Abstract
本发明提供一种麦默通术后止血、预防结节再生的中药制剂及其制备方法,中药制剂取下述重量配比的原料制备而成:柴胡5~30份、全瓜蒌7~30份、制香附5~30份、白及5~30份、蒲黄5~30份、乳香5~30份、没药5~30份、焦山栀5~30份、三七5~30份、白花蛇舌草15~40份、猪苓5~30份。本发明制备的麦默通术后止血、预防结节再生的中药制剂为无菌粉剂,可直接用于手术创面,也可加入水中冲服,具有止血生肌、散结消肿的功效,起到止血、促进创面愈合、预防结节再生的作用。

技术领域
本发明涉及一种中药制剂,具体涉及一种麦默通术后止血、预防结节再生的中药制剂及其制备方法。
背景技术
目前乳腺疾病发病率的提高,乳腺肿块患者日益增多,对于女性患者来说,乳房的外观完好十分重要,所以麦默通真空辅助旋切微创手术受到了广大女性患者的喜爱。相较于传统手术,微创手术具有手术创伤小、术后愈合快、麻醉风险小、不影响外观等优势。但是在临床操作中,术区止血成了一大难题,由于手术区域不直接暴露在外,因此无法直接止血,只能通过肿块上方压迫15分钟、术后加压包扎的方式止血。然而,因为患者的活动使绷带移位,或者血管压迫止血后又重新搏动开,导致术区空腔内血肿形成,影响了术后恢复,甚至影响了手术效果。而且紧缚的绷带往往给患者带来各种不适,如夜不能寐、呼吸困难、胸痛等,极大影响了微创手术的意义。此外,手术后是最佳用药时期,直接作用于乳腺组织,药物更易充分吸收,从而调节乳腺增生,预防结节再生。
发明内容
本发明的目的是解决上述问题,提供一种麦默通术后止血、预防结节再生的中药制剂及其制备方法,可直接用于手术创面,也可加入水中冲服,具有止血生肌、散结消肿的功效,起到止血、促进创面愈合、预防结节再生的作用。
为解决上述技术问题,本发明的实施例提供一种麦默通术后止血、预防结节再生的中药制剂,取下述重量配比的原料制备而成:柴胡5~30份、全瓜蒌7~30份、制香附5~30份、白及5~30份、蒲黄5~30份、乳香5~30份、没药5~30份、焦山栀5~30份、三七5~30份、白花蛇舌草15~40份、猪苓5~30份。
进一步,所述麦默通术后止血、预防结节再生的中药制剂取下述重量配比的主要原料制备而成:柴胡7~15份、全瓜蒌10~18份、制香附6~14份、白及7~15份、蒲黄7~13份、乳香6~15份、没药6~14份、焦山栀7~15份、三七8~16份、白花蛇舌草25~33份、猪苓6~15份。
优选的,所述麦默通术后止血、预防结节再生的中药制剂取下述重量配比的主要原料制备而成:柴胡10份、全瓜蒌12份、制香附10份、白及10份、蒲黄10份、乳香10份、没药10份、焦山栀10份、三七10份、白花蛇舌草30份、猪苓10份。
本发明采用的上述中药原料具有以下特点:
柴胡,有疏肝解郁、升举阳气的功效,现代药理研究表明柴胡具有抗炎、增加蛋白质生物合成、抗肿瘤、镇痛、增强免疫等作用。
三七,具有化瘀止血、活血定痛的功效,既能止血、又能去瘀生新,具有止血不留瘀,化瘀不伤正的特点。其有效成分三七氨酸能够明显缩短出血及凝血时间,还具有抗炎镇痛的作用。
全瓜蒌、制香附:有清热化痰、行气散结的功效,其有效成分具有调节雌激素水平的作用。
蒲黄,有止血化瘀功效,其提取物有促凝血作用。
白及有收敛止血、消肿生肌的作用,其提取物可明显缩短出血和凝血时间,促进肉芽生长、促进创面愈合。
乳香、没药,有活血止痛、消肿生肌的作用,其提取物能够镇痛、消炎、升高白细胞,促进肉芽生长。
焦山栀,栀子炒焦,具有凉血止血的功效,其有效成分具有抗炎、镇静、促进凝血的作用。
白花蛇舌草、猪苓具有清热解毒的功效,现代药理研究表明白花蛇舌草有效成分具有抗肿瘤的作用,猪苓有效成分具有消除水肿、抗肿瘤的作用。
本发明实施例还提供一种麦默通术后止血、预防结节再生的中药制剂的制备方法,包括如下步骤:
(1)将配方量的三七粉碎,制成三七粉,加入65%~90%乙醇提取两次,每次2~3小时,过滤,收集第一提取液;
(2)将配方量的乳香、没药切片,用水蒸汽蒸馏法提取其挥发油,收集为第二提取液;
(3)将配方量的柴胡、全瓜蒌、制香附、白及、焦山栀、白花蛇舌草、猪苓混合后加入足量的蒸馏水,煎煮30~60分钟,加入纱布包裹的蒲黄,继续煎煮30~45分钟,过80目筛,得第三提取液;
(4)将步骤(1)所得第一提取液和步骤(2)所得第二提取液加入步骤(3)制得的第三提取液内,采取柜式减压干燥器减压干燥法,制得干燥药粉;
(5)利用中药超微粉碎机将步骤(4)所得药粉进一步粉碎成100目以上的细粉;
(6)在臭氧浓度50g/m3、气流速度40m/s、气源氧气纯度>85%、灭菌温度小于40℃条件下对步骤(5)所得细粉进行臭氧灭菌,最大条件保存药物成分,制备成麦默通止血、预防结节再生的中药制剂。
其中,步骤(5)中制得的细粉要求:100%过100目筛,95%过120目筛。
本发明的上述技术方案的有益效果如下:本发明制备的麦默通术后止血、预防结节再生的中药制剂为无菌粉剂,可直接用于手术创面,也可加入水中冲服,具有止血生肌、散结消肿的功效,起到止血、促进创面愈合、预防结节再生的作用。
具体实施方式
为使本发明要解决的技术问题、技术方案和优点更加清楚,下面将结合具体实施例进行详细描述。
本发明提供一种麦默通术后止血、预防结节再生的中药制剂,取下述重量配比的原料制备而成:柴胡10g、全瓜蒌12g、制香附10g、白及10g、蒲黄10g、乳香10g、没药10g、焦山栀10g、三七10g、白花蛇舌草30g、猪苓10g。
按照上述配方,取配方量的各中药原料,按照下述制备方法制备麦默通术后止血、预防结节再生的中药制剂,制备步骤如下:
(1)将10g三七粉碎,制成三七粉,加入75%乙醇提取两次,每次2小时,过滤,收集第一提取液;
(2)将乳香10g、没药10g切片,用水蒸汽蒸馏法提取其挥发油,收集为第二提取液;
(3)将柴胡10g、全瓜蒌12g、制香附10g、白及10g、焦山栀10g、白花蛇舌草30g、猪苓10g混合后加入足量的蒸馏水,煎煮30分钟,加入纱布包裹的10g蒲黄,继续煎煮30分钟,过80目筛,得第三提取液;
(4)将步骤(1)所得第一提取液和步骤(2)所得第二提取液加入步骤(3)制得的第三提取液内,采取柜式减压干燥器减压干燥法,制得干燥药粉;
(5)利用中药超微粉碎机将步骤(4)所得药粉进一步粉碎成细粉,要求100%过100目筛,95%过120目筛;
(6)在臭氧浓度50g/m3、气流速度40m/s、气源氧气纯度>85%、灭菌温度小于40℃条件下对步骤(5)所得细粉进行臭氧灭菌,最大条件保存药物成分,制备成麦默通止血、预防结节再生的中药制剂。
利用上述配方和方法制备的麦默通术后止血、预防结节再生的中药制剂经过大鼠实验证明无毒副作用。
下面结合实验数据以证明本发明制备的中药制剂可用于麦默通术后止血、预防结节再生。
临床实验纳入南通市中医院2014~2016年住院行麦默通手术患者共计240人,按照评分标准(表一)分为高危组(84人)及低危组(156人),两组随机均分为实验组和对照组,实验组和对照组初始评分无统计学差异。实验组术后将本发明制备的中药制剂导入术区空腔,常规覆盖无菌敷料,无压迫止血及加压包扎,口服本发明制备的中药制剂10g tid 七天;对照组采用压迫止血及加压包扎。对实验组和对照组的患者观察有无术区血肿及血肿完全消散天数(无血肿计0分,每多一天血肿计1分),并于术后1年复查乳房B超,观察有无术侧乳房结节(按照表一评分标准进行评分)。
其中,高危组脱落2人,低危组脱落8人,所谓脱落是指患者没有来复查,因此,实际参与实验患者数为高危组82人,低危组148人。实验结果及评分标准如下。

注:肿块大于2cm不建议做微创手术。

实验结论:本发明制备的中药粉剂于麦默通术后应用于创口及口服较传统的单纯压迫及加压包扎止血能够促进止血,减小术后血肿发生几率,长远看能够减小术侧乳房结节再生几率,效果明确,安全有效。

Claims (4)
1.一种麦默通术后止血、预防结节再生的中药制剂,其特征在于,取下述重量配比的原料制备而成:柴胡5~30份、全瓜蒌7~30份、制香附5~30份、白及5~30份、蒲黄5~30份、乳香5~30份、没药5~30份、焦山栀5~30份、三七5~30份、白花蛇舌草15~40份、猪苓5~30份;
所述的麦默通术后止血、预防结节再生的中药制剂的制备方法包括如下步骤:
(1)将配方量的三七粉碎,制成三七粉,加入65%~90%乙醇提取两次,每次2~3小时,过滤,收集第一提取液;
(2)将配方量的乳香、没药切片,用水蒸汽蒸馏法提取其挥发油,收集为第二提取液;
(3)将配方量的柴胡、全瓜蒌、制香附、白及、焦山栀、白花蛇舌草、猪苓混合后加入足量的蒸馏水,煎煮30~60分钟,加入纱布包裹的蒲黄,继续煎煮30~45分钟,过80目筛,得第三提取液;

3.根据权利要求1或2所述的麦默通术后止血、预防结节再生的中药制剂,其特征在于,取下述重量配比的主要原料制备而成:柴胡10份、全瓜蒌12份、制香附10份、白及10份、蒲黄10份、乳香10份、没药10份、焦山栀10份、三七10份、白花蛇舌草30份、猪苓10份。
4.根据权利要求1所述的麦默通术后止血、预防结节再生的中药制剂,其特征在于,步骤(5)中制得的细粉要求:100%过100目筛,95%过120目筛。
