目前看来JAK抑制剂存在这样几个亟待解决的痛点:1)pan-JAK抑制剂由于抑制3种以上的JAK亚型,特别是抑制JAK2造成贫血和凝血障碍,口服治疗非恶性的炎症性疾病前景不乐观;2)抑制1-2种JAK亚型,如JAK1和TyK2,应能做到安全性更高,但是特异性存在的前提下抑制效应不够;3)抑制1-2种JAK亚型的JAK抑制剂生物利用度不够高,无法通过口服进行系统治疗,极大 地限制了其临床应用;4)同时具有高特异性抑制Tyk2和JAK1亚型的小分子化合物比较稀缺,缺乏这类化合物在自身免疫性疾病上的临床应用数据。
发明内容
本发明的目的在于克服如上所述的现有技术中的不足,提供一种具有高活性、高选择性和高口服生物利用度的JAK抑制剂。
具体地,本发明的发明人发现现有的JAK抑制剂可适用于外用涂抹给药,在炎症性皮肤病动物模型上,该类JAK抑制剂药物通过被动/主动吸收进入皮肤局部病变组织控制炎症、治疗炎症性皮肤疾病并显示很好疗效。然而,该类小分子JAK抑制剂的口服生物利用度极低,从而不适用作为口服药物进一步开发。
为解决口服生物利用度低的障碍,本发明通过进一步化合物修饰以改变一类或几类小分子化合物的物理化学特性,从而提高该类化合物的体内细胞吸收性能,极大提高药物的生物利用度,对该类化合物药物的给药方式及使用提供了新的可能。因此,对在体外和体内细胞水平对JAK激酶抑制很强的JAK抑制剂的应用治疗相关疾病拓展了临床使用途径,是新药研发领域解决未满足的临床需求的一个重要的创新性手段。本发明也是基于上述发现而完成的。

在本发明的一个实施方式中,所述自身免疫性疾病选自类风湿性关节炎、炎症性肠病、系统性红斑狼疮、干燥综合征、皮肌炎、强直性脊柱炎、多发性硬化、白塞氏病、Covid-19重症肺炎、Reiter综合征和葡萄膜炎中的至少一种。
在本发明的一个实施方式中,所述相关的炎症性皮肤疾病选自银屑病、自 身免疫相关血管炎、硬皮病、皮肌炎、肠病性肢端皮炎、化脓性汗腺炎、扁平苔藓、白癜风、皮肤型红斑狼疮和硬化萎缩性苔藓中的至少一种。
在本发明的一个实施方式中,所述药物为口服施用的药物。
在本发明的一个实施方式中,所述口服施用的药物为片剂、丸剂、颗粒剂、胶囊剂、锭剂或液体剂。
本发明的作用和效果:
本发明提供的具有高口服生物利用度的JAK抑制剂能够克服现有的JAK抑制剂口服生物利用度低的障碍,通过进一步化合物修饰以改变一类或几类小分子化合物的物理化学特性,从而提高该类化合物的体内细胞吸收性能,极大提高药物的生物利用度,也对该类化合物药物的给药方式及使用提供了新的可能。
具体地,本发明的JAK抑制剂通过增加可水解代谢的基团以降低化合物的极性,提高水溶性,以其通过延迟原药的半衰期,延缓原药的清除,提高原药的血药浓度来显著提升原药的生物利用度,将一类高效JAK抑制剂由无法口服施用改造成可以口服施用,拓展药物使用途径,从而相应地拓展药物的治疗适应症。

在本发明中,“药学上可接受的赋形剂、载体或稀释剂”包括但不限于任何被相关的政府管理部门许可为可接受供人类或家畜使用的佐剂、载体、赋形剂、助流剂、增甜剂、稀释剂、防腐剂、染料/着色剂、矫味剂、表面活性剂、润湿剂、分散剂、助悬剂、稳定剂、等渗剂、溶剂或乳化剂等。
根据本发明,进一步地,上述用途中制得的药物除了可以包含本发明的式(I)化合物作为有效成分之外,还可以包含其他可用于预防或治疗自身免疫性疾病、以及与免疫有关的炎症性皮肤疾病的药剂作为另一种有效成分。所述药剂的实例包括但不限于维生素D衍生物、维生素A衍生物、糖皮质激素、钙调神经磷酸酶抑制剂或非甾体类抗炎药等。当该药物包含多种有效成分时,各有效成分可以根据医师的判断同时、依次或分开施用。
另外,本发明的式(I)化合物可以通过多种途径施用于患者,这些途径诸如口服、透皮、皮下、鼻内、静脉内、肌内、鞘内、区域或局部(例如粘膜)。在任何给定情况下最适合的施用途径将取决于受试者和疾病的性质和严重程度、以及受试者的身体状况等。在本发明的一个实施方式中,本发明的式(I)化合物制得的药物可以经口服施用。在此情况下,所述口服施用的药物可以为片剂、丸剂、颗粒剂、胶囊剂、锭剂或液体剂。
经发明人研究发现,本发明的式(I)化合物或其制得的药物在用于预防或治疗JAK相关的自身免疫性疾病、以及相关的炎症性皮肤病时(特别是即便以口服方式施用时)能够发挥优异的效果。具体地,所述自身免疫性疾病选自类风湿性关节炎、炎症性肠病、系统性红斑狼疮、干燥综合征、皮肌炎、强直性脊柱炎、多发性硬化、白塞氏病、Covid-19重症肺炎、Reiter综合征和葡萄膜炎等;而所述相关的炎症性皮肤疾病选自银屑病、自身免疫相关血管炎、硬皮病、皮肌炎、肠病性肢端皮炎、化脓性汗腺炎、扁平苔藓、白癜风、皮肤型红斑狼疮和硬化萎缩性苔藓等。
以下,将通过实施例对本发明的特定化合物的效果进行详细描述。
实施例
实施例1:合成化合物1的一般方法(TDM-181055)
步骤1:化合物1(TDM-181055)的制备((S)-(2,2-二氟-N-(5-(2-(((1-甲基-1H-吡唑-4-基)氨基)嘧啶-4-基)吡啶-2-基)环丙烷-1-羧酰胺基)新戊酸甲酯)
向化合物1a(500mg,1.346mmol)的N,N-二甲基甲酰胺(15mL)溶液中加入碳酸钾(372mg,2.692mmol)和化合物1b(608mg,4.039mmol),将混合物用氩气吹扫几次并加热至40℃,搅拌过夜。然后将混合物在减压下浓缩,向残余物中加入水,并通过过滤收集固体,将固体通过硅胶色谱法(二氯甲烷: 10%甲醇的二氯甲烷=0-30%)纯化得到黄色固体产物(TDM-181055,化合物1,19.8mg,收率3%)。LCMS[M+1]

向N-氯代丁二酰亚胺(4.85g,36.32mmol)的乙腈(20mL)溶液中加入2N的盐酸溶液(2.5mL)和化合物7c(1.74g,9.08mmol),反应液室温搅拌30分钟。后处理:反应完成后,将乙腈浓缩除去,加入水(15mL),有白色固体析出,过滤,固体用油泵拉干得到白色目标化合物(化合物7d,1.776g,收率90.7%)。
步骤3:化合物7(TDM-180958)的制备
向化合物7e(500mg,1.878mmol)的无水吡啶(20mL)溶液中加入化合物7d(1g,4.64mmol),反应液升温至80℃反应30分钟,LCMS[M+H]
+=446,检测反应完全。后处理:反应液浓缩至干,粗品过柱[洗脱剂:(D:M=10:1)/DCM=0~50%],粗品用DCM/MeOH=30/1打浆得到黄色固体,再通过制备型HPLC纯化得到黄色目标化合物,即1-(3-氰基苯基)-N-(4-(2-(((1-甲基-1H-吡唑-4-基)氨基)嘧啶-4-基)苯基)甲磺酰胺(化合物7,155.4mg,收率18.6%)。

对比例5:合成化合物10的一般方法(TDM-180972)
步骤1:化合物10c(4-(2-氯嘧啶-4-基)苯胺)的制备
向三口烧瓶加入化合物10a(2g,9.129mmol),化合物10b(1.36g,9.129mmol),四三苯基膦钯(527g,0.45mmol),碳酸钾(2.5g,18.258mmol),二氧六环(20mL)和水(20mL),用氮气置换数次,然后将混合物加热至80℃并搅拌45分钟。反应结束减压浓缩反应物,残余物通过硅胶色谱法进行纯化(石油醚/乙酸乙酯=0-60%),以得到淡黄色固体目标化合物(化合物10c,394mg,收率21%)。LCMS[M+1]
+=206。
步骤2:化合物10e((S)-N-(4-(2-氯嘧啶-4-基)苯基)-2,2-二氟环丙烷-1-羧酰胺)的制备
向三口烧瓶中加入化合物10c(300mg,1.459mmol)和化合物10d(187mg,1.531mmol),将混合物用氮气置换几次,然后在0℃下加入吡啶(10mL)和三氯氧磷(335.6mg,2.189mmol)。将混合物在室温搅拌1h,然后减压浓缩,残余物用乙酸乙酯(30mL*3)萃取,合并有机层,并用水(50mL*3)和饱和盐水(50mL*2)洗涤,用硫酸钠干燥,将滤液减压浓缩,残余物通过硅胶色谱法进行纯化(石油醚/乙酸乙酯=0-12%),以得到黄色固体目标化合物(化合物10e,327.7mg,收率72.5%)。LCMS[M+H]
+=310。
步骤3:化合物1((S)-4-((4-(4-(4-(2,2-二氟环丙烷-1-羧酰胺基)苯基)嘧啶-2-基)氨基)-N-乙基苯甲酰胺)的制备
向化合物10e(80mg,0.258mmol)的正丁醇(8mL)溶液加入化合物1f(85mg,0.517mmol)和对甲苯磺酸一水合物(98mg,0.517mmol)。将所得混合物加热至110℃并搅拌3小时。反应结束将混合物在减压下浓缩,残余物通过制备型HPLC(甲酸)纯化,以得到黄色固体目标化合物TDM-180972(化合物10,19.7mg,收率17.5%)。LCMS[M+H]
+=438.2。

大鼠口服PK生物学描述:
为验证本发明实施例的化合物1-5是否比与其具有类似结构的对比例的化合物6-10具有显著提高的口服生物利用度,将合成的化合物1-10在6-8周左右的Sprague Dawley(SD)大鼠(体重大约250-300克)体内进行了单次口服给药的药代动力学(PK)的测试,以便将化合物1-10的各项PK参数进行对比。简言之,化合物在溶媒[DMA:30%Solutol HS 15:Saline=10:10:80(v/v/v)]中配制成透明溶液制剂,浓度为1mg/mL,每个测试化合物使用3只大鼠(n=3)进行单次口腔灌胃给药(5mg/kg)。给药前以及给药后0.25,0.5,1,2,4,8,12, 24小时分别从每只实验大鼠经锁骨下静脉取大约150μL的全血,收集到含有EDTA-K2的Eppendorf管中,随后取100μL全血加入含有300μL的氰基甲烷(ACN)的Eppendorf管中,混合后离心分离血浆,确保样品没有溶血后将血浆样品在零下90到零下60度冰箱保存,直到通过LC-MS/MS对样品中的药物浓度进行分析。所有口服PK试验均未发现大鼠对任何测试化合物产生不良反应。
血浆样品中的化合物浓度通过LC-MS/MS系统(API4000:LC-MS-MS-001)进行检测分析,取10μL的样品和100μL含有标准品的ACN进行混合,随后加入110μL的水并进行震荡混合,最后取2μL的混合溶液注射进LC-MS/MS系统。
测试结果如下所示:
1)对比例1化合物:C
max=116ng/mL,AUC
0-t=891ng·h/mL,进一步合成为实施例1化合物后,C
max=668ng/mL,AUC
0-t=1670ng·h/mL。
2)对比例2化合物:C
max=20.2ng/mL,AUC
0-t=47.4ng·h/mL,进一步合成为实施例2化合物后,C
max=791ng/mL,AUC
0-t=1580ng·h/mL。
3)对比例3化合物:C
max=85.5ng/mL,AUC
0-t=381ng·h/mL,进一步合成为实施例3化合物后,C
max=1860ng/mL,AUC
0-t=2890ng·h/mL。
4)对比例4化合物:C
max=5.02ng/mL,AUC
0-t=30.7ng·h/mL,进一步合成为实施例4化合物后,C
max=635ng/mL,AUC
0-t=3090ng·h/mL。
5)对比例5化合物:C
max=16.5ng/mL,AUC
0-t=141ng·h/mL,进一步合成为实施例5化合物后,C
max=689ng/mL,AUC
0-t=2710ng·h/mL。
根据大鼠口服PK生物学数据可以看出,在进一步合成后,化合物的系统暴露得到了极大提升,表现为血浆药物浓度峰值比原药提高5-120倍,AUC提高2-100倍,证明了本发明实施例的化合物的口服生物利用度明显提高,同时体内清除速率则明显降低。
以上详细描述了本发明的优选实施方式,但是,本发明并不限于上述实施方式中的具体细节,在本发明的技术构思范围内,可以对本发明的技术方案进行多种简单变型,这些简单变型均属于本发明的保护范围。
另外需要说明的是,在上述具体实施方式中所描述的各个具体技术特征, 在不矛盾的情况下,可以通过任何合适的方式进行组合,为了避免不必要的重复,本发明对各种可能的组合方式不再另行说明。
此外,本发明的各种不同的实施方式之间也可以进行任意组合,只要其不违背本发明的思想,其同样应当视为本发明所公开的内容。
Claims (10)
- 根据权利要求1所述的JAK抑制剂,其中,所述R 4选自氟或氰基。
- 根据权利要求1所述的JAK抑制剂,其中,所述五元或六元芳基或杂芳基选自苯基、吡啶基、嘧啶基、吡唑基、吡咯基、咪唑基、吡嗪基或哒嗪基。
- 根据权利要求1所述的JAK抑制剂,其中,所述C 1-C 3烷基为甲基、乙基或丙基;所述C 1-C 3羟烷基为羟甲基、羟乙基或羟丙基;所述C 1-C 3烷基酰胺基为甲基甲酰胺基、二甲基甲酰胺基、乙基甲酰胺基或甲基乙酰胺基。
- 式(I)化合物,或其立体异构体、几何异构体、互变异构体、水合物、溶剂化物、以及药学上可接受的盐在制备用于预防或治疗自身免疫性疾病、以及相关的炎症性皮肤病的药物中的用途。
- 根据权利要求6所述的用途,其中,所述自身免疫性疾病选自类风湿性关节炎、炎症性肠病、系统性红斑狼疮、干燥综合征、皮肌炎、强直性脊柱炎、多发性硬化、白塞氏病、Covid-19重症肺炎、Reiter综合征和葡萄膜炎中的至少一种。
- 根据权利要求6所述的用途,其中,所述相关的炎症性皮肤疾病选自银屑病、自身免疫相关血管炎、硬皮病、皮肌炎、肠病性肢端皮炎、化脓性汗腺炎、扁平苔藓、白癜风、皮肤型红斑狼疮和硬化萎缩性苔藓中的至少一种。
- 根据权利要求6所述的用途,其中,所述药物为口服施用的药物。
- 根据权利要求9所述的用途,其中,所述口服施用的药物为片剂、丸剂、颗粒剂、胶囊剂、锭剂或液体剂。
