背景技术
口腔癌(包括嘴唇、舌部、咽喉和口腔)在所有的癌症形式中排列第12位。尽管原发恶性肿瘤当前在治疗上有所进展,但由于其确诊时通常已是晚期,因而仍然是一种预后不佳的癌症。白光检查和触诊通常用于定位活体检视部位。在进行了放射性胶体和蓝色染料的间质注射后,术中淋巴造影已被确认和广泛接受为常规的手术程序,其主要用于乳腺癌、皮肤黑色素瘤以及较小范围的头颈癌。

金丝桃素,一种羟基化二苯基二萘嵌苯喹啉(phenantroperylenequinone),已被考虑用来进行荧光诊断。已评价了这种染料在异常细胞特别是癌性细胞中较高的上染率对于提高膀胱癌检出率(D'Hallewin M-A.et al.,"Hypericin-based fluorescencediagnosis of bladder carcinoma",BJU International 2002;89:760-3)和恶性口腔损害检出率(Thong P.S.P.et al.,"Clinical application of fluorescence endoscopicimaging using hypericin for the diagnosis of human oral cavity lesions",Br JCancer 2009;101:1580-4)的可能性。在后来的研究中,诊断标准是基于红蓝强度比,在非恶性损害中的红蓝强度比大于恶性损害中的红蓝强度比。
甲苯胺蓝,一种来自噻嗪族的公知异染性染料,现已研究了其在口腔宽场检查中的可能性,通过某种局部施用对恶性和恶变前的损害进行染色:根据爱泼斯坦(Epstein)所述,利用这种处理方法,具有较高风险发展成癌症的恶变前损害会被优先着色。利用这种活体染料进行宏观检查时,主要的假阳性结果通常与炎症或创伤区域有关。因此,为了消除这些因素,强烈推荐在两星期后进行复查(Epstein J.B.et al.,2009)。一种现有的元分析法被用来评价甲苯胺蓝在口腔癌筛查中的有效性。结果表明,在高风险人群中,该方案的灵敏度范围在93.5%至97.5%之间,特异性范围在73.3%至92.9%之间。
为了突出用接触式内镜检查术进行检查的细胞结构,已考虑用亚甲基蓝(一种与甲苯胺蓝密切相关的噻嗪染料)对口部和喉部上皮的表面层进行染色。可对核质比例以及细胞核颜色、大小和形状进行原位分析,然而尽管其具有作为诊断工具的潜在价值,但是近年来几乎没有什么研究描述这种技术的应用。
里亚兹(Riaz)提出了,在口腔中进行了局部施用后,可利用亚甲基蓝优先染色癌症前期和癌性损害的能力(Riaz et al.,"Methylene blue as an early diagnosticmarker for oral precancer and cancer",SpringerPlus 2013,2:95)。一项对120名病人的试验性研究表明,这种宏观方法的灵敏度为91.4%,特异性为66.6%,且阳性预测值为97.7%,阴性预测值为33%。出现假阳性的原因可能在于炎症性和非正常损害。这一方法已被推荐用于高风险人群的大规模口部筛查。
最后,专利蓝V(一种来自三芳基甲烷族染料的活体蓝色染料)在临床实践中被主要用来进行前哨淋巴结造影。在欧洲主要使用专利蓝。其对应的异构体,异硫蓝,已在美国被食品药品监督管理局批准。在进行间质注射后,两种染料的较低浓度就足以宏观地显现出染成蓝色的淋巴结及其输入淋巴管。在各种临床研究中已用钠荧光素和吲哚氰绿描述了用于前哨淋巴结检测的宏观荧光(Dan A.G.et al.,"1%Lymphazurin vs 10%Fluorescein for Sentinel Node Mapping in Colorectal Tumors",Arch Surg 2004;139:1 180-4)。
甲苯胺蓝、亚甲基蓝、专利蓝、吲哚氰绿和金丝桃素是当前在临床实践或临床试验中用于诊断程序宏观指导的染料,且它们对病人的副作用极小(Narui K.et al.,"Observational study of blue dye-assisted four-node sampling for axillarystaging in early breast cancer",Eur J Surg Oncol 2010;36:731-6)。
在本公开中,为了表征正常和癌性头颈组织,以及在细胞级上进行淋巴结成像,我们首次评估了单独或组合使用红荧光染料金丝桃素,以及远红甲苯胺蓝(TB)、专利蓝(PB)染色剂和红外吲哚氰绿(ICG)染色剂的荧光属性。对人类手术样品和大鼠淋巴结进行高分辨率荧光成像是利用膝上型共焦激光扫描显微镜(CLSM)、非线性(多光子)显微镜和基于探头的共焦激光内镜(pCLE)实现的。然后,我们将对应的共焦及非线性图像与利用标准的组织病理学方法获得的结果进行了比较。此外,还讨论了一种可能的结合共焦显微术、反射和荧光信息的多模式方案,以便改善活体检视及前哨淋巴结部位的定位。
发明内容
在至少一个方面,本文公开的实施例涉及观察生物组织形态的方法,其包括:
-在所述生物组织上使用荧光染料,其中所述荧光染料选自专利蓝V、异硫蓝、甲苯胺蓝、金丝桃素、吲哚氰绿、MVAC、或亚德里亚霉素;

申请人在本公开中示出了,上述染料的未知属性,或者未知荧光属性(例如:专利蓝V、异硫蓝),或者在细胞级揭示对比图像的未知能力(例如:金丝桃素、MVAC、亚德里亚霉素、吲哚氰绿)。
更具体地,本公开示出了专利蓝V、用于原位加亮前哨淋巴结细胞结构的吲哚氰绿、以及更广泛地,所有荧光染料;金丝桃素、甲苯胺蓝、专利蓝V、吲哚氰绿,在局部施用后,提供了有关恶性头颈损害的形态信息。
MVAC和亚德里亚霉素用于在细胞级进行“治疗成像”,允许精确施用和监视药物的体内分解,以及更好地理解这一方案的体内分子治疗效能。
根据一实施例,所述方法包括通过按染料浓度引入荧光染料,以满足特定的细胞检查条件,其中所述染料浓度显著低于传统用于通行临床用途的染料浓度。在这些新条件下,获得一种可接受的对比,其允许病理学家对组织执行细胞级的形态记录,这种记录允许识别关键参数和细胞结构,以支持诊断。

金丝桃素购自Invitrogen(塞吉蓬图瓦兹,法国);MB购自Aguettant实验室(MB1%注射,里昂,法国);甲苯胺蓝(TB)购自Fluka(Sigma Aldrich,圣康坦法拉维耶法国);专利蓝(PB)购自Guerbet(钠专利蓝V2.5%注射,维勒潘特,法国);盐酸吖啶黄素(ACF)购自Sigma Aldrich(圣康坦法拉维耶,法国);吲哚氰绿25mg(ICG)购自SERB Laboratoires(巴黎,法国),乙酸由Gyneas实验室提供(乙酸5%,古桑维尔,法国)。
人类样本
因鳞状细胞癌(HNSCC)在古斯塔夫鲁西研究院(维勒瑞夫,法国)经历了整个喉(全喉)切除术或者口部和上颌面外科手术的共25名病人被计入预期原始记录中(从2009年1月至2011年4月)。机构审查委员会批准了该项研究,而且所有病人均知情同意。在荧光成像之前,30份新收集的最大表面面积为1cm3的组织在临床上由病理学家判别为非肿瘤黏膜和肿块组织。
动物淋巴结
10只重量为350g的雄性威斯塔大鼠,购自詹维尔。根据保护用于试验及其他科学目的的脊椎动物的欧洲公约,关于动物试验规定的欧洲规约和法国法律,进行了动物关爱和研究。所有的实验规程均获得位于古斯塔夫鲁西研究院的实验动物关爱和使用委员会的批准。这些大鼠被饲养在隔离器中的标准鼠笼中,舍养的明/暗周期为12小时,温度22-24℃,湿度50%,且这些大鼠被给与了不限量的食物和水。
人类手术样品的制备
一般性的样品预处理
头颈黏膜经常出现角质化。由于这些角质层会阻碍染料和光透入组织中,因而可通过剥离胶带(3M手术胶粘带)来减少样品鲁米那(luminal)表面处的角质层。
金丝桃素染色
新鲜的手术样品用10%的乙酰半胱氨酸处理2min,用新鲜8μm金丝桃素溶液在37℃培养30min。
甲苯胺蓝染色
用1%乙酸清洗新鲜样品,用TB 1%(w/v)短暂染色。为了对荧光染料散布到组织中进行优化,需要一等待时间(高达5min),该等待时间过后,在1%乙酸中清洗样品。
亚甲基蓝染色
用10%乙酰半胱胺酸来处理手术样品2min,用0.25%MB来短暂载入。在一段等待时间(高达5min)后,在0.9%的氯化钠溶液中对样品进行清洗。
专利蓝染色
用10%乙酰半胱氨酸来处理手术样品2min,然后浸入0.025%专利蓝溶液中10s,最后在0.9%氯化钠溶液中进行清洗。
吲哚氰绿染色
用10%乙酰半胱氨酸处理手术样品2min,然后用0.25mg/ml ICG溶液对其进行局部染色10s,最后在0.9%氯化钠溶液中清洗。
组织的多重染色
在少量实例中,用0.01%(w/v)的盐酸吖啶黄素对样品进行额外染色45s,然后在0.9%的氯化钠溶液中清洗1min,从而使得,出于得到额外形态信息的目的,能够讨论甲苯胺蓝-吖啶黄以及亚甲基蓝-吖啶黄可能的组合。
淋巴结的制备
通过让动物吸入异氟烷(2.5%用于诱发,1.5%用于维持)来使其短暂麻醉。然后,将100μL的1.25%专利蓝、或者1%甲苯胺蓝、或者0.25%亚甲基蓝通过皮下施用注射入动物的后爪,允许其朝淋巴结扩散5min。通过静脉注射戊巴比妥来对大鼠进行无痛致死术,并且可对具有宏观性蓝色外观的腿弯部和/或腹股沟淋巴结进行解剖。然后,通过共焦激光扫描显微术和基于探头的共焦激光显微内镜(pCLE)来对体外淋巴结进行成像。
共焦激光扫描显微术(CLSM)
我们利用DMI400倒置显微镜(莱卡TCS SPE,马内姆,德国)进行荧光成像。头颈组织和淋巴结被置于35mmμ-碟(Ibidi,生物谷,法国)中。利用以下激光谱线来评定组织的形态和构造:盐酸吖啶黄素(激发波长λex=488nm;发射波长λem=500-600nm);金丝桃素(激发波长λex=532nm;发射波长λem=550-650nm);TB、MB和PB(λex=635nm;λem=650-800nm)。利用对应1.1mm x 1.1mm视场的10x物镜(干/NA 0.3)、或者带550μm x 550μm视场的20x物镜(干/N 0.7)、或者带275μm x 275μm视场的40x物镜(油/NA 1.25)来形成这些图像。为了对不同组织层的细胞分布和形态进行成像,用5μm梯级来产生Z系列。
非线性激光扫描显微术(多光子)
我们利用SP8倒置多光子显微镜(莱卡TCS SPE,马内姆,德国)来进行荧光成像。头颈组织和淋巴结被置于35mmμ-碟(Ibidi,生物谷,法国)中。利用830nm激光谱线,以830nm脉冲宽度100fs和80Mhz的重复率来评定组织的形态和构造。通过带通滤波器580-680nm来收集荧光。利用对应550μm x 550μm视场的20x物镜(干/NA 0.75)来获得图像。为了对不同组织层的细胞分布和形态进行成像,用5μm梯级来产生Z系列。
基于探头的共焦激光内镜(pCLE)
由于CLSM和多光子显微镜是桌上型仪器,并不适于用来进行体内探测,因此要利用光纤成像系统,由Mauna Kea Technologies(巴黎,法国)友好提供的来辅助记录荧光图像,该光纤成像系统已被成功测试用于胃肠、泌尿和肺道的临床研究。pCLE包括连接到激光器件的柔性光纤微探头,其具备工作于660nm(用于TB、MB和PB激发)或568nm(用于金丝桃素激发)的激光二极管、快速扫描激光器(帧速率为8至12图每秒)、和检测荧光标志的光电二极管。不同于传统组织病理学,从样品获得的“光学切片”并不是横向的,而是面对组织表面部分。利用共焦微探头胃挠(gastroflex)UHD(Mauna Kea technologies,巴黎,法国)来获取数据,该UHD具有240μm直径的视场。这一由30000根光纤构成的光纤束具有1μm的横向分辩率和10μm的轴向分辨率。这些图像是在皮肤表面之下55μm至65μm之间变化的深度处获取的。利用视频拼接软件来进行图象重构,以通过产生较大静态图象来更完整地表现出给定研究区(图1)。还对来自各种解剖部位的癌瘤和非肿瘤黏膜记录了组织自发体荧光控制图像。
组织病理学
先前用pCLE和CLSM分析的样品对于头颈组织的实例是被置于甲醛中,或者对于淋巴结是被置于Finefix(一种体外固定处理剂)中,然后植入石蜡中,垂直于表面黏膜以3μm厚度分段,并用苏木精和曙红-番红花色素(HES)染色。头颈组织的组织学检查和荧光图像由专门研究头颈病理学的病理学家判读和解释。
结果
用甲苯胺蓝进行头颈组织成像
在TB染色后可直接对样品进行有效的荧光成像。染料载入后,观察表面(胃挠UHD的焦平面)下方60μm的最佳散布15min。
在来自化生变化的鳞状上皮假声带样品中,用双系统来观察这些细胞的细胞核中明亮的荧光和细胞质内的散布染色(图2a和2b)。可观察到细胞核大小与形状上的细微非均匀性,这种非均匀性与无规律的细胞分布有关。仅在上皮表面下范围从0至65μm的深度处收集图像,这是因为染料散布的范围有限,而且pCLE焦平面被限制在55μm至65μm之间的范围中。通过HES切片检查,病理学家可确定高等级的发育异常。对于高度角质化的样品,有些较深区域并不可见,这可能是由于染料散布受限的缘故导致的,因为黏膜表面上残余有角质且其造成了上皮厚度增加。
图2d和2e示出了原位乳头状癌瘤。用双系统来观察典型的微观乳头状构造。可辨别出异常的细胞分布和非典型性。面对地呈现的乳头状结构为环状,因为是垂直于它们的轴线对其进行成像的。细胞周边出现强烈的环形染色可能要归因于乳头状结构表面处的角质层较薄。有趣的是,利用CLSM,在有些肿瘤细胞(图2d,黄色箭头)的细胞核中可辨别出染色质(图2d,白色箭头)和有丝分裂。
分化良好的HNSCC的图像揭示了在定向基质细胞(黄星)周围细胞密度高的非均匀细胞分布(图2g和2h)。在患癌细胞的情况中,可注意到无分化的细胞核和细胞质染色,因为它们的核质比例较高。基质中的周围定向细胞(纤维原细胞和成肌纤维细胞)可能不同于患癌细胞(图2d和2e)。通过局部施用甲苯胺蓝不能识别出毛细管网。双共焦成像系统产生的所有TB图像由病理学家解读。由于CLSM具有在不同深度生成一系列图像的能力,使得在浅表位面中可更加容易地辨析细胞和亚细胞细节。这是由于组织表面与深度间存在TB荧光梯度造成的。
用亚甲基蓝进行头颈组织成像
MB染色后立即对样品进行成像。我们观察到,在2小时的时段内,由于染料从组织中渗漏,使得荧光强度和对比图像逐渐下降。
对非肿瘤样品,诸如来自舌部侧缘的鳞状上皮(图3a和3b),我们用双成像系统观察到了细胞质中散布的染色以及细胞核中较高的荧光强度。可以预期的是,可观察到规则的细胞核分布,而且利用基于传统组织学的表层分析证实了具有较低核质比例的细胞(图3c)。
对于HNSCC样品,在MB载入后,容易从基质中辨别出患癌细胞,它们非均匀地排列在径列条(图3d和3e,这里是一种中度分化的HNSCC)或叶突中。还注意到了细胞密度出现增加。尽管我们对缺少用pCLE进行对比感到遗憾,但还是认识到了细胞学的非典型性。利用CLSM,还可从显示为纺锤形细胞核的纤维原细胞中辨别出带有小圆形细胞核(图3d,白色椭圆)的炎症细胞。而且,对于来自外阴浸润癌瘤的手术样品,我们能用双系统清楚地观察到渗透到肌肉中的高密度癌性细胞(白色箭头)。
用专利蓝进行头颈组织成像
PB染色后立即对样品进行成像。由于蓝色染料渗漏,因而可观察到对比图像的质量会出现迅速衰退(在1至2小时的时间内)。图4提供了鳞状上皮(4a,4b)、组织化生管(4d,4e)和HNSCC(4g,4h)的高质量荧光图像,可将这些图像与相应的传统组织病理学图像(4c,4f和4i)进行比较。非病理性组织的图像示出了整个细胞质中的PB染料分布。对于癌性样品,容易辨别出非均匀的细胞形态,其与带有强烈细胞核染色的多径列条有关。增大的细胞密度也是容易注意到的。
用金丝桃素进行头颈组织成像
将样品浸入37℃金丝桃素溶液中30min后,对其进行成像。可辨别出细胞质和核膜,它们与细胞质中染色的散布荧光团有关。在肿块组织例如中度分化鳞状细胞癌(图5)的情况中,在荧光下壳观察到非均匀的细胞分布。
用吲哚菁绿进行头颈组织成像
ICG染色后立即对样品进行成像。由于在激发光下ICG会褪色(beaching),因而可看到荧光出现轻微减弱,但在成像过程中,并没有观察到染料渗漏。图10利用整个细胞质中分布的ICG荧光示出了HNSCC的高质量荧光图像。对于两个CLS(A)和pCLE(B)图像,在这一高度分化的癌瘤上,细胞形态是容易辨别的,组织构造的分裂也能被很好地观察。
头颈组织的多光谱和多模式共焦成像
我们评价了一种用多参数分析来改善HNSCC样品图像质量的可能性,因为不可能通过上文所述染料中的仅仅一种染料对组织的全部详细结构进行染色。开始时,在进行了405nm激光激发后,将组织样品自发体荧光与TB或MB染色结合。首先用TB对HNSCC样品进行上料,加亮了由微弱荧光组织界定的无规律径列条(图7A)。当额外地记录了自发体荧光标志时,病理学家可有效地识别出主要的纤维变性,这与相应的H和E部分有关。在第二个例子中,可容易地识别出用MB染色的分化良好HNSCC的癌瘤叶突。基质的表征通过基于自发体荧光标志的富弹性蛋白母质的图像补充(图7b)。在这一样品中,癌珠也用MB染色(黄色箭头)。在所有的图像中,自发体荧光提供的额外信息使得可通过H和E部分让其更接近肿块组织的惯用表示法。
在第二种方案中,用甲苯胺蓝-吖啶黄或者亚甲基蓝-吖啶黄对手术样品进行双重染色。吖啶黄(ACF)是一种绿色荧光染料,其添入DNA座子中,强烈地对细胞核进行染色。图6c示出了载入甲苯胺蓝-吖啶黄后的疣状鳞癌,其在混乱的构造中在癌性细胞和小细胞之间渐进分化,从而用主要的角质化和漩涡倾向来弄平细胞。用两种单一荧光染料可使角化病的处理清楚可见,但角化不全仅由ACF来揭示,因为细胞核和角质在这一区域不能单独用TB来判别(白色椭圆)。图7d给出了一个用ACF和MB染色的分化良好HNSCC的示例。除了细胞核和细胞质染色外,也用MB辨别一些细胞的边缘(白色箭头)。在肿块组织的大部分区域上,观察到了ACF和MB的共局部化。然而,我们注意到,在未载入MB的区域中,可用ACF使一些细胞核变得可见。最后,利用自发体荧光和反射作为附加参数,也对载有MB的分化良好HNSCC进行成像,以便细化病理学家的解读。反射成像利用了来自组织的反向散射光,以及与其结构有关的各种折射率。获得的多色图像从干扰构造中提供了附加数据;MB图像呈现出了可辨别的肿瘤细胞和炎症细胞(图8a)。在图8d中,炎症细胞包括淋巴细胞和巨噬细胞,淋巴细胞被看作带有圆细胞核的小细胞,而巨噬细胞为包含小细胞核的、具有各种形状的大型细胞形式(黄色箭头)。
微观大鼠淋巴结成像
在我们的可行性研究中,仅对健康的淋巴结进行成像。
在进行了PB、TB或MB的皮下注射后,仅经过5min之后,蓝色染料散布到腿弯部和/或腹股沟淋巴结是宏观上可见的。利用CLSM和pCLE对淋巴结进行体外成像(图9a、d、g和9b、e、h)。用MB、TB和PB对淋巴细胞进行均匀染色。在结包膜下,可容易地辨别出均匀分布的淋巴腺细胞。
在对ICG染色样品进行的多光子显微术中,在该模式的皮层区中可看到该细胞的细胞质中存在局部化染色。在局部施用了ICG染料后,图像上细胞核呈现黑色,这表明缺少核荧光。由于样品是非病变的无炎症淋巴结,因此观察到的细胞分布相当均匀(图10)。
用于原位加亮前哨淋巴结细胞结构的专利蓝V和ICG的荧光属性先前从来没有被描述过。这种新的方案可仅在组织上进行染料局部施用后,提供与恶性损害有关的形态信息。
讨论
在本公开中,我们提出了一种新的细胞级荧光成像方案,更具体地,其用于头颈组织的成像和淋巴结检查,尤其是前哨淋巴结的检查。
TB、MB、PB、ICG和金丝桃素的荧光特征用来在细胞和组织级形成高质量的图像,使得病理学家能够根据这些“光学活体检视”作出诊断。由这些染料提供的组织形态和构造信息被用来评价人体肿瘤和非肿瘤头颈样品以及评价动物淋巴结。
在临床实践中,癌症前期的和癌性损害对于TB和MB在宏观级别上呈现出一种优先的视觉颜色上染率(Epstein J.B.et al.,2009)。对本公开中的双染料,在鳞状上皮中发现显著的荧光标志是非常少见的预期结果。对于HNSCC样品,利用双染料可在显微镜下从均匀且规则染色的鳞状上皮中很好地辨别肿瘤细胞以及它们周围的基质。可通过诸如细胞核大小和形状上的变化(异常核变)、核质比例增大等细胞学异常性在细胞级上,以及通过对细胞和无规律的细胞结构进行分类甚至可在组织级上,精确地判别癌性损害。共焦CLSM图像还揭示了有丝分裂模式,其详细程度类似于用传统活体检视法获得的程度。有趣的是,这些红和远红荧光造影剂的局部施用还有助于对角化作用异常结构(诸如癌珠)染色。
与常常局部使用的MB和TB相反,PB已经被例行地以允许在微观级上进行荧光成像的浓度,通过组织间隙施用(或称隙间施用)(主要是瘤周注射)用于临床程序。这些染料在组织上的局部输送与得到病理学家可解读的图像有关。当利用局部输送模式时,染料到组织中的有限散布必须被考虑,因为它将限制成像深度在表面下约60μm,这与用MB和TB观察的一样。进一步地,我们可对表面黏膜进行机械剥离,以在HNSCC样品中去除大部分的角质层,因而改善微观级上可读图像的形成。用双蓝色染料(TB,PB)进行的组织标记几乎是立即发生的(小于1min),而且它们在体外样品上的裕量可适合手术程序的时间安排(大于15min)。与蓝色染料和ICG相比,用金丝桃素进行的细胞成像在染色后需要较长的时间间隔,以便提供可读的微观图像(至少30min)。然而,这一持续时间与传统的临床规程是相适合的。在局部施用后,MB、TB和金丝桃素出现的宏观假阳性通常与炎症或再生组织有关。利用红和远红荧光染料产生的所有共焦显微术或显微内镜图像的质量足以用来供病理学家进行解读。然而,单独获取这些荧光标记用于识别肿块基质病症可能存在一些限制,因为纤维变性网可能会有一些微弱的荧光出现。我们揭示了,多模式显微镜成像可用于在图像中获得额外的有用信息,从而允许改善对纤维网络和炎症细胞的识别。共焦显微术可能的反射应用被成功地查验用于诊断喉部损害和用于体内口部损害的检测。尽管在反射模式中可对未染色组织进行成像,但诱导荧光模式允许用高信噪比改善结构成像,并且揭示的特征更加接近于通过组织病理学观察的特征。
在本公开中,我们还揭示了,通过结合来自远红造影剂诱导荧光的图像与自发体荧光或反射标志,可以一定的准确度识别出所有的细胞和细胞外结构,该准确度接近于传统组织学实现的准确度。本公开中描述的多光谱成像,结合使用双荧光染色法(ACF染料和近红外荧光染料),确实地提供了附加的组织学数据,其改善了病理学家的诊断能力。
Pb被例行用于检测前哨淋巴结,其在蓝色染料注射后变成宏观性蓝色(Gill J.etal.,"Sentinel Lymph Node Biopsy in Breast Cancer:An Analysis of the MaximumNumber of Nodes Requiring Excision",The Breast J 2011;17:3-8)。临床研究也描述了间质注射MB用于局部化(Varghese P.et al.,"Methylene Blue Dye-A Safe andEffective Alternative for Sentinel Lymph Node Localization",The Breast J2008;14:61-7)。然而,动物研究显示了基于MB注射后来自淋巴结的宏观红荧光标志的结果。在本公开中,我们第一次对健康的大鼠进行了实验,在注射了蓝色染料和吲哚氰绿后,利用微观级别上的荧光成像结合视觉色彩淋巴结检测的可能性。淋巴结的组织结构由病理学家辨别,这一结果提高了在前哨淋巴结切除术之前将pCLE用于淋巴结原位成像的可能性。我们还示出了利用非线性激发ICG所获得的意料之外的显著荧光。这一结果允许很好地描述大鼠淋巴结中的皮层细胞结构。
本公开中使用的所有对比试剂被批准用于人体,以及被批准使用它们的形态染色能力。这里提出的新成像技术包括那些已经被卫生当局批准的临床规程、在乳腺癌研究或黑素瘤研究中将蓝色染料用于前哨淋巴结规程、将金丝桃素用于光力治疗、将ICG用于视网膜血管造影术。
值得注意的是,我们推荐使用的标记浓度或者等于对病人施用的最大临床剂量,而更通常的是低于批准的临床剂量的5分之一到10分之一。
我们建议使用的荧光标记施用途径与那些已经用于在临床实践数年后非常少或无证据被观察到其对病人存在有害作用的临床程序中的相同(参见Uhara H et al."Applicability of radiocolloids,blue dyes and fluorescent indocyanine green tosentinel node biopsy in melanoma",J Dermatol.2012Sep 20.doi:10.11 1 1/j.1346-8138.201 1.01340.x.)。而且,我们证明了主要途径对于待成像的大多数组织而言可以是一种定位性局部施用。
这些新的功能性或分子成像方案已在共焦显微内镜中示出,它们改善了早期诊断,改善了更安全更有效疗法的研发。在同样的上下文中,带固有荧光属性的治疗分子会被认为是在细胞级上进行“治疗成像”,例如亚德里亚霉素或MVAC,其允许精确施用以及监视药物的体内分解,以及更好地理解这一方案的体内分子治疗效能。
当与光纤显微镜成像结合时,在这一研究中评价的染料通过更好地澄清在中型构造水平观察时不易理解的区域的内容,为外科医生提供了重要的帮助。在局部施用了染料后,pCLE可以和宏观检验一起执行。医生在首次检查时可确定可疑区域的性质,从而使得不必进行检查炎症和创伤损害的复查。而且,由于pCLE适合于实时体内组织病理学检查,使得这一成像技术可以通过揭示是否需要执行活体检视,或允许进行待定切除手术的程度,为临床医生提供帮助。宏观和微观荧光检查是对组织异常结构诊断的补充,可为医生提供一种快速、新颖、非侵入式的多标度(多比例)医学成像方案。
Claims (8)
1.一种用于观察生物组织形态的方法,包括:
-在所述组织上使用近红外荧光染料,其中所述近红外荧光染料为吲哚氰绿;
-使用显微成像系统来形成所述生物组织的图像,其中所述近红外荧光染料的荧光在细胞级揭示了所述组织的形态,其中,所述显微成像系统包括基于探头的共焦激光内镜,所述基于探头的共焦激光内镜包括连接到激光器件的柔性光纤微探头,所述柔性光纤微探头包括光纤束。
2.根据权利要求1所述的方法,进一步包括将吲哚氰绿引入所述组织,其中吲哚氰绿的浓度根据施用途径的不同在0.05mg/ml至2.5mg/ml之间选择。
