发明概述

图6示出了在等效于图1的A-A的位置处截取的本发明的导尿管的横截面的替代配置,并且在该导尿管中设置了两个引流开口3,在第二管7的壁中一起设置有两个孔8。此外,设置了模制到引流管2的壁中的灌注内腔10。在这个实施方式中,灌注内腔界定了在引流管2的内壁中的与沿着第二管7的外壁的纵向槽14相匹配的突起部13。这些相应成型的轮廓再次提供了防止在引流管和第二管之间的相对旋转移动的阻力,确保了孔8与引流开口3轴向对准。
图7再次示出了在对应于图1的截面A-A的位置处的替代横截面,其中第二管7相对于引流管2的滑动移动是相对旋转移动,使得引流开口3可以与第二管7的壁中的孔8对准,或者如所示,设定离开与第二管7中的壁中的孔8对准。
图8示出了图7中示出的特征的优选变型,其中相对应的凹槽15和止动部16设置在第二管7的外表面和引流管2的内壁上,以允许在第二管7和引流管2之间旋转移动,但是防止在第二管7和引流管2之间的相对轴向移动。此外,这种配置允许引流出口3和孔7可靠地对齐定位或不对齐可靠地定位。
应当理解,这种止动部和凹槽机构可以被布置成例如通过提供与突出的止动部16配合的螺旋形设置的凹槽15来提供旋转移动和轴向移动的组合。
图9和图10分别示出了本发明的导尿管的远端和近端。为清楚起见,没有示出导尿管的中间长度,但是通常将具有在例如30cm到1m之间的长度。和在先前的实施方式中示出的那些特征共同的特征被相应地编号。在图9中示出的导尿管1的远端处,第二管7的末端被示出为用塞子9密封,但是由于在近端处被描述的布置同样可以打开,如在图3和图4中的实施方式示出的。
图10示出了导尿管1的近端,显示了灌注内腔10通到邻近导尿管的近端的灌注端口17的出口。在该图中,端口17仅仅显示为管的末端,但是可以优选且便利地设置有例如鲁尔接口连接器(Luer connector)的合适的连接器,或者在特别优选的实施方式中设置有自密封隔膜和/或单向阀(未示出)。
图10示出了在本实施方式中,第二管7通过通常由18表示的棘轮机构连接到引流管2。棘轮机构18具有附接到长型构件20的并通过连接到第二管7的外侧的压力板21连接到第二管7的相互配合的倒钩19。引流管2连接到棘轮机构18的接收部分22。在使用中,可以朝着导尿管的远端推动该机构的压力板21,从而在引流管2内可滑动地移动第二管7并将管从图9中显示的配置移动到图5中显示的配置,从而通过第二管7的内腔关闭在导尿管的远端和近端之间的流动路径。棘轮机构18防止导尿管返回到其流动配置,从而防止设备的重复使用并且因此防止可能由于设备的重复使用造成的交叉感染。
图11示出了本发明的导尿管的另外的实施方式的远端的轴向横截面视图。此外,与在其它附图中示出的那些特征相对应的特征相应地进行编号。在这个实施方式中,导尿管还设置有邻近引流管2的靠近导尿管的远端4的外壁的但是接近于引流开口3和优选地灌注开口11的气囊23和气囊23’。气囊23的内部通过气囊控制通道24连接到在导尿管的近端处的气囊控制端口(未示出),气囊控制端口允许气囊从第一位置23充气到膨胀位置23’以将导尿管固定在体腔内,例如,在膀胱的颈部处。一旦药物的引流和/或灌注已经被执行,则气囊可以随后被放气以允许导尿管被移除。
图12和图13示出了本发明的通常由1表示的导尿管的替代实施方式的远端。在这个实施方式中,再次设置了引流管2,引流管2在其侧壁中具有引流开口3。此外,引流管2在其远端4处通过盖5来密封。在这个实施方式中,以塞子25的形式的封闭构件位于引流管2的内腔内,并成形和设置大小,以便在塞子25的外表面和引流管2的内表面之间提供可滑动的但通常是耐流体的密封。塞子25连接到丝状物26,丝状物26延伸到导尿管的近端并或者在其末端处或者更加优选地穿过引流管2的侧壁退出引流管2。在使用中,当需要密封引流出口3时,拉力可以施加到丝状物26以带动塞子25经过引流出口3的表面(或沿引流管甚至更远),从而密封流动路径。使用丝状物,例如尼龙线的长度,由于丝状物25能够在张紧中而不是在压缩中传递力的事实,致使密封机构是有效地不可逆的。
在本文描述的任何实施方式中,当封闭构件是塞子26或第二管7时,特别优选的是流体,例如被施加在引流管的内表面和封闭构件的外表面之间的光硅酮脂。使用这种流体用于两个目的:首先,润滑封闭构件和引流管的相对滑动移动,和其次,提供在封闭构件和引流管之间的更不透水的密封。
在使用中,导尿管将被初始配置,使得在引流开口3和引流出口端口27之间存在开放通道。导尿管将被插入到体腔中,例如,通过尿道插入到膀胱中,直到看到流体例如尿液从引流出口端口27排出,引流出口端口27可以通过例如导尿管尖端连接器便利地连接到收集袋。一旦看到流体从端口排出,这将表示导尿管是在例如膀胱内的合适位置,并且如果提供的话,则定位气囊23可以被充气以将导尿管固定在合适的位置。然而,特别优选和设想的是,不提供这样的气囊23,而是导尿管的近端可以仅仅用胶带封在病人的肢体上例如病人的腿上的位置,以将导尿管固定在合适的位置,同时进行药品的引流和/或灌注。为了有助于这样的固定,可以设置夹子构件,导尿管的近端可固定到夹子构件中,并且夹子或者用粘合胶带或者用一些其它可释放的装置紧固到病人。
一旦足够的流体(例如尿液)已经从体腔中排出,则封闭构件可以被致动以关闭在引流开口和引流出口之间的流动路径。在这个阶段,如果药物将被给予,则药物可以穿过在导尿管的近端处的灌注端口17通过注射被引入到体腔中,以通过灌注内腔10出现在灌注出口11处。
适于给予药物的注射器和连接器的例子在图14-16中被示出。标准注射器140用于保持定量的体积并通过注射器出口141递送定量的体积。注射器出口141被容纳在灌注端口17中并通过适合的鲁尔接口滑动连接器连接到灌注端口17(图10)。下面描述的调整防止连接器与标准鲁尔接口夹具连同使用。
连接器包括两个主要元件。第一元件是接头(adapter)142,其在第一端处具有锥形凹部143以接收注射器出口141。尽管可以包括附加的粘接装置以提供更固定的连接,但是出口141和凹部143通过推入配合连接方式连接。在端视图15a中示出的接头142的第二端具有凹入通道144和凸肋145,这实现了防止接头142装配到如上所述的标准鲁尔接口。
接头142本身被放置在单向阀配件146中,该单向阀配件146在第一端处具有与通道144和肋145的形状互补的形状的凹部147以提供固定装配。在凹部147内的凸肋148防止标准鲁尔接口配件被插入。阀配件146的第二端可以被安装到灌注端口17。单向阀配件146包括弹簧149或本领域中已知的其它弹性装置以当没有压力施加在注射器140中的流体上时将阀146偏置到关闭位置。在图16中示出了包括接头142和阀配件146的组装的注射器和连接器。
一旦完成给药,则定位气囊23(如果存在的话)可以被放气,并且从体腔抽出导尿管进行处理。

Claims (14)

1.一种用于插入到患者的体腔中的导尿管,包括:
引流管,其从邻近所述导尿管的远端的引流开口延伸到邻近所述导尿管的近端的引流出口;
封闭构件,其是可安装的,以用于在所述引流管内在第一位置和第二位置之间滑动移动,所述第一位置使得所述引流开口与所述引流出口流体连通,在所述第二位置,所述封闭构件阻挡在所述引流开口和所述出口端口之间的流体连通;和
致动器,其从所述导尿管的所述近端是可操作的以在所述第一位置和所述第二位置之间移动所述封闭构件。
2.根据权利要求1所述的导尿管,还包括灌注内腔,所述灌注内腔从邻近所述导尿管的近端的灌注端口延伸到邻近所述导尿管的远端的灌注出口。
3.根据权利要求2所述的导尿管,还包括止回阀,所述止回阀被配置为阻止在所述灌注内腔中在从所述远端到所述近端的方向上的流动。
4.根据任一前述权利要求所述的导尿管,其中所述致动器被配置为在所述第一位置和所述第二位置之间不可逆地移动所述封闭构件。
5.根据任一前述权利要求所述的导尿管,其中所述封闭构件包括塞子,并且所述致动器包括附接到所述塞子的丝状物,所述丝状物延伸到所述导尿管的所述近端。
6.根据权利要求1到4中任一项所述的导尿管,其中所述封闭构件包括位于所述引流管内的第二管,所述第二管在其壁中具有孔,使得在所述第一位置,所述孔与所述引流开口流体连通,并且在所述第二位置,所述流体连通被阻挡,并且其中所述第二管延伸到所述导尿管的所述近端,使得所述第二管能够被操纵以在其第一位置和第二位置之间可滑动地移动所述管,从而充当所述致动器。
7.根据权利要求6所述的导尿管,其被配置成使得所述滑动移动是在轴向方向。
8.根据权利要求7所述的导尿管,其中所述引流管的横截面轮廓和所述第二管的外表面的轮廓被成形,以便阻止在所述引流管和所述第二管之间的相对旋转移动同时允许在所述引流管和所述第二管之间的相对轴向移动。
9.根据权利要求6所述的导尿管,其被配置成使得所述滑动移动包括所述第二管相对于所述引流管的旋转。
10.根据权利要求9所述的导尿管,其中配合的凹槽和止动部被设置在所述第二管的外表面和所述引流管的内壁上,所述配合的凹槽和止动部配置成允许在所述第二管和所述引流管之间的相对旋转移动同时阻止在所述第二管和所述引流管之间的相对轴向移动。
11.根据任一前述权利要求所述的导尿管,其中在所述引流管的内壁和所述封闭构件之间设置了润滑剂,以方便在所述封闭构件和所述引流管之间的相对移动和/或以在所述封闭构件和所述引流管之间提供耐流体密封。
12.根据权利要求6到11中任一项所述的导尿管,其中设置了棘轮机构,以确保所述第二管相对于所述引流管的不可逆的移动。
13.根据任一前述权利要求所述的导尿管,还包括气囊,所述气囊邻近所述导尿管的外壁、靠近所述导尿管的远端且接近于所述引流开口,并且通过气囊控制通道连接到在所述近端处的气囊控制端口,所述气囊是可充气的和可放气的以分别增加和减少所述导尿管的局部直径以允许所述导尿管可逆地固定在体腔内。
14.一种参考如由附图的任何合适的组合说明的实质上本文所公开的导尿管。
