背景

人免疫缺陷病毒(HIV)转录反式激活因子(Tat)是可变的RNA结合肽,其提高病毒RNA转录并且可以引起T4细胞和巨噬细胞的凋亡,还可能刺激α干扰素的过量生产。然而,从经HIV感染的长期无进展者(LTNP)中分离的Tat蛋白与在由于HIV感染已经发展成获得性免疫缺陷综合症(AIDS)的患者中发现的Tat不同。在LTNP中发现的Tat蛋白能够反式激活病毒RNA;然而,这种免疫刺激性Tat不诱导T4细胞或巨噬细胞的凋亡且无免疫抑制性。在感染猴物种的慢病毒中发现的免疫刺激性Tat的变体也不导致免疫缺陷的发展且流行性感染指导单核细胞分化为刺激细胞毒性T淋巴细胞(CTL)应答的树突细胞(DC)。因此,免疫刺激性Tat可用于刺激对人类癌症的免疫应答。
癌症和慢性感染是对基于免疫的治疗应答的常见人类疾病的最突出的例子。尽管感染是要通过免疫来控制的首要疾病,但是人类中的临床试验已经确定了免疫应答尤其是免疫系统的CTL臂能够使一些人类黑色素瘤和肾癌消退。这些观察结果被下述发现扩展:一类特别的抗原呈递细胞(APC)DC特别有效地引起针对癌症和其它疾病的CTL活性。靶向和激活DC的技术已经导致针对由人类乳头状瘤病毒(HPV)感染引起的人类宫颈前恶性肿瘤和人类肺癌的一些早期成功。与目前针对癌症使用的化学治疗药物相反,激发针对癌症的CTL应答的药剂由于免疫应答的巨大特异性而伴随着极少的副作用。
开发治疗癌症的免疫治疗药物的努力被下述技术困难阻碍:靶向和激活DC,从而将所需的进入信号递送并维持至CTL。诱导CTL应答的抗原靶向在下述范围内是一种挑战:天然加工需要抗原进入细胞的细胞质,从而结合免疫系统的主要组织相容性复合物(MHC)I类抗原,这是CTL激活的必要条件,因为激活CTL上T细胞受体的配体是抗原和MHCI类的复合物。在几乎所有情况下,蛋白质抗原甚至在它们与DC辅助激活因子偶联时排他地进入取代性的MHCII类抗原呈递途径,其排除CTL刺激。这可以部分通过基于肽的技术来克服,因为肽结合已经位于DC表面上的MHCI类。然而,该技术是非特异性的,并且大部分肽是差的DC激活因子,这限制了它们作为人类癌症治疗的功效。
已知有限的一组生物蛋白质刺激CTL应答。人免疫缺陷病毒1(HIV-1)转录反式激活因子(Tat)的变体和衍生物能够刺激该CTL应答。目前已知直接引发CTL应答的其它生物品以热休克蛋白(HSP)为基础,或者以某些细菌的外壳蛋白为基础。在与HPV感染相关的某些生殖器瘤的治疗中,热休克蛋白显示有限的功效。
发明概述

在Tat衍生多肽的一个实施方式中,HIV是HIV-1或HIV-2。在另一个实施方式中,HIV-1Tat来自长期无进展者。在另一个实施方式中,SIV来自选自表2的宿主。在另一个实施方式中,防御素是α-防御素或β-防御素。在另一个实施方式中,Tat衍生多肽还包含来自HIV-1或HIV-2Tat的富精氨酸结构域。

发明详述
设计了在癌症中有活性的一系列人工人类免疫缺陷病毒(HIV)转录反式激活因子(Tat)蛋白衍生物。所述分子在本文中被称作“Tat衍生多肽”、“Tat衍生物”或“精确免疫刺激物”(PINQ),并且包括下述Tat分子,所述Tat分子具有将Tat从免疫抑制性改变为免疫刺激性的修饰。
尽管具有相对丰富的肿瘤相关抗原,但是癌症已被证实是一种困难的免疫疗法靶标。已积累了下述证据:癌症对免疫疗法的难治状态可源自伴随确立的癌症的免疫抑制。流行病学研究显示,患有HIV感染甚至获得性免疫缺陷综合征(AIDS)的女性被矛盾地保护免于发生乳腺癌,即使在免疫缺陷显著的晚期疾病中也是如此。

Tat衍生多肽
HIVTat蛋白是长度为86到110个氨基酸的可变RNA结合蛋白,其在HIV基因组的两个分开的外显子上编码。Tat在所有人类慢病毒之间是高度保守的,并且是病毒复制所必需的。慢病毒Tat与TAR(反式激活响应性)RNA区结合时,转录(病毒RNA到DNA随后到信使RNA的转化)水平显著提高。已经证实Tat提高病毒RNA转录,已经提出Tat可在T4细胞和巨噬细胞(针对HIV感染的机体免疫监督系统的关键部分)中引起凋亡(程序化细胞死亡),并且可刺激α干扰素的过量生产(α-干扰素是一种公认的免疫抑制性细胞因子)。
HIV感染过程早期的细胞外Tat存在可降低患者的免疫应答,为病毒提供超过宿主的优势。另外,Τ4细胞的直接破坏和α-干扰素生产的诱导能够帮助解释在AIDS患者中观察到的强烈细胞免疫应答的缺失,以及说明初始时剧烈的免疫抑制。
基于分子分析,Tat蛋白(SEQIDNO:1)包含四个不同的结构域:(1)转导(SH3)结构域(第3-19个氨基酸);(2)富半胱氨酸配体结合结构域(第22-37个氨基酸);(3)膜转位序列(第47-57个氨基酸)和(4)由第二外显子编码的尾巴部分(第73-101个氨基酸)。
Tat的氨基端部分包括来自核转录因子(TF)的短肽区,其侧翼典型地是脯氨酸残基。该区域至少部分决定了Tat多肽对免疫系统细胞、尤其是天然免疫细胞例如树突细胞(DC)和巨噬细胞(抗原呈递细胞或APC)有多刺激或有多抑制。因此,预测对TF区的修饰能够使多肽在癌症和其它慢性疾病的治疗中更有活性。
HIV-1TatSH3结合结构域与在另一TF蛋白无毛(hr)中发现的序列相同,所述无毛(hr)先前被显示在小鼠中具有免疫抑制性质。带有hr突变的小鼠发生免疫失调,目前最常被称作“TH1到TH2的转变”,这是被HIV感染的个体发展成为AIDS的必要条件。进一步的分析确定,源自hr基因的SH3结合序列是从HIV-1分离的Tat的几乎不变的特征,以及HIV-2的非常一致的特征。
相反,被与HIV密切相关的一种慢病毒—猴免疫缺陷病毒(SIV)的某些毒株感染的灵长动物很少发展成AIDS,或者其无法预测。该观察结果与免疫抑制性HIV-1Tat中推定为免疫抑制性的hrTF片段的发现结合,表明一些灵长动物会在SIVTat的氨基端具有不同的(或不具有)TF片段。来自某些具有衰减的免疫缺陷过程的被SIV感染的乌黑白眉猴的Tat在其氨基端具有来自TFTARA而不是TFhr的片段。

推翻(override)免疫检查点
免疫检查点,例如肿瘤特异性T细胞上表达的细胞毒性T-淋巴细胞抗原4(CTLA-4)和程序性死亡1(PD-1),导致受损的激活和抑制的效应子功能例如增殖、细胞因子分泌和肿瘤细胞裂解。特别地,利用免疫检查点抑制剂调节这些受体是癌症免疫疗法中的新方法。
PD-1(又名CD279)、及其配体结合伴侣PD-L1(又名B7-H1和CD274)和PD-L2(又名B7-DC和CD273)提供了重要的调节T细胞激活的负面共刺激信号。PD-1与CD28和CTLA-4相关,但缺少允许同源二聚化的近膜半胱氨酸。PD-1的胞质结构域含有免疫受体酪氨酸基抑制基序(ITIM,V/IxYxxL/V)。到目前为止,仅有的鉴定出的PD-1配体是PD-L1和PD-L2。
肿瘤微环境的免疫抑制属性有助于解释伴随癌症发展的免疫功能失调。PD-1/PD-L1信号通路是肿瘤位点免疫逃避的一个新型模型并且代表了有效免疫应答的重要检查点和屏障。
由于用来重新编程肿瘤微环境中存在的宿主细胞的活性肿瘤介导过程,肿瘤位点中PD-L1的存在被认为促进免疫逃避。表面浸润T-细胞上PD-L1及其PD-1受体的参与可诱导T-细胞程序性细胞死亡、不应和耗竭。肿瘤微环境中PD-L1的诱导可充当保护肿瘤免受细胞介导的免疫应答的“分子防护”。
癌症对免疫治疗的不应状态可以是伴随确立的癌症中疾病发展的免疫抑制的结果。本文所公开的Tat衍生多肽通过触发单核细胞来源的树突细胞来刺激CD8+CTL和推翻PD-L1免疫抑制从而引起抗肿瘤免疫应答。因此,PD-1/PD-L1免疫抑制信号转导通路可以提供乳腺肿瘤逃避宿主肿瘤免疫的潜在机制,因此在面对PD-L1免疫抑制时,Tat衍生多肽可以通过诱导肿瘤浸润CD8+CTL来影响实体瘤的发展。
通过PD-L1调节信号转导,从而防止PD-L1向T细胞发送负面共刺激信号可能增强应答感染(例如,急性感染和慢性感染)的免疫和肿瘤免疫。此外,本文所公开的Tat衍生多肽可以与PD-1:PD-L1信号转导的其它组分的拮抗剂(例如,拮抗剂抗-PD-1和抗-PD-L2抗体)组合。

Tat衍生多肽的用途

通过肿瘤和入侵免疫细胞的PD-L1的表达和存在可用来预测对疾病的治疗和/或预后的应答。因此,在本文所公开的一个实施方式中,受试者基于其肿瘤组织中PD-L1的表达被选择为利用Tat衍生多肽来治疗。在某些实施方式中,在利用任何癌症疗法治疗受试者之前,评估肿瘤组织的PD-L1表达。在另一个实施方式中,在用本文所公开的Tat衍生多肽治疗受试者之前,评估肿瘤组织的PD-L1表达。
PD-L1表达可以通过肿瘤组织的免疫学分析来测定,所述免疫学分析例如但不限于,免疫组织化学、免疫测定(ELISA,、ELISPOT、放射免疫测定)、蛋白微阵列、流式细胞术、定量免疫荧光和表面等离子共振。也可以采用非免疫学测定例如定量聚合酶链式反应(qPCR)和信使RNA测定。
因此,在一些实施方式中,如果治疗前肿瘤含有多于5%表达PD-L1的细胞、多于6%表达PD-L1的细胞、多于7%表达PD-L1的细胞、多于8%表达PD-L1的细胞、多于9%表达PD-L1的细胞、多于10%表达PD-L1的细胞、多于11%表达PD-L1的细胞、多于12%表达PD-L1的细胞、多于13%表达PD-L1的细胞、多于14%表达PD-L1的细胞、多于16%表达PD-L1的细胞、多于18%表达PD-L1的细胞或多于20%表达PD-L1的细胞,则患者被选择为利用Tat衍生多肽治疗。
药物组合物
本公开还涉及包含上述Tat衍生多肽的药物组合物。所公开的药物组合物的剂量和期望的药物浓度可根据展望的具体用途而变化。适当剂量或施用途径的决定完全在普通医生的技术范围内。动物实验提供了测定用于人治疗的有效剂量的可靠指导。可以按照ToxicokineticsandNewDrugDevelopment,Yacobi等人编,PergamonPress,NewYork1989,第42-96页中Mardenti,J.和Chappell,W.“Theuseofinterspeciesscalingintoxicokinetics”制定的原则进行有效剂量的物种间缩放。在一个实施方案中,存在疾病。在另一个实施方案中,细胞或个体的寿命由于本文所述的方法而被延长。
可以按照对具体应用所适用的配制上述Tat衍生多肽来施用给哺乳动物(包括人),而无需过度实验。另外,可以使用标准剂量-应答流程来确定组合物的适当剂量,而无需过度实验。

透皮施用包括通过皮肤经皮吸收组合物。透皮配制物包括贴片、离子电渗装置、软膏剂、霜剂、凝胶、油膏剂等等。
组合物可包括修饰固体或液体剂量单位的物理形式的多种材料。例如,组合物可包括在活性成分周围形成包衣壳的材料。形成包衣壳的材料典型地是惰性的,并且可以选自例如糖、紫胶和其它肠溶包衣试剂。或者,活性成分可以被包装在明胶胶囊或扁囊中。
本公开的Tat衍生多肽组合物可以根据适当的给药方案,以治疗有效量被施用。如本领域技术人员所理解的,所需的精确量可因受试者而异,这取决于受试者的物种,年龄和一般条件,感染的严重性,具体的药剂和施用模式。在一些实施方案中,每天一次或多次,基于受试者的体重施用约0.001mg/kg到约50mg/kg的组合物,以获得期望的治疗效果。在另一些实施方案中,每天一次或多次,基于受试者的体重施用约1mg/kg到约25mg/kg的组合物,以获得期望的治疗效果。
组合物的总日剂量将由主治医师在合理的医疗判断范围内决定。针对任何具体患者或受试者的特异性治疗有效剂量水平会取决于多种因素,包括被治疗的疾病和疾病的严重性;使用的特定化合物的活性;使用的特定组合物;患者或受试者的年龄、体重、一般健康、性别和饮食;施用时间、施用途径和使用的特定化合物的排泄速率;治疗的持续时间;与使用的特定化合物组合使用或同时使用的药物;和医学领域中公知的其它因素。
所公开的组合物也可以在组合疗法中使用。也就是说,本文公开的组合物可以在一种或多种其它期望的组合物、疗法、治疗或医学程序的同时、之前或之后被施用。所施用的具体疗法组合将由主治医师决定,并且会考虑治疗的相容性和期望要实现的治疗效果。应当理解,组合使用的治疗活性剂可以在单一组合物、治疗或程序中一起施用,或者可以单独施用。
在另一个实施方案中,考虑了所公开的Tat衍生物的重复性给药或频繁给药,这可在快速减敏之前进行,以及逆转在癌症发展期间确立的免疫抑制性趋势。频繁给药是例如在变态反应疗法中使用的一种程序,其能够支持对药剂的免疫学耐受。一旦Tat衍生物能够被用于恢复对肿瘤的免疫反应性,则由于晚期疾病而丧失了益处的其它免疫疗法能够潜在地重获功效。在第二方案中,使用化学治疗方案,所述方案能够与Tat衍生物免疫疗法交替地释放大量肿瘤抗原。因为晚期阶段的人类癌症往往耐受多种药物,所以放射性疗法可以是人类试验中实用的替代方案。
Tat衍生多肽的重复给药次数可以由医学专业人士基于患者对剂量的应答而确立。在一个实施方案中,Tat衍生多肽每3天施用一次,在10天的时间段中施用3剂。然后将该施用方案重复多个循环。本公开考虑了多种不同的施用方案,其中Tat衍生多肽在选择的时间框内被施用多次,然后将所述施用方案重复多个循环。在另一个实施方案中,Tat衍生多肽的施用可以与一种或多种其它抗癌剂、免疫调节剂或免疫抑制剂的施用交替进行。在一个实施方案中,免疫抑制剂是环磷酰胺。

Tat衍生多肽在乳腺癌中的效果
在三种不同的被广泛接受的鼠乳腺癌模型:4T1、SM1和TS/A中用重组生产的Tat蛋白衍生物进行的动物试验提供了下述支持:Tat衍生物在小鼠中抑制原发性乳腺癌生长是有活性的。另外,一种衍生物Nani-P2显著地抑制了自发性4T1肺转移的发生,并且与对照小鼠相比提高了存活。显著地,用Tat衍生物治疗鼠乳腺癌伴随着提高的IFN-γ生产水平。在研究中,通过与经PBS处理的对照相比时原发性肿瘤生长、存活和转移性肺负担的降低来评价时,在开始治疗之前已接种过十四天4T1乳腺癌时,Tat衍生物与在肿瘤植入时给予的情况同等有效。

本文报道的研究使用通过IV或SC任一给予的每周约三剂或四剂Tat衍生物的流程,其中IV施用证实对于提高存活和降低转移最为有效。在给予了这些组合物的超过250只的小鼠中未观察到毒性。与4-8mg/kg为标准疗法的(Genentech)剂量应答曲线相比,乳腺癌对Tat衍生物的灵敏度有利地与之不同。估计Tat衍生物在人中的生物活性为在小鼠中的多达100倍,这表示:与进一步更低的毒性风险相关的进一步更低的剂量可能证明是成功的。
本文中确定了Tat衍生物激活抗癌T细胞免疫应答的INF-γ臂(图5)。由来自用Nani-P2处理的小鼠脾细胞分泌的INF-γ基线水平为来自用PBS处理的对照小鼠的8倍。可以通过天然免疫激动剂GM-CSF和IL-4体外额外地增强(高达53倍)应答体内Tat衍生物处理的INF-γ分泌,但来自对照小鼠的脾细胞即使在尝试用高剂量的GM-CSF和/或IL4共同刺激后仍保持被抑制。
更致免疫的乳腺癌模型(SM1)和/或具有免疫优势表位的乳腺肿瘤(TS/A)在Tat衍生物疗法后具有相对高的消退率,而“非致免疫”的4T1模型更加难治。这与下述模型相一致:免疫抑制是乳腺癌发展中的主要因素,事实上可有助于乳腺癌侵袭。该模型得到下述观察结果的支持:4T1以高水平表达一些常见的乳腺癌抗原,包括乳黏着蛋白和雄激素结合蛋白,在没有Tat衍生物诱导的反抑制时针对所述抗原的免疫应答显然被完全抑制。
实施例1
Tat衍生物的体外活性
将人单核细胞与Tat衍生物(Nani-P2)、toll样受体(TLR)的免疫刺激性序列(ISS)(图1)或脂多糖(LPS)(图2)—起培养24-48小时,然后洗涤细胞并用荧光标记的CD86染色。Tat衍生物刺激了比ISS(TLR)或LPS任一更高的CD86表达。
实施例2
在乳腺癌小鼠模型中评价Tat衍生物
材料和方法
动物。6到8周龄的雌性BALB/c小鼠购自Jackson实验室(BarHarbor,NE)。使用前使小鼠适应至少1周。在哥伦比亚大学医学中心的动物维护设施(AnimalMaintenanceFacility)中将小鼠保持在无病原体的条件下,并且所有实验由哥伦比亚大学医学中心的机构动物保护和利用委员会(InstitutionalAnimalCareandUseCommittee)批准。
细胞系。源自BALB/c自发性乳腺癌的6-硫鸟嘌呤抗性细胞系4T1细胞得自ATCC;鼠腺癌细胞系TS/A由SandraDemaria博士(DemariaS.等人ClinCancerRes.11:728-34,2005)提供;源自BALB/C的乳腺癌SM1由伯克利加利福尼亚大学的JamesAllison博士友好提供。所有肿瘤细胞系在补充了2mML-谷氨酰胺、10mMHEPES、150单位/ml青霉素/链霉素、10%热失活的FCS(Invitrogen)、50μM2-巯基乙醇(Sigma)和50mg/L庆大霉素(Lanza)的DMEM中培养。
肿瘤攻击和治疗。第0天在左乳腺垫中分别用1x104个4T1、1x105个TS/A或2x105个SM1细胞(SC)注射BALB/c小鼠。通过在确立肿瘤后第0、7、12和17天将Tat衍生物直接注射进右肋腹中进行免疫治疗。对照组接受PBS注射。在一些实验中,当所有小鼠具有确立的可测量的肿瘤(肿瘤注射后14天直径3-5mm)时,将动物随机指定至所示的多个处理组中。每周三次测量并记录肿瘤负荷(肿瘤体积)。当肿瘤达到直径15mm的体积时处死动物,收获肿瘤并称重。
检测肺转移。如先前所述检查肺的4T1转移(PulaskiB.等人.CancerRes.60:2710-2715,2000)。在绝大多数动物中,BALB/c小鼠中已确立2-3周的原发性4T1肿瘤转移至肺。简而言之,在试验开始时根据确立的IACUC指南处死小鼠,取出肺,由对处理流程不知情的两个独立的研究者用肉眼计数肺表面上的肿瘤结节。
免疫脾细胞的IFN-γ生产的ELISA分析。通过OptEIATMELISA试剂盒(BDBiosciences)评价IFN-γ的脾细胞分泌。简而言之,在96孔板中,以20:1的E:T(效应物:肿瘤)比例在含IL-2(50ng/mL)和GM-CSF(100ng/ml)的DMEM中,使用或不使用5x103/孔经丝裂霉素C(50μg/ml)处理的4T1细胞(用于提供肿瘤抗原)地培养来自带4T1肿瘤小鼠的脾细胞(1x105/孔)。72小时后收集上清液并在-80℃下保持冷冻直至分析,而不丧失活性。根据制造商的说明,通过ELISA在一式两份孔中无细胞的上清液中测量IFN-γ。如下计算肿瘤特异性IFN-γ生产:减去在单独使用培养基培养的脾细胞上清液中测量的背景值,并使用重组的IFN-γ标准曲线将光密度(OD)数值转化成以pg/ml为单位的IFN-γ量。刺激指数(SI)被计算为被刺激的培养物与对照培养物相比的IFN-γ比例。
统计学分析。使用Student'st-检验(GraphPadPrism第5版;GraphPad)对数据进行统计学分析。使用时序检验(GraphPadPrism第5版)对来自动物存活实验的数据进行统计学分析。
结果
研究了乳腺癌鼠模型中合成的、源自Tat的组合物的全身性施用的疗效。为了比较一小组不同衍生物针对原发性乳腺肿瘤生长所赋予的相对保护,将1x104个4T1乳腺肿瘤细胞SC注射进雌性BALB/c小鼠的乳腺垫中,然后在第0、7、14和21天用400ng部分纯化的Tat衍生物处理(PBS中SC注射)进腋下引流淋巴结中。
与单独接受PBS注射的对照小鼠相比时,衍生物中的两种(Nani-P1和Nani-P2)显著地减小了肿瘤负荷,所述差异在肿瘤植入后第15天首次开始明显(图3A,第15天p<0.05)。相比之下,用与其它衍生物相同的流程生产并部分纯化的第三种衍生物Nani-P3即使在五倍高的剂量(2μg,图3B)下在抑制4T1原发性肿瘤生长或者延长存活(未显示)方面都效力更低。这些结果有效地排除了制剂中的污染物有助于抗肿瘤功效,尤其是在随后的试验用低得多的剂量下高度纯化(>95%纯)的材料进行时。Nani-P2的功效比Nani-P1显著更持久,因而在第21天(最后一次给药)时,经Nani-P2和Nani-P1处理的肿瘤之间原发性肿瘤负荷的差异变为18mm3,并且是高度统计学显著性的(p<0.01)。尽管不再有进一步疗法,但是该效应持续该试验的整个剩余部分。
高度纯化的Nani-P2对4T1肿瘤的乳腺肿瘤生长抑制效应是剂量依赖型的,SC施用低至0.4ng化合物之后观察到显著的效应(图4)。将在引流腋窝侧翼(drainingaxillaryflank)SC施用的Nani-P2剂量通过对数增量从0.4ng每剂递增至40ng每剂逐渐地抑制了4T1乳腺肿瘤生长。0.4ng到40ng之间更高剂量Nani-P2下更强的4T1生长抑制是统计学显著性的(p<0.01),而将剂量递增至400ng甚至2μg不导致进一步的抗肿瘤功效(数据未显示)。重要的是,在使用多次给药方案的多次试验中SC或IV施用40ngNani-P2后,未观察到毒性。选择40ngNani-P2的剂量用于进一步研究。
为了测定Nani-P2处理在小鼠中是否能够除了缩小原发性肿瘤之外还延长存活,进行了使用多种给药方案和途径(SC、IV或IT)施用40ngNani-P2的处理流程。针对每组十只小鼠的小组监督存活长度,所述存活长度使用Kaplan-Meier产物限制方法来评价。按照哥伦比亚大学医疗中心动物研究所规程,在约15mm平均肿瘤直径下使每只小鼠安乐死,或者在小鼠濒死时更早,使得这两种结果之一成为死亡率的定义标准。
在评价Nani-P2的第一个试验中,与肿瘤植入同时开始SC处理。对照(经PBS处理的)小鼠的中位存活时间是30天,100%的死亡率发生于第36天。通过Nani-P2施用(21天中4剂),35%经处理的小鼠在第48天仍然存活(p<0.001,图5A),此时所有小鼠由于原发性肿瘤负荷而被处死。
在第二个存活试验中,允许肿瘤确立十四天,以更好地评价在转移性疾病中的功效,之后通过若干途径(SC、IV或IT)之一每周施用三个循环的Nani-P2疗法,以比较每种给药途径的相对功效(图5B)。与先前的试验相似,对照(经PBSSC处理)小鼠的中位存活是32天,第36天100%死亡。IV施用Nani-P2延长了存活(p<0.005,图5B),在第47天有60%存活,与之相比,经SC处理的小鼠在第47天有20%存活(p<0.05)。化合物的瘤内施用略次于SC施用。
选择4T1鼠乳腺肿瘤模型进行研究,因为这是一种侵略性和迅速浸润的肿瘤;其通常在植入后第十四天转移,此时已难以治疗。为了学习Nani-P2的效能是否能够扩展至其它鼠乳腺肿瘤模型,研究了两种额外的乳腺肿瘤TS/A和SM1(图6)。TS/A原发性乳腺肿瘤大致与4T1同样具有侵略性,在第30天达到15mm的肿瘤体积(图6A)。然而,TS/A肿瘤可观地更加应答于Nani-P2处理,用0.4ngNani-P2处理后具有约50%的生长抑制,第30天总计40%消退率。
SM1乳腺癌模型(图6B)最初作为原发性肿瘤侵略性较低,并且显示死亡通过除转移疾病之外的机制发生。在处理的第30天,SM1肿瘤达到比TS/A或4T1任一小约33%的平均体积。这表示与4T1相比,SM1肿瘤对Nani-P2免疫疗法具有提高的灵敏度,从而在16天中100%的动物中肿瘤生长被抑制,甚至在植入后28天和终止给药方案后整整一周,40%的动物仍保持减轻。
为了测定细胞毒性T淋巴细胞在Nani-P2疗法诱导的肿瘤排斥中是否起作用,进行了IFN-γELISA测定(图7),以比较不用(对照)或用Nani-P2处理的带4T1肿瘤小鼠的脾细胞(图7)。在无菌条件下取出脾,并如别处(duPre'S.等人Exp.Mol.Path.85:174-188,2008)所述制备。简而言之,将脾匀浆,将作为全身溶细胞性T细胞和APC的丰富来源的脾细胞与经丝裂霉素C处理的4T1刺激细胞共同培养,以诱导记忆性免疫应答。单独用培养基培养对照孔。
通过商业ELISA(BDBiosciences)定量CTL激活的标准代用品——IFN-γ浓度。在测定的所有条件下,来自经Nani-P2处理的BALB/c小鼠的脾细胞培养物中IFN-γ生产显著(p<0.01**)更高。经Nani-P2处理的动物(对照动物中没有)中的IFN-γ活性能够在显示促进DC分化的条件下通过添加IL-4和GM-CSF来增强(p<0.05),如果在开始培养时添加回肿瘤刺激物(次级指数=53比对照,3S+IL4+GM-CSF)则甚至能够被进一步增加,证实Nani-P2与其它CTL激动剂的协同效力。
为了进一步研究Nani-P2抵抗确立和转移性乳腺癌的功效,在小鼠的腹部乳腺垫中SC注射4T1细胞,并且延迟治疗直至肿瘤转移至肺并且大小平均为3.5mm(图8A,第13天),对应于2.4cm或T2阶段人乳腺肿瘤。监测小鼠的肿瘤生长(图8A)和肺转移(图8B)。在尸检时,与对照相比,接受Nani-P2处理的动物显示可视数量的肺转移的极大降低(图9)。用Nani-P2IV处理的小鼠肺中显著可视的肿瘤结节的平均数量为7,与之相比,PBS对照组具有35.3的平均值(p<0.01**)。这对应于原发性肿瘤侵略性较低的表现,以及大小平均小得多的肺转移灶(图8B)。
通过比较静脉内处理的动物(40ngIVNani-P2)和对照(经PBS处理的)动物的原发性肿瘤负荷,清楚并显著地证实了预先确立的、侵略性4T1乳腺癌背景中的Nani-P2功效(第18天p<0.01**,图10)。在原发性肿瘤抑制中的该统计学显著的益处(图10)在持续了50天的试验的整个持续时间内保持(p<0.01**),即使在第14天和28天之间仅有三次每周Tat衍生多肽给药。另外,有7/10的小鼠在第14天开始治疗肿瘤时证实有肿瘤消退。这被翻译成对存活的非常的高统计学显著益处(p<0.005**,见图5B)。可观的是,一只动物经历了完全消退,并且在研究终结的第50天时保持无疾病,支持了已经显然使该动物无肿瘤的推论。
实施例3
Tat衍生物和环磷酰胺的重复给药疗法
将1x104个4T1细胞SC植入四组10只BALB/c小鼠的乳腺脂肪垫中。第10天肿瘤直径达到4-5mm时开始治疗。以3天的间隔对对照小鼠IV注射PBS,而交替治疗小鼠在轮转的10天循环中每3天接受3剂药物。使用标准式计算肿瘤负荷(肿瘤大小mm3)。CY(单独环磷酰胺)小鼠从第10天开始,每只小鼠每周用80mg/kgIP注射。Cy/Nani-P2(首先环磷酰胺,之后Nani-P2)小鼠从第10天开始,首先以3天的间隔用环磷酰胺(80mg/kg)对小鼠IP注射三剂,然后以3天的间隔用Nani-P2(40ng)IV注射3剂,轮转进行。重复3剂CY之后3剂Nani-P2的循环,直至第50天。Nani-P2/CY(首先Nani-P2,之后环磷酰胺)小鼠从第10天开始首先以3天的间隔用Nani-P2(40ng)对小鼠IV注射3剂,然后以3天的间隔用环磷酰胺i.p.注射,轮转进行。重复3剂Nani-P2之后3剂CY的循环,直至第50天。
Nani-P2/CY组中与CY组相比降低的肿瘤负荷是具有非常高度统计学显著性的(图11,p=0.003077)。
与环磷酰胺交替的Nani-P2推注治疗相比每周环磷酰胺的的存活益处是高度统计学显著的(图12,p=0.0001)。第50天时Nani-P2小组有3/10的小鼠完全缓解,9/10的小鼠部分缓解(未显示),而第42天时10/10用环磷酰胺治疗的小鼠死亡。
实施例4
接受Nani-P2的小鼠中脾CD8+CTL的存在
脾是主要的淋巴器官和其中抗原递呈细胞展示出捕获肿瘤相关抗原以刺激细胞毒性T细胞应答的位点。肿瘤特异性CTL将迁移至感染位点并裂解靶细胞。
在乳腺脂肪垫中用同系的高转移性4T1乳腺癌细胞接种雌性BALB/c小鼠以模拟第IV期人类乳腺癌。肿瘤细胞接种后7天开始Nani-P2免疫治疗。在整个研究期间通过卡钳测量对肿瘤进行评估并在第29/30天切除。对经福尔马林固定的切除的脾组织的石蜡包埋样本的CD8进行免疫组织化学染色(IHC)。
如图13中所示,IHC染色显示了:相对于无处理(PBS,图13A),用Tat衍生物处理(图13B)后的小鼠中脾CD8+细胞群体增加。
实施例5
在PD-L1的存在下通过Nani-P2诱导4T1乳腺肿瘤浸润CD8+细胞毒性T淋巴细胞
癌症对免疫疗法的不应状态可能是伴随确立的癌症中疾病发展的免疫抑制的结果。在肿瘤微环境中,程序性细胞死亡受体-配体-1(PD-L1)的表达通过抑制肿瘤浸润CD8+细胞毒性T淋巴细胞(CTL)的功能而被认为是疾病发展、不良预后和宿主肿瘤免疫受损的标记。因此,PD-L1在多种肿瘤类型中的存在代表开发有效免疫疗法的主要障碍。
本文所公开的Tat衍生多肽通过触发单核细胞来源的树突细胞来刺激CD8+CTL和推翻PD-L1免疫抑制从而引起抗肿瘤免疫应答。因此,PD-1/PD-L1免疫抑制信号转导通路可以提供4T1肿瘤逃避宿主肿瘤免疫的潜在机制,因此在面对PD-L1免疫抑制时,Tat衍生多肽可以通过诱导肿瘤浸润CD8+CTL来影响实体瘤的发展。
在乳腺脂肪垫中用同系的高转移性4T1乳腺癌细胞接种雌性BALB/c小鼠以模拟第IV期人类乳腺癌。肿瘤细胞接种后7天开始Nani-P2免疫治疗。在整个研究期间通过卡钳测量对肿瘤进行评估并在第29/30天切除。对经福尔马林固定的原发性4T1肿瘤的石蜡包埋样品的PD-L1和CD8进行免疫组织化学染色(IHC)。
如图14中所示,相对于对照(图14A),接受Nani-P2处理的动物(图14B)中的PD-L1表达降低。在与骨髓来源抑制细胞形态相似的细胞、肿瘤相关巨噬细胞,以及肿瘤相关树突细胞和纤维原细胞中观察到了PD-L1染色。PD-L1减少是基于经Nani-P2处理的动物相对于对照的体内肿瘤测量数据和较低PD-L1染色强度。相较于对照,经Nani-P2处理的动物中大多不存在含有大部分PD-L1染色的肿瘤边缘。在PBS对照中,极少细胞呈CD8+CTL染色阳性(图14C);而在经PIN-2处理的小鼠中,浸润CD8+CTL前进到肿瘤边缘周围(图14D)。
相较于PBS对照,施用PIN-2的小鼠中确立的原发性4T1乳腺肿瘤的免疫染色显示出肿瘤浸润CD8+CTL群体显著增加。PD-L1在肿瘤边缘的存在可通过参与PD-1/PD-L1信号转导通路而充当抑制CTL活性的分子防护,从而促进肿瘤恶性和逃脱免疫监督。在经Nani-P2处理的小鼠中,肿瘤浸润CD8+CTL看起来位于肿瘤边缘附近;而这些CTL在PBS对照的肿瘤边缘大多不存在。由于PD-L1是与疾病发展、恶性和不良预后相关的标记,所以可以基于PIN-2处理观察到的抗肿瘤CTL应答来解释肿瘤PD-L与CD8+CTL的负相关。
综上所述,(i)用PIN-2处理观察到了肿瘤边缘附近减少的PD-L1存在;(ii)CD8+CTL有助于在经PIN-2处理的小鼠中观察到抗肿瘤免疫应答;(iii)PD-L1+原发性乳腺肿瘤的CD8+CTL浸润提示PINS将被癌症用作屏障的免疫抑制性机制(免疫检查点)推翻为成功的抗肿瘤免疫应答;(iv)确立的4T1原发性乳腺肿瘤中PD-L1的IHC阳性检测提示了免疫抑制性PD-1/PD-L1轴发挥了作为肿瘤逃逸的重要机制的作用;(v)在表达PD-L1的乳腺肿瘤中,本文所公开的Tat衍生多肽能够推翻PD-1/PD-L1通路;和(vi)施用本文所公开的Tat衍生多肽逆转在肿瘤发展期间建立的免疫抑制性趋势并重建免疫反应性。
