Descrizione
La presente invenzione concerne un apparecchio per effettuare suture di due tronchi di condotto umano o animate , comportante: ad una estremità -una testa realizzatrice di suture comprendente almeno una cartuccia con associate graffette e fustelle, ed una parte fungente da incudine; all'altra esstremità una parte dì commando dotata di manopola e leva; e una parte intermedia flessibile di trasmissione dei commando e delle sollecitazioni dalla parte di comando alla testa.
Come noto, una suturatrice per operazioni chirurgiche è utilizzata per effettuare l'unione meccanica (seguita da quella naturale biologica) di due tronchi aperti di condotti del corpo umano o animale.
Detti due tronchi vengono preventivamente preparati con l'ausilio di una attrezzatura atta a formare sulle loro estremità una legatura detta «a borsa di sacco». La suturatrice viene introdotta in uno dei due tronchi tramite l'apertura più facilmente access-sibile (ad es. nell'intestino tramite l'ingresso rettale) fino a raggiungere con la sua testa la zona di operazione dove i due tronchi staccati vengono stretti in una cartuccia fissata alla testa e vengono uniti meccanicamente tramite graffette (dispensate dalla cartuccia). La parte di tessuto organico eccedente all'interno del condotto viene asportata tramite fus-tellatura effettuata dalla stessa suturatrice dopo la graffatura. Successivamente estraendo la suturatrice dall'apertura (ad es. rettale) si recupera anche il tessuto eccedente tagliato dalla fustella. Dopo un certo tempo si ha la saldatura organica nella zona di unione dei due tronchi, il distacco del tessuto meccanicamente connesso tramite le graffette e la sua eliminazione pervia naturale (ad es. feci).
Sistemi ed apparecchi per l'effettuazione di questa importante operazione chirurgica sono largamente descritti in letteratura.
Ad esempio nei brevetto US No. 4 485 817 è rivendicata una suturatrice consistente di:
A) mezzi per innestare graffette al tessuto (testa) comprendenti un organo di applicazione delle graffette (cartuccia) ed un'incudine; B) mezzi per comandare gli elementi di detta testa; e C) mezzi di trasmissione tra A) e B) consìstenti di un albero flessibile pieghevole durante l'inserzione in grado di comformarsi alla configurazione dell'apertura del corpo umano e dì ritenere la configurazione così assunta durante l'operazione.
L'apparecchio brevettato ha certamente non pochi vantaggi in particolare quello di provvedere una suturatrice con albero flessibile tuttavìa esso presenta anche non pochi inconvenienti fra i quali ci si limita a citare: l'inamovibilità della testa, la necessità di regolarla in situ, il suo azionamento a compressione, e, soprattutto, la necessità di avere un diametro del tubo articolato da infilare nel condotto da suturare, piuttosto grande, in tutti i casi non riducibile a valori che rendono meno sofferto l'inserimento. Inoltre il comando è alquanto complesso e le sue flessibilità e memoria elastica sono limitate.
Tanto per fissare l'idea nello schema dì fig. 18 è rappresentato il metodo operativo degli apparecchi convenzionali compreso quello secondo il brevetto
US No. 4 485 817 che comporta: la fase l di inserimento della suturatrice con testa chiusa (con diametro ad es. di ben 32 mm); la fase 11 di apertura della testa all'interno del condotto intestinale (girando il bottone del dispositivo di comando); Io stadio IH di calzatura della borsa di tabacco, in IV) la chiusura della cartuccia (girando il bottone in senso inverso), ed in V) la graffetatura e fustellatura.
L'operazione nel suo complesso non e scevra non solo da dolori per il paziente ma anche da sforzi per il chirurgo che deve effettuare da solo tutte le fasi e manovre»
Primo scopo del presente trovato è ora quello di provvedere una suturatrice che non abbia gii inconvenienti suddetti; in particolare sia senza testa nella fase di inserzione, e perciò con diametro piccolo e con la possibilità di preregolazione della testina all' esterno.
Un altro scopo è un metodo per effettuare suture in tempi brevi, con la massima sicurezza operativa, senza pericoli e occasioni di contaminazioni, e senza la necessità dì fare intervenire il chirurgo in tutte le fasi.
Questi ed altri scopi vengono raggiunti con l'apparecchio secondo l'invenzione avente le caratteristiche precisate nella rivendicazione 1.
I diversi aspetti e vantaggi dell'invenzione appariranno meglio dalla descrizione della forma di realizzazione (preferita e non limitativa) rappresentata nei disegni allegati, nei quali:
- le fig. 1,4, 8,11 e 15 illustrano il dispositivo di comando B (all'estremità di sinistra nelle figure) della suturatrice secondo il trovato rispettivamente: la fig. 1 a riposo; la fig. 4 allorché la cartuccia è aperta per il posizionamento dei due tronchi di tessuto organico da suturare; la fig. 8, a graffatura e fustella-tura avvenute; la fig. 11, allorché la cartuccia è stata preregolata all'esterno ad es. dal chirurgo o da un suo aiuto, tramite il nottolino in base allo spessore del tessuto da suturare; e la fig. 15 durante la fase di riapertura;
- le fig. 3, 7, 9, 10,14 e 16 rappresentano le viste parzialmente in sezione della parte operativa ed incudine, (parte A all'altra estremità), cioè della testa della suturatrice nelle posizioni corrispondenti precisate per il dispositivo di comando; in particolare: fig. 3, l'estremità della suturatrice con cartuccia suturante montata (fig. 9); la fig,7, la cartuccia in posizione di utilizzo; la fig. 9, i componenti della cartuccia preregolata, in scala ingrandita; fig. 10, la cartuccia a graffatura e fustellatura avvenute (fig. 8), la fig. 14 la stessa cartuccia preregolata, e la fig. 16, la cartuccia dopo la fustellatura con cedimento dell'anello di sicurezza 14 (fig. 9),
- la fig. 2 riporta i particolari delle estremità della suturatrice senza testa ma col cappuccio 32 che si utilizza nella fase di introduzione attraverso un'apertura organica, (ad es.rettale)
- la fig. 5 è una vista dal basso della disposizione delle sedi S delle graffette G (particolari 11 e 12 in fig. 9);
- la fig. 6 è una sezione laterale della cartuccia con vista della graffetta G nella sua sede S e della linguetta L, (particolare 8 in figura 9) che spìnge la graffetta G;

le fig. 12 e 13 rappresentano il sistema di sicurezza 22 (fig.1l) innestato rispettivamente disinnestato, e le figure 18 e 19 sono due schemi a blocchi rappresentanti il medoto convenzionale, rispettivamente secondo l'invenzione.
L'apparecchio secondo il trovato comprende all'estremità di sinistra, un dispositivo B) di comando della suturatrice 21-22-23, un mezzo intermedio C, ditrasmissionè 31 ed un mezzo A) di esecuzione delle suture all'altra estremità di destra (nei disegni). Partendo da quest'ultimo rappresentato appunto a destra nelle fig. 3,7,10,14 e 17 su scala normale, ma al centro delle tavole di disegno su scala ingrandita nelle fig. 9 e 16, esso comprende un bottone o nottolino graduato 1 per la preregolazione dello spessore del tessuto organico da suturare; un cursore filettato 2 per determinare la distanza minima di chiusura della cartuccia il cui corpo principale è indicato con
7 ed II cui involucro esterno è indicato con 3; un anello 4 preferibilmente in acciaio inossidabile per trattenere il nottolino 1 ; il cilindretto 5 di contrasto tra la base 15 ed II detto cursore 2; e 6 il piano mobile per la fuoriuscita delle linguette 8 (che a loro volta comandano le graffette G) e della fustella 10.
Come sopra anticipato, la cartuccia consiste del corpo principale 7, della molla 9 per trattenere in posizione la fustella 10 ed il cursore 2, e delle linguette
8 per il comando delle graffette (G) esterna 11 ed interna 12.
Al di sotto della base inferiore di detta testa si trova l'incudine o anello 13 di ribaditura delle graffette 11—12, l'anello di sicurezza 14 e la base di attacco 15 della cartuccia. Il mezzo (C) di trasmissione 31 tra il dispositivo di comando B) (che si descriverà più avanti) e la testa ad incudine A) è rappresentato più dettagliatamente e su scala ingrandita nelle figure 2 e 16; esso comprende la guaina esterna 31, il cappuccio di chiusura 32, un tirante 18 con estremità sagomata 16, il corpo 17 di avvitamento della cartuccia, ed i giunti snodabili 19. Dalla fig. 2 si vede l'interfaccia tra il mezzo C) di trasmissione 30-31 e la parte A) cioè la testa + incudine 7-13.
Le fig. 1 e 11 rappresentano dettagliatamente il disposi tivo di comando B) consistente di una leva dentata 23 impegnata, a mò di cremagliera, con la dentatura esterna di un nottolino 21 di regolazione grossolana (a passo lungo) per l'apertura della cartuccia 7. (La fig. 3 fà vedere l'estremità della suturatrice con cartuccia suturante A montata e la fig. 7 rappresenta la cartuccia in posizione di utilizzazione).
Tornando alla fig. 1, il riferimento 24 indica l'estremità inferiore del tirante 29 (continuazione di 18), mentre 28 indica il dispositivo meccanico di accoppiamento tra detti 29 e 24. La parte inferiore di inserzione del cavo flessibile (mezzo di trasmissione) nel dispositivo di comando B), comporta la ghiera 26 di bloccaggio della guaina 31 ed una corrispondente estremità di attacco a snodo 27. II dispositivo di comando B comporta anche un organo di sicurezza 22 che nella fig. 12 assume la posizione 22a allorché è innestato mentre nella fig. 13 assume la posizione 22b allorché è disinnestato.
In fig. 5 si vede dal basso la disposizione delle sedi S delle graffette G (corrispondenti ai particolari 11 e 12 di fig. 9) mentre la fig. 6 è una sezione laterale della cartuccia, con vista della graffetta G nella sua sede S e delia linguetta (8 in fig. 9) che spinge la detta graffetta.
Alla luce di quanto premesso nell'introduzione circa il funzionamento in generale delle suturatrici, quello dell'apparecchio secondo il trovato può essere descritto brevemente come segue:
In base allo spessore dell'intestino del paziente da operare, l'operatore regola la distanza fra l'incudine (13) ed il corpo principale (7) agendo sul bottone graduato (1). Vantaggiosamente questa regolazione viene fatta fuori dal campo operatorio (anche se è fattibile in sito). Tramite un orifizio o apertura disponibile nel corpo umano (ad es. ingresso rettale per l'intestino) si introduce la parte esecutiva dell'apparecchio (quella rappresentata in fig. 9) senza cartuccia con il cappuccio di protezione 3 montato. Dopo aver preparato la parte da suturare ad es. l'intestino, si toglie detto cappuccio di protezione (3) e si avvita la testa. Con la manopola (21) situata sul manico a leva (23) si allontana il corpo principale (7) ad esempio fino a 25 mm in modo da agevolare l'inserimento dell'intestino e la relativa chiusura a sacco. Sempre manovrando la detta manopola sul manico a leva si avvicina il corpo principale 7 all'incudine 13 nella direzione della freccia F. L'intestino non può essere schiacciato più dello spazio lasciato libero in fase di preregolazione. Arrivato al fermo la manopola presenta una forte resistenza. Agendo sulla leva si tira il cursore (6) comprimendo la molla 9. Le linguette 8 spingono le graffette G verso l'incudine 13 chiudendole. Contemporaneamente la fustella (10) taglia l'intestino e rompe l'anello di sicurezza 14 segnalando che l'operazione è compiuta.
Un primo aspetto del trovato è legato al fatto che ora la testa è amovibile e inizialmente cioè all'inserimento è asportata e regolata di per sé all'esterno.
Secondo un secondo aspetto dell'invenzione la testa viene azionata a trazione nel senso della freccia F. In un primo momento si gira la manopola (23) In senso orario che agendo sul filo 18 lo mette in trazione per cui il corpo 7 si sposta verso sinistra fin quando i perni o simili di fine corsa 5 vanno ad appoggiarsi sul corpo inferiore 15, cioè sui reggispinta relativi. A questo punto l'operatore sente una resistenza all'ulteriore avanzamento della testa in direzione F, resistenza che è data dalla molla 9 che è infilata sulla fustella 10.
Come già anticipato, a questo punto la rotazione della manopola 23 diventa dura, si abbandona 23 e si và ad agire sulla leva a cremagliera 21 che continua con forza moltiplicata, lo spostamento delle fustelle 10 e contemporaneamente spinge le graffette 12 fino a portarle contro l'incudine 13; inizia così la piegatura delle graffette 12, che hanno già attraversato i due lembi del tessuto e nella continuazione della corsa l'estremità della fustella 10 taglia detto tessuto dell'intestino 14, avvisando così l'operatore chirurgico che l'operazione è terminata e che le graffette 12 sono ormai chiuse intorno ai lembi del tessuto. Questa posizione di tranciatura è segnalata acusticamente dal «tac» del cedimento dell'anello e meccanicamente dal fine corsa.

rici leggermente pressati e incastrati (preferibilmente inpoliammidi). La ghiera 26 biocca la guaina 31 mentre 27 è l'estremità dell'attacco a snodo.
E' essenziale che ci sia giogo ridotto al minimo tra il filo 28 e la zona a minor diametro.
Il flessibile può essere coperto da guaina 31. Per azionare la leva 21 alla fine della rotazione della manopola 23, la leva deve essere sbloccata azionando un cuneo 22, che libera la leva. Le posizioni del dispositivo di sicurezza sono indicate con 22a e 22b.
Lo schema a blocchi di pag. 19 fa vedere ora che in l'l'apparecchio è inserito senza testa e con capsula protetta per cui il diametro è praticamente quello del flessibile e può scendere anche al disotto 15 mm; in II' si effettua la preregolazione della testina all'esterno mentre in III', IV', V', VI' sì ripetono le fasi classiche di apertura della testina, calzatura della borsa di tabacco, chiusura cartuccia, e graffatura e fustellatura.
Riassumendo, tra le caratteristiche salienti del trovato vanno menzionate oltre a quelle relative alla testa amovibile e alla sua preregolazione all'esterno, anche quelle derivanti dal fatto che: la testa lavora all'incontrano (rispetto ai citato brevetto US), cioè a trazione e non a compressione; il comando è unico cioè comporta un solo filo; il flessibile è un corpo piegabile fino a 90° e non tende a raddrizzarsi neanche sotto minima sollecitazione - l'apparecchio funziona con cartuccia invertita, cioè la parte fissa fà da incudine e la parte mobile da martello - la cartuccia viene montata dal chirurgo all'ultimo momento nella zona di suturazione dopo che la parte flessibile ha già percorso il condotto ed è sbucata tra i tronchi nella zona da suturare, il che consente di inserire nell'intestino o in un altro tubo o condotto un dispositivo di diametro molto inferiore (circa la metà) che danneggia molto meno l'orifizio ed il tubo stesso; esiste un sistema si sicurezza che segnala al chirurgo con una variazione dello Sforzo, che la fustellatura dell'intestino è avvenuta.
L'apparecchio secondo il trovato è anche vatag-gioso perchè è sterilizzabile tutto intero, ed in un'unica fase con ossido di etilene (invece di ricorrere alle sterilizzazioni differenziate per i singoli pezzi, a vapore e/o con soluzioni secondo la tecnica nota).
Claims (5)
1. Apparecchio per effetuare Suture dì due tronchi di condotto umano e animale, comportante: ad una estremità una testa realizzatrice (A) di suture comprendente almeno una cartuccia (7) con associate graffette e fustelle, ed una parte fungente da incudine (13) all'altra estremità una parte (B) di comando dotata di manopola e leva (23); e una parte intermedia (G) flessibile dì trasmissione dei comandi e delle sollecitazioni dalla parte di comando (21-22-23) alla testa, caratterizzato dal fatto che la testa è amovibile e che l'estremità della parte intermedia (C) privata della testa è protetta da un cappuccio (32).
2. Apparecchio secondo la rivendicazione 1, caratterizzato dal fatto che alla testa è associato un tirante (18) di avvicinamento del corpo di testa con cartuccia (7) all'incudine fissa (13).
3. Apparecchio secondo là rivendicazione 2, caratterizzato dal fatto che è provvisto un anello di sicurezza (14) che segnala al chirurgo, con una variazione di sforzo, l'avvenuta fustellatura.
4. Apparecchio secondo la rivendicazione 1, caratterizzato dal fatto che la parte intermedia flessibile (C) è formata da un tirante (18) e da una successione di snodi (30) costituiti da giunti articolati frenati in materiale plastico, che conferiscono flessibilità fino a 90° e mantengono la configurazione assegnatagli fino a che non si imprime loro uno sforzo di modifica.
5. Apparecchio secondo la rivendicazione 4, in cui gli snodi sono in poliammide e sono coperti da guaine (31).
