Description
本発明は、H.スイスの外膜画分を用いたヘリコバクター・スイスの抗体の測定方法に関する。
現在、慢性胃炎、胃潰瘍や十二指腸潰瘍、胃癌、胃Mucosa-Associated Lymphoid Tissue(MALT)リンパ腫、びまん性大細胞型B細胞性リンパ腫などに、ヘリコバクター・ピロリ(Helicobacter pylori、ヒトの胃に寄生する微好気性のグラム陰性螺旋菌、以下、「H.ピロリ」という)による感染が関与することが知られている。H.ピロリの感染検査方法として、分離培養法や尿素呼気試験(UBT)、血清や尿中のH.ピロリ抗体価の測定(ELISAとラッテクス凝集法)、便中のH.ピロリ抗原測定(イムノクロマト法)、胃生検体の迅速ウレアーゼ試験(RUT)が主に採用されている。
しかし、近年、H.ピロリの診断や除菌の普及によるH.ピロリの感染率の低下に伴い、H.ピロリ以外のヒトに重篤な胃疾患を誘発するヘリコバクターとして、NHPH(Non-Helicobacter pylori hericobacters; H.ピロリ以外のヘリコバクター)が問題となってきた。NHPHには、ヘリコバクター・スイス(以下、H.スイス)、H.bizzozeronnii、H.felis、H.salmonis、H.ailurogastricus、H.cynogasticus、H.baculiformis、H.mustelae、H.acinonychls、H.cetorumおよびH.heilmanniiが含まれ、このうちヒトの胃から見つかる大部分は、H.スイスである。
H.ピロリは霊長類にしか感染せず、乳幼児期においてのみ近親者からの感染が想定されるが、H.スイスは年齢に関係なく、豚、猿などの動物から感染する。
H.スイスは約10万年前には、サルの胃に寄生していた。その後1.5万年前にブタにも感染するようになった。ブタの家畜化に伴い感染が爆発的に広まり、ヒトにも感染するようになった(非特許文献1)。世界各地から分離された181株のH.スイス遺伝子のMLST(Multi Locus Sequencing Typing)解析を行ったところ、感染宿主動物により独立のクラスターを形成したが、ヒトから分離した株は、独立のクラスターを形成せず、ブタのクラスターに含まれることから、ヒトへの感染源はブタと判定された(非特許文献2)。そこで、ブタとヒトにおいて感染診断と除菌剤が必要となる。
H.スイスの感染検査方法として、血清中のH.スイス抗体価の測定(ELISAとラッテクス凝集法)が報告されている(特許文献1、2)。

本発明は、H.スイスの外膜画分を用いたヘリコバクター・スイスの抗体の測定方法および測定試薬の提供を目的とする。
特許文献1にはH.スイスのF2R2蛋白をプレートに固相して血清中の抗体を測定することが開示されている。そして、特許文献2にはF2R2蛋白質より抗体価が高いとさされるHsvA蛋白をプレートに固相して血清中の抗体を測定することが開示されている。
しかしながら、従来の方法より感度と特異性の高い方法が求められていた。

今回、本発明者らは、H.スイス外膜画分、または外膜画分に含まれる蛋白質を用いることで、感染者の体内の抗H.スイス抗体の測定感度と特異性を高めることを新規に発見し、新しい測定法を見出した。
より具体的には、本発明は、以下の発明に関する:
[1] 少なくともH.スイス外膜画分、または外膜画分に含まれる蛋白質および抗Ig抗体を含む、被験体由来の検体のH.スイス外膜画分、または外膜画分に含まれる蛋白質と結合する抗体を検出する試薬。
[2] 被験体が哺乳動物であることを特徴とする、[1]の試薬。
[3] 検体が血液由来であることを特徴とする、[1]または[2]の試薬。
[4] さらに、H.ピロリの菌体成分またはH.ピロリ菌体成分に対する抗体を含む、被験体由来の検体のH.ピロリと結合する抗体またはH.ピロリ菌体成分の抗原も検出することを特徴とする、[1]~[3]のいずれかの試薬。
[5] ELISAまたはイムノクロマト用試薬である、[1]~[4]のいずれかの試薬。
[6] 以下の工程を含む、被験体由来の検体の抗H.スイス外膜画分または外膜画分に含まれる蛋白と結合する抗体を検出することを特徴とする方法:
(a)被験体由来の検体をH.スイス外膜画分、または外膜画分に含まれる蛋白質と接触させる工程、および
(b)H.スイス外膜画分、または外膜画分に含まれる蛋白質と結合した、前記検体中の抗体を検出する工程。
[7] 被験体が哺乳動物であることを特徴とする、[6]の方法。
[8] 検体が血液由来であることを特徴とする、[6]または[7]の方法。
[9] さらに、以下の工程を含む、被験体由来の検体のH.ピロリと結合する抗体も検出することを特徴とする、[6]~[8]のいずれかの方法:
(a)被験体由来の検体をH.ピロリ菌体成分と接触させる工程、および
(b)H.ピロリ菌体成分と結合した、前記検体中の抗体を検出する工程。
[10] さらに、以下の工程を含む、被験体由来の検体のH.ピロリ菌体成分の抗原も検出することを特徴とする、[6]~[8]のいずれかの方法:
(a)被験体由来の検体をH.ピロリ菌体成分に対する抗体と接触させる工程、および
(b)H.ピロリ菌体成分に対する抗体と結合した、前記検体中のH.ピロリ菌体成分の抗原を検出する工程。
[11] [1]~[5]のいずれかの試薬を用いた測定および/または[6]~[10]のいずれかの方法により抗体が検出された被験体をH.スイスに感染していると判定する、H.スイスの感染の検出方法。

(a)被験体由来の検体をH.ピロリ菌体成分と接触させる工程、および
(b)H.ピロリ菌体成分と結合した、前記検体中の抗体を検出する工程。
[10] さらに、以下の工程を含む、被験体由来の検体のH.ピロリ菌体成分の抗原も検出することを特徴とする、[6]~[8]のいずれかの方法:
(a)被験体由来の検体をH.ピロリ菌体成分に対する抗体と接触させる工程、および
(b)H.ピロリ菌体成分に対する抗体と結合した、前記検体中のH.ピロリ菌体成分の抗原を検出する工程。
[11] [1]~[5]のいずれかの試薬を用いた測定および/または[6]~[10]のいずれかの方法により抗体が検出された被験体をH.スイスに感染していると判定する、H.スイスの感染の検出方法。

H.スイス外膜画分、または外膜画分に含まれる蛋白質を用いることで、感染者の体内の抗H.スイス抗体を高感度に測定することができる。H.スイス外膜画分、または外膜画分に含まれる蛋白質は、H.スイス感染診断対象体由来血液中の抗H.スイス抗体の有無または抗体価を測定するために使用することができる。
以下、本発明の方法について説明する。
本発明は、被験体の生体試料中のH.スイスの菌体の外膜画分、または外膜画分に含まれる蛋白質に対する抗体を検出する方法、および該抗体を検出する試薬である。本発明は、被験体におけるH.スイスの存在を判定する方法、または被験体のH.スイスの感染の検出方法でもある。あるいは、被験体のH.スイス感染症を診断するための補助的データを取得する方法である。
本発明は、被験体の生体試料中のH.スイスの菌体の外膜画分、または外膜画分に含まれる蛋白質に対する抗体を検出する方法、および該抗体を検出する試薬である。本発明は、被験体におけるH.スイスの存在を判定する方法、または被験体のH.スイスの感染の検出方法でもある。あるいは、被験体のH.スイス感染症を診断するための補助的データを取得する方法である。
(H.スイス:Helicobacter suis)
本明細書において、「H.スイス」とは、50種類以上報告されているヘリコバクター属細菌の中で、NHPH(Non-Helicobacter pylori helicobacters; H.ピロリ以外のヘリコバクター)あるいは、Helicobacter heilmannii sensu lato(広義のヘリコバクター・ハイルマニイ)に含まれるH.スイスの菌種を意味する(非特許文献3)。H.スイスは、ヒト以外にブタ、サル、イノシシ、ネコ、イヌなどの胃に感染することが知られている。本明細書におけるH.スイスが単離された感染哺乳動物は特に限定されるものではなく、例えば、ヒト、サル、イノシシ、またはブタであってよい。

本明細書において、「H.スイス外膜画分」とは、H.スイスを超音波破砕後のH. スイス菌体外膜断片を集めることで調製した画分である。超音波破砕は、H.スイス菌体を緩衝液に懸濁させ超音波破砕機を用いて行うことができる。超音波破砕機としてサンプル密閉型超音波破壊装置 Bioruptor(ビーエム機器)サンプル密閉型超音波破壊装置 Bioruptor(ビーエム機器)が挙げられる。超音波破砕により、H.スイス菌体は破壊され、菌体外膜断片が残る。菌体外膜断片は、遠心分離やろ過により回収することができる。「H.スイス外膜画分に含まれる蛋白質」とは、H.スイスを超音波破砕後のH.スイス菌体外膜断片を集めることで調製した画分に存在するH. スイス由来の蛋白質をいう。

H.スイス外膜画分に対する抗体は、H.スイス外膜画分に含まれる蛋白質、糖等の抗原に対する抗体をいい、種々の物質に対する複数の抗体が含まれ得る。本発明において、H.スイス外膜画分に含まれる蛋白質、糖等の抗原のうち少なくともいずれかの抗原に対する抗体をH.スイス外膜画分に対する抗体という。また、H.スイス外膜画分に含まれる蛋白質に対する抗体は、H.スイス外膜画分に含まれる抗原のうち蛋白質に対する抗体をいう。H.スイス外膜画分には複数の蛋白質が含まれており、H.スイス外膜画分に含まれる蛋白質に対する抗体は、種々の蛋白質に対する複数の抗体が含まれ得る。本発明において、H.スイス外膜画分に含まれる蛋白質のいずれかの蛋白質のうち少なくともいずれかの蛋白質に対する抗体をH.スイス外膜画分に含まれる蛋白質に対する抗体という。実際には、H.スイスが感染した被験体において、体内でH.スイス菌体が分解された結果、被験体中でH.スイス外膜画分に対する抗体、または外膜画分に含まれる蛋白質に対する抗体が産生される。
H.スイス外膜画分に対する抗体は、H.スイス外膜画分に含まれる蛋白質、糖等の抗原に対する抗体をいい、種々の物質に対する複数の抗体が含まれ得る。本発明において、H.スイス外膜画分に含まれる蛋白質、糖等の抗原のうち少なくともいずれかの抗原に対する抗体をH.スイス外膜画分に対する抗体という。また、H.スイス外膜画分に含まれる蛋白質に対する抗体は、H.スイス外膜画分に含まれる抗原のうち蛋白質に対する抗体をいう。H.スイス外膜画分には複数の蛋白質が含まれており、H.スイス外膜画分に含まれる蛋白質に対する抗体は、種々の蛋白質に対する複数の抗体が含まれ得る。本発明において、H.スイス外膜画分に含まれる蛋白質のいずれかの蛋白質のうち少なくともいずれかの蛋白質に対する抗体をH.スイス外膜画分に含まれる蛋白質に対する抗体という。実際には、H.スイスが感染した被験体において、体内でH.スイス菌体が分解された結果、被験体中でH.スイス外膜画分に対する抗体、または外膜画分に含まれる蛋白質に対する抗体が産生される。
なお、抗体がH.スイス外膜画分に含まれる抗原と結合することを、抗体がスイス外膜画分と結合するという。
H.スイス外膜画分、または外膜画分に含まれる蛋白質に対する抗体を検出する試薬は、公知の方法に適用し得る試薬として作製することができる。公知の方法としては、例えば、酵素免疫測定法(EIA法)、簡易EIA法、酵素結合イムノソルベントアッセイ法(ELISA法)、ラジオイムノアッセイ法(RIA法)、蛍光免疫測定法(FIA法)等の標識化免疫測定法;イムノブロッティング法;イムノクロマト法;クロマトグラフィ法;比濁法(TIA法);比ろう法(NIA法);比色法;ラテックス凝集法(LIA法);粒子計数法(CIA法);化学発光測定法(CLIA法、CLEIA法);沈降反応法;表面プラズモン共鳴法(SPR法);レゾナントミラーディテクター法(RMD法);比較干渉法等を挙げることができる。本発明の試薬が、所望の測定法に適用することが可能であるか否かは、H.スイス外膜画分、または外膜画分に含まれる蛋白質に対する抗体を含む検体または該検体と同じ濃度のH.スイス外膜画分、または外膜画分に含まれる蛋白質に対する抗体を含むサンプルを用いて、各測定法を実施することにより、検出可能であるか否かを測定することで確認することができる。
H.スイス外膜画分、または外膜画分に含まれる蛋白質に対する抗体をH.スイス感染のマーカーとして用いることができる。
すなわち、被験体がH.スイスに感染しているか否かは、被験体自身の免疫系が生み出したH.スイス外膜画分、または外膜画分に含まれる蛋白質に対する抗体の存在を確認することにより決定することができる。これらの抗体の存在は、H.スイス外膜画分、または外膜画分に含まれる蛋白質を利用した抗原抗体反応により検出し確認することができる。H.スイス外膜画分、または外膜画分に含まれる蛋白質を測定試薬として利用したH.スイス外膜画分、または外膜画分に含まれる蛋白質に対する抗体の検出は、酵素免疫測定法(EIA法)、簡易EIA法、酵素結合イムノソルベントアッセイ法(ELISA法)、ラジオイムノアッセイ法(RIA法)、蛍光免疫測定法(FIA法)等の標識化免疫測定法;ウェスタンブロッティング法等のイムノブロッティング法;金コロイド凝集法等のイムノクロマト法;イオン交換クロマトグラフィ法、アフィニティクロマトグラフィ法等のクロマトグラフィ法;比濁法(TIA法);比ろう法(NIA法);比色法;ラテックス凝集法(LIA法);粒子計数法(CIA法);化学発光測定法(CLIA法、CLEIA法);沈降反応法;表面プラズモン共鳴法(SPR法);レゾナントミラーディテクター法(RMD法);比較干渉法等の免疫学的測定法により行うことができる。
一態様において、本発明は、以下の工程を含むH.スイス感染の判定方法に関する:
(a)被験者由来の検体を本発明のH.スイス外膜画分、または外膜画分に含まれる蛋白質と接触させること、
(b)H.スイス外膜画分、または外膜画分に含まれる蛋白質と結合した、前記検体中の抗体を検出する工程、および
(c)H.スイス外膜画分、または外膜画分に含まれる蛋白質と結合した抗体が検出された被験体をH.スイスに感染していると判定し、一方、H.スイス外膜画分、または外膜画分に含まれる蛋白質と結合した抗体が検出されなかった被験体を、H.スイスに感染していないと判定する工程。
(a)被験者由来の検体を本発明のH.スイス外膜画分、または外膜画分に含まれる蛋白質と接触させること、
(b)H.スイス外膜画分、または外膜画分に含まれる蛋白質と結合した、前記検体中の抗体を検出する工程、および
(c)H.スイス外膜画分、または外膜画分に含まれる蛋白質と結合した抗体が検出された被験体をH.スイスに感染していると判定し、一方、H.スイス外膜画分、または外膜画分に含まれる蛋白質と結合した抗体が検出されなかった被験体を、H.スイスに感染していないと判定する工程。
他の態様において、本発明は、以下工程を含むH.スイス感染の判定方法に関する:
(a)被験体由来の検体を、H.スイス外膜画分、または外膜画分に含まれる蛋白質と接触させる工程、
(b)H.スイス外膜画分、または外膜画分に含まれる蛋白質と結合した、前記検体中の抗体のレベルを測定する工程、および
(c)測定された抗体のレベルが、同様の方法により陰性対照について測定された抗体のレベルよりも高い場合、該被験体をH.スイスに感染していると判定し、一方、測定された抗体のレベルが、同様の方法により陰性対照について測定された抗体のレベルと同等か低い場合、該被験体を、H.スイスに感染していないと判定する工程。ここで、陰性対象とは、H.スイスに感染していない被験体由来の検体をいう。抗体のレベルとは、抗体の定量値、すなわち抗体価をいい、例えば、検体中の濃度で表される。また、抗体のレベルの測定は、抗体の検出に含まれる。
(a)被験体由来の検体を、H.スイス外膜画分、または外膜画分に含まれる蛋白質と接触させる工程、
(b)H.スイス外膜画分、または外膜画分に含まれる蛋白質と結合した、前記検体中の抗体のレベルを測定する工程、および
(c)測定された抗体のレベルが、同様の方法により陰性対照について測定された抗体のレベルよりも高い場合、該被験体をH.スイスに感染していると判定し、一方、測定された抗体のレベルが、同様の方法により陰性対照について測定された抗体のレベルと同等か低い場合、該被験体を、H.スイスに感染していないと判定する工程。ここで、陰性対象とは、H.スイスに感染していない被験体由来の検体をいう。抗体のレベルとは、抗体の定量値、すなわち抗体価をいい、例えば、検体中の濃度で表される。また、抗体のレベルの測定は、抗体の検出に含まれる。
H.スイス外膜画分、または外膜画分に含まれる蛋白質と被験体由来の検体中の抗体が結合したか否かは、以下に例示される方法で確認すればよい。例えば、H.スイス外膜画分、または外膜画分に含まれる蛋白質を固相化担体に結合させ、被験体由来の検体を、H.スイス外膜画分、または外膜画分に含まれる蛋白質と接触させた後に、反応系を洗浄して非結合の抗体を除去する。この結果、反応系には、H.スイス外膜画分、または外膜画分に含まれる蛋白質と被験体由来の検体中の抗体が結合した複合体が存在する。その後、標識化した抗Ig抗体と接触させて、H.スイス外膜画分、または外膜画分に含まれる蛋白質と結合した被験体由来の抗体と該標識抗Ig抗体とを結合させた後に、反応系を洗浄して非結合の標識抗Ig抗体を除去して、残った標識抗Ig抗体の標識を検出し、該標識抗Ig抗体の標識が検出された場合にH.スイス外膜画分、または外膜画分に含まれる蛋白質と被験体由来の検体中の抗体が結合したと判断することができる。該標識抗Ig抗体のレベルを測定することで、H.スイス外膜画分、または外膜画分に含まれる蛋白質と被験体由来の検体中の抗体との結合レベルを測定することもできる。抗Ig抗体は、抗IgG抗体でも、抗IgM抗体等でもよいが、好ましくは抗IgG抗体である。また、抗IgG抗体は好ましくはモノクローナル抗体を用いる。また、Fab、Fab’、F(ab’)2、一本鎖抗体(scFv)、VHHシングルドメイン抗体(ナノボディ)dsFv、diabody、minibodyなどの特異的な抗原結合性を有する断片(抗原結合性断片)を用いることもできる。このような方法の代表例として、ELISA等やイムノクロマト法を挙げることができる。抗Ig抗体の標識には、アルカリフォスファターゼや西洋ワサビペルオキシダーゼのような酵素、金コロイドのような金属コロイド、シリカ粒子、セルロース粒子、磁性粒子、蛍光粒子、着色ポリスチレン粒子および着色ラテックス粒子等が用いられることが多い。金属コロイド粒子、着色ポリスチレン粒子や着色ラテックス粒子等の着色粒子を用いる場合には、これらの標識試薬が凝集することによって着色が生じるので、この着色を測定する。結合レベルを指標として利用する場合、予め存在量が判明している標準溶液を用いて標準曲線を作成することにより、測定値から抗体量を計算することができる。あるいは、表面プラズモン共鳴センサーを用いて、H.スイス外膜画分、または外膜画分に含まれる蛋白質と被験体由来の検体中の抗体の結合を検出または測定することもできる。
上記の免疫測定法の中でも、サンドイッチ法が好ましい。サンドイッチ法自体は免疫測定の分野において周知であり、例えばラテラルフロー式に免疫測定を行うイムノクロマト法やELISA法により行うことができる。これらのサンドイッチ法自体はいずれも周知であり、本発明の方法は周知のサンドイッチ法により行うことができる。以下、ELISA法について説明する。
ELISA用プレートに、H.スイス外膜画分、または外膜画分に含まれる蛋白質(抗原)を固相化し、該固相化担体にH.スイス外膜画分、または外膜画分に含まれる蛋白質に対する抗体を含む可能性がある検体を添加して接触させ、固相化担体上で抗原と抗体の複合体を形成させる。さらに、ヒト抗体に対する特異的抗体(抗Ig抗体)であって酵素で標識した抗体を添加して接触させ、固相化担体上の抗原と抗体の複合体に抗Ig抗体を結合させる。次いで、酵素に対する基質を添加し、酵素反応を行わせ、発色を吸光度の測定により測定しプレート上の抗原と抗体のサンドイッチ複合体を検出すればよい。
抗原抗体反応は4℃~45℃、好ましくは20℃~40℃、さらに好ましくは25℃~38℃で行うことができ、また、それぞれの結合反応の反応時間は、10分~18時間、より好ましくは10分~3時間、さらに好ましくは30分~2時間程度である。
H.スイス外膜画分、または外膜画分に含まれる蛋白質(抗原)の担体への結合は、物理的吸着、官能基を利用した共有結合等、公知の方法で行うことができる。固相化量は、特に限定されないが担体が96ウェルマイクロタイタープレートの場合、1ウエル当たり数ngから数十μgが望ましい。固相化は固相化すべきH.スイス外膜画分、または外膜画分に含まれる蛋白質(抗原)の溶液を担体と接触させることにより行うことができる。例えば、H.スイス外膜画分、または外膜画分に含まれる蛋白質(抗原)の溶液をマイクロタイタープレートのウエルに分注し一定時間置くことにより固相化することができる。H.スイス外膜画分、または外膜画分に含まれる蛋白質(抗原)を固相化した後は、アッセイ中の非特異的結合を防ぐためにウシ血清アルブミン、ヒト血清アルブミン、ウサギ血清アルブミン、卵白アルブミン等を含んだブロッキング溶液を用いてブロッキングを行うことが好ましい。
H.スイスは広く哺乳動物に感染することから、本発明のH.スイスの存在を判定する方法及びH.スイスへの感染を判定する方法における被験体は、ヒト、サル、ブタ、ネコ、イノシシ、イヌ、ウサギ、ネズミ及びヒツジ等の哺乳動物とすることができ、好ましくは、ヒトである。また、より好ましくは、胃炎、慢性胃炎、鳥肌胃炎、A型胃炎、胃MALTリンパ腫、びまん性大細胞型B細胞性リンパ腫、胃癌、胃・十二指腸潰瘍、特発性血小板減少紫斑病、機能性ディスペプシア、またはパーキンソン病に罹患しているヒト患者である。また、本発明の方法は、定性的、定量的または半定量的に行うことができる(非特許文献4)。
本発明のH.スイスの存在を判定する方法において用いる検体としては、例えば、生検として被験体から採取した組織試料または被験体から採取した液体試料を使用することができる。検体は、本発明の方法の対象として用い得るものであれば特に限定はなく、例えば、組織、血液、血漿、血清、リンパ液、尿、便、漿液、髄液、関節液、眼房水、涙液、唾液またはそれらの分画物若しくは処理物を挙げることができる。本発明のH.スイスの存在を判定する方法において抗体を検出対象とする場合には、好ましい検体は、血液、血漿、血清、リンパ液、及び尿である。
本発明は、検体中のH.スイス外膜画分、または外膜画分に含まれる蛋白質に対する抗体を測定するためのキットを包含し、該キットは少なくともH.スイス外膜画分、または外膜画分に含まれる蛋白質を固相化した担体を含み、さらに、標識した抗Ig抗体を含む。
本発明のH.スイス外膜画分、または外膜画分に含まれる蛋白質は、本明細書の開示を参照して当業者周知の方法により適宜調製することができる。また、本発明の試薬は、当該技術分野周知の方法により、適宜製造することができる。
一態様において、本発明は、検体中の、H.スイス外膜画分、または外膜画分に含まれる蛋白質に対する抗体を検出する方法を含む、当該検体を採取して被験体におけるH.スイスの存在を判定する方法に関する。特に、本発明は、H.スイス外膜画分、または外膜画分に含まれる蛋白質に対する抗体を検出することを含む、被験体におけるH.スイスの感染を判定する方法に関する。本方法において、H.スイス外膜画分、または外膜画分に含まれる蛋白質に対する抗体が検出された検体を採取した被験体は、それぞれH.スイスが存在する、またはH.スイスに感染していると判定される。
本発明の方法が、前記検体中の抗体と、被検試薬との結合を測定することにより行われる場合、判定において「検出された」か否かは、絶対的な検出の有無である必要はなく、他の検体との比較において決定してもよい。すなわち、±の検出結果に基づくものではなく、測定値から検出の有無を決定してもよい。すなわち、本発明の方法において、「検出する」工程は、必要に応じて「測定する」と置き換えることができ、「検出された」か否かは、対象物質の測定値に基づき陰性対象との比較において決定してもよい。例えば、陰性対象、即ち、H.スイスを含まない検体またはH.スイスに感染していないことが明らかな被験体由来の検体等において目的の物質が検出される場合、検被体の測定値が当該陰性対象の測定値と同等であれば、微量ながら検出されていても、本発明の方法においては「検出されなかった」としてH.スイスが存在しない、またはH.スイスに感染していないと判定される。一方で、陰性対象の測定値と比較して、被験体の測定値が高い場合には、「検出された」としてH.スイスが存在する、またはH.スイスに感染していると判定される。よって、本発明の方法において、陰性である対象についてわずかな測定値が認められることは、本発明が予め許容する範囲内である。
本発明の方法においては、H.スイス感染者とH.スイス非感染者の検体中の抗体の抗体価を測定し、カットオフ値を設定することができる。カットオフ値以上であれば、H.スイスが存在する、またはH.スイスに感染していると判定することが可能である。
カットオフ値は、例えば、ROC(receiver operating characteristic curve:受信者動作特性曲線)解析により定めることができる。また、ROC解析により本発明の方法による診断精度(感度及び特異度)を決定することができる。ROC解析は、H.スイス感染者から採取した検体とH.スイス非感染者から採取した検体について抗H.スイス抗体を測定し、各カットオフ値での感度及び偽陽性率(1-特異度)を算出し、横軸を(1-特異度)とし、縦軸を感度とした座標上にプロットする。ROC解析により診断精度を解析した場合の感度は80%以上、好ましくは85%以上、さらに好ましくは90%以上であり、特異度は75%以上、好ましくは80%以上であることが良い。
(H.ピロリ(H.pylori)に対する抗体またはH.ピロリ抗原の検出)
本発明は同一の被験体においてH.スイス外膜画分、または外膜画分に含まれる蛋白質と結合する抗体とH.ピロリに対する抗体を検出する方法および該方法に用いる試薬も包含する。ここで、H.ピロリに対する抗体は、H.ピロリの全菌体のいずれかの構成成分、すなわち菌体成分に対する抗体をいう。例えばH.ピロリの検査で陰性だが胃疾患の症状が示される場合は、H.スイスの感染が原因の可能性が考えられる。同一の被検体からH.スイス外膜画分、または外膜画分に含まれる蛋白質と結合する抗体とH.ピロリに対する抗体を検出することで、H.ピロリの感染の有無だけではなく、これまでH.ピロリ陰性で見逃されていた被検体に対してもH.スイス感染の確認をすることができる。早期に感染を確認することで、適切な治療方針を定めることができる。同一の被験体由来の検体はH.ピロリとH.スイスで同じであっても、異なってもよい。すなわち、H.スイス外膜画分、または外膜画分に含まれる蛋白質と結合する抗体の検出とH.ピロリに対する抗体の検出は、同じ検体を用いて同時に行ってもよいし、別の時に採取された異なる検体を用いて別途行ってもよい。H.ピロリに対する抗体の検出は、H.スイス外膜画分、または外膜画分に含まれる蛋白質に対する抗体の検出と同様の方法で行えばよい。

本発明は、検体中のH.スイス外膜画分、または外膜画分に含まれる蛋白質に対する抗体、およびH.ピロリに対する抗体を測定するためのキットを包含し、該キットは少なくともH.スイス外膜画分、または外膜画分に含まれる蛋白質を固相化した担体およびH.ピロリ全菌体のいずれかの構成成分を固相化した担体を含み、さらに、標識した抗ヒトIg抗体を含む。
さらに、被験体由来の試料中のH.ピロリ抗原を検出してもよい。H.ピロリ抗原の検出によってもH.ピロリの感染を検出することができる。H.ピロリ抗原を検出するための試料は糞便が好ましい。本発明は、検体中のH.スイス外膜画分、または外膜画分に含まれる蛋白質に対する抗体、およびH.ピロリ抗原を測定するためのキットを包含し、該キットは少なくともH.スイス外膜画分、または外膜画分に含まれる蛋白質を固相化した担体およびH.ピロリに対する抗体を固相化した担体を含み、さらに、標識した抗ヒトIg抗体を含む。
以下に実施例を挙げて本発明を具体的に説明するが、本発明はこれに限定されるものではない。なお、本願全体を通して引用される全文献は参照によりそのまま本願に組み込まれる。
(実施例1)ヒト検体の測定
ELISA(EnzymeLinked ImmunoSorbent Assay ;酵素免疫測定法)を用いた感染者(ヒト)の抗H.スイス抗体価の測定

ブルセラブロス(BD BBL) 2.8 g

非働化牛胎児血清(FBS) 20 mL
蒸留水を加えて、全量を100 mLとした。
75cm2のフラスコに、寒天培地を9mL加え斜面培地を作製し、-80℃保存の鳥肌胃炎の患者から分離したH.スイス SNTW101c株(非特許文献2)を植菌し、液体培地を12mL加え、温度37℃,湿度100%,微好気条件(5%O2,12%CO2,83%N2)で、振とう培養を1週間行った。その後、培養液を遠心分離(13,420 G x 10分間)により菌体を集めた。菌体はPBS(リン酸緩衝生理食塩水、pH7.4)で2回洗浄後、蒸留水に懸濁し、氷冷条件下で5分間の超音波破砕処理を行った(Bioruptor II超音波細胞破壊装置(設定High)で、30秒間の音波処理と30秒間の休止を5回繰り返す)。破壊液を「H.スイス全菌体懸濁液」とし、遠心分離(30,190 G x 10分間)により沈渣を「H.スイス菌体外膜画分」とし、蒸留水に懸濁した。蛋白質の定量には、BSAを標準蛋白質としてBio-Rad Protein Assayキットを用いた。
0.1M炭酸-重炭酸緩衝液(pH9.4)に溶解したH.スイス全菌体懸濁液またはH.スイス菌体外膜画分(4μg/mL)100μL/wellを96-well NUNC ImmunoPlate #439454へ滴下し、4℃に一晩置いた。翌日菌体溶液を廃棄し、PBS-T(0.05%(V/V)Tween20を含むPBS)200μL/wellで3回洗浄した。Blocker BSA(ウシ血清アルブミン)10X in PBS(Thermo Scientific)より調製したブロッキング溶液(1%(V/V)BSA、PBS系pH7.4)を200μL/well滴下し、37℃に1時間置いた。ブロッキング溶液を廃棄し、PBS-T 200μL/wellで3回洗浄した。H.ピロリ感染者(既存の検査法(尿素呼気試験や抗体検査、抗原検査など)で判定)、およびH.スイス感染者(被検者の胃生検体からDNAを調製し、特許文献2に記載のリアルタイムPCR法あるいは胃生検体からの培養によりH.スイス感染を判定)から血清をそれぞれ採取した。また対照として、いずれの菌にも感染していない健常人からの血清を採取した(非感染)。ブロッキング溶液を用いて希釈したヒト血清を50μL/well滴下し、37℃に1時間置いた。血清溶液を廃棄し、PBS-T 200μL/wellで3回洗浄した。ブロッキング溶液を用いて100,000倍希釈した西洋わさびペルオキシダーゼ標識二次抗体(Goat anti-Human IgA+IgG+IgM(H+L),Jackson ImmunoResearch, Inc.)を50μL/well滴下し、37℃で1時間置いた。二次抗体溶液を廃棄し、PBS-T 200μL/wellで3回洗浄した。SuperBlue TMB Microwell Peroxidase Substrate(1-Component),Kirkegaard&Perry Laboratories,Inc.(KPL)を50μL/well滴下し青く発色後に、1N塩酸を50μL/well滴下し黄色へ変色させた。プレートリーダーを用いて450nm(リファレンス波長:620nm~630nm)の吸光度を測定した。
ヒト血清(3600倍希釈)を用いたELISAの結果を図1および図2に示す。H.スイス陽性の対象はH.スイス全菌体およびH.スイス可溶化画分のいずれの抗原に対しても吸光度が得られたが、H.スイス可溶化画分でより高い吸光度が得られた。また、H.スイスに感染していないH.ピロリ感染者および非感染者はいずれの抗原に対する吸光度も低かった。このことから、H.スイスに感染したヒト血液中にH.スイス可溶化画分に対する抗体が産生されていること、および、H.スイス可溶化画分を抗原として用いることで、高感度に抗体を捉えられることが初めて示された。これにより、H.スイス可溶化画分を用いることにより、ヒトにおいてH.ピロリ感染と区別してH.スイス感染の高感度な診断ができることが明らかとなった。
(実施例2)哺乳動物検体の測定
培養したH.スイス SNTW101c株を遠心分離により菌体を集めた。菌体はPBSに懸濁し、4週齢のメスのC57BL/6マウスへ1x108Colony forming unit(CFU)を1回経口感染投与した。一方、-80℃保存のH.ピロリSS1株をニッスイプレートヘリコバクター寒天培地に植菌し、温度37℃,湿度100%,微好気条件(5%O2,12%CO2,83%N2)で3日培養し、生じたコロニーを100 mLのブルセラブロス(10mLの非働化FBSと0.4mLのSR0147 Helicobacter pylori Selective Supplement(Dent)を含む)で温度37℃,湿度100%,微好気条件(5%O2,12%CO2,83%N2)で3日間振とう培養を行った後に、遠心分離により菌体を集めた。菌体はPBSに懸濁し、4週齢のメスのC57BL/6マウスへ2x108CFUを1日置きに3回経口投与した。H.スイスを感染させたマウス胃粘膜からDNAを抽出し、PCRで感染を確認した。また、H.ピロリを感染させたマウス胃粘膜を緩衝液に懸濁した液をニッスイヘリコバクター寒天培地(日水製薬)に塗布し、培養して感染を確認した。最終感染日から4週間後のH.ピロリ感染マウス、およびH.スイス感染マウスからの血清をそれぞれ採取した。また対照として、いずれにも感染していない健常なマウスからの血清を採取した(非感染)。

マウス血清(3600倍希釈)を用いたELISAの結果を図3および図4に示す。H.スイス陽性の対象はH.スイス全菌体及びH.スイス外膜画分のいずれの抗原に対しても吸光度が得られたが、H.スイス外膜画分でより高い吸光度が得られた。また、H.スイスに感染していないH.ピロリ感染マウス及び非感染マウスはいずれの抗原に対する吸光度も低かった。このことから、H.スイスに感染したマウス血液中にH.スイス外膜画分に対する抗体が産生されていること、及び、H.スイス外膜画分を抗原として用いることで、高感度に抗体を捉えられることが初めて示された。これにより、H.スイス外膜画分を用いることにより、マウスにおいてH.ピロリ感染と区別してH.スイス感染の高感度な診断ができることが明らかとなった。
(実施例3)ヒト検体の測定
ELISA(EnzymeLinked ImmunoSorbent Assay ;酵素免疫測定法)を用いた感染者(ヒト)の抗H.ピロリ抗体価および抗H.スイス抗体価の測定
(1)ELISA用H.ピロリ抗原(H.ピロリ菌体外膜画分)の調製
H.ピロリ TN2GF4株(非特許文献2)は、10%(V/V)牛胎児血清(FCS)を含むブルセラブロスで温度37℃、湿度100%、微好気条件(5%O2、10%CO2、85%N2)で48時間振とう培養を行った後で、培養液を遠心分離(13,420 G x 10分間)により菌体を集めた。菌体はPBSリン酸緩衝生理食塩水、pH7.4)で2回洗浄後、蒸留水に懸濁し、氷冷条件下で5分間の超音波破砕処理を行った(Bioruptor II超音波細胞破壊装置(設定High)で、30秒間の音波処理と30秒間の休止を5回繰り返す)。破壊液を「H.ピロリ全菌体懸濁液」とし、遠心分離(30,190 G x 10分間)により沈渣を「H.ピロリ菌体外膜画分」とし、蒸留水に懸濁した。蛋白質の定量には、BSAを標準蛋白質としてBio-Rad Protein Assayキットを用いた。

0.05M炭酸-重炭酸緩衝液(pH9.6)に溶解したH.ピロリ菌体外膜画分およびH.スイス菌体外膜画分(4μg/mL)100μL/wellを96-well NUNC ImmunoPlate #468667へ滴下し、4℃に一晩置いた。翌日菌体溶液を廃棄し、PBS-T(0.05%(V/V) Tween20を含むPBS)250μL/wellで3回洗浄した。ブロッキング溶液(1%(V/V)BSA、PBS系pH7.0)を200μL/well滴下し、37℃に1時間置いた。ブロッキング溶液を廃棄し、PBS-T 250μL/wellで3回洗浄した。胃疾患の症状を有するH.ピロリ感染者(既存の検査法(尿素呼気試験や抗体検査、抗原検査など)で判定)、および胃疾患の症状を有するH.スイス感染者(被検者の胃生検体からDNAを調製し、特許文献2に記載のリアルタイムPCR法あるいは胃生検体からの培養によりH.スイス感染を判定)から血清をそれぞれ採取した。また対照として、いずれの菌にも感染していない健常人からの血清を採取した(非感染)。検体希釈液(0.5%(V/V)BSA、PBS系pH7.0)を用いて希釈したヒト血清を50μL/well滴下し、37℃に1時間置いた。血清溶液を廃棄し、PBS-T 250μL/wellで3回洗浄した。二次抗体希釈液(1.0%(V/V)BSA、PBS系pH7.0)を用いて20,000倍希釈した西洋わさびペルオキシダーゼ標識二次抗体(Goat anti-Human IgG(H+L),Jacson ImmunoResearch, Inc.)を50μL/well滴下し、37℃で1時間置いた。二次抗体溶液を廃棄し、PBS-T 250μL/wellで3回洗浄した。TMB One Component HRP Microwell Substrate,(Surmodics, Inc.)を50μL/well滴下し青く発色後に、0.17M硫酸を50μL/well滴下し黄色へ変色させた。プレートリーダーを用いて450nm(リファレンス波長:620nm~630nm)の吸光度を測定した。
ヒト血清(3,600倍希釈)を用いたELISAの結果を図5および図6に示す。H.ピロリ菌体外膜画分では、H.ピロリ感染群(胃疾患‐A、B、C、D)の吸光度がH.スイス感染群(胃疾患‐E、F、G、H)または非感染群(健常人‐A、B、C、D)よりも優位に高い結果が得られた(H.ピロリ感染群 vs.H.スイス感染群:P=0.0209、H.ピロリ感染群 vs.非感染群:P=0.0202)。また、H.スイス菌体外膜画分では、H.スイス感染群(胃疾患‐E、F、G、H)の吸光度がH.ピロリ感染群(胃疾患‐A、B、C、D)または非感染群(健常人‐A、B、C、D)よりも優位に高い結果が得られた(H.スイス感染群 vs.H.ピロリ感染群 or 非感染群:P=0.0209)。このことより、同一の被検体に対し抗H.ピロリ抗体と抗H.スイス抗体の両方の検査をすることで、H.ピロリ感染の有無だけではなく、今までH.ピロリ陰性で見逃されていた対象についても、H.スイス感染と診断できることが明らかとなった。
(実施例4)ヒト検体の測定

ヒト血清(3,600倍希釈)を用いたELISAの結果を図7-1および7-2に示す。H.スイス菌体外膜画分ではH.スイス感染者に対して吸光度が最小であった検体でも0.658(Abs.450nm~630nm)と高い吸光度が得られ、H.スイスに感染していないH.ピロリ感染者および非感染者に対しては吸光度が最大であった検体でも0.461(Abs.450nm~630nm)に止まり、H.スイス感染者と非感染者とを明確に区別することができた。一方HsvA抗原ペプチドではいずれの配列ペプチドにおいてもH.スイス感染者に対して高い吸光度が得られず、H.スイスに感染していないH.ピロリ感染者や非感染者に対する吸光度と明確に区別することができなかった。このことから、H.スイス菌体外膜画分を用いることにより、HsvA抗原ペプチドを用いた検出系よりも、感度・特異度ともに優れたH.スイスの診断ができることが明らかとなった。
本発明の方法により、H.ピロリ感染と区別してH.スイス感染の高感度な診断が可能となる。

- 少なくともH.スイス外膜画分、または外膜画分に含まれる蛋白質および抗Ig抗体を含む、被験体由来の検体のH.スイス外膜画分、または外膜画分に含まれる蛋白質と結合する抗体を検出する試薬。
- 被験体が哺乳動物であることを特徴とする、請求項1記載の試薬。
- 検体が血液由来であることを特徴とする、請求項1または2に記載の試薬。
- さらに、H.ピロリの菌体成分またはH.ピロリ菌体成分に対する抗体を含む、被験体由来の検体のH.ピロリと結合する抗体またはH.ピロリ菌体成分の抗原も検出することを特徴とする、請求項1~3のいずれか1項に記載の試薬。
- ELISAまたはイムノクロマト用試薬である、請求項1~4のいずれか1項に記載の試薬。

- さらに、以下の工程を含む、被験体由来の検体のH.ピロリと結合する抗体も検出することを特徴とする、請求項6~8のいずれか1項に記載の方法:
(a)被験体由来の検体をH.ピロリ菌体成分と接触させる工程、および
(b)H.ピロリ菌体成分と結合した、前記検体中の抗体を検出する工程。 - さらに、以下の工程を含む、被験体由来の検体のH.ピロリ菌体成分の抗原も検出することを特徴とする、請求項6~8のいずれか1項に記載の方法:
(a)被験体由来の検体をH.ピロリ菌体成分に対する抗体と接触させる工程、および
(b)H.ピロリ菌体成分に対する抗体と結合した、前記検体中のH.ピロリ菌体成分の抗原を検出する工程。 - 請求項1~5のいずれか1項に記載の試薬を用いた測定および/または請求項6~10のいずれか1項に記載の方法により抗体が検出された被験体をH.スイスに感染していると判定する、H.スイスの感染の検出方法。
