Abstract

发明背景
细胞凋亡或程序性细胞死亡在所有多细胞生物的发育和维持稳态过程中起着重要作用。癌症的常见标志是抵抗天然的细胞凋亡信号。根据癌症的类型,这种抵抗通常是由于细胞凋亡级联中关键蛋白的上调或下调,或者是由于编码这些蛋白的基因发生突变。这些变化在内源性细胞凋亡途径和外源性细胞凋亡途径中均有发生,其中内源性凋亡途径经由(funnel through)线粒体和半胱天冬酶-9(caspase-9),外源性凋亡途径涉及死亡受体与半胱天冬酶-8的作用。例如,已经在癌症中观察到了蛋白的适当水平的改变,所述蛋白诸如p53、Bim、Bax、Apaf-1、FLIP和许多其他蛋白。这些改变可导致有缺陷的细胞凋亡级联,其中之一是上游的促细胞凋亡信号没有被充分地传递以激活执行者半胱天冬酶——半胱天冬酶-3和半胱天冬酶-7。
由于大多数细胞凋亡途径最终都涉及半胱天冬酶原-3酶原(procaspase-3)的激活,因而上游的基因变异为细胞凋亡通路中的有效“中断”,因此,这些细胞非典型地增殖。鉴于细胞凋亡在癌症中的重要作用,一直在努力开发以细胞凋亡级联中的特异蛋白为靶的治疗剂。例如,与级联成员诸如p53和Bcl家族中的蛋白结合的肽的或小分子的结合剂,或者与蛋白的细胞凋亡抑制剂(IAP)家族结合的肽的或小分子的结合剂,具有促细胞凋亡活性,促进Apaf-1低聚化的化合物也同样具有促细胞凋亡活性。然而,因为这些化合物以细胞凋亡级联中的早期(或中间的至高的)位置为靶,因此,携带影响这些成员下游蛋白的突变癌症仍然可以抵抗这些化合物的可能的有益效果。
对治疗目的有利的是鉴定直接激活细胞凋亡级联中极下游(fardownstream)的促细胞凋亡蛋白的小分子。这种方法能够影响级联中比较低的位置,因此,使得甚至能够杀死那些携带影响上游细胞凋亡机制的突变的细胞。此外,如果促细胞凋亡蛋白质在癌细胞中被上调了或者以增加的水平存在于癌细胞中,则这样的治疗策略成功的可能性较高。因此,鉴定以细胞凋亡的下游效应蛋白——半胱天冬酶-3酶原——为靶的小分子,将显著有助于目前的癌症治疗。
半胱天冬酶-3酶原到半胱天冬酶-3的转化或激活导致活化的“执行者”半胱天冬酶形式的产生,所述活化的“执行者”半胱天冬酶形式随后催化众多蛋白底物的水解。活化的半胱天冬酶-3是异二聚体的均二聚体,并且是通过半胱天冬酶-3酶原的蛋白酶解产生的。在体内,这种蛋白酶解激活通常经由半胱天冬酶-8或半胱天冬酶-9的作用而发生。为确保酶原(前酶)不被过早地激活,半胱天冬酶-3酶原具有12个阻止接近蛋白酶解的ETD位点(氨基酸序列,ile-glu-thr-asp)的氨基酸“安全制动片(safety catch)”。这种安全制动片使得半胱天冬酶-3酶原能够抵抗自催化激活和经半胱天冬酶-9的蛋白酶解。突变研究表明,三个相连的天冬氨酸残基似乎是该安全制动片的关键性组成。安全制动片的位置对pH敏感,因此,一旦发生细胞酸化(如在细胞凋亡过程中发生),安全制动片被认为允许接近蛋白酶解的所述位点,活化的半胱天冬酶-3可通过半胱天冬酶-9的作用或经由自身激活机制而产生。
在某些癌症中,半胱天冬酶-3酶原的水平相对于正常组织是提高的。来自20位结肠癌患者的原代分离物(primary isolates)的研究显示,平均起来,相对于临近的非癌性组织,半胱天冬酶-3酶原在这些分离物中上调了六倍。此外,在某些神经母细胞瘤、淋巴瘤和肝癌中,半胱天冬酶-3酶原上调。而且,美国国家癌症研究所(National CancerInstitute,NCI)治疗发展部(Developmental Therapeutics Program)对用于癌症筛选的60个细胞系调查对象(cell-line panel)中的半胱天冬酶-3酶原的水平进行了系统评价,揭示某些肺癌、黑素瘤、肾癌和乳腺癌表现出极大增强的半胱天冬酶-3酶原表达水平。
鉴于活化的半胱天冬酶-3在实现细胞凋亡中的作用、某些癌性细胞类型中相对高水平的半胱天冬酶-3酶原以及备受关注的安全制动片-介导的半胱天冬酶-3酶原自身激活的抑制,直接修饰半胱天冬酶-3酶原的小分子能够在靶向癌症治疗中具有非常大的适用性。
对于治疗癌症患者,组合治疗已标准化。组合治疗药物鸡尾酒疗法的目标是在化学治疗剂之间实现协同或相加作用,借此促进缩短治疗时间,降低毒性并增加患者存活。作用于单一生化途径的药物对协同或增强作用而言是特别强有力的候选者,因为,它们可以模拟“合成致死”基因组合("synthetic lethal"genetic combinations)。例如,如在细胞培养、动物模型和人体临床试验中所证明的,聚(ADP-核糖)聚合酶-1(PARP-1)(一种促进DNA损伤修复的酶)的抑制剂,有效地与DNA损伤剂协同。然而,仍需要更多的有效治疗用于治疗多种形式的癌症,抗癌药物的新的协同组合将有助于实现这一目的。相应地,需要鉴定新的细胞毒性药剂,它们在杀死癌细胞方面是有效的,还保护正常宿主组织免受细胞毒性药剂的不期望的毒性。
发明内容
本发明大体上提供化合物、组合物以及治疗学治疗的方法。在不同的实施方案中,本发明适用于多种癌症和癌细胞类型诸如乳腺癌、淋巴瘤、肾上腺癌、肾癌、黑素瘤、白血病、神经细胞瘤、肺癌、脑癌和本领域已知的其他癌症。本文尤其公开了包括能够诱导细胞死亡的小分子的组合物和方法。在某些实施方案中,所述组合物和方法涉及能够直接或间接与程序性细胞死亡途径成员诸如半胱天冬酶-3酶原相互作用的化合物。在某些实施方案中,与直接或间接与程序性细胞死亡途径成员诸如半胱天冬酶-3酶原相互作用的其他化合物相比,所述组合物和方法具有降低的神经毒性。
组合抗癌治疗可由以不同生化途径为靶的药物组成,或者由那些在相同途径中打击不同靶点的化合物组成,模拟“合成致死”的基因组合。半胱天冬酶-3酶原激活剂PAC-1和烷基化剂替莫唑胺(TMZ)的组合对于诱导癌细胞的细胞凋亡性死亡显示相当大的协同作用,达到远远超过相加作用的程度。PAC-1和TMZ的组合有效降低肿瘤模型中的肿瘤负荷,其中单独的化合物具有极微小的作用或没有作用。这些数据表明PAC-1/TMZ组合用于治疗癌症的有效性,更广泛而言,表明该协同组合提供显著加强的治疗益处。
因此,本发明提供一种组合物,其包含(a)式(I)的化合物、(b)化合物PAC-1和(c)药学上可接受的稀释剂、赋形剂或载体,
式(I)的化合物可以是替莫唑胺(TMZ)。在其他实施方案中,式(I)的化合物的结构可替换为TMZ的衍生物或其前药。所述载体可包括水和有利于递送活性成分的任选组分诸如缓冲剂、糖、环糊精或其各种结合。在一个实施方案中,所述环糊精是2-羟丙基-β-环糊精。
式I化合物的浓度可为约100μM至约1mM,通常为约250μM、约500μM或约750μM。PAC-1的浓度可为约2μM至约50μM,通常为约2.5μM、约5μM、约7.5μM、约10μM、约12.5μM、约15μM、约20μM、约25μM、约30μM、约40μM或约50μM。例如,在一个实施方案中,式I化合物的浓度可为约250μM至约750μM,并且PAC-1的浓度可为约5μM至约30μM。
本发明还提供抑制癌细胞生长或增殖的方法。该方法包括使癌细胞与有效量的本文所描述的组合物接触,其中所述组合物可包括PAC-1和TMZ中的一种或两种。当所述组合物只包括PAC-1和TMZ中的一种时,该方法包括随后使癌细胞与另一种接触。癌细胞与这些活性剂(PAC-1和TMZ)的接触抑制所述癌细胞的生长或增殖。
本发明进一步提供在癌细胞中诱导细胞凋亡的方法,包括使癌细胞与有效量的式(I)的化合物和有效量的化合物PAC-1接触,从而在癌细胞中诱导细胞凋亡,
所述接触可以是体外的。或者,所述接触可以是体内的。在一个实施方案中,所述癌细胞可与式(I)的化合物和PAC-1同时接触。在另一实施方案中,可先使所述癌细胞与式(I)的化合物接触,再使所述癌细胞与PAC-1接触。在另一个实施方案中,可先使所述癌细胞与PAC-1接触,再使所述癌细胞与式(I)的化合物接触。
本发明还提供在有需要的患者中治疗癌症的方法。该方法包括同时或相继地给予患者治疗有效量的式(I)的化合物和有效量的化合物PAC-1,从而治疗癌症,
在一个实施方案中,式(I)的化合物和化合物PAC-1可同时给药。在另一实施方案中,式(I)的化合物和化合物PAC-1相继给药。相继给药时,式(I)的化合物可在化合物PAC-1之前给药,或者式(I)的化合物可在化合物PAC-1之后给药。
不同实施方案中的所述癌细胞可以是位于脑组织中的癌细胞或位于骨组织中的癌细胞。例如,所述癌细胞可以是胶质母细胞瘤细胞或少突神经胶质瘤细胞。在另一实施方案中,癌细胞可以是骨肉瘤细胞。下面将进一步描述可被抑制的其他类型的癌细胞以及可被治疗的其他类型的癌性病症。
因此,本发明提供本文描述的组合物用于医学治疗的用途。所述医学治疗可以是治疗癌症,例如,乳腺癌、肺癌、胰腺癌、前列腺癌、结肠癌以及本文描述的其他癌症。本发明还提供本文描述的组合物用于制备治疗哺乳动物中的疾病的药物的用途,例如,治疗人类的癌症。因此,本发明提供本文描述的化合物用于制备用于治疗哺乳动物癌症的药物的用途,所述哺乳动物诸如人类。所述药物可包含药学上可接受的稀释剂、赋形剂或载体。

具体实施方式
作为更进一步的介绍,已经发现了能够激活酶的化合物,所述酶在癌细胞中常常是过表达的或以其非活性形式的增加的水平存在。所述化合物可诱导癌细胞的程序性细胞死亡(细胞凋亡),包括那些具有上调的或增加水平的半胱天冬酶-3酶原的癌细胞。许多癌症抵抗标准的化学疗法。本文所描述的组合治疗利用半胱天冬酶-1酶原经由PAC-1的激活,这与TMZ的DNA烷基化性质产生协同作用,从而在以下情况下提供有效性:一种单独的活性剂可能较低效或完全无效。这些化合物也可成功用于靶向的癌症治疗,其在杀死癌细胞的选择性方面具有优势,同时相对地降低对具有较低水平半胱天冬酶-3酶原的非癌性细胞的副反应。这些副反应可包括毒性,特别是神经毒性。
本文所描述的化合物的结合物、组合物和方法可经由调控细胞凋亡或程序性细胞死亡以及在癌细胞的治疗中有效的DNA烷基化而起作用。在一个实施方案中,细胞凋亡的调控是通过诱导或激活细胞凋亡。在不同的实施方案中,化合物的给药可以是同时的或可相继的。
因此,本发明提供通过PAC-1增强替莫唑胺(TMZ)作用的方法,例如,用于治疗胶质母细胞瘤或骨肉瘤。在细胞凋亡过程中,酶原半胱天冬酶-3酶原经由蛋白酶解被激活为半胱天冬酶-3,然后,这种活化的半胱天冬酶-3切割细胞基质的刻痕(scores),执行细胞凋亡程序。因为半胱天冬酶-3酶原蛋白水平在不同的肿瘤组织学中均被提高,所以,药物介导的半胱天冬酶-3酶原的直接激活作为选择性的抗肿瘤策略是高效的。
某些化合物可增强半胱天冬酶-3酶原的活性和自成熟(automaturation),并诱导癌细胞的细胞凋亡。激活半胱天冬酶酶原的化合物-1(PAC-1)经由抑制锌离子的螯合作用而增强半胱天冬酶-3酶原的活性,在培养物中诱导癌细胞的细胞凋亡,在多个鼠肿瘤模型中具有有效性。已经发现治疗剂的新的组合——PAC-1与TMZ——在治疗癌细胞时是协同有效的,特别是本文所描述的胶质母细胞瘤细胞和骨肉瘤细胞。
在9L大鼠胶质母细胞瘤细胞系中的模型试验提供明确的数据来支持药物结合物的协同作用和有效的活性。体内大鼠试验利用9L的细胞颅内植入于3个治疗组(单独的PAC-1,单独的TMZ以及PAC-1和TMZ的结合物)和一个对照组的大鼠中,是一种非常具有侵入性的肿瘤模型。
将PAC-1悬浮于水中,以50mg/kg(相对低的剂量)的剂量经由口服灌胃对大鼠进行给药,持续5天,随后5天以TMZ给药。当经口给药化合物高达200mg/kg时,在小鼠中未观察到PAC-1的神经毒性,在当前的试验中未观察到神经毒性。采用所述组合治疗的大鼠的存活益处是显著的并且异常令人关注(图1)。
图1示意性地说明当9L细胞颅内植入于大鼠中获得的数据。在0-4天时经口服灌胃给药PAC-1(50mg/kg的水溶液),在5-9天时经口服灌胃给药TMZ(50mg/kg的H2O溶液)。每组8只大鼠;相对于单独的TMZ,获得了0.001的p-值(Gehan-Breslow-Wilcoxon检验)。
采用9L细胞系最初评估的PAC-1的IC50为约7mM(72小时试验)。所述9L细胞系脑肿瘤通常是出血性的,但在PAC-1/TMZ治疗的大鼠中,所述肿瘤不是出血性的,表明抗血管生成的效果。
图2示出了PAC-1与TMZ协同以诱导培养物中胶质母细胞瘤细胞死亡的三个实施例。图3示出了PAC-1与TMZ协同以诱导培养物中骨肉瘤细胞死亡的两个实施例。图4示出了PAC-1与TMZ协同以延长转移性骨肉瘤在小鼠模型中的存活的实施例。

PAC-1(2-(4-苄基哌嗪-1-基)-N-[(2-羟基-3-丙-2-烯基-苯基)亚甲基氨基]乙酰胺)选择性地诱导癌性细胞的细胞凋亡。制备PAC-1的方法描述于U.S.专利公开号2012/0040995(Hergenrother等)中。
替莫唑胺(TMZ)是属于烷基化剂的细胞毒化学治疗药物。TMZ是咪唑并四嗪的衍生物,并且是MTIC(3-甲基-(三氮烯-1-基)咪唑-4-羧酰胺)的前药。TMZ及其衍生物的制备描述于U.S.专利号5,260,291(Lunt等)中。
替莫唑胺的治疗益处源于其能够烷基化/甲基化DNA,这可发生在鸟嘌呤残基的N-7或O-6位上。该甲基化破坏DNA并触发肿瘤细胞的死亡。一些肿瘤细胞能够修复这种类型的DNA损伤,因此减小替莫唑胺的治疗效果。这种抵抗的机制可能是通过表达蛋白质O-6-甲基鸟嘌呤-DNA甲基转移酶(MGMT)或O-6-烷基鸟嘌呤-DNA烷基转移酶(AGT或AGAT)。O-6-甲基鸟嘌呤-DNA甲基转移酶(MGMT)蛋白质在脑肿瘤中的存在预示着对替莫唑胺差的响应,并且这些患者从采用替莫唑胺的化学治疗中受到很少的益处。相应地,对于脑肿瘤和相关病症的治疗而言需要新的疗法。
TMZ已经被用于治疗IV级星形细胞瘤,也被称为多形性胶质母细胞瘤,是一种非常具有侵入性的脑肿瘤,以及少突神经胶质瘤脑肿瘤。TMZ已经被用于治疗黑素瘤,并被进一步标示用于复发的III级多形性成胶质细胞瘤。
虽然利用作用于不同靶点药物的组合的抗癌策略具有明确的益处,然而,本文所描述的工作证明采用经由不同机制起效的化合物可观察到备受关注的协同作用。当追求酶的激活时,该多靶点方法具有独特的优点。
PAC-1在哺乳动物中是安全的,并且PAC-1的一种衍生物在患有淋巴瘤的宠物狗I期临床试验中是有效的(Peterson等,Cancer Res70,7232-7241(2010)),因此观察到的与TMZ的协同作用将具有显著的临床效果。对采用小分子激活酶的关注日益增加。本文所记载的数据表明,使用PAC-1和TMZ的靶向策略是用于备受关注的增强预期生物学活性的普遍方法,并由于其有效性而具有重要的临床效果。
发明方法
本发明提供在癌细胞中选择性地诱导细胞凋亡的方法,包括给予癌细胞能够修改所述癌细胞中半胱天冬酶-3酶原分子的化合物的结合物;其中所述化合物的结合是PAC-1和TMZ。还提供在癌细胞中选择性地诱导细胞凋亡的方法,包括给予癌细胞能够修改所述癌细胞中半胱天冬酶-3酶原分子的化合物的结合物;其中所述化合物的结合是PAC-1和TMZ,例如,其中所述癌细胞位于需要治疗的患者中。
本发明提供另外的方法,其中所述化合物的结合是PAC-1和TMZ,例如,作为治疗癌细胞的方法,包括(a)鉴定对采用半胱天冬酶酶原激活剂化合物治疗癌细胞的潜在敏感性;和(b)将所述癌细胞暴露于有效量的半胱天冬酶酶原激活剂化合物和TMZ的结合物。还提供治疗癌细胞的方法,包括(a)鉴定对采用半胱天冬酶酶原激活剂化合物治疗癌细胞的潜在敏感性;和(b)将所述癌细胞暴露于有效量的PAC-1和TMZ;其中所述PAC-1能够激活半胱天冬酶-3酶原和半胱天冬酶-7酶原中的至少一种。还提供在癌细胞中诱导死亡(例如,杀死癌细胞)的方法,包括给予癌细胞TMZ以及能够激活癌细胞中半胱天冬酶-3酶原分子的化合物。
本发明进一步提供包含有效量的PAC-1和TMZ的结合物的药物。所述药物可用于诱导细胞凋亡的方法中。在某些实施方案中,化合物的所述结合不穿过血脑屏障到在患者中引起可评估的神经毒性作用的程度。本发明的方法包括使一种或更多种细胞与本文所描述的有效量的化合物的结合在体内或体外接触。因此,本发明还提供治疗细胞的方法,包括使细胞与本文所描述的有效量的化合物的结合物接触。

术语“癌症”通常是指由异常细胞的不受控生长引起的超过100种疾病的组中的任何一种。癌症可表现为实体瘤和淋巴瘤的形式,以及非实体瘤诸如白血病。与正常细胞再生直至成熟然后只在必要时替换受伤细胞不同,癌细胞可不断地生长和分裂,将邻近的细胞挤出并最终蔓延到身体的其它部分。
本发明提供用于治疗癌症或癌性病症的方法。术语“癌性病症”涉及细胞以异常状态存在的任何病症或以快速增殖或瘤形成为特征的病症。癌性病症在本质上可以是恶性的或非恶性的(例如癌前病症)。为了进一步描述“癌性病症”,可使用术语“过度增殖的”、“增生的”、“超常增生”、“恶性的”、“肿瘤的”和“瘤形成”。这些术语可互换地使用,并意图包括所有类型的过度增殖生长、增生性生长、癌性生长或致癌过程、转移性组织或恶性转化的细胞、组织或器官,不考虑组织病理学类型、侵袭力阶段或癌性确定(cancerous determination)(例如恶性的和非恶性的)。

已经发现PAC-1和TMZ的组合对于治疗脑的癌症是特别有效的。脑的癌症包括,但不限于,少突神经胶质瘤和胶质母细胞瘤包括多形性胶质母细胞瘤(GBM)。被癌性细胞侵袭的组织可位于脑本身(例如,颅或中央椎管)或在淋巴组织中,在血管中,在颅神经中,在脑外膜(脑脊膜)、头盖骨、垂体或松果腺中。可被治疗的脑癌的具体形式包括星形细胞瘤、软骨瘤、软骨肉瘤、脊索瘤、CNS(中枢神经系统)淋巴瘤、颅咽管瘤、室管膜瘤、神经节神经胶质瘤、神经节瘤(也称作神经节细胞瘤)、神经胶质瘤包括星形细胞瘤、少突神经胶质瘤和室管膜瘤,血管母细胞瘤(也称作脉管肿瘤)、原始神经外胚瘤(PNET)诸如髓母细胞瘤、脑膜瘤和前庭神经鞘瘤(以前称作听神经瘤/神经鞘瘤)。
所述结合物还可用于治疗源于身体其它器官的癌症而侵入颅内球的转移性肿瘤。这些病症通常称为继发性脑肿瘤。可采用PAC-1和TMZ的组合治疗的继发性脑肿瘤包括源于肺癌、乳腺癌、恶性黑素瘤、肾癌、结肠癌和其它癌症的脑转移瘤。
本发明范围内的癌性病症的其它实例包括,但不限于,神经母细胞瘤和成骨的癌(例如,骨癌或骨组织的肿瘤生长)。可采用PAC-1和TMZ的组合治疗的恶性原发性骨肿瘤的实例包括骨肉瘤、软骨肉瘤、尤因瘤、纤维肉瘤等,以及继发性骨肿瘤诸如已从其它器官包括乳腺癌、肺癌和前列腺癌扩散的转移瘤。
药物制剂
本文所描述的化合物可用于制备治疗学药物组合物,例如,通过将所述化合物与药学上可接受的稀释剂、赋形剂或载体组合。所述化合物可以盐或溶剂化物的形式加入到载体中。例如,在化合物具有足够的碱性或酸性以形成稳定无毒的酸的或碱的盐的情况中,所述化合物作为盐给药是适当的。药学上可接受的盐的实例是采用形成生理学上可接受的阴离子的酸形成的有机酸加成盐,例如,甲苯磺酸盐、甲磺酸盐、乙酸盐、柠檬酸盐、丙二酸盐、酒石酸盐、琥珀酸盐、苯甲酸盐、抗坏血酸盐、α-酮戊二酸盐和β-甘油磷酸盐。还可形成适宜的无机酸盐,包括盐酸盐、卤化物、硫酸盐、硝酸盐、碳酸氢盐和碳酸盐。

可将本文所描述的化合物配制成多种形式的药物组合物并给予哺乳动物宿主,诸如人类患者。所述形式特别适用于所选择的给药途径,例如,口服或经由静脉、肌内、局部或皮下途径的肠胃外给药。
本文所描述的化合物可与药学上可接受的媒介物诸如惰性稀释剂或可吸收的可食载体组合而全身给药。可通过使用环糊精诸如2-羟丙基-β-环糊精增加活性成分的溶解性。对口服给药而言,可将化合物包封于硬或软壳胶囊中、压成片剂,或直接掺入患者饮食的食物中。化合物还可与一种或多种赋形剂组合,并以可吸收的片剂、口含片剂、锭剂(troches)、胶囊、酏剂(elixir)、混悬剂、糖浆剂、糯米纸囊剂(wafers)等形式使用。所述组合物和制剂通常含有至少0.1%的活性化合物。组合物和制剂的百分数可变化并便利地为给定单位剂量形式重量的约1%至约60%或约2%至约25%。活性化合物于所述治疗上有用的组合物中的量是如此,以至于可获得有效的剂量水平。
片剂、锭剂、丸剂、胶囊剂等还可含有下列中的一种或更多种:粘合剂诸如黄蓍胶、阿拉伯胶、玉米淀粉或明胶;赋形剂诸如磷酸二钙;崩解剂诸如玉米淀粉、马铃薯淀粉、海藻酸等;以及润滑剂诸如硬脂酸镁。可加入甜味剂诸如蔗糖、果糖、乳糖或阿司帕坦,或芳香剂诸如薄荷、冬青油或樱桃香精。当单位剂量形式是胶囊时,除上述类型的材料外,它可含有液体载体诸如植物油或聚乙二醇。多种其他材料可作为包衣存在或者否则改变固体单位剂量形式的物理形式。例如,片剂、丸剂或胶囊可包覆明胶、蜡、虫胶或糖等。糖浆剂或酏剂可含有活性化合物,蔗糖或果糖作为甜味剂,对羟基苯甲酸甲酯和对羟基苯甲酸丙酯作为防腐剂,着色剂和调味剂诸如樱桃或橙子香精。用于制备任何单位剂量形式的任何材料应该是药学上可接受的并且在所使用的量下是基本无毒的。此外,活性化合物可被掺入缓慢释放的制剂和装置中。
活性化合物可通过输注或注射而静脉内或腹膜内给药。活性化合物或其盐的溶液可在任选混合有无毒表面活性剂的水中制备。分散体(dispersion)可在甘油、液态聚乙二醇、三醋汀或其混合物中,或在药学上可接受的油中制备。在普通的贮存和使用条件下,制剂可包含防腐剂以防止微生物的生长。
适于注射或输注的药物剂量形式可包括无菌水溶液、分散相或无菌粉末,所述无菌粉末包含适合临时配制成无菌可注射的或可输注的溶液或分散相的活性成分,任选包封于脂质体中。最终的剂量形式在生产和贮藏条件下应当是无菌的、液态的和稳定的。液态载体或媒介物可以是溶剂或液态分散介质,包括,例如,水、乙醇、多元醇(例如,甘油、丙二醇、液态聚乙二醇等)、植物油、无毒甘油酯和它们的适宜混合物。例如,可通过形成脂质体、通过维护分散相中所要求的颗粒大小或通过使用表面活性剂,而维持适当的流动性。防止微生物的作用可通过多种抗菌剂和抗真菌剂,例如,对羟基苯甲酸类、三氯叔丁醇、苯酚、山梨酸、硫柳汞等,而实现。在许多情况下,优选包含等渗剂,例如,糖、缓冲剂或氯化钠。可注射组合物的延长吸收可通过延迟吸收剂,例如,单硬脂酸铝和/或明胶,而实现。
无菌可注射溶液根据需要可通过将活性化合物以所需的量掺入含有上面列举的多种其它成分的适当溶剂中而制备,任选随后进行过滤灭菌。在用于配制无菌可注射溶液的无菌粉末的情况中,制备方法可包括真空干燥和冷冻干燥技术,得到活性成分加上存在于先前无菌滤过溶液中的任何所需附加成分的粉末。
本文所描述的化合物的有用剂量可通过比较它们的体外活性以及在动物模型中的体内活性而测定。用于小鼠和其它动物中的有效剂量外推至人类的方法是本领域已知的;例如,参见U.S.专利号4,938,949(Borch等)。治疗应用中所需的化合物或其活性盐或衍生物的量不仅将随着所选择的特定化合物或盐而变化,还将随着给药途径、待治疗病症的性质以及患者的年龄和状况而变化,并将最终由巡诊医生或临床医生决定。
化合物的所述结合可方便地以单剂量形式给药,例如,每单位剂量形式中含有100至5,000mg/m2、300至4,000mg/m2、370至3,700mg/m2、50至750mg/m2或750至4,000mg/m2的活性成分。每种化合物各自地或在结合物中还可以如下剂量给药:约1mg/kg至约250mg/kg、约10mg/kg至约100mg/kg、约10mg/kg至约50mg/kg、约50mg/kg至约100mg/kg、约10mg/kg至约50mg/kg或约10mg/kg、约25mg/kg、约50mg/kg、约75mg/kg、约100mg/kg或约150mg/kg、或从任何一个前述数值至任何其它前述数值的范围。所述化合物还可单独或在组合中给予受试者以提供约1μmol/L至约25μmol/L或约10μmol/L或约15μmol/L的稳态血浆药物浓度。
在某些实施方案中,本发明提供有效浓度为约10nM至约100μM的化合物。在另一实施方案中,有效浓度为约200nM至约50μM、约500nM至约40μM、约750nM至约25μM、约1μM至约20μM或约1μM至约10μM。在另一实施方案中,所述有效浓度被认为是,例如,在直接半胱天冬酶酶原激活试验中、在细胞凋亡诱导试验中或在动物临床治疗评估中的50%活性浓度。在一个实施方案中,所述数值小于约200μM。在另一实施方案中,所述数值小于约10μM但大于约10nM。所期望的剂量可方便地以单次剂量给药或作为分份剂量以适当的间隔给药,例如每天两个、三个、四个或更多个亚剂量。所述亚剂量本身可被进一步分为例如许多离散的松散间隔的给药。
本文所描述的化合物是有效的抗肿瘤剂,并且与任何单个活性剂给药相比,具有更高的效力和/或降低的毒性。本发明提供在哺乳动物中治疗癌症的治疗方法,包括给予患有癌症的哺乳动物有效量的本文所描述的化合物或组合物。哺乳动物包括灵长类、人类、啮齿动物、犬科动物、猫科动物、牛科动物、绵羊、马科动物、猪、山羊、牛科动物等。癌症是指本文描述中所包括的任何不同类型的恶性肿瘤,例如,结肠癌、乳腺癌、黑素瘤和白血病,一般而言,癌症的特征为不希望的细胞增殖,例如,不受调节的生长、缺乏分化、局部组织侵入和转移。
本发明化合物治疗癌症的能力可使用本领域公知的试验而测定。例如,治疗方案的设计、毒性评估、数据分析、杀死的肿瘤细胞的量化以及使用可移植肿瘤筛选(transplantable tumor screen)的生物学意义是已知的。此外,化合物治疗癌症的能力可通过使用上面描述的以及本文所引用的引文和专利文件中描述的试验来测定。

实施例
实施例1.PAC-1与替莫唑胺(TMZ)结合在9L大鼠神经胶质瘤中的体内有效性
9L神经胶质肉瘤作为实体皮下团块维持在F344大鼠的两肋。对于颅内植入而言,将所述9L神经胶质肉瘤肿瘤从载体动物中手术分离并在植入时切成1mm3的小片。将这些1mm3肿瘤小片颅内植入于32只之前所描述的F344大鼠中(Joshi等,Evaluation of tyrosine kinaseinhibitor combinations for glioblastoma therapy.PLoS One(2012)7:e44372;Gallia等,Inhibition of Akt inhibits growth of glioblastoma andglioblastoma stem-like cells.Mol.Cancer Ther.(2009)8:386-393)。
将这些动物分为以下四个试验组,每组8只动物:(1)对照组,(2)单独的PAC-1,(3)单独的TMZ和(4)PAC-1+TMZ。对照组动物只接受水。单独的PAC-1和PAC-1+TMZ两组动物在起始0天于植入后6小时时接受悬浮于水中的PAC-1口服灌胃。PAC-1只给予5天,从0天至4天。来自单独的TMZ和PAC-1+TMZ组的动物接受5剂量的于水中的TMZ,经口给予,从5天至9天。此后试验动物不接受任何处理并评价总存活数。与采用单独的TMZ处理的动物(中位存活期=20天)或未处理的对照组动物(中位存活期=20天)相比,PAC-1+TMZ组合处理的动物具有显著增加的存活期(中位存活期=28天)(图1)。
实施例2.药物剂型
以下制剂举例说明可用于本文所描述的结合化合物(例如,PAC-1和TMZ)或其药学上可接受的盐或溶剂合物(下文中称为‘化合物X’)的治疗或预防给药的代表性药物剂型:
这些制剂可通过制药领域公知的常规步骤而制备。应理解,上述药物组合物可根据公知的制药技术而变化以适应活性成分‘化合物X’不同的量和类型。气雾剂(vi)可与标准的、计量剂量的气雾剂分配器一起使用。此外,具体的成分和比例用作说明目的。根据对目标剂量形式所期望的性质,成分可互换为适宜的等效物并且比例可改变。
虽然上面已经参考所公开的具体实施方案和实施例描述了具体的实施方案,然而,所述实施例仅是说明性的,并非限制本发明的范围。在不偏离本发明在下面权利要求中定义的其较宽泛方面的情况下,可由本领域技术人员作出变化和修改。

Claims (22)
1.一种组合物,包括:
(a)式(I)的化合物、(b)化合物PAC-1和(c)药学上可接受的稀释剂、赋形剂或载体,
2.权利要求1的组合物,其中所述载体包括水和任选地包括缓冲剂、环糊精或其结合。
3.权利要求2的组合物,其中所述环糊精是2-羟丙基-β-环糊精。
4.权利要求1的组合物,其中式I化合物的浓度为约100μM至约1mM。
5.权利要求1的组合物,其中PAC-1的浓度为约2μM至约50μM。
6.权利要求1的组合物,其中式I化合物的浓度为约250μM至约750μM,并且PAC-1的浓度为约5μM至约30μM。
7.一种抑制癌细胞生长或增殖的方法,包括使癌细胞与有效量的权利要求1的组合物接触,从而抑制癌细胞生长或增殖。
8.权利要求7的方法,其中所述癌细胞是胶质母细胞瘤细胞或少突神经胶质瘤细胞。
9.权利要求7的方法,其中所述癌细胞是骨肉瘤细胞。
10.一种在癌细胞中诱导细胞凋亡的方法,包括使癌细胞与有效量的式(I)化合物和有效量的化合物PAC-1接触,从而在癌细胞中诱导细胞凋亡,
11.权利要求10的方法,其中所述接触在体外。
12.权利要求10的方法,其中所述接触在体内。
13.权利要求10的方法,其中所述癌细胞与式(I)的化合物和PAC-1同时接触。
14.权利要求10的方法,其中先使所述癌细胞与式(I)的化合物接触,后使所述癌细胞与PAC-1接触。
15.权利要求10的方法,其中先使所述癌细胞与PAC-1接触,后使所述癌细胞与式(I)的化合物接触。
16.一种在有需要的患者中治疗癌症的方法,包括同时或相继给予患者治疗有效量的式(I)化合物和有效量的化合物PAC-1,从而治疗癌症,
17.权利要求16的方法,其中式(I)的化合物与化合物PAC-1同时给药。
18.权利要求16的方法,其中式(I)的化合物与化合物PAC-1相继给药。
19.权利要求18的方法,其中式(I)的化合物在化合物PAC-1之前给药。
20.权利要求18的方法,其中式(I)的化合物在化合物PAC-1之后给药。
21.权利要求1-6中任一项的组合物用于制备治疗癌症的药物的用途。
