Abstract
本发明涉及一种针对脑胶质母细胞瘤靶向纳米磁共振对比剂及制备、应用;所述磁共振对比剂由具有靶向结合功能的小分子、荧光标记物和磁性氧化铁纳米载体组成;所述的具有靶向结合功能的小分子为针对脑胶质母细胞瘤细胞表面高表达的EGFRvIII具有亲和力的短肽;所述荧光标记物以共价连接的方式包载在纳米载体表面,短肽通过共价连接方式与纳米粒表面的聚乙二醇相连。该对比剂可通过静脉注射,使得短肽与肿瘤细胞表面高表达的EGFRvIII结合,促进纳米载体在肿瘤部位浓聚,有助于临床提高治疗的精确性和安全性,为肿瘤放化疗的疗效评价和患者预后的评估提高无创性可视化手段。

技术领域
本发明属于分子影像学技术领域,涉及一种针对脑胶质母细胞瘤的靶向纳米磁共振对比剂及制备、应用。
背景技术
胶质母细胞瘤是成人最常见的原发性脑肿瘤,由于其具有血管丰富、浸润生长以及易复发的特点,使之成为恶性程度最高、最难治愈的肿瘤之一,发病率为5.26/10万,每年临床新增17000确诊病例。占所有原发性脑肿瘤的52%和颅内肿瘤的20%。有资料显示,胶质母细胞瘤经确诊后1年生存率仅为30%左右,平均生存期仅53周,即使在联合了手术、放疗和替莫唑胺化疗等治疗后,胶质母细胞瘤患者的平均生存期也只有19.6个月。由于血脑屏障(blood‐brain barrier,BBB)的存在,使98%的小分子化合物和几乎所有大分子物质不能达到胶质瘤所在部位,造成了早期诊断困难,并限制了对化疗药物的选择。
传统的X线、超声、CT、MRI和PET难以发现早期阶段的肿瘤,对其定位、定性诊断相当困难。分子影像作为当今医学影像学发展的方向,以分子生物学为基础,借助现代医学影像技术真正实现在活体上用无创伤可视化技术,从细胞及分子水平动态定量观测功能蛋白(受体、酶)和功能基因表达及产生作用的实时成像,其优势是动态客观的定量描述启动疾病发生的分子作用、促进疾病发展的基因表达、反映疾病预后的蛋白变化、评估治疗效果的动态反映、设计研发新药的靶点定位与机制研究等。多模态分子影像技术是将同时具有多种显像功能的分子探针注入机体,通过多种成像技术的检测,获取病变部位多种信息的一门新兴的分子影像技术。目前,应用于肿瘤的分子显像技术有光学成像技术、CT成像技术、MRI分子成像技术和超声分子影像技术等。光学成像可显示组织功能、代谢及分子信息,MRI具有安全、无创、高空间分辨率、多方位及多参数成像等特点,可作为分子影像的重要成像手段,但是,单一分子影像技术存在着自身难以客服的缺陷,任何一种单一的成像方式都不足以充分地获取肿瘤的信息,联合使用多种分子成像技术可实现彼此优势互补,能够为明确诊断提供更加精确而全面的信息。当前,本领域中以超小型超顺磁性氧化铁纳米粒(UltrasmallSuperparamagnetic IronO xide nanoparticels,USPIO)为基础的纳米探针,因其半衰期较长、在组织中广泛分布,易于在细胞间通透移动,T2*效应致信号强度明显降低,方法简单,重复性好,适用于带有特定分子的细胞及组织显像,已成为分子影像学研究热点。
研究表明,许多恶性肿瘤均呈现表皮生长因子受体(epidermal growth factorrece ptor,EGFR)过表达,EGFR与这些肿瘤的发生发展有着密切关系,以EGFR作为靶点的抗肿瘤治疗研究已取得了令人瞩目的进展。由于EGFR在人体许多正常组织亦有表达,针对EGFR的靶向治疗药物不可避免地带来一定副作用,从而限制了其临床应用。近年研究发现,EGFR存在多种突变体,其中最多见的,也是目前人们研究最为深入的突变类型为EGFRvIII型突变体(epidermal growth factor receptor variant IIl,EGFRvIII),是EGFR mRNA外显子中2~7区域被删除所致,这种突变体仅表达于胶质母细胞瘤和其它一些肿瘤细胞表面,正常组织细胞中并不表达,因而成为肿瘤诊断和治疗的理想靶点。
目前,现有影像诊断技术无法从分子机制层面解决胶质瘤的早期准确诊断及对肿瘤边界的明确显示这一问题,也不存在一种递药系统导向性的将相关治疗药物靶向运至病灶、从而实现显影、治疗一体化。
发明内容
本发明的目的在于针对现有技术缺陷,拟以短肽为靶向结合功能分子,聚乙二醇‐超小型超顺磁性氧化铁纳米粒(PEG‐USPIO)为载体,合成特异性、高效性的标记胶质瘤的荧光-MRI双模态探针,作为全新的脑胶质母细胞瘤靶向纳米磁共振对比剂;具体提供了一种针对脑胶质母细胞瘤病灶的靶向纳米磁共振对比剂的制备及应用。本发明的对比剂可通过短肽与脑胶质母细胞瘤细胞表面的EGFRvIII突变体特异性结合,可在术前准确的显示胶质母细胞瘤病灶。
本发明的目的是通过以下技术方案的内容来实现的:
第一方面,本发明提供一种针对脑胶质母细胞瘤的靶向纳米磁共振对比剂,所述纳米磁共振对比剂包括超小型超顺磁型氧化铁纳米颗粒(USPIO)、短肽PEPHC1、免疫荧光标记物、聚乙二醇(PEG);所述免疫荧光标记物以共价连接的方式标记在所述超小型超顺磁性氧化铁粒子表面,所述短肽PEPHC1通过共价连接方式与所述超小型超顺磁性氧化铁粒子表面的聚乙二醇相连。
优选地,所述超小超顺磁性氧化铁粒子表面连接有活性基团,所述活性基团包括羧基、氨基、巯基、生物素或亲和素。
优选地,所述超小超顺磁性氧化铁纳米粒(USPIO)的直径在50nm以下。所述US PIO安全性高,可生物讲解,主要缩短组织T2时间,可与多种配体连接形成靶向对比剂,灵敏度、特异度高。
更优选地,所述超小型超顺磁性氧化铁纳米粒(USPIO)的粒径为10~20nm。
优选地,所述免疫荧光标记物包括香豆素6和/或Cy7.5。
优选地,所述聚乙二醇的平均分子量为1000~20000。
更优选地,所述聚乙二醇的平均分子量为1000~4000。
最优选地,所述聚乙二醇的平均分子量为2000。
第二方面,本发明提供一种针对脑胶质母细胞瘤靶向纳米磁共振对比剂的制备方法,所述方法包括以下步骤:
步骤一、将超小超顺磁性氧化铁粒子(USPIO)、磷脂酰乙醇胺‐聚乙二醇(DSPE‐PEG)、磷脂酰乙醇胺‐聚乙二醇‐活性酯(DSPE‐PEG‐NHS)及免疫荧光标记物按重量比(4‐8):(20‐40):2:1混合,溶于二氯甲烷,超声,旋蒸去除二氯甲烷,水分散,获得水分散纳米粒;当上述原料的重量比小于4:20:2:1,或大于‐8:40:2:1时,聚乙二醇无法均匀包被于超小超顺磁性氧化铁粒子表面而使制备的对比剂易凝聚,从而影响成像效果;
更优选,将超小超顺磁性氧化铁粒子(USPIO)、磷脂酰乙醇胺‐聚乙二醇(DSPE‐PEG)、磷脂酰乙醇胺‐聚乙二醇‐活性酯(DSPE‐PEG‐NHS)及免疫荧光标记物按重量比420:2:1混合,溶于二氯甲烷,超声10min,37℃旋蒸去除二氯甲烷后呈薄膜覆盖在瓶内壁,用双蒸水分散薄膜,获得水分散纳米粒;
步骤二:向水分散纳米粒加入巯基活化的短肽PEPHC1,超声,10000‐12000rpm、4℃离心,弃上清,用双蒸水分散沉淀,得靶向纳米磁共振对比剂;
更优选,12000rpm、4℃离心。
步骤二中,所述水分散纳米粒中超小型超顺磁性氧化铁纳米粒(USPIO)与巯基活化的短肽PEPHC1重量比为(1‐5):(1‐5)。更优选重量比为1:1。
步骤二中,水分散纳米粒和短肽PEPHC1通过共价结合反应得靶向纳米磁共振对比剂。
第三方面,本发明提供一种针对脑胶质母细胞瘤的靶向纳磁共振对比剂在用作肿瘤的多模态针对中的用途。
第四方面,本发明提供一种针对脑胶质母细胞瘤的靶向纳磁共振对比剂在制备特异性显示高表达EGFRvIII突变体胶质瘤的临床诊断试剂中的应用。
优选地,所述临床诊断试剂包括胶质瘤病灶的早期MRI分子影像诊断试剂。
与既往抗原抗体结合不同,本发明筛选出的短肽PEPHC1分子量小,结构稳定,与EGFRvIII突变体亲和力高,不干扰细胞代谢等优点,易通过共价键与USPIO结合,首次利用该短肽与超顺磁性纳米粒子连接构建磁共振对比剂,即纳米粒子+短肽+聚乙二醇+免疫荧光标记物构成的完整的化合物分子,可更好的显示肿瘤病灶。与现有技术相比,本发明具有如下有益效果:
1、本发明针对脑胶质母细胞瘤的靶向纳磁共振对比剂,是以胶质母细胞瘤细胞表面高表达EGFRvIII突变体为靶点,由聚乙二醇(PEG),超小型超顺磁性氧化铁纳米粒(USPIO),荧光标记物香豆素‐6(Cou‐6)或Cy7.5及导向性短肽PEPHC1组成。该递药系统一方面可通过与EGFRvIII突变体特异性结合,并通过长循环纳米粒的增强渗透和滞留作用在肿瘤组织内蓄积,蓄积在组织内的纳米粒可以被MRI及活体荧光成像仪所识别,实现脑胶质母细胞瘤的双模态显像。
2、本发明的靶向纳磁共振对比剂可通过静脉注射,使得短肽与肿瘤细胞表面高表达的EGFRvⅢ结合,促进纳米载体在肿瘤部位浓聚,有助于临床提高治疗的精确性和安全性,为肿瘤放化疗的疗效评价和患者预后的评估提高无创性可视化手段。

具体实施方式
下面结合实施例对本发明进行详细说明。以下实施例将有助于本领域的技术人员进一步理解本发明,但不以任何形式限制本发明。应当指出的是,对本领域的普通技术人员来说,在不脱离本发明构思的前提下,还可以做出若干调整和改进。这些都属于本发明的保护范围。
实施例1靶向纳米磁共振对比剂的制备与表征
PEG‐USPIO纳米粒的制备:将超小超顺磁性氧化铁粒子(USPIO)、磷脂酰乙醇胺‐聚乙二醇(DSPE‐PEG)、磷脂酰乙醇胺‐聚乙二醇‐活性酯(DSPE‐PEG‐NHS)及免疫荧光标记香豆素6或Cy7.5按重量比4:20:2:1混合,溶于二氯甲烷后,超声10min,37℃旋蒸60min去除二氯甲烷呈薄膜覆盖在瓶内壁,水分散薄膜,获得水分散纳米粒;
PEG‐USPIO‐PEPHC1纳米粒的制备:在制备所得的去离子水分散纳米粒4ml(0.25mg/mL USPIO)加入巯基活化的短肽PEPHC1 20mg/mL 50μL,超声10min,12000rpm 4℃离心,弃上清,用双蒸水分散沉淀,得靶向纳米磁共振对比剂。
靶向纳米磁共振对比剂的表征:采用透射电子显微镜观察粒子形态,采用粒径分析仪测定粒子的粒径和Zeta电位,结果如图1所示,PEG‐USPIO‐PEPHC1纳米粒形状规则圆整,呈分散的单球状,平均粒径约为110nm,电位为‐35.6mV。
实施例2靶向纳米磁共振对比剂的磁敏感和荧光特征表征图
取适量纳米磁共振对比剂溶液样品,用去离子水配制不同浓度的PEG‐USPIO‐PEPHC1,铁浓度依次为0、5、10、20、40ug/mL,分别置于2ml离心管内。再分别配置相应铁浓度的PEG‐USPIO作对照。各管按顺序摆放在塑料试管架上,然后放在装满水的盒子里。去离子水作空白对照。利用临床3.0T MR system(Discovery MR750,GE M edical System,USA)及配套小动物线圈进行T2Map序列扫描,评价负性强化效果。采用GE Function tool 4.6专用软件对T2Map图像进行后处理。
PEG‐USPIO‐PEPHC1‐Cy7.5荧光强度检测:配置浓度5、10、20、40、80ug/mL的纳米磁共振对比剂溶液,放入96孔黑板中,激发波长为788nm,发射波长为808n m,小动物活体成像仪IVIM检测。
磁敏感如图2A所示,与对照组比较,PNP图像呈明显低信号,随着铁浓度的逐渐增加,T2值逐渐下降。经过测定与计算,T2弛豫时间与对比剂浓度呈良好的线性关系,随着对比剂浓度的增加,相应线性曲线的斜率逐渐增加。以上述两组溶液中不同Fe2+的浓度和相应的1/T2作散点图,如图2B所示,NP的线性回归方程为:y=69.6x+4.272(R2=0.972,P<0.05),其T2弛豫率69.60mM‐1·s‐1;PNP的线性回归方程为:y=52.35x+1.578(R2=0.989,P<0.05),其T2弛豫率为和52.35mM‐1·s‐1。
荧光特征表征如图2C、D所示,随着纳米探针浓度的增加,其荧光强化明显增强。进一步荧光强度定量分析,当PNP纳米探针浓度为80μg/mL时,其荧光强度值为(3.687±0.322)x108,此外荧光强度与纳米粒浓度间具有良好的线性关系。
实施例3体外细胞抗体与短肽分别检测EGFRvIII突变体的表达
细胞表面抗原表达检测
1)倒置荧光显微镜分析:
将U87或U87‐EGFRvIII细胞以1x105细胞密度接种于12孔板中,24h贴壁,吸弃培养液,PBS洗3遍,每次3分钟;4%多聚甲醛固定20min,自然干燥10min,PBS洗3遍,每次5分钟;加入免疫染色封闭液室温放置1h,吸取封闭液后,加入一抗,4℃孵育过夜;次日,采用PBS充分清洗,加入二抗,室温下避光孵育1h,PBS充分清洗后,加入1%DAPI染色液,室温下细胞核避光染色5分钟,PBS清洗3次,每次5分钟,荧光显微镜拍照。短肽PEPHC1检测步骤基本同上,不同之处在于不需4℃过夜处理。
2)流式细胞仪荧光定量分析
将U87或U87‐EGFRvIII细胞用0.25%胰蛋白酶‐0.02%EDTA消化液制成单细胞悬液;离心收集细胞,重新悬浮于PBS中,调整细胞密度至5×105;加入一抗(1μg/mL),室温下孵育30分钟;用冰冷PBS离心、洗涤两次后,加入二抗(15μg/mL)于室温下孵育30分钟;离心收集细胞,并重新悬浮于PBS中。采用流式细胞仪进行分析,激发波长为488nm,收集525nm处的Cou‐6荧光,使用FlowJo 7.6.1软件进行直方图分析,分析时设门以去除细胞碎片和细胞团块,对测定的干扰,根据荧光峰的位移情况来判断的EGFRvIII表达水平。短肽PEPHC1检测步骤基本同上。
对照组细胞和实验组细胞的EGFRvIII突变体表达效果图如图3所示,由图3A可知,通过免疫荧光共聚焦观察可见细胞核DAPI染色呈蓝色,EGFRvIII用Alexa Fluor 647标记呈红色。阴性对照组细胞U87表面未见明显红色显影,阳性实验组细胞U87‐EGFRvIII可见明亮的红色荧光,说明EGFRv III受体转染成功,阳性实验组细胞U87‐EGFRvIII表面EGFRvIII呈高表达。由图3C、E可知,与空白对照组相比,阳性实验组细胞U87‐EGFRvIII在加入EGFRvIII单克隆抗体后,其荧光峰的位置明显向右移,而阴性对照组细胞U87同样在加入EGFRv III单克隆抗体后,其荧光峰的位置未见明显移位,表明EGFRvIII受体转染成功,阳性实验组细胞U87‐EGFRvIII表面EGFRv III呈高表达。由图3B可知,通过荧光显微镜观察可见细胞核DAPI染色呈蓝色,EGFRvIII在细胞膜表面标记呈绿色。阴性对照组细胞U87表面未见明显绿色显影,阳性实验组细胞U87‐EGFRvIII可见明亮的绿色荧光,说明EGFRv III受体转染成功,阳性实验组细胞U87‐EGFRvIII表面EGFRvIII呈高表达。由图3D、F可知,与空白对照组相比,阳性实验组细胞U87‐EGFRvIII在加入5FAM标记的短肽PEPHC1后,其荧光峰的位置明显向右移,而阴性对照组细胞U87同样条件下其荧光峰的位置未见明显移位,表明EGFRv III受体转染成功,阳性实验组细胞U87‐EGFRvIII表面EGFRvIII呈高表达。
实施例4磁性纳米对比剂体外毒性实验研究
MTT细胞活性检测:取对数期生长状态的细胞U87、U87‐EGFRvIII,以1x103/孔的密度接种于96孔培养板内,每孔200ul培养液,置于37℃、含5%CO2、湿度>95%培养箱中培养24h,待细胞近于融合时,弃去培养液,加入含不同浓度PEG‐SPIO、PEG‐SPIO‐PEPHC1(0、25、50、100、200ug/mL)培养液200uL至孔内,培养24h,弃去培养液,PBS洗两遍,每孔加入MTT溶液(5mg/ml)20uL,37℃继续孵育4h,终止培养,小心吸弃孔内培养液,每孔加入150uLDMSO,振荡10min,使甲臜充分溶解。比色:选择490nm波长,在酶联免疫检测仪上测定各孔光吸收值。
靶向纳米磁共振对比剂对EGFRvIII突变体特异性细胞的生存率的影响如图4所示,由图4可知,不同USPIO浓度(0、25、50、100、200μg/mL)的PNP和NP与U87‐EGFRv III和U87细胞共孵育72h后从直方图中我们可以看出,NP和PNP纳米探针溶液在USPIO浓度≤200μg/mL时对U87‐EGFRvIII和U87两种细胞增殖无明显抑制效应,细胞毒性低。
实施例5磁性纳米对比剂体外结合实验研究
荧光显微镜及流式细胞仪检测:取对数生长期细胞U87、U87‐EGFRvIII,以4x104/孔种于6孔板,培养24h,观察细胞贴壁情况。配置终浓度为0.2mg/mL的PEG‐SPIO、PEG‐SPIO‐PEPHC1溶液,孵育2h,PBS冲洗3次,加入4%多聚甲醛1mL,室温固定15min,吸弃,PBS冲洗3次,加入DAPI染色液5min,吸弃,PBS冲洗3次,荧光显微镜观察。流式细胞仪检测步骤基本同上,不同之处在于细胞与纳米对比剂孵育完后,需将细胞消化下来收集,然后用流式细胞仪分析。
靶向纳米磁共振对比剂对EGFRvIII突变体特异性细胞的体外结合能力如图5所示,由图5A可知,阴性对照组细胞与PNP及NP纳米探针共孵育后,未见明显绿色荧光或荧光很弱,与空白组细胞无明显差异。阳性实验组细胞与PNP及NP纳米探针共孵育后,与NP共孵育的细胞表面未显示明显的绿色荧光,而与靶向纳米探针PNP结合的阳性细胞膜表面可见明显的绿色荧光,与空白组相比较,有明显的差异。由图5B、C可知,阴性对照组细胞与PNP及NP纳米探针共孵育后,荧光峰位置未见明显右移,与空白组细胞比较无明显差异。由图5D、E可知,阳性实验组细胞与PNP及NP纳米探针共孵育后,与NP共孵育的细胞其荧光峰轻度右移,而与靶向纳米探针PNP结合的阳性细胞可见荧光峰明显右移,通过定量分析其荧光强度值,与空白组的荧光强度相比,有明显的统计学差异。
实施例6磁性纳米对比剂体内实验研究
立体定位建立裸鼠原位胶质母细胞瘤模型,将裸鼠随机分为2组,标记为PEG‐SPIO组和PEG‐SPIO‐PEPHC1组,尾静脉注射PEG‐SPIO或PEG‐SPIO‐PEPHC1磁性纳米对比剂,磁共振分别于注射药物8h和24h后观察纳米粒在肿瘤部位的蓄积,小鼠活体成像仪分别于注射药物2h、4h、8h和24h后观察纳米粒在小鼠体内的分布,如图6、7所示,随着时间的延长,纳米粒在肿瘤部位的蓄积逐渐增加,信号强度及荧光强度越来越强。所有裸鼠经生理盐水及4%多聚甲醛心脏灌流,取脑及主要脏器(心、肝、脾、肺、肾)行荧光拍摄,可见纳米磁共振对比剂在肿瘤部位明显蓄积,此外,该纳米磁共振对比剂在肝、脾等网状细胞分布的器官中分布较多。
实施例7磁性纳米对比剂体内安全性实验
将实施例5中的两组裸鼠随机各取一只离体主要器官(心、肝、脾、肺、肾),做石蜡组织切片后H&E染色,光学显微镜拍照如图8所示,可见两组间各内脏无明显差异,都没有明显病变、坏死等,该结果表明PEG‐SPIO和PEG‐SPIO‐PEPHC1对裸鼠内脏无明显毒副作用。
以上对本发明的具体实施例进行了描述。需要理解的是,本发明并不局限于上述特定实施方式,本领域技术人员可以在权利要求的范围内做出各种变形或修改,这并不影响本发明的实质内容。
Claims (7)
1.一种针对脑胶质母细胞瘤靶向纳米磁共振对比剂,其特征在于,所述纳米磁共振对比剂包括超小型超顺磁性氧化铁粒子、短肽PEPHC1、免疫荧光标记物及聚乙二醇;所述免疫荧光标记物以共价连接的方式标记在所述超小型超顺磁性氧化铁粒子表面,所述短肽PEPHC1通过共价连接方式与所述超小型超顺磁性氧化铁粒子表面的聚乙二醇相连;
所述的针对脑胶质母细胞瘤靶向纳米磁共振对比剂是通过包括以下步骤的方法制备而得:
S1、将超小超顺磁性氧化铁粒子、磷脂酰乙醇胺-聚乙二醇、磷脂酰乙醇胺-聚乙二醇-活性酯、免疫荧光标记物按重量比(4-8):(20-40):2:1混合,溶于二氯甲烷,超声,旋蒸去除二氯甲烷,水分散,获得水分散纳米粒;
S2、向所述水分散纳米粒加入巯基活化的短肽PEPHC1,超声,10000-12000rpm、4℃离心,弃上清,用双蒸水分散沉淀,即得所述靶向纳米磁共振对比剂;
所述磷脂酰乙醇胺-聚乙二醇-活性酯为DSPE-PEG-NHS;
所述免疫荧光标记物包括香豆素6和/或Cy7.5。
2.如权利要求1所述的针对脑胶质母细胞瘤靶向纳米磁共振对比剂,其特征在于,所述超小超顺磁性氧化铁粒子表面连接有活性基团,所述活性基团包括羧基、氨基、巯基、生物素或亲和素。
3.如权利要求1或2所述的针对脑胶质母细胞瘤靶向纳米磁共振对比剂,其特征在于,所述超小超顺磁性氧化铁粒子的直径在50nm以下。
4.如权利要求1所述的针对脑胶质母细胞瘤靶向纳米磁共振对比剂,其特征在于,所述聚乙二醇的平均分子量为2000。
5.如权利要求1所述的针对脑胶质母细胞瘤靶向纳米磁共振对比剂,其特征在于,步骤S2中,所述水分散纳米粒中的超小超顺磁性氧化铁粒子与所述巯基活化的短肽PEPHC1的重量比为(1-5):(1-5)。
6.一种如权利要求1所述的针对脑胶质母细胞瘤靶向纳米磁共振对比剂在制备特异性显示高表达EGFRvIII突变体胶质瘤的诊断试剂中的用途。
7.如权利要求6所述的用途,其特征在于,所述诊断试剂包括胶质瘤病灶的早期MRI分子影像诊断试剂。
