Abstract

背景技术
前庭失衡是由于多种前庭障碍导致的前庭反馈丧失而引起的严重问题。前庭康复治疗通过正规训练使得通过来自视觉和本体感觉的补偿而使大脑能够适应前庭功能丧失,从而对于一些患者来说可以减轻该问题。然而,失去的前庭功能不能完全由视觉和体感线索补偿,并且适应过程可能花费几个月或几年,并且有些时候适应过程可能无法完成。患者可能会变得过度依赖视觉和本体感觉,并且当视觉参考不可靠或者行走在不平坦的表面上时,平衡不稳定性经常会恶化。
用于提供人工平衡功能的各种装置可能是植入式的或非植入式的。植入式刺激器是侵入式的,并且典型地需要外科手术以嵌入在患者的皮肤下,例如在耳道内,从而提供前庭系统的直接刺激。
非植入式假体典型地从外部施加至皮肤,因此是提供自运动反馈的侵入性很小的方法。例如,这样的假体通过经由表面电极刺激前庭神经或者通过使用“感觉替代”、例如通过声音指示、振动触觉反馈或提供给舌头的电流以显示自运动线索来操作。

在舌头的电触觉刺激的情况下,外观是一个大问题。头部运动和言语被限制。患者宁可将它用在一个私人环境下的康复训练。
已经研发了相对大但是可佩戴的运动传感前庭假体,以装配在人的躯干周围。这些能够提供具有涉及多轴中的倾斜和摇摆的信息的主题,并且能够被实施为具有“气囊”以在用户跌倒时展开。
发明内容
一种用于前庭替代的装置,包括完全装配在人口中的口器,其形状符合口的上腭或口盖(roof)。该口器封装以电子系统为主体的电路板,该电子系统可操作为基于头部运动将电脉冲传递给接触上腭的刺激器。该电子系统包括运动传感器,控制单元;刺激电路;以及电池。该运动传感器优选地是加速计。该控制单元优选地是具有数字输入/输出能力、串行外围接口/跨集成电路协议、计时器和用于传感器接口和时序控制的振荡器的微控制器。该微控制器优选地具有在存储器中被编码的指令,这使得能够处理来自加速计的数据以指示头部运动,并且控制每个嵌入电刺激器以传递具有可调的波形参数的电脉冲。该装置还可以包括无线收发器,以用于从人口外部远程控制该装置。该刺激电路以不具有净直流的电压控制的单相电脉冲的形式提供电荷平衡的刺激。该控制单元优选地使用脉冲频率调制,其中由较高频率的电脉冲指示较高大小的加速度。
技术问题
由于与尺寸相关的固有局限性,假体的可见性,构件的粗劣,以及与其他身体功能的干扰,现有的用于前庭替代的非植入式装置对于患者谨慎地在各种室内和室外活动中使用来说都不是实用的。
非临床环境中的听觉反馈的主要问题是它使用已经负载很重的感觉,并且它可能会阻碍在室内和室外活动中同样重要的语音通信和环境声音的块感知。否则,周围的噪声可能会引入错误的反馈以触发不平衡。
与振动触觉反馈系统相关联的问题包括功率损耗和随之而来的快速电池耗尽,大的系统尺寸,重的构件重量,缺乏易用性,以及不期望的外观。
对于使用舌头的电触觉刺激的装置,外观是一个大问题。头部移动和语言被限制。使用该装置的患者典型地在私人环境中经过正规的训练,然后依靠用于改进平衡的康复训练的残留影响而没有训练设备的磨损。在统计上,来自装置训练的残留影响的效果不能与康复训练治疗的效果区分开来。
解决问题的方法
由于前庭反馈是现场机构,对于室内和室外活动中的前庭替代来说,非常需要能够提供现场感觉反馈的隐藏装置。根据这样的装置,前庭患者在最需要的时候能够获得平衡帮助。
本发明的有利效果

如图2所示的优选实施例具有四个刺激器的构造,这在允许使用电触觉刺激表示头部运动的方向以及范围的同时,易于电子系统的紧凑和节能的实施。关于头部运动的感觉反馈由经过刺激器传递的小的电脉冲序列表示。范围信息被编码为刺激器的波形参数,它们能够被感知为具有变化的感觉特性。具体地,频率编码被用于指示运动的大小。该实施方式允许整个装置由微型电池、例如硬币电池供电,同时在上颚上产生用于强烈和舒适的感觉的适当强度。
电子系统被组装到嵌入在牙齿保持器中的微型电路板中。在本实施例中,牙齿保持器是具有包围上部牙齿从而保持该装置抵靠着上颚的金属钢丝夹(220)的口器。由牙齿保持器提供的机械支撑是用于将口器抵靠着保持器固定在适当位置上的优选方法。胶水或其他固定方法也是可行的。如图2所示,有四个嵌入电刺激器,也被称为刺激器。本实施例中的刺激器对称地分布在口器(110)的口盖上,并且返回电极(215)位于中心。两个刺激器相对于垂直中心线横向定位,以指示沿横向的头部运动,而另两个刺激器用于指示沿前后方向的运动。头部运动由局部刺激指示,其中局部刺激被施加至上颚表面的主要区域的神经末梢,即上颚神经以及鼻腭神经的左和右分支。本实施例中的刺激器是稍微凸起的的电极,即从口器表面突出的电极。这些电极涂敷有贵金属,以使在传递从电子系统(300)产生的刺激脉冲的同时,在装置-组织界面上的任何潜在的不可逆的电化学反应最小化。
实例2.基于微控制器的电子系统
如图3所示,优选实施例包括具有微控制器、加速计芯片和刺激电路的电子系统(300)。电子系统(300)由硬币电池供电,同时在上颚上产生用于强烈和舒适的感觉的适当强度。微控制器具有芯片上外围设备,包括数字I/O、SPI/I2C接口、计时器和用于传感器接口和时序控制的振荡器。微控制器被编程以处理来自加速计的指示头部运动数据,并且控制刺激器传递具有可调波形参数的电脉冲。使用表面安装构件使得基于微控制器的系统能够紧凑集成到微型电路板上。该示例性系统包括无线通信能力以用于从外部远程控制该装置。
在其他实施例中,电子系统可以是印刷电路板或柔性印刷电路。整个电子系统的一部分或全部可以使用专用集成电路制成,从而使得该装置能够被进一步小型化。
实例3.电荷平衡的刺激
本实例中的刺激电路(325)以不具有净DC电流的电压控制的单相脉冲的形式提供电荷平衡的刺激。在图4中示出了对于一个通道的刺激电路(一个刺激器)。来自控制单元(315)的控制信号(445)使开关S(430)暂时闭合较短时间,例如50微秒,在该期间负电荷被注入刺激电极(210),形成刺激脉冲的阴极相位。然后开关S(430)打开,并且电容器C(435)经过电阻器R(420)逐渐放电使电流反向,从而使得被注入的前一个相位的电荷缓慢地撤回。该控制序列可以被重复以产生许多刺激脉冲。刺激电路具有电压转换(410),其将来自电池的电压转换为对于上颚刺激优选的电压(大约15-30伏),这能够由另一个微控制器控制(405)调节。对置电极、也被称为返回电极(215)允许电流在上颚组织中来回流动。
实例4.头部运动的电触觉表示
在本实例中,四个刺激器被用于表示中间-侧面平面中的二维加速度矢量,两个正交分量分别沿x和y轴。每个分量的符号由四个刺激器中的一个上的局部刺激指示。通过使被用于该刺激器的波形参数之一变化,而将该分量的大小编码成刺激波形。本实例使用脉冲频率调制,其中由较高频率的刺激脉冲指示较高大小的加速度。
在本实例中,根据不同的函数调制脉冲频率,该函数包括:
线性函数:f=c(|ai|-Amin)+Fmin和c=(Fmax-Fmin)/(Amax-Amin)。
反正切函数:
指数函数:f=Fmax-(Fmax-Fmin)exp[-c(|ai|-Amin)]。
线性函数在从Amin至Amax的加速度动态范围内具有由c限定的灵敏度常数,但是较高的灵敏度通常与减小的动态范围相关。超过该范围,脉冲频率典型地不响应于加速度变化。反正切函数在加速度的较低范围内几乎是线性的,同时在较高端保持脉冲频率响应于加速度变化。指数函数典型地在加速度的低端(Amin)上具有最大灵敏度,然后随着加速度增加而指数地降低。Amin限定了没有反馈的区域,在该区域中被认为保持平衡。频率能够在Fmin和Fmax之间的范围内变化,其应该被选择例如在2Hz与80Hz之间,从而增强大脑的辨别力和感知力。
实例5.正交分量的表示
在本实例中,微控制器包括在非暂态存储器上被编码的指令,这些指令在被实施时执行优选的方法以呈现加速度矢量的正交分量。这些方法包括:1)单激活。在任意时刻仅四个刺激器中的一个被激活,其总是指示加速度矢量中的较大分量。这种方法为人体识别提出更容易的任务。随着个人尝试进行身体调整,两个正交分量中的任何一个可以变得更显著。只有更显著的分量被呈现用于人体感知。2)双激活。两个刺激器被同时激活以表现两个正交分量。两个激活的刺激器可以使用不同的脉冲频率。双激活的方法可能会对大脑提出更复杂的任务,这是因为它需要区别这两个刺激,识别哪个分量更显著,并且形成加速度矢量所指向的感知。
本发明可以应用于其他用途。例如,如果运动传感器被安装在外部物体(例如飞行器)上,并且与口腔内装置无线通信,则本发明能够为用户提供关于外部物体的倾斜或运动的反馈。

Claims (5)
1.一种在人口内使用的用于前庭替代的装置,人口包括上腭,所述装置通过在上腭上的电刺激来提供头部运动或倾斜的替代感觉反馈,所述装置包括:
口器,所述口器完全装配在人口中,所述装置可移除地抵着上腭佩戴在人口中,所述口器配置为符合上腭,
所述口器包括:
电路板,所述电路板被封装在所述口器中,所述电路板限定电子系统,所述电子系统能够操作以产生电脉冲而不需要通过电线电连接至人口外部的其他构件,所述电子系统包括:
运动传感器,用于检测头部的运动,所述运动传感器包括位置传感器、速度传感器、加速度传感器、旋转传感器中的一个或多个;
控制单元;
刺激电路;以及
电池;
所述口器进一步包括多个嵌入电刺激器,当所述口器被安装在人口中时,所述嵌入电刺激器与上腭接触,并且
其中,所述嵌入电刺激器被连接至所述电子系统,从而使得所述多个嵌入电刺激器中的每个嵌入电刺激器都能够从所述电子系统接收电脉冲并且将所述电脉冲传送至上腭,
所述控制单元使用脉冲频率调制,在所述脉冲频率调制中,由电脉冲的频率指示所述运动的大小。
2.根据权利要求1所述的装置,其中:
所述运动传感器包括加速计;并且
所述控制单元包括微控制器,所述微控制器包括芯片上外围设备,所述芯片上外围设备包括:数字I/O;SPI/I2C数字接口;计时器;以及用于传感器接口和时序控制的振荡器;所述微控制器被配置为:处理来自所述加速计的数据以指示头部运动,并且控制每个嵌入电刺激器来传递具有可调波形参数的电脉冲。
3.根据权利要求1所述的装置,进一步包括无线收发器,以用于从人口外部远程控制所述装置。
4.根据权利要求1所述的装置,其中,所述电子系统中的所述刺激电路以不具有净直流的电压控制的单相电脉冲的形式提供电荷平衡的刺激。
5.根据权利要求1所述的装置,其中,所述运动传感器是加速度传感器,并且其中,所述控制单元使用脉冲频率调制,在所述脉冲频率调制中,由较高频率的电脉冲指示较大的加速度。
