背景技术
不断精化的免疫抑制方法已经大幅减少了在实体器官移植后急性排斥的发生。但是,长期效果部分由于与免疫抑制有关的发病率和死亡率而停滞。对免疫抑制的传统方法强调T细胞应答的非特异性抑制。

发明概述
本公开总体涉及用于免疫疗法的调节T细胞(Treg)的制造。具体而言,本公开涉及用于同种异型抗原-反应性Treg(allo Treg)体外增殖的有效方法。以这种方式生产的alloTreg适用于诱导和/或保持同种异型移植在受体中的免疫耐受。

图4A提供了供体特异性检测的结果。如所示,供体-反应性Treg使用CFSE标记并再刺激。图4B提供了混合淋巴细胞反应(MLR)抑制检测的结果。滴定数量的供体-反应性Treg和多克隆增殖的Treg与2.5x104同源的PBMC和1.25x105经辐射的供体的PBMC混合,并温育6天。在后16小时中加入氚标记的胸腺嘧啶脱氧核苷酸。通过与不具有Treg的孔中每分钟的计数(CPM)相比较来计算对引入的胸腺嘧啶脱氧核苷酸的抑制。
图5示出了供体-反应性T细胞频率检测的结果。受体PBMC是使用CFSE标记的,并使用供体sBc刺激3.5天。收获培养物,对CD3、CD4、CD8、Foxp3和Helios进行染色,并在流式细胞仪上分析。CD8、CD4+Tconv、和Treg的CFSE的图谱用于计算各亚群中供体-反应性T细胞的频率。

图10A、图10B和图10C示出了由BALB/c.Rag2-/-γc-/-小鼠得到的数据,该小鼠移植有人类的皮肤,并使用与皮肤供体为同种异型的PBMC再生。通过计数每种移植物每个色斑的4至6个高功率视野来分析免疫荧光显微照片的图像。然后比较由4个试验组得到的定量结果。使用具有Bonferroni多重比较检验的单向ANOVA来测定差异的统计学意义。
图11A为试验模型的示意图,并示出过程。图11B示出了在试验结束时测定的PBMC再生,证明Treg的共注入没有显著地改变PBMC再生的程度。图11C示出了4个试验组中BALB/c.Rag2-/-γc-/-小鼠的体重,评估体重以用于测定大致的健康状态,从而证明PBMC的注入不会诱导移植物抗宿主的疾病。

发明详述
本公开总体涉及用于免疫疗法的调节T细胞(Treg)的制造。具体而言,本公开涉及用于同种异型抗原-特异性Treg(allo Treg)体外增殖的有效方法。以这种方式生产的同种异型抗原-特异性Treg适用于诱导和/或保持同种异型移植在受体中的免疫耐受。
本公开提供了在不到20天内将供体-反应性Treg选择性地增殖200至1000倍的方法。教条认为树突状细胞是增殖T细胞最有效的,与此相反,发现CD40配体刺激的人类B细胞在诱导Treg的生殖中是特别有效的。图1示出了供体-反应性Treg制造的工作流程。简言之,所述的工艺以刺激纯化的供体B细胞开始,其中所述的细胞具有致命辐射的、经药品生产管理规范(GMP)认证的K562-hCD40L转染子。刺激的供体B细胞经经辐射,并通过荧光激活细胞分类用于由受体外周血分离得到的CD4+CD25+CD127lo Treg选择性地增殖供体-反应性Treg(图2A)。经过9至12天,培养物中保留的Treg实际上均是供体-反应性的。使用抗CD3和抗CD28缀合的珠子再刺激Treg,使细胞进一步再增殖5天。该方法诱导了Treg有效的增殖(图2B),并可以由1个单位的血液生产超过十亿的供体-反应性Treg。增殖的Treg为:>95%CD4+;>60%Foxp3+;>90%具有去甲基化的Foxp3启动子;>90%供体-反应性的;并且在Treg:应答者PBMC的比例为1:125时,可抑制供体刺激的T细胞的增殖。供体-反应性Treg(在不发明中也称为同种异型抗原特异性Treg或alloTreg)在用于促进抑制耐受和治疗移植物抗宿主的疾病的方法中具有用途。
就Treg支持性的免疫抑制方案而言,示例性的实施方案涉及供体-反应性Treg在诱导肝脏移植的耐受(Ltx)的方法中的用途。Treg疗法用于增加发展耐受的可能性和/或加速耐受的发展。因为供体-反应性T细胞的频率特别高,所以必须“压制”宿主同种异型反应性的全部体系及辅助的免疫抑制,从而创建Treg更有利的环境,以控制同种异型免疫性并确保长期移植物耐受(Wells et al.,Nat Med,5:1303-1307,1999;Li et al.,Curr OpinImmunol,12:522-527,2000;and Wells et al.,Philos Trans R Soc Lond B Biol Sci,356:617-623,2001)。重要的是,一些免疫抑制药品有利于Treg的发展和/或存在,而其他一些免疫抑制药品则是中性的或抗性的。因此,在一些实施方案中,在器官移植环境下,给予Treg与给予Treg支持性的免疫抑制方案组合进行。
在过去15年中,在Treg研究中的发现为供体-反应性Treg在移植中的治疗用途提供了可信的理论基础。本公开提供了一期临床试验,涉及将供体-反应性Treg给予实体器官移植受体。符合药品生产管理规范(GMP)的方法在可靠地增殖人类供体-反应性Treg方面的发展(实施例1)使得上述努力成为可能。此外,已经发展一套免疫监测检验以详细分析移植患者中的同种异型免疫应答,与之前所述的检验相比,所述的免疫监测检验具有显著改善的敏感性和可重复性。

实施例1
供体-反应性调节T细胞的生产
本实施例提供了在大约2周内由人类外周血单个核细胞(PBMC)选择性地体外增殖至十亿(109)个同种异型抗原-特异性Treg的、示例性符合GMP的方法(参见图1)。
材料和方法
受体T细胞的纯化和库存。使用ficoll密度离心,由全血或白细胞分离法产物纯化PBMC。将细胞洗涤2次,并以100-200万个细胞/ml/低温瓶再次悬浮于冰冷的CS10冷冻保藏溶液(BioLife Solutions)中。将细胞在速率受控的冰箱中冷冻,并储存在液氮的汽相中直至其他用途。
供体T细胞的纯化和库存。收集由尸体供体得到的供体脾脏或淋巴结、或者由活体供体得到的PBMC,并运送至GMP工厂,从而加工成单一细胞悬液。在CliniMACS仪器上,使用CD19阳性选择来纯化B细胞。通过冷冻保藏将纯化的CD19+B细胞库存起来,直至用于Treg增殖。
饲养细胞的制备。使用慢病毒转染人类慢性粒细胞白血病细胞K562(ATCCNo.CCL-243),以表达人类CD40L、CD64和HLA-DR0401(K562-hCD40L或K40L)。这些细胞在免疫缺陷的小鼠中并非致瘤性的。K40L饲养细胞经受10,000拉德的γ辐射,并库存起来直至其他用途。
库存的供体B细胞的激活。使用经修改的复合GMP的方法(Zand et al.,Am JTransplant,5:76-86,2005)来形成刺激的B细胞(sBc)。具体而言,使用库存的符合GMP的γ辐射的K40L细胞以1-2:1比例(B:K40L)在包括10%人类AB血清、胰岛素、转铁蛋白、人类重组IL-4和环孢霉素A的培养基中,对在CliniMACS(Miltenyi)上经顺磁抗CD19微珠纯化的~1-100x106供体B细胞刺激7天。在第7天,使用K40L饲养细胞以1-10:1比例(B:K40L)再刺激3天。平均增殖10至20倍。sBc通过ficoll除去死细胞(包括死K40L细胞)。对sBc进行一组质量保证检验,其包括qPCR基的EBV再激活检验(Viracor)和流式细胞仪,以确定HLA-DR、CD80和CD86的纯度和表达。sBc是经γ辐射的(1000拉德)并库存起来,直至其他用途。
Treg增殖。将受体PBMC解冻、计数,并使用临床级荧光缀合抗体(CD4-PerCP Ab、CD25-APC Ab和CD127-PE Ab)染色。通过FACS由染色的PBMC纯化CD4+CD127lo/-CD25+细胞(图2A)。在包括GMP级的Optimizer Medium(Invitrogen)(包括补体、GlutaMAX-1CTS和2%人类AB血清)的生长培养基中,将FACS纯化的Treg与库存的经辐射的sBc混合(sBc:Treg比例为4:1)。在第2天,在培养基的体积达到2倍时,将总浓度为300IU/ml的人类重组IL-2加入到培养物中。在第5、7和9天,向培养物中供入包括IL-2的新鲜培养基,从而将细胞浓度保持为2-3x105个细胞/ml。在培养的第11天,在培养期的剩余时间,使用与抗CD3和抗CD28单克隆抗体缀合的珠子以1:1的比例再刺激细胞。在第1天供入培养物,并在第16天收获培养物。在16天的培养期内,Treg增殖200至1600倍。
将Treg再次悬浮于Hypo Thermosal溶液中,并保持在4℃下,同时等待释放检验、质量保证审查和批准的结果。在产物释放时,将Treg运送至临床以便注入。由1个单位的受体全血纯化得到超过5x106个的Treg。在保守估计增殖200倍的情况下,在增殖期结束时,预计收获至少1x109供体-反应性Treg。
释放检验和释放标准。在Treg释放前,使用以下检验和标准:生活力>99%;流式细胞仪测定:CD4>90%,CD8<5%,CD19<5%,Foxp3>60%和TSDR>80%。在培养的第12天,细菌、真菌、支原体和内毒素的微生物检验为阴性。使用的TSDR检验为目前用于Treg的纯度和稳定性的最精确且可靠的检验。甲基化检验确认了通过流式细胞仪测定的Foxp3+细胞的百分率。此外,强有力的证据证明Foxp3可以在激活的Tconv细胞中表达。但是,Foxp3TSDR座位在激活的Tconv细胞中是甲基化的,而在真正的Treg中是去甲基化的。
释放后的检验。对各产物实施以下检验,以完全证明细胞的表现型和功能性:1)使用2个面板进行增值的流式细胞计数分析,所述的2个面板由CD4/Foxp3/CD27/CD62L和CD4/Foxp3/CD25/Helios组成;2)供体特异性抑制检验;3)供体特异性检验;4)长期的14天微生物检验;以及5)由供体sBc、PMA和毒素诱导的细胞因子(IL-2、IFN-γ和IL-17)。
最近的试验证据表明Foxp3+Treg是“塑性的”,并且可以获得效应器细胞因子的表达,例如IFN-γ和IL-17(Zhou et al.,Curr Opin Immunol,21:281-285,2009;Zhou etal.,Immunity,30:646-655,2009;and Hori et al.,Curr Opin Immunol,22:575-582,2010)。区分2种的塑性Treg的命运是有用的,一种命运导致Foxp3表达及伴随的效应器细胞因子(exTreg)的表达丧失(Komatus et al.,Proc Natl Acad Sci USA 106:1903-1908,2009;Xu et al.,J Immunol,178:6725-6729,2007;Osorio et al.,Eur J Immunol,38:3274-3281,2008;Yang et al.,Immunity,29:44-56,2008;and Zhou et al.,NatImmunol,10:1000-1007,2009),而另一种命运则导致Foxp3和效应器细胞因子(效应器Treg)的共表达(Tartar et al.,J Immunol,184:3377-3385,2010;Beriou et al.,Blood,113:4240-4249,2009;Radhakrishnan et al.,J Immunol,181:3137-3147,2008;Oldenhove et al.,Immunity,31:772-786,2009;Stroopinsky et al.,Eur J Immunol,39:2703-2715,2009;Koch et al.,Nat Immunol,10:595-602,2009;and Hvhannisyan etal.,Gastroenterology,140:957-965,2011)。尽管exTreg具有低的或不具有抑制活性,并且在试验的自体免疫环境中可以是致病性的,但是重要的是应该注意,exTreg在充满淋巴的宿主中的出现主要是在极端试验条件下发生的(Rubtsov et al.,Science,329:1667-1671,2010)。此外,在所有的条件下,即使在使用PMA和毒素进行体外超生理的刺激之后,大部分的exTreg均不表达效应器细胞因子。使用示例性的方法生产的供体-反应性Treg,我们具有高水平表达的Foxp3、TSDR和Helios。这些细胞在Treg支持的免疫抑制条件下被注入到患者中,由此,注入的供体-反应性Treg在体内转变成成熟的致病性效应器的机会较低。与exTreg相反,效应器Treg显示在许多试验条件下都是抑制性的。具体而言,已经显示通过Treg生产的IFN-γ对于对抗同种异型移植物排斥的抑制功能和保护是必要的(Sawitzkiet al.,J Exp Med,201:1925-1935,2005)。因此,预计通过Foxp3+Treg生产的效应器细胞因子是耐受性的,而不是致病性的。预计在Treg支持的免疫抑制条件下,将供体-反应性Treg(根据TSDR检验,其具有高的且稳定的Foxp3表达)注入到患者中会阻止供体-反应性Treg潜在地转变为致病性exTreg。
结果
供体-反应性Treg的增殖。上述使用CD40L刺激的供体B细胞(sBc)作为抗原呈递细胞(APC)的方法适用于选择性地增殖供体-反应性调节T细胞,其起始于由受体PBMC得到的、FACS纯化的CD4+CD127lo/-CD25+Treg。大量检验显示,实际上在刺激后的第8至10天,在培养物中保持的所有活细胞均是供体反应性的。然后,使用与抗CD3和抗CD28缀合的珠子,通过多克隆再刺激进一步增殖细胞。按照之前所述,使用FACS,基于CD4+CD127lo/-CD25+细胞表面表现型,由PBMC纯化Treg(Putman et al.,Diabetes,58:652-662,2009)。使用抗CD19的CliniMACS珠子(Miltenyi)纯化供体B细胞,并使用符合GMP的、经辐射的K562细胞(表达人类CD40L)刺激供体B细胞。通过ficoll密度梯度离心由sBc中除去死的K540L细胞,并使纯化的sBc经经辐射,然后加入到纯化的Treg中。使用该方法,使得Treg增殖达~1600倍。考虑到≤10%的Treg对全部的HLA错配的供体具有反应性,在16天的培养期中,1600倍的总体的增殖可解释为在供体-反应性Treg中增高16000倍。
建立一系列的方法来评估增殖的供体-反应性Treg的表现型和功能能力。与相似的增殖的Tconv细胞相比,增殖的Treg培养物为CD3+CD4+CD8-CD19-、Foxp3+、Helios+、CD27+和CD62Lhi(图2B)。几乎所有的供体sBc增殖的Treg都对供体sBc的再刺激产生应答,而对同系的sBc不应答,表明它们是刺激物-反应性的(图4A)。供体sBc增殖的Treg与多克隆增殖的Treg相比,表现为增强的供体-特异性抑制活性(图4B)。重要的问题是供体-反应性Treg是否是稳定的Treg或T效应器细胞,其可以瞬间上调节Foxp3。Foxp3启动子的去甲基化显示为表达Foxp3的稳定的Treg的有效标志物(Wang et al.,Eur J Immunol,37:129-138,2007;and McClymont et al.,J Immunol,186:3918-3926,2010)。如通过定量Treg-特异性去甲基化区域(TSDR)检验测定的那样,大于94%的供体-反应性Treg,及小于1%的dsTconv具有去甲基化的Foxp3启动子(图3B)(Wieczorek et al.,Cancer Res,69:599-608,2009)。总之,这些结果证明示例性的方法可靠地增殖GMP级的供体-反应性Treg。由典型的增殖得到的释放检验结果示于图5中。
实施例2
使用供体-反应性Treg和Treg支持的免疫抑制的肝脏移植
本实施例描述了剂量递增临床试验,来评估同源的供体-反应性Treg疗法在肝脏移植(Ltx)受体中的安全性。但是,本公开的方法和组合物不限于该内容。事实上,预计本公开的方法和组合物在其他实体器官的同种异型移植物、以及在治疗或预防移植物抗宿主的疾病中具有用途。预计供体-反应性Treg和Treg支撑的免疫抑制适用于诱导或保持同种异型移植物的耐受,其中所述的移植物选自但不限于心脏、肺、心脏/肺、肾、胰腺、肾/胰腺、肠和肝脏的同种异型移植物。
将剂量递增的Treg(使用激活的供体B细胞体外增殖)与改善的免疫抑制方案(设计以有利于Treg的发展、持续和功能)一起给予Ltx受体。该方案由兔抗胸腺细胞球蛋白(rATG)的诱导;剂量减少的皮质甾类(Pred)、霉酚酸酯(MMF)和他克莫司(tac);然后延迟地引入欧列姆(SRL)组成。该主题在移植后持续1年,在其间,收集临床数据、以及外周血(PBMC和血清)和肝脏活检样品,并进行分析。
主要目的。对成人评估以下结果,新发Ltx受体:一年急性排斥反应发生率(“Banffschema for grading liver allograft rejection:an international consensusdocument,”Hepatology,25:658-663,1997);一年慢性排斥反应发生率(“Liver biopsyinterpretation for causes of late liver allograft dysfunction,”,Hepatology,44:489-501,2006);在注入Treg后3个月,≥3级感染的发生率;≥3级伤口并发症的发生率;≥3级贫血、嗜中性白血球低下和/或血小板减少症的发生率。
第二目的。此外,评估以下结果:超过基线的Treg百分率的增加;供体-反应性Treg频率的增加;供体-反应性Treg活性的增加;以及PBMC和/或肝脏组织中,耐受基因表达图谱的检测。
患者群体和纳入/排除标准。临床试验涵盖3个时期,每个时期都具有特定的纳入/排除方案以使参与者的安全性最大化。
前期和Ltx期。患者选自Ltx等待列表,其年龄在20-70岁,患有晚期肝病并且计算的MELD得分不超过25分(Kamath et al.,Hepatology,33:464-470,2001)。试验特异性地排除了急性排斥和疾病复发风险增大的Ltx受体,并限定了肝病、门静脉高压和/或脾脏功能亢进的严重性。在一些实施方案中,仅选择其PBMC中存在的Treg高于10/μl的患者。
合适的患者经白细胞分离法分理出PBMC,将其冷冻保藏以用于随后的Treg纯化和增殖。在tx时并证明患者持续合格之后,收集供体脾脏和/或淋巴结、以及肝脏活检组织,并库存起来。
Treg支持的免疫抑制时期。Ltx受体必须脱离ICU并在tx后的3天内开始rATG诱导。他们接受总剂量为3至4.5mg/kg的rATG,以使淋巴细胞被去除,定义为CD3计数<50/mm3。选择该剂量范围,从而形成充分的压制(Wong et al.,Transpl Int,19:629-635,2006),同时减小免疫抑制。选择给予rATG的定时和环境,以避免在医学不稳定的受体中过多的免疫抑制和/或细胞因子释放综合症/血液学毒性的可能。患者被评估为合格,则转变为欧列姆(SRL)基的免疫抑制,并且必须具有正常的同种异型移植物功能,以及在Ltx后的4至6周内具有足够的肾功能、血液学参数、伤口愈合和肝动脉开放。
特异地设计研究受试对象的免疫抑制方案,以促进Treg的发展,同时优化参与者的安全性。根据护理标准(SOC),研究参与者的免疫抑制起始于一半剂量的皮质甾类和一半剂量的霉酚酸酯(MMF)。新加入他克莫司(Tac),目的在于水平降低至6至8μg/L(与SOC(10-15μ/L)相比)。在tx后的3天内,患者接受一个流程或rATG(总剂量为3.0至4.5mg/kg)以去除淋巴细胞(CD3计数<50/mm3或者当给与最大剂量时)。取消皮质甾类的参与者在tx后的4至6周转变为SRL基的免疫抑制,并且开始时SRL的目标水平为6至8μg/L,并将标签tag降低至3至5μg/L的低谷水平。终止MMF。在转变为SRL基IS后的4周(在tx后的8至10周),参与者经历最终的评估,包括同种异型移植物活检,以确保接受Treg注入的合格性。Tx后的6个月,SRL被进一步降低至目标水平4至6μg/L。

Treg注入时期:在Ltx之后大约10至12周,对参与者接受供体-反应性Treg的适应性进行评估。关于rATG后T细胞恢复的动力学数据显示在tx后的4至12周T细胞数量稳定。因此,在tx后的10至11周注入Treg为免疫系统受到压制的环境。就稳定的SRL基免疫抑制而言,参与者必须具有正常的同种异型移植物功能。
在免疫抑制转变的同时,供体B细胞增殖10天,然后在另外的16天内用于增殖Treg(实施例1)。通过所有释放标准的增殖供体-反应性Treg可用于在tx后的10至11周注入。
在注入Treg后,在第1、3、7和28天收集血液用于机械学研究。每周进行临床试验室学评估,持续另外的4周。如果肝脏检验保持稳定,则临床试验室学评估恢复为SOC用于其余的研究。在Ltx后的1年时吸取另外的血液用于机械学研究,并在Ltx后的1年时进行附加协议的肝脏活检用于详细的组织学和免疫组织化学分析。
剂量递增计划:合格患者不接受Treg注入,或者单一注入3个剂量水平的供体-反应性Treg:50、200和800百万。由一组至下一组的级数是以剂量限制性毒性为基础的。

免疫学分析
本实施例描述了对外周血和肝脏组织的分析,以评估Treg支持的免疫抑制的作用及Treg对同种异型免疫应答的治疗。预计供体-反应性Treg疗法以及Treg支持的免疫抑制对供体-反应性Treg的频率和抗供体T细胞的应答具有可测量的影响。此外,预计实施例2中所述的示例性治疗方案将得到比常规(SOC)免疫抑制方案发展得更早的免疫耐受标志。分析包括以下的一种或几种:1)T细胞的功能和表现型分析;2)在PBMC和协议活检样品中的耐受基因表达标志;以及3)寻找原因及协议活检样品的组织性分析。
T细胞表现型和功能分析。多参数流式细胞仪(MFC)用于绘制白细胞亚群图谱,测定供体-反应性T细胞的频率,评估由Treg引起的供体-特异性抑制,以及绘制供体原因诱导的基因和细胞因子表达的图谱。总之,这些检验允许对的4种已知的免疫耐受(清除,偏离,无能/耗竭,和调节)机制的贡献进行评估。
供体-反应性T细胞的频率。本检验用于测定供体-反应性CD4+Tconv细胞、CD8+T细胞和Treg的频率。对库存PBMC样品进行比较,其中所述的PBMC是在移植前/移植、Treg前/SRL后转变、在Treg注入后的第1、3、7和28天,以及在移植后的1年时得到的。预计在注入后的短时间内供体-反应性Treg增加,特别是在接受200至800x106dsRegs的一群人中。
体外抑制检验。本检验用于评价由Treg引起的抑制,其中所述的Treg是由移植前,注入Treg前/SRL后转变,在Treg注入后的第1和28天,以及在肝脏移植时间点后的1年时分离得到的。将移植前白血球电泳法处理(leukophoresed)的PBMC与由各时间点分离得到的Treg混合作为应答者。使用经辐射的供体PBMC刺激培养物以评估供体-特异性移植,或者使用抗CD3和抗CD28刺激培养物以评估非特异性移植。
多参数流式细胞仪(MFC)。MFC用于测定白细胞亚群在外周血中的百分率,其中使用在我们试验室研发的抗体面板。由面板收集得到的样品和使用的标志物概括与表3-1中。

T细胞激活/分化检验。使用供体sBc对CD4+Tconv细胞和CD8+T细胞(由移植前、Treg前/SRL后转变、在Treg注入后的第1、3、7和28天,以及在移植后的1年时得到的)刺激3.5天。使用qPCR阵列分析样品(由移植前、Treg前/SRL后转变、在Treg注入后的第1、3、7和28天的时间点收集得到的)中细胞因子的基因的表达,并使用42-plex Luminex检验分析样品中分泌至上清液中的细胞因子。在移植前和移植后1年收集得到的样品用于分析基因的表达图谱(使用基因阵列),并使用42-plex Luminex检验分析上清液中的细胞因子。使用qPCR检验,预计在肝脏移植患者中可观察到供体sBc刺激的基因表达的改变。本检验允许更换待测定的供体抗原刺激的基因表达图谱。
基因表达分析。使用具有基因的、之前所识别的数目有限的亚群的微阵列来分析外周血样品,其中所述的基因代表了目前可用于检测肝脏移植后的操纵耐受的最有前途的生物标志物(Martinez-Llordella et al.,J Clin Invest,118:2845-2867,2008)。
组织学分析和多元免疫组织化学(mIHC)。对在移植前和肝脏移植后1年时获得的协议活检样品进行广泛的组织学和mIHC分析。组织学分析评价了40种组织病理学特征,以测定组织完整性和炎症程度,如表3-2所示。

实施例4
同种异型抗原-反应性人类调节T细胞在移植中的临床级制造和治疗优点
本实施例证明了使用符合GMP的试剂可以在短期培养物中产生数十亿的人类同种异型抗原-反应性调节T细胞(Treg)的制造工艺。该工艺使用CD40L激活的同种异型B细胞来选择性地增殖同种异型抗原-反应性Treg,然后通过多克隆再刺激以增加产率。增殖达200至1600倍的Treg是高度的同种异型抗原反应性的,并且表达稳定的Treg表现型。在体外,同种异型抗原增殖的Treg比多克隆增殖的Treg更有效力,达5至25倍,并且在皮肤移植的人源化小鼠模型中,在体外控制同种异型移植物损伤中更有效。
材料和方法
细胞来源。募集正常供体,并通过捐赠全血。当需要大量细胞时,由UCSF BloodCenter获得由正常供体得到的去识别的单采产物。利用Ficoll-Paque PLUS密度梯度(GEHealthcare Bio-Sciences AB,Pittsburgh,PA)分离PBMC,并在新鲜时使用,或使用装置(BioCision,Mill Valley,CA),在CryoStor CS10冷冻培养基(BioLifeSolutions,Bothell,WA)中冷冻保藏后使用。在知情同意下,由尸体器官供体获得脾脏。所有程序均由Committee on Human Research(在University of California SanFrancisco)和Guy’s hospital(在King’s College London)批准。
表达CD40L的饲养细胞的产生。按照之前所述34,生产编码人类CD40L(NM_000074)、CD64(BC032634)、DRA(BC071659)和DRB 040133的慢病毒载体。这些载体用于转导K562细胞,从而产生KT64-CD40L。按照之前所述35,HLADR0401细胞系和FACS用于产生单细胞克隆。使用由BD Biosciences,San Jose,CA得到的针对CD40L、HLA-DR和CD64的抗体,通过流式细胞仪来证明增殖细胞的稳定表达。
CD40L-sBc的产生。使用未触碰人类B细胞富集试剂盒由PBMC或脾脏富集B细胞(Invitrogen,Carlsbad,CA)。按照之前所述36,将富集的B细胞与表达人类CD40L的经辐射的(40Gy)3T3或K562细胞一起培养。对于一些试验而言,将解离的脾细胞与表达CD40L的细胞一起培养,而未经之前的B细胞的富集。CD40L-sBc经经辐射(30Gy)并用于刺激Treg或在CryoStor CS10冷冻培养基中冷冻保藏,直至使用。对于符合GMP的增殖而言,在CliniMACS(Miltenyi Biotech,Germany)上,使用CD19阳性选择来纯化外周血B细胞,并在包含转铁蛋白的X-VIVO15培养基(Lonza,Walkersville,MD)中,使用经辐射的(100Gy)K-CD40L细胞进行刺激,其中所述的培养基补充有10%人类AB血清(Valley Biomedical,Winchester,PA)、GMP grade IL-4(Miltenyi)和环孢霉素A(Teva Pharmaceuticals,North Wales,PA)。
MLR。用经辐射的同种异型CD40L-sBc(每个PBMC使用2个sBc)或用经辐射的同种异型PBMC(每位应答者使用5种刺激物)来刺激使用1.25μMCFSE(Invitrogen)标记的应答者PBMC。在84至96小时后收获培养物,使用抗CD3PerCP(BD)、抗CD4PE-Cy7(BD)、抗CD8APC-Cy7(BioLegend,San Diego,CA)、efluor 506可固定的活力细胞染料(eBioscience,SanDiego,CA)染色。然后,使用FOXP3固定/渗透缓冲剂套装(eBioscience)固定细胞,再使用抗FOXP3-Alexa Fluor 647(eBioscience)和抗HELIOS PE(BioLegend)染色。在Fortessa(BD)上实施流式细胞仪,并使用FACSdiva(BD)或FlowJo软件(Treestar,Ashland,OR)进行分析。
Treg增殖。基于CD4+CD127lo/-CD25+的细胞表面表现型,使用BD FACSAria II(BD)分离Treg,并按照之前所述28,进行Treg的多克隆增殖。符合临床的分类使用了由NoelWarner(BD)生产并友好提供的cGMP mAb。对于同种异型抗原-反应性Treg增殖而言,将培养物保持在OpTmizer T Cell Expansion Medium(Invitrogen)(其中补充有1%GlutaMAX(Invitrogen)、盘尼西林/链霉素和2%人类AB血清)中,或保持在补充有10%人类AB血清的X-VIVO15培养基中。在sBc与Treg的比例为4:1的条件下,将FACS纯化的Treg与CD40L-sBc混合。将培养物在包含300IU/ml人类IL-2的培养基中保持直至第9或11天,此时使用经辐射的sBc以4sBc/Treg的比例对细胞进行再刺激,或者使用抗CD3/抗CD28涂敷的珠子以珠子与细胞比例为1:1对细胞进行再刺激。在之后的3天供入培养物,并在再刺激的第5天收货培养物。
流式细胞仪。使用3中流式细胞计数面板对增殖Treg的表现型进行评估。第一面板由抗CD8FITC、抗CD4PerCP、抗CD3PE和抗CD19APC组成。第二面板由抗CD4PerCP、抗CD62LPE、抗CD27APC和抗FOXP3Alexa Fluor 488(BioLegend,Clone 206D)组成。第三面板由抗CD4PerCP、抗CD25APC、抗HELIOS PE(BioLegend)和抗FOXP3Alexa Fluor 488组成。小鼠IgG1Alex Fluor 488和小鼠IgG1PE(BioLegend)分别用于控制FOXP3和HELIOS染色。在FACSCalibur上分析染色的细胞,并使用FlowJo分析数据。在使用抗HLA-DRPE、抗CD80FITC、抗CD86PerCP-Cy5.5和抗CD19APC染色后,在AccuriC6(BD)流式计数仪上分析CD40L-sBc。使用Cflow PLUS软件(BD)分析数据。除非另作说明,所有抗体得自BD Biosciences。
Treg特异性检验。将增殖的Treg使用1.25μM CFSE标记,并使用同种异型或同源CD40L-sBc、抗CD3和抗CD28涂敷的珠子刺激,或者在包括30IU/mlIL-2的培养基中保持未刺激的状态。在72小时后,收集细胞,并使用抗CD4APC(BD)和碘化丙啶染色,再在AccuriC6流式细胞仪上分析。
体外抑制检验。将滴定数量的增殖Treg在V底96孔板中与由Treg供体得到的3x104PBMC混合,并做3个重复。使用由sBc供体得到的经辐射的PBMC将所述的细胞刺激7天,并在培养的最后16至20小时过程中引入3[H]胸腺嘧啶脱氧核苷酸,用于测量生殖情况。不包含增殖的Treg的培养物用作对照。抑制百分率按以下计算:[1-(具有Treg的平均cpmPBMC/不具有Treg的平均cpmPBMC)]×100。
TSDR甲基化检验。根据Epiontis GmbH建立的方法37,使用由Epiontis GmbH(Berlin,Germany)得到的经许可的试剂由0.5x106增殖的Treg分离基因组DNA。在3个重复中进行检验,并按以下计算甲基化的TSDR的百分率:[未甲基化的DNA的平均拷贝数/(未甲基化的DNA的平均拷贝数+甲基化的DNA的平均拷贝数)]x100。对于使用雌性供体增殖的培养物而言,将上述计算的百分率乘以2,以纠正X染色体的失活。
在皮肤移植的人源化小鼠模型中,体内评估Treg功能。繁殖BALB/c.Rag2-/-γc-/-小鼠(Charles River)并将其保持在the Biological Services Unit ofKing’s CollegeLondon无特异性病原体的条件中。在知情同意及伦理批准的条件下,由经历常规腹壁成形术和乳房缩小形成术的患者得到去识别的人类皮肤。将该皮肤移植到8至12周龄的BALB/c.Rag2-/-γc-/-小鼠上,并允许在植入6周后,输注10x106HLA错配的、CD25去除的人类PBMC。一些小鼠与2x106体外增殖的多克隆或同种异型抗原-反应性Treg一起共同输注。每周2次对移植物进行视觉和触觉检查。在输注PBMC后的6周内,对移植物进行组织学分析。就这些试验的整个过程而言,每4至5天腹膜内输注100μg经纯化的抗小鼠Gr1mAB(Bio X Cell,West Lebanon,NH),从而去除小鼠的粒细胞。所有程序均是根据制度化的指导方针和theHome Office Animals Scientific Procedures Act(1986)实施的。使用5%多聚甲醛固定人类皮肤移植物的冷冻切片(6至8μm),并使用针对人类抗原ki67(克隆4A1,Abcam,Cambridge,MA)、CD45(克隆HI30,eBioscience)、CD3(A0452,Dako,Denmark)、FOXP3(克隆259D/C7,eBioscience)、外皮蛋白(克隆SY5,Sigma)和CD31(ab28364,Abcam)的抗体进行染色,然后与合适的荧光染料缀合的二级抗体温育,并使用Prolong Gold方褪色试剂4-6-二脒基-2-苯基吲哚(DAPI)(Invitrogen)固定。使用荧光显微镜,通过计数4至6个不重叠的视野对样品进行定量分析。读取载玻片的个体对于处理条件是盲态的。
统计学。借助于Prism GraphPad软件进行统计学分析。
结果
CD40L刺激的B细胞为同种异型抗原-反应性Treg的有效刺激物。之前使用同种异型PBMC、树突状细胞(DC)、新鲜的B细胞和CD40L刺激的B细胞(也称为CD40L-sBc)来选择性地刺激人类同种异型抗原-反应性细胞的增殖13-16。但是,关于这些细胞的亚群在刺激Treg中的能力知道的较少。经辐射的PBMC、新分离的B细胞和CD40L-sBc在单向混合的淋巴细胞反应(MLR)中的相对效力的比较证明CD40L-sBc是最有效的刺激物。使用CFSE染料稀释检验来监测CD4+和CD8+T细胞的生殖,发现在使用CD40L-sBc刺激3.5天后可以检测到有效的生殖应答,而在使用经辐射的PBMC刺激后,仅观察到微弱的应答(图6A)。通过进一步在CD4+FOXP3+HELIOS+Treg上开门(gate),发现CD40L-sBc刺激了Treg在这些MLR培养物中有力的生殖(图6B)。根据之前的报告23,新分离的外周血B细胞不会刺激T细胞的生殖。为了测定生殖是否是响应于在CD40L-sBc上表达的同种异型抗原,对同源CD40L-sBc和同种异型CD40L-sBc的刺激能力进行比较,其中所述的同种异型CD40L-sBc与应答者的T细胞具有不同程度的HLA错配。发现,对于相同应答的PBMC而言,应答CD4+常规T细胞(Tconv)和Treg的频率与HLA-DR错配的数量呈正相关,而应答CD8+T细胞的频率与HLA-AB错配的数量呈正相关(图6C和6D)。引人注意的是,应答Treg的频率一惯高于应答CD4+Tconv和CD8+T细胞的频率。这些结果证明CD40L-sBc是同种异型抗原-反应性Treg的有效刺激物,并且促进了CD40L-sBc在选择性地增殖具有临床用途的同种异型抗原-反应性Treg中的用途的探索。
符合药品生产管理规范(GMP)的、表达CD40L的饲养细胞的产生。产生符合GMP的、表达人类CD40L的细胞系KT64-CD40L.HLADR0401(简写为K-CD40L),从而能够制造具有临床用途的Treg。慢病毒转导用于在髓细胞白血病细胞系K562中表达CD40L,该细胞系已经用作用于癌症疫苗的媒介物,并且在制造用于临床用途的治疗T细胞中的人工抗原呈递细胞24-27。CD40L的表达对于CD40L-sBc的产生是必需的。CD64和HLADR0401表达不会干扰CD40L的活性,同时允许所述的细胞系用于其它用途,包括抗原特异性的T细胞和多克隆T细胞的增殖。在第0和7天使用K-CD40L细胞进行两轮刺激并且持续供入IL-4使得由外周血或脾脏纯化得到的B细胞增殖10至50倍(图7A)。当与新分离的B细胞比较时,CD40L-sBc表达明显较高量的HLADR、CD80和CD86(图7B和7C),这符合它们在刺激同种异型T细胞中的增强的效力。尽管在CD40L-sBc上HLA-DR、CD80和CD86的表达一致增高,但是其水平在供体之间不同。而且,由多个供体产生的CD40L-sBc能够诱导MLR和Treg增殖,表明CD40L-sBc的效力并非严格地与共同刺激物和II类MHC分子的绝对水平相关,只要达到阈值即可。
CD40L-sBc有效地诱导同种异型抗原-反应性Treg的增殖。对使用CD40L-sBc对同种异型抗原-反应性Treg的最佳刺激条件进行检验。已知多克隆增殖Treg的方案,其中使用抗CD3和抗CD28涂敷的珠子对荧光激活的细胞分类(FACS)纯化的CD4+CD127lo/-CD25+Treg进行2轮刺激(第0和9天)28。就增殖同种异型抗原-反应性Treg而言,采用相似的方法,但是在第0和9天,采用经辐射的CD40L-sBc替代所述的珠子。使用上述方案,在14天内增殖50至300倍(图8A)。在培养结束时(第14天),增殖的Treg对用于刺激Treg增殖的相同的CD40L-sBc是高度应答的,但是对自身的CD40L-sBc不产生应答(图8B)。该结果证明了Treg的标志性的富集,其中所述的Treg对由CD40L-sBc(用于刺激Treg增殖)表达的同种异型抗原产生反应。事实上,到初次刺激后的第9天,Treg已经对CD40L-sBc产生高度的反应,与在第14天观察到的相似,表明就同种异型的反应性而言,在再刺激过程中不必进一步富集。考虑到使用抗CD3和抗CD28刺激可有效地增殖PolyTreg、以及使用基于珠子的方法易于标准化和实施,在再刺激过程汇总使用抗CD3和抗CD28涂敷的珠子来替代CD40L-sBc可以得到可比的增殖。然而,结果显示对总体Treg增殖而言,在sBc和珠子再次之间不具有显著的差异(图8C)。因此,采用初次sBc刺激、然后使用抗CD3和抗CD28涂敷的珠子进行多克隆再刺激的方案。
在FACS纯化后,1个单位的血液平均产生5百万个Treg。因此,使用图8所示的方案,根据50至300倍的增殖,可以产生2.5亿至15亿的同种异型抗原-反应性Treg。估计在人类中达到移植功效所需的Treg的数量为300百万至几十亿个细胞20。为了确保恒定地生产超过300百万的同种异型抗原-反应性Treg,探索了Treg增殖的改善的条件。观察到,与第0天使用珠子激活的PolyTreg不同,CD40L-sBc刺激的Treg持续聚集,并在初始刺激后的第9天分开(blast)(图8D)。这种观察现象表明CD40L-sBc比mAb涂敷的珠子更有效力,使得Treg的活性延长。对激活的T细胞进行再刺激可以使激活诱导的细胞死亡,由此限制了最佳的增殖。因此,延迟再刺激直至第11天,此时细胞由聚集物解离,并变得更小(图8E)。延迟再刺激显著改善了总体的增殖(图8F)。除了对再刺激定时以外,发现用于再刺激的珠子的来源会极大地影响Treg增殖的速率(图8G)。总之,通过优化再刺激定时和再刺激试剂,同种异型抗原-反应性Treg通常会增殖200至1600倍,从而在16天的时间内可靠地产生超过1x109的同种异型抗原-反应性Treg。
CD40L-sBc增殖的Treg的体外特征。发现使用CD40L-sBc方案增殖的Treg为CD3+CD4+,并最少地污染有CD8+T细胞和CD19+B细胞(图9A)。多数的CD4+T细胞为FOXP3+HELIOS+、以及共表达的CD27和CD62L(图9B),其与在相似增殖的Tconv细胞上的表达模式不同(图9B)。最后,增殖的Treg具有>80%去甲基化的Treg特异性去甲基化区域。总之,使用同种异型CD40L-sBc增殖的Treg的表现型表明它们是稳定提供的Treg。
为了测定扩增的Treg对用于初次刺激的同种异型CD40L-sBc的反应性,使用相同的CD40L-sBc再刺激在第16天收获的Treg。平均87.5%(72.5至95.2%的范围)的同种异型抗原增殖的Treg响应于相同sBc的再刺激而生殖,这与使用抗CD3和抗CD28的珠子诱导的生殖相似(平均88.8%,73.6至96%的范围),表明极大量的Treg对由CD40L-sBc表达的同种异型抗原具有反应性(图9C和9D)。
与这些表现型数据和增强的同种异型抗原的识别一致,当增殖的Treg在体外被PBMC激活时,是高度抑制的,其中所述的PBMC由与CD40L-sBc相同的供体得到(图9E)。同种异型抗原增殖的Treg和多克隆增殖的Treg的并列比较显示,供体同种异型抗原-反应性Treg在抑制MLR的效力方面比PolyTreg高5至25倍(图9E)。通过使用CD40L-sBc、或者抗CD3和抗CD28珠子再刺激而增殖的Treg在抑制MLR方面具有一致的活性(图9E),证明多克隆再刺激在体内不会改变它们的同种异型活性或抑制活性。此外,还比较了通过PBMC(由相同的CD40L-sBc供体或第三方供体)刺激的抑制活性。通过相关的PBMC刺激的Treg(使用同种异型sBc增殖)所产生的抑制作用是通过第三方细胞刺激的Treg所产生的抑制作用的9至27倍之多(图9F)。总之,结果显示CD40L-sBc增殖的Treg对通过B细胞表达的同种异型抗原(用于Treg的增殖)具有高度富集的反应性和抑制活性。
同种异型抗原-反应性Treg在体内保护皮肤同种异型移植物中是优异的。使用人类皮肤同种异型移植物的、同种异型免疫介导的损伤的模型(图11A)13,比较同种异型抗原-反应性Treg和PolyTreg的保护功能。BALB/c.Rag2-/-γc-/-小鼠移植由HLA-DR0401+供体得到的人类皮肤,并使移植物愈合6周,然后过继转移单独的去除了CD25+的同种异型PBMC,或者与同系Treg的不同制备物(效应器细胞:Treg细胞的比例为5:1)结合的同种异型PBMC。PBMC供体为HLA-DR0401-,并使用HLA-DR0401+CD40L-sBc增殖由这些供体得到的同种异型抗原-反应性Treg。监测移植物直至发生排斥,或者直至在PBMC重新构建之后的最多6周,此时收集移植物以用于组织学分析。脾脏中人类白细胞植入物的水平与3组小鼠中的相似,其中所述的小鼠接受单独的人类PBMC,或者与Treg结合的PBMC(图11B)。没有动物发展出异种移植物抗宿主反应疾病的症状,其中所述的症状可通过保持稳定的体重来证实(图11C)。
与未接受PBMC的对照动物中的皮肤移植物(表4-1)相比,单独PBMC的组中的皮肤移植物显示人类CD45+单个核细胞渗透物在真皮-表皮结合处集中并伴随角化细胞生殖增多,上层棘细胞层和颗粒层中外皮蛋白损失,以及血管化降低,这可通过真皮中成簇的CD31+细胞的减少来表示(表4-1)。这些变化表明了由同种异型的人类白细胞介导的活性皮肤炎症及真皮-表皮完整性的丧失。如之前的研究所报告13,移植物中所有这些炎症参数均是通过共同输注Poly Treg来降低的,这与FOXP3+细胞的升高有关(表4-1)。引人注目的是,接受同种异型抗原-反应性Treg的小鼠中的皮肤移植几乎完全受到保护而免于移植物损伤的组织性特征,并且除了FOXP3+细胞在真皮-表皮结合处的渗透以外,这些皮肤移植物不能与对照移植物中的那些相区分(表4-1)。这些组织学发现的定量分析证明在Ki67+角化细胞中显著降低,在CD31+血管内皮细胞中增加,这与输注同种异型抗原-反应性Treg的小鼠的移植物中,FOXP3+与CD3+的细胞比例显著高于使用Poly Treg处理的小鼠有关(表4-1)。这些结果证明同种异型抗原-反应性Treg在体内控制同种异型移植物中比等量的Poly Treg更有效。在效应器:Treg的比例为5:1的条件下,同种异型抗原-反应性Treg完全保护皮肤移植物免于效应器细胞诱导的病理学改变。

Claims (16)
1.一种用于生产人类供体-反应性调节T细胞(Treg)的方法,包括:
a)在有效生产刺激的B细胞(sBc)的条件下,使人类供体与经辐射的CD40L+人类白血病饲养细胞共培养;
b)在有效地选择性地增殖人类供体-反应性调节T细胞(Treg)的条件下,使人类受体的CD4+、CD25+、CD127-/lo T细胞与所述的sBc共培养,和
c)在有效生产CD4+、Helios+且Foxp3+的再刺激的供体-反应性Treg的条件下,通过所述供体-反应性Treg的CD3与CD28的交联来再刺激所述供体-反应性Treg,
其中所述供体相对于所述人类受体是HLA错配的。
2.权利要求1所述的方法,其中所述的HLA错配包括HLA-A、HLA-B、HLA-C和HLA-DR中的至少1个、2个、3个或4个错配。
3.权利要求2所述的方法,其中所述的步骤c)在所述的步骤b)开始后的9-12天开始。
