Abstract

Description
本発明は、卵巣癌に対するPARP阻害剤の有効性を検査する方法等に関する。
近年、日本においては卵巣癌治療薬としてPARP阻害剤が導入されている。現在、PARP阻害剤の選択基準は、遺伝性卵巣癌陽性であること、又はMyChoice(商標)診断システム(非特許文献1)陽性であること、である。しかしながら、70%前後の患者がいずれの選択基準も陰性であるし、また、myChoiceは乳癌患者データで作成されたアルゴリズムであり、陰性であってもPARP阻害剤が有効である患者や、陽性であってもPARP阻害剤が有効でない患者が存在することが報告されている。

本発明は、卵巣癌に対するPARP阻害剤の有効性を検査する方法を提供することを課題とする。
本発明者は上記課題に鑑みて鋭意研究を進めた結果、(1)卵巣癌を有する被検体から採取された生体試料における、(A)CTNNB1、GABRA6、MYC、及びRB1の4種の遺伝子A、及び/又は(B)CTNNB1、GABRA6、MYC、RB1、ARID1A、BRAF、NOTCH3、PIK3CA、BRCA2、CSMD3、NF1、PTEN、PUM3、ELP4、JAG1、BARD1、MSH2、RPP2R1A、及びMDM4からなる群より選択される少なくとも1種の遺伝子Bのコピー数を測定する工程を含む、卵巣癌に対するPARP阻害剤の有効性を検査する方法、であれば、上記課題が解決できることを見出した。本発明者はこの知見に基づいてさらに研究を進めた結果、本発明を完成させた。即ち、本発明は、下記の態様を包含する。

本発明によれば、卵巣癌に対するPARP阻害剤の有効性を検査する方法を提供することができる。
本明細書中において、「含有」及び「含む」なる表現については、「含有」、「含む」、「実質的にからなる」及び「のみからなる」という概念を含む。
1.検査方法
本発明は、その一態様において、(1)卵巣癌を有する被検体から採取された生体試料における、(A)CTNNB1、GABRA6、MYC、及びRB1の4種の遺伝子A、及び/又は(B)CTNNB1、GABRA6、MYC、RB1、ARID1A、BRAF、NOTCH3、PIK3CA、BRCA2、CSMD3、NF1、PTEN、PUM3、ELP4、JAG1、BARD1、MSH2、RPP2R1A、及びMDM4からなる群より選択される少なくとも1種の遺伝子Bのコピー数を測定する工程を含む、卵巣癌に対するPARP阻害剤の有効性を検査する方法(本明細書において、「本発明の検査方法」と示すこともある。)に関する。以下、これについて説明する。
本発明は、その一態様において、(1)卵巣癌を有する被検体から採取された生体試料における、(A)CTNNB1、GABRA6、MYC、及びRB1の4種の遺伝子A、及び/又は(B)CTNNB1、GABRA6、MYC、RB1、ARID1A、BRAF、NOTCH3、PIK3CA、BRCA2、CSMD3、NF1、PTEN、PUM3、ELP4、JAG1、BARD1、MSH2、RPP2R1A、及びMDM4からなる群より選択される少なくとも1種の遺伝子Bのコピー数を測定する工程を含む、卵巣癌に対するPARP阻害剤の有効性を検査する方法(本明細書において、「本発明の検査方法」と示すこともある。)に関する。以下、これについて説明する。
1-1.工程(1)
卵巣癌としては、特に制限されず、例えば表層・上皮性・間質性悪性腫瘍(例えば、漿液性(嚢胞)腺癌、粘液性(嚢胞)腺癌、類内膜腺癌、明細胞腺癌、腺癌線維腫(上記の各型)、腺肉腫、中胚葉性混合腫瘍、[ミューラー管混合腫瘍][癌肉腫]、悪性ブレンナー腫瘍、移行上皮癌、未分化癌等)、性索間質性腫瘍(例えば、線維肉腫、セルトリ・間質細胞腫瘍(低分化型)等)、胚細胞腫瘍(例えば、未分化胚細胞種、卵黄嚢腫瘍[内胚葉洞腫瘍]、胎芽性癌[胎児性癌]、多胎芽腫、絨毛癌、悪性転化を伴う成熟嚢胞性奇形腫、未熟奇形腫(G3)等)、癌腫、肉腫、悪性リンパ腫(原発性)、二次性[転移性]腫瘍等が挙げられる。卵巣癌の進行度、状態等に関する各種分類基準における全てのクラス、グレード、ステージの卵巣癌が検査対象となり得る。
卵巣癌としては、特に制限されず、例えば表層・上皮性・間質性悪性腫瘍(例えば、漿液性(嚢胞)腺癌、粘液性(嚢胞)腺癌、類内膜腺癌、明細胞腺癌、腺癌線維腫(上記の各型)、腺肉腫、中胚葉性混合腫瘍、[ミューラー管混合腫瘍][癌肉腫]、悪性ブレンナー腫瘍、移行上皮癌、未分化癌等)、性索間質性腫瘍(例えば、線維肉腫、セルトリ・間質細胞腫瘍(低分化型)等)、胚細胞腫瘍(例えば、未分化胚細胞種、卵黄嚢腫瘍[内胚葉洞腫瘍]、胎芽性癌[胎児性癌]、多胎芽腫、絨毛癌、悪性転化を伴う成熟嚢胞性奇形腫、未熟奇形腫(G3)等)、癌腫、肉腫、悪性リンパ腫(原発性)、二次性[転移性]腫瘍等が挙げられる。卵巣癌の進行度、状態等に関する各種分類基準における全てのクラス、グレード、ステージの卵巣癌が検査対象となり得る。
被検体は、本発明の検査方法の対象生物であり、その生物種は特に制限されない。被検体の生物種としては、例えばヒト、サル、マウス、ラット、イヌ、ネコ、ウサギなどの種々の哺乳類動物が挙げられ、好ましくはヒトが挙げられる。
被検体の状態は、卵巣癌を有する限り特に制限されない。被検体としては、例えばPARP阻害剤が有効であるかどうか不明な検体、PARP阻害剤が有効であると既に別の方法により判定されている検体、PARP阻害剤が有効ではないと既に別の方法により判定されている検体、PARP阻害剤の治療中の検体等が挙げられる。
生体試料は、生体に由来し、且つ卵巣癌由来のゲノムDNAを含有し得るものであれば特に制限されない。生体試料としては、例えば全血、血清、血漿、腹水、卵胞液、経血、唾液、髄液、関節液、尿、組織液、汗、涙、唾液などの体液や、これらの体液由来の試料が挙げられる。また、生体試料としては、生体組織、好ましくは卵巣癌組織や、これらの組織由来の試料であることもできる。体液/組織由来の試料としては、体液/組織から調製される試料である限り特に制限されず、例えば体液/組織から、該体液/組織に含まれる核酸又は細胞外小胞を濃縮、精製などして得られる試料などが挙げられる。
体液は、当業者に公知の方法で被検体から採取することができる。例えば、全血は、注射器などを用いた採血によって採取することができる。血清は、全血から血球及び特定の血液凝固因子を除去した部分であり、例えば、全血を凝固させた後の上澄みとして得ることができる。血漿は、全血から血球を除去した部分であり、例えば、全血を凝固させない条件下で遠心分離に供した際の上澄みとして得ることができる。
細胞外小胞は、細胞から分泌、放出等される膜小胞である限り特に制限されない。細胞外小胞は、通常は、細胞内のタンパク質や遺伝情報(mRNA, microRNA等)を細胞外に運搬することにより、局所や全身における細胞間の情報伝達を担っている膜小胞として定義される。細胞外小胞としては、例えばエクソソーム、微小小胞体、アポトーシス小体、エクトソーム、マイクロパーティクル、分泌マイクロベシクル等が挙げられる。検査精度等の観点から、細胞外小胞は、特に好ましくはエクソソームである。
細胞外小胞は、体液から、公知の方法に従って又は準じて、精製、分離、濃縮等することができる。細胞外小胞を精製、分離、濃縮等する方法としては、例えば超遠心法(例えばペレットダウン法、スクロースクッション法、密度勾配遠心法等)、イムノアフィニティー担体を用いる方法、ゲルろ過法、フィールド・フロー分画法、FACS法等が挙げられる。また、細胞外小胞の精製、分離、濃縮等は、市販のキットを用いて行うことも可能である。これらの方法は、1種単独で採用してもよいし、2種以上を組み合わせて採用してもよい。
生体試料は、好ましくは体液から精製された細胞外小胞、細胞外小胞を含有する体液、卵巣癌組織等であることができる。
工程(1)では、(A)CTNNB1、GABRA6、MYC、及びRB1の4種の遺伝子A、及び/又は(B)CTNNB1、GABRA6、MYC、RB1、ARID1A、BRAF、NOTCH3、PIK3CA、BRCA2、CSMD3、NF1、PTEN、PUM3、ELP4、JAG1、BARD1、MSH2、RPP2R1A、及びMDM4からなる群より選択される少なくとも1種の遺伝子B(本明細書において、遺伝子A及び遺伝子Bをまとめて「対象遺伝子」と示すこともある。)のコピー数を測定する。
本発明の一態様において、有効性の予測能が特に高いという観点から、対象遺伝子は遺伝子Aを含むことが特に好ましい。この場合、遺伝子Aにさらに他の遺伝子を組み合わせることにより、有効性の予測能を高めることができる。他の遺伝子としては、遺伝子Bが好ましい。また、他の遺伝子としては、後述の試験例1において、CV scoreが0.6以下であった遺伝子でも良い。このような遺伝子としては、好ましくはAKT2、CCND2、KRAS、RAD51、MLH1等が挙げられる。
対象遺伝子が遺伝子Aを含む場合、対象遺伝子は、さらに
(BX)ARID1A、BRAF、NOTCH3、PIK3CA、BRCA2、CSMD3、NF1、PTEN、PUM3、ELP4、JAG1、BARD1、MSH2、RPP2R1A、及びMDM4からなる群より選択される少なくとも1種の遺伝子BX、及び/又は
(C)AKT2、CCND2、KRAS、RAD51、及びMLH1からなる群より選択される少なくとも1種の遺伝子C、
を含むことが好ましい。好ましい一例として、対象遺伝子は、遺伝子AとBARD1及びRAD51とを含むことができる。
(BX)ARID1A、BRAF、NOTCH3、PIK3CA、BRCA2、CSMD3、NF1、PTEN、PUM3、ELP4、JAG1、BARD1、MSH2、RPP2R1A、及びMDM4からなる群より選択される少なくとも1種の遺伝子BX、及び/又は
(C)AKT2、CCND2、KRAS、RAD51、及びMLH1からなる群より選択される少なくとも1種の遺伝子C、
を含むことが好ましい。好ましい一例として、対象遺伝子は、遺伝子AとBARD1及びRAD51とを含むことができる。
本発明の一態様において、遺伝子Bの中でも、有効性の予測能が高いという観点から、好ましくはCTNNB1、GABRA6、MYC、RB1、ARID1A、BRAF、NOTCH3、PIK3CA、BRCA2等が挙げられ、より好ましくはCTNNB1、GABRA6、MYC、RB1、ARID1A、BRAF、NOTCH3、BRCA2等が挙げられ、特に好ましくはCTNNB1、GABRA6、MYC、RB1、BRCA2等が挙げられル。
本発明の一態様において、有効性の予測能が高いという観点から、対象遺伝子は、CTNNB1、GABRA6、MYC、RB1、ARID1A、BRAF、NOTCH3、PIK3CA、及びBRCA2からなる群(好ましくはCTNNB1、GABRA6、MYC、RB1、ARID1A、BRAF、NOTCH3、及びBRCA2からなる群、より好ましくはCTNNB1、GABRA6、MYC、RB1、及びBRCA2からなる群)より選択される少なくとも2種(好ましくは少なくとも3種、より好ましくは少なくとも4種)の遺伝子を含む。
本発明の一態様において、対象遺伝子、遺伝子Bは、BRCA2を含まない。
本発明の一態様において、対象遺伝子の中でも、PARP阻害剤の有効性予測への寄与度が高いという観点から、好ましくはPIK3CA、RB1、GABRA6、BRCA2、ARID1A、NOTCH3、CTNNB1、BRAF、MYC等が挙げられ、特に好ましくはPIK3CA、RB1、GABRA6、BRCA2、ARID1A等が挙げられる。
工程(1)でコピー数を測定する対象遺伝子の数は、1種以上であれば良いが、検査精度等の観点から、好ましくは2種以上、3種以上、4種以上、5種以上、6種以上、7種以上、8種以上、9種以上、10種以上、11種以上、12種以上、13種以上、14種以上、15種以上、16種以上、17種以上、18種以上、又は19種以上であることができる。また、効率性の観点からは、対象遺伝子の数は、例えば10種以下、8種以下、6種以下、又は5種以下であることが好ましい。
PIK3CA遺伝子は公知であり、ヒトの場合であれば、例えばNCBIGene ID:5290の遺伝子が該当する。当該遺伝子の塩基配列は、公知の遺伝子情報に基づいて把握することができる。当該遺伝子は、個体間で認められる変異を含むものも包含される。
RB1遺伝子は公知であり、ヒトの場合であれば、例えばNCBIGene ID:5925の遺伝子が該当する。当該遺伝子の塩基配列は、公知の遺伝子情報に基づいて把握することができる。当該遺伝子は、個体間で認められる変異を含むものも包含される。
GABRA6遺伝子は公知であり、ヒトの場合であれば、例えばNCBIGene ID:2559の遺伝子が該当する。当該遺伝子の塩基配列は、公知の遺伝子情報に基づいて把握することができる。当該遺伝子は、個体間で認められる変異を含むものも包含される。
BRCA2遺伝子は公知であり、ヒトの場合であれば、例えばNCBIGene ID:675の遺伝子が該当する。当該遺伝子の塩基配列は、公知の遺伝子情報に基づいて把握することができる。当該遺伝子は、個体間で認められる変異を含むものも包含される。

RAD51遺伝子は公知であり、ヒトの場合であれば、例えばNCBIGene ID: 5888の遺伝子が該当する。当該遺伝子の塩基配列は、公知の遺伝子情報に基づいて把握することができる。当該遺伝子は、個体間で認められる変異を含むものも包含される。
MLH1遺伝子は公知であり、ヒトの場合であれば、例えばNCBIGene ID: 4292の遺伝子が該当する。当該遺伝子の塩基配列は、公知の遺伝子情報に基づいて把握することができる。当該遺伝子は、個体間で認められる変異を含むものも包含される。
対象遺伝子のコピー数とは、ゲノムDNA内の対象遺伝子の数を示す。遺伝子のコピー数は、通常は2である。本発明では、卵巣癌における対象遺伝子のコピー数がPARP阻害剤の有効性を予測するための指標になることを見出した。
対象遺伝子のコピー数の測定方法は、特に制限されず、CNV解析手法として用いられている方法を採用することができる。測定方法としては、具体的には、例えばデジタルPCR法、シーケンシング法、リアルタイムPCR法、アレイ比較ゲノムハイブリダイゼーション法、比較ゲノムハイブリダイゼーション法、蛍光in situハイブリダイゼーション法等が挙げられる。これらの中でも、測定精度等の観点から、特に好ましくはデジタルPCR法が挙げられる。
対象遺伝子のコピー数の測定方法には、検出剤として、対象遺伝子に対するプライマー、プローブ等を使用することができる。
検出剤は、その機能が著しく損なわれない限りにおいて、修飾が施されていてもよい。修飾としては、例えば、標識物、例えば蛍光色素、酵素、タンパク質、放射性同位体、化学発光物質、ビオチン等の付加が挙げられる。
本発明において用いられる蛍光色素としては、一般にヌクレオチドを標識して、核酸の検出や定量に用いられるものが好適に使用でき、例えば、HEX(4,7,2’,4’,5’,7’-hexachloro-6-carboxylfluorescein、緑色蛍光色素)、フルオレセイン(fluorescein)、NED(商品名、アプライドバイオシステムズ社製、黄色蛍光色素)、あるいは、6-FAM(商品名、アプライドバイオシステムズ社製、黄緑色蛍光色素)、ローダミン(rhodamin)またはその誘導体〔例えば、テトラメチルローダミン(TMR)〕を挙げることができるが、これらに限定されない。蛍光色素でヌクレオチドを標識する方法は、公知の標識法のうち適当なものを使用することができる〔Nature Biotechnology, 14, 303-308 (1996)参照〕。また、市販の蛍光標識キットを使用することもできる(例えば、アマシャム・ファルマシア社製、オリゴヌクレオチドECL 3’-オリゴラベリングシステム等)。
検出剤は、任意の固相に固定化して用いることもできる。この場合、例えば、検出剤を固定化した基板(例えばプローブを固定化したマイクロアレイチップ等。)を用いてコピー数を測定することができる。
固定化に使用される固相は、ポリヌクレオチド等を固定化できるものであれば特に制限されることなく、例えばガラス板、ナイロンメンブレン、マイクロビーズ、シリコンチップ、キャピラリーまたはその他の基板等を挙げることができる。固相への検出剤の固定は、特に制限されない。固定方法は、例えばマイクロアレイであれば、市販のスポッター(Amersham社製など)を利用するなど、固定化プローブの種類に応じて当該技術分野で周知である〔例えば、photolithographic技術(Affymetrix社)、インクジェット技術(Rosetta Inpharmatics社)によるオリゴヌクレオチドのin situ合成等〕。
プライマーやプローブ等は、対象遺伝子を選択的に(特異的に)認識するものであれば、特に限定されない。
プライマーやプローブの具体例としては、下記(a)に記載するポリヌクレオチド並びに下記(b)に記載するポリヌクレオチド:
(a)対象遺伝子の塩基配列において連続する少なくとも15塩基を有するポリヌクレオチド及び/若しくは当該ポリヌクレオチドに相補的なポリヌクレオチド、並びに
(b)対象遺伝子の塩基配列若しくはそれに相補的な塩基配列にストリンジェントな条件下でハイブリダイズする、少なくとも15塩基を有するポリヌクレオチド
からなる群より選択される少なくとも1種が挙げられる。
(a)対象遺伝子の塩基配列において連続する少なくとも15塩基を有するポリヌクレオチド及び/若しくは当該ポリヌクレオチドに相補的なポリヌクレオチド、並びに
(b)対象遺伝子の塩基配列若しくはそれに相補的な塩基配列にストリンジェントな条件下でハイブリダイズする、少なくとも15塩基を有するポリヌクレオチド
からなる群より選択される少なくとも1種が挙げられる。
相補的なポリヌクレオチド又は相補的な塩基配列(相補鎖、逆鎖)とは、対象遺伝子の塩基配列からなるポリヌクレオチドの全長配列、または該塩基配列において少なくとも連続した15塩基長の塩基配列を有するその部分配列(ここでは便宜上、これらを「正鎖」ともいう)に対して、A:TおよびG:Cといった塩基対関係に基づいて、塩基的に相補的な関係にあるポリヌクレオチド又は塩基配列を意味するものである。ただし、かかる相補鎖は、対象とする正鎖の塩基配列と完全に相補配列を形成する場合に限らず、対象とする正鎖とストリンジェントな条件でハイブリダイズすることができる程度の相補関係を有するものであってもよい。なお、ここでストリンジェントな条件は、Berger and Kimmel (1987, Guide to Molecular Cloning Techniques Methods in Enzymology, Vol. 152, Academic Press, San Diego CA) に教示されるように、複合体或いはプローブを結合する核酸の融解温度(Tm)に基づいて決定することができる。例えばハイブリダイズ後の洗浄条件として、通常「1×SSC、0.1%SDS、37℃」程度の条件を挙げることができる。相補鎖はかかる条件で洗浄しても対象とする正鎖とハイブリダイズ状態を維持するものであることが好ましい。特に制限されないが、より厳しいハイブリダイズ条件として「0.5×SSC、0.1%SDS、42℃」程度、さらに厳しいハイブリダイズ条件として「0.1×SSC、0.1%SDS、65℃」程度の洗浄条件を挙げることができる。具体的には、このような相補鎖として、対象の正鎖の塩基配列と完全に相補的な関係にある塩基配列からなる鎖、並びに該鎖と少なくとも90%、好ましくは95%、より好ましくは98%以上、さらに好ましくは99%以上の同一性を有する塩基配列からなる鎖を例示することができる。
プライマーやプローブ等は、例えば対象遺伝子の塩基配列をもとに、例えば各種設計プログラムを利用して設計することができる。具体的には前記対象遺伝子の塩基配列を設計プログラムにかけて得られる、プライマーまたはプローブの候補配列、若しくは少なくとも該配列を一部に含む配列を、プライマーまたはプローブとして使用することができる。
プライマーやプローブ等の塩基長は、上述のように連続する少なくとも15塩基の長さを有するものであれば特に制限されず、用途に応じて適宜設定することができる。塩基長としては、例えばプライマーとして用いる場合であれば、例えば15塩基~50塩基が例示でき、例えばプローブとして用いる場合であれば、例えば15塩基~150塩基が例示できる。
工程(1)を含む本発明の検査方法によれば、卵巣癌に対するPARP阻害剤の有効性の検査指標である対象遺伝子のコピー数を提供することができ、これにより当該検査などを補助することができる。
1-2.工程(2)
本発明の検査方法は、一態様として、さらに、(2)前記工程(1)で測定された前記遺伝子A及び/又は前記遺伝子Bのコピー数に基づいて、前記被検体の卵巣癌に対するPARP阻害剤の有効性を判定する工程、を含むことが好ましい。

工程(2)は、より具体的には、(2a)前記工程(1)で測定された前記遺伝子A及び/又は前記遺伝子Bのコピー数がカットオフ値以上である場合に、前記被検体の卵巣癌に対してPARP阻害剤が有効であると判定する及び/又は前記被検体に対してPARP阻害剤を投与することを決定する工程、を含むことができる。
カットオフ値は、評価用集団について、対象遺伝子のコピー数と、PARP阻害剤の有効性とについて追跡したデータベースを用意して、PARP阻害剤有効群とPARP阻害剤非有効群との対象遺伝子のコピー数のデータの統計解析又はROC解析に基づいて予め設定される。或いは、カットオフ値は、その都度設定することもできる。前記カットオフ値を統計解析により決定する場合には、例えば、前記評価用集団の対象遺伝子のコピー数のデータの、中央値、算術平均その他の平均値を用いることができる。前記カットオフ値をROC解析により決定する場合には、例えば、ROC解析に基づくカットオフ値は、ROC曲線グラフの縦軸(感度又は真の陽性度)が1.0で、横軸(1-特異度)が0.0の点との距離が最小となるROC曲線上の点の対象遺伝子のコピー数とすることができ、あるいは、ROC曲線のYoudenインデックス(Cancer 1950;3:32-35.)から導かれるカットオフ値とすることができる。評価用集団のデータベースは、一度確立した後は全く変更せずに、カットオフ値の設定に用いてもよい。あるいは、本発明の被検体を含む、新たな卵巣癌患者を前記評価用集団に組み込んで、評価用集団のデータベースを適宜更新しながら、カットオフ値の設定に用いてもよい。

PARP阻害剤としては、ポリ(ADP-リボース)ポリメラーゼ(PARP)を阻害する薬剤である限り、特に制限されない。PARP阻害剤としては、例えばオラパリブ(Olaparib)、ルカパリブ(Rucaparib)、ニラパリブ(Niraparib)、タラゾパリブ(Talazoparib)、アタムパリブ(Atamparib)、カピバセルチブ(Capivasertib)、フルゾパリブ(Fluzoparib)、パミパリブ(Pamiparib)、ステノパリブ(Stenoparib)、ベリパリブ(Veliparib)、ベナダパリブ(Venadaparib)等が挙げられる。また、これら以外にも、例えばAZD9574、CBX15、IMP4297(JS109)、OX401、KT-2000、KT-3000、KT-4000、RP12146、RBN012759(RBN3143)、OPAL0001(OPL0001)等又はこれらと同じ構造からなる化合物も挙げられる。上記PARP阻害剤の中でも、オラパリブ、ニラパリブ等が好ましく、オラパリブがより好ましい。
工程(2)でPARP阻害剤が有効であると判定された被検体及び/又はPARP阻害剤を投与すると決定された被検体には、PARP阻害剤を投与することができる。PARP阻害剤の投与方法は、特に制限されず、各PARP阻害剤について適切な方法(例えば添付文書に記載されている方法)を採用することができる。
2.検査薬
本発明は、その一態様において、対象遺伝子のコピー数の検出剤を含む、卵巣癌に対するPARP阻害剤の有効性の検査薬(本明細書において、「本発明の検査薬」と示すこともある。)、に関する。以下、これについて説明する。

対象遺伝子、検出剤等、前項で説明した内容については、その説明に従う。
本発明の検査薬は、検出剤を含む組成物の形態であってもよい。該組成物には、必要に応じて他の成分が含まれていてもよい。他の成分としては、例えば基剤、担体、溶剤、分散剤、乳化剤、緩衝剤、安定剤、賦形剤、結合剤、崩壊剤、滑沢剤、増粘剤、保湿剤、着色料、香料、キレート剤等が挙げられる。
本発明の検査薬は、検出剤を含むキットの形態であってもよい。該キットには、本発明の検査方法の実施に用いられ得る器具、試薬などが含まれていてもよい。
器具としては、例えば試験管、マイクロタイタープレート、アガロース粒子、ラテックス粒子、精製用カラム、スライドガラスなどが挙げられる。
試薬としては、例えばリファレンス遺伝子の検出剤などが挙げられる。
以下に、実施例に基づいて本発明を詳細に説明するが、本発明はこれらの実施例によって限定されるものではない。
試験例1.卵巣癌に対するPARP阻害剤の有効性の検査1
<1-1.患者の選択>
PARP阻害剤の有効性予測のため、2018年から2021年にPARP阻害剤(オラパリブ)を投与された卵巣癌患者(HGSC(high-grade serious carcinoma:高異型度漿液性癌)患者)を収集した(n=49)。1年以上継続して投与した7名を「Responder」、再発・病勢進行により6ヶ月以内に投与を中止した7名をPARP阻害剤の「Non-responder」と定義した。
<1-1.患者の選択>

<1-2.DNA抽出>
Responder及びNon-responderの卵巣癌組織(冷凍卵巣癌組織又はFFPE組織)から、冷凍卵巣癌組織についてはDNeasy Blood & Tissue Kits (Qiagen)を用いて、FFPE組織についてはQIAamp DNA FFPE Tissue Kit(Qiagen)を用いて、製造元のプロトコルにしたがって全DNAを抽出した。抽出したDNAの濃度はQubit 4.0 Fluorometer (Invitrogen)を用いて定量した。
Responder及びNon-responderの卵巣癌組織(冷凍卵巣癌組織又はFFPE組織)から、冷凍卵巣癌組織についてはDNeasy Blood & Tissue Kits (Qiagen)を用いて、FFPE組織についてはQIAamp DNA FFPE Tissue Kit(Qiagen)を用いて、製造元のプロトコルにしたがって全DNAを抽出した。抽出したDNAの濃度はQubit 4.0 Fluorometer (Invitrogen)を用いて定量した。

<1-4.ドロップレットデジタルPCR>
抽出したDNAについて、選択した30遺伝子のコピー数(CNV:copy number variation)を、QX200 Auto DG droplet digital PCR system(Bio-Rad)を用い、製造者の説明書に従って測定した。11 μL 2x ddPCR Supermix for Probes (Bio-Rad), 1.1 μL FAMラベル化標的プローブ, 1.1 μL HEX ラベル化リファレンスプローブ, 1 μL 制限酵素, 5 μL DNAテンプレート, 及びnuclease-free water を含む22 μL反応液をddPCRアッセイに使用した。リファレンス遺伝子には、ddPCRコピー数測定用RPP30プローブを使用した。ERBB2には制限酵素としてMse Iを用いた。他の標的遺伝子にはHae IIIを使用した。Mse I と Hae III はカットスマートで 5 倍に希釈し、ヌクレアーゼフリー水で 10 倍に希釈した。DNAは少なくとも1ngであった。PCR反応は以下のサイクリング条件で行った。95℃、10分;40サイクル(94℃、30秒、60℃、1分);98℃、10分;12℃ホールド。Bio-Rad QX200 droplet reader (Bio-Rad) でプレートを読み取り、Quanta Soft software (Bio-Rad) を用いて解析した。
抽出したDNAについて、選択した30遺伝子のコピー数(CNV:copy number variation)を、QX200 Auto DG droplet digital PCR system(Bio-Rad)を用い、製造者の説明書に従って測定した。11 μL 2x ddPCR Supermix for Probes (Bio-Rad), 1.1 μL FAMラベル化標的プローブ, 1.1 μL HEX ラベル化リファレンスプローブ, 1 μL 制限酵素, 5 μL DNAテンプレート, 及びnuclease-free water を含む22 μL反応液をddPCRアッセイに使用した。リファレンス遺伝子には、ddPCRコピー数測定用RPP30プローブを使用した。ERBB2には制限酵素としてMse Iを用いた。他の標的遺伝子にはHae IIIを使用した。Mse I と Hae III はカットスマートで 5 倍に希釈し、ヌクレアーゼフリー水で 10 倍に希釈した。DNAは少なくとも1ngであった。PCR反応は以下のサイクリング条件で行った。95℃、10分;40サイクル(94℃、30秒、60℃、1分);98℃、10分;12℃ホールド。Bio-Rad QX200 droplet reader (Bio-Rad) でプレートを読み取り、Quanta Soft software (Bio-Rad) を用いて解析した。
ddPCRでCNV値を測定した後、その状態をCross Validation(CV)を用いた機械学習で解析し、PARP阻害剤反応の寄与度を評価した。Cross Validation(CV)スコアに基づいて30遺伝子をランク付けし、CVスコア>0.6を有意でPARP阻害剤反応予測に寄与していると判断した。結果を表1に示す。
卵巣癌を有する被検体から採取された生体試料における、PIK3CA、RB1、GABRA6、BRCA2、ARID1A、NOTCH3、CTNNB1、BRAF、MYC、CSMD3、NF1、PTEN、PUM3、ELP4、JAG1、BARD1、MSH2、RPP2R1A、及びMDM4からなる群より選択される少なくとも1種の遺伝子のコピー数を測定することにより、卵巣癌に対するPARP阻害剤の有効性を評価できることが分かった。
試験例2.卵巣癌組織のCNVと体液中EVのCNVとの相関
試験例1の卵巣癌患者(n=49)の腹水から細胞外小胞(EV)を単離するために、各患者の腹水サンプル1 mlを使用した。各サンプルは、500 x g、4℃で15分間遠心分離し、浮遊細胞をペレット化した。上清を10,000 x g、40分間、4℃で遠心分離した。ペレットを再懸濁し、再度10,000×g、4℃で40分間遠心分離した。ペレットをPBSに再懸濁し、ラージEV(L-EV)として4℃で保存した。上清を0.22μmフィルターでろ過し、残った大きなベシクルを除去した。ろ過後の培地を超遠心機で100,600×g、4℃、70分間遠心分離した。ペレットをPBSに再懸濁し、再度100,600×g、4℃で70分間遠心分離した。最終ペレットをPBSに再懸濁し、エクソソームを含む小型EV(S-EV)として4℃で保存した。

得られた小型EVサンプル及び卵巣癌患者(n=49)の卵巣癌組織から、試験例1と同様にして、DNAを抽出し、ドロップレットデジタルPCRにより、選択した30遺伝子のコピー数を測定した。
結果を図1に示す。卵巣癌組織におけるCNVと体液中EVにおけるCNVとは相関することが分かった。
試験例3.コピー数に基づいたAUCの解析
試験例1で選別された遺伝子の内の9つの遺伝子(CTNNB1、GABRA6、MYC、RB1、ARID1A、BRAF、NOTCH3、PIK3CA、及びBRCA2)のコピー数を指標として、試験例1のResponderとNon-responderとを判別するROC解析を行った。
各遺伝子のコピー数を指標として用いた場合のROC曲線及びAUCを図2~10に示す。上記9遺伝子の内の4遺伝子のコピー数を組み合わせて指標として用いた場合のROC曲線及びAUCを図11に示す。当該4遺伝子に加えてさらに2遺伝子(試験例1)のコピー数を組み合わせて指標として用いた場合のROC曲線及びAUCを図12に示す。
各遺伝子単独でも比較的高いAUCが得られ、一部の遺伝子は単独でも顕著に高いAUCが得られることが分かった。また、遺伝子を組み合わせることにより、さらに高いAUCが得られることが分かった。
試験例1で選別された遺伝子の内の9つの遺伝子(CTNNB1、GABRA6、MYC、RB1、ARID1A、BRAF、NOTCH3、PIK3CA、及びBRCA2)のコピー数を指標として、試験例1のResponderとNon-responderとを判別するROC解析を行った。
各遺伝子のコピー数を指標として用いた場合のROC曲線及びAUCを図2~10に示す。上記9遺伝子の内の4遺伝子のコピー数を組み合わせて指標として用いた場合のROC曲線及びAUCを図11に示す。当該4遺伝子に加えてさらに2遺伝子(試験例1)のコピー数を組み合わせて指標として用いた場合のROC曲線及びAUCを図12に示す。
各遺伝子単独でも比較的高いAUCが得られ、一部の遺伝子は単独でも顕著に高いAUCが得られることが分かった。また、遺伝子を組み合わせることにより、さらに高いAUCが得られることが分かった。
Claims (10)
- (1)卵巣癌を有する被検体から採取された生体試料における、
(A)CTNNB1、GABRA6、MYC、及びRB1の4種の遺伝子A、及び/又は
(B)CTNNB1、GABRA6、MYC、RB1、ARID1A、BRAF、NOTCH3、PIK3CA、BRCA2、CSMD3、NF1、PTEN、PUM3、ELP4、JAG1、BARD1、MSH2、RPP2R1A、及びMDM4からなる群より選択される少なくとも1種の遺伝子B
のコピー数を測定する工程を含む、卵巣癌に対するPARP阻害剤の有効性を検査する方法。 - 前記工程(A)においてコピー数を測定する遺伝子が前記遺伝子Aを含む、請求項1に記載の方法。
- 前記工程(A)においてコピー数を測定する遺伝子が前記遺伝子A及び/又はBRCA2以外の遺伝子Bを含む、請求項1に記載の方法。
- 前記生体試料が、体液から精製された細胞外小胞、細胞外小胞を含有する体液、又は卵巣癌組織である、請求項1に記載の方法。
- さらに、(2)前記工程(1)で測定された前記遺伝子A及び/又は前記遺伝子Bのコピー数に基づいて、前記被検体の卵巣癌に対するPARP阻害剤の有効性を判定する工程、
を含む、請求項1~4のいずれかに記載の方法。 - 前記工程(2)が、(2a)前記工程(1)で測定された前記遺伝子A及び/又は前記遺伝子Bのコピー数がカットオフ値以上である場合に、前記被検体の卵巣癌に対してPARP阻害剤が有効であると判定する及び/又は前記被検体に対してPARP阻害剤を投与することを決定する工程、
を含む、請求項5に記載の方法。 - 前記被検体がヒトである、請求項1~4のいずれかに記載の方法。
- 前記遺伝子A及び/又は前記遺伝子Bのコピー数の測定方法がデジタルPCR法である、請求項1~4のいずれかに記載の方法。
- (A)CTNNB1、GABRA6、MYC、及びRB1の4種の遺伝子A、及び/又は
(B)CTNNB1、GABRA6、MYC、RB1、ARID1A、BRAF、NOTCH3、PIK3CA、BRCA2、CSMD3、NF1、PTEN、PUM3、ELP4、JAG1、BARD1、MSH2、RPP2R1A、及びMDM4からなる群より選択される少なくとも1種の遺伝子B
のコピー数の検出剤を含む、卵巣癌に対するPARP阻害剤の有効性の検査薬。 - 前記検出剤が、前記遺伝子A及び/又は前記遺伝子Bに対するプローブ又はプライマーである、請求項9に記載の検査薬。
