慢性咽炎、扁桃体炎中医称之为慢喉痹、乳蛾,不仅是耳鼻喉科的常见病、多发病,也是内科、儿科的常见病、多发病,因其反复发作、缠绵难愈,中西医治疗均难以取得理想疗效。慢性咽炎、扁桃体炎是临床常见病,虽不威胁患者的生命,但其多种不适感觉给患者的精神造成较大的负担,使患者长期处于亚健康状态,严重影响其生活质量,尤其是儿童,因反复发作,经常长期使用抗菌药物,使患儿机体抵抗力下降,甚至影响患儿的生长发育。而中医药因毒副作用小,可长期服用,得到患者及患儿家长的认可。笔者临床观察发现,慢性咽炎、扁桃体炎的治疗,需要医患的共同努力,即长期不间断的服用,保证足够疗程( 1个疗程 2个月,或 2~3 个疗程) ,中医药治疗方可取得显著疗效。慢性咽炎、扁桃体炎属中医学“慢喉痹”“乳蛾”的范畴,其发病与肺脾肾的关系密切。究其病因,发病早期多为风热毒邪循口鼻入侵,肺胃所属之咽喉首当其冲而发病。病久体弱则致脏腑之阴阳气血津液失调,邪毒久滞咽喉,气血痰浊郁滞,咽喉失养,易致病程迁延反复发作。
广西狗牙花:为夹竹桃科狗牙花属植物广西狗牙花Ervatamia kwangsiensisTsiang的根皮、叶。根皮全年可采,洗净,鲜用;叶鲜用。【性味】苦;辛;性平。【功能主治】活血止痛。主跌打损伤;骨折肿痛。【原植物形态】 灌木,高达5m。除花外无毛;枝和小枝有皮孔,节间达2-3cm。假托叶基部扩大而合生,卵圆形,长达2mm。叶对生;叶柄长5-12mm;叶片坚纸质,椭圆状卵圆形,长5-15cm,宽3-6.5cm,先端短渐尖,基部楔形,表面深绿色,背面淡绿色;中脉在叶面凹陷,在背面凸起;侧脉12-14对,弧形上升。聚伞花序腋生,通常二歧,生于小枝顶端,有花6-7朵;总花梗长3-4cm;苞片和小苞片卵圆形;花5数:花蕾圆筒形;花萼宽钟形,基部内面无腺体,萼片有缘毛;花冠白色,早落;花冠筒上部向右旋转,裂片基部向左覆盖,两面有微柔毛;雄蕊着生于近花喉部;花药近箭头形;柱头2裂。蓇葖双生,150°叉开,长圆状披针形,具短喙;果柄长约1cm,果长5-6cm。种子在每个蓇葖内有5-6颗。花期5-9月,果期9-11月。收载于中药大辞典。
蝈蝈:本品为螽斯科螽斯属动物螽斯Gampsaocleis gratiosa BrunnerWattenwyl.的全体。夏、秋季捕捉,捕后沸水烫死,晒干或烘干。【性味】味辛;微甘;性平。【功能主治】利水消肿;通络止痛。主水肿尿少;腰膝肿痛;湿脚气。【性状】本品全体呈长圆形,灰绿色或黄褐色。头略呈圆形,复眼1对,卵圆形,触角1对,长鞭状,多脱落;前胸背板略呈细长圆柱形,中后胸被翅;胸足3对,多脱落,后足较大。气腥。收载于中药大辞典。
树扁竹:本品为兰科鸢尾兰属植物鸢尾兰Oberonia iridifolia (Roxb.)Lindl.[Cymbidium iridifolium Roxb.]的全草。全年均可采收,开水烫后晒干。【性味】 微苦;性凉。【归经】 膀胱经。【功能主治】 清热利湿;活血通络。主尿路感染;支气管炎;哮喘;肠炎;消化不良;跌打损伤;骨折。【原植物形态】鸢尾兰,多年生附生草本,高12-30cm,簇生多数须根。叶厚,两侧压扁并互相套叠,剑形或长圆状披针形,长4-13cm,宽1-2cm,先端有时稍歪斜,基部有关节。总状花序从植株中心抽出,远长于叶,直立,圆柱形,具多数密集的小花;小苞片薄革质,卵形,长1.5-2mm。花小,直径约2mm,白绿色;萼片与花瓣均反折,花瓣比萼片狭;唇瓣近半圆形,边缘近啮蚀状,先端2裂。蒴果倒卵状椭圆形,长约5mm,柄长1.5mm。花期10-11月,果期11-12月。收载于中药大辞典。
藜芦托素(Veratrosine):CAS号475-00-3,分子式C33H49NO7,分子量571.74。【生物活性】抗高血压。【成分来源】绿藜芦 Veratrum viride,毛叶藜芦 Veratrumgrandiflorum。
樟柳碱(Anisodine):CAS号52646-92-1,分子式C17H21NO5,分子量319.36。【生物活性】抗胆碱能,解痉剂,唾液分泌抑制剂,散瞳。【成分来源】藏茄Anisodus tanguticus[Syn. Scopolia tangutica]。
橙黄决明素(Aurantio-obtusin):CAS号67979-25-3,分子式C17H14O7,分子量330.29。【药理作用】抑制血小板凝集。【成分来源】决明子Cassia tora。

本发明的目的是克服背景技术的不足,提供一种有效防治扁桃体炎的药物组合物及其制备方法。
本发明是采用如下技术方案实现的:
制成该防治扁桃体炎的药物组合物的原料药的组成和重量份为:
广西狗牙花530-540重量份 蝈蝈525-530重量份 树扁竹510-520重量份 藜芦托素15-20重量份 樟柳碱10-13重量份 橙黄决明素6-9重量份。
优选的用于防治扁桃体炎的药物组合物,是由如下重量份的原料药组成:
广西狗牙花535重量份 蝈蝈528重量份 树扁竹515重量份 藜芦托素18重量份 樟柳碱12重量份 橙黄决明素8重量份。
一种防治扁桃体炎的药物组合物,其特征在于药物组合物可以采用制剂学的常规方法制备成片剂或胶囊剂或滴丸。

(1)按原料药配比取蝈蝈、树扁竹、广西狗牙花、藜芦托素、樟柳碱、橙黄决明素,混匀,用重量百分比浓度37%乙醇作为溶剂,在25℃温浸提取,提取次数为9次,每次提取时间为23小时,每次溶剂用量为原料药总重量的55倍,滤过,得药渣A和提取液A,提取液A回收乙醇,浓缩至相对密度1.03,滤过,药液通过DM133 大孔吸附树脂柱,先用水洗脱,再用重量百分比浓度48%乙醇溶液洗脱DM133 大孔吸附树脂柱,收集重量百分比浓度48%乙醇洗脱液,回收乙醇,浓缩干燥,即得提取物A;
(2)取步骤(1)药渣A,用重量百分比浓度58%乙醇作为溶剂,加热回流提取9次,每次提取时间为0.2小时,每次溶剂用量为药渣A重量的43.5倍,滤过,得药渣B和提取液B,提取液B回收乙醇,浓缩至相对密度1.10,滤过,药液通过X-5大孔吸附树脂柱,先用水洗脱,再用重量百分比浓度69%乙醇溶液洗脱X-5大孔吸附树脂柱,收集重量百分比浓度69%乙醇洗脱液,回收乙醇,浓缩干燥,即得提取物B;
(3)将提取物A和提取物B混匀,即得药物组合物。

制备方法:
(1)按原料药配比取蝈蝈、树扁竹、广西狗牙花、藜芦托素、樟柳碱、橙黄决明素,混匀,用重量百分比浓度37%乙醇作为溶剂,在25℃温浸提取,提取次数为9次,每次提取时间为23小时,每次溶剂用量为原料药总重量的55倍,滤过,得药渣A和提取液A,提取液A回收乙醇,浓缩至相对密度1.03,滤过,药液通过DM133 大孔吸附树脂柱,先用水洗脱,再用重量百分比浓度48%乙醇溶液洗脱DM133 大孔吸附树脂柱,收集重量百分比浓度48%乙醇洗脱液,回收乙醇,浓缩干燥,即得提取物A;

制备方法:
(1)按原料药配比取蝈蝈、树扁竹、广西狗牙花、藜芦托素、樟柳碱、橙黄决明素,混匀,用重量百分比浓度37%乙醇作为溶剂,在25℃温浸提取,提取次数为9次,每次提取时间为23小时,每次溶剂用量为原料药总重量的55倍,滤过,得药渣A和提取液A,提取液A回收乙醇,浓缩至相对密度1.03,滤过,药液通过DM133 大孔吸附树脂柱,先用水洗脱,再用重量百分比浓度48%乙醇溶液洗脱DM133 大孔吸附树脂柱,收集重量百分比浓度48%乙醇洗脱液,回收乙醇,浓缩干燥,即得提取物A;
(2)取步骤(1)药渣A,用重量百分比浓度58%乙醇作为溶剂,加热回流提取9次,每次提取时间为0.2小时,每次溶剂用量为药渣A重量的43.5倍,滤过,得药渣B和提取液B,提取液B回收乙醇,浓缩至相对密度1.10,滤过,药液通过X-5大孔吸附树脂柱,先用水洗脱,再用重量百分比浓度69%乙醇溶液洗脱X-5大孔吸附树脂柱,收集重量百分比浓度69%乙醇洗脱液,回收乙醇,浓缩干燥,即得提取物B;
(3)将提取物A和提取物B混匀,即得药物组合物。
实施例4:片剂的制备
取实施例1药物组合物410g,加入淀粉530g,混匀,制粒,干燥,加微晶纤维素200g,硬脂酸镁22g,混匀,压制成6000片, 即得药物组合物片剂。
实施例5:胶囊的制备
取实施例2药物组合物370g,加入淀粉420g,混匀,制粒,干燥,整粒,加入适量硬脂酸镁,混匀,装胶囊2100粒,即得药物组合物胶囊。
实施例6:滴丸的制备
称取聚乙二醇 6000 233g水浴(80℃)加热煮熔,加入实施例3药物组合物13g,充分搅拌均匀,以液体石蜡为冷却剂,置玻璃管(4*80cm)中,冷却温度为2℃,滴口内外径为7.0/2.0(mm/mm),滴口距液面为2.0cm,滴速以每分47滴为最佳条件,用棉布吸干滴丸表面的冷凝剂,即得药物组合物滴丸。
实验例1:防治扁桃体炎的试验研究
1 临床资料
1.1 一般资料
本组 25 例病例均为门诊患者,男 13例,女12 例,年龄最大 71岁,最小 5岁,其中慢性咽炎10例,慢性扁桃体炎 15 例,病程最长 27年,最短 0.5年,均有反复发作史,25例慢性扁桃体患者中儿童 14例,多数每月急性发作 1 次,甚则一月发作数次。
1.2 诊断标准
诊断依据《中医病证诊断疗效标准》。①慢喉痹:以咽部干燥,或痒、疼、异物感、胀紧感等为主要症状,或常有喀咳清嗓声,甚则刺激性咳嗽;病程较长,咽部不适症状时轻时重。常有急喉痹反复发作史,或因鼻窒而长期张口呼吸,或因烟酒过度,环境空气干燥、粉尘异气刺激等导致发病。咽部检查黏膜肿胀,或有萎缩,或有暗红色斑块状、树枝状充血。咽侧索肿大,咽后壁淋巴滤泡增生。②慢乳蛾: 扁桃体肿大、充血呈暗红色,或不充血,表面有脓点,或挤压后有少许脓液溢出。排除咽喉部、食道肿瘤及可能引起以上症状的其他疾病。
1.3 治疗方法
本组 27例患者均采用药物组合物(实施例1药物组合物)治疗,每次1.5g,每日3 次口服,2 个月为1 个疗程。对咽痛、咽部明显充血、咽部有脓点者,嘱频频含服,一日数次。
2 结果
2.1 疗效评定标准
疗效评定依据《中医病证诊断疗效标准》慢喉痹、乳蛾。治疗结束后复查并判断疗效。治愈: 咽部症状及体征消失,检查正常,扁桃体不充血,无脓点;好转: 咽部症状和体征明显减轻,扁桃体脓点消除;未愈: 症状和体征无明显变化。
2.2 治疗结果
服药 1 个疗程有效者 17 例,2 个疗程有效者6 例,未愈 2例,均未能坚持中药治疗。痊愈19例,好转4例,未愈2例,总有效率92.00%。部分患者疗程结束后行肝肾功能检查无异常。
2.3 临床病例
李某某,男,10岁,2011年 5月初诊,患儿有支气管哮喘、慢性扁桃体炎、腺样体肥大病史。因慢性扁桃体炎反复发作,每月 1~3次,曾 2 次住院欲行扁桃体腺样体摘除术,均因感冒未能手术而出院。经本院儿科医生介绍而转诊我处中医治疗。初诊时见鼻塞涕稠,夜寐鼾声,纳食二便正常。查体: 咽部充血,咽后壁滤泡呈铺路石样改变,扁桃体Ⅲ度肿大。喉镜、CT 检查示: 扁桃体、腺样体Ⅲ度肿大。经用药物组合物治疗 1 个疗程初见效果,即夜寐鼾声减轻,2 个月内仅发作扁桃体炎、哮喘 1 次,静滴抗生素 3 天症状即缓解( 未服中药前至少需静滴抗生素 1 周),效果显著。
张某某,男,58 岁,退休工人。初诊 2011年8月24 日,主诉: 反复咳嗽、咽部不适6 年余,伴干咳少痰,咽痒不适,怕冷,见风咳甚,纳食二便正常,舌红,苔黄厚腻,脉弦。查体: 咽后壁、咽颊部充血明显 +++ ,扁桃体Ⅰ度肿大。经用药物组合物治疗 2 周,咳止,后用药物组合物治疗 1 个疗程,上述症状悉平,舌淡红,苔薄白,脉弦。咽部症状消失,检查正常。
尚某某,女,34 岁,教师。初诊 2011 年 10月5日。主诉: 反复发作刺激性咳嗽,少痰,咽痒不适,胸闷憋气,气喘三个多月,曾经中西医治疗未效,舌尖红,苔黄腻,脉细。查体:咽部充血 + + ,扁桃体无肿大。经用药物组合物治疗半个月,咳止,后用药物组合物治疗半个月,上述症状悉平,痊愈。
