背景技术
支架广泛地用于在病人的身体中维持敞开的腔管。例如,支架可以用于维持冠状动脉、颈动脉、脑动脉、腘动脉、髂动脉、股动脉、胫动脉、肾动脉、包括静脉在内的其他血管、或诸如输尿管、尿道、支气管、食道、泪道、输卵管、鼻腔之类的其他身体腔管、或其他通道的通畅。
支架一般为由不锈钢、镍钛诺、埃尔基洛伊耐蚀游丝合金(elgiloy)、钴铬合金、钽、和其他金属制成的金属管状结构,尽管聚合物支架也是已知的。支架能够为长期持久的植入物,或者能够为至少部分生物可吸收的。生物可吸收的支架能够为聚合的、生物聚合物的、陶瓷的、生物陶瓷的、或金属的,并且能够随时间流出诸如药物之类的物质。非生物可吸收的支架也可以随时间释放药物。在一些设计中,支架为敞开单元式或闭合单元式圆筒形结构。支架以折叠状态穿过身体腔管。在身体腔管中的梗阻点或其他部署位置处,支架扩展到扩展直径以在部署位置处支承腔管壁并且维持敞开的腔管。
一种支架常常称为“囊状可扩展”支架。用于囊状可扩展支架的支架输送系统通常包括安装在多腔管上的可充气的囊状物。在其上带有被卷曲的支架的支架输送系统能够常常通过导丝行进至治疗位置,并且囊状物能够充气以扩展和部署支架。
在形状记忆支架的情况下,支架被预编程以记住在一个温度下的减小的直径或形状以及在较高温度下的扩展的直径或形状。支架在较冷的温度下被压缩到支架输送系统的远端部上,而在其上带有卷曲的支架的支架输送系统能够在维持为低于支架的相变温度的同时常常通过导丝行进至治疗位置。在部署位置处,支架变暖或被允许变暖到一较高温度,在该温度下,形状记忆材料的相变导致支架从折叠状态扩展到扩展状态。
其他支架是所谓的“自扩展”支架并且不使用囊状物来导致支架的扩展。自扩展支架的一个示例是由可弹性形变的材料(例如,诸如镍钛诺之类的超弹性材料)制成的管(例如,盘管、网状管、或包括具有或不具有焊接线接头的成形线的管)。敞开式或闭合式单元支架一般通过对管的激光切割、或者在将板材焊接为管形状之前或之后将图案切割到板材中、以及其他方法形成。一种很普遍的自扩展支架由超弹性镍钛诺形成,例如由明尼苏达州普利茅斯市的ev3公司制造的EverFlex支架。
自扩展支架一般以折叠状态在径向压缩或轴向拉紧的作用下固定到支架输送系统上。这种系统能够包括外管状构件和内管状构件。内管状构件和外管状构件能够相对于彼此轴向滑动。支架(处于折叠状态下)围绕内管状构件的远端部安装到支架输送系统上。外管状构件(也称为外护套)在远端部处围绕支架。
在使支架输送系统通过身体腔管行进之前,导丝首先穿过身体腔管到达部署位置。输送系统的内管在其长度的至少一部分上为中空,以使其能够通过导丝行进至部署位置。组合的结构(即,安装在支架输送系统上的支架)穿过病人的腔管直到输送系统的远端部抵达身体腔管内的部署位置为止。部署系统和/或支架可以包括不透射线的标记以容许医生在部署之前通过荧光透视法看到支架的定位。在部署位置处,将外护套缩回以使支架暴露。被暴露的支架在身体腔管中自由地自扩展。在支架的扩展之后,内管自由地穿过支架,以使输送系统能够通过身体腔管被移除,从而将支架留在部署位置中的适当位置。
支架输送系统可以包括通过经导丝(OTW)输送导管、快速交换型(RX)输送导管、或固定导丝型(FW)输送导管。OTW输送导管允许导丝穿过在输送导管的整个长度上延伸的腔管。RX输送导管允许导丝穿过在输送导管的部分长度(通常10cm-30cm)上延伸的腔管。OTW输送导管比RX输送系统提供更好的支承,但是它们需要使用较长的导丝,这对于操作来说是不方便的。FW输送系统非常简单之处在于它们没有导丝腔管。FW系统行进至治疗位置而没有通过预设的导丝进行跟踪的益处。为了确保支架能够被输送到预期的治疗位置,在其上安装有支架的支架输送导管必须至少具有足够的跟踪能力、柔性和抗绞缠性,并且支架一旦被植入就必须至少具有足够的径向作用力、抗绞缠性和疲劳寿命。
植入到一些位置中的支架与植入到其他位置中的支架相比可能需要不同的物理属性。在管状动脉或肾动脉中使用的支架可能因脉管的狭窄区域通常很短而相当短,然而在腿部中使用的支架常常必须很长,以使腿部脉管中常见的病症能够被治疗而没有短支架的交迭。
在其他示例中,植入到动脉中的支架必须具有高的脉动疲劳寿命以经得住由动脉的直径变化(随每次心跳发生)所导致的小的直径变化,而植入到非脉管管道中的支架不需要该属性。在又一示例中,植入到诸如股浅动脉(SFA)之类的四肢血管中的支架必须具有高的疲劳寿命以经得住在腿部弯曲或伸直时发生的血管长度和方向的变化,而植入到冠状动脉中的支架不需要满足该需求。长支架还必须容忍因在支架植入期间医生的技术以及支架输送系统的实际限制而导致的与设计长度大不同(通常更长)的植入长度。
不幸地,对于部署在诸如SFA之类的一些血管中的支架的常见问题是支架因血管的动态载荷环境而随时间破裂。次于支架破裂的问题可能包括内膜增生、疼痛、出血、血管栓塞、血管穿孔、高的支架内再收窄速率、不均匀的药物输送分布、不均匀的血管覆盖以及其他问题,并且可能需要再干预以解决问题。如在不同的临床试用(Durability I,Resilient,Sciroccoand Absolute)和研究论文(例如,Prevalence and clinical impact of stentfractures after femoropoliteal stenting;J.Am.Coll.Cardiol.2005Jan 18;45(2);312和Long-Segment SFA stenting-The Dark Sides:In-StentRestenosis,Clinical Deterioration,and Stent Fractures;J Endovasc Ther2005;12:676-684)中所报告的,在SFA中的支架破裂速率范围为在植入之后的1年-3年内从2%到28%。
尽管支架通常获得高的脉动疲劳寿命,但是它们在除脉动以外的病人体内的载荷条件下比如对于一些植入位置合适的纵向伸展、扭转及屈曲等常常具有不足的对破裂的抵抗性。植入到腘动脉、髂动脉、股动脉、胫动脉、颈动脉、股腘动脉及其他位置中的支架可能经受较大量的轴向的、弯曲的或扭转的循环载荷。近期的数据已经显示,诸如那些植入到SFA中的一些支架在植入时必须经得住高达35%的轴向伸长率/压缩率。尽管已经进行多种尝试来改进可植入支架的抗疲劳性,但是具有用于这些需要的解剖学位置的合适属性(尤其是高的疲劳寿命)的植入物仍有待开发。
所需要的是一种能够容易制造的支架,并且该支架在被植入到除经受由心脏跳动产生的那些力以外还经受由病人活动产生的高机械力的位置中时能够经受得住而不会破裂。还需要的是一种在植入之后具有高的疲劳寿命的支架、以及一种经受得住植入期间的伸长而不会破裂的支架。
发明内容
根据本发明的一个方面,抗疲劳支架包括柔性管状结构,该柔性管状结构具有内径、外径和位于它们之间的侧壁,并且具有延伸穿过所述侧壁的孔隙。根据本发明的其他方面,公开了用于制造抗疲劳支架的过程。根据本发明的另外的方面,提供了用于抗疲劳支架的输送系统及使用方法。

当支架在支架完全扩展时在支杆的顶点附近没有应变的情况(如用于自扩展支架的常见情况)下,在顶点附近在支杆中的初始应变为零,从而当变形时在支架的顶点附近在支杆中的应变等于使用以上公式所计算的。重要地,从该公式,我们知道,计算得来的支杆应变的变化在角18的变化较小时较小,在支杆宽度SW较小时较小,并且在支杆长度LS增大时较小。
在形变支架中的最大应变被称为峰值应变。峰值应变通常出现在顶点的邻近区域中的单个支杆中,尽管取决于支架设计的细节其也可以出现在支架的其他位置。在支架中的主要应变可以为拉伸应变(通常定义为正)或压缩应变(通常定义为负)。在一些植入位置例如SFA或腘动脉中的支架可能经受除上述拉伸和压缩应变以外的弯曲应变和扭转应变。在一些支架实施例中,在支架上的这种扭转和弯曲应变将主要分解为在支架顶点的邻近区域中的拉伸或压缩应变。高的拉伸应变可能导致裂缝出现并且通过支架扩散,从而引致减小的疲劳寿命和支架的破裂。压缩应变不倾向使裂缝敞开,从而除非应变的大小非常大,一般不会导致减小的支架寿命。形变支架的一些部分在使用期间可能为高应变,而其他部分可能根本没有应变,所述后面的部分具有零应变。形变支架能够被认为是微小体积区域的集合体,每个区域均具有应变水平;共同地,区域的应变水平范围为从最大负值到最大正值。对于在身体中服务的支架而言,如果支架应变被维持为低于支架材料的耐久极限,则只要支架材料已经经受适当的材料处理、表面加工并且具有合适的生物相容性,可以期望高的疲劳寿命。但是,如果在身体中服务的支架经受比支架材料的耐久极限高的支架应变,则不管支架材料处理、表面加工以及生物相容性也不能期望高的疲劳寿命。
诸如支架10之类常见的现有技术的支架设计为使得在脉动载荷的条件下,即在振动的周向压缩应变下,将支架中的应变维持在低的水平。支架10还已知的是提供有限量的轴向伸长率(最多大约10%-15%)而不会减少脉动疲劳寿命。该有限量的轴向伸长率适合于一些支架植入位置。但是,已经确定,植入到其他位置例如SFA或腘动脉中的支架可能使支架经受很大的伸长率。例如腿部能够因正常的腿部运动而给予±5%的伸长率,而医生可能在支架植入期间向支架给予多达30%的伸长率并且可能更大。另外,支架可能因正常的腿部运动而被加载扭转载荷和弯曲载荷。
支架的百分数伸长率能够定义为支架单元的长度沿轴线A-A的变化。图1示出了支架10处于静止中并且具有如先前定义的长度L。图1A示出了支架10伸长50%并且具有长度L’。支架伸长率定义为长度的增加(伸展的长度L’减去未伸展的长度L)除以未伸展的长度L。如果支架已经在其整个长度上均匀地伸长,则每个支架单元均将伸长与支架总体相同的量。图2示出了具有如前面定义的单元长度22的未伸长的支架10。图3示出了支架10伸长50%并且具有单元长度22’。单元伸长率定义为单元长度的增加(单元长度22’减去长度22)除以未伸展的单元长度22。实践中,发现的是支架伸长率通常在支架的长度上不是均匀的,其中一些单元比其他单元伸长得更长。有时还见到一些单元缩短,即一些单元具有负的计算伸长率。还可能的是支架的总体长度缩短。支架伸长是用于在较复杂的载荷条件下预测支架疲劳寿命的有用的代替模型。支架伸长能够导致在支架顶点的邻近区域中的高的拉伸应变,该拉伸应变能够代表由较复杂的载荷条件比如当支架经受拉伸、扭转和弯曲的组合时所给予的应变。
图3示出了轴向伸长50%的支架10,其表示为伸长条件下的支架10’。支架10’示出为径向扩展、平行于轴线A-A纵向切开并铺平。支架10’单元的几何形状基本变形于支架10的几何形状(图2)。该变形能够通过对伸长支架10’中的角18与未伸长支架10中的那些角进行比较而看到。能够见到的是角18K、18N、18P和18r已经分别减小到18K’、18N’、18P’和18r’,而角度18M、18Q、18S和18T已经分别增大到18M’、18Q’、18S’和18T’。图3中见到的变形的单元几何形状通过将一些支架支杆中的拉伸应变增大到较高的水平而导致伸长支架10’的减小的疲劳寿命。

当与支架10的支杆17相比时,支架40的支杆47已被修改。支杆47B沿着支架的周向方向加厚,而支杆47A沿着支架的轴向方向加长并且沿着支架的周向方向变薄。在所有其他尺寸和材料处理方面,支架40与支架10相类似。作为上述支杆修改的结果,与相似伸长的支架10(即,支架10’)相比,支架40在伸长时具有改进的疲劳寿命。改进的疲劳寿命因所述支杆修改在支架40或者一个或更多个单元45伸长、弯曲或扭转时防止在顶点49的邻近区域中产生高拉伸应变而实现。
图5示出了轴向伸长50%的支架40,其表示为伸长条件下的支架40’。支架40’示出为完全径向扩展、平行于轴线A-A纵向切开并铺平。支架40’单元几何形状45’变形于支架40的几何形状。该变形能够通过对伸长支架40’中的角48与非伸长支架40中的那些角进行比较而看到。能够见到的是角48K、48N、48P和48R已经分别减小到角度48K’、48N’、48P’和48R’,而角48M、48Q、48S和48T已经分别增大到角48M’、48Q’、48S’和48T’。与支架10’不同,由支架40的伸长或者在顶点49X的邻近区域中的一个或更多个单元45的伸长而引致的拉伸应变尽管增大但是已经增大到可接受的水平,这是因为支杆47B的增大的厚度限制了增大到角48M’、48Q’的量,并且当角48S’、48T’增大时,较薄且较长的支杆47A减小了在顶点49X的邻近区域中所引致的拉伸应变。
换言之,支架40或者一个或更多个单元45的伸长导致了在顶点49X的邻近区域中的支架体积内的拉伸应变的较小集中和在顶点49Y的邻近区域中的体积内的压缩应变的较大集中。在图5中所见的伸长的单元几何形状通过将拉伸应变限制到在支架单元顶点的邻近区域中的可接受的水平而允许伸长支架40’的改进的疲劳寿命。
支架40具有另一特性,该特性在其部署在治疗位置中时提高其抗破裂性和疲劳寿命。在单元45的内部中,顶点49没有沿着轴线A-A彼此直接相对,相反,它们彼此周向偏移。因此,当支架40经受轴向压缩(负的计算伸长率)时,不相对的顶点不会彼此相撞或摩擦。相对顶点的这种碰撞或摩擦能够导致支架表面缺陷形成,这些缺陷能够随时间循环扩散,从而导致支架破裂。
计算在顶点的邻近区域中的材料应变的另一方法涉及到有限元分析(FEA)的使用。在一种方法中,利用市售FEA软件包(Abaqus)来计算应变。关键的模型参数如下:使用20节点三维砖形状的二阶连续元;使用在高应变区中的高的局部网格密度和在低应变区域中的较低的网格密度;以及在较高与较低网格密度区域之间的合适的网格过渡(即,10或更小的砖面长宽比以及介于45度与135度之间的砖夹角)。为了使拟合收敛速度和计算时间与拟合准确性平衡,对高应变区域执行试运行并且选取合适的网格密度,在该密度处,更小的网格不会在公差范围内改变结果。
支架材料的性能通过对具有与模拟支架相同的组成和相似的热-机械历史的试样的拉伸试验来确定。支架的最终几何形状通过模拟前兆管以及支架制造工艺(热膨胀、退火、锉削、打磨、喷砂、以及电解抛光)对模拟管的影响来确定,尽管能够使用其他确定最终的支架几何形状的方法。通过以上模拟确定的最终的支架几何形状在FEA拟合中在拉伸载荷条件下发生应变,并且算出最大理论应变、平均应变以及在静态条件下应变的值。通过以上模拟确定的最终的支架几何形状还在FEA拟合中在股浅动脉植入位置的载荷条件(包括收缩、伸展、用于疲劳的单轴拉伸重复载荷、走动、以及爬梯载荷)下发生应变。最大理论应变、平均应变、以及在循环条件下应变的振幅被算出并且预测应变支架的疲劳寿命。
图3A示出了在支架10’(支架10,伸长50%)中的应变的FEA分析的部分结果。仅仅示出了支架10’中具有最高应变的部分。能够见到,6.3%的峰值拉伸应变位于与角18S’相对的顶点19X的邻近区域中。因为在该顶点附近在支杆中的高拉伸应变,支杆趋于低疲劳寿命。而且,对单元15的16个顶点中的每一个的邻近区域中的最高应变的平均显示出在顶点的邻近区域中的平均应变为3.0%±2.6%。
图4A示出了在支架40’(支架40,伸长50%)中的应变的FEA分析的部分结果。仅仅示出了单元45’中具有最高应变的部分。能够见到,2.97%的峰值拉伸应变位于与角48T’相对的顶点49X的邻近区域中。因为与现有技术相比的减小的拉伸应变,与支架10相比,在该顶点附近支杆能够维持改进的疲劳寿命。而且,对单元45’的16个顶点中的每一个的邻近区域中的最高应变的平均显示出在顶点的邻近区域中的平均应变为2.6%±0.6%,因较低的平均应变及应变中的减小的变化而同样是对现有技术支架10的改进。应变中的减小的变化导致支架的沿着支架的周向确定的径向扩展力更均匀。因此,根据以上教导设计的支架将具有更均匀的部署性能并且将更均匀地抵抗趋于使支架的部署直径减小的血管作用力。
已经使用FEA分析在不同的伸长率下对在支架40的单元45、45’的16个顶点的邻近区域中的平均应变作进一步表征。这些结果显示在下面的表格中。

尽管支架40具有如上述的应变和伸长特性,但是能够构思出根据本文教导设计的具有其他应变和伸长特性的支架。在一些实施例中,发明支架能够在峰值应变保持低于0.5%的情况下伸长10%、15%、20%、25%、30%、35%、40%、45%或50%。在其他实施例中,发明支架能够在峰值应变保持低于1%、或低于2%、或低于3%、或低于4%、或低于5%的情况下伸长10%、15%、20%、25%、30%、35%、40%、45%或50%。在其他实施例中,根据本文教导设计的支架能够具有如在本段落中引用的峰值应变和伸长率的任一组合,其中峰值应变的标准方差为应变的0.3%、应变的0.5%、或应变的1.0%中的一个。
在其他实施例中,构思出根据本文教导设计的具有不同的支架单元应变和伸长特性的支架。在一些实施例中,发明支架单元能够在单元的顶点的邻近区域中的峰值应变保持低于0.5%的情况下伸长10%、15%、20%、25%、30%、35%、40%、45%或50%。在其他实施例中,发明支架单元能够在单元的顶点的邻近区域中的峰值应变保持低于1%、或低于2%、或低于3%、或低于4%、或低于5%的情况下伸长10%、15%、20%、25%、30%、35%、40%、45%或50%。在其他实施例中,根据本文教导设计的支架单元能够具有如在本段落中引用的峰值应变和伸长率的任一组合,其中峰值应变的标准方差为应变的0.3%、应变的0.5%、或应变的1.0%中的一个。
支架已经根据本发明的原理进行制造并且如在下面的示例中所示已被发现具有期望的应变分布和优异的抗疲劳性。根据下面的方法对示例性支架和对比支架的抗疲劳耐久性进行测试。
单轴测试法:使用Bose 3300单轴测试机,能够以步进的疲劳测试法来确定支架寿命。根据本方法,步骤1包括使支架样品伸长到表格所示的用于步骤1的平均应变,后继使样品长度沿着轴线A-A的方向以40Hz振动500,000次循环到表格所示的用于步骤1的循环应变。如果样品在步骤1期间没有破裂,则样品另外经受步骤2。步骤2包括如在步骤1中执行的使样品伸展和振动但是以表格所示的用于步骤2的不同的预定的平均应变和循环应变。如果样品在步骤2期间没有破裂,则样品另外地在每个随后的步骤(如下面表格所示)中依次经受拉伸和振动500,000次循环直到样品破裂。在出现样品破裂的步骤中的平均应变被记录为支架的失效平均应变。疲劳测试在样品被浸入到37℃的水中时执行,并且专用的固定装置用于抓住单个支架样品的端部。基于在未应变样品的抓握处之间的初始长度来计算应变值。

本方法特别适于快速的和长期的耐久性研究。其允许调节施加到测试样品上的平均应变和循环应变。这些参数能够被细调以模仿具体的临床环境能够施加到植入支架上的实际的平均应变值和循环应变值。
多轴周向支架(MAPS)疲劳测试法:该系统由Bose公司(型号9400多轴周向支架测试设备)开发并且其模拟由周边动脉因例如由日常活动(比如走动、坐下和爬梯)所导致的弯曲、扭转、拉伸、压缩和径向延伸而施加到支架上的动态载荷。支架以8%±2%的伸长率部署在乳胶管中,该乳胶管具有与健康的周边脉管相似的机械性能,其中带有支架的管被安装到测试机上,并且允许被维持在37℃的磷酸盐缓冲盐水以~64ml/min和可忽略的压力流过支架。在一次循环中,所部署的支架沿着支架轴线A-A伸展3.5%并且同时围绕轴线A-A每mm长度扭转0.8°;随后所部署的支架的轴线在位于支架的中点处的20mm曲率半径上从0°(无弯曲——支架轴线A-A为直线)弯曲到69°;随后去除弯曲回到0°,随后同时去除扭转并且缩短3.5%而回到初始的起始构型。该伸展、扭转、弯曲、去除弯曲、去除扭转和去除伸展的循环以1.5Hz的频率发生。支架在出现破裂之前维持的循环数被记录为支架的失效循环数。

参照图6和图7,支架包括沿支架的周向方向形成起伏的排状图案的一系列支杆6A、7A、顶点6B、7B和接头6C、7C。支架包括沿支架的纵向方向连贯地多个这种排状图案。除了近端排和远端排,每对相邻的排均经由接头6C、7C彼此相连以形成单元。例如,如图6C、图7C中所示,单元可以包括八片式单元。当然,可通过本公开构思出具有多于或少于八片的单元。如所示,接头6C、7C可以设置在比每个单元的全部顶点少的顶点处。在一些方面中,接头6C、7C可以绕支架周向等距地间隔开。
根据本公开的不同方面,从接头6C延伸的支杆6A中的至少一个可以具有比从接头6C延伸的另一支杆6A小的宽度尺寸。例如,从接头6C延伸的一个支杆可以包括0.0044英寸的宽度,而从接头6C延伸的另一支杆可以包括0.0053英寸的宽度。在一些方面中,从接头6C延伸的支杆6A可以具有比沿任一周向方向与从接头6C延伸的支杆6A紧邻的支杆6A大的纵向尺寸。
根据本公开的不同方面,从接头7C延伸的两个支杆7A可以具有比与接头7C间隔开的一个或更多个支杆7A小的宽度尺寸。例如,从接头7C延伸的两个支杆7A可以包括0.0045英寸的宽度,而与接头7C间隔开的多个支杆7A可以包括0.0051英寸的宽度。在一些方面中,沿任一周向方向与从接头7C延伸的支杆7A紧邻的支杆7A还可以具有0.0045英寸的减小的宽度。在一些方面中,从接头7C延伸的支杆7A可以具有比沿任一周向方向与从接头7C延伸的支杆7A紧邻的支杆7A大的纵向尺寸。在图6和图7所示的实施例中,单元包括蜿蜒的支杆图案,其对于每个单元的右侧和左侧而言构成反向镜像。

图9示出了OTW输送系统70,该系统70包括支架82和具有远侧区域80的植入物输送导管76。植入物输送导管76包括导管杆75、导丝腔管(未示出)、近侧导丝出口79、近侧把手78、护套84和远侧歧管77A。护套84可选地包括渗出孔84B,而远侧歧管77A包括带有止阻栓77B的注射管。导管杆75、导丝腔管、近端把手78以及远侧歧管具有与上面结合图7A到图7C描述的导管杆65、导丝腔管65A、近侧把手68以及远侧歧管67基本相同的构型、尺寸和功能。支架82可以包括支架40或其他支架。
用于输送系统60、70的部件的可选材料在本领域中是众所周知的,只要满足组件的功能需求,就能够替代以上列出的任一非限制性示例。
现在对使用支架输送系统将抗疲劳支架输送到病人身体内的一示例方法进行描述。使用本领域中众所周知的技术,将导丝经由皮肤插入病人的血管内并且行进至病人的身体中的期望区域。使用诸如荧光透视法之类的成像技术来识别血管的有病部分并且选取具有对于治疗位置适当的长度和直径的支架。参照图8和图9,自扩展支架输送系统60、70通过导丝行进至治疗位置,并且通过使用诸如荧光透视法之类的成像技术将支架例如支架40相对于治疗位置设置在期望的位置处。
将导管杆65、75保持静止并且使护套84收回以暴露出支架40。当护套84被收回时,支架40扩展到与血管的腔管壁相接触。在一些方法中,支架40在无伸长的情况下部署。在其他方法中,医生在部署期间例如通过在支架部分地部署时收回导管杆65、75、或者通过快速收回护套84、或者因在弯曲的治疗位置中护套84抵靠支架40的摩擦、或者因其他原因而使支架伸长。在一种方法中,支架伸长15%。在其他方法中,支架可以伸长10%、20%、25%、30%、35%、40%、45%或50%。在另外的方法中,支架可以不均匀地伸长,使得一些支架单元例如单元45’比支架40的总伸长量伸长得更多或更少。在一种方法中,支架单元伸长15%。在其他方法中,支架单元可以伸长10%、20%、25%、30%、35%、40%、45%或50%。
在抗疲劳支架例如支架40植入病人之后,病人可以因后续拜访而回见植入医生。在后续工作期间,支架可以使用诸如荧光透视法、超色波、或磁共振成像之类的成像技术进行成像以评估支架是否已经破裂或因其他原因。
所构思的发明还适于除上述那些以外的支架。例如,抗疲劳支架可以包括锥形支架、漏斗式支架、编织式支架、分叉支架、以及在本领域中已知的其他支架。锥形支架通常具有一个直径的近端部和第二直径(通常更小直径)的远端部。漏斗式支架通常在圆筒形支架的近端部处具有短的锥形部分,其中漏斗部分的直径比圆筒形部分大。编织式支架通常包括通过使用编织方法制成的管。编织式支架的示例为由Boston Scientific,Natick,MA销售的Wallstent。分叉支架在血管分叉的位置处置于病人中。分叉支架通常包括单个支架部分,该单个支架部分分叉为两个支架部分并且外形类似于用于将一件式管件连接到两件式管件上的Y形配件。

Claims (18)

所述孔隙被切割到所述侧壁中以形成单元和支杆,所述单元由在顶点处结合的多个支杆界定,所述顶点具有显示拉伸应变的水平的结构,纵向相邻的单元通过多个连接件彼此连接,所述连接件在纵向相邻的单元的周向对齐的相邻顶点之间延伸;
所述支架具有沿所述中心轴线的长度,其中,所述支架沿纵向方向的伸长导致在所述顶点中的拉伸应变的水平的变化;
其中,每个单元均包括具有第一长度和第一宽度的第一组支杆、以及具有第二长度和第二宽度的第二组支杆,所述第二长度和第二宽度与相应的第一长度和第一宽度不同,使得当所述支架伸长至少10%时,在每个顶点中的拉伸应变的水平小于3%。
