Abstract

一种新型肱骨中下段前外侧解剖锁定接骨板,分为近侧部、移行部和远侧部。近侧部位于肱骨干中段的前外侧,包含3、4、5、6、7、8对螺钉孔,每一对螺钉孔均由锁定孔联合滑动加压孔构成;移行部位于肱骨中下1/3交界处的前外侧,包含2个锁定孔,呈向前外7°的屈曲及向后20°的扭转延续至远侧部;远侧部位于肱骨下段的前外侧,包含4个锁定孔,呈双排对称分布。接骨板的远、近端4个角,分别有一个直径2mm的圆孔,用于临时放置克氏针。本发明符合肱骨中下段前外侧的解剖特点,使得螺钉可以同时垂直于接骨板和骨面置入。近侧部滑动加压孔可以实现滑动加压作用。远侧部双排钉孔设计可以允许置入更多的螺钉,提供更加可靠的固定。

Description

新型肱骨中下段前外侧解剖锁定接骨板
技术领域:
本发明涉及一种医用接骨板,放置于肱骨中下段的前外侧,用于固定肱骨中下段骨折。
背景技术:
肱骨中下段解剖形状特殊,为圆柱形与三棱形之交界处,是肱骨力学上的薄弱点,为骨折的好发部位。切开复位钢板内固定手术在肱骨中下段骨折的手术治疗中一直占主导地位,内固定材料包括加压钢板、桥接钢板、锁定钢板及锁定加压钢板等。肱骨前外侧入路是临床上常用的入路,能够很容易的在肱桡肌和肱肌间隙内找到桡神经并加以保护,从而避免桡神经的损伤,故肱骨的前外侧面是放置钢板的较为理想的位置。然而肱骨中下段前外侧面并不是一个平直的平面,由近及远有一个向前外的弧度及一个向后的扭转,故普通的直型接骨板不能很好的贴附于骨的表面,螺钉拧入时易造成复位的丢失。同时若骨折线位置较低,普通直型接骨板在骨折远端不能提供足够多的螺钉固定,难以达到可靠的固定强度。因此有必要设计与成人肱骨干中下段解剖形态相匹配的解剖锁定接骨板用于肱骨干中下段骨折的治疗。
发明内容:
为了解决现有肱骨直型接骨板难以与骨面良好贴附、骨折远端固定螺钉少的问题,本发明提供一种肱骨中下段前外侧解剖锁定接骨板,依据肱骨中下段的解剖特点进行设计,实现接骨板与骨面的良好贴附,增加骨折远端固定螺钉的熟练,提高复位固定后的稳定性。
本发明解决其技术问题所采用的技术方案是:本发明由接骨板、锁定孔、滑动加压孔、克氏针孔构成。接骨板分为近侧部、移行部和远侧部,近侧部平直,位于肱骨干中段的前外侧,宽度为13mm,根据接骨板的不同长度,分别包含3、4、5、6、7、8对螺钉孔,每一对螺钉孔均由锁定孔联合滑动加压孔构成,滑动加压孔位于锁定孔的近侧;移行部位于肱骨中下1/3交界处的前外侧,包含2个锁定孔,呈向前外7°的屈曲及向后20°的扭转延续至远侧部;远侧部位于肱骨下段的前外侧、肱骨外上髁嵴的内侧,远侧部较近侧部宽且薄,包含4个锁定孔,呈双排对称分布于接骨板上。在接骨板的远、近端4个角处,分别有一个直径2mm的小圆孔,用于临时放置克氏针。
本发明的有益效果是,符合肱骨中下段前外侧的解剖特点,能够良好的贴附于骨面,使得螺钉可以同时垂直于接骨板和骨面置入,避免螺钉置入时复位的丢失。近侧部滑动加压孔允许置入普通皮质骨螺钉,实现接骨板的滑动加压作用。远侧部双排钉孔设计可以允许置入更多的螺钉,提供更加可靠的固定。接骨板两端的克氏针孔允许在置入螺钉前用克氏针临时固定接骨板与骨骼的相对位置,便于接骨板位置的调整,并避免置钉过程中接骨板的移位。

具体实施方式:
本接骨板应用于肱骨中下段骨折,骨折复位后,根据骨折线长短选择长度合适的接骨板,将接骨板近侧段放置于骨折近端与肱骨干前外侧骨皮质相贴附,移行段跨过骨干形态移形处,远侧段放置于骨折远端前外侧面与骨皮质相贴附,接骨板最远端不要低于鹰嘴窝上缘,以免螺钉打入鹰嘴窝内。骨折复位后放置接骨板,在接骨板远近端克氏针孔内置入克氏针,将接骨板临时固定于骨面,透视观察接骨板位置,位置满意后,根据骨折情况于接骨板合适位置置入螺钉固定。

1.一种用于固定肱骨中下段骨折的新型前外侧解剖锁定接骨板,包含有锁定孔、滑动加压孔和克氏针孔,其特征是:接骨板分为近侧部、移行部和远侧部,近侧部平直,位于肱骨干中段的前外侧,根据接骨板的不同长度,分别包含3、4、5、6、7、8对螺钉孔,每一对螺钉孔均由锁定孔联合滑动加压孔构成,滑动加压孔位于锁定孔的近侧;移行部位于肱骨中下1/3交界处的前外侧,包含2个锁定孔,呈向前外7°的屈曲及向后20°的扭转延续至远侧部,远侧部位于肱骨下段的前外侧、肱骨外上髁嵴的内侧,远侧部较近侧部宽且薄,包含4个锁定孔,孔呈双排对称分布于接骨板上。在接骨板的远、近端4个角处,分别有一个直径2mm的小圆孔,用于临时放置克氏针。
