Definitions
- the technology disclosed in this specification relates to stents and methods for manufacturing stents.
- stent placement is used when a narrowing or obstruction (hereinafter collectively referred to as "stenosis") occurs in a body lumen (digestive organs such as the bile duct, gallbladder, pancreas, esophagus, duodenum, small intestine, large intestine, blood vessels, ureters, trachea, etc.).
- Stent placement is a procedure in which a stent is placed either at the location of the narrowing or at a location that bypasses the narrowing, thereby securing the lumen.
- the stent has a tubular main body and a cover membrane that covers the main body (see, for example, Patent Document 1).
- Stents having a tangled portion in which the first bent portion and the second bent portion of the wire forming the main body are tangled with each other are already known (see, for example, Patent Document 2).
- a longitudinal compressive force acts on a portion of the stent's main body.
- the first and second bent portions of the wire at the entangled portion move relative to each other, forming an opening surrounded by the first and second bent portions. Therefore, if a cover membrane were provided on a stent with a known entangled portion, there is a risk of bodily fluids leaking from the opening.
- the stent disclosed in this specification comprises a main body portion and a cover membrane.
- the main body portion is a tubular member formed from braided wire.
- the cover membrane covers the main body portion.
- the main body portion has an entanglement portion in which a first bent portion and a second bent portion of the wire are entangled with each other.
- the main body portion has a first state in which a longitudinal compressive force acts on a specific portion that is at least a part of the main body portion, and a second state in which no longitudinal compressive force acts. In the first state, the entanglement portion located at the specific portion is surrounded by the first bent portion and the second bent portion, forming an opening of a first shape having a first opening area.
- the cover membrane has a blocking portion that blocks the opening of the first shape.
- the cover membrane has a blocking portion that closes the opening formed in the entanglement portion in the first state, in which a compressive force acts on a specific portion of the main body. Therefore, this stent can prevent holes from forming in the cover membrane as it transitions from the second state, in which no longitudinal compressive force acts, to the first state, and can prevent bodily fluids from leaking through those holes.
- the periphery of the occlusion portion may be fixed to the first bent portion and the second bent portion. This stent can effectively prevent holes from forming in the cover membrane when transitioning from the second state to the first state.
- the occlusion portion may be loose in the second state. This stent can more effectively prevent holes from forming in the cover film when transitioning from the second state to the first state.
- the entangled portion located at the specific portion may not have an opening surrounded by the first bent portion and the second bent portion. Even in a configuration in which no opening is formed in the entangled portion in the second state, this stent can prevent holes from forming in the cover film when transitioning from the second state to the first state.
- the entangled portion located at the specific portion may have a second type of opening surrounded by the first bent portion and the second bent portion and having a second opening area smaller than the first opening area. Even in a configuration in which an opening with a relatively small area is formed in the entangled portion in the second state, this stent can prevent holes from occurring in the cover film when transitioning from the second state to the first state.
- the method for manufacturing a stent disclosed in this specification includes preparing a tubular main body formed from braided wire and having an entanglement where first and second bent portions of the wire are entangled with each other, placing the main body in a first state in which a longitudinal compressive force acts on a specific portion that is at least a part of the main body, thereby forming an opening surrounded by the first and second bent portions at the entanglement located in the specific portion, forming a cover film that covers the main body in the first state and has a blocking portion that closes the opening, blocking the opening with the blocking portion of the cover film, and placing the main body in a second state in which no longitudinal compressive force acts.
- This stent manufacturing method forms a cover film having a blocking portion that closes the opening formed in the entangled portion in the first state. Therefore, this method can prevent holes from forming in the cover film when the stent transitions from the second state to the first state.
- forming the cover film may include forming a liquid film of a raw material liquid for the cover film around the main body portion and solidifying the liquid film to form the cover film. This method can reliably and efficiently form a cover film having a blocking portion that blocks the opening formed in the entangled portion in the first state.
- forming the cover film may include forming the cover film while a core rod having an outer diameter larger than the inner diameter of the main body portion is inserted into the hollow of the main body portion. This method can easily maintain the first state of the main body portion when forming the cover film, and can more efficiently form a cover film having a blocking portion that blocks the opening formed in the entanglement portion in the first state.
- the main body portion may have a first length in the first state and a second length longer than the first length in the second state. This method can reliably and efficiently form a cover film having a blocking portion that blocks the opening formed in the entangled portion in the first state.
- the first length may be 70% or more and 95% or less of the second length. This method can more reliably and efficiently form a cover film having a blocking portion that blocks the opening formed in the entangled portion in the first state.
- a liquid film of a raw material liquid for the cover film may be formed around the main body portion in the first state, and the cover film may be formed by solidifying the liquid film in the second state. This method can effectively prevent holes from occurring in the cover film when the cover film transitions from the second state to the first state.
- the technology disclosed in this specification can be realized in various forms, such as a stent, a method for manufacturing a stent, etc.

- the stent 10 of this embodiment is a self-expanding stent that contracts in diameter when a compressive force is applied in the radial direction, and elastically expands to its pre-compressed state when the compressive force is released.
- Figure 1 shows the stent 10 when expanded.
- the stent 10 is a so-called covered stent, and comprises a main body 100 and a cover membrane 200.
- the cover membrane 200 is indicated by a dashed line.
- the main body portion 100 is a portion that constitutes the framework of the stent 10.

- the main body portion 100 is a tubular member extending along the central axis Ax.
- the main body portion 100 is formed from braided wire W.
- the main body portion 100 is braided using a so-called hook braid.
- the main body portion 100 has a regular arrangement of cells CE, which are approximately diamond-shaped voids surrounded by wire W.
- the wire W is formed from a superelastic alloy such as a nickel-titanium (Ni-Ti) alloy.
- the wire W may be formed from other metals such as stainless steel, tantalum, titanium, cobalt-chromium alloy, magnesium alloy, etc., or from resins such as polyolefin, polyester, fluororesin, etc.

- the reference state is an example of a second state.
- the main body 100 has multiple intersections 102 and multiple entanglement portions 101.
- the intersections 102 are locations where the wires W intersect without becoming entangled with each other.
- the entanglement portion 101 is a location where a first bent portion 141, which is a bent portion of the wire W, and a second bent portion 142, which is also a bent portion of the wire W, intersect in a hook-like shape and become entangled with each other.
- the first bent portion 141 is, for example, a generally V-shaped portion that is convex upward.
- the second bent portion 142 is, for example, a generally V-shaped portion that is convex downward.
- the entanglement portion 101 in the standard state, does not have an opening surrounded by the first bent portion 141 and the second bent portion 142.
- the first bent portion 141 and the second bent portion 142 intersect at one location.
- the first bent portion 141 and the second bent portion 142 are connected in an inseparable manner but are capable of relative movement. This allows the main body portion 100 to be compressed or bent in the longitudinal direction.
- Protrusions 130 are formed at the upper and lower ends of the main body portion 100.
- the protrusions 130 are approximately V-shaped portions formed by the wire W.

- the length L100a of the main body 100 in the standard state is an example of the second length.
- the cover film 200 is a film that covers the main body 100. In this embodiment, the cover film 200 covers almost the entire main body 100.
- the cover film 200 is formed from a resin material such as silicone resin or polyurethane resin.

- the bent state is an example of a first state.
- the compressed portion CP is an example of a specific portion.
- the first bent portion 141 and the second bent portion 142 move relatively away from each other in the entangled portion 101 located at the compressed portion CP, forming an opening 11 surrounded by the first bent portion 141 and the second bent portion 142. That is, the first bent portion 141 and the second bent portion 142 intersect at two locations in the entangled portion 101.
- This opening 11 is an example of a first-form opening
- the area SO1 of this opening 11 is an example of a first opening area.
- the cover film 200 has a blocking portion 210 that blocks the opening 11 formed in the entangled portion 101 located at the compressed portion CP in the bent state.
- the periphery of the blocking portion 210 is fixed to the first bent portion 141 and the second bent portion 142.
- the blocking portion 210 is in a state with little slack.
- a state with little slack includes a state with no slack.
- the blocking portion 210 of the cover film 200 has slack in the reference state.
- the slack of the blocking portion 210 in the reference state is greater than the slack of the blocking portion 210 in the bent state.
- the slack of the blocking portion 210 in the entangled portion 101 is the difference obtained by subtracting the area of the region surrounded by the first bent portion 141 and the second bent portion 142 from the surface area of the blocking portion 210.
- the above-mentioned "area of the region” corresponds to the area of the opening 11, and when an opening 11 is not formed in the entangled portion 101, the above-mentioned "area of the region” is zero.
- the main body portion 100 of the stent 10 can also be compressed and deformed in the longitudinal direction.
- a longitudinal compressive force acts on the entire main body portion 100.
- the entire main body portion 100 becomes the compressed portion CP.
- an opening 11 surrounded by the first bent portion 141 and the second bent portion 142 is formed in the tangled portion 101 located in the compressed portion CP, and the occlusion portion 210 of the cover film 200 blocks the opening 11.
- the compressed state is an example of the first state.
- FIG. 3 is a flowchart showing a method for manufacturing the stent 10 in the first embodiment.
- Figs. 4 to 11 are explanatory views that schematically show the method for manufacturing the stent 10 in the first embodiment.
- the worker prepares the main body 100 (S110, Figure 4).
- the main body 100 is a tubular member formed from braided wire W.
- the main body 100 has multiple entangled portions 101 in which first bent portions 141 and second bent portions 142 of the wire W are entangled with each other.
- the main body 100 In a standard state in which no longitudinal compressive force is acting on the entire main body 100, the main body 100 has a length L100a.
- the worker inserts the core rod 30 into the hollow of the main body 100 (S120, Figure 5).
- the outer diameter of the core rod 30 is larger than the inner diameter of the main body 100. Therefore, once the core rod 30 is inserted, the main body 100 expands in diameter, and the main body 100 is supported by the core rod 30.
- the outer diameter of the core rod 30 may be, for example, 1.05 to 1.5 times the inner diameter of the main body 100, or 1.1 to 1.4 times the inner diameter of the main body 100.
- the difference between the outer diameter of the core rod 30 and the inner diameter of the main body 100 may be, for example, 0.3 mm to 3 mm, or 0.5 mm to 2 mm.
- Examples of materials that can be used to form the core rod 30 include fluororesin (such as PTFE), glass, and stainless steel.
- fluororesin such as PTFE
- glass such as glass
- stainless steel When silicone resin is used as the material for forming the cover film 200, PTFE may be used as the material for forming the core rod 30 from the perspective of ease of peeling.
- the worker compresses the main body portion 100 in the longitudinal direction (S130, Figure 6). This places the main body portion 100 in the compressed state described above, and an opening 11 is formed in the entangled portion 101 of the main body portion 100.
- the length L100b of the main body portion 100 in the compressed state is shorter than the length L100a of the main body portion 100 in the reference state.
- the length L100b of the main body portion 100 in the compressed state is, for example, 70% to 95% of the length L100a of the main body portion 100 in the reference state, or may be 80% to 93% of the length L100a, or 85% to 90% of the length L100a.
- the outer diameter of the core rod 30 is larger than the inner diameter of the main body portion 100, and therefore the compressed state of the main body portion 100 is maintained even after the worker stops applying compressive force to the main body portion 100.
- the length L100b of the main body 100 in the compressed state is an example of the first length.
- the worker immerses the main body portion 100 supported by the core rod 30 in the raw material liquid 40 for the cover film 200 (S140, Figure 7).
- the material of the raw material liquid 40 is set according to the material used to form the cover film 200, and for example, a solution of silicone resin or polyurethane resin is used.
- the worker removes the main body portion 100 supported by the core rod 30 from the raw material liquid 40 ( Figure 8). This causes a liquid film 200a of the raw material liquid 40 to form around the main body portion 100 and the core rod 30.
- the compressed state of the main body portion 100 is maintained during the formation of this liquid film 200a, so the formed liquid film 200a tightly covers the opening 11 formed in the entangled portion 101 of the main body portion 100.
- the worker pulls the end of the main body 100 to return the length of the main body 100 to its original length L100a and return it to the standard state (S150, Figure 9).
- the portion of the liquid film 200a that formed to cover the opening 11 of the entangled portion 101 in the compressed state described above is in a slack state.
- the worker dries the liquid film 200a (S160).
- the worker uses a rotation device to rotate the main body 100 supported by the core rod 30 while air-drying the liquid film 200a. This makes it possible to make the thickness of the liquid film 200a uniform.
- the worker heats the liquid film 200a (S170, Figure 10).
- the worker uses a vacuum oven to heat the liquid film 200a formed on the main body 100 supported by the core rod 30.
- the heating temperature of the liquid film 200a is, for example, 80 degrees or higher and 200 degrees or lower.
- a reaction occurs in the liquid film 200a, and the cover film 200 is formed.
- a stent 10 is produced in which the main body 100 is covered with the cover film 200.
- the worker cuts off both ends of the cover film 200 (S180, Figure 11). For example, the worker uses a feather blade to cut off any unnecessary portions of the cover film 200 that protrude from the main body portion 100.
- the stent 10 is manufactured using the above manufacturing method.
- the occlusion portion 210 of the cover film 200 is loose in the standard state, so that in the compressed and bent states, the occlusion portion 210 of the cover film 200 closes the opening 11 formed in the entangled portion 101.

- the cover film 200 has a blocking portion 210 that blocks the opening 11 formed in the entangled portion 101 in a bent state in which a compressive force acts on the compressed portion CP of the main body portion 100. Therefore, the stent 10 of this embodiment can prevent holes from forming in the cover film 200 as it transitions from the standard state to the bent state, and can prevent bodily fluids from leaking through these holes.
- the periphery of the occlusion portion 210 of the cover membrane 200 is fixed to the first bent portion 141 and the second bent portion 142.

- the manufacturing method also includes compressing the main body portion 100 in a state in which a longitudinal compressive force acts on the compressed portion CP of the main body portion 100, thereby forming an opening 11 surrounded by the first bent portion 141 and the second bent portion 142 in the entangled portion 101 located in the compressed portion CP, forming a cover film 200 that covers the main body portion 100 in the compressed state and has an occlusion portion 210 that blocks the opening 11, and returning the main body portion 100 to a reference state in which no longitudinal compressive force acts.
- the term "forming the cover film 200” means performing at least one step for forming the cover film 200 and is not necessarily limited to completing the formation of the cover film 200.
