我们以前提出动脉中的流型包括漩涡型,其由支架的非平面几何形状引起、并能够抑制如血栓症、动脉粥样硬化和内膜增生这样的血管病的发展。
在WO 98/53764中,公开了一种用于血管支撑部分的支架。该支架包括支撑部分,欲用于移植的血管的一部分被放置在其周围或其内部,以使支架在内部或在外部支撑血管的所述部分。支架的支撑部分被形成为能使在移植和宿主脉管之间的流动遵循非平面曲线。这产生旋流,提供有利的血流速度模式,从而减小血管病尤其是内膜增生的发生。
在WO 00/32241中,公开了另一种支架,在这种情况下包括支撑部分,完整的血管而不是移植的一部分被放置在其周围或其内部。这种支撑部分可以防止由于堵塞、弯折或萎陷而引起的血管故障。此外,支架的支撑部分具有能使脉管中的流动遵循非平面曲线的形状和/或方位。有利的血流速度模式可以通过在支架内部和外部的旋流的产生而实现。由疾病如血栓症、动脉粥样硬化以及内膜增生导致的血管故障可以被大大减少。
上述公专利还进一步解释了旋流的其它有益效果。Caro等在(1998)J.Physiol.513P,2P中进一步解释了管的非平面几何形状如何抑制流动不稳定性。
我们现在已经找到一种制造内部支架的方式,其有利于在由支架支撑的流体导管内的流遵循非平面曲线,即涡旋形流动。
根据本发明的第一个方面提供了一种用于插入于人或动物体中的流体导管的支架,包括具有大致周向延伸的部分的空心结构,其中所述大致周向延伸的部分具有纵向的相对端部分,所述大致周向延伸的部分沿着所述空心结构的长度方向被接合到纵向相邻的周向部分,其中,相邻的周向部分的端部分不在平行的平面内,以及/或同一周向部分的端部分不在平行的平面内,并且其中不平行的端部分沿着所述空心结构的长度方向设置,以使得所述空心结构限定流腔,并且当在所述流体导管中被所述空心结构施加影响时,流体导管遵循非平面曲线。
在使用中,当支架在流体导管中处于扩张态时,将限定并影响其中的非平面的流腔。在由支架支撑的流体导管中的流体将遵循非平面曲线,从而促进旋流,其优点在上面被讨论过。因此,考虑到导管的流腔,随着其在纵向(x轴)延伸,其以不止一个平面(即y轴和z轴)的方式弯曲。也就是说,流腔在纵向上一般以螺旋形地延伸。
公知地,支架被设计成易曲的并且采用被其插入的脉管的流腔曲率。这些支架在插入到自然的非平面脉管时,有可能会被导致自身采用非平面形状。然而,与此相反,本发明中的支架被预先定形以表现非平面曲线。一旦支架被装入流体导管中,支架对导管施加影响,并且预先定形的非平面曲线由导管壁、周围组织以及支架的空心结构的各自的刚度引起的变化最小。支架的自然形状(即没有外应力,如在部署装置中保持支架的应力)使得提供非平面流腔。
优选地,支架的中心线在扩张态时遵循基本为螺旋的路径。也就是说,通过支架的相邻交叉部分(slice)的质心限定螺旋轨迹或中心线。
本发明可以被应用于被置于完整的血管或要移植的血管的内部的支架。
当支架处在扩张态时,随着其向纵向延伸,使流体导管遵循非平面曲线。优选地,曲线至少为一圈。通过优选的支架能够在没有实际的扭曲的情况下(即,支架的一个末端相对于另一端没有大的旋转)从萎陷态扩张到扩张态,可以提供超过一个的圈,更优选地为多圈。这可以通过当支架处在萎陷态和扩张态具有相同数目圈的支架的空心结构来实现。支架的这种特性意味着可以扩张而不引起导管扭曲——由于导管在人或动物体内的束缚这是不希望的。
空心体的周向扩张部分在某些实施例中可以是环形的(endless)管状部分。通过使一连串的管状部分端到端地排成一排,可以限定支架的空心结构。
每一管状部分具有长侧和与长侧直径相对的短侧,其中每一管状部分的长侧旋转地偏移于下一纵向相邻的管状部分的长侧。这样,管状部分可以产生非平面的流腔。空心结构的中心线可以遵循基本为螺旋的路径,即便是每一独立管状部分的中心线具有直的中心线。在这些实施例中,同一管状部分的末端不在平行的平面中。从侧面看,管状部分一般是“楔(wedge)”形。在一个实施例中,管状部分和下一管状部分的旋转偏移是10°,所以在36个管状部分后管腔的螺旋经历了一个完全的360°旋转。
管状部分可以被直接相互连接,在它们之间没有纵向间隔。优选地,管状部分彼此纵向间隔并通过连接装置连接在一起。该连接装置可以采用配置成绕着空心结构的周向设置的多个支杆的形式。
连接装置可以绕着空心结构的周向具有相同的长度,以例如互连的上述的楔形管状部分。作为一种选择,连接装置可以具有在空心结构的一侧具有长的长度,并在另一侧——空心结构直径相对的一侧——具有短的长度。每一连接装置的长的长度优选地与下一纵向相邻的连接装置的长的长度偏移。在这样的配置中,相邻周向部分的端部分不在平行的平面里,因为连接装置彼此相对“倾斜”。每一管状部分可自身使其端部分配置成互相平行。或作为一种选择,管状部分可以都为“楔”形,并且通过长的和短的长度的连接装置互相连接,各自“不平行”的特征优选地互相补充(compliment),以产生空心结构的非平面曲线。
如果连接装置为一组支杆的形式,支杆可以周向(即侧向地位于空心结构的表面)和纵向延伸。优选地,相互连接相邻的周向部分的一组支杆包括至少一个以第一侧向和纵向定位的支杆,以及至少一个以第二侧向和纵向定位的支杆。这有助于通过避免周向部分发生重大的相对扭曲,确保通过这组支杆来实现相邻周向部分之间的纵向间隔。
在某些实施例中,周向延伸部分可以是属于空心结构的周向和纵向延伸的单螺旋线圈的圈。因为支杆的空心结构提供非平面(即螺旋)流腔,这些实施例以“盘绕线圈”的形式起作用。单螺旋线圈形成空心结构的壁。螺旋线圈可以采用封闭的区域的“条”形式、或Z字形线或杆的形式。形成圆柱形结构的螺旋线圈支架是已知的。然而,在本发明中实施的螺旋线圈支架的一种形式中,螺旋线圈的宽度沿着线圈的长度方向变化。因此,在线圈的给定圈的纵向上相对的端部分(例如条形线圈的相对的边缘)将不在平行的平面里。
螺旋线圈的圈可以被彼此纵向间隔并通过连接装置连接在一起。连接装置可以绕着圈的周向具有相同的长度,以例如互相连接上述具有变化宽度的螺旋线圈。或者作为一种选择,连接装置可以在空心结构的一侧具有长的长度,并在另一侧——空心结构的直径上对置的一侧——具有短的长度。每一连接装置的长的长度优选地与下一纵向相邻的连接装置的长的长度偏移。在这样的配置中,螺旋线圈的相邻圈的端部分不在平行的平面里,因为连接装置将相邻的圈彼此相对“倾斜”。螺旋线圈可以具有固定宽度,以使相对的端部分(例如条形线圈的相对的边缘)将在平行的平面里。或者作为一种选择,螺旋线圈可以都具有变化的宽度,并且通过长和短的连接装置互相连接,各自“不平行”的特征优选地互补以产生空心结构的非平面曲线。
在支架的空心结构中通常优选地为避免任何显著的沟槽、隆起、肋条或叶片,因为它们可能会有与提高流动特性的要求的效应相反的效应,即,它们会阻塞流动,促进沉淀物积累或产生迟滞区。因此,优选地,支架基本上没有肋或叶片,例如没有较厚的线(与相邻的线相比)——否则其可以像肋条一样凸入到流体导管的流腔中。在提供连接装置如支杆的位置处,连接装置可以从周向部分径向向外地移位,以辅助锚定流体导管。
支架改善了流动特性。众所周知,在直管的情况下,由于粘度效应,与管中心处的速度相比,靠近壁的速度非常慢。当管在单一平面内被弯曲的情况下,在弯曲的外侧的流速增加但内侧的流速更加迟缓。在这两种情况下,由惯性控制的流动如动脉中,跨越管的宽度的轴向速度有相当大的变化。使用非平面流腔后,产生了旋流并且跨越流体导管的流动的轴向速度分布通常变得更一致或“平缓”,流动的轴向速度在流体导管的外侧和内侧都更接近平均轴向速度。
因此,流动特性被通过引起涡旋以及轴向和近壁速度的相对一致的分布而改善。剖面上的混合(mixing)也被改善,并且流动不稳定发生的可能性下降。有助于停滞区的避免和冲洗。在移植物下游沉淀物的积累和病变的潜在被下降。
如果需要的话,螺旋中心线的幅度和螺距可以沿着支架的长度方向被选择地改变。幅度的变化可以通过增加或减少非平行平面之间的角度来实现,同时螺距的改变可以通过改变相邻非平行平面对之间的旋转偏移来实现。如果希望在支架的上游末端引入缓和的漩涡并在下游方向增加涡旋效应,则这样的改变例如是期望的。
支架可以是自扩张式或球囊扩张式。
支架的基本几何形状可以是许多可得到的形状,如管状支架,螺旋线圈支架,编织支架,连续环状支架,闭腔连续环状支架,以及开腔支架。这些支架可通过盘绕、编织或接合线,通过激光从管料切割,通过电火花铣削加工(EDM),通过化学蚀刻或通过其他已知方法来制造。它们可以通过多种材料制造,包括不锈钢,镍钛合金,钽,铂,铌合金,钴合金或聚合物(如生物降解聚合物)。
在某些方面,本发明涉及用于插入到人或动物体内的易曲的导管中的支架,其中流体导管的流腔中的螺旋中心线具有相对小的幅度。
优选地,由空心体限定的流腔具有基本遵循螺旋的路径的中心线,螺旋中心线具有小于或等于65°的螺旋角,并且小于或等于流腔内径一半的幅度。
在本说明书中,螺旋的幅度表示从平均位置到侧向极限的间隔的程度。所以,当流腔具有螺旋中心线时,幅度是螺旋中心线的全侧宽度的一半。
在螺旋的幅度小于或等于流腔内径的一半的流腔中,具有沿着流腔的“瞄准线(line of sight)”,这和螺旋围绕核心(固态的,或具有空气核的“虚”的)缠绕的螺旋形配置的情况不同。我们已经发现在瞄准线处的流体一般有涡旋成分,即使其可能遵循直的路径。
为了说明的目的,螺旋流腔的“相对幅度”这个词被视为被内径分割的幅度。所以,在流腔的幅度小于或等于流腔内径的一半的流腔中,这意味着相对幅度小于或等于0.5。在一些情况里相对幅度小于或等于0.45,0.4,0.35,0.3,0.25,0.2,0.15或0.1是优选的。相对幅度至少为0.05也是优选的,更优选地为0.1。这有助于确保引起期望的旋流。
相对幅度可以根据支架的用途和其设计上的空间限制而变化。然而应当理解,通过保持幅度小于流腔内径的一半,可以引起旋流而无需额外的大装置。被支架的导管占据的“包层(envelope)”可以符合流体导管周围组织中的可用空间,并且即使上述包层被导管放置的局部环境导致遵循特定路径,也可以保持流腔的期望螺旋几何形状。
希望导管可以阻止支架扩张到其全尺寸。所以,虽然支架可以被设计成具有相对幅度大于0.5(如0.6或0.7),但在使用上流腔的相对幅度等于或小于0.5。在某些优选的配置中,扩张的支架在体外(exvivo)的相对幅度小于或等于0.5。
螺旋角还是平衡流管的空间限制的相关因素(期望用于流动的截面最大)。螺旋角小于或等于65°,优选地小于或等于55°,45°,35°,25°,20°,15°,10°或5°。因为具有相对幅度,螺旋角可以根据粘度,密度和流动速度等条件而被最优化。
一般地,雷诺数越高,可以使用越小的螺旋角来获得满意的旋流,同时,雷诺数越低,将需要越高的螺旋角产生满意的旋流。更高的螺旋角的使用通常是不希望的,因为可能存在接近迟滞流的壁穴(wallpockets)。因此,对给定的雷诺数(或雷诺数的范围),螺旋角将优选地选择尽可能低以产生满意的旋流。与等价的圆柱形的支架相比,更低的螺旋角将在长度上导致较小的增加。在某些实施例里,螺旋角小于20°或小于15°。
应当理解在脉动流中,雷诺数将在一个范围内变化。典型的平均静止的动脉血流,雷诺数约为100到200,在搏动血流中达到2或3倍的峰值,并且在发挥作用的期间为平均的3到4倍。因此旋流被促进的程度也将改变。即使在更低的雷诺数时具有迟滞流区域,由于选择了例如低的螺旋角和/或低的相对幅度,所以当雷诺数较高时在流动期间这些区域将倾向于被冲洗。
为了制造简单,支架优选地具有沿其长度基本恒定的剖面区域。在使用中会有支架载荷引起的变化。
支架的非平面部分可以仅沿着支架全长的部分长度延伸、或延其整个长度充分延伸。例如,支架可以定在其部分长度或在基本整个长度上限定非平面流腔。支架的其他特性也可以随支架的长度而变化,如像硬度这样的结构特性。
支架可以经历一个完整圈的小部分,例如一圈的四分之一、一半或四分之三。优选地,支架经历至少一圈,更优选地至少多圈。沿着支架的螺旋的重复的圈将倾向于确保旋流被产生和保持。
支架通常可以直线延伸。也就是说,支架的中心线绕其遵循基本为螺旋的路径的轴可以是直的。或者作为一种选择,轴本身可以是弯曲的,由此被支架导管占据的外层导管是弯曲的,例如产生“拱”形导管。拱形的弯曲可以是平面的或非平面的,但优选地应当使漩涡被保持并且不被弯曲的几何形状消除。因此,例如,支架通常可以为“拱”形(平面或非平面),具有根据本发明设置的大致周向延伸的部分,以产生非平面流腔。
如果需要,支架可以包括药物层。这样的层可被提供用于在一段时间里使药物持续释放。所以,支架可以提供用于疾病的初始治疗的药物,并且由于其给予到流中的特点,支架在最初和长期内具有治疗的好处。
支架可以具有真正的圆形的剖面,并因而具有最小可能的湿周边与剖面区域比,同时还仍然具有必要的特性以引起旋流。当然,也有可能会有支架具有非圆剖面的情况以例如帮助接口(或压力损失考虑不重要时)。
这里公开的支架可以被用在不同的生物医学的应用,例如在不同的动脉(像冠状动脉,颈动脉,下肢和肾动脉)、静脉、以及非心脏血管的应用,例如在胃肠的(如胆汁或胰液管)、泌尿生殖器(如输尿管或尿道)或呼吸系统(气管)。因此,本发明延伸到用于除了血液以外的体内流体的支架。通常,本发明的几何形状的使用可以避免迟滞区的存在,并因而是有益的。
下面将通过示例的方式参照附图描述本发明的一些优选实施方式,其中:

图1中示出了支架的管状部分2。管状部分2是圆柱形的结构,其中该结构侧壁由被排列成Z字形配置的杆4限定。该管状部分周向延伸并且是周向部分的一种形式。
图2中示出了一连串被并排连续配置以形成螺旋形支架6的管状部分2。支架具有螺旋中心线40。管状部分2通过支杆(strut)8互相连接,如图3(为清楚支杆被从图2中略去)中所示。每一管状部分2具有相对的端部分3a和3b。每一端部分事实上由给定的管状部分2的末端处的Z字形的顶点构成。纵向相邻的管状部分被配置成在其纵向上其端部分彼此以角度α面对,即,它们是不平行的,而在传统的圆柱形的支架中它们是平行的。
在图3a中所示的支架的部分中,第一管状部分2a具有纵向端部分3a和3b,同时第二管状部分2b同样具有纵向端部分3a和3b。第一管状部分2a的端部分3b与第二管状部分2b的端部分3a纵向间隔。各自的支杆8的长度不同,从而以彼此相对的角度α支撑管状部分2a和2b。弯曲因而被引入到支架支撑的流体导管10。
第三管状部分2c被配置成纵向相邻第二管状部分2b,并且通过另一组支杆8与其连接。该管状部分也位于与管状部分2b成角度α的位置。然而,支杆组8b可相对于支杆组8a旋转偏移,以使得支架将流体导管10支撑成具有螺旋形的流腔。
被支架6支撑的流体导管10具有如图2和3中所示的螺旋中心线40。每个管状部分2a、2b和2c的中心线依靠支杆8被配置成落在中心线40上。当然,因为每个管状部分2在纵向上具有有限的宽度,因此这将是一个近似。跨越每一管状部分的宽度的中央平面趋向于垂直于螺旋中心线40的切线,并且与中心线相交。
图3b、3c和3d中示出了支杆8另一可选配置,在这种配置中扭转稳定性得到了改善。支杆向侧面方向和纵向延伸,并且一些支杆在一个方向上延伸侧向分量,而其他支杆则在相对方向上延伸侧向分量。这些支杆的排列还适于连接在本文中描述和示出的其他周向部分。
图3b中示出了支杆8,其中交替的支杆具有相对的侧向分量,但是这些支杆不接触。在图3c中的实施例里,交替的支杆也具有相对的侧向分量,但是它们在其连接到管状部分2的末端接触。在图3d中,具有相对侧向分量的支杆对8在管状部分2之间的纵向空间的中间交叉。
图4中示出了另一种支架设计,其具有通过支杆8a和8b互相连接的管状部分2a,2b和2c。在这种情况下,管状部分具有封闭的环状设计。相同的原理用来获得具有螺旋流腔的流体导管。
在图5中所示的支架,一连串管状部分被彼此直接连接在点12处,而无需使用互连支杆8。因此,纵向相邻管状部分2a和2b的端部分3a和3b被连接在一起。在这种情况下,每一单独的管状部分的相对纵向端部分3a和3b互相成角度α。纵向相邻的管状部分旋转偏移,以为支架提供螺旋中心线40,并能支撑具有螺旋流腔的流体导管10。
下面参照图6和7进一步描述图5中的支架。管状部分2一侧的长度SL略长于相对侧的长度SS。为产生具有螺旋弯曲的支架,管状部分被连接在一起,并将每一部分相对于邻近部分稍微旋转。这显示在图7中。在支架的左手端,长侧SL被示为第一少部分,并且可以看出在管状部分之间具有相对旋转。相对旋转的量决定螺旋的螺距(pitch),小的相对旋转产生具有小的螺旋角和大的螺距的螺旋,并且大的相对旋转产生具有大的螺旋角和小的螺距的螺旋。
应该理解,如果管状部分的末端是通过并不是严格垂直于圆柱的轴平面切割圆柱而形成,则管状部分的至少一个末端将是稍微椭圆形的,而不是完全的圆形的。然而,考虑到通常用于制造支架的材料的柔韧性,在管状部分的末端部分将会有一定程度的柔度,所以在被相互连接的末端间它们可以适应旋转偏移和任何形状上的变化。
图8和9中示出了由连续的螺旋线圈制成的支架,其中线圈的螺旋角变得相对大。使用这样的螺旋配置来制造圆柱形的支架是公知的。线圈14由许多匝15组成,每一匝对应着螺旋的360°缠绕(在这种情况下由排列成Z字形配置的杆形成)。每一匝周向延伸并且具有周向部分的形式。线圈14具有前沿16a和后沿16b,其中一匝的前沿通过以支杆8的连接装置的形式与相邻匝的后沿纵向间隔。这些支杆具有不同的长度以使线圈14的中心线40遵循螺旋。
图3a,4,8和9中所示的支杆通常纵向延伸。然而,它们可以采取不同的配置,例如即具有纵向分量和侧向分量,如图3b,3c和3d中所示。因此,在给定的纵向方向中,一些形成连接装置的支杆组可以具有向左的侧向分量,而其他支杆组则具有向右的侧向分量。这将有助于扭转稳定性,从而有效确保了扩张的支架采用理想的非平面形状。
图10中所示的管1表示受本发明中的支架影响的流体导管的一种可能形状。管具有圆形的截面、外径DE、内径DI和壁厚T。管被绕成具有恒定幅度A(从平均到极限测量)的螺旋、恒定的螺距P、恒定的螺旋角θ以及扫掠宽度W。管状部分1被包含在假想的包层20中,其纵向延伸并具有与螺旋的扫掠宽度W相同的宽度。包层20可以被看作具有中心纵向轴30——其还可以被称为螺旋旋转的轴。示出的管1具有直的轴30,但是应当理解在另一设计里中心轴可以是弯曲的。管具有遵循绕中心纵向轴30的螺旋路径的中心线40。
可以看出幅度A小于管的内径DI。通过保持幅度在这个尺寸之下,被管占据的空间可以保持相对小,同时管的螺旋配置促进了沿着管的流动中旋流的产生。
例1
实验使用具有圆形截面的聚氯乙烯管来实施。参考图10中的参数,管的外径DE为12mm,内径DI为8mm并且壁厚T为2mm。管被绕成具有45mm螺距P和8°螺旋角的螺旋。被通过将管置于两个直边之间而测量直边之间的间距而确定幅度A。通过从扫掠宽度W中减去外径DE而确定幅度:

水沿着管流过。为了观察流动特性,放射状地穿过管壁的两根针80和82被用来把可见染料注射到流中。注射部位靠近中心轴30,即在流的“核心”。一根针80注射红墨水而另一根针82注射蓝墨水。
图11中示出了当雷诺数RB分别为500,250和100时三个实验的结果。可以看到在所有的情况里墨水丝84和86缠绕在一起,表示在核心处存在旋流,即流动通常是旋转的。
例2
除了针80和82被配置成在管壁附近释放墨水丝84和86外,这个例子中的参数与例1中的相同。图12中示出了近壁墨水释放,雷诺数RB分别为500和250时的两个实验的结果。可以看出,在这两种情况下墨水丝遵循螺旋管的几何形状,表示近壁漩涡。此外,促进了墨水丝和水的混合。
优选的支架具有的相对幅度的值AR小于或等于0.5,即小的相对幅度。在直的支架里幅度A和相对幅度AR都为0,因为这里没有螺旋。因此相对幅度AR的值接近0时,支架引起涡旋的能力将下降。对任意给定情况,相对幅度AR的最低可工作值将取决于流动的速度,以及流体的粘度及密度(即雷诺数),并且取决于螺距(螺旋角)以及支架的具体使用。至少为0.05,0.10,0.20,0.25,0.30,0.35,0.40或0.45的相对幅度AR是优选的。
Claims (9)
1.一种用于插入于人或动物体中的流体导管的支架,包括具有大致周向延伸的部分的空心结构,其中所述大致周向延伸的部分具有纵向的相对端部分,所述大致周向延伸的部分沿着所述空心结构的长度方向被接合到纵向相邻的周向部分,其中,相邻的周向部分的端部分不在平行的平面内,以及/或同一周向部分的端部分不在平行的平面内,并且其中不平行的端部分沿着所述空心结构的长度方向设置,以使得所述空心结构限定流腔,并且当在所述流体导管中被所述空心结构施加影响时,流体导管遵循非平面曲线。
