Abstract

本发明提供一种中药复方灌肠制剂，由下列原料药及质量份数比配制而成：柴胡10‑15、黄芩10‑15、茵陈12‑15、栀子13‑15、厚朴10‑15、枳实10‑15、延胡索10‑18、白芍12‑16、蒲公英20‑35、大青叶22‑36、鱼腥草20‑40、大黄12‑20、芒硝25‑35。本发明采用中药进行配伍，相比于现有的中药外用药，适应性更强。并可反复多次应用。具有消炎、促进胃肠蠕动、改善肠道通透性、平衡肠道菌群的特点。采用中药复方制剂使在肠道粘膜内吸收，提高治疗作用。灌肠相对口服更为安全、有效。

技术领域
本发明属于中药制剂，具体涉及一种治疗重症急性胰腺炎或不全性肠梗阻的中药复方灌肠制剂。
背景技术
目前，临床医学应用的灌肠成品介质主要有硫酸镁、液体石蜡油、开塞露等，但多次使用后对肠道粘膜刺激大、有损伤，不宜多次使用，并且存在成本高等问题。相较于西药和中医内科药物，中医外治法具有作用迅速、疗效显著，副作用少，使用方便，能直接观察，患者乐于接受等优点。目前尚无中药复方成品药物灌肠介质。然而，临床实际工作中医师决定应用中药复方灌肠，从决定应用到实际实施至少需要2小时以上，往往可能会更长时间。使得患者的不能及时得到中药灌肠治疗，特别是对于重症急性胰腺炎患者，其次是不完全性肠梗阻、肠道功能障碍的患者。如果是在夜间、中医科以外的科室需要使用中药灌肠时，往往需要等等相当长的时间。
发明内容
针对现有技术存在的上述不足，本发明的目的是为临床肠道功能障碍的患者提供无毒副作用、使用方便、疗效显著的新型中药复方灌肠制剂，无给患者更安全、有效的选择。
实现上述目的，本发明采用如下技术方案：一种治疗重症急性胰腺炎或不全性肠梗阻的中药复方灌肠制剂，由下列原料药及质量份数比配制而成：
柴胡10-15、黄芩10-15、茵陈12-15、栀子13-15、厚朴10-15、枳实10-15、延胡索10-18、白芍12-16、蒲公英20-35、大青叶22-36、鱼腥草20-40、大黄12-20、芒硝25-35。
进一步，一种中药复方灌肠制剂的制备方法，包括如下步骤：
1)将柴胡、黄芩、茵陈、栀子、厚朴、枳实、延胡索、白芍、蒲公英、大青叶和鱼腥草按上述重量配方备料，分别用适量的蒸馏水浸泡40-90分钟，用煎药机煎制20-40分钟，煎制浓缩为生药制剂浓度为0.46-0.50g/ml的中药煎剂；
2)生大黄单独沸水浸泡5分钟，浓缩成0.46-0.50g/ml生药浓度的制剂，再与芒硝一起加入上述中药煎剂中兑匀，供制粒备用；
3)浓缩、喷雾干燥、制粒。制成后为三次灌肠用的剂量的免煎剂。
相比现有技术，本发明具有如下有益效果：
1、本发明采用柴胡、黄芩、茵陈、栀子、厚朴、枳实、延胡索、白芍、蒲公英、大青叶、鱼腥草、大黄和芒硝进行配伍，相比于现有的中药外用药，适应性更强；并且中药复方灌肠对肠道粘膜刺激小，临床24小内可使用1-12次。
2、解决目前临床无中药成品灌肠制剂的问题，制成后可以随时供临床使用，具有消炎、促进胃肠蠕动、改善肠道通透性、平衡肠道菌群的特点。本发明中药复方灌肠免煎剂，患者就可以随时获得中药灌肠。缩短病情、尽快缓解病情。
3、采用中药复方制剂使在肠道粘膜内吸收，提高治疗作用。灌肠相对口服更为安全、有效。
4、中药复方灌肠制剂制备和使用方法操作简单，使用方便，容易控制，适合推广。
具体实施方式
下面结合具体实施方式对本发明作进一步详细说明。
一、一种治疗重症急性胰腺炎或不全性肠梗阻的中药复方灌肠制剂，以传统中医学理论为基础，通过科学配伍制得。由下列原料药及质量份数比配制而成：
柴胡10-15、黄芩10-15、茵陈12-15、栀子13-15、厚朴10-15、枳实10-15、延胡索10-18、白芍12-16、蒲公英20-35、大青叶22-36、鱼腥草20-40、大黄12-20、芒硝25-35。
二、中药复方灌肠制剂的制备方法，制备方法包括如下步骤：
1、将柴胡、黄芩、茵陈、栀子、厚朴、枳实、延胡索、白芍、蒲公英、大青叶和鱼腥草按上述重量配方备料，分别用适量的蒸馏水浸泡1小时，用煎药机煎制30分钟，煎制浓缩为生药制剂浓度为0.46-0.50g/ml的中药煎剂。
2、生大黄单独沸水浸泡5分钟，浓缩成0.46-0.50g/ml生药浓度的制剂，再与芒硝一起加入上述中药煎剂中兑匀，供制粒备用。
3、浓缩、喷雾干燥、制粒。

四、一种中药复方灌肠制剂的使用方法，采用上述配方和制备方法制成的中药复方灌肠制剂加蒸馏水溶解后直接用于灌肠，制剂与蒸馏水的重量体积比为60-100g/200ml。
较优比例为：制剂与蒸馏水的重量体积比为85g/200ml。

综上，本发明采用大柴芩承气汤制作成中药免煎剂，具有适应性强，治疗效果显著。使用时只需将上述中药复方灌肠制剂加蒸馏水溶解后直接用于灌肠，制剂与蒸馏水的重量体积比为60-100g/200ml。可以根据症状适当调整浓度。具有操作简单、无不适感、疗效好的特点。

所述中药复方灌肠制剂的制备方法，包括如下步骤：
1）将柴胡、黄芩、茵陈、栀子、厚朴、枳实、延胡索、白芍、蒲公英、大青叶和鱼腥草分别用适量的蒸馏水浸泡1小时，用煎药机煎制30分钟，煎制浓缩为生药制剂浓度为0.46-0.50g/ml的中药煎剂；
2）生大黄单独沸水浸泡5分钟，浓缩成0.46-0.50g/ml生药浓度的制剂，再与芒硝一起加入上述中药煎剂中兑匀，供制粒；
3）浓缩、喷雾干燥、制成颗粒。
