背景技术

发明目的
本发明的目的是设计用于提供通过可注射的辣椒素来缓解哺乳动物中的疼痛的制剂和方法,其克服了施用辣椒素经历的强烈的灼痛。
本发明的进一步的目的是提供用于减轻或阻止与辣椒素注射给药相关的灼痛或刺痛的制剂和方法。
本发明的另一个目的是提供含有完全混溶的水性和亲脂性组分的光学透明的溶液。
本发明的另一个目的是提供用于在没有镇静或麻醉(例如神经、脊髓或硬膜外阻滞)的情况下延长疼痛缓解的制剂和方法,用于一系列的治疗应用。
其他目的和益处从下面的说明书来看是显而易见的。
发明内容
本发明涉及用于在哺乳动物中注射的水性组合物,包括:
i)0.0002%-0.1%重量的辣椒素,
ii)99.9%-99.9998%重量的水性载体,所述水性载体包括溶解所述辣椒素的增溶剂以及在哺乳动物中注射所述组合物时使所述辣椒素从所述水性组合物中的释放减慢的延长释放剂,
其中所述水性载体减少或消除了在注射时所述辣椒素产生的灼热和刺痛。辣椒素通常为反式辣椒碱。增溶剂可以为聚乙二醇,延长释放剂通常为具有平均分子量约1000千道尔顿的透明质酸。延长释放剂也可为胶原蛋白、弹性蛋白和可生物降解的聚合物基体。
在一个有利的实施例中,所述水性组合物包括:
0.005–0.05%重量的辣椒碱,
5-25%重量的聚乙二醇,以及
0.2–1%重量的透明质酸。
在其他实施例中,所述水性组合物可包括抗炎性剂、镇痛剂和/或表面活性剂(.002-2%重量),例如非离子表面活性剂聚氧乙烯40氢化蓖麻油或PS 80。辣椒素与多山梨醇酯80的重量比可以为1至5。
在一个有利的实施例中,所述水性组合物包括:
0.005–0.05%重量的辣椒碱,
0.025–0.15%重量的多山梨醇酯80或聚氧乙烯40氢化蓖麻油,
0–25%重量的聚乙二醇300或400,以及
0.2–1%重量的透明质酸。
本发明还涉及一种用于治疗哺乳动物(例如人)的部位疼痛的方法,包括向所述哺乳动物施用治疗上有效量的本发明的水性组合物。通常在施用所述组合物之前施用麻醉剂。在一个实施例中,在施用所述辣椒素组合物后的第一分钟期间,要反复弯曲和伸展该关节。而且,在施用所述辣椒素组合物之前、期间和/或之后,可向所述部位使用冷却手段。以约0.1ml至25ml体积的药学上可接受的载体施用辣椒素组合物。
本发明还涉及通过关节腔内注射施用辣椒素的试剂盒,包括:a)至少一个单位剂量的麻醉剂,以及b)至少一个单位剂量的含有辣椒素和透明质酸的辣椒素溶液。该试剂盒被设置为在辣椒素溶液之前施用所述麻醉剂。该试剂盒可包括用于给药a)和b)的装置,并可以有给药说明书。
具体实施方式
本发明的制剂提供了一种用于管理温和到严重的慢性(持续大于3个月)或急性疼痛的长效的、非阿片类的治疗方案。有效的辣椒素注射疗法必须包括处理辣椒素给药后灼热疼痛的位置的治疗方式。
通过部位特异性的辣椒碱注射引起患有致残慢性疼痛病情的患者中的感觉神经元的选择性和可逆性的机能丧失,对于长期(数周到数月)缓解疼痛是一种具有吸引力的方法。然而,由于注射时辣椒碱所引起的强烈的灼痛,采用辣椒碱注射缓解疼痛的关节等的治疗是非常复杂。
本发明公开的辣椒素制剂和方法可被用来治疗/减轻哺乳动物中的疼痛,通常通过在离散部位注射来提供延长的一段时间的缓解疼痛。以药学上可接受的渗透载体施用制剂。所述方法和制剂进一步包括以减弱辣椒素给药产生的灼热和痛觉过敏效果的量给药延长释放剂、抗炎药、镇痛剂和/或麻醉剂。这可以与在辣椒素注射前、期间或之后冷却方法例如冰袋或凝胶袋的应用结合进行。
通过部位特异性的辣椒碱注射引起的感觉神经元的选择性和可逆性的机能丧失在患有致残慢性疼痛病情的患者中提供了长期(数周到数月)的疼痛缓解,并且没有或仅轻微的系统性副作用。本发明的辣椒素制剂可在长的时间段里减轻或减弱部位处的疼痛。关于与关节炎病情(例如骨关节炎)相关的疼痛,在某些实施例中,一个单位剂量的辣椒素注射或植入减轻部位疼痛至少3个月,至少4个月、5个月或6个月。关于关节疼痛,在某些实施例中,一个单位剂量的辣椒素注射或植入减轻部位疼痛至少1个月,至少3个月,和3至6个月,以及大于6个月的时间段,例如6至12个月。关于术后疼痛,一个单位剂量的辣椒素注射或植入减轻部位疼痛至少1周,并且在某些实施例中至少1个月。
本发明的组合物
通过将辣椒素掺入到药学上以及生理上可接受的水性载体中制备辣椒素药剂用来注射、植入、渗透或局部应用进行降低基于应用产生的灼热感。本发明的目的是将非常小量的辣椒碱注射给药到离散的局部区域来治疗以及减轻疼痛。,显著的益处产生于毫克级和/或毫克量分数的辣椒碱,目的是通过改变受限区域的感觉神经的功能(TRPV-1)产生治疗结果。

辣椒素
对本发明的辣椒素的一般性讨论见美国专利申请2008/0262091,以及共同拥有的美国系列申请号13/609100,其通过引用全部并入本申请。如在此使用的术语“辣椒素”包括辣椒碱和除辣椒碱以外的辣椒素即二氢辣椒碱、降二氢辣椒碱、高二氢辣椒碱、高辣椒碱和香草壬酰胺,以及辣椒碱与一种或多种其他辣椒素的混合物。药物的使用量基于一剂辣椒碱的治疗上的剂量。辣椒素为制剂的0.0002-.1%wt,有益的为制剂的0.005-0.05%wt。
或者,“辣椒碱类似物”(例如树脂毒素)可部分或全部代替辣椒素给药。被施用的类似物以治疗上与辣椒碱等效剂量的量给药-见美国专利申请2008/0262091,通过引用将其全部并入于此。在其他的实施例中,在本发明的制剂和方法中使用TRPV1促效药而不是辣椒素或辣椒碱类似物。
输送载体
本发明使用的水性药物组合物为单相水性/水系统的药学上可接受的输送载体,所述单相水性/水系统包括药学上可接受的溶剂(例如聚乙二醇(如PEG 300和PEG 400)、乙醇和非离子表面活性剂(例如聚氧乙烯40氢化蓖麻油(克列莫佛RH40)和多山梨醇酯80(PS 80)))。延长释放剂(例如透明质酸)可控制制剂的粘性并有助于制剂的稳定性。公开的水基制剂的显著的益处为水浓度可超过95wt.%。
水性制药载体中的额外组分的示例包括调节渗透压和肌肉弹性的葡萄糖(蔗糖)和/或氯化钠,以及调节pH的缓冲剂。在辣椒碱/PS 80的水性溶液中包含0.9%NaCl、0.25%苯酚、0.25%薄荷醇和5%葡萄糖不会导致混浊态或形成沉淀。
延长释放剂
已经发现能够使辣椒素从制剂中缓慢释放出来(引起延长释放)的某些化合物减少了灼热和刺痛效果。有利地,延长释放剂将会在超过15分钟、30分钟、1小时,或超过4小时的时间里释放所述辣椒素。在一个实施例中,在超过一周的时间里释放所述辣椒素。通常,较高的剂量会在更长的时间里被释放出来。在本发明的一个实施例中,所述延长释放剂不是糖胺聚糖或蛋白聚糖。
透明质酸
辣椒碱和透明质酸及其盐(天然存在于人体各种组织(皮肤、关节滑膜液和结蹄组织)中的物质)的增溶有助于改善与局部使用和注射辣椒碱制剂相关的灼热和刺痛。有利地,使用1%至2%重量的透明质酸或其盐。在本发明中描述的实验测试中使用的透明质酸的分子量具有800至1200千道尔顿的平均分子量。然而不希望被理论束缚,人们认为向辣椒碱中加入透明质酸组分会在水溶液中形成多糖网,并引起辣椒碱以可控的方式缓慢地被释放出来,导致灼热和刺痛的疼痛减轻。在本发明的一个实施例中,所述透明质酸为交联水凝胶的形式。
胶原蛋白和弹性蛋白
胶原蛋白是动物中各种结蹄组织的主要结构蛋白。作为结缔组织的主要组分,其是哺乳动物中最丰富的蛋白,占据了整个身体蛋白含量的25%至35%。胶原蛋白为细长纤维的形态,主要见于纤维组织,例如腱、韧带和皮肤,并且在眼角膜、软骨、骨头、血管、肠和椎间盘中也很丰富。
弹性蛋白是结缔组织中的一种蛋白,其具有弹性并让身体中的许多组织在拉伸或收缩后恢复它们的形状。弹性蛋白帮助被戳或捏的皮肤回到其原来的位置。弹性蛋白还是脊椎动物身体中的重要的承重组织以及在需要存储机械能的地方使用。
与辣椒素一起的胶原蛋白和/或弹性蛋白在治疗包括OA疼痛的疼痛治疗中是有利的。加入到增溶的辣椒碱制剂中的胶原蛋白和/或弹性蛋白组分可在水溶液中形成蛋白网,这有助于延迟或延长辣椒碱的释放,从而有助于最小化来自局部应用或注射辣椒碱的灼热的不舒服感。此外,在用辣椒素注射时,这些天然存在的高分子量蛋白的功能是控制辣椒碱向神经释放的速率以减轻灼热,提供了滑动骨表面的润滑作用。向含有一种或两种这些天然存在的高分子量蛋白的辣椒素制剂中添加透明质酸进一步使耐受性和有效性最优化。在本发明的组合物中,胶原蛋白或弹性蛋白必须为液体或凝胶形式。
生物可吸收聚合物基体
生物可吸收聚合物基体,例如交联氧化葡聚糖水凝胶,也可作为延长释放剂被用于本发明的制剂中。参见美国专利8435565,其内容通过引用全部结合在本申请中。
溶剂
聚乙二醇被称为PEG,其作为溶剂、增塑剂、软膏和栓剂的基质,以及在片剂和胶囊中作为润滑剂被用作非活性组分。PEG具有低毒性,全身吸收小于0.5%。术语“PEG”与数字结合使用。在制药工业中,所述数字表示平均分子量。低分子量液体聚乙二醇PEG 300和400为优良的溶剂,并且是大量不易溶于水的物质的共溶剂。因此,它们广泛地被用作活性物质的溶剂和增溶剂以及液体和半固体制剂中的赋形剂。PEG与活性物质形成复合物的能力使其具有优良的溶剂能力。聚乙二醇也可被用来调节液体制药制剂的粘度、调节它们的吸收性能以及稳定制剂。
具有平均分子量达400的聚乙二醇在室温下为非挥发性液体。直到PEG600的液体PEG能与水以任意比例混合。但即使较高分子量的固体PEF等级在水中也有优良的溶解度。
聚乙二醇显示出了在急性和慢性口服毒性、胚胎毒性或皮肤兼容性方面的优秀的毒理学安全性,这通过肠胃外/吸收/排泄研究得到了支持。它们已经在化妆品、食品和制药工业中使用了很多年。这些化合物中的许多列于FAD非活性成分列表中用于处方产品。本发明的制剂包括多达25%重量的溶剂,通常为5-25%的重量。
PEG化合物的替代物为聚丙二醇。此外,醇类(包括乙醇、甘油和聚乙烯乙二醇等等)可作为辣椒素增溶剂被添加到制剂中。
表面活性剂
表面活性剂,例如非离子表面活性剂,如PS80和/或RH 40(聚氧乙烯40氢化蓖麻油);或者ELP或HS 15,可与溶剂(如上面所描述的)或作为溶剂的替代物一起用于本发明的制剂中。表面活性剂用作润滑剂和乳化剂,并可减轻与辣椒素给药相关的初始刺痛或灼热的不舒服感。
进一步地,可形成用于在共同拥有的美国专利8637569描述中的该发明的制剂和方法的表面活性剂/辣椒碱(或其他辣椒素)的浓缩物,通过引用所述美国专利8637569的全部内容并入于此。可在本发明的制剂中使用这些浓缩物。
本发明的制剂包括.001-2.5%重量的表面活性剂,通常为0.1-0.5%的重量。
镇痛剂和麻醉剂
在本发明的制剂中使用镇痛剂组分来改善或预防辣椒碱相关的初始急性灼痛或刺痛。在本发明中可以使用各种镇痛剂。
苯酚和薄荷醇
可在被治疗的手术切口、裸露的伤口或注射部位施用苯酚和/或薄荷醇。可在施用辣椒素之前施用苯酚和薄荷醇,或与辣椒素联合施用。弹丸注射苯酚和薄荷醇浓度的有效的24小时囊内胶囊浓度对于10ml关节液体注射体积应该为~25μg/ml或~250μg。
苯酚和薄荷醇(1)都会快速渗透到与注射剂接触的表面;(2)有助于减少或消除与TRPV1促效药(例如辣椒碱)给药相关的急性灼痛和刺痛。也可以用丁香油酚。本发明包括使用特定的可注射的镇痛剂(例如苯酚和薄荷醇),所述镇痛剂具有相对于辣椒碱的快速“起效”以有效地减缓辣椒碱的灼热感的作用。化合物的起效与其物理化学性质相关,一些药剂列于表III中。

当伴随给药时,这些具有快速起效的选择的镇痛药的使用有效地减缓了辣椒碱的灼热效果,而且提供了更直接的辣椒碱相关的疼痛缓解。在本发明的一个实施例中,注射的镇痛剂具有160或更小的分子量。辣椒碱提供了更长期/长持久性的与注射镇痛药相关的疼痛的缓解。
局部麻醉剂(例如利多卡因)通常添加到关节腔内注射中。它们被用于许多基础医疗手术中以产生短时间的麻木,包括被用于医生的办公室或医院以麻醉受伤的或需要小外科手术操作的区域。局部麻醉剂可添加到注射载体中以提供局部的疼痛缓解。麻醉剂的示例为利多卡因、布比卡因、罗哌卡因、辛可卡因、普鲁卡因、氯普鲁卡因、普鲁卡因、马比佛卡因、依替卡因、依替卡因和赛罗卡因。有利地,由于化合物起效的不同,在辣椒素给药2-20分钟之前,施用麻醉剂。
抗炎剂
抗炎剂任选地被包括在所述制剂中。
非类固醇抗炎药(NSAID)
非类固醇抗炎药(例如阿司匹林、萘普生、吲哚美辛、双氯芬酸钠、罗非昔布和布洛芬)抑制环氧合酶(COX-2抑制剂),并因此降低前列腺素的合成。前列腺素是过敏和炎症过程期间被释放出的炎症介质。NSAID完全阻止前列腺素被合成、减少或消除疼痛。
NSAID抗炎剂(例如双氯芬酸、阿司匹林、布洛芬、萘普生、苯恶洛芬、氟比洛芬、非诺洛芬以及其他等等)可被添加到可注射组合物中来提供局部的疼痛缓解,同时伴随最小程度的或无全身吸收。
皮质类固醇
在本发明的一个实施例中,在辣椒碱之前或同时地施用辅助剂例如皮质类固醇(糖皮质激素),以减轻来自施用辣椒碱药剂的初始急性灼痛和刺痛。一些最有效的抗炎药为皮质类固醇。皮质类固醇已经被证实通过抑制引起炎症的物质的产生来减轻炎症。可注射的皮质类固醇在疼痛管理中被广泛应用。这些药物凭借其抗炎特性可减少或消除疼痛的病灶。皮质类固醇注射可以很高效,因为其可以直接向炎症的部位输送药物。,皮质类固醇注射是减轻包括由来自TRPV-1神经末梢的炎症物质由于辣椒碱的存在引起的炎症的有效的方式。虽然不希望受限于理论,人们认为皮质类固醇给药通过安神和减少某些生物递质(例如P物质和缓激肽)的炎症反应起作用,其产生于辣椒素给药。
有多种皮质类固醇可以使用,包括地塞米松、醋酸甲基强的松龙、甲基泼尼松龙琥珀酸钠及其混合物。也可以使用本领域技术人员已知的其他等效的皮质类固醇。在一个实施例中,根据美国专利8637569(其全部内容通过引用并入于此),皮质类固醇的溶解度通过形成皮质类固醇的浓缩物(例如用PS 80或聚氧化40氢化蓖麻油)来增加。
可注射的皮质类固醇能够以水溶性或长效制剂获得。水溶性皮质类固醇通常不被用于关节腔内注射,因为它们会从注射区域快速地分散开,发挥全身作用。长效制剂会在被注射的部位保留较长的时间,维持局部效果,对关节注射有益。有各种可以加以使用的长效皮质类固醇,包括甲基强的松龙醋酸酯、倍他米松磷酸钠、倍他米松醋酸酯、醋酸氢化可的松、泼尼松龙(tebulate)、曲安奈德、己曲安奈德及其混合物。相比于较高溶解度的化合物,具有较低溶解度的皮质类固醇具有长时间保持有效的滑膜液水平并产生较低的全身水平的额外好处。
当皮质类固醇与辣椒素一起施用时,所述施用向被治疗的区域提供了灼痛和刺痛的缓解。皮质类固醇给药与辣椒碱或辣椒碱类似化合物给药产生了较轻的辣椒碱给药的疼痛,并且在该部位长时间(大于1个月,有利的是大于3个月,最有利的是大于6个月)的缓解疼痛。
在矫形外科中,具有有效抗炎性活性的皮质类固醇与辣椒素一起使用消除了或实质上减少了来自辣椒素给药的急性疼痛。这可以以安全并有效的方式延长疼痛的缓解。
水基辣椒碱可注射制剂–胶束/无
在表IV中所列的制剂依赖于增溶辣椒碱和镇痛剂的PEG和乙醇、苯酚以及薄荷醇。需要~15-40重量%PEG和~0-40重量%乙醇的名义上的含量来维持苯酚和薄荷醇的单相水溶液。

如在美国专利8637569的实施例V和实施例VIA至VID指出的,在水溶液中含有1mg/ml辣椒碱的组合物需要在水溶液中有10mg/ml的PS80。还需要指出,在PS80浓缩物中每增加10mg/ml,辣椒碱的水溶解度增加约1mg/ml。
然而,共同拥有的美国专利8637569的实施例意外地教导了在水溶液中需要一半(1/2)的PS 80浓度,即5mg/ml的PS 80溶解1mg/ml的辣椒碱。在相对水不溶的辣椒碱中以5mg/ml的PS 80获得1mg/ml浓度的比例表明辣椒碱被包含在胶束内。(见实施例1)因此,为了获得0.06mg/ml的辣椒碱浓度,仅需要0.3gm/ml的PS 80。
在表V中引用的制剂依赖于PS 80、苯酚和薄荷醇,所述PS 80通过增溶和胶束剂溶解辣椒素,并且PEG和乙醇的含量溶解镇痛剂。需要5–20重量%PEG和~0-40重量%乙醇来维持苯酚和薄荷醇的单相水溶液。

制备本发明制剂的方法
在下面的实施例1-6中讨论了制备本发明的制剂的方法。
使用本发明的组合物的方法和给药
本发明提供了用于治疗疼痛的水性辣椒素制剂的给药。在离散部位、手术部位或裸露的伤口施用有效量的单剂的辣椒素制剂,以去除注射部位、手术部位(例如腹腔镜检查)或伤口(例如骨折或韧带撕裂)神经。部位的示例包括关节、肌肉、腱、神经质或肿瘤。
本发明的制剂对1)缓解关节腔内部位或体间隙的疼痛;2)减轻随着卸下临床护理设施的患者所经历的术后疼痛;3)提供有效的术后痛觉缺失,是有用的,从而减少了患者摄入的镇定剂的量,因此减少了术后康复的时间。
本发明的组合物的用途和方法包括但不限于膝盖、肩部、臂部、背部、脊柱、颈、肘部、手、足的骨科障碍以及在特定部位涉及疼痛的其他障碍。关节腔内给药的示例包括膝盖、肘部、臂部、腕骨、跗骨、腕、椎间盘、踝以及关节炎状况的其他关节主体的注射。体间隙的示例包括囊或腹膜。
骨关节炎(OA)与软骨的减少有关。软骨由称作软骨细胞的特化细胞组成,所述软骨细胞产生大量的由胶原蛋白纤维组成的细胞外基质、富含蛋白聚糖的丰富的基质以及弹性蛋白纤维。
本发明的组合物可用于管理和延长缓解哺乳动物中与骨关节炎、类风湿性关节炎、肌腱炎、滑囊炎、术前和术后状况有关的伤害性疼痛,以及用于发生继发性神经损伤的神经性疼痛。在美国专利8420600和共同拥有的美国系列号13/609100中描述了给药的其他用途和方法,其全部内容通过引入并入本申请。
如在本发明中使用的“注射”的意思是通过皮肤向部位给药。“植入”的意思是通过嵌入一剂材料到皮肤、肌肉、腱或关节中在部位给药。
辣椒碱制剂可通过注射或渗透向部位给药。对于外科手术或伤口,向外科手术或伤口的部位周围的肌肉、组织或骨头施用药剂。当通过渗透进行辣椒素给药时,根据本领域技术人员已知的工具将辣椒素施用到手术部位或伤口以通过渗透来给药,例如针及注射器。
如在本发明中使用的“治疗上有效量”是指产生临床所需结果(例如生物应答)的或减轻疾病或状况的症状(例如减轻或消除疼痛)的药剂的数量或剂量。
当通过注射单剂进行辣椒碱给药时,辣椒碱的注射体积取决于给药的局部部位。被输送的合适的注射体积有利地在约0.1至约20ml,更有利地在约0.5至约10ml,以及最有利地在约1.0至约5ml,取决于被治疗的部位。
接下来是治疗关节痛并最小化或消除可注射辣椒碱的灼热和刺痛效果的本发明的可注射组合物有益的用途。为了减少感染的风险,对于注射,必须使用无菌技术。皮肤最先用聚维酮碘或其他合适的防腐剂清洗。
注射程序
采用下面的程序将辣椒碱制剂注射到关节(例如膝关节)中:
1.将针插入到关节,并抽出多余的液体。
2.将麻醉剂(1%利多卡因)注射到受影响的关节并允许渗透到周围的关节腔的表面约5至10分钟。在这段时间里,让主体走动、弯曲以及伸展(屈伸)膝盖来分散利多卡因。有利的是,在辣椒素制剂给药之前,没有从关节移走麻醉剂。在注射利多卡因之前、期间和/或之后,任选地将冷却手段(例如冰袋)施用于受影响的膝盖。
3.注射利多卡因5至10分钟后,以在5ml溶液中的0.2至0.5mg的剂量注射辣椒碱。在辣椒碱制剂注射之前、期间和之后,任选地将冰袋或凝胶袋或冷却装置应用于受影响的膝盖。为了在关节中分散辣椒碱,在第一个1分钟里(有利的为30秒),充分地反复弯曲和伸展膝关节,而主体在台上检查。也可以让主体走动以及弯曲膝盖。
注射辣椒碱后弯曲关节
显著地,辣椒碱给药后的第一分钟内,通过屈伸(弯曲和伸展)关节和行走活动关节有助于使制剂循环,并减轻灼痛和刺痛。
冷却
通过外部冷却手段(例如冰袋或冷却装置)冷却关节或给药部位也可以减轻辣椒素给药的疼痛效果。在一个实施例中,冷袋或凝胶袋产生高于结冰的温度,例如38-50华氏度。
本发明的治疗方法使用辣椒碱(例如反式辣椒碱)的剂量范围为0.001mg至1.0mg,有利的是0.2mg至0.4mg。所述方法使用体积约0.1ml至25ml的水性载体,有利的是5-10ml。用于关节或非关节应用的辣椒素和载体的量根据本领域的技术人员决定而变化。
本发明的组合物通过包括0.0002%-0.1%重量的辣椒素,有利的是0.01-0.05%重量的辣椒素。
本发明的制剂和方法提供了缓解疼痛2至5天,有利的是7至21天,或更有利的是6至8周,或者大于14周(例如大于1个月,有利的是大于3个月,以及最有利的是大于6个月)。
可以使用相似的技术来缓解不同于关节痛的疼痛。
下面的实施例用作说明,但不限于本发明的组合物和方法。通常会遇到多种条件和参数的其他合适的改变以及变通,这对本领域的技术人员是显而易见的,在本发明的精神和范围内。
实施例1
浓缩的辣椒碱/PS 80溶液的制备
美国专利8637569教导了需要5mg/ml的PS 80溶解1mg/ml的辣椒碱,即为了获得0.06mg/ml的辣椒碱浓度,需要仅0.3gm/ml的PS 80。而且,测定了在胶束中含有相对地水不溶性的辣椒碱,由于在辣椒碱溶解度方面,每增加1mg/ml,则需要5mg/ml的PS 80。在购自Croda公司的高纯度的PS80中增溶辣椒碱方面使用了该专利的教导。.
步骤I–辣椒碱和PS 80浓缩物的制备
成分包括:
10克的多山梨醇酯80(PS 80),超精,Croda公司,CAS#9005-65-6
2克的反式辣椒碱,Aversion Technologies公司,USP 30,95.7%反式-辣椒碱,平衡顺式辣椒碱
程序:
1.将10克的PS 80加入到50ml“Pyrex”的玻璃小瓶中。
2.将2克的反式辣椒碱加入到步骤1中的PS 80中。
3.将步骤2的混合物加热到~125℃以溶解反式辣椒碱从而形成反式辣椒碱/PS80浓缩物。
将少量的辣椒碱颗粒加入到室温的辣椒碱/PS 80溶液中没有导致形成辣椒碱沉淀,因此表明辣椒碱溶液没有过饱和。获得了意想不到的高浓度水平,即在1.0ml的PS 80中200mg的辣椒碱(200mg/ml)。将2ml的该辣椒碱/PS 80浓缩物加入到98克的~70℃的水中导致浑浊的溶液,当溶液混合物冷却到室温时,所述浑浊的溶液逐渐变得完全透明。因此将该20%辣椒碱/PS 80浓缩物加入到水中获得了4mg/ml的水性辣椒碱溶解度水平。相似地,将10ml的该辣椒碱/PS80浓缩物加入到90克的~70℃的水中获得浑浊的溶液,当溶液混合物冷却到室温时,所述浑浊的溶液逐渐变得完全透明。因此,将该2%辣椒碱/PS 80的浓缩物加入到水中获得了20mg/ml的水性辣椒碱的溶解度水平。
还需要注意的是,在PS 80浓度每增加5mg/ml,辣椒碱的水性溶解度增加约1mg/ml。不希望受到理论的限制,这表明在PS 80胶束中含有辣椒碱。
实施例2
200克含有来自实施例1的辣椒碱/PS80浓缩物和透明质酸的水性溶液的制备
步骤I-透明质酸溶液的制备

来自步骤I的192.8克的透明质酸。
程序:
1.向步骤I中制备的并包含在400cc Pyrex烧杯内的192.8克的透明质酸溶液中缓慢加入7.2克实施例1中制备的辣椒碱/PS 80浓缩物,同时充分搅拌。
2.为了后续的包装,现在准备好来自步骤1的混合物。
在充分混合所述成分之后,获得的混合物完全透明并且为中等粘度。混合物的粘度可通过改变透明质酸的含量来调节。
在PS80中的辣椒碱的高温增溶是重要的。多次尝试将PS80和辣椒碱加入到水中,接着加热到~70℃–90℃的温度导致只有一部分辣椒碱溶解。
如前文所描述的,在水性介质中获得增加辣椒碱溶解度水平的能力使得被注射的辣椒碱的液体体积显著减少。当与PS80(具有胶束增溶作用)表面活性剂相比时,采用PEG-400的实验没有获得辣椒碱的溶解度水平。
实施例3
200克的0.06mg/ml辣椒碱可注射溶液(无胶束)的制备

实施例6
HA/PEG/辣椒碱注射溶液的制备
在可注射制剂中使用的辣椒碱是用于药物用途的高纯合成的反式辣椒碱产品。台耀化学(Formosa Laboratories)为经过认证的制造商(DMF#16769)。马萨诸塞州的JohnsonCompounding of Waltham制备作为无菌液体的辣椒碱注射制剂,Bioventus LLC of Durham,NC供应用于制备无菌辣椒碱-HA注射溶液的无菌透明质酸(HA)制剂,透明质酸钠注射剂。辣椒碱溶液和透明质酸钠注射剂的制造和发行符合药品生产质量管理规范(GMP)。
用溶解于100%聚乙二醇300(PEG)的辣椒碱制备辣椒碱溶液。将由此获得的溶液无菌过滤并无菌地装入5ml的I型玻璃小瓶中,然后密封。辣椒碱溶液的组分和组合物列于表VII中。

实施例7
HA/PEG/辣椒碱溶液注射到膝盖中
患有疼痛的膝骨关节炎的72岁男性接受含有1%利多卡因的8ml溶液注射到他的左膝关节的关节腔内。在5ml的水性辣椒碱-HA无菌溶液关节腔内注射之前,通过行走和膝关节屈伸约5分钟使利多卡因溶液分布到整个关节腔。通过0.5ml的辣椒碱溶液与4.5ml含有透明质酸钠注射剂载体的HA混合获得0.2mg辣椒碱的辣椒碱剂量水平。在注射水性辣椒碱-HA无菌溶液之前,将膝关节预冷约3分钟,在后续的治疗过程中继续进行膝关节冷却。辣椒碱注射后,立即猛力地屈伸(弯曲和伸展)膝关节约1分钟来将关节腔内的水性辣椒碱-HA无菌溶液进行分散。疼痛水平一度上升到中等水平,通过休息以及屈伸膝关节快速消退。注射水性辣椒碱-HA无菌溶液后7分钟,完成程序,主体经历了骨关节炎疼痛症状的完全缓解。表IX总结了过程疼痛水平作为经过的时间的函数。

HA/PEG/辣椒碱溶液注射到膝关节中

Claims (37)
1.一种用于在哺乳动物中注射的水性组合物,包括:
0.0002%-0.1%重量的辣椒素,
99.9%-99.9998%重量的水性载体,所述水性载体包括溶解所述辣椒素的增溶剂以及在哺乳动物中注射所述组合物时使所述辣椒素从所述水性组合物中的释放减慢的延长释放剂,
其中,所述水性载体减轻或消除了在注射时所述辣椒素产生的灼热和刺痛。
2.根据权利要求1所述的水性组合物,其中所述辣椒素为辣椒碱。
3.根据权利要求1所述的水性组合物,其中所述辣椒素为反式辣椒碱。
4.根据权利要求1所述的水性组合物,其中所述辣椒素为含有非离子表面活性剂的浓缩物的形式。
5.根据权利要求1所述的水性组合物,其中所述增溶剂选自聚乙二醇-300、聚乙二醇-400和聚丙二醇组成的群组。
6.根据权利要求1所述的水性组合物,其中所述延长释放剂为透明质酸。
7.根据权利要求6所述的水性组合物,其中所述缓释剂为0.5%-1.5%重量的透明质酸。
8.根据权利要求6所述的水性组合物,其中所述透明质酸具有1000千道尔顿的平均分子量。
9.根据权利要求6所述的水性组合物,其中所述透明质酸为水凝胶的形式。
10.根据权利要求1所述的水性组合物,其中所述延长释放剂选自胶原蛋白、弹性蛋白和可生物降解的聚合物基体组成的群组。
11.根据权利要求1所述的水性组合物,包括:
0.005–0.05%重量的辣椒碱,
5–25%重量的聚乙二醇,以及
0.2–1%重量的透明质酸。
12.根据权利要求1所述的水性组合物,进一步包括非离子表面活性剂。
13.根据权利要求12所述的水性组合物,其中所述辣椒素为辣椒碱,所述非离子表面活性剂为聚氧乙烯40氢化蓖麻油。
14.根据权利要求12所述的水性组合物,其中所述辣椒素为辣椒碱,所述非离子表面活性剂为多山梨醇酯80。
15.根据权利要求14所述的水性组合物,其中辣椒碱与多山梨醇酯80的重量比为1至5。
16.根据权利要求1所述的水性组合物,进一步包括抗炎剂。
17.根据权利要求16所述的水性组合物,其中所述抗炎剂为皮质类固醇。
18.根据权利要求1所述的水性组合物,进一步包括镇痛剂。
19.根据权利要求18所述的水性组合物,其中所述镇痛剂为苯酚、薄荷醇和丁香油酚组成的群组中的一种或多种。
20.根据权利要求1所述的水性组合物,进一步包括麻醉剂。
21.根据权利要求1所述的水性组合物,包括:
0.005–0.05%重量的辣椒碱,
0.025–0.15%重量的多山梨醇酯80或聚氧乙烯40氢化蓖麻油,
0–25%重量的聚乙二醇300或400,以及
0.2–1%重量的透明质酸。
22.一种用于治疗哺乳动物部位疼痛的方法,包括:
向所述哺乳动物的所述部位施用治疗有效量的根据权利要求1所述的水性组合物。
23.根据权要求22所述的方法,其中所述哺乳动物为人。
24.根据权要求22所述的方法,其中在施用所述水性组合物之前施用麻醉剂。
25.根据权要求24所述的方法,其中以约0.1ml至25ml体积的药学上可接受的载体施用所述麻醉剂。
26.根据权要求22所述的方法,其中在施用所述水性组合物之后的第一分钟期间,重复弯曲和伸展关节。
27.根据权要求22所述的方法,其中在施用所述水性组合物之前、期间和/或之后,向所述部位使用冷却手段。
28.根据权要求22所述的方法,其中通过注射施用所述水性组合物。
29.根据权要求22所述的方法,其中通过渗透施用所述水性组合物。
30.根据权要求22所述的方法,其中所述疼痛源自伤口或手术部位。
31.根据权要求22所述的方法,其中辣椒素为0.001mg至0.5mg范围的反式辣椒碱水平。
32.根据权要求22所述的方法,其中以约0.1ml至25ml的体积施用所述水性组合物。
33.根据权要求22所述的方法,其中所述水性组合物包括辣椒素、透明质酸和聚乙二醇。
34.一种用于通过关节腔内注射施用辣椒素的试剂盒,包括:
a)至少一个单位剂量的麻醉剂,
b)至少一个单位剂量的辣椒素溶液,所述辣椒素溶液含有辣椒素和透明质酸,
其中所述试剂盒被设置为在所述辣椒素溶液之前施用所述麻醉剂。
35.根据权利要求34的试剂盒,进一步包括用于施用a)和b)的装置。
36.根据权利要求34的试剂盒,进一步包括给药说明书。
37.根据权利要求34的试剂盒,其中所述辣椒素、透明质酸和麻醉剂被配制在一起。
