Die Erfindung betrifft ein Verfahren zur Herstellung einer topischen Benzoylperoxydzusammen- setzung, wobei in einen wässerigen alkoholischen Träger 0, 5 bis 15 Gew.-% eines Geliermittels, vorzugsweise kolloidales Magnesiumaluminiumsilikat, Hydroxypropylmethylzellulose, mikrokristalline
Zellulose oder hydroylierte Vinylpolymere, sowie 1 bis 30 Gew.-% mikronisiertes Benzoylperoxyd mit einer Teilchengrösse von weniger als 150 11m bei einer durchschnittlichen Teilchengrösse in der Zusammensetzung von weniger als 35 I1rn sowie gegebenenfalls ein oder mehrere Tensid (e) ein- gearbeitet werden. Die erfindungsgemäss herstellbaren topischen Benzoylperoxydzusammensetzungen sind zur Behandlung von Akne und Seborrhoe geeignet.
Akne und Seborrhoe stellen einen Zustand der menschlichen Haut dar, der durch einen übermässigen Fluss von Sebum (Talg) oder Hautöl aus den sich im piloseboroischen System befinden- den seboroischen Drüsen gekennzeichnet ist. Der Kanal, durch den der Talg die Hautoberfläche erreicht, ist der Ductus (Gang) des Haarfollikels. Das Vorhandensein von überschüssigen Mengen an Talg in dem Ductus und auf der Haut führt zur Blockierung oder zum Stocken des kontinuier- lichen Flusses von Talg aus dem Follikelductus, was wieder zu einer Verdickung des Talgs, der zu einem festen, als Komedo bekannten Pfropfen wird, führt. Wenn dies geschieht, wird die Hyper- keratose der Follikelöffnung so stimuliert, dass der Ductus völlig verschlossen wird.
Das übliche
Ergebnis ist ein Knötchen (Papel), eine Pustel oder eine Zyste, welche oft mit zu sekundären
Infektionen führenden Bakterien verunreinigt sind. Diese Abläufe charakterisieren die Krankheit, die heutzutage als Akne und in weniger ernsten Fällen als Seborrhoe bekannt ist.
Für die Behandlung von Akne und Seborrhoe werden viele therapeutische Mittel eingesetzt, um die Blockierung der Follikelgänge zu verhindern, um den Gang (Ductus) wieder zu öffnen, wenn er einmal blockiert ist, um gegen die infizierenden Bakterien oder den verdickten Talg vorzugehen oder um Kombinationen dieser Wirkungen zu erzielen. Es ist bekannt, Schwefel als ein mildes Hautreizmittel zu verwenden, um die hornige Schicht der Haut und mit dieser die die Follikelöffnungen verstopfenden Ansammlungen zu entfernen. Benzoylperoxydgelzusammensetzungen haben sich bei der Behandlung von Akne, Seborrhoe und den damit verbundenen sekundären Infektionen als wirksam erwiesen.
In der US-PS Nr. 3, 535, 422 ist eine therapeutische Zusammensetzung für die Behandlung von Akne beschrieben, die aus einer gleichförmigen Dispersion von Benzoylperoxyd in einem flüssigen, Wasser und mindestens ein organisches Erweichungsmittel enthaltenden Medium besteht.
In der US-PS Nr. 4, 056, 611 ist eine therapeutische Zusammensetzung für die Behandlung von Akne beschrieben, die aus einer stabilen, eine einzige Phase aufweisenden Dispersion von feinteiligen Teilchen von Benzylperoxyd in einem'wässerigen Alkoholträger besteht. Die Phase dieser Zusammensetzung ist nicht lipoid und enthält ein nichtionisches Tensid, das in dem wässerigen Alkoholträger löslich ist.
Zum Stand der Technik gehörende Benzoylperoxydzusammensetzungen, die einfach feinteilige Benzoylperoxydteilchen in einer Emulsion von Wasser und bestimmten ausgewählten Erweichungsmitteln enthalten, besitzen den Nachteil, dass bei Verdunstung des Wassers der Emulsion der grösste Teil der organischen Erweichungsmittel und die grossen Benzoylperoxydteilchen auf der Oberfläche der Haut nahe oder in Berührung mit den Aknestellen verbleiben, was zu Reizungen führen kann.
Ausserdem bewirkt die Verwendung von grossen Mengen nichtionischer Tenside in derartigen Zusammensetzungen mit grosser Wahrscheinlichkeit Reizungen durch das Benzoylperoxyd, es sei denn, es werden extrem feine Benzoylperoxydteilchen verwendet.
Auf Grund der stark oxydierenden Eigenschaften von Benzoylperoxyd führt die Verwendung dieser Substanz in einer herkömmlichen Salbe oder Emulsion zu instabilen Zusammensetzungen, die nach kurzer Zeit einen nicht akzeptablen Verlust an keratolytischer Wirksamkeit zeigen.
So wird auch in der GB-PS Nr. 1, 407, 937 ausgeführt, dass, wie aus der Literatur bekannt, Benzoylperoxyd in wässerigem Medium an Wirksamkeit verliert und dass ein oberflächenaktives Mittel notwendig ist, um das Peroxyd in der Zusammensetzung auf den Aknestellen eines Patienten richtig zu verteilen. Die vorgenannte GB-PS gibt als wesentlichen Bestandteil ein oberflächenaktives Mittel an, das einen hydrophilen Teil aufweist, welcher ein Polyoxyalkylenrest, insbesondere Polyäthylenlauryläther, ist. Ferner ist in der vorgenannten GB-PS besonders angegeben, dass das oberflächenaktive, in der Zusammensetzung angewendete Mittel nichtionisch und in dem

wässerig alkoholischen Träger löslich sein muss.
Die Verwendung des oberflächenaktiven Mittels gemäss der vorgenannten GB-PS, insbesondere Polyoxyäthylenlauryläther, führt zu bestimmten Nach- teilen auf Grund des Umstandes, dass Polyoxyäthylenlauryläther bekanntlich Reizungen bei
Patienten bewirkt, insbesondere in Kombination mit einem Reizmittel, wie Benzoylperoxyd.
Es wurde nun gefunden, dass die Verwendung von Dioctylnatriumsulfosuccinat überraschender- weise die Stabilität des Peroxyds in einem wässerigen Träger verbessert und die Tendenz zur
Reizwirkung, selbst in Gegenwart von Bestandteilen wie Polyoxyäthylenlauryläther, vermindert.
Insbesondere wurde gefunden, dass bei der Einarbeitung von 0, 5 bis 15 Gew.-% eines Geliermittels und 1 bis 30 Gew.-%, vorzugsweise 5 bis 15 Gew.-%, mikronisiertem Benzoylperoxyd mit einer
Teilchengrösse von weniger als 150 11m bei einer durchschnittlichen Teilchengrösse in der Zusammen- setzung von weniger als 35 11m sowie gegebenenfalls eines oder mehrerer Tenside in einen wäs- serigen alkoholischen Träger eine bezüglich der Benzoylkomponente völlig stabile topische Benzoylperoxydzusammensetzung erhalten werden kann, wenn als Stabilisierungsmittel 0, 1 bis 3 Gew.-%, bezogen auf die Gesamtzusammensetzung, Dioctylnatriumsulfosuccinat eingebracht werden.
Die völlige Stabilität bezüglich der Benzoylperoxydkomponente zeigt sich selbst dann, wenn sie höheren Temperaturen, als normalerweise bei der üblichen Verwendung des Produkts zu erwarten, ausgesetzt wird. Ausserdem erlaubt ein so hergestelltes alkoholisches Gel bei Verdunstung eine gleichmässige Freisetzung des mikronisierten Benzoylperoxyds, so dass dem Brennen und der entzündlichen Rötung, die bei andern scharfen Gelformulierungen beobachtet werden, vorgebeugt wird.
Das erfindungsgemässe Verfahren der eingangs angegebenen Gattung zur Herstellung einer topischen Benzoylperoxydzusammensetzung besteht demgemäss in seinem Wesen darin, dass man als Stabilisierungsmittel 0, 1 bis 3 Gew.-%, bezogen auf die Gesamtzusammensetzung, Dioctylnatriumsulfosuccinat einbringt. Eine bevorzugte Menge des erfindungsgemäss eingesetzten Stabilisierungsmittels liegt im Bereich von 0, 1 bis 1 Gew.-%.
Der Ausdruck "Tenside", der bei der Herstellung der erfindungsgemäss erhältlichen topischen Benzoylperoxydzusammensetzungen fakultativ einsetzbare Tensidkomponenten bezeichnet, schliesst im Rahmen dieser Erfindung das als Stabilisierungsmittel verwendete Dioctylnatriumsulfosuccinat nicht ein. Das letztgenannte Stabilisierungsmittel stabilisiert, wie erwähnt, die erfindungsgemäss erhältliche Benzoylperoxydzusammensetzung und verringert die Tendenz zur Reizwirkung, selbst in Gegenwart des Tensids Polyoxyäthylenlauryläther.
Eine bevorzugte Ausführungsform des erfindungsgemässen Verfahrens, bei der etwa 5 Gew.-% mikronisiertes Benzoylperoxyd, 0, 3 bis 3 Gew.-% eines hydroxylierten Vinylpolymer-Geliermittels, vorzugsweise eines neutralisierten hydroxylierten Vinylpolymer-Geliermittels, und etwa 15 Gew.-% eines Alkanols mit Wasser vermischt werden, besteht in ihrem Wesen darin, dass man etwa 0, 2 Gew.-% Dioctylnatriumsulfosuccinat einbringt. Falls die erfindungsgemäss erhältliche topische Benzoylperoxydzusammensetzung ein oder mehrere Tensid (e) enthält, beträgt die Menge davon vorteilhafterweise 1, 0 bis 6, 0 Gew.-% und vorzugsweise 3 bis 6 Gew.-%.
Die erfindungsgemäss herstellbare topische Gelzusammensetzung muss ausreichend Benzoylperoxyd enthalten, um therapeutisch wirksam zu sein. Sie sollte nicht mehr Peroxyd enthalten, als gleichmässig im Träger dispergiert werden kann, damit eine sanft auftragbare Zusammensetzung gebildet wird. Diese Erwägungen bestimmen, dass die Zusammensetzung mindestens 1 Gew.-%

mikronisierter, feinverteilter, kristalliner Teilchen mit einer durchschnittlichen Teilchengrösse von weniger als 35 11m vorliegen.
Die erfindungsgemäss herstellbare wässerige Gelzusammensetzung enthält vorteilhafterweise ein Tensid, wie die Ester von Polyolen und Zuckern, die Produkte der Kondensation von Äthylenoxyd mit Fettsäuren, Fettalkohole, langkettige Alkylphenole, langkettige Mercaptane, langkettige Amide, Polyäther von polyhydroxylierten Fettalkoholen und Alkylpolyglykoläther, in einer Menge von 3 bis 6 Gew.-%. Es wurde gefunden, dass die erfindungsgemäss herstellbare topische Zusammensetzung eine aussergewöhnliche Lagerbeständigkeit besitzt, wenn Polyglykoläther in etwa der gleichen Menge wie das Stabilisierungsmittel Dioctylnatriumsulfosuccinat verwendet wird.
Eine weitere wichtige Komponente der erfindungsgemäss erhältlichen topischen Zusammen-

setzung ist das Geliermittel. Dieses kann sowohl nach Typ als auch Menge ausgewählt werden, so dass Produkte verschiedener Viskosität erhalten werden. In der bevorzugten Ausführungsform der Erfindung wird das Geliermittel so ausgewählt, dass ein elegant gebildetes und stabiles Gel erhalten wird. Es können für die erfindungsgemässen Zwecke verschiedene Geliermittel verwendet werden. Bevorzugte Geliermittel sind jedoch reine mikrokristalline Zellulose, kolloidales Magnesium- aluminiumsilikat, Hydroxypropylmethylzellulose und die sogenannten hydroxylierten Vinylpolymere, insbesondere die in der US-PS Nr. 2, 798, 053 beschriebenen.
Die hier besonders interessierenden hydroxylierten Vinylpolymeren werden allgemein als Interpolymeren einer monomeren monoolefinischen
Acrylsäure und etwa 0, 1 bis 10 Gew.-%, bezogen auf die gesamten Monomeren, eines monomeren
Polyäther eines Oligosaccharids beschrieben, bei welchem die Hydroxylgruppen, die modifiziert sind, mit Allylgruppen verestert sind, wobei der Polyäther mindestens zwei Allyläthergruppen je Oligosaccharidmolekül enthält. Im Handel erhältliche Interpolymeren dieses Typs werden unter der Handelsbezeichnung"Carbopol"vertrieben. Diese werden als Polymeren von Acrylsäure be- schrieben, die mit etwa 1% eines Polyallyläthers von Saccharose mit durchschnittlich etwa 5, 8
Allylgruppen je Saccharosemolekül vernetzt sind.
Diese Polymeren besitzen ein Molekulargewicht im Bereich um 1000000 und sind im Handel von der Firma B. F. Goodrich Chemical Company unter den Handelsbezeichnungen"Carbopol 934","Carbopol 940"und"Carbopol 941"erhältlich.
Die verschiedenen Carbopol-Produkte werden vom Hersteller nach ihrer Viskosität unter- schieden. Die Polymeren werden geliert, indem sie mit einem alkalischen Material neutralisiert werden. Geeignete Neutralisierungsmittel sind organische Amine, von denen Triäthanolamin, Tri- äthylamin, Isopropylamin, Diisopropylamin usw. erwähnt seien, und anorganische Basen, von denen NaOH, KOH, Ca (CO) : usw. erwähnt seien.
Die Menge an Geliermittel, die in den erfindungsgemäss erhältlichen Zusammensetzungen ent- halten sein kann, kann etwas schwanken. Gewöhnlich liegt das Geliermittel, bezogen auf das
Gesamtgewicht der fertigen Zusammensetzung, in einer Menge von 0, 5 bis 15 Gew.-% und vorzugs- weise etwa 1 bis 5 Gew.-% vor.
Die erfindungsgemässen Zusammensetzungen enthalten gewöhnlich auch erhebliche wässerige
Komponenten. Wenn Wasser vorhanden ist, kann dessen Menge je nach Art des gewünschten Produkts schwanken. Gewöhnlich beträgt die Menge an Wasser bezogen auf das Gesamtgewicht des fertigen Produkts 30 bis 70 Gew.-%. Es ist bevorzugt, demineralisiertes Wasser zu verwenden.
Der im wässerigen Alkoholgelträger verwendete Alkohol muss in Wasser löslich sein, als Colösungsmittel für das Stabilisierungsmittel und das bzw. die gegebenenfalls vorhandene (n) Tensid (e) in dem Träger und auch bei Auftragung auf die Haut als antiseptisches und trocknendes Mittel dienen, Alkylalkohole mit 1 bis 6 Kohlenstoffatomen erfüllen die zuvor genannten Bedingungen und werden bei der erfindungsgemässen Formulierung der Zusammensetzung verwendet. Eine Menge von 10 bis 80 Gew.-% eines oder mehrerer dieser Alkohole ist ausreichend, um sicherzustellen, dass das Stabilisierungsmittel und das bzw. die gegebenenfalls vorhandene (n) Tensid (e) in der Zusammensetzung gelöst sind.
Manchmal ist es vorteilhaft, den erfindungsgemäss erhältlichen Zusammensetzungen zusätzliche therapeutisch aktive Bestandteile zuzusetzen, die dazu dienen können, die Aktivität des Benzoylperoxyds zu steigern oder zu ergänzen. Für diesen Zweck können eine Vielzahl von Materialien verwendet werden. Von besonderem Interesse als zusätzliches Hautbehandlungsmittel sind die keratolytischen Mittel und insbesondere Salicylsäure. Wenn diese verwendet werden, können sie über einen Konzentrationsbereich eingesetzt werden, der zwischen 0, 2 und 8 Gew.-% liegen kann.
Die Zusammensetzung kann ausserdem vorteilhafterweise 1 bis 25 Gew.-% feinteiligen mikronisierten oder kolloidalen Schwefel enthalten. Schwefel ist ein antimikrobielles und keralytisches Mittel, das seit langem bei der Behandlung von Akne verwendet wird. Wenngleich nicht genau bekannt ist, wie Schwefel seine keratolytische Wirksamkeit auf der Haut ausübt, wird angenommen, dass der Schwefelwasserstoff, der sich beim Kontakt des Schwefels mit dem Tiergewebe bildet, mit Alkali in den Gewebeflüssigkeiten zu aktiven Sulfiden reagiert, die wieder die Keratose fördern. Die Kombination von Schwefel mit Benzoylperoxyd ergibt eine bedeutend grössere Keratosis als bei Verwendung dieser Substanzen allein.
Gewünschtenfalls können der Zusammensetzung geringe Mengen einer verträglichen Säure

oder Base zugesetzt werden, um die relative Acidität oder Alkalinität der Zusammensetzung einzu- stellen. Der PH-Wert der Zusammensetzung wird gewöhnlich auf einen Bereich von 3, 5 bis 7, 5 und vorzugsweise 5 bis 6,5 eingestellt.
Um elegantere Produkte zu formulieren können weiterhin zusätzliche Additive in die erfindungsgemässe Zusammensetzung eingearbeitet werden. Typische Additive sind unter anderem Emulgiermittel, Erweichungsmittel, Konservierungsmittel usw.
Die erfindungsgemäss erhältliche Gelzusammensetzung wird äusserlich auf der Haut des Patienten angewandt, indem das Gel ein- oder mehrmals täglich in Abhängigkeit vom Hautzustand eingerieben wird, um trocknend, desquamativ (abschuppend) und antiseptisch zu wirken. Bei fast allen Personen, die die erfindungsgemäss erhältlichen Zusammensetzungen verwenden, zeigt sich innerhalb der ersten wenigen Wochen der Behandlung eine deutliche Unterdrückung der Aknebildung. Darüber hinaus konnte gezeigt werden, dass die erfindungsgemäss erhältlichen Zusammensetzungen deutlich effektiver und schneller wirksam sind als Benzoylperoxyd enthaltende Emulsionen oder Gelformulierungen vergleichbarer Stärke, die aus dem Stand der Technik bekannt sind.
Im folgenden soll die Erfindung an Hand von erfindungsgemäss erhaltenen Beispielen näher erläutert werden.
Beispiel 1 : Zu 495, 0 mg gereinigtem Wasser wurden unter Rühren 15, 0 mg Carbopol 940 (ein Carboxyvinylpolymeres von B. F. Goodrich Co. in der Säureform) gegeben. Das Rühren der Mischung wurde 45 min lang fortgesetzt. Dann wurden 4, 095 mg Natriumhydroxyd in 4, 91 ml gereinigtem Wasser zugegeben. Das Rühren der Mischung wurde 10 min fortgesetzt. Dann wurden 150,0 mg Äthylalkohol, 0, 50 mg Parfüm und 0, 50 mg Methylsalicylat zugesetzt. Zu der gerührten Mischung wurde dann eine Mischung gegeben, die 210, 0 mg nassverpacktes mikronisiertes Benzoylperoxyd (50% Benzoylperoxyd - 50% Wasser), 2, 0 mg Dioctylnatriumsulfosuccinat, 41, 0 mg Alkylpolyglykol- äther und 41, 0 mg gereinigtes Wasser enthielt.
Die Mischung wurde weitere 30 min gerührt, bis eine weiche und elegante Gelmischung erhalten wurde.

The phase of this composition is not lipoid and contains a nonionic surfactant that is soluble in the aqueous alcohol vehicle.
Prior art benzoyl peroxide compositions which simply contain finely divided benzoyl peroxide particles in an emulsion of water and certain selected emollients have the disadvantage that when the water of the emulsion evaporates, most of the organic emollients and the large benzoyl peroxide particles on or near the surface of the skin remain in contact with the acne spots, which can lead to irritation.

The gel composition obtainable according to the invention is applied externally to the patient's skin by rubbing the gel once or several times a day depending on the skin condition in order to have a drying, desquamative (desquamating) and antiseptic effect. Almost all people who use the compositions according to the invention show a clear suppression of acne formation within the first few weeks of the treatment.
