技术领域
本发明涉及一种植入体,更具体地说涉及一种关节假体,更具体地说涉及一种椎间植入体,甚至更具体地说涉及一种用于代替椎间盘的关节假体。
背景技术
椎间的脊椎盘处于脊柱中的相邻椎骨之间。各脊椎盘形成软骨关节,该软骨关节允许椎骨稍微运动,并用作韧带以便将椎骨保持在一起。由于普通的磨损和撕裂,脊椎盘可能受损或移位,从而产生通常称为“椎间盘突出”的问题。过去,受损或患病的脊椎盘通过除去该脊椎盘和用骨片填充该空间(以便促进相邻椎骨的熔合)而进行治疗。不过,该方法导致病人背部的运动性损失。用于治疗受损或患病的脊椎盘的另一方案是用假体脊椎盘植入体代替受损的脊椎盘。不过,当前的假体脊椎盘植入体通常不能复制由健康的脊椎椎骨能够进行的运动范围,和/或包含导致磨损和可能碎屑的运动部件。因此,希望开发一种假体脊椎盘植入体,它能够更接近和允许通常由健康脊椎区段进行的运动范围,而不会有由于沿其它表面运动的部件引起的磨损和摩擦。
发明内容

椎间间隔件还可以有可选的销元件,该销元件与端板或中间部件中的至少一个操作相联,并接收在由铰接元件的相邻区段限定的空间内。销元件可以接收在由最内侧区段限定的中心空间内。销元件可以与端板或中间部件中的一个固定连接,并朝着端板或中间部件中的另一个延伸,且可以压配合到中心空间内。也可选择,销元件可以松弛地接收在使得铰接元件的区段分开的至少一个空间内。端板或中间部件中的至少一个还包括至少一个孔,该孔设置成用于除去制造过程的碎屑。
另外,上部端板、下部端板和铰接元件可以由单个材料体整体制造。第一和第二中间部件以及铰接元件也可以由单个材料体整体制造。间隔件还可以包括第二铰接元件,该第二铰接元件有:多个区段,各区段由至少一个空间分开;以及至少一个桥接元件,该桥接元件连接相邻区段。间隔件还包括中心轴杆元件,该中心轴杆元件沿竖直方向将铰接元件连接在一起。该中心轴杆元件可以由与上部端板、下部端板、第一中间部件、第二中间部件或第一和第二铰接元件相同的材料体来制造。
另外,弹性体元件可以填充在上部和下部端板之间的间隙和空间,并用作负载分担部件。另外,间隔件还可以有至少一个椎骨附接装置,该椎骨附接装置与上部和下部端板中的至少一个操作相联,该椎骨附接装置包括从以下组中选择的至少一个,该组包括:龙骨、齿、脊、长钉和螺钉。另外,间隔件还可以有在上部和下部端板的其中一个中的至少一个孔,用于接收紧固部件。
本发明的第一实施例还可以包括一种用于治疗受损或患病的脊椎盘的方法,该方法包括以下步骤:(a)形成通向受损或患病脊椎盘的进入口;(b)除去受损或患病脊椎盘的至少一部分;(c)提供铰接间隔件,该铰接间隔件包括上部端板和下部端板以及至少一个铰接元件,该铰接元件有:多个区段,这些区段由至少一个空间分开;以及至少一个桥接元件,该桥接元件连接这些区段,铰接元件包括置入最外侧区段内部的最内侧区段,其中,铰接元件的第一部分可以与上部和下部端板中的一个操作相联,铰接元件的第二不同部分可以与上部和下部端板中的另一个操作相联,以便使得上部和下部端板能够通过铰接元件的弯曲而相对运动,而没有相互摩擦的相对表面;以及(d)将间隔件植入椎骨之间。
本发明的第二实施例还包括一种用于治疗受损或患病的脊椎盘的方法,该方法包括以下步骤:(a)形成通向受损或患病脊椎盘的进入口;(b)除去受损或患病脊椎盘的至少一部分;(c)提供用于附接在椎骨上的至少两个端板,各端板有凹口;(d)将各端板植入相邻椎骨中;(e)提供芯组件,该芯组件包括第一和第二中间部件以及至少一个铰接元件,该铰接元件有:多个区段,这些区段由至少一个空间分开;以及至少一个桥接元件,该桥接元件连接这些区段,铰接元件包括置入最外侧区段内部的最内侧区段,其中,铰接元件的第一部分可以与第一和第二中间部件中的一个操作相联,铰接元件的第二不同部分可以与第一和第二中间部件中的另一个操作相联,以便使得中间部件能够通过铰接元件的弯曲而相对运动,而没有相互摩擦的相对表面;以及(f)将芯元件植入端板中。
治疗受损或患病的脊椎盘的方法还可以包括通过使得第一中间部件滑入一个端板的凹口内和使得第二中间部件滑入另一端板的凹口内而插入芯组件。该方法还可以包括使得芯组件与端板固定连接。

下面的说明书中使用的某些术语只是为了方便,而不是限定。词语“右”、“左”、“下侧”、“上侧”、“顶部”和“底部”表示所参考的图中的方向。词语“向内”或“远侧”和“向外”或“近侧”是指分别朝向和远离椎间间隔件装置和相关部件的几何中心的方向,且并不是限制。词语“前面”、“后面”、“上面”、“下面”、“侧面”、“中间”和相关词语和/或短语是指所参考的人体的优选位置和方位,而不是进行限制。术语包括上述词语、衍生词和类似意思的词。
图1-4表示了铰接椎间间隔件装置100的第一优选实施例。间隔件100优选地用于脊柱中,并可以用于脊柱的颈部、胸部和/或腰部区域。尽管间隔件100被描述为大致用于脊柱,但是本领域技术人员应当知道,间隔件100可以有其它用途,并可以用作其它关节的假体,其它关节例如肩、肘、腕、臀、膝盖、踝、脚趾和手指。图14表示了间隔件装置500的优选用途,其植入脊柱中,在相邻的上部椎骨25和下部椎骨50之间。尽管图14表示了间隔件500,但是任意间隔件100、500、700、800和900可以优选地以这样的方式用于相邻椎骨之间。而且,大致在整个申请中,间隔件的各个实施例可以以参考标号100、500、700、800和900来表示。
第一优选实施例的间隔件100优选地允许有至少六个运动自由度,且在运动部分之间摩擦最小或没有摩擦。这六个运动自由度包括屈曲、伸展、横向弯曲、轴向旋转、水平移动和轴向阻尼以及它们的组合。在摩擦很小或没有摩擦的情况下进行相对运动而提供的一个优点是摩擦部件几乎不产生或不产生磨损,因此在间隔件100的操作过程中几乎不产生或不产生碎屑或外来材料。
在脊柱中,屈曲是当脊柱沿前部方向进行向前弯曲的动作时,例如当人向前弯曲以便看他的脚趾时。伸展是当脊柱沿后部方向进行向后弯曲的动作时,例如当人向后弯曲以便看天空时。轴向旋转是当脊柱绕它的竖直轴线进行旋转运动时,例如当人绕脊柱的竖直轴线扭转他们的身体时。横向弯曲是当脊柱沿侧向,即沿中间或横向方向(正交于前或后方向)进行弯曲的动作时。水平移动是当脊柱中的椎骨的旋转轴线横向移动时。水平移动可以沿前后方向、沿中间和横向方向以及沿它们的组合来进行。
轴向压缩在脊柱沿竖直方向负载(该负载压缩脊柱中的脊椎盘)时进行。轴向压缩在人与地面连接时进行,例如行走、奔跑、跳跃等。脊椎盘吸收它们的轴向压缩力,这称为轴向阻尼。
当软骨脊椎盘受损时,或者在患病的脊椎盘的情况下,外科医生可能进行局部或完全椎间盘切除术,并植入间隔件100,以便优选地模拟天然、健康的脊椎盘的运动类型和程度以及阻尼能力。如图1-4中所示,间隔件100包括上部板101、下部板103和铰接元件102,其中,铰接元件102位于上部板101和下部板103之间。间隔件100设计成这样,上部板101在原位相对于下部板103运动,优选地有很小摩擦或没有摩擦。优选地,间隔件100没有彼此一起摩擦或沿着彼此运动的元件,因此几乎没有或没有由于部件的运动而产生的摩擦,且优选地几乎不产生或不产生磨损碎屑。上部板101和下部板103通过铰接元件102而相互操作关联,优选地设置为形成关节假体,以便局部或完全代替在两个相邻椎骨之间的椎间盘。上部板101优选地并不与下部板103接触或碰撞,而是上部板101通过铰接元件102而与下部板103分离。尽管上部板和下部板101、103相对彼此的位置运动,但是它们与铰接元件102的部分操作相联,优选地固定附接在该部分上,从而产生铰接的、大致无摩擦或几乎无摩擦的本体。由于铰接元件102的柔性和能够弹性偏转,因此间隔件100的上部和下部板101、103能够相对彼此运动。
间隔件100的上部和下部板101、103优选地设置为端板,优选地有外表面101a、103a,该外表面101a、103a设置成与位于脊柱中的相邻上部和下部椎骨操作相联,优选地附接在该上部和下部椎骨上。外表面101a、103a可以相对平滑。也可选择或者另外,外表面101a、103a可以粗糙,或者它们的一部分粗糙,例如有脊101c、103c,以便于接合和附接在椎骨上。另外和/或也可选择,外表面101a、103a可以有椎骨附接元件,例如龙骨、齿、脊、用于骨向内生长的多孔结构、或者其它目前已知或以后发现的附接装置。
上部和下部板101、103可以有外表面,该外表面弯曲、相对较平和/或倾斜,以便帮助保持脊柱中的椎骨的曲率和/或对齐,上部和下部板101、103的所需形状(如相对较平、弯曲或倾斜)将取决于间隔件100在脊柱中的位置和脊柱的状态。间隔件100的上部和下部板101、103可以有外周边106、107,该外周边106、107可以为肾形,或者为大致“D”形,如图所示,或者可以采取其它示例形状,例如圆形、矩形、八边形、三角形、椭圆形等。外表面101a、103a优选地成形为与椎骨(间隔件100在植入结构中安装在该椎骨之间)的端板的大部分接触。
间隔件100的上部和下部板101、103优选地还有内表面101b、103b,它们优选地可以相对较平和/或具有相对较平的一个或多个部分101d、103d。上部和下部板101、103可以为任意合适的厚度。优选地,铰接元件102与上部和下部板101、103的内表面101b、103b操作相联,更优选地固定附接在该内表面101b、103b上,优选地连接在该内表面101b、103b上。
间隔件100优选地具有外周边104,该外周边104可以为任意形状,包括但不局限于:圆形、矩形、椭圆形、三角形、八边形、“D”形或肾形。间隔件100的外周边104可以分别由上部和下部板101、103的外周边106、107来限定。间隔件100的尺寸可以设置成提供用于特定用途所需的最佳尺寸和特性。
在优选地用于脊柱的腰部区域的第一优选实施例中,间隔件100为“D”形或肾形,具有宽度Wt,该宽度Wt优选地在从大约20mm至大约30mm的范围内,更优选地在大约24mm至大约26mm的范围内,更优选地大约25mm。第一实施例的间隔件100优选地有长度Lt,该长度Lt在从大约17mm至大约24mm的范围内,更优选地在大约19mm至大约21mm的范围内,更优选地大约20mm。间隔件100优选地具有高度Ht,该高度Ht在从大约3mm至大约7mm的范围内,更优选地在大约5mm至大约6mm的范围内。也可以考虑其他的尺寸用于间隔件,包括但不局限于间隔件100的长度、宽度、深度和高度尺寸。
在优选地用于脊柱的颈部区域的优选实施例中,间隔件100的宽度Wt可以优选地在从大约15mm至大约19mm的范围内,它的长度Lt可以在从大约12mm至大约18mm的范围内。也可以考虑其他的尺寸用于间隔件,包括但不局限于上述间隔件100的长度、宽度、深度和高度尺寸。
铰接元件102优选地包括一个或多个区段202,优选地为置入的区段,优选地同心区段,它们由空间或间隙201分开,并通过一个或多个桥接元件203连接。优选地,区段202有增大或减小的内周和外周(例如外部区段205a、205b的内周和外周),从而使得区段202能够相互之间插入配合。区段202优选地由位于区段202之间的空间或间隙201分开,优选地使得区段202能够相对运动。桥接元件203优选地使得一个或多个区段202相互连接,并优选地连接相邻区段202。桥接元件203还可以与上部或下部板101、103连接。空间201优选地由区段202和桥接元件203来限定和界定。区段202优选地布置和设置成通过桥接元件203而柔性连接,该桥接元件203优选地为柔性和可弹性偏转,以便允许区段202相对彼此运动。此外,区段202自身可以为柔性和可弹性偏转。铰接元件102优选地形成柔性芯组件。
在第一优选实施例中,铰接元件102由钴-铬合金构成,而上部和下部板101、103由钛或可生物相容的钛合金构成。在另一实施例中,铰接元件102可以由与上部和下部板101、103相同的材料构成。铰接元件102和/或上部和下部板101、103可以由任意可生物相容的金属、金属合金、聚合物、聚合物的混合物、镍钛诺、形状记忆合金、陶瓷或复合材料来制造。区段202可以由与桥接元件203不同的材料来构成。
在第一优选实施例中,5个同心区段202a-e由四个桥接元件203a-d连接。区段202为连续封闭形状,即区段202形成封闭空间,优选地区段202成形为封闭的环或O形环。不过,区段202可以为任意形状,包括但不局限于:圆形、矩形、椭圆形、“C”形、马蹄形、三角形、八边形或肾形形状。在图1-4的实施例中的铰接元件102具有外部区段202a、中间区段202b-d和内部区段202e。中间空间201a-d环绕中间区段202b-d,内部或中心空间211位于内部区段202e的内部。区段202、空间201和桥接元件203的数目可以变化,且并不局限于特定数目、值或范围。尽管图2表示了具有封闭同心区段202的实施例,但是区段202也可以为“开放”的,例如形成C形区段、马蹄形或者任意其它合适形状。
下部板103的内表面103b优选地具有凹入部分105,用于接收和容纳铰接元件102。铰接元件102可以与下部板103的壁105b操作相联,优选地附接在该壁105b上,优选地固定附接在该壁105b上。铰接元件可以压配合、焊接、电子束焊接、粘接或者紧固在下部板103上,可以使用任意其它目前已知或以后发现的连接机构或装置将铰接元件与下部板103连接。更具体地说,铰接元件102的外部区段202a的外表面102a的下部部分102b可以优选地与凹入部分105的壁105b充分接触。优选地,铰接元件102的外部区段202a的外表面102a的下部部分102b可以附接在凹入部分105中,以便在铰接元件102的底部表面111和下部板103的底部表面110之间留有空间或间隙109,使得铰接元件102的区段202优选地浮动在凹入部分105的底部表面110上面。
铰接元件102也可以与上部板101操作相联,优选地附接在该上部板101上。区段202a-e或桥接元件203a-d中至少一个的顶表面可以优选地附接在上部板101的内表面101b上,优选地固定附接在其上。铰接元件可以压配合、焊接、电子束焊接、粘接或紧固在上部板101上,且可以使用任意其它目前已知或以后发现的连接机构或装置将铰接元件102与上部板101连接。优选地,铰接元件102的内部区段202e可以附接在上部板101的内表面101b上,优选地固定附接在内表面101b上。在一个实施例中,内部区段202e的上表面202f优选地通过焊接而固定附接在上部板101上。在可选实施例中,可选的销元件108可以从上部板101的内表面101b伸向下部板103,并伸入铰接元件102的内部空间211中。另外和/或也可选择,销元件108可以位于相邻区段202之间的空间201中。销元件108附接在铰接元件102上,优选地以允许力从相邻椎骨传递给上部和下部板101、103以及铰接元件102的方式来附接。可选的销元件108可以压配合到内部空间211中,以便将铰接元件102固定附接在上部板101上。可以使用如上所述的其它附接和连接装置来使得销元件108与内部区段202c连接。
另外和/或也可选择,销元件108可以并不与铰接元件102连接,并可以松弛地装配至内部空间211或其它空间201中,这样,销元件108可以在内部空间211或其它空间201中运动。当销元件108在内部空间201c中运动并与内部区段202c接触时,销可以从上部板101向铰接元件102传递力。
当力施加给上部和下部板101、103时,铰接元件102优选地以区段202的相对运动来进行响应。当施加力时,区段202和/或桥接元件203弹性偏转,并相对彼此运动。例如,当横向弯曲施加在上部和下部板101、103上时,在铰接元件102的、施加力的一侧的区段202的部分将运动得相互靠近,从而使它们之间的空间201变窄,在相对侧的区段202的部分将运动得彼此远离,从而使得它们之间的空间201变宽。区段202优选地相对彼此以及相对于上部和下部板101、103无摩擦地运动。
区段202优选地允许沿至少6个运动度运动,包括屈曲、伸展、横向弯曲、轴向旋转、水平移动和阻尼,如上所述。图4表示了间隔件100的剖视图,其中,上部和下部板101、103相对于铰接元件102运动,允许进行屈曲和伸展。上部和下部板101、103还可以相对于铰接元件102沿横向/中间方向(未示出)运动。区段202和桥接元件203也可以沿竖直轴线305提供相对于压缩力的阻尼。因此,当压缩力施加在上部和下部板101、103上时,区段202可以沿轴向方向平移,或者沿施加力的方向偏转。在铰接元件102下面的空间105允许区段202平移,优选地并不与凹入部分105的底部表面110接触。另外和/或也可选择,桥接元件203可以在施加力时偏转。区段202和桥接元件203响应压缩力而进行的偏转和平移将在间隔件100中产生阻尼效果。
当间隔件100设计用于脊柱的腰部区域中时,间隔件100优选地使得脊柱能够进行以下范围的运动:(i)在屈曲和伸展过程中大约+/-10度;(ii)在横向弯曲过程中大约+/-7度,以及(iii)在轴向旋转过程中大约+/-10度。
第一优选实施例的桥接元件203连接区段202,并可以允许和向区段202、上部和下部板101、103的相对运动提供阻力。对于区段202(和因此上部和下部板101、103)的相对运动的阻力可以通过改变区段202的数目、高度、材料、厚度(或宽度)、形状或其它特性而变化和进行控制。对于区段202(和因此上部和下部板101、103)的相对运动的阻力可以通过改变桥接元件203的数目、宽度、厚度、材料、形状或其它特性而变化和进行控制。对于区段202(和因此上部和底部端板101、103)的相对运动的阻力还可以通过增加连接两个区段202的桥接元件203的数目而变化和进行控制。多个桥接元件203可以用于连接相邻区段202,以便改变对于在区段202之间的相对运动的阻力。尽管间隔件100的第一优选实施例包括沿中间/横向轴线302(即与前后轴线301正交)对齐的桥接元件203,但是桥接元件203可以以任意合适的方式定位在铰接元件102的区段202之间。当以对齐方式定位时,如图所示,桥接元件203大致优选地对于施加的力提供对称响应,并可以沿对齐方向提供更坚硬的阻力。当希望非对称响应的用途时,桥接元件203的定位可以相应地变化。
铰接元件102优选地可以为圆形形状,如图2中所示,具有总半径R,该总半径R优选地在从大约7mm至大约19mm的范围内,更优选地从大约11mm至大约13mm,更优选地大约12mm。区段202优选地可以有宽度Ws,该宽度Ws优选地在从大约0.5mm至大约2mm的范围内,更优选地大约1mm。桥接元件203优选地可以有宽度Wb,该宽度Wb在从大约1mm至大约10mm的范围内,更优选地大约2mm至大约7mm,且它的厚度T在从大约0.1mm至大约1mm的范围内,更优选地大约0.4mm至大约0.6mm的范围内,更优选地大约0.5mm。空间201优选地可以有宽度Wp,该宽度Wp在从大约0.1mm至大约1mm范围内,更优选地大约0.4mm至大约0.6mm,更优选地0.5mm。区段202优选地可以有高度Hs,该高度Hs在从大约3mm至大约10mm的范围内,更优选地大约5mm至大约7mm。应当知道,这些尺寸只是示例,可以考虑其它尺寸用于铰接元件102、区段202、桥接元件203和空间201。
另外,铰接元件102和间隔件100的高度Hs可以在前侧与在后侧不同(即沿前/后轴线301)。铰接元件102和间隔件100的高度也可以在侧部与在中部不同(即沿中间/横向轴线302)。优选地,这可以通过使得区段202的某些部分比其它部分偏离得更高或更低来实现,或者通过使得特定区段202制造为具有更大高度来实现。这使得间隔件100能够特别适应病人(考虑到他们个人的脊柱前凸、个人的脊柱后凸以及治疗的脊柱的情况)。
铰接元件102可以由任意材料来制造,优选地允许有一些柔性的材料。在第一优选实施例中,铰接元件102由钴-铬合金构成。也可选择,铰接元件102可以由钛或可生物相容的钛合金、形状记忆材料例如镍钛诺、或者任意可生物相容的金属、金属合金、聚合物、聚合物的混合物、陶瓷、复合材料或者这些材料的组合来构成。
间隔件100优选地还可以有可选的硅酮、聚氨酯弹性体或其它合适填料,它们注入间隔件的空间中,以便提供负载支承和负载分担能力。也可选择和/或另外,夹子(未示出)优选地可以插入间隔件100中,以便限制或防止上部和下部板101、103和/或铰接元件102的运动。
限制的轴向旋转306优选地绕竖直中心轴线305沿两个方向产生,顺时针方向和逆时针方向。在区段202之间桥接元件203的存在以及它们的柔性限制和阻止了完全旋转的能力。铰接元件102还允许区段202进行有限的水平移动,因此允许上部和下部板101、103在水平平面中进行有限的水平移动,即沿前/后轴线301和中间/横向轴线302进行有限的水平移动或沿两个轴线的移动。当施加平移力时,在区段202之间存在桥接元件203以及存在用于接收铰接元件102的、凹入部分105的壁105b将允许和/或限制区段的水平移动。另外,在区段202之间的桥接元件203可以提供对于轴向旋转和水平移动的阻力,并允许同时进行该轴向旋转和水平移动。
在第一优选实施例中,间隔件100优选地有开口空间405a,优选地横向位于铰接元件102的侧部,且优选地在上部和下部板101、103之间,这样,当上部和下部板101、103植入病人的脊柱内并工作时,并不相互抵靠或接触,优选地即使当完全铰接运动、枢转、旋转或完全压缩时也是如此。优选地,开口空间405a可以通过使得区段202a-e中的每一个沿向上竖直轴线方向305偏离而实现,这样,最外侧的区段202a处于最低位置112,最内侧的区段202e处于最高位置113。相邻区段202的竖直偏离(如图4中所示)也优选地通过提供在区段202和上部板101之间的另一开口空间405b而允许上部板101绕铰接元件102进行枢转和铰接运动。另外,区段202沿竖直向上轴线方向305的偏离优选地可以产生在铰接元件102和下部板103之间的间隙109。该间隙109提供用于区段202的空间,以便沿轴向方向305运动,从而当压力施加在上部和下部板101、103上时提供阻尼特性。
也可选择,开口空间405b可以通过使得区段202a-c中的每一个沿竖直向下方向偏离而实现,这样,最外侧的区段202a处于最高位置,最内侧的区段202e处于最低位置。在另一可选结构中,各区段202的端部可以相对于它的相邻区段202交替地沿向上和向下竖直轴线方向(即沿竖直轴线305)偏离,例如形成之字形形状。另外,在之字形结构中的区段202的偏离优选地可以在铰接元件102和下部板103之间产生间隙105。该间隙105提供了用于区段202的空间,以便沿轴向方向305平移,从而当压力施加在上部和下部板101、103上时提供了阻尼特性。如上所述,铰接元件102沿任意方向运动的阻力和柔性可以通过改变各个区段202、空间201和桥接元件203的数目、宽度、高度、厚度、材料或其它特性而变化。
也可选择,代替区段202a-202c的偏离,区段202可以增加或减小高度Hs,从而提供用于上部和下部板101、103铰接的类似开口空间405b。

优选实施例的上部和下部板501、503还包括龙骨513,优选地用于提高上部和下部板501、503附接在相邻椎骨(未示出)上的稳定性。龙骨513可以由包括上部或下部板501、503的单件来制造,或者可以通过压配合、焊接、粘接、利用紧固件、或者任意其它目前已知或以后发现的连接机构或装置而附接在上部或下部板501、503上。另外和/或也可选择,上部和下部板501、503可以包括其它椎骨附接装置,例如齿、长钉、螺纹或其它目前已知或以后发现的附接机构或装置。另外,用于使得板501、503与仪器接合的孔515可以布置在板501、503的任意一个或两个上,用于平移、去除或以其它方式操作该间隔件500。间隔件500优选地还可以有硅酮、聚氨酯弹性体或其它合适填料,它们注入间隔件500的空间和间隙中,以便提供负载支承和负载分担能力。另外,夹子(未示出)优选地可以插入间隔件500中,以便限制上部和下部板501、503和/或铰接元件502的运动。
参考图7和图8,椎间间隔件700的第三优选实施例包括上部板701,该上部板701具有铰接元件702、销元件705、龙骨元件704和下部板703。销元件705附接在最内侧区段202e的一部分上、与最内侧区段202e的一部分连接和形成该最内侧区段202e的一部分,而桥接元件203使得最外侧区段202a与下部端板703连接。上部和下部板701、703优选地由单件材料制造。这例如可以通过选择性激光烧结(SLS)或选择性激光熔化(SLM)或者通过任意其它目前已知或以后发现的制造技术来实现。孔706可以布置在上部和下部板701、703中,以便能够除去制造技术固有的多余材料。第三优选实施例的间隔件700还包括铰接元件702,该铰接元件702允许板701、703相对运动,与第一和第二优选实施例的铰接元件102、502类似。间隔件700优选地还可以有硅酮、聚氨酯弹性体、聚碳酸酯聚氨酯(PCU)或任意其它合适填料,它们注入间隔件700的空间或间隙中,以便提供负载支承和负载分担能力。另外,夹子(未示出)优选地可以插入间隔件700中,以便限制上部和下部板701、703和/或铰接元件702的运动。
参考图9和图10,在第四优选实施例中,间隔件800包括形成为上部和下部中间部件804、805的上部和下部板801、803。铰接元件802与中间部件804、805操作相联,优选地附接在该中间部件804、805上,以便形成芯组件820。优选地,以上述第一优选实施例中将铰接元件102附接在上部和下部板101、103上的任意方法将铰接元件802固定地附接在上部和下部中间部件804、805上,这些方法包括压配合、焊接、粘接、使用紧固件,如图1-6中所示和所述,或者目前已知或以后发现的任意其它连接机构或装置,或者铰接元件802和中间部件804、805可以由单件材料整体制造。间隔件800还可以包括上部和下部端板801、803,并可以包括附加椎骨附接装置,例如龙骨810、长钉808、脊(例如见101c、103c),或者目前已知或以后发现的任意其它附接装置。在第四优选实施例中,上部和下部端板801、803还包括凹入部分806、807,用于接收中间部件804、805,该中间部件804、805在装配过程中或在处理过程中滑入凹入部分806、807中。上部和下部端板801、803还可以包括凸肩811,用于引导中间部件804、805进入凹入部分806、807中。为了避免中间部件804、805和铰接元件802退回或滑出凹入部分806、807,以及为了将中间部件804、805竖直固定在上部和下部板801、803上,紧固方式(例如压配合、焊接、粘接、使用紧固件、螺栓、螺钉、卡扣或目前已知或以后发现的任意连接机构或装置)优选地可以用于将中间部件804、805固定在上部和底部端板801、803上。由第四实施例提供的一个优点是可以产生“一套工具”,其中,各种芯元件820提供了不同高度、相对硬度和/或限制运动,它们可以根据病人的特定需要而装配在上部和下部端板中。此外,上部和下部端板801、803可以最先安装在椎骨V1、V2上,椎骨可以分散,中间部件804、805和铰接元件802可以在原处滑入端板801、802中。
第四优选实施例的间隔件800的运动和阻尼能力可以通过插入刚性插入件而限制或完全阻止,例如附加夹子(未示出),以便阻止和防止在中间部件804、805和铰接元件802之间相对运动。夹子(未示出)可以布置在铰接元件802或铰接元件802的一部分的周围,并阻止或防止中间部件804、805更靠拢和/或更远离。当脊柱的状态退化成必须使得椎骨V1、V2熔合时,优选的间隔件800可以阻止铰接运动。这样的夹子的插入可以通过相对较小的外科手术介入来进行。也可选择,当使用具有中间部件804、805的铰接元件802时,芯组件820可以除去,并由刚性插入件(未示出)代替,同时上部和下部端板801、803保持附接在椎骨V1、V2上。
为了最小化骨向内生长至椎间间隔件800内的危险,保护片(未示出)可以环绕椎间间隔件800。这样的保护片可以由可生物相容的弹性体或织物状结构或者任意其它目前已知或以后发现的合适材料来制造。间隔件800优选地还可以有硅酮、聚氨酯弹性体、聚碳酸酯聚氨酯(PCU)或任意其它合适填料,它们注入间隔件800的空间和间隙815中,以便提供负载支承和负载分担能力。
参考图11-13,在第四优选实施例中,间隔件900包括上部和下部板901、903、轴杆元件905和双铰接元件902a、902b。间隔件900可以选择还包括板插入件910a、910b和弹性元件920。上部和下部板901、903、双铰接元件902a、902b和轴杆元件905优选地可以由单件材料来制造。各铰接元件902a、902b的最外侧区段950优选地可以与上部和下部板901、903操作相联,或者制造成与上部和下部板901、903连接,如图中906a、906b所示。连接元件906a、906b可以为桥接元件。轴杆元件905优选地使得上部铰接元件902a与下部铰接元件902b连接。上部和下部板901、903优选地可以包括外表面901a、903a,该外表面901a、903a有凹入部分907,用于接收板插入件910a、910b。板插入件910a、910b优选地可以与上部和下部板901、903操作相联和连接。
各双铰接元件902a、902b优选地包括区段202、桥接元件203和在区段202之间的空间201。双铰接元件902a、902b优选地可以由开口空间904分开,该开口空间904提供了用于铰接元件902a、902b的空间,以便在双铰接元件902a、902b之间没有接触或者上部和下部板901、903之间没有接触的情况下运动。控制轴杆905优选地竖直连接铰接元件902a、902b。优选地,间隔件900提供了至少六个运动度,包括屈曲、伸展、横向弯曲、轴向旋转、水平移动和轴向阻尼,大致几乎没有摩擦或没有摩擦,如上所述。
优选地,间隔件900由可生物相容金属来构成,优选地,上部和下部板901、903、双铰接元件902a、902b和轴杆905由单块金属材料整体形成。形成间隔件900的一种优选方式是在材料块的上部和下部部分中形成凹入部分907。然后,通过机械加工形成区段202和桥接元件203以及轴杆905,可以在材料块中形成同心“C”形环形式的空间201。然后,位于中部并环绕材料块的周边的开口空间904通过机械加工而形成,以便形成间隔件900。由单块材料形成间隔件900的步骤可以变化,例如通过在“C”形空间201之前形成空间904。空间201可以从块的一侧形成,并形成为使得上部铰接元件902a与下部铰接元件902b相同。也可选择,上部铰接元件902a可以形成为与下部铰接元件902b不同,且在一个实施例中可以通过从基体块的不同侧以不同模式产生空间201而形成。
弹性体元件920可以与上部和下部板901、903操作相联,并优选地附接在上部和下部板901、903之间。优选地,弹性体元件920(例如硅酮、聚氨酯弹性体、聚碳酸酯聚氨酯或任意其它合适的可生物相容材料)注入间隔件900的芯中和环绕间隔件900的芯注入和/或注入空间904中。弹性体元件920优选地填充在上部和下部板901、903、双铰接元件902a、902b和任意其它合适区域之间的一些或全部空隙。优选地,弹性体元件920是椎间间隔件900的负载分担部件,提供了屈曲、伸展、横向弯曲、轴向旋转、水平移动和轴向压缩力的某些吸收。弹性体元件920可以由硅酮基的材料、聚碳酸酯-聚氨酯或任意合适的可生物相容弹性体材料来构成。第五优选实施例的间隔件900优选地可以有与第一、第二、第三和第四优选实施例中所述的元件相同的运动和阻力特性。
如本领域技术人员已知,这里所述的任意或全部部件可以成组或成套提供,这样,外科医生可以选择部件的各种组合,以便形成植入体和产生专门设置成用于病人的特殊需要/解剖结构的椎间盘替换系统。应当知道,各部件中的一个或多个可以成组或成套提供。在一些成套或成组工具中,相同部件或元件可以以不同形状和/或尺寸来提供。外科医生或工作人员可以将这些部件混合和匹配,以便在处理之前或处理过程中产生间隔件100、500、700、800、900。
用于植入间隔件100、500、700、800、900或其它间隔件的优选外科手术技术包括以下步骤:(1)露出相邻椎骨V1、V2的相邻椎骨本体和椎间盘;(2)清理椎间空间,从椎骨本体之间除去椎间盘组织和软骨碎片;(3)可选地通过撑开器而使得椎间空间扩散,以便恢复椎间空间的高度;(4)可选地,插入试验植入体,以便确定最终尺寸、高度和脊柱前凸或脊柱后凸角度以及要插入的间隔件的位置;(5)可选地,进行凿或铣,以便产生用于间隔件的龙骨床、槽或槽道,如果需要时或其他情况下准备椎骨的端板,以便接收间隔件;(6)将间隔件插入在椎骨V1、V2之间的制备区域中;以及(7)封闭切口。
另一优选的外科手术技术大致包括以下步骤:(1)露出相邻椎骨V1、V2的相邻椎骨本体和椎间盘;(2)清理椎间空间,从椎骨本体之间除去椎间盘组织和软骨碎片;(3)可选地通过撑开器而使得椎间空间扩散,以便恢复椎间空间的高度;(4)可选地插入试验植入体,以便确定最终尺寸、高度或脊柱前凸或脊柱后凸角度以及要插入的间隔件的位置;(5)可选地进行凿,以便产生用于间隔件的龙骨床、槽或槽道,如果需要时或在其他情况下准备椎骨V1、V2的端板,以便接收间隔件;(6)插入端板;(7)通过将铰接元件滑动到上部板和下部板凹部中而插入铰接元件;以及(8)封闭切口。
本申请的优选间隔件100、500、700、800、900在除去椎间盘或除去椎间盘的一部分之后大致安装在两个相邻椎骨V1、V2之间,如图14中所示。不过,间隔件100、500、700、800、900并不局限于安装在两个相邻椎骨V1、V2之间,而是可以设计和设置成安装在两个椎骨V1、V2(其中,在它们之间的椎骨或椎骨本体已经除去)之间。另外,当间隔件100、500、700、800、900描述为大致用于脊柱,但是本领域技术人员应当知道,间隔件100、500、700、800、900可以有其它用途,并可以用作其它关节的假体,例如肩、肘、腕、臀、膝盖、踝、脚、脚趾和手指。间隔件还可以用于非医疗用途。
尽管前面的说明书和附图表示了本发明的优选实施例,但是应当知道,在不脱离由附加权利要求确定的本发明精神和范围的情况下可以进行各种添加、变化、组合和/或代替。特别是,本领域技术人员应当知道,本发明可以以其它特定形式、结构、布置、比例或其它元件、材料和组分来实施,而并不脱离本发明的精神和基本特征。本领域技术人员应当知道,在本发明的实施中,本发明可以用于多种变化的结构、布置、比例、材料和组分,它们特别适合特定环境和操作需要,而并不脱离本发明的原理。此外,这里所述的特征可以单独使用或与其它特征组合使用。因此,这里所述的实施例在所有的方式是作为示例说明,而不是进行限制,本发明的范围由附加权利要求来确定,而不限于前述说明。
Claims (28)
1.一种用于放置在上部椎骨和下部椎骨之间的椎间间隔件,所述椎间间隔件包括:
上部端板,所述上部端板具有外表面,用于与所述上部椎骨和所述下部椎骨中的一个接触;
下部端板,所述下部端板具有外表面,用于与所述上部椎骨和所述下部椎骨中的另一个接触;
至少一个铰接元件,所述铰接元件具有由至少一个空间分开的多个区段以及连接相邻区段的至少一个桥接元件,所述铰接元件具有被置入最外侧区段内部的最内侧区段;
其中,所述铰接元件的第一部分与所述上部端板和下部端板中的一个操作相联,所述铰接元件的第二不同部分与所述上部端板和所述下部端板中的另一个操作相联,以便通过铰接元件的弯曲允许上部端板和下部端板进行相对运动,而没有相互摩擦的相对表面。
2.根据权利要求1所述的间隔件,其中:被置入的区段是同心O形环。
3.根据权利要求1所述的间隔件,其中:当上部端板和下部端板相对彼此运动时,被置入的区段相对彼此弹性偏转。
4.根据权利要求1所述的间隔件,其中:当上部端板和下部端板相对彼此运动时,所述桥接元件弹性偏转。
5.根据权利要求1所述的间隔件,其中:所述铰接元件还包括至少一个中间区段,所述中间区段置入最内侧区段和最外侧区段之间。
6.根据权利要求1所述的间隔件,其中:各区段的高度在从大约3mm至大约10mm的范围内。
7.根据权利要求1所述的间隔件,其中:各区段的宽度在从大约0.5mm至大约2mm的范围内。
8.根据权利要求1所述的间隔件,其中:各桥接元件的宽度在从大约1mm至大约10mm的范围内。
9.根据权利要求1所述的间隔件,其中:所述桥接元件的其中之一使得所述铰接元件与所述上部端板和下部端板中的一个连接。
10.根据权利要求1所述的间隔件,其中:所述最外侧区段固定地连接到所述上部端板和下部端板中的一个上,所述最内侧区段固定地连接到所述上部端板和下部端板中的另一个上。
11.根据权利要求10所述的间隔件,其中:所述区段通过以下组中的至少一种与它们的相应端板固定连接,所述组包括焊接、胶接、压配合和粘接。
12.根据权利要求1所述的间隔件,其中:所述铰接元件由以下组中的至少一个来构成,所述组包括可生物相容的金属、钛、钛合金、钴-铬合金、可生物相容的聚合物、可生物相容的聚合物的混合物、镍钛诺、形状记忆材料、陶瓷和复合材料。
13.根据权利要求1所述的间隔件,其中:所述间隔件具有竖直轴线,各相邻区段沿所述竖直轴线的高度从所述最外侧区段朝着所述最内侧区段增加。
14.根据权利要求1所述的间隔件,其中:所述间隔件具有竖直轴线,各相邻区段沿所述竖直轴线的高度从所述最外侧区段朝着所述最内侧区段减小。
15.根据权利要求1所述的间隔件,其中:所述间隔件具有竖直轴线,相邻区段的端部的定位沿所述竖直轴线上下交替,从而形成之字形结构。
16.根据权利要求1所述的间隔件,还包括:位于上部端板和下部端板的一个中的凹入部分,所述凹入部分用于接收所述铰接元件。
17.根据权利要求16所述的间隔件,其中:内壁限定所述凹入部分,所述内壁沿向外方向朝着内壁形成于其中的端板的周边成角度。
18.根据权利要求17所述的间隔件,其中:所述铰接元件的最外侧区段具有外表面,所述外表面形成从所述铰接元件的顶部到底部向内凸出的角度,所述凹入部分的向外成角度的内壁设置成接收并接触所述最外侧区段的向内凸出的外表面。
19.根据权利要求1所述的间隔件,还包括:位于所述铰接元件和其中一个端板之间的间隙区域,以允许所述区段运动至所述间隙区域中。
20.根据权利要求1所述的间隔件,还包括:销元件,所述销元件与所述端板中的至少一个操作相联,当所述间隔件处于装配结构时容纳在将所述铰接元件的相邻区段分开的至少一个空间内。
21.根据权利要求20所述的间隔件,其中:所述销元件松弛地接收在将所述铰接元件的被置入的区段分开的至少一个空间内。
22.根据权利要求20所述的间隔件,其中:所述销元件与一个端板固定地连接并朝着另一个端板延伸,且被压配合到将所述铰接元件的区段分开的至少一个空间内。
23.根据权利要求1所述的间隔件,还包括:形成于所述上部端板和下部端板的至少一个中的至少一个孔。
24.根据权利要求1所述的间隔件,其中:所述上部端板、下部端板和铰接元件由单个材料体整体制造。
25.根据权利要求1所述的间隔件,还包括:第二铰接元件,所述第二铰接元件具有:多个区段,各区段由至少一个空间分开;以及至少一个桥接元件,所述桥接元件连接相邻区段。
26.根据权利要求25所述的间隔件,还包括:中心轴杆元件,所述中心轴杆元件将铰接元件竖直连接在一起。
27.根据权利要求26所述的间隔件,其中:所述中心轴杆元件由与上部端板、下部端板以及第一铰接元件和第二铰接元件相同的材料体制造。
28.根据权利要求1所述的间隔件,还包括:弹性体元件,所述弹性体元件填充在所述上部端板和下部端板之间的间隙和空间。
