Abstract
本发明公开了ε‑聚赖氨酸联合抗真菌药物在制备抗真菌产品中的应用以及该抗真菌产品,属于医药技术领域。本研究发现ε‑聚赖氨酸与伊曲康唑或卡泊芬净联用具有协同杀灭致病菌烟曲霉常见临床菌株的作用,ε‑聚赖氨酸与伊曲康唑联用时的最低抑菌浓度为1mg/ml与1μg/ml,ε‑聚赖氨酸与卡泊芬净联用时的最低抑菌浓度为1mg/ml与0.094μg/ml,ε‑聚赖氨酸作为抗真菌药伊曲康唑和卡泊芬净的协同增效杀菌剂,可降低多用药剂量减轻药物的毒副作用,有效治疗临床深部烟曲霉感染以及控制耐药菌株,具有重要的临床应用价值。

技术领域
本发明涉及ε-聚赖氨酸联合抗真菌药物的应用及其产品,属于医药技术领域。
背景技术
近年来,在医学界由于广谱抗生素、抗肿瘤药物、肾上腺皮质激素、免疫抑制剂等的广泛应用甚至滥用,随着器官移植和外科其他介入性治疗的不断开展,导致菌群的失调以致机体对真菌的抵抗力降低,侵袭性曲霉病(invasive aspergillosis,IA)感染的发生呈现上升趋势,仅次于假丝酵母(Candida)引起的感染,病死率达60%-100%,成为白血病和骨髓移植等重症免疫受损患者死亡的主要原因。在所有曲霉感染中,以烟曲霉(Aspergillus fumigatus)引起的最常见,所以烟曲霉是引起大部分侵袭性曲霉病的主要病原菌。
目前治疗深部曲霉感染的临床应用最广泛、研究最热门的一类药物是三唑类抗真菌药物,主要有伏立康唑(Voriconazole)、伊曲康唑(Itraconazole)等,但均有毒副作用,且由于唑类药物在医学以及农业上的大量应用,已经发现越来越多的临床分离菌对唑类药物产生耐药性,其特点是除了对某种特定药物的耐药以外,还有高比例的菌株出现多药耐药性(MDR)。目前,新型抗真菌药物棘白菌素类(echinocandins)中的卡泊芬净Caspofunginacetate(CAS)正在被推荐为侵袭性曲霉感染的挽救疗法。但是,随着卡泊芬净的应用,临床上已经出现少量耐药念珠菌和烟曲霉菌株。因此,烟曲霉对于唑类药物的抗药性是人类健康越来越严重的一大威胁。尤其是频繁的城市改建和人口的大量流动都加剧了世界公共卫生面临的这一问题的严重性。
抗菌肽又称宿主防御肽,指生物体内经诱导而产生的一类具有先天免疫相关的有抗菌活性小分子多肽物质,最早是在昆虫中发现,随后在各种生物体都有发现,包括细菌、植物、动物甚至人体中都有。1977年日本学者S.Shima和H.Sakai在从微生物中筛选发现一株放线菌能产生大量而稳定一种含有25-30个赖氨酸残基的同型单体聚合物,称为ε-聚赖氨酸(ε-PL)。ε-聚赖氨酸是一种具有抑菌功效的多肽,这种生物防腐剂在80年代就首次应用于食品防腐,安全性高于其他化学防腐剂。
发明内容
本发明的目的是解决现有技术的不足,提供一种ε-聚赖氨酸联合抗真菌药物的应用及其产品,使在提高抗真菌疗效的同时还能减轻药物的毒副作用,并且可有效治疗临床深部烟曲霉感染以及控制耐药菌株。
技术方案
一种ε-聚赖氨酸联合抗真菌药物在制备抗真菌产品中的应用。
所述抗真菌药物为伊曲康唑或卡泊芬净。
所述ε-聚赖氨酸与伊曲康唑联用时的最低抑菌浓度为1mg/ml与1μg/ml。
所述ε-聚赖氨酸与卡泊芬净联用时的最低抑菌浓度为1mg/ml与0.094μg/ml。
所述真菌为烟曲霉。

有益效果:本研究发现了ε-PL与伊曲康唑或卡泊芬净联用具有协同杀灭致病菌烟曲霉常见临床菌株的作用,并且同时具有体外和体内协同杀菌作用,ε-PL作为抗真菌药伊曲康唑和卡泊芬净的协同增效杀菌剂,可降低多用药剂量减轻药物的毒副作用,有效治疗临床深部烟曲霉感染以及控制耐药菌株,具有重要的临床应用价值。

具体实施方式
下面结合附图和具体实施例对本发明作进一步说明。
实施例1
ε-多聚赖氨酸(ε-PL)与伊曲康唑联(ITC)用对抗烟曲霉的效果测定:
实验材料:
ε-PL(sigma),DMSO和ITC(sigma),E-test药敏条(biomerieux)。本研究用到的试剂均为粉末,纯度均>99%。ε-PL用无菌水配制成2g/ml储存液;ITC用DMSO溶解配制成2mg/ml储存液。用时根据需要配制成所需浓度的工作液。
实验方法:
1.微量液基稀释法:菌株抗菌药物最小抑菌浓度(MIC)测定:按照美国国家临床实验室标准化委员会(clinical and laboratory standards institute,CLSI)推荐的M38-A2微量液基稀释法,测定标准株AF1160对ITC的敏感性,每孔中孢子接种量为0.4×104~5×104CFU/ml;抗真菌药物工作浓度:ITC为0.03~16μg/ml。96孔板在35℃下孵育48h后读取结果。ITC其MIC值判定为100%抑制生长时的最低药物浓度。
(1)菌悬液制备:受试菌株在培养基YUU上于37℃培养2天,收孢子。用无菌生理盐水重悬,调整OD值至0.09~0.3(约0.4×106~5×106/ml)。将上述悬液用RPMI1640液体培养基进行1:50稀释,即得到工作液(约2倍于终浓度0.4×104~5×104/ml)。
(2)加样:将ITC药物储存液和ε-PL储存液以RPMI1640液基倍比稀释后混合到一起,配制成2倍于终浓度的药液。取100μl药液分别加入96孔圆底药敏板第1~10列各孔,第1孔为最高浓度,第10孔为最低浓度,第11列中加入100μl RPMI1640作为生长对照。第12列仅加入200μl RPMI1640培养基作阴性对照。向第1~11孔分别加入100μl菌悬液后,35℃孵育48h判读结果。
(3)重复:复孔3~5个,求均值。先后重复三次实验,同样得均值。将3次均值再求均值及标准差方差分析。
2.E-test实验法:使用YUU培养基作为基底,取100μl 106个孢子/ml测试菌的孢子悬液、10μlε-PL储存液和20ml融化了的培养基混匀。略干,然后用灭菌的镊子夹取ITC E-test试条(从上到下药物浓度一次降低),轻轻贴于平板中间,注意不要有气泡。37℃培养48h。
实验结果:
1.微量液基稀释法:ITC其MIC值判定为100%抑制生长时的最低药物浓度。本实施例的实验结果为ITC单独作用MIC值为2μg/ml,联用1mg/ml的ε-PL时MIC值为1μg/ml。
2.E-test实验法:ITC其MIC值判定为抑菌圈边缘与E-test药敏条相交处为MIC值。本实施例的实验结果为ITC独作用MIC值为2μg/ml,联用1mg/ml的ε-PL时MIC值为1μg/ml。
实施例2
ε-多聚赖氨酸(ε-PL)与卡泊芬净(CS)联用对抗烟曲霉的效果测定:
实验材料:
ε-PL(sigma),CS(南京森贝伽sbjbio),E-test药敏条(biomerieux)。本研究用到的试剂均为粉末,纯度均>99%。ε-PL用无菌水配制成2g/ml储存液;CS用无菌水配制成2mg/ml储存液。用时根据需要配成所需浓度的工作液。
实验方法:
1.微量液基稀释法:菌株抗菌药物最低有效浓度(MEC)测定:按照美国国家临床实验室标准化委员会(clinical and laboratory standards institute,CLSI)推荐的M38-A2微量液基稀释法,测定标准株AF1160对CS的敏感性,每孔中孢子接种量为0.4×104~5×104CFU/ml;抗真菌药物工作浓度:CS为0.03~16μg/ml。96孔板在35℃下孵育48h后读取结果。CS其MEC值判定为抑制生长为小而圆形紧缩的菌丝体时的最低药物浓度。

(2)加样:将CS药物储存液和ε-PL储存液以RPMI1640液基倍比稀释后混合到一起,配制成2倍于终浓度的药液。取100μl药液分别加入96孔圆底药敏板第1~10列各孔,第1孔为最高浓度,第10孔为最低浓度,第11列中加入100μl RPMI1640作为生长对照。第12列仅加入200μl RPMI1640培养基作阴性对照。向第1~11孔分别加入100μl菌悬液后,35℃孵育48h判读结果。
(3)重复:复孔3~5个,求均值。先后重复三次实验,同样得均值。将3次均值再求均值及标准差方差分析。
2.E-test实验法:使用YUU培养基作为基底,取100μl 106个孢子/ml测试菌的孢子悬液、10μlε-PL储存液和20ml融化了的培养基混匀。略干,然后用灭菌的镊子夹取CS E-test试条(从上到下药物浓度一次降低),轻轻贴于平板中间,注意不要有气泡。37℃培养48h。
实验结果:
1.微量液基稀释法:CS其MEC值判定为出现小而圆形紧缩的菌丝体生长时的最低药物浓度。本实施例的实验结果为CS单独作用MEC值为4μg/ml,联用1mg/ml的ε-PL时MEC值为1μg/ml。
2.E-test实验法:CS其MIC值判定为抑菌圈边缘与E-test药敏条相交处为MIC值。本实施例的实验结果为CS单独作用MIC值为0.38μg/ml,联用1mg/ml的ε-PL时MIC值为0.094μg/ml。
实施例3
ε-多聚赖氨酸(ε-PL)与伏立康唑(VRC)联用对抗烟曲霉的效果测定:
实验材料:
ε-PL(sigma),DMSO和VRC(sigma),E-test药敏条(biomerieux)。本研究用到的试剂均为粉末,纯度均>99%。ε-PL用无菌水配制成2g/ml储存液;VRC用DMSO配制成2mg/ml储存液。用时根据需要配制成所需浓度工作液。
实验方法:
1.微量液基稀释法:菌株抗菌药物最小抑菌浓度(MIC)测定:按照美国国家临床实验室标准化委员会(clinical and laboratory standards institute,CLSI)推荐的M38-A2微量液基稀释法,测定标准株AF1160对VRC的敏感性,每孔中孢子接种量为0.4×104~5×104CFU/ml;抗真菌药物工作浓度:VRC为0.03~16μg/ml。96孔板在35℃下孵育48h后读取结果。VRC其MIC值判定为100%抑制生长时的最低药物浓度。
(1)菌悬液制备:受试菌株在培养基YUU上于37℃培养2天,收孢子。用无菌生理盐水重悬,调整OD值至0.09~0.3(约0.4×106~5×106/ml)。将上述悬液用RPMI1640液体培养基进行1:50稀释,即得到工作液(约2倍于终浓度0.4×104~5×104/ml)。
(2)加样:将VRC药物储存液和多聚赖氨酸储存液以RPMI1640液基倍比稀释后混合到一起,配制成2倍于终浓度的药液。取100μl药液分别加入96孔圆底药敏板第1~10列各孔,第1孔为最高浓度,第10孔为最低浓度,第11列中加入100μl RPMI1640作为生长对照。第12列仅加入200μl RPMI1640培养基作阴性对照。向第1~11孔分别加入100μl菌悬液后,35℃孵育48h判读结果。
(3)重复:复孔3~5个,求均值。先后重复三次实验,同样得均值。将3次均值再求均值及标准差方差分析。

2.E-test实验法:VRC其MIC值判定为抑菌圈边缘与E-test药敏条相交处为MIC值。本实验结果为VRC单独作用MIC值为0.125μg/ml,联用1mg/ml的ε-PL时MIC值为0.064μg/ml。
实施例4
动物实验
实验材料:大蜡螟幼虫(天津惠裕德生物科技),ε-PL(sigma),CS(南京森贝伽sbjbio)
实验方法:
受试菌株AF1160在培养基YUU上于37℃培养2天,收孢子。用无菌生理盐水重悬,调整孢子浓度到1×108/ml。大蜡螟幼虫共4组,每组15只。1组为非感染对照组,注射PBS 5μl,另外3组作为感染组,用液相上样注射器取5μl孢子悬液通过大蜡螟幼虫的腹足位置注射到体腔中,其中1组作为感染对照,之后对另外2组分别注射不同的药物及其组合:CS 3μl;CS+ε-PL 3μl+3μl(体内药物浓度:CS:100mg/kg;CS+ε-PL:100mg/kg+2500mg/kg)。注射后的大蜡螟37℃培养并观察存活的幼虫数量,排除变黑不动的死亡虫子。
实验结果:
大蜡螟动物模型体内实验结果见图1,根据每10小时记录的存活幼虫的数量计算各组大蜡螟幼虫的存活率,PBS非感染组存活率:100%;AF1160菌株感染组存活率:20%;AF1160菌株感染单独CS治疗组存活率:33.3%;AF1160菌株感染CS联用ε-PL治疗组存活率:53.3%。这说明ε-PL和CS联用在以大蜡螟幼虫为模式动物的体内具有协同治疗作用。
Claims (2)
1.一种ε-聚赖氨酸联合抗真菌药物在制备抗真菌产品中的应用,其特征在于,所述真菌为烟曲霉,所述抗真菌药物为伊曲康唑或卡泊芬净;
所述ε-聚赖氨酸与伊曲康唑联用时的最低抑菌浓度为1mg/ml与1μg/ml;
所述ε-聚赖氨酸与卡泊芬净联用时的最低抑菌浓度为1mg/ml与0.094μg/ml。
2.一种抗真菌产品,其特征在于,所述真菌为烟曲霉,所述抗真菌产品以ε-聚赖氨酸联合伊曲康唑或卡泊芬净为主要活性成分;所述抗真菌产品中,ε-聚赖氨酸联合伊曲康唑时,ε-聚赖氨酸的最低浓度为1mg/ml,伊曲康唑最低浓度为1μg/ml;ε-聚赖氨酸联合卡泊芬净时,ε-聚赖氨酸的最低浓度为1mg/ml,卡泊芬净的最低浓度0.094μg/ml。
