由尿嘧啶(U)取代。因此,A与T互补,G与C互补。在RNA中,A与U互补,反之亦然。通常,“互补的”是指核苷酸序列与感兴趣的靶点完全互补,使得序列中的每个核苷酸与靶核酸中相应位点的每个核苷酸互补。在某些情况下,核苷酸序列可与靶点部分地互补,其中不是所有的核苷酸都与靶核酸中所有的相应位点的每个核苷酸互补。
术语“测定(determining)”、“测量(measuring)”、“评估(evaluating)”、“评价(assessing)”、“分析(analyzing)”和“测定(assaying)”在本文中可互换使用,其是指任何形式的测量,且包括测定某一要素是否存在。这些术语包括定量和/或定性测定。评价可以是相对的或绝对的。“评价存在”包括确定存在的某物的数量,以及确定其是否存在。

本文所用的术语“染色体重排”是指染色体的一个或多个部分在单个染色体内或染色体之间重排的事件。在某些情况下,染色体重排可反映染色体结构中的异常。例如,染色体重排可以是倒置、缺失、插入或易位。

在潜在的易位断裂点的上下文中,术语“一侧”和“另一侧”是指潜在的易位断裂点的位置的相对侧的区域。在一些情况下,“一侧”和“另一侧”可能指的是第一侧和第二侧,其中第一和第二侧是潜在的易位断裂点的相对侧。如果探针与潜在的易位断裂点的一侧或另一侧结合,则该探针可与距离该潜在的易位断裂点小于10MB(例如小于5MB、小于1MB、小于500kb或小于100kb)处的序列结合,尽管在某些情况下可使用与距离潜在的易位断裂点大于10MB处的序列结合的探针。

术语“ALK”是指编码间变性淋巴瘤激酶的基因(也称为ALK酪氨酸激酶受体或CD246(分化簇246));参见Morris等Science 1994 263:1281–4和NCBI Entrez Gene ID:238。该基因编码受体酪氨酸激酶,其属于胰岛素受体超家族。该蛋白质包含细胞外域(对应于单次通过跨膜区的疏水性延伸)和细胞内激酶结构域。它在大脑发育中起重要作用,并对神经系统中特定神经元发挥其作用。已发现该基因在一系列肿瘤中重排、突变或扩增,所述肿瘤包括间变性大细胞淋巴瘤、神经母细胞瘤和非小细胞肺癌。染色体重排是该基因中最常见的遗传改变,染色体重排在肿瘤发生中导致产生多个融合基因,包括ALK(2号染色体)/EML4(2号染色体)、ALK/RANBP2(2号染色体)、ALK/ATIC(2号染色体)、ALK/TFG(3号染色体)、ALK/NPM1(5号染色体)、ALK/SQSTM1(5号染色体)、ALK/KIF5B(10号染色体)、ALK/CLTC(17号染色体)、ALK/TPM4(19号染色体)和ALK/MSN(X染色体)。该基因位于人2号染色体的chr 2:29.19–29.92Mb。
术语“ROS1”是指编码原癌基因酪氨酸-蛋白激酶ROS(也称为ROS1、MCF3、ROS和c-ros-1)的基因(参见如Galland等1992Cytogenetics and Cell Genetics 60(2):114–6和NCBI Entrez Gene ID:6098)。首先在胶质母细胞瘤肿瘤和细胞系中检测到涉及ROS1基因的基因重排。2007年,在源自肺腺癌患者的细胞系中发现了ROS1重排。自此发现以来,多项研究表明肺癌发病率约为1%。该基因位于人2号染色体的Chr 6:117.29–117.43Mb。

探针系统

大约60%的间变性大细胞淋巴瘤(ALCL)与ALK易位相关。这些易位多数产生融合基因,其由来自2号染色体的ALK基因的3'半部分和来自5号染色体的核仁磷酸蛋白(NPM)基因的5'部分组成。NPM-ALK融合基因的这种产物是致癌的。在其他易位中,ALK的3'半部分与TPM3基因(编码原肌球蛋白3)的5'序列融合。在极少数情况下,ALK与其他5'融合配偶体(如TGF、ATTIC、CLTC 1、TPM4、MSN、ALO 17、MYH9)融合。EML4易位是约3-5%的非小细胞肺癌(NSCLC)的原因。绝大多数情况是腺癌。ALK肺癌发现于所有年龄段的患者中,但平均而言这些患者趋向于更年轻。ALK肺癌在轻度吸烟者或不吸烟者中更常见,但是相当多的患有这种疾病的患者是当前或以前的吸烟者。NSCLC中的EML4-ALK重排是独有的,在EGFR或KRAS突变的肿瘤中未发现。ALK易位还与神经母细胞瘤、炎性肌纤维母细胞瘤、成人和小儿肾细胞癌、食道鳞状细胞癌、乳腺癌(特别是炎性亚型)、结肠腺癌、多形性胶质母细胞瘤和间变性甲状腺癌的遗传情况相关。
ROS1的遗传变化(例如基因重排)产生的癌基因也可导致癌症(Stumpfova和Janne,Clin Cancer Res.2012 18:4222-4)。ROS1在NSCLC患者中以融合蛋白(ROS1的6个不同配偶体)的形式被发现,并且在大约2%的NSCLC患者中发现(J Clin Oncol.2012Mar 10;30(8):863-70)。已在多种其他癌症(包括多形性胶质母细胞瘤、胆管癌、卵巢癌、胃腺癌、结肠直肠癌、炎性肌纤维母细胞瘤、血管肉瘤和上皮样血管内皮瘤)中检测到另外两种ROS1基因重排。ROS1基因重排产生具有组成型活性的激酶域的融合蛋白,其激活下游信号传导途径,导致细胞中的致癌特性,包括不受控制的增殖和对细胞死亡的抗性,伴随肿瘤细胞的存活延长。上述途径包括用于细胞增殖的Ras-ERK和调节细胞存活(抗凋亡)和增殖的JAK-STAT和PI3K/AKT途径。ROS1融合蛋白也可激活mTOR途径,mTOR途径对于调控蛋白质翻译至关重要。这些途径激活的癌症倾向于更具攻击性,发生侵袭和转移,导致患者的生存率低。
在一些实施方案中,第一和第三探针用第一荧光团(即相同的荧光团)标记。第二和第四探针可以用第二荧光团(可与前述第一荧光团区分的荧光团)标记。第五探针应与第一、第二、第三和第四探针可区分地标记,例如,使用第三种可区分的荧光团。在一些实施方案中,第一和第三探针用第一荧光团标记,第二和第四探针用第二荧光团标记;且第五探针用第三荧光团标记。如本文所用,术语“可区分地标记”表示各标记可以被分别检出,即使它们位于相同位置。因此,应选择所用的荧光团,使得它们可以彼此区分,即可彼此独立地检测,这意味着各标记独立地被检出和测量,即使当这些标记被混合在一起时也是如此。换句话说,每个标记的存在应是可以分别确定的,即使当所述标记位于同一位置时也是如此。

样品分析方法
还提供了样品分析方法。此方法涉及(a)使所述探针系统与染色体原位杂交以产生标记的样品;(b)读取所述标记的样品以检测所述标记的探针的杂交,例如使用装有针对每个荧光团的适当滤光片的荧光显微镜或者使用观察多个荧光团的三重带通滤光片组(参见如美国专利号5,776,688);且(c)使用获自读取步骤(b)的结果来确定所述样品是否包含ALK或ROS1中的重排。显而易见的是,如果第一和第二基因座是ALK和ROS1,则所述方法将包含(c)使用获自读取步骤(b)的结果来测定所述样品是否包含ALK或ROS1中的重排。
通过检查读取步骤中产生的图像来确定重排。具体地,如果第五探针与所述第一基因座中潜在的易位断裂点的两侧杂交,那么:
a)在“正常”细胞(不具有所述第一基因座或所述第二基因座中的重排的细胞)中:i.第一和第二探针与彼此共定位并与第五探针共定位,且ii.第三和第四探针与彼此共定位但不与第五探针共定位;
b)在包含所述第一基因座中的易位的细胞中:i.第一探针和第二探针与第五探针共定位但与彼此不共定位,且ii.第三和第四探针与彼此共定位但不与第五探针共定位;且
c)在包含所述第二基因座中的易位的细胞中:i.第一和第二探针与第五探针共定位且彼此共定位,且ii.第三和第四探针不与彼此共定位亦不与第五探针共定位。
同样,如果第五探针与所述第二基因座中潜在的易位断裂点的两侧杂交,那么:
a)在“正常”细胞(不具有所述第一基因座或所述第二基因座中的重排的细胞)中:i.第一和第二探针与彼此共定位但不与第五探针共定位,且ii.所述第三和第四探针与彼此共定位且与第五探针共定位;

鉴于该方法的原理,可以设计和实现更高复杂性的探针系统,例如包含四个或甚至五个可区分的荧光团的探针系统。
原位杂交方法是本领域熟知的,其通常涉及将细胞加载到支持物上,固定和透化细胞,使探针与细胞中的染色体原位杂交,洗掉未结合的细胞并使用荧光显微镜成像标记的细胞,因此,在参考本说明书的基础上,可以借鉴已知的规程来适用该方法。参见如Jin,Journal of Clinical Laboratory Analysis 1997 11(1):2–9。确实,在参考本说明书的基础上,可以借鉴临床上应用的用于评定ALK中的重排的方法学(参见如Gao等J.Thorac.Oncol.201510:1648-52;Conde等PLoS One 2014 9:e107200,Kim等Transl.LungCancer Res.2015 4:149-55和Teixidó等Transl.Lung Cancer Res.2014 3:70-4)来适用该方法的一些实施方案。
在一些实施方案中,所述样品可以在三个通道中备好,所述三个通道对应于所使用的各标记,以产生样品的多个图像。该方法产生的多个图像可以并排观察,或者在一些实施方案中,可以叠加或组合这些图像。在某些情况下,图像可以是彩色的,其中图像中所用的颜色可对应于所用的标记。
在一些实施方案中,所述方法可进一步包括分析、比较或重叠所述图像中的至少两个。在一些实施方案中,所述方法可进一步包含重叠所有的图像以产生显示所有探针与所述样品结合的模式。使用的图像分析模块可以转换来自每个荧光团的信号以产生多个伪彩色图像。图像分析模块可以重叠多个伪彩色图像(例如在每个像素处叠加伪彩色)以获得多重化的伪彩色图像。可以将多个图像(例如,未加权的或加权的)转换成单个伪彩色,以便例如表示特定探针的结合所表征的感兴趣的生物学特征。可以基于用户的手动输入将伪彩色指配给特定探针或探针组合。可以进一步配置图像分析模块,以调整(例如,归一化)信号强度或伪彩色的强度和/或对比度,进行卷积操作(诸如强度或伪彩色的模糊或锐化),或进行任何其他适当的操作来增强所述图像。图像分析模块可进行任何上述操作以对准获自连续图像的多个像素,和/或以使获自连续图像的多个像素的强度或伪彩色模糊或平滑。
图像分析可在计算机上实现。在某些实施方案中,可将通用计算机配置为用于本文公开的方法和程序的功能布局。这种计算机的硬件体系结构是本领域技术人员所熟知的,且可包括:硬件组件,其包括一个或多个处理器(CPU)、随机存取存储器(RAM)、只读存储器(ROM)、内部或外部数据存储介质(如硬盘驱动器)。计算机系统还包含一个或多个用于处理和输出图形信息以显示资料的图形卡。上述组件可以通过计算机内的总线适当地互连。计算机可进一步包括用于与诸如显示器、键盘、鼠标、网络等通用外部组件通信的合适接口。在一些实施方案中,计算机能够进行并行处理,或者可以是配置为并行或分布式计算的网络的一部分,以增加本方法和程序的处理能力。在一些实施方案中,从存储介质读出的程序代码可以写入由插入计算机的扩展卡或连接到计算机的扩展单元所提供的存储器中,并且该扩展卡或扩展单元中提供的CPU等可根据程序代码的指令来实际执行部分或全部操作,以完成如下所述的功能。在其他实施方案中,可以使用云计算系统来实施该方法。在另一个实施方案中,可将数据文件和程序导出到云计算机,所述云计算机运行程序并将输出返回至用户。
可使用此方法分析任何类型的细胞。在某些情况下,所分析的样品可以是获自患者的新鲜或包埋的(例如包埋在FFPE中的)组织活检。感兴趣的组织活检包括皮肤(黑色素瘤、恶性上皮肿瘤等)、软组织、骨、乳房、结肠、肝、肾、肾上腺、胃肠组织、胰腺、胆囊、唾液腺、宫颈、卵巢、子宫、睾丸、前列腺、肺、胸腺、甲状腺、甲状旁腺、垂体(腺瘤等)、脑、脊髓、眼组织、神经和骨骼肌等的肿瘤活检和非肿瘤性活检。
在任何实施方案中,可将数据(如细胞的图像)转发到“远程位置”,其中“远程位置”表示不同于产生数据的位置的位置。例如,远程位置可以是同一城市中的另一个位置(例如办公室、实验室等),不同城市中的另一个位置,不同州中的另一个位置,不同国家中的另一个位置。因此,当说明一个物品相对于另一个项目为“远程”时,这意味着这两个物品可以在同一个房间中但是是分开的,或者至少在不同的房间或不同的建筑物中,并且可以相隔至少一英里、十英里或至少一百英里远。“通信”信息是指通过合适的通信信道(例如私有或公共网络)传送表示该信息的数据。“转发”物品是指任何使一个项目从一个位置到下一个位置的方式,无论是通过物理运输该项目还是以其他方式(在可能的情况下),并且包括,至少对于数据而言,物理运输携带数据的介质或传递数据。通信介质的实例包括无线电或红外传输信道以及与另一计算机或网络设备连接的网络,以及互联网,或者包括电子邮件传输和记录在网站上的信息等。在某些实施方案中,可由医学博士(MD)或其他合格的医学专业人员分析由该方法产生的一个或多个图像,并且可以将基于图像分析结果的报告转发给样品所来源的患者。
效用
可将本方法用于各种诊断、药物发现和研究应用,其包括但不限于诊断或监测疾病或病症(其中第一或第二基因座中的重排可以是该疾病或病症的标记物)、药物靶标的发现(其中第一或第二基因座中的重排可以是药物治疗的靶标)、药物筛选(其中通过第一或第二基因座中的重排监测药物的效果)、确定药物敏感性(其中药物敏感性与第一或第二基因座中的重排相关)和基础研究(其中期望鉴定第一或第二基因座中的重排)。因此,在一些实施方案中,该方法可包括:如果所述样品包含重排,则提供诊断、治疗或预后。
在一些情况下,本文描述的方法可用于确定患者的治疗计划。第一或第二基因座中重排的存在或不存在可指示患者对特定疗法是有反应的或不起反应的。例如,一种或多种生物标志物的存在或不存在可指示:疾病对特定疗法是不起反应的,可以施用替代疗法。
在所述第一和第二基因座分别为ALK或ROS1的实施方案中,在任一基因座中的重排可指示应该用克唑替尼或另一种激酶抑制剂治疗患者。在这些实施方案中,如果鉴定出ALK或ROS1中的重排,则医护专业人员(如医学博士等)可提供克唑替尼治疗的建议。克唑替尼是一种抗癌药物,起抑制ALK(间变性淋巴瘤激酶)和ROS1(c-ros致癌基因1)的作用(参见Forde Expert Opin.Pharmacother.2012 13:1195–201;Roberts Biologics 2013 7:91–101;Sahu等South Asian J.Cancer 2:91–7),美国和其他国家已批准其用于治疗某些非小细胞肺癌(NSCLC),且正在进行临床试验,测试其在成人和儿童的间变性大细胞淋巴瘤、神经母细胞瘤和其他晚期实体肿瘤中的安全性和有效性。
备选实施方案
在一些备选实施方案中,所述系统不包含第五探针。在这些实施方案中,所述探针系统可包含:(a)第一标记的探针,其与第一基因座(如ALK)中潜在的易位断裂点的一侧杂交;(b)第二标记的探针,其与所述第一基因座(如ALK)中潜在的易位断裂点的另一侧杂交;(c)第三标记的探针,其与第二基因座(如ROS1)中潜在的易位断裂点的一侧杂交;(d)第四标记的探针,其与所述第二基因座(如ROS1)中潜在的易位断裂点的另一侧杂交;其中:i.所述第一和第二探针是可区分地标记的;ii.所述第三和第四探针是可区分地标记的。在使用此类探针系统中,如果鉴定出染色体易位,则必须进行后续工作以鉴定哪个基因座具有易位。

实施方案2.实施方案1的方法,其中所述第一和第三探针是用第一荧光团标记的。
实施方案3.任何前述实施方案的方法,其中所述第二和第四探针是用第二荧光团标记的。
实施方案4.任何前述实施方案的方法,其中:所述第一和第三探针是用第一荧光团标记的;所述第二和第四探针是用第二荧光团标记的;且所述第五探针是用第三荧光团标记的。
实施方案5.任何前述实施方案的方法,其中每个探针跨越至少10kb。

实施方案11.实施方案10的方法,其中所述读取是通过荧光显微法进行的。
实施方案12.任何前述实施方案的方法,其中所述细胞是哺乳动物细胞。
实施方案13.任何前述实施方案的方法,其中所述第一基因座是ALK,且所述第二基因座是ROS1。

实施例
本研究的目的是为了证明使用单一DNA FISH探针测定可以区分具有ALK或ROS1任一基因重排的样品(即,每个样品将会具有独特的荧光信号特征)。为此,产生了表1和图2A和B中描述的各个组成探针,并将它们组合以制备靶向ALK和ROS1两个基因座的最终探针。对于ROS1重排的样品,将会看到至少一种红色/蓝色和/或绿色/蓝色信号对(图3A和3B)。ALK重排的样品中,将会观察到至少一种红色和/或绿色的信号(图4)。
设计并合成了靶向表1中概述的区域的长寡核苷酸库。使用所述寡核苷酸作为模板,利用聚合酶链式反应(PCR)来掺入红色、绿色或蓝色荧光团,以产生标记的组成探针。组合所述标记的组成探针,并与FISH杂交缓冲液混合以产生最终探针。使此探针与具有ALK或ROS1任一基因重排的细胞系杂交。使用装有Cy3(红色)、FITC(绿色)和Aqua(蓝色)滤光片的落射荧光显微镜可视化所述杂交的样品。捕获反映在重排的样品上观察到的信号特征的图像(图3和4)。

Claims (12)
1.一种探针系统,其包含:
(a)第一标记的探针,其与ALK中潜在的易位断裂点的一侧杂交;
(b)第二标记的探针,其与ALK中潜在的易位断裂点的另一侧杂交;
(c)第三标记的探针,其与ROS1中潜在的易位断裂点的一侧杂交;
(d)第四标记的探针,其与ROS1中潜在的易位断裂点的另一侧杂交;和
(e)第五标记的探针,其与ALK及ROS1中的任一者,但不是二者,中潜在的易位断裂点的两侧杂交;
其中:i.所述第一和第二探针是可区分地标记的;
ii.所述第三和第四探针是可区分地标记的;且
iii.所述第五探针是与第一、第二、第三和第四探针可区分地标记的。
2.权利要求1的方法,其中所述第一和第三探针是用第一荧光团标记的。
3.任一前述权利要求的方法,其中所述第二和第四探针是用第二荧光团标记的。
4.任一前述权利要求的方法,其中:
所述第一和第三探针是用第一荧光团标记的;
所述第二和第四探针是用第二荧光团标记的;且
所述第五探针是用第三荧光团标记的。
5.任一前述权利要求的方法,其中(a)-(e)中的每个探针跨越至少10kb。
6.任一前述权利要求的方法,其中每个探针包含多个标记的核酸片段。
7.任一前述权利要求的方法,其中每个探针包含标记的双链的核酸。
8.一种样品分析的方法,其包含:
(a)使任一前述权利要求的探针系统与染色体原位杂交以产生标记的样品;
(b)读取所述标记的样品以检测所述标记的探针的杂交;且
(c)使用获自读取步骤(b)的结果来确定所述样品是否包含ALK或ROS1中的重排。
9.权利要求8的方法,其中所述读取是通过荧光显微法进行的。
10.权利要求8-9中任一项的方法,其中所述细胞是哺乳动物细胞。
11.权利要求8-9中任一项的方法,其进一步包括:如果所述样品包含所述重排,则提供诊断、治疗或预后。
12.权利要求8-9中任一项的方法,其进一步包括:如果所述样品包含所述重排,则提供用克唑替尼治疗的建议。
