Abstract

Description
技术领域
本发明涉及一种二期(所谓的附加)眼内透镜,IOL,其手术地植入患者的人工晶状体眼内,即在所述二期IOL植入之前,另外还有已经植入到患者眼睛的眼后房中的至少一个一期IOL。
背景技术
老年性黄斑病变(AMD)是影响老年患者视网膜中心(黄斑)并导致中央视觉损失的医疗状况。周边视野通常不受影响并且患者保持辨向能力。虽然如此,多数患者至少在该疾病的后期损失阅读的能力,并且在西方世界年龄大于50岁的患者中,AMD是引起患者失明和视觉障碍的主要原因。
已经提出了通过特定透镜和设备植入的多种手术干预。一些系统基于图像的放大,但同时引起视野的严重减小,比如一些可植入望远镜,如EP1475055中所描述的。由于视场的减小并且因为它是笨重且难以植入的,此外它在独眼患者中是禁忌的,该解决方案没有变得流行。
使用菲涅尔透镜系统的其他系统,比如在专利申请WO2005039451中描述的,或具有非重合轴的组合收敛和发散透镜,比如在专利申请WO2010136798和WO2010131955中描述的,提出了使光束光学转向并将焦点位移至视网膜中央凹外侧的视网膜区域。由于将焦点位移至视网膜中央凹以外的其他视网膜区域将不允许阅读视觉,因为这些视网膜部分具有降低强度的感光细胞,这些系统变得不流行。
其他系统提出了具有至少一个负度数眼内透镜部的特定眼内植入物与外部透镜(眼镜)交互的组合,如申请WO0132105和EP2319457中所描述的。由于需要使用特定眼镜,因此它们不能提供超出传统放大眼镜的任何优势,这些系统没有变得流行。
专利申请WO8707496和WO8909576分别描述了在同轴实施方式中单件双焦眼内透镜构造,提及了瞳孔的收缩。然而,这些申请中透镜描述为独立透镜,未设计为需要与一期IOL光学上合作的二期IOL。前面提及的其他透镜仅描述为刚性透镜。最后,前面提及的文献中透镜的光焦度分布被限制为用于老花眼,在针对近视眼的中央透镜部中需要另外2-4D。
所有前面提及发明的主要问题是所提出的这些系统通常设计为被植入以替代标准眼内透镜,并且多数外科医生会反对该选择。
事实上,超过三分之二的具有早期AMD和0.3或更小视力的患者已经是人工晶状体眼,即它们已经做了将眼内透镜植入到囊袋内的白内障手术。
发明内容
通过本发明,我们设定自己的目标是交付简单、价廉且安全的方案,用于改进遭受老年性黄斑病变(AMD)的人工晶状体眼患者的近视。
我们意识到我们能够受益于近视瞳孔缩小的效应,其中在眼睛聚焦于附近物体时瞳孔在反射中收缩。该反射也可靠地运行于老年人中。瞳孔的这种收缩限制了进入眼睛内透镜中心的光束。
我们的发明设计通过使用瞳孔缩小作为所谓的近三体(Near Triad)的一部分的三次自然眼睛反射中的一次而用于遭受AMD的人工晶状体患者,即,伴随两只眼睛适应性调节和会聚的瞳孔尺寸减小。
因此,本发明涉及一种二期(所谓的附加)眼内透镜,IOL,其由可折叠软材料如丙烯酸脂或硅树脂制成。二期IOL手术地植入到患者的人工晶状体眼的睫状沟内,即在所述二期IOL植入之前,另外还有已经植入到患者眼睛的眼后房的至少一个一期IOL。
二期IOL设计为与一期IOL光学地协作以在患者眼睛的视网膜上共轴地聚焦组合图像,这通过另外地放大投影到视网膜中央凹上的一期IOL的图像的至少中间部从而改善近视来改进了患者的视觉能力。
可折叠二期IOL包括一个或多个触觉部,用于将其固定并稳定在患者眼睛的睫状沟内,和光学活性部,其设计为将图像穿过一期IOL投影到视网膜上。
二期IOL的光学活性部包括中央光学部,其优选地具有小于1.8mm的直径,和外周光学部,其绕中央光学部延伸,因此形成来自单块的、两个不同但共轴定位的透镜。
中央光学部设计为形成正透镜,其为由二期IOL的外周光学部提供的折射提供优选大于5屈光度的附加折射,因此与一期IOL和二期IOL的外周光学部的组合折射相比提供大于5屈光度的附加折射。
通过该构造,为患者提供了在不使用眼镜或放大镜的情况下具有放大图像的能力。如果患者对二期IOL不满意,它能够手术地移除而保持一期IOL的功能。由于近视瞳孔缩小的效应,提供放大图像的中央光学部将在患者聚焦在附近物体上时运行而在患者通过放大的瞳孔聚焦在远处物体时将不会显著地影响远距视觉。
我们的发明不旨在且不工作于远视眼、晶状体眼或白内障患者。它目标仅在于患有早期AMD的人工晶状体眼患者,为他们提供便捷、简单和安全方案以恢复他们受AMD损伤的近视视力。总之,我们的发明不能够被认为是在前面提及的现有技术上的改进,它关于目的和功能是明显不同的。

具体实施方式
如图1-2中所描述的,提供二期眼内透镜,IOL,其由可折叠软材料如丙烯酸脂或硅树脂制成。二期IOL2包括触觉部7,用于将其固定并稳定在患者眼睛内,和光学活性部9。这些触觉部能够具有当前IOL中公知的任意形状:开放C形(如我们的图解中)或Z形环、封闭环或盘状触觉部,它们具有或不具有开窗、具有或不具有轴向角。
光学活性部9包括中央光学部10,和外周光学部11,其绕中央光学部10延伸。中央光学部10和外周光学部11形成来自单块的、两个不同但共轴定位的透镜。光学活性部9可具有4mm和10mm之间、优选地在5mm和7mm之间的直径13。

中央光学部10的直径可以小于1.8mm,从而完全地使用但不超出收缩的瞳孔的直径(太多),并且不显著干扰通过放大的瞳孔的远视视力。中央光学部10可具有大于0.5mm的直径,从而产生能够被患者所感知的最小期望放大效果。优选地,中央光学部10可具有0.8mm和1.6mm之间的直径,从而产生上述提及的状况之间的合理平衡。
中央光学部10超出外周光学部11的附加折射可以大于5屈光度,从而产生能够恢复患者阅读能力的放大。中央光学部10超出外周光学部11的附加折射可以小于25屈光度,这是因为在现实生活中将难以操纵靠近眼睛小于4cm的任意物体。因此中央光学部10可除了外周光学部11之外具有+5屈光度和+25屈光度之间的、优选为+8屈光度和+12屈光度之间的折射。因此本发明与一期IOL1和二期IOL2的外周光学部11的组合折射相比可提供+5屈光度和+25屈光度之间的、优选为+8屈光度和+12屈光度之间的附加折射。
外周光学部11可设计为形成具有零折射率的透镜,因此不干扰由一期IOL1提供的图像,为患者留下由一期IOL 1提供的他的大部分视力。在另一实施方式中,外周光学部11可设计为形成具有-5D和+15D之间的一定折射,从而矫正折射中的任意先前误差或由一期IOL 1提供的患者视力中的任意非故意、非期望变化。
二期IOL 2的中央光学部10和光学活性部9的直径之间的比值可以是0.05和0.45之间,优选为0.15和0.35之间。

二期IOL 2的效果能够通过对比图3和图4来理解。图3和图4在由虹膜13形成的瞳孔15的尺寸上不同。
在图3中,瞳孔15是收缩的,从而光束主要限制到二期IOL 2的中央光学部10上,提供了黄斑6上的放大图像。这是患者聚焦在附近物体时的情形,即,阅读报纸或价格标签,并且近视瞳孔缩小的反射收缩了瞳孔15。因此投影到视网膜5上的图像与通过远视在眼睛上生成的图像相比被放大,这使得患者眼睛在AMD的情形中也能够还原图像。由于中央光学部10与基本透镜光学倍率相比的相对高的折射率,敏锐视觉处于非常近的距离d,对于此典型数值是5-20cm。
在图4中,患者聚焦在远处物体上并且瞳孔15是放大的,在中央光学部10周围留下足够空间用于光线也穿过二期IOL 2的外周光学部11。因此,来自远处物体的、穿过外周光学部11并在视网膜上形成目标远处图像的光线将超过穿过中央光学部10、未聚焦在视网膜(虚线)上的光线而主导患者的感知。
尽管在附图中已经图解且在前面详细描述中已经描述了本发明的一个优选实施方式,应该理解的是本发明不限于所公开的实施方式,而是能够在不偏离本发明的情况下作出针对IOL注入器的多种重新布置、修改、和替代。
Claims (13)
1.一种由可折叠软材料制成的二期眼内透镜IOL(2),其附加于在所述二期IOL(2)植入之前已经植入到患者眼睛(4)的眼后房(3)内的至少一个两面凸的一期IOL(1)被手术地植入,所述二期IOL(2)手术地植入到所述患者眼睛的睫状沟内,位于所述眼睛的瞳孔和所述一期IOL(1)之间并邻近所述眼睛的瞳孔和所述一期IOL(1);其中
