Abstract

本发明公开了一种用于输尿管封阻和牵拉力减张装置,该减张器用高分子医用材料一次整体制备成型,包括头端、连接杆、尾端。头端呈伞状、倒刺样结构,可以顺利向前推进,逆向受阻,可防止该装置滑脱。连接杆上均匀分布着螺纹结构,便于膀胱镜下异物钳夹持,也便于腹腔镜下钳持牵拉输尿管下端。尾端为“凹”字形提篮样结构,向头端牵拉时此处受力,输尿管无张力,避免输尿管断裂致使末端残留。该减张装置结构简单,操作简便,适合于肾输尿管切除手术时,通过牵拉力减张,达到充分切除输尿管的效果。

一种用于输尿管封阻和牵拉力减张装置
技术领域
本发明涉及在肾输尿管切除术中使用的装置,且特别是在肾输尿管切除术过程中使用的的输尿管阻塞和牵拉力减张装置。
发明背景
1991年美国华盛顿大学首次报道了腹腔镜肾根治性切除术治疗肾癌的新方法,因其围手术期并发症发生率降低、创伤小、切口美观及术后恢复快等优点已被广泛认可。随着越来越多的泌尿科医师对腹腔镜技术的认可,腹腔镜微创技术已在全球范围内得到快速发展。目前腹腔镜技术已逐渐成为临床上诊断和治疗肾盂癌和输尿管癌疾病的重要手段,然而由于存在术者操作技术不够熟练、器械配备不够完善、病情较复杂等因素,致使在手术操作过程中,输尿管穿孔、撕裂甚至断裂并发症时有发生。在手术切除过程中,术中挤压可能造成瘤细胞膀胱种植,也可能因为输尿管口切开深度不够或者输尿管水肿、质脆、过度牵拉等因素发生输尿管断裂,有瘤细胞术野播散风险,如遗留残端可能发生输尿管残端癌。这时如需做手术切除残端,需要重新摆体位,加做下腹部斜切口,势必延长手术时间,手术风险加大。加重患者痛苦,降低手术成功率。因此,如何设计出一种特殊的装置,能在手术切输尿管过程中,减少输尿管牵张力并且能有效封阻输尿管,防止输尿管断裂致使末端残留发生输尿管残端癌至关重要。通过我们的检索发现,迄今为止仍未发现现有此类设备的报道。
发明内容
技术问题:本发明所要解决的技术问题是:提供一种适合于输尿管的封阻减张器,该减张器结构简单、操作方便,通过其特殊结构,达到输尿管减张的目的并且能有效封阻输尿管,最终能使输尿管全长切除。
技术方案:为解决上述技术问题,本发明采用的技术方案是:
一种用于输尿管封阻和牵拉力减张装置,该减张装置包括头端、连接杆和尾端。头端,呈伞状、倒刺样结构;链接杆,杆上均匀分布螺纹状结构;尾端,其特征在为“凹”字形提篮样结构。
有益效果:与现有技术相比,本发明具有以下有益效果:
1.结构简单:本装置为一体加工成型,从结构上看仅包括头端、连接杆和尾端。结构简单,制造加工成本低。
2.操作方便:本发明的减张器在输尿管口环形切开操作前,将其通过膀胱镜鞘用闭孔器内芯推入膀胱,在膀胱镜下用异物钳夹持将其插入患侧输尿管口。操作简便,学习曲线短,经试验实施后,效果良好。
3.安全:一切操作均在显示屏监测指示下进行,装置由医用高分子材料制成,而且所有构件的末端均做成光滑的钝圆形态,避免了尖端锐性结构刺伤肾脏、血管、肠管等脏器引起大出血及肠瘘。

具体实施方式
下面通过以下实施例对本发明作进一步详细描述,以便本领域的技术人员进一步理解本发明,但不对本发明构成任何限制。
本发明的减张器在输尿管口环形切开操作前,将其通过膀胱镜鞘用闭孔器内芯推入膀胱,在膀胱镜下用异物钳夹持将其插入患侧输尿管口。头端倒刺样结构可避免向下滑脱。然后围绕其尾端,电切或激光环形切开输尿管口周围粘膜和肌层。其后手术中在输尿管下端周围游离充分后,可用手或腹腔镜下在输尿管外钳持该装置的螺纹样结构向上牵拉,力量传至尾端带动已经环切接近游离的输尿管末端,输尿管无张力下完整切除。
实施例:
该装置在临床实际中初步应用于3例患者,3例患者均为肾盂或输尿管癌拟行一侧的肾输尿管全长切除手术。手术前通过膀胱镜镜鞘通道放置该装置入膀胱内,在膀胱镜下将其插入病侧输尿管内,到位后留置,然后在膀胱镜下环形切开病侧输尿管口。手术中切除病侧肾脏后,在腹腔镜下用钳夹病侧输尿管下段,可扪及管腔内的本装置主体柱形部分及其螺纹样结构,钳夹此处后稍用力牵拉,将输尿管连带该装置一并与膀胱分离拉出,检查见输尿管全长完整,没有断裂遗漏,效果较满意。

1.一种用于输尿管封阻和牵拉力减张装置,其特征在于:该减张装置包括(1)头端、(2)连接杆和(3)尾端
2.根据权利要求1所述的头端,其特征在于呈伞状、倒刺样结构,其末端直径在20Fr;
3.根据权利要求1所述的链接杆,其特征在于杆上均匀分布螺纹状结构,长度为10cm;
4.根据权利要求1所述的尾端,其特征在为“凹”字形提篮样结构直径在20Fr。
