Description
技术领域和背景技术
本发明,在其一些实施方案中,涉及用于组织再生并且,更特别地,但不完全用于软组织再生的包含胶原和富血小板血浆(PRP)的组合物。
富血小板血浆(PRP)是富集血小板的血浆。作为自体血小板的浓缩来源,PRP含有(并且通过脱粒释放)几种不同的生长因子和刺激骨和软组织愈合的其它细胞因子。
随着血小板通过经由α-颗粒脱粒释放局部作用生长因子而引发伤口修复,PRP起到组织封闭剂和药物递送系统的作用。血小板的α-颗粒中所含的分泌蛋白包括血小板源生长因子(PDGF-AA、BB和AB异构体)、转化生长因子-β(TGF-β)、血小板因子4(PF4)、白细胞间素-1(IL-1)、血小板源血管生成因子(PDAF)、血管内皮生长因子(VEGF)、上皮生长因子(EGF)、血小板源内皮生长因子(PDEGF)、上皮细胞生长因子(ECGF)、胰岛素样生长因子(IGF)、骨钙素(Oc)、骨粘连蛋白(On)、纤维蛋白原(Ff)、玻连蛋白(Vn)、纤连蛋白(Fn)和血小板反应蛋白-1(TSP-1)。这些生长因子通过吸引新形成的基质中的未分化细胞并触发细胞分裂而帮助愈合。PRP可抑制细胞因子释放并限制炎症,与巨噬细胞相互作用以提高组织愈合和再生,促进新毛细血管生长,并加速慢性伤口中的上皮形成。
PRP中的血小板通过生成吸引巨噬细胞的信号蛋白在伤口部位的宿主防御机制中也起作用;PRP也可含有少量合成作为非特异性免疫反应的一部分的白细胞间素的白细胞。
PRP向损伤部位的递送和滞留由于其液体状态并且因此物质可能向周围组织损失而仍然是一个挑战。凝血酶(主要为牛源性)是用于凝块形成和增加PRP胶凝化的常见血小板活化因子。凝血酶的使用具有几个缺点。凝血酶在人类中具有不良免疫反应。另外,体外研究已显示出对细胞增殖和活力的抑制(LawsonJH.SeminThrombHemost,2006;32(增刊1)98-110;MurrayMM等,JOrthopRes.200735(1)第81-91页)。
发现天然介入内在凝血级联的I型胶原是用于血小板活化的凝血酶的有吸引力的替代物。除了是哺乳动物结缔组织中的主要蛋白组分外,I型胶原是研究最多的用于再生医学和组织工程学的天然支架。
几项体外研究调查了从受凝血酶或胶原活化的PRP凝块的细胞因子释放以便表征其释放曲线(TsayRC等,JOralMaxillofacSurg,200563第521-528页;FufaD等,JOralMaxillofacSurg,200866(4)第684-690页)。经证实呈不同物理状态、可溶或原纤维的I型胶原在几天内刺激TGF、PDGF和VEGF的释放与凝血酶一样有效。培育经凝血酶或I型胶原活化的PRP的培养物长达15天并且证实基于胶原的凝块在大部分基于凝血酶的凝块降解时保持其初始形状和尺寸。
Laci等,YaleJBiolMed.2010March;83(1):1–9教导称氯化钙可用于活化PRP凝块。
美国专利申请第20120201897号教导了用于活化PRP凝块的氯化钙和I型胶原的组合。
发明内容
根据本发明一些实施方案的一个方面,提供了一种包含交联胶原、富血小板血浆(PRP)和无机盐的物质组合物。
根据本发明一些实施方案的一个方面,提供了一种包含胶原、富血小板血浆(PRP)和无机盐的物质组合物,其在37℃下培育10天后能够释放超过4000pg/ml的血小板源生长因子(PDGF)。

图4为与凝血酶+PRP和PBS+CaCl2+PRP的对照相比基于rh胶原的凝块的剪应力-应变曲线。通过rh胶原薄片与CaCl2+PRP直接混合(胶原直接混合)或通过首先用PBS水合rh胶原薄片,然后与CaCl2+PRP混合(经由PBS的rh胶原)获得两种类型的基于rh胶原的凝块。
图5A-B为说明2天(图5A)和12天(图5B)后与基于凝血酶的凝块(蓝色圆圈)相比,基于rh胶原的凝块的降解测定的体内结果的照片。可清楚地观察到基于凝血酶的凝块完全降解。基于rh胶原的凝块在相同时间段内保持稳定。
图6A-B说明了接种3和7天后nHDF细胞增殖的结果。图6A表示通过使用WST染色检测到的定量增殖。图6B展示了对于30mg/ml原纤维、30mg/ml原纤维交联胶原的凝块样品及基于CaCl2和PBS+PRP(无rh胶原)的混合物的凝块而言,3和7天后在transwell周围增殖的细胞的显微镜检查。
具体实施方式
本发明,在其一些实施方案中,涉及用于组织再生并且,更特别地,但不完全用于软组织再生的包含胶原和富血小板血浆(PRP)的组合物。
在详细解释本发明的至少一个实施方案之前,应理解本发明在其应用上不一定限于以下描述中提出或通过实施例举例说明的细节。本发明能够有其它实施方案或以各种方式实践或实施。
PRP是具有浓缩血小板的血浆。在PRP中发现的血小板高度浓缩并且包含生长因子,以及大量对激活和加速组织修复和再生至关重要的生物活性蛋白。生物活性蛋白增加了引发结缔组织的愈合、骨再生和修复、新血管的促进和伤口愈合过程的刺激的干细胞的产生。
除非经由凝血机制实现胶凝化,否则呈液体形式的PRP施加于伤口部位可伴有PRP大量损失到周围间隙中。目前使用牛凝血酶实现胶凝化。
虽然牛凝血酶是有效的血小板活化因子,但是也引起抗凝血酶、凝血酶原、因子V和心磷脂的抗体发展,及在动物研究中作为结果而发生的范围从严重术后出血到类似于狼疮的自身免疫综合征的临床问题。使用牛凝血酶还导致成纤维细胞通过胶原-PRP凝块的迁移受损,以及凝块的强度受损。另外,用凝血酶-活化凝块所见的高度凝缩使其难以用于伤口-间隙填充应用。
本发明人提出使用胶原作为PRP的活化因子,由此配制胶原以便控制生长因子从凝块释放的速率。例如,如图3A-B所示,本发明人证实与使用非交联胶原活化PRP凝块时相比,当使用交联胶原活化PRP凝块时释放更多的血小板源生长因子(PDGF)。另外,本发明人提出可向制剂中添加氯化钙以控制凝块形成所需的时间。
虽然为实践而简化了本发明,但是本发明人已经证实胶原在PRP凝块中起填料的作用,由此胶原纤维被截留在凝块的纤维蛋白成分中(图2A-B)。有人提出胶原使纤维蛋白成分相互连接的能力是造成凝块强度和可缝合性的原因。
因此,根据本发明的一个方面提出了一种包含交联胶原、富血小板血浆(PRP)和氯化钙的物质组合物。
如本文中所用,短语“富血小板血浆”是指血小板的浓度高于外周血液的血浆。虽然正常血小板计数范围可从约140,000至约400,000个/微升,但是PRP的一些血小板浓度可在约500,000至约1,200,000/微升或更高的范围内。PRP可由全血形成,并且可使用自体、异体或混合的血小板和/或血浆来源获得。PRP可由多种动物来源,包括人类来源形成。
通常,PRP可含有95%血小板与4%红血细胞和1%白血细胞。
在一些实例中,PRP可经进一步加工,包括但不限于去白细胞和免疫吸附。其它PRP组合物在2003年4月11日提交的Mishra的美国专利第6,811,777号中有进一步描述,其特此通过引用整体并入本文。
可使用标准程序从患者抽取全血。
全血在采集后可能或可能不经冷却。血小板从全血中的分离依赖于血小板和红血细胞之间的密度差。血小板和白血细胞浓缩于介于去血小板血浆(上层)和红血细胞(下层)之间的层中(即,“血沉棕黄层”)。例如,下部浮体和上部浮体可用于将富血小板层截留在上下相之间。然后可使用注射器或移液管吸取这个富血小板层。通常,可捕获血液样品中至少60%、至少70%或至少80%的可用血小板。这些血小板可重新悬浮在可为样品体积的约3%至约20%或约5%至约10%的体积中。
在一些实例中,然后可使用重力血小板系统,例如CellFactorTechnologiesGPSSystemRTM离心机离心血液。装有约20cc至约150cc血液(例如,约55cc血液)和约5cc柠檬酸盐葡萄糖的充血注射器可缓慢地转移到可装入GPS离心机的孔内的一次性分离管中。样品可经加盖并置于离心机内。然后可使样品旋转以从血液和血浆中分离血小板。可使样品在约2000rpm至约5000rpm下旋转约5分钟至约30分钟。例如,可在3200rpm下进行离心以从离心管的一侧提取,然后可通过搅拌使分离的血小板悬浮在约3cc至约5cc的血浆中。然后可使用例如10cc注射器从侧孔提取PRP。如果可从患者采集约55cc的血液,则可获得约5cc的PRP。
因为PRP组合物包含活化血小板,所以释放出血小板内的活性剂。这些试剂包括但不限于细胞因子(例如,IL-1B、IL-6、TNF-.α.)、趋化因子(例如,ENA-78(CXCL5)、IL-8(CXCL8)、MCP-3(CCL7)、MIP-1A(CCL3)、NAP-2(CXCL7)、PF4(CXCL4)、RANTES(CCL5))、炎症介质(例如,PGE2)和生长因子(例如,血管生成素-1、bFGF、EGF、FGF、HGF、IGF-I、IGF-II、PDAF、PDEGF、PDGFAA和BB、TGF-β1、2和3及VEGF)。
如本文中所用的术语“胶原”是指具有三螺旋结构并且含有重复Gly-X-Y三联体的多肽,其中X和Y可为任何氨基酸,但是常常为亚氨基酸脯氨酸和羟脯氨酸。根据一个实施方案,胶原为I、II、III、V、XI型或来自其的生物活性片段。
本发明的胶原也指同源物(例如,正如使用国家生物技术信息中心(NCBI)的BlastP软件,使用默认参数所测定的,与表1所列胶原序列至少50%、至少55%、至少60%、至少65%、至少70%、至少75%、至少80%、至少85%、至少87%、至少89%、至少91%、至少93%、至少95%或更高称为100%同源的多肽)。同源物也可指其缺失、插入或取代变体,包括氨基酸取代,及其生物活性多肽片段。

下面是对获得用于本文所述PRP组合物的胶原的各种方法的描述。
从动物分离胶原的方法在本领域中已知。可使用破坏分子间键并去除免疫原性非螺旋端肽,而不影响赋予胶原所需特征的基础、刚性三螺旋结构的各种蛋白水解酶(例如猪粘膜胃蛋白酶、菠萝蛋白酶、木瓜凝乳蛋白酶、胰凝乳蛋白酶、胶原酶、无花果蛋白酶、木瓜蛋白酶、肽酶、蛋白酶A、蛋白酶K、胰蛋白酶、微生物蛋白酶和相似酶或此类酶的组合)实现天然动物胶原的分散和溶解(制备纯化可溶性胶原的一般方法,参见美国专利第3,934,852、3,121,049、3,131,130、3,314,861、3,530,037、3,949,073、4,233,360和4,488,911号)。随后可通过在低pH和高离子强度下重复沉淀,接着洗涤并且在低pH下重新溶解而纯化所得可溶性胶原。

应了解本发明还考虑到了胶原/去端肽胶原的基因修饰形式-例如胶原酶耐受性胶原等[Wu等,ProcNatl.AcadSci,第87卷,第5888-5892页,1990]。
重组胶原可在任何动物或非动物细胞中表达。非动物细胞的实例包括但不限于植物细胞和其它真核细胞例如酵母和真菌。动物细胞的实例包括但不限于CHO细胞和乳。
可在其中生成(即表达)人胶原的植物可具有低等(例如苔藓和藻类)或高等(维管)植物种类,包括其组织或离体细胞和提取物(例如细胞悬浮液)。优选的植物为能够累积大量胶原链、胶原和/或下文所述的加工酶的植物。此类植物也可根据其对应力条件的抵抗力和可以提取表达的组分或组装的胶原的简易度选择。可在其中表达人原胶原的植物的实例包括但不限于烟草、玉米、紫花苜蓿、水稻、马铃薯、大豆、西红柿、小麦、大麦、菜籽、胡萝卜、莴苣和棉花。
重组原胶原的生成通常通过受编码人原胶原的外源多核苷酸序列的稳定或瞬时转化而实现。
植物中人端肽胶原的生成通常通过受编码人原胶原的外源多核苷酸序列和编码有关蛋白酶的至少一个外源多核苷酸序列的稳定或瞬时转化而实现。
胶原的三螺旋结构的稳定性需要通过酶脯氨酰基-4-羟化酶(P4H)羟基化脯氨酸以在胶原链内形成羟脯氨酸。虽然植物能够合成含羟脯氨酸的蛋白质,但是在植物细胞中负责合成羟脯氨酸的脯氨酰基羟化酶与哺乳动物P4H相比表现出相对宽松的底物序列特异性。因此,仅在Gly–X–Y三联体的Y位生成含羟脯氨酸的胶原需要胶原和人或哺乳动物P4H基因的共同表达[Olsen等,AdvDrugDelivRev.2003年11月28日;55(12):1547-67]。
因此,根据一个实施方案,胶原定向于植物的无内源P4H活性的亚细胞区室。如本文中所用,短语“无内源P4H活性的亚细胞区室”是指细胞的不包括植物P4H或具有植物样P4H活性的酶的任何分隔区域。
根据一个实施方案,所述亚细胞区室为空泡。
表达的胶原在无内源性P4H活性的亚细胞区室内的累积可经由几种方法中的任一种实现。
例如,表达的胶原可包括用于使表达的蛋白质靶向亚细胞区室例如空泡的信号序列。因为P4H与表达的胶原链一起共同累积是必需的,所以其编码序列优选经相应地修饰(例如,通过信号序列的添加或缺失)。因此,P4H与胶原一起在植物中共同表达,由此P4H也包括用于靶向相同亚细胞区室例如空泡的信号序列。优选地,胶原序列和P4H序列两者均无内质网滞留信号,以便其通过ER并留在空泡中,序列在空泡中羟基化。
因此本发明考虑到了共同表达人胶原和能够正确羟基化胶原α链[即仅羟基化Gly–X–Y三联体的脯氨酸(Y)位置]的P4H两者的基因修饰细胞。P4H是由基因库第P07237和P13674号中提出的两个亚基α和β组成的酶。两个亚基是形成活性酶所必需的,而β亚基还具有伴侣功能。
本发明的基因修饰细胞所表达的P4H优选为哺乳动物P4H(例如,如SEQIDNo:3和4编码的人P4H)。另外,也可使用表现出更强的底物特异性的P4H突变体或P4H同源物。
在哺乳动物细胞中,胶原还经赖氨酰羟化酶、半乳糖基转移酶和葡糖基转移酶修饰。这些酶依次将特定位置的赖氨酰残基修饰为在特定位置的羟基赖氨酰基、半乳糖基羟基赖氨酰基和葡糖基半乳糖基羟基赖氨酰基残基。单一人类酶,如基因库第O60568号所示的赖氨酰羟化酶3(LH3)可催化如同在羟赖氨酸连接的碳水化合物形成中所见的全部三个连续修饰步骤。
因此,本发明经基因修饰的细胞也可表达哺乳动物LH3。LH3编码序列例如用SEQIDNO:5表示的LH3编码序列可用于此类目的。
可由包括位于功能启动子的转录控制下的编码原胶原α链和/或修饰酶(例如P4H和LH3)的多核苷酸序列的稳定整合或瞬时表达核酸构建体表达上述胶原和修饰酶。此类核酸构建体(本文也称为表达构建体)可配置用于在整个生物体(例如植物、规定组织或规定细胞)和/或在生物体的规定发育阶段表达。此类构建体也可包括选择标记(例如抗生素抗性)、增强子元件和用于细菌复制的复制起点。
存在各种将核酸构建体引入单子叶和双子叶植物中的方法(Potrykus,I.,Annu.Rev.Plant.Physiol.,Plant.Mol.Biol.(1991)42:205-225;Shimamoto等,Nature(1989)338:274-276)。此类方法依赖于核酸构建体或其一部分稳定整合到植物基因组中,或依赖于核酸构建体的瞬时表达,在这种情况下这些序列未被植物的后代遗传。
另外,存在可将核酸构建体直接引入含DNA的细胞器例如叶绿体的DNA中的几种方法。

存在将DNA直接转移到植物细胞内的各种方法。在电穿孔中,将原生质体短暂地暴露于强电场。在显微注射中,使用非常小的微量移液管将DNA直接机械注入细胞内。在微粒轰击中,DNA被吸附在微弹例如硫酸镁晶体、钨粒或金粒上,并且微弹经物理加速到细胞或植物组织内。
与采用的转化技术无关,一旦鉴定了表达原胶原或胶原的后代,就在使其表达达到最大限度的条件下进一步栽培此类植物。可通过使用核酸或蛋白质探针(例如抗体)检验外源mRNA和/或多肽的存在选择由转化植物产生的后代。后一种方法使得表达的多肽组分的定位成为可能(例如通过探测分级植物提取物)并且因此还检验了植物正确加工和组装外源蛋白质的潜力。
栽培此类植物后,通常收获所述胶原。可在任何时间(例如在成熟期)收获植物组织/细胞,并且使用提取方法分离原胶原分子。优选地,实现收获使得原胶原仍然处于可被蛋白酶裂解的状态。根据一个实施方案,由本发明的转基因植物生成粗提取物并且随后与蛋白酶接触。
为了生成去端肽胶原或胶原,可在用蛋白酶培育之前由基因工程化细胞纯化包含前肽或端肽的胶原,或可选地可在用蛋白酶培育之后纯化。还可选地,可在蛋白酶处理之前部分纯化包含前肽或端肽的胶原,然后在蛋白酶处理后完全纯化。仍可选地,可伴随其它提取/纯化程序,用蛋白酶处理包含前肽或端肽的胶原。
纯化或半纯化本发明的包含端肽的胶原的示例性方法包括但不限于用硫酸铵等盐析和/或通过超滤或通过色谱法去除小分子。
根据一个实施方案,用于裂解包含前肽或端肽的重组胶原的蛋白酶并非源自动物。示例性蛋白酶包括但不限于某些植物源蛋白酶例如无花果蛋白酶(EC3.4.22.3)和某些细菌源蛋白酶例如枯草杆菌蛋白酶(EC3.4.21.62)、中性蛋白酶。根据一个特定实施方案,所述蛋白酶为无花果蛋白酶。本发明人还考虑到重组酶例如rh胰蛋白酶和rh胃蛋白酶的用途。几种此类酶可购买获得例如来自于无花果树树浆的无花果蛋白酶(Sigma,产品目录号#F4125和EuropeBiochem)、来自于地衣型芽胞杆菌(Bacilluslicheniformis)的枯草杆菌蛋白酶(Sigma,产品目录号#P5459)、来自于细菌解淀粉芽孢杆菌(Bacillusamyloliquefaciens)的中性蛋白酶(Novozymes,产品目录号#PW201041)和TrypZeanTM,在玉米中表达的一种重组人胰蛋白酶(Sigma产品目录号#T3449)。
不管如何生成或分离胶原,胶原通常溶于其呈单体形式(即非原纤化形式)存在的酸性溶液中。溶解单体胶原的示例性酸包括但不限于盐酸(HCl)和醋酸。
如本文中所用,短语“胶原单体”是指尚未经历原纤维组装过程的单体胶原。
所述胶原可以约1-100mg/ml的浓度存在于酸性溶液中。根据一个特定实施方案,所述胶原可以约3-20mg/ml的浓度存在于酸性溶液中。可用于溶解胶原单体的HCl的示例性浓度为约10mMHCl。
根据一个实施方案使用约0.05mM–50mM浓度的醋酸溶解胶原单体。可用于溶解胶原单体的醋酸的示例性浓度为约0.5M醋酸。
胶原溶解之后,可任选地处理胶原以便促进其原纤维生成。
如本文中所用的术语“原纤维生成”是指呈原纤维形式的可溶性胶原的析出。
原纤维生成受熵驱动—水分子从单体表面的损失驱动胶原单体从溶液中析出并与圆形横截面组装,以便最小化表面积。原纤维生成可按多种方式进行,包括pH中和、增加温度和/或离子强度。
可添加以升高胶原pH的示例性碱性溶液为Na2HPO4(pH11.2)。通常,计算碱性溶液的量使得胶原的最终pH为约7-7.5(例如7.4)。通常按1:7-1:9v/v的比率添加Na2HPO4(162mM)。
根据一个特定实施方案,胶原以介于10-50mg/ml之间,更优选介于20-30mg/ml之间的浓度存在于所述组合物中。
为了生成本文所述的组合物,可制备胶原和无机盐的混合物。
所述无机盐可为钠盐、氯盐、钾盐、钙盐、镁盐、磷酸盐、硫酸盐或羧酸盐。
优选地,所述无机盐选自NaCl、KCl、CsCl、CaCl2、CsF、KClO4、NaNO3和CaSO4。
根据一个特定实施方案,所述无机盐为CaCl2。
添加CaCl2,使其以介于5-100mM之间,更优选介于7-60mM之间,更优选介于10-50mM之间且更优选介于10-30mM之间的最终浓度存在。

可根据本发明的教导使用的颗粒的实例包括但不限于钛酸钙、羟磷灰石(HA)、磷酸三钙(TCP)、双相磷酸钙及其它磷酸钙和钙-磷化合物、羟磷灰石钙盐、珊瑚羟磷灰石、碳酸钙无机骨、牙釉质、霰石、方解石、珍珠母、石墨、热解碳、生物玻璃、生物陶瓷及其混合物。
在添加无机盐之前,原纤化胶原可经交联。纤维的交联可使用以下方法中的任一种实现:1.在N-羟基琥珀酰亚胺(NHS)、PEG树枝状大分子和多臂PEG、京尼平(genipin)或其它化学交联剂存在或缺乏时,用戊二醛、N-乙基-N'-[3-二甲氨基丙基]碳二亚胺(EDC);2.通过使用不同的糖糖化;3.通过使用金属离子例如铜进行芬顿反应(Fentonreaction);4.用赖氨酸氧化酶;5.通过紫外线辐射(例如在光引发剂如4-(2-羟基乙氧基)苯基-(2-羟基-2-丙基)酮-Irgacure2959的存在下)或6.通过脱氢热(DHT)交联。
本发明考虑到交联也可在添加无机盐之后实现(例如可添加无机盐,之后使用DHT进行交联)。
根据一个实施方案,EDC的浓度选为在10-50mM的范围内。
根据另一个实施方案,实现DHT交联的天数介于1和5天之间。
如下文进一步所描述,根据凝块稳定性和生长因子释放的需要选择交联剂的量和/或胶原交联的时间的量。
在生成胶原/无机盐混合物之后,可任选地干燥所述混合物。
根据一个特定实施方案,胶原/盐混合物经冷冻并且冷冻-干燥。
通常,冷冻-干燥通过使用标准商用冷冻-干燥设备完成。在一个特定实施方案中,混合物的冷冻-干燥利于到处多孔结构的形成。
在任选干燥阶段之后,混合物与PRP接触。应了解如果混合物经干燥,则还考虑到了用适合液体预水合。此类液体包括生理缓冲液(例如PBS)和/或贫血小板血浆(PPP)溶液。
短语贫血小板血浆(PPP)是指具有极少量的血小板(通常低于10X103/μL)的血浆。
通常由制备PRP的相同全血样品制备PPP。
生成的组合物可呈液态或半固态。例如在低于约37℃的温度下,组合物呈液态。当温度为约37℃时,发生PRP的活化并且生成胶状物质(凝块)。
优选地所述组合物能够在体外在37℃下在约5-10分钟内,更优选在介于约6-7分钟之间产生凝块。
本发明人提出当成半固态或固态时胶原纤维使凝块转化为可缝合的组合物。
正如所提到的,组合物中组分的比率和组分的状态(例如交联或非交联胶原)指示凝块的物理参数。

可使用本文描述的方法治疗的受试者通常为哺乳动物,更优选为人。
根据一个优选实施方案,用于制备组合物的PRP为受治受试者自体的。
可使用本文描述的组合物修复的组织包括例如软骨、半月板、韧带、腱、骨、皮肤、骨、手术伤口、角膜、牙周组织、上颌面组织、颞下颌组织、脓肿、切除肿瘤和溃疡。
本文描述的PRP组合物(即PRP、胶原和无机盐)可在紧急情况下或作为非必要性手术的一部分向患者递送。为治疗受损结缔组织,PRP组合物可在组织损伤发生几天、几周、几个月或几年后作为住院患者或门诊患者手术的一部分递送。可使用PRP治疗的结缔组织损伤的实例包括但不限于外侧上髁炎(即,肘部发炎)、足底筋膜炎、膝盖骨肌腱炎(即,跳跃者膝(Jumper'sKnee))、跟腱炎(Achillestendonitis)、肩袖肌腱炎、踝关节扭伤和韧带撕裂(部分或全部)。可使用一种或多种医学成像技术例如但不限于x-射线成像、磁共振成像(MRI)和超声成像鉴定组织损伤。为治疗对心肌的损伤,在MI经鉴定时可在急救室和/或由紧急医疗服务机构递送PRP组合物。在其它情况下,可在MI后在再灌注治疗期间递送PRP组合物。
PRP组合物可按任何适合剂量递送。在一些实施方案中,剂量可介于约1cc和约3cc之间,介于3cc和约5cc之间,介于约5cc和约10cc之间,介于约10cc和约20cc之间,或更多。可根据医疗程序(例如,在手术中的特定时间点)和/或根据时间表递送所述剂量。
在一些实例中,可向受伤关节内或周围的受损结缔组织递送PRP组合物。可通过使用注射器或导管注射向有需要的个体递送PRP组合物。PRP组合物也可经由皮肤贴片、喷雾装置或与软膏、骨移植物或药物组合递送。PRP组合物可进一步用作缝线、支架、螺钉、板或一些其它植入式医疗装置上的涂层。最后,PRP组合物可连同生物可吸收药物或装置一起使用。
根据一个实施方案,PRP组合物并入缝合材料中。PRP组合物可织入缝合材料中。可选地,在使用之前可用PRP培育缝合材料。培育时间可从几秒到可为医疗程序的持续时间的任何便利时间。PRP可在使用之前与缝合材料一起培育几秒至几小时,例如少于1分钟、5-10分钟、10分钟至1小时、1-3小时、4-12小时、13-24小时、1-3天或3-31天。
PRP组合物的递送部位通常在组织损伤部位处或附近。通过已确立的方法,包括成像研究和患者反馈或其组合确定组织损伤的部位。使用的优选成像研究可基于组织类型而确定。常用的成像技术包括但不限于MRI、X-射线、CT扫描、正电子发射断层扫描术(PET)、单光子发射计算机控制断层扫描术(SPECT)、电阻抗断层扫描术(EIT)、电源成像(ESI)、磁源成像(MSI)、激光光学成像和超声技术。患者也可通过指出特别疼痛和/或不适的区域帮助定位组织损害或损伤的部位。
配制为凝胶或其它粘性液体的PRP组合物可能难以经由针头或注射器递送。因此,在需要使用针头或注射器的变型中,可能需要在原位向PRP组合物添加胶凝剂和/或硬化剂。可配置一根或多根针头或导管以同时或基本上同时向受伤组织递送PRP组合物和/或所述试剂。例如,如果使用针头递送PRP组合物,则针头可包含多个管腔,PRP组合物和所述试剂单独通过管腔。可选地或另外,可使用单独的针头同时或相继地向组织递送所述组分。
可最低限度侵入性地和/或用外科手术递送PRP组合物。例如,可使用经由股静脉或动脉、颈内静脉或动脉或任何其它适合的静脉或动脉插入患者体内的导管向心脏递送PRP组合物。PRP组合物也可连同一种或多种医疗装置、仪器或试剂一起递送以治疗MI和/或其它心脏病状。
用于注射或递送PRP组合物的装置(导管或其它)可包括冷却部件或其它温度控制机械装置以将PRP组合物保持在所需温度下。递送装置的各种实施方案可包括冷却室和/或PRP室或注射室内的搅拌器机械装置以防止PRP组分沉降或凝集。例如,在一些变型中,导管或其它递送装置具有冷却管腔或用于在递送期间保持PRP组合物冷却的管腔。递送装置可另外或可选地包括用于在递送之前混合PRP组合物的混合室。PRP组合物也可储存在搅拌/振荡室内,或PRP一旦在递送装置内,医师就可通过倾斜或以另外的方式操纵所述装置搅拌PRP组合物。
PRP组合物可单独地或与其它疗法组合使用,包括但不限于干细胞(胚胎或成体)祖细胞、体细胞、脐血、药物、基因工程化分子或其它生物活性物质。

一般材料和方法
rh胶原的制备-对于不同制剂
将适当体积的rh胶原原液(3mg/ml,10mMHCl)冷冻干燥并用1mlDDW重新悬浮以获得所需浓度。向重新悬浮的浓缩rh胶原溶液(1:7v/v)添加二碱式磷酸盐缓冲液(162mM二碱式磷酸盐,pH11.2)产生浑浊沉淀,表明由于中和rh胶原溶液,rh胶原分子自我组装成原纤维。
部分高度浓度的原纤维rh胶原溶液用1-[3-(二甲基氨基)丙基]-3-乙基碳二亚胺甲碘化物(EDC,Sigma)在(20mM、50mM)的不同浓度下交联3小时。通过在8000RPM下离心10分钟洗涤样品两次,在洗涤间期排出液体。
制备浓度为100mg/ml的氯化钙。用rh胶原制剂通过向rh胶原填充管添加适当体积试验7mM、20mM和60mM浓度的添加。原纤维和交联原纤维浓缩rh胶原溶液两者均冷冻干燥。
PRP提取:使用EstarMedical的自体血小板制备系统(PPKE-2)PRP试剂盒。向装有基于凝胶和柠檬酸盐的抗凝剂的10ml真空管吸取10ml血液。在1500RCF下离心血液10分钟以沉淀红血细胞和部分白血细胞。过滤之前去除4ml贫血小板血浆以便获得贫血小板血浆。过滤之后,剩余1.5-2ml的血浆含有富血小板血浆(PRP)。轻轻混合PRP并转移到空管内。将PRP保持在冰上直至使用。
rh胶原薄片与PRP混合:使用两种使胶原薄片水合的方法:
1.直接混合干燥rh胶原薄片:使用填充有固定重量的干燥rh胶原薄片的注射器。为另一注射器填充PRP。通过在两个注射器之间使用卢尔锁用PRP溶液使rh胶原薄片重新悬浮并轻轻匀化。将最终的浓缩rh胶原-PRP悬浮液转移到一个注射器并注入模具内。
2.混合干燥rh胶原与PBS/PPP(贫血小板血浆),然后与PRP混合:使用填充有固定重量的干燥rh胶原薄片的注射器。为另一注射器填充0.5mlPBS/PPP以使rh胶原薄片重新悬浮和匀化。混合工序通过介于两个注射器之间的卢尔锁连接而进行。将获得的悬浮液转移到其中一个注射器中。断开空注射器。将装有0.5mlPRP的第三个注射器连接到填充有rh胶原悬浮液的注射器并轻轻混合物多达3-4次。将最终的浓缩rh胶原-PRP悬浮液转移到一个注射器并注入模具内。
在37℃下培育所有样品以便形成凝块。形成凝块的培育时间取决于制剂中所用的氯化钙浓度。
优化凝块形成时间的氯化钙浓度:使用不同的氯化钙浓度来表征PRP活化和凝块形成的速率。试验浓度为7mM、20mM和60mM的氯化钙。
生长因子释放研究:如上所述使不同的rh胶原悬浮液与PRP混合。将200μl等分试样的不同PRP-rh胶原制剂置于适合24孔板的8μm网目transwell内。
凝血酶样品制备:由牛凝血酶粉剂(Sigma)制备1,000IU/ml的凝血酶溶液。首先,将PRP添加到transwell内,接着按1:10v/v凝血酶:PRP比率添加凝血酶溶液。通过混合等体积的PRP和含20mMCaCl2的PBS制备受CaCl2活化的PRP。然后在37℃下连同凝血酶样品一起培育所有含20mMCaCl2的rh胶原样品15分钟以允许凝块形成。向每个孔添加0.5ml的DMEM培养基(1%血清)并且向transwell内在形成的凝块上再添加200μl。
在加湿培育箱内(5%CO2、37℃)培育24-孔板。在不同时间点(1、3、5、10天)收集所有释放培养基并向每个孔添加新鲜培养基。将培养基样品储存在-80℃冷冻器中直至收集所有样品。使用市场上可买到的QuantikineELISA试剂盒(R&DSystems)测定人PDGF-AB和TGF-β的浓度。对于细胞因子检测未使用稀释液。
成纤维细胞增殖测定研究:如上所述将原纤维和原纤维交联(20mMEDC)的rh胶原悬浮液(30mg/ml)与PRP混合并且在从凝块样品释放的生长因子的存在下分析正常人皮肤成纤维细胞(nHDF)的增殖。另外,分析来自于受20mMCaCl2活化的血小板和来自于受凝血酶活化的血小板的细胞增殖的效果。将来自于每种制剂的200μl等分试样插入transwell内(每种制剂3个transwell)并且在每个的底部接种10,000个nHDF并且为空填充0.5mlDMEM+1%胎牛血清(FBS)。分别按照在20%FBS和1%FBS环境中增殖的细胞的相同数量获得实验的阳性和阴性对照。增殖是在接种3天和7天后。
用于SEM分析的制剂:如上所述制备基于rh胶原-PRP和凝血酶-PRP的凝块样品。立即在室温下将样品浸入二甲胂酸钠缓冲甲醛-戊二醛固定剂中24小时。用0.1M二甲胂酸盐缓冲液洗涤样品3次。固定后,用1%四氧化锇为样品染色1小时。再用0.1M二甲胂酸盐缓冲液洗涤样品2次并用DDW洗涤两次。随后,通过在递增浓度的乙醇(10-100%)中连续转移使样品脱水并且在临界干燥点之前用液体二氧化碳浸透。最后,将样品固定在铝上并用喷金法涂布至的厚度。
机械表征:制备三种类型的样品:
(i)在20mM氯化钙存在下的rh胶原-PRP;
(ii)凝血酶-PRP;和
(iii)PRP和20mM氯化钙。
如上所述制备样品并注入适合平行板几何流变仪(HAAKE,Rheostress600)的圆柱体模具中。在37℃下培育样品以便凝块形成。将每个凝块样品置于流变仪中并且施加1-100Pa的剪应力。记录作为剪应力函数的剪应变。比较每个样品经程序处理的曲线的斜率以估计样品对剪切变形的抗性并因此估计其机械稳定性。
降解测定:如上所述制备基于rh胶原和凝血酶的凝块。向24孔板内注入各0.5ml的制剂。试验来自于每个样品的两份样品。在37℃下凝块形成之后,向每个孔添加0.5mlDMEM培养基(1%血清)。跟踪样品尺寸和形态变化并且评估长达30天。
实施例1
混合PRP与rh胶原
试验两种PRP与rh胶原混合的方法。在两种方法中使用用用卢尔锁偶联的两个注射器(图1)。一种方法基于干燥rh胶原与PRP溶液的直接混合(两个注射器)。另一种方法基于两个步骤。首先,通过使用PBS初始水合rh胶原形成均匀悬浮液,接着混合填充有rh胶原悬浮液的注射器与PRP溶液。如下面所详述,通过两种方法获得的凝块具有相似的物理性质。
实施例2
基于rh胶原的凝块的结构
使用SEM分析基于rh胶原的凝块的结构并且与基于凝血酶的凝块比较。研究得到构成PRP凝胶凝块的成分在与凝血酶混合时或与rh胶原混合时的清晰图像。基于凝血酶的PRP凝块由沿其整个长度厚度均匀的随机排列的原纤维成分与呈面包屑样外观排列在其间的血小板细胞成分构成(图2A-B)。原纤维成分被鉴定为纤维蛋白纤维。在基于rh胶原的凝胶凝块中,除纤维蛋白连同活化血小板一起存在外,鉴定出rh胶原粗纤维岛。发现这些rh胶原纤维被截留在由纤维蛋白纤维形成的细孔内(图2C-D)。
实施例3
生长因子从基于rh胶原的凝块释放
将从不同的rh胶原活化PRP制剂释放的PDGF-AB的量与从凝血酶活化PRP、氯化钙活化PRP和自活化型PRP释放的PDGF-AB的量比较。正如图3A-B中所见,10天内PDGF的释放曲线在制剂间不同。含浓度为30mg/ml的交联(20mMEDC)原纤维rh胶原的样品相对于多种对照(自活化型血小板、通过使用氯化钙或凝血酶活化的血小板)而言具有最高的PDGF累积释放量。不添加胶原,用氯化钙活化的血小板显示出比具有相同氯化钙浓度的胶原制剂低得多的PDGF释放量的事实证明,活性作用胶原在PDGF的释放曲线中起作用,特别是对于交联制剂而言。表征较低浓度的rh胶原(10mg/ml、20mg/ml)的释放曲线。发现从用较低浓度的rh胶原活化的PRP凝块释放的PDGF-AB的量与用氯化钙活化的PRP凝块获得的值相似。因此,rh胶原的最佳工作浓度为约30mg/ml。
上述结果证明了在与PRP混合后根据rh胶原浓度和在不同物理状态下(原纤维/原纤维交联),控制PDGF释放的能力。
实施例4
成纤维细胞增殖测定研究
通过使用WST(水溶性四唑盐)定量测量细胞增殖证明在基于胶原-PRP的凝块的成纤维细胞增殖和基于凝血酶的凝块的成纤维细胞增殖与在含1%胎牛血清(饥饿状态)的培养基中增殖的对照细胞之间存在显著差异(图6A-B)。发现基于胶原的凝块和对照(凝血酶和1%FBS)之间的显著差异在7天后增大。围绕基于交联胶原的凝块的细胞具有最高增殖,甚至高于在含20%FBS的培养基的存在下增殖的细胞。另外,显微镜检查图像展示出与基于CaCl2-PRP的凝块相比,在3和7天后在基于原纤化胶原的凝块周围增殖的大量细胞。
实施例5
基于rh胶原的凝块的机械强度
表征基于rh胶原的凝块的机械强度并且与受凝血酶活化的PRP和用CaCl2活化的PRP比较。图4中描绘了结果。根据施加的负载测量凝块样品的变形和对负载的抵抗力。对基于凝血酶和CaCl2-PRP的凝块施加高达10P的最高剪切负载。基于凝血酶的凝块的变形较高并且随着施加的剪切负载增加到400%以上。发现凝块的最大变形是要通过将PRP与CaCl2和PBS混合在一起。这表明当施加10Pa的最高剪切负载时,在其变形上增加高达180%。相反,当对基于rh胶原的凝块施加10Pa的剪切负载时,未检测到变形。当对通过直接混合制备的或首先用PBS水合,然后与PBS混合的两种类型的基于rh胶原的凝块施加高达100Pa的更高剪切负载时,检测到17-27%的变形。对试验的所有类型的基于rh胶原的凝块而言,包括呈原纤维、交联和两者组合形式的胶原,均检测到如此低的变形值。这些结果强调了相对于凝血酶而言基于rh胶原的凝块的稳定性并且可暗凝块更长时间用作支架并因此以持续和渐进方式释放生长因子的能力。
实施例6
基于rh胶原的凝块的体外降解测定
将基于不同rh胶原的凝块的降解与基于凝血酶的凝块和单独用CaCl2活化的血小板比较。跟踪凝块尺寸和形态上的变化30天。基于凝血酶的凝块的降解(图5A-B,蓝色圆圈)在第7天开始并且到第12天完全溶解。另一方面,将不同的rh胶原薄片构型(原纤维状、原纤维状且交联)直接与PRP混合或首先与PBS混合,然后与PRP混合以形成凝块并且发现在培育30天后稳定并不降解。上述结果加强了基于rh胶原的凝块比基于凝血酶的凝块稳定得多并且因此可留在组织内更长时间以允许生长因子持续释放并因此增加愈合功效的想法。

1.一种包含交联胶原、富血小板血浆(PRP)和无机盐的物质组合物。
2.根据权利要求1所述的物质组合物,其中所述无机盐选自由钠盐、氯盐、钾盐、钙盐、镁盐、磷酸盐、硫酸盐和羧酸盐所组成的组。
3.根据权利要求2所述的物质组合物,其中所述无机盐选自由NaCl、KCl、CsCl、CaCl2、CsF、KClO4、NaNO3和CaSO4所组成的组。
4.根据权利要求3所述的物质组合物,其中所述无机盐为CaCl2。
5.根据权利要求1所述的物质组合物,其呈液态。
6.根据权利要求1所述的物质组合物,其呈半固态。
7.根据权利要求6所述的物质组合物,其为可缝合的。
8.根据权利要求1所述的物质组合物,其中所述胶原包含人胶原。
9.根据权利要求1所述的物质组合物,其中所述胶原包含I型胶原。

16.一种包含胶原、富血小板血浆(PRP)和无机盐的物质组合物,其在37℃下培育10天后能够释放超过4000pg/ml的血小板源生长因子(PDGF)。
17.根据权利要求16所述的物质组合物,其能够在37℃下在约6-7分钟内产生凝块。
18.根据权利要求17所述的物质组合物,其中所述凝块为可缝合的。
19.根据权利要求16所述的物质组合物,其在37℃下培育30天后耐降解。
20.一种在有需要的受试者中治疗伤口或诱导组织再生的方法,其包括向所述受试者施用治疗有效量的根据权利要求1或16所述的物质组合物,从而治疗所述伤口或诱导组织再生。
21.根据权利要求20所述的方法,其中所述受试者具有腱或骨疾病。
22.根据权利要求20所述的方法,其中所述组织为腱或骨。

24.一种生成用于治疗伤口或诱导组织再生的物质组合物的方法,其包括:
(a)生成胶原和无机盐的混合物;并且
(b)使所述混合物与PRP接触,从而生成用于治疗伤口或诱导组织再生的所述物质组合物。
25.根据权利要求24所述的方法,其中所述无机盐选自由钠盐、氯盐、钾盐、钙盐、镁盐、磷酸盐、硫酸盐和羧酸盐所组成的组。
26.根据权利要求25所述的方法,其中所述无机盐选自由NaCl、KCl、CsCl、CaCl2、CsF、KClO4、NaNO3和CaSO4所组成的组。

28.根据权利要求24所述的方法,其中通过原纤化重组胶原溶液获得所述胶原。
