中枢神经系统损伤,包括脊髓损伤,是最具破坏性和致残性的可能损伤之一。取决于损伤严重性,可导致不同程度的瘫痪。截瘫和四肢瘫痪经常由脊髓严重损伤引起。
全世界每年脊髓损伤(SCI)的发病率估计为22人/百万人,并且SCI存活者的总数估计为2百5十万人。SCI最常发生于二十几岁的青年人中,这些人可预期具有准正常的预期寿命,但是难以维持可接受的生活质量。这种情况产生重大的个人、社会和经济成本。虽然预期寿命是非常好的,但是SCI患者患有一些重大障碍(取决于病变的程度和严重性),这些障碍严重削弱他们的生活质量(如瘫痪、感觉缺失、顽固性疼痛、褥疮和泌尿等感染)。尽管在过去的二十年中,科研和临床界已经投入了密集的研究,但是到目前为止还没有可以扭转SCI后所丧失功能的良药或疗法。

在本发明人的实验室中证明与皮肤片段一起孵育的来源于血液的单核细胞获得了类似于文献中所述的抗炎症M2单核细胞的非炎症性表型(Bomstein等人,2003)。在大鼠中将单核细胞注射至损伤脊髓中诱导更好的SCI恢复(US 5,800,812;US 6,117,424;和US 6,267,955)。这种方法在对于患有急性切断脊髓损伤的患者的临床研究中得到测试,呈现令人鼓舞的成果(WO 03/044037;Knoller等人,2005)。因此,治疗需要包括椎板切除术的外科手术来暴露损伤脊髓并且将细胞注射于病变部位的边界,这种病变部位是很难定位的。发明人认为,发现克服定位病变部位和使患者暴露于侵入性手术的难题的方法会有助于SCI的治疗。
在发明人的实验室进行的近期研究表明,在脊髓损伤后,在损伤后三到四天,血液产生的单核细胞自发地浸润受损的CNS,优先聚集在病变部位的边缘,在恢复的过程中起到了举足轻重的作用。这些细胞调节受伤组织的免疫活性以变得具有较少发炎,并产生了支持愈合的分子,因而有利于细胞再生和组织修复(Shechter等人,2009)。尽管它们的积极作用,但是在严重损伤中,不足以诱导完全恢复,甚至部分功能恢复。
外周血浸润/募集至病变部位中受到从病变部位引出的信号控制,从而影响大脑-CSF屏障。CNS损伤后的有限自发恢复可部分归因于有效子集的单核细胞不充分地、不合时宜地、自发地募集至病变部位。本着这一精神,我们已经证明使外周血单核细胞池富集来源于骨髓的CD115细胞可增强小鼠在SCI之后的功能恢复(Shechter等人,2009)。

在人类中,有人提出,单核细胞上的CD16的表达可以区分三个子集,即CD14++CD16-(经典)CD14++CD16++和CD14dimCD16++(非经典)单核细胞,但它们在生理和病理条件中的作用尚不完全清楚。
发明概述

发明详述
本发明是基于以下发现,由不存在、或者几乎不存在表达CD3、CD19、CD56和任选CD16的细胞来定义的单核细胞亚群能够从脑脊液(CSF)归巢到损伤的脊髓部位,促进组织修复和改善功能恢复。现有技术的方法教导将单核吞噬细胞准确施用至病灶边界的必要性,以便获得满意疗效(例如美国专利号5,800,812)。
根据本发明进一步发现,细胞在治愈受伤脊髓中的有益效果通过以下方法来获得:施用与皮肤片段一起共孵育的细胞或将未活化的细胞施用至受伤脊髓的CSF。
在其表面上表达某种可识别标记,如分化抗原簇(CD)分子X的细胞在本文中称为CD X+。例如,在其表面上表达CD3分子的细胞在本文中称为CD3+。表达于细胞表面上的CD分子的相对量是通过添加“+”来指示的,例如,CD3++表示高含量的CD分子,或者术语“dim”表示CD分子的相对较低水平。包括由表达某种CD来定义的细胞类型的细胞群体、以这些细胞相对富集的群体,或缺乏此类细胞的群体分别被称为CD+、CD++或CD-。
因此,在某些实施方案中,外周血单核细胞(PBMC)亚群基本上不含CD3+细胞、CD19+细胞和CD56+细胞。
在其它实施方案中,另外,CD16+细胞已经从PBMC亚群中去除,导致PBMC亚群进一步基本上不含CD16+细胞,即亚群基本上不含CD3+细胞、CD19+细胞、CD56+细胞和CD16+的细胞。

因此,在某些实施方案中,注入CSF的本发明的PBMC亚群细胞是皮肤活化细胞。
在某些实施方案中,注入CSF的本发明的PBMC亚群细胞是未活化细胞。
根据本发明的PBMC亚群,以及包含它们的组合物可用于治疗CNS损伤,例如脊髓损伤。具体地说,所述治疗包括促进脊髓组织修复,包括例如预防或抑制神经元变性、促进神经元存活、轴突再生和/或萌芽、受伤脊髓中的神经发生,和/或促进功能恢复,如通过大鼠中的巴索-比蒂-布雷斯纳汉(BBB)评分或小鼠中的小鼠巴索量表(BMS)测量,如下文所定义。

实施例
材料与方法
(i)动物。在实验中使用的动物,如果没有不同地标明,都是由Harlan Laboratories,位于以色列的ISO认证SPF实验动物饲养者和以色列魏兹曼科学研究所的动物繁殖中心(Animal Breeding Center ofthe Weizmann Institute of Science(Rehovot,以色列))来提供。所有动物均根据由魏兹曼研究所的动物护理和使用委员会制定的规定办理。
(ii)脊髓损伤。将大鼠(野生型Sprague-Dawley大鼠)麻醉(氯胺酮70毫克/千克,Bedford Laboratories,OH,US,甲苯噻嗪10毫克/千克,VMP,Bioulab,法国),在T9处将椎板切除并且使用NYU冲击器来挫伤(Gruner(1992)A monitored contusion model of spinal cord injury inthe rat.J Neurotrauma.Summer;9(2):123-6;论述126-8。综述),所述冲击器从50毫米高度将10克的金属棒落在暴露的脊髓上(被认为会导致严重的伤害)。将小鼠麻醉,它们的脊髓通过在T12处进行椎板切除术来暴露,并用Infinite Horizon脊髓冲击器(Precision Systems andInstrumentation,Lexington,KY)在椎板切除的脊髓上施加200kdyn的力,所述冲击器是被证明可施以脊髓的良好校准挫伤性损伤的装置。
(iii)皮肤制备,以及与单核细胞一起共同孵育。从同一供体小鼠的背部制备小片皮肤(2x6毫米),所述小鼠已被放血以制备单核细胞。上背部的皮毛被剃光,并在切割前将皮肤用乙醇消毒。将两片皮肤组织与同源单核细胞部分(见上文)的5x106个细胞一起置于5mlDCCM-1(Biological Industries,Beit HaEmek,以色列)中,并在37℃,5%CO2下孵育16小时。在孵育结束时,在皮肤片去除,将细胞通过离心回收。
(v)BBB评分:大鼠中的行为分析使用旷场运动巴索-比蒂-布雷斯纳汉(BBB)评分来进行[Basso,D.M.,Beattie,M.S.&Bresnahan,J.C.A sensitive and reliable locomotor rating scale for open field testing inrats.J.Neurotrauma 12,1-21(1995).]小鼠中的行为分析使用巴索小鼠量表(BMS)针对后肢运动功能评定在旷场进行[Basso DM,Fisher LC,Anderson AJ,Jakeman LB,McTigue DM,Popovich PG.Basso MouseScale for locomotion detects differences in recovery after spinal cordinjury in five common mouse strains.J Neurotrauma.2006年5月;23(5):635-59]。
(vii)统计分析:重复测量ANOVA用于BBB和BMS评分,每周通过对比t-检验对治疗进行后续比较,并且通过Holm方法来校正多重比较(p=0.05)。
实施例1:腰椎穿刺注入皮肤活化巨噬细胞促进脊髓严重损伤后的大鼠功能恢复。
起初,我们寻找使用微创方式来增加受伤脊髓中的具有所需表型的血源性单核细胞数量的临床可行方法。我们研究了通过腰椎穿刺(LP)将皮肤活化血源性单核细胞注入脑脊液(CSF)中在减少大鼠中脊髓损伤后功能障碍方面的疗效。

野生型Sprague-Dawley大鼠遭受严重脊髓挫伤(见材料和方法)。8天后,向受伤的动物通过LP将0.5x106或1x106皮肤活化巨噬细胞(MQ)或媒介物(PBS)注射到CSF中。展示了在促进运动功能恢复方面的剂量依赖效应,对于注射了最大剂量的巨噬细胞的动物组观察到运动有显著改善(图1,表1)。
实施例2:将非活化来源于骨髓的小鼠单核细胞注射至CSF中促进脊髓损伤后的小鼠功能恢复。
另外我们测试了通过脑室内(ICV)注射到CSF中的非活化来源于骨髓的单核细胞是否会归巢至受伤脊髓实质。使野生型小鼠经受脊髓挫伤(见材料和方法)。3天之后,向受伤的动物ICV注射分离的表达绿色萤光蛋白(GFP)的原生骨髓源性单核细胞(0.5x106Cx3crlGFP/+单核细胞),并且分析损伤部位的所注射细胞的存在。注射后4天损伤部位的免疫组化分析揭示所注入的表达GFP的细胞在受伤实质中的存在,靠近病变部位的边缘(未展示[参见附图简述中的说明)。以类似的方式处理的动物针对运动量表(BMS)评估的运动活性来跟踪:单核细胞池的扩增导致了恢复超出自发恢复水平,这可以从小鼠运动显著改善看出(图2)。
这些结果表明,未活化的骨髓源性单核细胞可以从CSF归巢至受伤实质,并且因此,血液-CSF屏障可能是单核细胞藉以自发地进入受损CNS的途径。
实施例3:从PBMC中分离表达高水平的CX3CR1和低水平的CCR2的人单核细胞
在人类中,单核细胞的三个群体是由CD14和CD16的表达来定义,即:CD14+CD16-、CD14+CD16+和CD14dimCD16+。CD14+CD16-单核细胞占血液单核细胞的80%至90%,并表达高水平的趋化因子受体CCR2和低水平的趋化因子受体CX3CR1(不规则趋化因子(Fractalkine)的受体)。与此主要子集相反,人CD16+单核细胞表达高水平的CX3CR1和低水平的CCR2(Cros等人,2010)。根据Cros等人(2010),基因表达分析指示人CD14dimCD16+与鼠料巡逻Grldim单核细胞之间的相似之处。CD14dimCD16+细胞是涉及组织的先天局部监视和自身免疫性疾病的发病机制中的善意单核细胞。
方案I
为了从外周血单核细胞(PBMC)中分离出表达CX3CR1和低水平CCR2的CD14dimCD16+细胞,我们使用CCR2的阴性选择(CCR2+消耗)与CD14+的阳性选择(CD14+)相结合的方法,而从PBMC分离CD14+作为对照,如方案I示出。
3.1富集CD14dimCD16+细胞群:
(i)从人外周血中分离单核细胞:从健康供体采集的新鲜血液(8毫升)以PBS中的2.5%FCS 1:1稀释,并且装载于聚蔗糖梯度上(Ficoll-Paque plus,Amersham Biosciences)。试管在1000克下、在20℃下离心20分钟。将单核细胞相收集,并且用PBS洗涤两次。
(ii)CCR2+消耗:首先,单核细胞通过在室温下用FcR阻断试剂(2.5μl/106细胞)(130-059-901,Miltenyi Biotec)处理15分钟来进行Fc受体阻断。然后,无需洗涤,加入单克隆抗人CCR2-生物素试剂(FAB151B,R&D Systems)(10μl/106细胞),并在2℃-8℃下孵育35分钟。然后将细胞用冷的MACSTM缓冲液(1mM的EDTA,在PBS中的2%FCS)洗涤,并且在2℃-8℃下添加链霉亲和素微珠(130-048-101,Miltenyi Biotec)(20μl/107细胞)20分钟。将细胞洗涤并重悬于0.5毫升MACSTM缓冲液中。CCR2+细胞的消耗使用LD柱(130-042-901,Miltenyi Biotec),根据制造商的协议来完成。
(iii)CD14+细胞的分离:将细胞重悬于MACSTM缓冲液(80μl/107细胞)并且在2℃-8℃下加入CD14+微珠(130-050-201,MiltenyiBiotec)(20μl/107细胞)15分钟。然后,将细胞洗涤并重悬于0.5毫升的MACS缓冲液。CD14+细胞的阳性选择通过使用基于LS柱(130-042-401,Miltenyi Biotec)上磁分离,根据制造商的协议完成。
(iv)人单核细胞的荧光活化细胞分选(FACSTM)染色,所有样品根据制造商的协议来染色。所有样品均通过70μm尼龙网来过滤并且在室温下用FCR阻断试剂(30μl/106细胞)(130-059-901,Miltenyi Biotec)阻断15分钟。下面的荧光染料标记的抗人单克隆抗体根据制造商的协议中使用:PerCP偶联的抗CD45(345809,BD)、FITC标记的抗CD115(FAB329F,R&D Systems)、Pacific BlueTM偶联的抗CD14(BLG-325616)、Alexa700标记的抗CD16(BLG-302026)、PE标记的抗CX3CR1(MBL-D070-5)和PerCP偶联的抗CCR2(BLG-335303)。
3.2结果
单核细胞群体从活细胞群体中进行分析(图3A),并使用CD45+细胞的部位散射分布来确定(图3B),由细黑线划定)。单核细胞亚群的分布由它们的CD14和CD16抗原表达来呈现(图3C)。根据通过FACS的CD14和CD16染色,对于来自健康供体的PBMC的单核细胞亚群的分析揭示了以下亚群的分布:CD14+CD16-(80.5%)、CD14+CD16+(4.8%)和CD14dimCD16+(3.5%),图3A-C。有趣的是,CD14dimCD16+亚群也用属于CSF-1受体并且表征骨髓源性单核细胞的标记CD115来染色(图3D)。
根据通过FACS的CCR2和CX3CR1染色,对于来自健康供体的PBMC的单核细胞亚群的分析揭示,不表达CD16的单核细胞(CD16-亚群)高度表达CCR2,而表达CD16的单核细胞(CD14+CD16+和CD14dimCD16+亚群)展示低表达的CCR2和高CX3CR1(图4A-E)。
这一结果鼓励我们消除CCR2+细胞亚群,以富集CD16+细胞的亚群。
如从图5可以看出,CCR2+细胞消耗揭示了CD14dimCD16+亚群富集约9倍(由8%至73%)并且CD14+CD16+亚群富集10倍(从1%到10%),同时降低了CD14 + CD16 - 亚群约5倍(从91%至17%)。
为了研究CD16+单核细胞在脊髓损伤后恢复过程中的作用,在受伤之后第4天将富集CD16+亚群的单核细胞icv注射(至CSF中)至小鼠。后肢运动活性使用BMS量表系统每周两次监测长达受伤后4周。用媒介物(PBS)治疗的对照动物表现出随着时间的推移的适度自发恢复,在BMS量表上平均达到约2.5。在受伤后各个时间点,治疗组(CD16+富集单核细胞)的评分均低于对照组,表明其运动活性相较于对照组降低。我们的结论是,该亚群在SCI后第4天注入到动物的CSF中时减弱自发恢复(图76)。
与我们对于CD16+单核细胞发现的对于恢复的破坏作用相反,用总单核细胞亚群的治疗对于脊髓损伤的恢复是有益的,这取决于分离的方法(图7)。
论述。对于单核细胞的分离,我们使用由Miltenyi Biotec开发的MACS技术,其基于磁分离。在原则上,血样用偶联到不同抗体的磁性微珠来标记。细胞被加载到放置于磁场中的MACS柱上。磁性标记的细胞保留于柱内,而没有标记的细胞通过。保留的细胞可以通过将柱从磁场中去除来洗脱,这被命名为阳性选择。未标记的部分,命名为阴性选择,也可被收集。
单核细胞用两种方法分离;阳性或阴性选择。对于阳性选择,我们使用Miltenyi的CD14微珠,并且对于阴性选择,我们使用Miltenyi的单核细胞分离试剂盒II。显然,与通过阳性选择分离的细胞相比,使用阴性选择分离的单核细胞对于动物的脊髓损伤恢复表现出更好、更一致的有益效果。
总之,对于脊髓损伤之后的治疗,CD16可被用作在有利(CD16-)与破坏性(CD16+)血单核细胞之间进行区分的标记。CD16-单核细胞被称为经典单核细胞,并且CD16+是只占所有单核细胞10%的促炎性单核细胞。
实施例4:将耗尽CD3、CD19、CD56和CD16细胞的单核细胞亚群注射至CSF中促进脊髓损伤后的小鼠功能恢复。
鉴于上述观察结果,我们开发了使用MACS阴性选择过程来分离CD16-单核细胞的程序。PBMC用偶联至CD3、CD19和CD56的微珠来标记以耗尽T细胞、B细胞和NK细胞。通过磁柱的未标记的细胞被收集。该部分用CD16微珠标记并且再次加载于磁柱上。将通过该柱的细胞收集,并且用CD14和CD16荧光抗体染色以在FACS中分析。左图表示分离之前的PBMC(图8A)并且右图(图8B)表示如上所述阴性选择之后的最终产物。在PBMC中,总活细胞的19%是单核细胞(CD14+)。在单核细胞中,约10%是CD16+。在最终产物中,约90%的活细胞是单核细胞,其中CD16+少于0.1%。
这两个亚群在体外刺激之后均表明达到不同成熟阶段(CarmenSánchez-Torres等人,CD16+and CD16-human blood monocyte subsetsdifferentiate in vitro to dendritic cells with different abilities to stimulateCD4+T cells.Int.Immunol.(2001)13(12):1571-1581),因此它可以解释对损伤脊髓的不同效果。
然后,我们检查上述亚群在CD1裸鼠中对于减少脊髓损伤之后的功能障碍的效果。使用OSU ESCID挫伤设备(Ma M,Basso DM,Walters P,Stokes BT,Jakeman LB.Behavioral and histological outcomesfollowing graded spinal cord contusion injury in the C57B1/6mouse.ExpNeurol.2001年6月;169(2):239-54)使小鼠经受受控的严重脊髓损伤。在伤后第4天将细胞或PBS通过ICV注射应用到CSF。使用关于运动的巴索小鼠量表(BMS)每周两次测量SCI后的运动功能恢复,历时受伤后4周。
小鼠在损伤后的前3周表现出轻微的自发运动功能恢复。BMS中的高于3的恢复(在从0到9的量表中,脚爪的足底置放和站立时的重量支持被评分为4)被认为是有意义的功能恢复。用纯化单核细胞治疗增加了动物的运动功能恢复的速率(图9)。与PBS治疗的对照组中的14%(14个中有2个)相比,42%的单核细胞治疗的动物恢复(12个中有5个)。

13.一种包括基本上不含CD3+细胞、CD19+细胞和CD56+细胞的外周血单核细胞(PBMC)亚群的组合物,其用于治疗CNS损伤。
14.根据权利要求13所述的组合物,其中所述PBMC亚群进一步基本上不含CD16+细胞。
15.一种包括基本上不含CD3+细胞、CD19+细胞、CD56+细胞和CD16+细胞的外周血单核细胞(PBMC)亚群的组合物,其用于治疗CNS损伤。
16.根据权利要求13或15所述的组合物,其中所述PBMC亚群基本上富集CD14+细胞。
17.根据权利要求13或15所述的组合物,其中所述亚群的PBMC悬浮在适于注射的药学上可接受的载体中。
18.根据权利要求17所述的组合物,其中所述注射是用于施用至CSF中。
19.根据权利要求18所述的组合物,其中所述施用至CSF中是通过鞘内注射、腰椎穿刺(LP)、通过枕大池(CM)注射、脑室内(ICV)注射,或它们的组合。
20.一种用于治疗CNS损伤的方法,包括向有需要的个体施用有效量的外周血单核细胞(PBMC)亚群,其基本上不含CD3+细胞、CD19+细胞和CD56+细胞。
21.根据权利要求20所述的方法,其中所述PBMC亚群进一步基本上不含CD16+细胞。
22.一种用于治疗CNS损伤的方法,包括向有需要的个体施用有效量的外周血单核细胞(PBMC)亚群,基本上不含CD3+细胞、CD19+细胞、CD56+细胞和CD16+细胞。
23.根据权利要求20或22所述的方法,其中所述PBMC亚群基本上富集CD14+细胞。
24.根据权利要求23所述的方法,包括将所述PBMC亚群注入CSF中。
25.一种用于从血液中分离基本上不含CD3+细胞、CD19+细胞和CD56+细胞的人外周血单核细胞(PMBC)亚群的方法,其包括以下步骤:
(i)从血液中分离单核细胞;
(ii)从(i)的所述单核细胞中去除所述CD3+细胞、CD19+细胞和CD56+细胞,其是通过使所述单核细胞与抗CD3+抗体、抗CD19+抗体和抗CD56+抗体接触,其中每一个抗体被连接到微珠,从而使所述细胞结合至所述微珠,并且将所述微珠去除,
由此从血液中得到人外周血单核细胞(PMBC)的亚群,其基本上不含CD3+细胞、CD19+细胞和CD56+细胞。
26.一种用于从血液中分离基本上不含CD3+细胞、CD19+细胞、CD56+细胞和CD16+细胞的人外周血单核细胞(PMBC)亚群的方法,其包括以下步骤:
(iii)从血液中分离单核细胞;
(iv)从(i)的所述单核细胞中去除所述CD3+细胞、CD19+细胞、CD56+细胞和CD16+细胞,其是通过使所述单核细胞与抗CD3+抗体、抗CD19+抗体、抗CD56+抗体和抗CD16+抗体接触,其中每一个抗体被连接到微珠,从而使所述细胞结合至所述微珠,并且将所述微珠去除,
由此从血液中得到人PMBC亚群,其基本上不含CD3+细胞、CD19+细胞、CD56+细胞和CD16+。
27.根据权利要求26所述的方法,其中步骤(iv)包括以下子步骤:
(v)从(i)的所述单核细胞中去除所述CD3+细胞、CD19+细胞和CD56+细胞,从而从血液中获得基本上不含CD3+细胞、CD19+细胞和CD56+细胞的人PMBC亚群;并且
(vi)从(v)的所述PMBC群体中去除所述CD16+细胞,
由此从血液中获得基本上不含CD3+细胞、CD19+细胞、CD56+细胞和CD16+细胞的人PMBC亚群。
28.根据权利要求27所述的方法,其中步骤(iv)是在单一步骤中进行。
