本发明新型药物涂层球囊,包括球囊和涂在球囊上的药物涂层,所述药物涂层包括药物和药物载体,所述药物为组织因子途径抑制物,所述组织因子途径抑制物为TFPI‑1或TFPI‑2,药物与药物载体的质量比为1:2~3.9。所述球囊的外壁上有药物涂层。所述药物涂层厚度为8~15微米。本发明的优点在于:1.可降低病人PCI术后无复流的发生率。2.可使病人支架内血栓的发生率降低。3.明显减低病人再梗死率及病死率,改善病人长期预后。4.组织因子途径抑制物为组织因子的生理抑制剂,与其他药物相比,具有良好的血液相容性和组织相容性,无毒副作用,对人体影响小。

新型药物涂层球囊
技术领域
本发明涉及医疗用球囊,特别涉及药物涂层球囊。
背景技术
目前急诊经皮冠状动脉介入术已成为急性心肌梗死再灌注治疗的主要方法,能尽早开通梗死的冠状动脉血管,心肌坏死面积可相应缩小,进而降低患者的病死率。但在PCI手术过程中发现,梗死的相关动脉虽尽早实施了开通,但微循环却经常出现无复流现象,导致严重缺氧缺血的心肌组织得不到有效的血液灌注,这一现象使急诊PCI术的患者临床获益大大下降,致AMI患者易发生心力衰竭、心包渗出、心律失常,使病死率增高。
药物洗脱支架的出现虽然大大降低了支架内再狭窄率,但越来越多的研究表明,药物洗脱支架与金属裸支架相比支架内血栓的风险更高,从而造成严重不良的临床后果。目前临床上用于药物涂层支架的多为化学小分子药物,其在抑制平滑肌增殖的同时,也抑制内皮细胞的生长,造成血管壁内皮化不完全、细胞基质暴露,从而促进凝血反应,增加了支架内血栓形成这一梗死率及死亡率极高的并发症的发生率。冠脉支架植入后支架内血栓形成的再次心肌梗死率和病死率高,给患者和社会带来极大危害。
生理情况下,直接与循环血液接触的血细胞和内皮细胞不表达组织因子,介入手术过程中,支架内置有球囊,球囊扩张支撑使支架撑开,现有的球囊机械扩张时,内皮细胞受损,粥样硬化斑块破裂,使组织因子暴露与FVII结合,激活凝血系统,引起血小板活化,促进血小板之间相互粘附和聚集是PCI术后无复流及支架内血栓形成的重要原因。
发明内容
本发明的目的在于克服上述不足之处而提供一种新型药物涂层球囊,球囊上的药物涂层包括药物和药物载体,所述药物为组织因子途径抑制物,可以降低病人PCI术后无复流及支架内血栓的发生率,改善病人预后。
本发明采用如下技术方案。
新型药物涂层球囊,包括球囊和涂在球囊上的药物涂层,所述药物涂层包括药物和药物载体,所述药物为组织因子途径抑制物,所述组织因子途径抑制物为TFPI-1或TFPI-2,药物与药物载体的质量比为1:2~3.9,所述药物涂层厚度为8~15微米。
所述药物为TFPI-1。
所述药物为TFPI-2。
所述球囊的外壁上有药物涂层。
所述药物涂层,药物与药物载体的质量比为1:2或1:3.5。
所述药物涂层厚度为9微米或14微米。
本发明的优点如下:
本发明通过在球囊局部释放组织因子途径抑制物,抑制组织因子活性,避免凝血系统激活。具有以下有益效果:组织因子是外源性凝血途径的启动因子,与PCI治疗后无复流及支架内血栓形成有关。因此,将组织因子途径抑制物作为球囊的药物涂层成分,在球囊机械扩张的时候,通过球囊充盈,把药物渗透到血管组织及血液内,及时抑制受损内皮细胞暴露的组织因子的活性,可以减少PCI术后无复流的发生,预防支架内血栓形成,改善病人预后,而且由于该物质仅在支架内释放,作用效率高,对全身凝血系统影响小。
1.可降低病人PCI术后无复流的发生率。
2.可使病人支架内血栓的发生率降低。
3.明显减低病人再梗死率及病死率,改善病人长期预后。
4.组织因子途径抑制物为组织因子的生理抑制剂,与其他药物相比,具有良好的血液相容性和组织相容性,无毒副作用,对人体影响小。

具体实施方式
新型药物涂层球囊,包括球囊和涂在球囊上的药物涂层,所述药物涂层包括药物和药物载体,所述药为组织因子途径抑制物,其英文名为tissue factor pathwayinhibitor,所述组织因子途径抑制物为TFPI-1或TFPI-2,药物与药物载体的质量比为1:2~3.9。所述药物涂层厚度为8~15微米。
所述药物为TFPI-1。
所述药物为TFPI-2。
所述球囊的外壁上有药物涂层。
所述药物涂层,药物与药物载体的质量比为1:2或1:3.5。
所述药物涂层厚度为9微米或14微米。
所述药物载体为现有技术。常用的药物载体为二氯甲烷、乙酸乙酯、丙烯酸甲酯、含有甲基丙烯酸甲酯单体的共聚物、聚乳酸、聚乙醇酸、聚-消旋乳酸、聚丙交酯-乙交酯聚合物、磷酰胆碱、聚氨甲酸乙酯、无机微孔三氧化二铝、纤维素中的一种或一种以上的组合物。
关于本发明的药物涂层实施例如下。
实施例一:药物涂层中,所述药物,为组织因子途径抑制物TFPI-1,所述药物载体为二氯甲烷,药物与药物载体的质量比为1:2;药物涂层厚度为9微米。
实施例二:药物涂层中,所述药物,为组织因子途径抑制物TFPI-2,所述药物载体为聚乳酸,药物与药物载体的质量比为1:3.5;药物涂层厚度为14微米。
实施例三:药物涂层中,所述药物,为组织因子途径抑制物TFPI-1,所述药物载体为乙酸乙酯,药物与药物载体的质量比为1:3;药物涂层厚度为10微米。

1.一种新型药物涂层球囊,包括球囊和涂在球囊上的药物涂层,所述药物涂层包括药物和药物载体,其特征是:所述药物为组织因子途径抑制物,所述组织因子途径抑制物为TFPI-1或TFPI-2,药物与药物载体的质量比为1:2~3.9,所述药物涂层厚度为8~15微米。

3.按照权利要求1所述的新型药物涂层球囊,其特征在于:所述药物为TFPI-2。

5.按照权利要求1所述的新型药物涂层球囊,其特征是:所述药物涂层,药物与药物载体的质量比为1:2或1:3.5。
