Abstract

Description

流体交换导管系统
本发明涉及一种导管系统,其可用于向人的身体和/或动物组织输注流体(药物、水和营养物),而且并行地从人的身体和/或动物组织抽吸生物材料(血液、脓、病理性组织、有毒物质),而没有任何阻塞问题。
在临床或临床前装置中,有许多种用于流体输注和抽吸的导管。传统上,导管的插入生物材料中的尖端被称作“远端”而留在外部的尖端被称作“近端”。
大多数现有的导管具有单个管腔,用户可通过该管腔交替地输注或抽吸液体。
例如,在临床装置中,常见的静脉内导管或者抽吸血样(通常在其插入静脉之后立即进行)或者输注药物和/或营养物的溶液(通常在插入以后持续进行许多小时或许多天)。
这些导管可输注或抽吸大量液体,但是它们不能并行地进行以便与组织的细胞外液或病理性液体积聚物不断地交换药物和营养物。
由于监控和治疗两个方面的原因,需要进行并行流体交换。
有少数带有多个管腔的导管,其可并行地输注和抽吸液体。
例如,微量透析导管在进入人或动物组织之后,从连接至其近端尖端的泵被连续地灌注液体溶液。导管包括两个同心的管腔(在远端尖端由薄膜覆盖的管)。通常,中心管为导管的输出部分而外围管为导管的输入部分。部分所灌注的液体被通过导管的在其远端的薄膜输注至组织,并且通过同一薄膜以及输出管腔抽吸细胞外液。
但是,微量透析导管及与其类似的导管却被设计成用于进行组织监控,而上述并行的输注和抽吸过程以几微升/微小的流率范围并通过很小的薄膜小孔进行。
为了用于治疗应用场合,需要大得多的液体交换速率和具有大孔的薄膜或罩以便使得可以抽出像脓之类的低粘度的液体,那些低粘度的液体阻塞所有现有的导管。
用于生物流体的各种现有的导管的一个常见问题在于由于将生物材料塞入它们的管腔的尖端或其覆盖物中而造成它们阻塞。
例如,内治疗导管系统宣称具有所需的液体交换速率和通过运动部件进行的无阻塞操作。
它包括两个同心的管,一个输注、一个抽吸,在它们的近端尖端适当地连接至输注装置和抽吸装置,并且具有覆盖它们的远端尖端的过滤器或薄膜或格网或网罩,其包含用于并行输注和抽吸的流体力学地运动的装置。输注管适当地连接至用于灌注导管周围空间的运动装置,同时随着其运动推进通过另一管进行的抽吸。

本发明的流体交换导管系统包括两个或多个管腔-管(1、2),在它们的近端尖端适当地连接至输注装置(E)和抽吸装置(A),并且在它们的远端尖端(O)具有过滤器或薄膜或格网或网罩或者没有覆盖物。这些输注装置和抽吸装置周期性地和/或连续地改变系统中的液体压力梯度(不过保证流速满足基础病理学的输注和抽吸的需要,或者监控或治疗或研究规程的需要),以便产生洗干净导管的尖端并保持导管与组织之间的流体交换无阻碍地进行的流体流(B),而不需要任何运动部件。
系统中的压力差通过输注泵的任何正压力模式和相应地同步的抽吸泵的负压力模式产生(压力都是指组织周围的导管尖端处的压力)。
这种系统容许充分和安全可控的输注-抽吸速率和无阻碍的流体交换。
例如,在关于构造和工作方式的许多可能系统型式的一种中,在30秒的工作周期中,对蠕动泵(E)编程以便在抽吸管被阻塞(N)时,输注具有+200mmHg压力的液体5秒,然后停止10秒,并且对蠕动泵(A)编程以便在输注管被阻塞(N)时在接着的15秒期间利用-100mmHg压力抽吸。
可根据基础病理学或研究规程应用许多压力变化的模式。可监控在系统的端部处的这两种(输注和抽吸)压力以便保持同步进入预定范围和相位差,并且无论何时如果检测到任何系统功能障碍,可通过报警和自动停止(N)来保护这两种压力,这种检测通过适当地置于系统中的流和/或压力检测器装置(N)实现,以便获得安全性(以防过输注、过抽吸等等)。
替代地,系统的输注装置和抽吸装置可为流体容器(E、A),在导管的插入点处,简单地使用输注装置和抽吸装置相对于导管尖端的位置所产生的流体静压力作为所输注流体进入组织和所抽吸流体离开组织的动力。
对于这种型式的流体交换导管系统,可简单地只包括一个自动按钮(N),该自动按钮(N)经过编程以便压缩抽吸(和输注)管5秒,然后所抽吸(和输注)的流体自由流动5秒,或者编程为自由流动和阻塞流动的任何其它模式的时间间隔。
在流体交换导管系统任一点处的压力的同步变化的任何方式都在导管的输注和抽吸尖端通过所输注或抽吸流体的液柱直接传递。
所附的设计1和2表示流体交换导管系统的许多可能变型中的一部分。
流体交换导管具有任何构型的分叉部件,以便分隔两个不同管腔中的两个相反的流。
外管腔(保持着交换表面的管,该交换表面可为过滤器或薄膜或格网或网罩或者什么也没有)的远端就是抽吸管腔的开口尖端。
流体可为从蒸馏水到带有药物的营养液,其通过输注装置(E)供应至内管腔(管(1)),并且到达导管的远端(O),在该处在所输注流体与周围组织的细胞外液中包含的物质发生物质交换。流体混合物返回至抽吸装置或收集罐(A)。箭头表示压力梯度。
为了除去积聚在交换表面上并因此阻塞导管的有机物质,接收从内管腔的孔供应的流体射流对着液体交换表面的内壁周期性地散布,由此不会阻塞薄膜或丝网或格网或过滤器覆盖物。当抽吸管腔的开口尖端就是交换表面时,来自输注管腔的射流不会阻塞抽吸管腔。
导管系统的结构材料应当符合临床和实验室导管现有的规范和规章,包括生物相容性问题等等。

Claims (1)
