[1]標準療法、並びに免疫チェックポイント阻害物質と組み合わせて投与されることを特徴とする、CD47阻害物質を有効成分として含む固形がんの進行抑制、再発抑制および/または治療剤、
[2]標準療法、並びにCD47阻害物質と組み合わせて投与されることを特徴とする、免疫チェックポイント阻害物質を有効成分として含む固形がんの進行抑制、再発抑制および/または治療剤、が提供される。
本発明において、CD47阻害物質は、CD47阻害作用を有する化合物であれば特に限定されないが、一実施形態において、CD47とSIRPαとの結合を阻害する薬剤(好ましくは抗体、さらに好ましくはモノクローナル抗体)であり、抗CD47抗体、抗SIRPα抗体等が挙げられ、前記抗体の抗原結合フラグメント(例えば、Fv、Fab、Fab’、(Fab’)2、scFv、scFv-Fc)であってもよい。抗CD47抗体の例としては、Magrolimab(Hu5F9-G4)、CC-90002、STI-6643、ZL-1201、TAY-018、SGN-CD47M、GenSci-059、lemzoparlimab、letaplimab、IMC-002、SHR-1603、AO-176、AVI-105、MIL-95、AK-117、HLX-24、SG-404、SY-102、IMM-01、KD-015、BAT-6004、ALX-148、SRF-231、TJ-011133、letaplimab、TQB-2928、AL-008、JMT-601、およびDSP-107が挙げられる。抗SIRPα抗体の例としては、ES-004、CTX-5861、ADU-1805、BI-765063、BYON-4228、FSI-189、およびCC-95251が挙げられる。CD47阻害物質は、1種を単独で用いてもよく、2種以上を組み合わせて用いてもよい。CD47阻害物質として好ましくは抗CD47抗体であり、抗CD47抗体として好ましくはMagrolimab(CAS番号:2169232-81-7)である。また、上記既知の抗体の重鎖および軽鎖相補性決定領域(CDRs)または可変領域(VR)を含む抗体またはその抗原結合フラグメントもCD47阻害物質の一態様である。例えば、抗CD47抗体の更なる一態様としては、例えばMagrolimabの重鎖および軽鎖相補性決定領域(CDRs)または可変領域(VR)を含む抗体またはその抗原結合フラグメントが挙げられる。また、抗CD47抗体の一態様としては、CD47への結合についてMagrolimabと競合する抗体、または、Magrolimabと同じCD47エピトープに結合する抗体、または、これらの抗原結合フラグメントが挙げられる。
本発明において、免疫チェックポイント分子は、抑制性共シグナルを伝達することで免疫抑制機能を発揮する分子を意味する。免疫チェックポイント分子としては、CTLA-4、PD-1、PD-L1(programmed cell death-ligand 1)、PD-L2(programmed cell death-ligand 2)、LAG-3(Lymphocyte activation gene 3)、TIM3(T cell immunoglobulin and mucin-3)、BTLA(B and T lymphocyte attenuator)、B7H3、B7H4、CD160、CD39、CD73、A2aR(adenosine A2a receptor)、KIR(killer inhibitory receptor)、VISTA(V-domain Ig-containing suppressor of T cell activation)、IDO1(Indoleamine 2,3-dioxygenase)、ArginaseI、TIGIT(T cell immunoglobulin and ITIM domain)、CD115等が知られているが(Nature Reviews Cancer、12、252-264ページ、2012年、Cancer Cell、27、450-461ページ、2015年を参照)、定義に一致する働きを有する分子であれば特に限定されない。
本発明における「標準療法」とは、多くの臨床試験の結果によって治療効果や安全性が確認されており、科学的根拠に基づいて最も推奨される治療のことである。本発明における「FOLFOX療法」とは、オキサリプラチン、レボホリナートカルシウム(以下、「レボホリナート」と略記する)およびフルオロウラシルの3剤併用によるがん治療法であり、一実施形態において(1a)オキサリプラチン 85mg/m2~130mg/m2の静脈内投与、(1b)レボホリナート 100mg/m2~200mg/m2の静脈内投与、(1c)フルオロウラシル 400mg/m2の急速静脈内投与、および(1d)フルオロウラシル 600mg/m2~2400mg/m2の静脈内投与を含み、当該(1a)、(1b)、(1c)および(1d)の一連の投与を2週間間隔で実施する治療法である。当該治療法における投与量は、全て投与1回当たりの体表面積換算における投与量である。FOLFOX療法は、投与される3剤の投与量や投与方法により複数の処方が知られており、その態様として、例えば、当該(1c)のフルオロウラシルの急速静脈内投与を実施しなくてもよく、当該(1a)のオキサリプラチンの投与量が、85mg/m2、100mg/m2、130mg/m2、または85mg/m2~130mg/m2の間の任意の投与量であってもよく、当該(1b)のレボホリナートの投与量が、100mg/m2、200mg/m2、または100mg/m2~200mg/m2の間の任意の投与量であってもよく、当該(1d)の静脈内持続投与されるフルオロウラシルの投与量が、600mg/m2、1500mg/m2、2400mg/m2または600mg/m2~2400mg/m2の間の任意の投与量であってもよい。
FOLFOX療法に分子標的薬(例えば、ベバシズマブ、セツキシマブ)を加えた併用療法との併用におけるCD47阻害物質は、一実施形態において、前記(1)CD47阻害物質に記載の用法用量で投与される。CD47阻害物質の一例であるMagrolimabは、一実施形態において、初回投与として1mg/kgを3時間(±30分)かけて静脈内投与し、その後、1回20mg/kgまたは30mg/kgを1週間間隔または2週間間隔で静脈内投与される。一実施形態において、第1サイクルの1日目にMagrolimabの初回投与として1mg/kgを3時間(±30分)かけて静脈内投与され、その後、第1サイクルは1週間間隔、第2サイクル以降は2週間間隔で2時間(±30分)かけて1回20mg/kgまたは30mg/kgを静脈内投与される。患者の副作用発現の程度に応じて、Magrolimabの投与量を1段階ずつ減量するか、または投与自体を中止してもよい。また、再開の基準を満たせば、Magrolimabの投与を再開することができ、再開する場合、治験責任医師または治験分担医師の判断により、Magrolimabの用量を減量して再開することができる。一実施形態において、CD47阻害物質としてのMagrolimabを投与開始用量30mg/kgで投与する場合、1段階減量後は20mg/kgであり、2段階減量後は15mg/kgである。CD47阻害物質としてのMagrolimabを投与開始用量20mg/kgで投与する場合、減量後は15mg/kgである。
本発明における「FFX療法」とは、オキサリプラチン、イリノテカン塩酸塩水和物(以下、「イリノテカン」と略記する)、レボホリナートカルシウム(以下、「レボホリナート」と略記する)およびフルオロウラシルの4剤併用によるがん治療法であり、推奨の用法用量として、例えば、(2a)オキサリプラチン 85mg/m2を2時間かけて静脈内投与すること、その後、(2b)レボホリナート 200mg/m2を2時間かけて静脈内投与すること、(2c)当該(2b)のレボホリナートの投与開始30分後からイリノテカン 180mg/m2を1.5時間かけて静脈内投与すること、(2d)さらに当該(2b)のレボホリナートの投与終了後に、フルオロウラシル 400mg/m2を急速静脈内投与すること、および(2e)当該(2d)のフルオロウラシルの投与終了後に、さらにフルオロウラシル 2400mg/m2を46時間かけて静脈内投与することを含み、当該(2a)、(2b)、(2c)、(2d)および(2e)の一連の投与を2週間間隔で実施する治療法である。当該治療法における投与量は、全て投与1回当たりの体表面積換算における投与量である。
FFX療法またはその減量レジメン(例えば、mFFX療法)との併用におけるCD47阻害物質は、一実施形態において、前記(1)CD47阻害物質に記載の用法用量で投与される。CD47阻害物質の一例であるMagrolimabは、一実施形態において、初回投与として1mg/kgを3時間(±30分)かけて静脈内投与し、その後、1回20mg/kgまたは30mg/kgを1週間間隔または2週間間隔で静脈内投与される。一実施形態において、第1サイクルの1日目にMagrolimabの初回投与として1mg/kgを3時間(±30分)かけて静脈内投与され、その後、第1サイクルは1週間間隔、第2サイクル以降は2週間間隔で2時間(±30分)かけて1回20mg/kgまたは30mg/kgを静脈内投与される。患者の副作用発現の程度に応じて、Magrolimabの投与量を1段階ずつ減量するか、または投与自体を中止してもよい。また、再開の基準を満たせば、Magrolimabの投与を再開することができ、再開する場合、治験責任医師または治験分担医師の判断により、Magrolimabの用量を減量して再開することができる。一実施形態において、CD47阻害物質としてのMagrolimabを投与開始用量30mg/kgで投与する場合、1段階減量後は20mg/kgであり、2段階減量後は15mg/kgである。CD47阻害物質としてのMagrolimabを投与開始用量20mg/kgで投与する場合、減量後は15mg/kgである。
本発明の治療法の適用疾患は、がんである。

[98]遠隔転移を有する膵臓がんに対する全身性抗悪性腫瘍剤による治療歴のない患者に一次治療として投与されることを特徴とする、前記[97]に記載の剤。
[99] FOLFIRINOX療法またはその減量レジメン、Magrolimab、およびNivolumabの投与を同日に開始する療法である、前記[95]~[98]のいずれか一項に記載の剤。
本試験は、根治切除不能な進行または再発の結腸・直腸がん患者を対象に、一次治療としてMagrolimab、NivolumabおよびFOLFOX療法にベバシズマブまたはセツキシマブを加えた併用療法を併用したときの忍容性、安全性および有効性を検討することを目的とする。当該治験により、Magrolimab、NivolumabおよびFOLFOX療法にベバシズマブまたはセツキシマブを加えた併用療法との併用効果が評価できる。
根治切除不能な進行または再発の結腸・直腸がん患者
登録時に、下記のすべての基準を満たす患者を選択した。なお、治験薬の初回投与前までに下記の基準を満たさないことが明らかとなった場合は、治験薬の投与を開始せず治験中止とした。
1.性別:不問。
2.年齢(同意取得時):20歳以上。
3.治験薬投与開始前日から少なくとも治験薬投与開始15日目の検査終了時まで入院が可能な患者(忍容性評価パートのみ)。
4.組織学的に結腸又は直腸原発の腺がん(虫垂がん、肛門管がんを除く)と病理学的に診断され、根治切除が適応されない患者。
5.治験薬投与前28日以内の画像診断において、RECISTガイドライン1.1版に定義される測定可能病変を有する患者。
・測定可能病変に対する放射線療法を受けた患者は、放射線療法施行後の画像診断において進行が確認されていること。
6.根治切除不能な進行又は再発の結腸・直腸がんに対する全身性抗悪性腫瘍剤による治療歴のない患者。
・治癒手術(R0切除が確認されている)と併せて術前・術後補助療法を実施した場合、補助療法完遂後6カ月以内に再発が認められていないこと。
7.バイオマーカー検査のための腫瘍組織検体を提供できる患者。
・腫瘍組織検体は、スクリーニング期に取得する。新たな腫瘍生検が困難な場合は、保存検体を利用することができる。ただし、保存検体は、治験薬投与前180日以内かつ直近の全身性抗がん療法の最終投与後に採取されていること。
8.RAS変異型で、FOLFOX(フルオロウラシル、レボホリナート、オキサリプラチン)とベバシズマブの併用療法を施行予定の患者(コホート1)、又はRAS野生型かつ原発巣占拠部位が左側(下行結腸、S状結腸、直腸)で、FOLFOX(フルオロウラシル、レボホリナート、オキサリプラチン)とセツキシマブの併用療法を施行予定の患者(コホート2)。
9.ECOG Performance Statusが0~1の患者。
10.登録時に3カ月以上の生存が期待される患者。
11.妊娠する可能性のある女性(化学閉経などの医学的理由により月経がない患者も含む)の場合、同意取得時から治験薬の最終投与後5カ月間及び標準治療薬の最終投与後、各薬剤で規定された避妊期間は二重避妊を行うことに同意した患者。また、同意取得後から治験薬最終投与後5カ月間及び標準治療薬の最終投与後、各薬剤で授乳が制限される期間に授乳しないことに同意した患者。ここで、妊娠する可能性のある女性とは、初潮を経験しており、不妊手術(子宮摘出術、両側卵管結紮又は両側卵巣摘出術など)を受けておらず、閉経していない女性すべてが含まれる。閉経の定義は、特筆すべき理由がないにもかかわらず、12カ月以上連続して無月経であることとする。経口避妊薬又は子宮内避妊器具やペッサリーを使用している女性は妊娠する可能性があるものとみなす。
12.男性の場合、治験薬投与開始時から治験薬の最終投与後4カ月間及び標準治療薬の最終投与後、各薬剤で規定された避妊期間は二重避妊を行うことに同意した患者。ここで、避妊については、男性患者又は男性パートナーの精管切除若しくはコンドーム、女性患者又は女性パートナーの卵管結紮、ペッサリー、子宮内避妊器具若しくは経口避妊薬のうち、いずれか2つによる二重の避妊を行う必要がある。
13.治験薬投与開始前14日以内に実施した最新の臨床検査値が下記の基準を満たす患者。なお、検査前14日以内に顆粒球コロニー刺激因子(G-CSF)等の造血因子製剤の投与又は輸血を受けていないこと。
・好中球数が1,500/mm3以上。
・血小板数が100,000/mm3以上。
・ヘモグロビンが11.0g/dL以上。
・AST(GOT)及びALT(GPT)が施設基準値上限の3.0倍以下(ただし、肝転移がある場合、施設基準値上限の5.0倍以下とする)。
・総ビリルビンが施設基準値上限の1.5倍以下(ただし、ジルベール症候群患者の場合、総ビリルビンは施設基準値上限の3.0倍以下とする)。
・血清クレアチニンが施設基準値上限の1.5倍以下又はクレアチニンクリアランス(実測値又はCockcroft/Gault式による推定値)が40mL/min以上。
・アルブミンが3.0g/dL以上。
・[コホート1のみ]尿蛋白が1+以下。ただし、尿検査又は尿試験紙検査で尿蛋白が2+以上の患者は、24時間蓄尿で尿蛋白が1g/24時間以下の場合、又は尿蛋白/クレアチニン比(UPCR)が1g/gCr未満の場合は登録可能とする。
14.治験責任医師又は治験分担医師より、本治験の内容について同意文書及び説明文書を用いて十分に説明を受け、自由意思により本治験参加に文書で同意する患者。
登録時に、下記のいずれかの基準に該当する患者は除外した。なお、治験薬の初回投与前までに下記のいずれかの基準に抵触した場合は、治験薬の投与を開始せず治験中止とした。
1.原発巣占拠部位が右側(盲腸、上行結腸、横行結腸)のRAS/BRAF野生型の患者。
2.BRAF変異型の患者及び高頻度マイクロサテライト不安定性(MSI-High)又はミスマッチ修復機構欠損(dMMR)を有する患者。
3.抗凝固療法を受けている又はそれらを必要とする疾患を有する患者。
4.Grade2以上の消化管出血を持続的に認める患者。
5.消化管穿孔、重度な瘻孔、気管食道瘻を合併している患者。
6.薬物療法では管理不能な心嚢液、胸水又は腹水の貯留を認める患者。
7.抗体製剤に対する高度な過敏反応の既往を有する患者。
8.各コホートで使用する標準療法のいずれかの薬剤に対する投与が禁忌とされている患者。
[コホート1]フルオロウラシル、レボホリナート、オキサリプラチン及びベバシズマブ。
[コホート2]フルオロウラシル、レボホリナート、オキサリプラチン及びセツキシマブ。
9.Grade2以上の末梢性ニューロパチーを有する患者。
10.自己免疫疾患の合併、又は慢性的若しくは再発性の自己免疫疾患の既往を有する患者。ただし、全身療法を必要としない皮膚疾患(白斑、乾癬、脱毛症など)又は外的誘因の非存在下では再発すると考えられない疾患を合併している患者は登録可能とする。
11.手術療法の影響のため、治験責任医師又は治験分担医師が治験薬の安全性及び有効性評価に影響を及ぼすと判断した患者。
12.重複がんを有する患者。ただし、完全切除された基底細胞がん、ステージIの有棘細胞がん、上皮内がん、粘膜内がん若しくは表在性膀胱がん又は3年以上再発が認められない他のがんを有する患者は登録可能とする。
13.脳又は髄膜に転移巣を有する患者。ただし、無症状かつ治療を必要としない患者は登録可能とする。
14.画像診断若しくは臨床所見により診断された間質性肺疾患、又は肺線維症の合併若しくは既往を有する患者。
15.腫瘍に関連する疼痛が安定してコントロールできない患者。
16.登録前90日以内に溶血性貧血の既往歴を有する患者。
17.登録前180日以内に一過性脳虚血発作、脳血管発作、血栓症又は血栓塞栓症(肺動脈塞栓症又は深部静脈血栓症)の既往を有しており、血栓症又は血栓塞栓症のコントロールが不良な患者。ただし、中心静脈カテーテル関連の静脈血栓は許容する。
18.下記のコントロール不能又は重大な心血管疾患を有する患者。
・登録前6カ月以内の急性心筋梗塞。
・不安定狭心症又は治療を必要とする狭心症。
・New York Heart Association(NYHA)心機能分類III度からIV度のうっ血性心不全。
・適切な治療にもかかわらず管理不能な高血圧(収縮期血圧150mmHg以上又は拡張期血圧100mmHg以上が24時間以上持続するなど)。
・コントロール不能又は重大な不整脈。
19.コントロール不能な糖尿病を合併している患者。
20.治療を必要とする全身性感染症を有する患者。
21.移植療法を必要とする患者又は移植療法の既往がある患者(自己移植を除く)。
22.HIV-1抗体及びHIV-2抗体検査、HTLV-1抗体検査、HBs抗原検査、HCV抗体検査のいずれかが陽性の患者。また、HBs抗原検査が陰性であるが、HBs抗体検査又はHBc抗体検査のいずれかが陽性かつHBV-DNA定量が検出感度以上の患者。なお、HCV抗体検査が陽性でも、HCV-RNAが陰性の場合、登録を許容する。
23.同意取得前及び治験薬投与前に以下の治療を受けた患者。
・過去にMagrolimab、CD47-SIRPα経路を標的とした薬剤の治療歴を有する患者。
・過去に抗PD-1抗体、抗PD-L1抗体、抗PD-L2抗体、抗CTLA-4抗体、若しくはその他のT細胞制御を目的とした抗体療法又はがんワクチンを含む治療歴を有する患者。
・同意取得前28日以内に2単位を超える赤血球輸血を受けた患者。
・治験薬投与前14日以内に局所麻酔を伴う手術療法を受けた患者。
・治験薬投与前14日以内に胸膜又は心膜などの癒着術を受けた患者。
・治験薬投与前14日以内に10mg/日を超えるプレドニゾン相当量の全身性副腎皮質ホルモン(検査、局所投与、造影剤アレルギー反応の治療及び予防又は放射線療法に伴う浮腫軽減などを目的とした一時的使用は除く)又は免疫抑制剤の投与を受けた患者。
・治験薬投与前28日以内に緩和的局所放射線療法を受けた患者。
・治験薬投与前28日以内に全身麻酔を伴う手術療法を受けた患者。
・治験薬投与前28日以内に生ワクチン又は弱毒化ワクチン接種を受けた患者。
・治験薬投与前56日以内に放射性医薬品(検査及び診断を目的とした放射性医薬品の使用を除く)の投与を受けた患者。
・治験薬投与前28日以内に他の未承認薬(臨床研究による投与や、未承認の配合薬、新剤形薬も含む)の投与を受けた患者。
24.妊娠中、授乳中又は妊娠している可能性のある患者。
25.認知症の合併などにより同意能力を欠く状態であると判断される患者。
26.その他、治験責任医師又は治験分担医師が本治験の対象として不適当と判断した患者。
[Magrolimab]
Magrolimabを第1サイクルの1日目に初回投与として1mg/kgを3時間(±30分)かけて静脈内投与し、その後、第1サイクルでは1回20mg/kgまたは30mg/kgを1週間間隔で2時間(±30分)かけて静脈内投与し、第2サイクル以降では1回20mg/kgまたは30mg/kgを2週間間隔で2時間(±30分)かけて静脈内投与し、Magrolimabに関する所定の投与中止基準に該当するまで継続投与した。なお、Magrolimab、NivolumabおよびFOLFOX療法にベバシズマブまたはセツキシマブを加えた併用療法を同日に投与する場合、Magrolimab、Nivolumab、FOLFOX療法にベバシズマブまたはセツキシマブを加えた併用療法の順で投与を開始した。
Nivolumab 480mgを4週間間隔で30分間かけて静脈内投与し、Nivolumabに関する所定の投与中止基準に該当するまで継続投与した。なお、Nivolumabの投与は、前回の投与から少なくとも24日以上空け、25日目以降に実施した。
ベバシズマブまたはセツキシマブの選択については、RAS変異型の患者に対してはベバシズマブを用い、RAS野生型の患者に対してはセツキシマブを用いた。
ベバシズマブの開始時用量は5mg/kg(体重)とし、90分かけて点滴静脈内注射した。初回投与時の忍容性が良好であれば、2回目の投与は60分間とすることも可能とした。また2回目の投与においても忍容性が良好であれば、それ以降の投与は30分間とすることも可能とした。投与間隔は2週間以上とした。
セツキシマブの開始時用量は400mg/m2(体表面積)とし、2時間かけて点滴静注した。2回目以降は週1回または2週間間隔で250mg/m2を1時間かけて点滴静注した。また必要に応じて、2週間に1回、セツキシマブとして、500mg/m2を2時間かけて点滴静注する2週間間隔投与も可能とした。なお、患者の状態により適宜減量した。
(1x)ベバシズマブまたはセツキシマブの投与後、(1a)オキサリプラチンは85mg/m2(体表面積)を2時間かけて静脈内投与した。(1b)オキサリプラチンの投与と同時に、レボホリナートカルシウムは200mg/m2(体表面積)を2時間かけて静脈内投与した。(1c)フルオロウラシルは、レボホリナートカルシウムの静脈内投与終了後、400 mg/m2(体表面積)を急速静脈内投与し、(1d)その後2400mg/m2(体表面積)を46時間かけて静脈内投与した。当該(1x)、(1a)、(1b)、(1c)および(1d)の一連の投与は2週間間隔で実施した。なお、ベバシズマブ、セツキシマブ、オキサリプラチン、レボホリナートカルシウム、およびフルオロウラシルはそれぞれ市販品を用いた。
本試験は、スクリーニング期、治療期および後観察期からなる。治験スケジュールの概要を図1に示す。
スクリーニング期は治験薬投与前28日以内とし、治験責任医師または治験分担医師は上記選択基準を満たし、かつ上記除外基準に抵触せず、本治験の対象として適格と判断した患者を組み入れた。
治療期は1サイクル28日間とし、初回の治験薬投与日をサイクル1の1日目とした。第2サイクル以降の各サイクルの1日目は、[28×(サイクル数-1)+1]日とした。Magrolimab、NivolumabおよびFOLFOX療法にベバシズマブまたはセツキシマブを加えた併用療法についてそれぞれ上記用法・用量に従って、投与を開始し、Magrolimab、NivolumabおよびFOLFOX療法にベバシズマブまたはセツキシマブを加えた併用療法についての投与基準、減量基準および減量時の投与量に従って投与を継続した。Magrolimab、NivolumabおよびFOLFOX療法にベバシズマブまたはセツキシマブを加えた併用療法の投与終了(中止)時の評価が終了した時点を治療期終了とした。治験薬を投与したすべての被験者のうち、Magrolimab、NivolumabおよびFOLFOX療法にベバシズマブまたはセツキシマブを加えた併用療法の投与を中止または終了した被験者は、投与終了(中止)時の評価を実施し、後観察期に移行した。後観察期終了後は、追跡調査を実施した。
被験者は、毎回の投与開始時において、MagrolimabおよびNivolumabに関してこれまでに実施された治験における投与基準を考慮して決定された所定の投与基準をすべてに合致しなければならない。当該基準のいずれかに合致しない場合、予定されたMagrolimabおよびNivolumabの投与を休薬した。休薬した被験者は、週1回又は臨床上必要な場合はそれ以上の頻度で検査を実施し、投与再開の可否を判断した。
治療期において、Magrolimabに関してこれまでに実施された治験における投与中止基準を考慮して決定された所定の投与中止基準のいずれかに該当した被験者はMagrolimabの投与を中止した。
治療期において、Nivolumabに関してこれまでに実施された治験における投与中止基準を考慮して決定された所定の投与中止基準のいずれかに該当した被験者はNivolumabの投与を中止した。
被験者は、毎回の投与開始時において、FOLFOX療法にベバシズマブまたはセツキシマブを加えた併用療法に関して最新の添付文書を考慮して決定された所定の投与基準をすべて合致しなければならない。投与予定日の臨床検査値が当該基準の条件を満たす状態へ回復するまで投与を延期し、各薬剤の禁忌に該当しないことを確認した上で投与した。
最新の添付文書に準じて減量を実施した。
治療期において、FOLFOX療法にベバシズマブまたはセツキシマブを加えた併用療法に関して最新の添付文書を考慮して決定された所定の投与中止基準のいずれかに該当した被験者はFOLFOX療法にベバシズマブまたはセツキシマブを加えた併用療法の投与を中止した。
(画像診断)
胸部、腹部および骨盤のCT/核磁気共鳴画像(MRI)撮影等を実施した。
治験責任医師または治験分担医師は、RECISTガイドライン1.1版に準じて標的病変の腫瘍径を計測し、抗腫瘍効果を判定した。
(評価項目)

(標的病変の評価)
完全奏功(complete response:CR)
すべての非リンパ節標的病変の消失し、標的病変として選択したすべてのリンパ節病変の短径が10mm未満に縮小した場合を意味する。
部分奏功(partial response:PR)
ベースラインの径和に比して、標的病変の径和が30%以上減少した場合を意味する。
進行(progressive disease:PD)
経過中の最小の径和に比して、標的病変の径和が20%以上増加、かつ径和が絶対値でも5mm以上増加した場合を意味する。
安定(stable disease:SD)
経過中の最小の径和に比して、PRに相当する縮小がなくPDに相当する増大がない場合を意味する。
評価不能(Not Evaluable:NE)
何らかの理由で検査が行えない場合若しくはCR、PR、PD又はSDのいずれとも判定できない場合を意味する。
奏効率は、最良総合効果がCRまたはPRと判定された被験者の割合を示す。
病勢制御率は、最良総合効果がCR、PRまたはSDと判定された被験者の割合を示す。
全生存期間は、以下の式から算出する。
全生存期間(日)=「あらゆる原因による死亡日」-「治験薬投与開始日」+1
なお、追跡不能となった、またはデータカットオフ日までに死亡していない被験者は、最終生存確認日を打切り日とする。
無増悪生存期間は、以下の式から算出する。
無増悪生存期間(日)=「総合効果がPDと判定された日、またはあらゆる原因による死亡日のうち早い日」-「治験薬投与開始日」+1
なお、総合効果がPDと判定されておらず、かつ死亡もしていない被験者は、最後の評価可能な画像診断が行われた日を打切り日とする。評価可能な画像診断が行われておらず、かつ死亡もしていない被験者は、治験薬投与開始日を打切り日とする。総合効果がPDと判定されるまたは死亡する前に、がんに対する後治療を受けた被験者は、がんに対する後治療開始前における最後の評価可能な画像診断が行われた日を打切り日とする。
奏効期間は、以下の式から算出する。

本治験を終了するまでに判定された総合効果から最良総合効果を判定する。ただし、CRとPRは4週(28日)以上間隔を経て連続した評価による確定を必要とし、表1の基準に従って判定する。
標的病変を有する被験者を対象に、標的病変の腫瘍径和の変化率を以下の計算式を用いて算出する。ただし、後治療が行われた後の標的病変の腫瘍径和の変化率は算出しない。
標的病変の腫瘍径和の最大変化率は、標的病変の腫瘍径和が最も小さくなった時点の変化率とする。ただし、総合効果がPDと判定された後または後治療が行われた後の標的病変の腫瘍径和は最大変化率の算出に使用しない。
腫瘍マーカーの変化率を以下の計算式を用いて算出する。ただし、後治療が行われた後の腫瘍マーカーの変化率は算出しない。
以下の項目について治験責任医師などにより、既定の時期に測定、検査および調査を実施した。
(1)用量制限毒性(DLT)、(2)有害事象、(3)臨床検査(血液学的検査、血液生化学的検査、免疫学的検査、ホルモン調査、血液凝固系検査、尿定性検査)、末梢血塗抹標本、(4)バイタルサイン(収縮期血圧/拡張期血圧、脈拍数、体温、呼吸数)、経皮的酸素飽和度(SpO2)、体重、(5)12誘導心電図、(6)ECOG Performance Status、および(7)胸部X線
Magrolimab、NivolumabおよびFOLFOX療法にベバシズマブを加えた併用療法について、1例目の被験者が登録されてから8ヶ月が経過した時点において、PRと判定された症例が2例であった。
Magrolimab、NivolumabおよびFOLFOX療法にセツキシマブを加えた併用療法について、1例目の被験者が登録されてから8ヶ月が経過した時点において、PRと判定された症例が3例であった。
本試験は、遠隔転移を有する膵臓がん患者を対象に、一次治療としてMagrolimab、NivolumabおよびmFFX療法を併用したときの忍容性、安全性および有効性を検討することを目的とする。当該治験により、Magrolimab、NivolumabおよびmFFX療法との併用効果が評価できる。
遠隔転移を有する膵臓がん患者
登録時に、下記のすべての基準を満たす患者を選択した。なお、登録から治験薬の初回投与前までに下記の基準を満たさないことが明らかとなった場合は、治験薬の投与を開始せず治験中止とした。
1.性別:不問。
2.年齢(同意取得時):20歳以上。
3.治験薬投与開始前日から少なくとも治験薬投与開始15日目の検査終了時まで入院が可能な患者(忍容性評価パートのみ)。
4.組織診又は細胞診により腺がんと診断された浸潤性膵管がんの患者。
5.遠隔転移を有する膵臓がんに対する全身性抗悪性腫瘍剤による治療歴がない患者。ただし、化学放射線療法、術後補助化学療法又は術前補助化学療法を過去に受けた患者で、抗悪性腫瘍剤の投与終了から180日以上経過して再発した場合、登録を許容する。
6.治験薬投与前14日以内の画像診断において、RECISTガイドライン1.1版に定義される測定可能病変を有する患者。ただし、放射線照射の治療歴がある測定可能病変のみを有する場合は、放射線療法施行後の画像診断において増悪が確認された病変に限る。
7.Eastern Cooperative Oncology Group(ECOG)Performance Statusが0又は1の患者。
8.90日以上の生存が見込まれる患者。
9.バイオマーカー検査のための腫瘍組織検体を提供できる患者。
腫瘍組織検体は、スクリーニング期に取得する。新たな腫瘍生検が困難な場合は、保存検体を利用することができる。ただし、保存検体は、治験薬投与前180日以内に採取されていること。
10.妊娠する可能性のある女性(化学閉経などの医学的理由により月経がない患者も含む)の場合、同意取得時から治験薬の最終投与後5カ月間及び標準治療薬の最終投与後各薬剤で避妊が規定された期間は二重避妊を行うことに同意した患者。また、同意取得後から治験薬の最終投与後5カ月間及び標準治療薬の最終投与後各薬剤で授乳が制限される期間は授乳しないことに同意した患者。ここで、妊娠する可能性のある女性とは、初潮を経験しており、不妊手術(子宮摘出術、両側卵管結紮又は両側卵巣摘出術など)を受けておらず、閉経していない女性すべてが含まれる。閉経の定義は、特筆すべき理由がないにもかかわらず、12カ月以上連続して無月経であることとする。経口避妊薬又は子宮内避妊器具や避妊ペッサリーを使用している女性は妊娠する可能性があるものとみなす。
11.男性の場合、治験薬投与開始時から治験薬最終投与後4カ月間及び標準治療薬の最終投与後各薬剤で避妊が規定された期間は二重避妊を行うことに同意した患者。ここで避妊については、男性患者又は男性パートナーの精管切除若しくはコンドーム、女性患者又は相手パートナーの卵管結紮、避妊ペッサリー、子宮内避妊器具若しくは経口避妊薬のうち、いずれか2つによる二重避妊を行う必要がある。
12.治験薬投与開始前14日以内に実施した最新の臨床検査値が下記の基準を満たす患者。なお、検査日前14日以内に顆粒球コロニー刺激因子(G-CSF製剤)等の造血因子製剤の投与又は輸血を受けていないこととする。
・好中球数:2,000/mm3以上。
・血小板数:100,000/mm3以上。
・ヘモグロビン:11.0g/dL以上。
・AST(GOT)及びALT(GPT):施設基準値上限の3.0倍以下(ただし、肝転移がある場合、施設基準値上限の5.0倍以下とする)。
・総ビリルビン:施設基準値上限の1.5倍以下。
・アルブミン:3.0g/dL以上。
・クレアチニン:施設基準値上限の1.5倍以下又はクレアチニンクリアランス(実測値又はCockcroft/Gault式による推定値)が40mL/min以上。
13.治験責任医師又は治験分担医師より、本治験の内容について同意文書及び説明文書を用いて十分に説明を受け、自由意思により本治験参加に文書で同意する患者。
登録時に、下記のいずれかの基準に該当すると考えられる患者は除外した。なお、登録から治験薬の初回投与前までに下記のいずれかの基準に抵触した場合は、治験薬の投与を開始せず治験中止とした。
1.オキサリプラチン、イリノテカン、フルオロウラシル又はレボホリナートカルシウムの投与が禁忌とされている患者。
2.臨床的に問題となる下痢(水様便を含む)を認める患者。
3.末梢性運動ニューロパチー又は末梢性感覚ニューロパチーを認める患者。
4.UDPグルクロン酸転移酵素1A1(UGT1A1)遺伝子多型;ホモ接合体(UGT1A1*6/*6、UGT1A1*28/*28)又はヘテロ接合体(複合ヘテロ接合体:UGT1A1*6/*28)を有する患者。
5.抗体製剤に対する高度の過敏反応の合併又は既往を有する患者。
6.手術療法の影響のため、治験責任医師又は治験分担医師が治験薬の有効性及び安全性評価に影響を及ぼすと判断した患者。
7.自己免疫疾患の合併又は慢性的あるいは再発性の自己免疫疾患の既往を有する患者。ただし、1型糖尿病、ホルモン補充療法により対処可能な甲状腺機能低下症又は全身療法を必要としない皮膚疾患(白斑、乾癬、脱毛症など)を合併している患者は登録可能とする。
8.重複がんを有する患者(完全切除された基底細胞がん、ステージIの有棘細胞がん、上皮内がん、粘膜内がん又は表在性膀胱がん、あるいは3年間以上再発が認められない他のがんを有する患者は登録可能とする)。
9.脳又は髄膜に転移巣を有する患者。ただし、無症状かつ治療を必要としない患者は登録可能とする。
10.画像診断又は臨床所見により診断された間質性肺疾患若しくは肺線維症の合併又は既往を有する患者。
11.憩室炎又は症候性消化管潰瘍疾患を合併している患者。
12.薬物療法では管理不能な心嚢液、胸水又は腹水の貯留を認める患者。
13.管理不能な癌性疼痛を合併している患者。
14.骨盤腔を超えて腹水の連続貯蓄があるなどの高度の腹膜播種を認める患者。
15.移植療法を必要とする患者又は移植療法の既往がある患者(自己移植を除く)。
16.登録前90日以内に溶血性貧血の既往歴を有する患者。
17.登録前180日以内に一過性脳虚血発作、脳血管発作、血栓症又は血栓塞栓症(肺動脈塞栓症又は深部静脈血栓症)の既往を有する患者。
18.下記の管理不能又は重大な心血管疾患を有する患者。
・登録前180日以内の心筋梗塞。
・登録前180日以内の管理不能な狭心症。
・New York Heart Association(NYHA)心機能分類III度又はIV度のうっ血性心不全。
・適切な治療にもかかわらず管理不能な高血圧(収縮期血圧150mmHg以上又は拡張期血圧90mmHg以上が24時間以上持続するなど)。
・治療を必要とする不整脈。
19.管理不能な糖尿病を合併している患者。
20.治療を必要とする全身性感染症を有する患者。
21.スクリーニング時の検査で、HIV-1抗体及びHIV-2抗体検査、HTLV-1抗体検査、HBs抗原検査、HCV抗体検査(HCV-RNA陰性を除く)のいずれかが陽性の患者。なお、HCV抗体検査が陽性でも、HCV-RNAが陰性の場合、登録を許容する。
22.スクリーニング時の検査で、HBs抗原検査が陰性であるが、HBs抗体検査又はHBc抗体検査のいずれかが陽性かつHBV-DNA定量が検出感度以上の患者。
23.同意取得前及び治験薬投与前に以下の治療を受けた患者。
・過去にMagrolimab又はCD47-SIRPα経路を標的とした薬剤、Nivolumab、抗PD-1抗体、抗PD-L1抗体、抗PD-L2抗体、抗CD137抗体、抗CTLA-4抗体又はその他のT細胞制御を目的とした抗体療法若しくはがんワクチンを含む薬物療法の治療歴を有する患者。
・同意取得前28日以内に2単位を超える赤血球の輸血を受けた患者。
・治験薬投与前14日以内に局所麻酔を伴う手術療法を受けた患者。
・治験薬投与前14日以内に胸膜又は心膜などの癒着術を受けた患者。
・治験薬投与前14日以内に10mg/日を超えるプレドニゾロン相当量の全身性副腎皮質ホルモン(検査、局所投与、造影剤アレルギー反応の治療及び予防又は放射線療法に伴う浮腫軽減などを目的とした一時的使用は除く)又は免疫抑制剤の投与を受けた患者。
・治験薬投与前28日以内に緩和を目的とした放射線照射を受けた患者。
・治験薬投与前28日以内に全身麻酔を伴う手術療法を受けた患者。
・治験薬投与前28日以内に生ワクチン又は弱毒化ワクチン接種を受けた患者。
・治験薬投与前28日以内(抗体製剤の場合は90日以内)に他の未承認薬(臨床研究による投与や、未承認の配合薬、新剤形薬も含む)の投与を受けた患者。
・治験薬投与前56日以内に放射性医薬品(検査及び診断を目的とした放射性医薬品の使用を除く)の投与を受けた患者。
24.生殖細胞系列のBRCA遺伝子変異(病的変異又は病的変異疑い)を有することが判明している患者
25.妊娠中、授乳中又は妊娠している可能性のある患者(授乳中の患者は、授乳を中断した場合においても、登録不可とする)。
26.認知症の合併などにより同意能力を欠く状態であると判断される患者。
27.その他、治験責任医師又は治験分担医師が治験対象として不適当と判断した患者。
[Magrolimab]
Magrolimabを第1サイクルの1日目に初回投与として1mg/kgを3時間(±30分)かけて静脈内投与し、その後、第1サイクルでは1回20mg/kgまたは30mg/kgを1週間間隔で2時間(±30分)かけて静脈内投与し、第2サイクル以降では1回20mg/kgまたは30mg/kgを2週間間隔で2時間(±30分)かけて静脈内投与し、Magrolimabに関する所定の投与中止基準に該当するまで継続投与した。なお、Magrolimab、NivolumabおよびmFFX療法を同日に投与する場合、Magrolimab、Nivolumab、mFFX療法の順で投与を開始した。
Nivolumab 480mgを4週間間隔で30分間かけて静脈内投与し、Nivolumabに関する所定の投与中止基準に該当するまで継続投与した。なお、Nivolumabの投与は、前回の投与から少なくとも24日以上空け、25日目以降に実施した。
(2a)オキサリプラチンは85mg/m2(体表面積)を2時間かけて静脈内投与した。(2b)レボホリナートカルシウムは200mg/m2(体表面積)を2時間かけて静脈内投与した。(2c)レボホリナートカルシウム投与開始30分後からイリノテカン塩酸塩水和物は150mg/m2(体表面積)を1.5時間かけて静脈内投与した。(2e)フルオロウラシルは2400mg/m2(体表面積)を46時間かけて静脈内投与した。当該(2a)、(2b)、(2c)および(2e)の一連の投与は2週間間隔で実施した。なお、オキサリプラチン、イリノテカン塩酸塩水和物、レボホリナートカルシウムおよびフルオロウラシルはそれぞれ市販品を用いた。
本試験は、スクリーニング期、治療期および後観察期からなる。治験スケジュールの概要を図2に示す。
スクリーニング期は治験薬投与前28日以内とし、治験責任医師または治験分担医師は上記選択基準を満たし、かつ上記除外基準に抵触せず、本治験の対象として適格と判断した患者を組み入れた。
治療期は1サイクル28日間とし、初回の治験薬投与日をサイクル1の1日目とした。第2サイクル以降の各サイクルの1日目は、[28×(サイクル数-1)+1]日とする。Magrolimab、NivolumabおよびmFFX療法についてそれぞれ上記用法・用量に従って、投与を開始し、Magrolimab、NivolumabおよびmFFX療法についての投与基準、減量基準および減量時の投与量に従って投与を継続した。Magrolimab、NivolumabおよびmFFX療法の投与終了(中止)時の評価が終了した時点を治療期終了とした。治験薬を投与したすべての被験者のうち、Magrolimab、NivolumabおよびmFFX療法の投与を中止または終了した被験者は、投与終了(中止)時の評価を実施し、後観察期に移行した。後観察期終了後は、追跡調査を実施した。
被験者は、毎回の投与開始時において、MagrolimabおよびNivolumabに関してこれまでに実施された治験における投与基準を考慮して決定された所定の投与基準をすべてに合致しなければならない。当該基準のいずれかに合致しない場合、予定されたMagrolimabおよびNivolumabの投与を休薬した。休薬した被験者は、週1回又は臨床上必要な場合はそれ以上の頻度で検査を実施し、投与再開の可否を判断した。
治療期において、Magrolimabに関してこれまでに実施された治験における投与中止基準を考慮して決定された所定の投与中止基準のいずれかに該当した被験者はMagrolimabの投与を中止した。
治療期において、Nivolumabに関してこれまでに実施された治験における投与中止基準を考慮して決定された所定の投与中止基準のいずれかに該当した被験者はNivolumabの投与を中止した。
被験者は、毎回の投与開始時において、mFFX療法に関して最新の添付文書を考慮して決定された所定の投与基準をすべて合致しなければならない。投与予定日の臨床検査値が当該基準の条件を満たす状態へ回復するまで投与を延期し、各薬剤の禁忌に該当しないことを確認した上で投与した。
最新の添付文書に準じて減量を実施した。
治療期において、mFFX療法に関して最新の添付文書を考慮して決定された所定の投与中止基準のいずれかに該当した被験者はmFFX療法の投与を中止した。
(画像診断)
胸部、腹部および骨盤のCT/核磁気共鳴画像(MRI)撮影等を実施した。
治験責任医師または治験分担医師は、RECISTガイドライン1.1版に準じて標的病変の腫瘍径を計測し、抗腫瘍効果を判定した。

Claims (23)
- 標準療法、並びに免疫チェックポイント阻害物質と組み合わせて投与されることを特徴とする、CD47阻害物質を有効成分として含む固形がんの進行抑制、再発抑制および/または治療剤。
- 標準療法、並びにCD47阻害物質と組み合わせて投与されることを特徴とする、免疫チェックポイント阻害物質を有効成分として含む固形がんの進行抑制、再発抑制および/または治療剤。
- 標準療法がFOLFOX療法にベバシズマブまたはセツキシマブを加えた併用療法である、請求項1または請求項2に記載の剤。
- FOLFOX療法にベバシズマブまたはセツキシマブを加えた併用療法が、
(1x)ベバシズマブ 5mg/kg(体重)を静脈内投与すること、またはセツキシマブ 1回目は400mg/m2(体表面積)、2回目以降は250mg/m2(体表面積)を静脈内投与すること、
(1a)オキサリプラチン 85mg/m2(体表面積)を静脈内投与すること、
(1b)レボホリナートカルシウム 200mg/m2(体表面積)を静脈内投与すること、

(1a)オキサリプラチン 85mg/m2(体表面積)を静脈内投与すること、
(1b)レボホリナートカルシウム 200mg/m2(体表面積)を静脈内投与すること、
(1c)フルオロウラシル 400mg/m2(体表面積)を急速静脈内投与すること、および
(1d)当該(1c)のフルオロウラシルの投与終了後、さらにフルオロウラシル 2400mg/m2(体表面積)を静脈内持続投与すること、
を含み、当該(1x)、(1a)、(1b)、(1c)および(1d)の一連の投与が2週間間隔で実施される、療法である、請求項4に記載の剤。 - 固形がんが、結腸・直腸がんである、請求項1~6のいずれか一項に記載の剤。
- 結腸・直腸がんが、根治切除不能な進行または再発の結腸・直腸がんである、請求項7に記載の剤。
- 根治切除不能な進行または再発の結腸・直腸がんに対する全身性抗悪性腫瘍剤による治療歴のない患者に一次治療として投与されることを特徴とする、請求項1~8のいずれか一項に記載の剤。
- 標準療法がFOLFIRINOX療法またはその減量レジメンである、請求項1または請求項2に記載の剤。
- FOLFIRINOX療法が、
(2a)オキサリプラチン 85mg/m2(体表面積)を静脈内投与すること、
(2b)レボホリナートカルシウム 200mg/m2(体表面積)を静脈内投与すること、
(2c)イリノテカン塩酸塩水和物 180mg/m2(体表面積)を静脈内投与すること、
(2d)フルオロウラシル 400mg/m2(体表面積)を急速静脈内投与すること、および
(2e)当該(2d)のフルオロウラシルの投与終了後、さらにフルオロウラシル 2400mg/m2(体表面積)を静脈内持続投与すること、
を含み、当該(2a)、(2b)、(2c)、(2d)および(2e)の一連の投与が2週間間隔で実施される、療法である、請求項10に記載の剤。 - FOLFIRINOX療法の減量レジメンが、
(2a)オキサリプラチン 85mg/m2(体表面積)を静脈内投与すること、
(2b)レボホリナートカルシウム 200mg/m2(体表面積)を静脈内投与すること、
(2c)イリノテカン塩酸塩水和物 150mg/m2(体表面積)を静脈内投与すること、および
(2e)フルオロウラシル 2400mg/m2(体表面積)を静脈内持続投与すること、
を含み、当該(2a)、(2b)、(2c)および(2e)の一連の投与が2週間間隔で実施される、療法である、請求項10に記載の剤。 - 固形がんが、遠隔転移を有する膵臓がんである、請求項1、2、10、11および12のいずれか一項に記載の剤。
- 遠隔転移を有する膵臓がんに対する全身性抗悪性腫瘍剤による治療歴のない患者に一次治療として投与されることを特徴とする、請求項1、2、10、11、12および13のいずれか一項に記載の剤。
- CD47阻害物質が、抗CD47抗体である、請求項1~14のいずれか一項に記載の剤。
- 抗CD47抗体が、Magrolimabである、請求項15に記載の剤。
- Magrolimabとして初回投与時は1mg/kg(体重)を静脈内投与し、2回目投与以降は1回15mg/kg(体重)~30mg/kg(体重)を1週間間隔または2週間間隔で静脈内投与されることを特徴とする、請求項16に記載の剤。
- Magrolimabとして初回投与時は1mg/kg(体重)を静脈内投与し、2回目投与以降は1回30mg/kg(体重)を1週間間隔または2週間間隔で静脈内投与されることを特徴とする、請求項16または請求項17に記載の剤。
- 免疫チェックポイント阻害物質が、抗PD-1抗体、抗PD-L1抗体または抗CTLA-4抗体である、請求項1~18のいずれか一項に記載の剤。
- 免疫チェックポイント阻害物質が、抗PD-1抗体である、請求項1~19のいずれか一項に記載の剤。
- 抗PD-1抗体が、Nivolumab、Cemiplimab、Pembrolizumab、Spartalizumab、Tislelizumab、AMP-514、Dostarlimab、Toripalimab、Camrelizumab、Genolimzumab、Sintilimab、STI-A1110、ENUM 388D4、ENUM 244C8、GLS010、Retifanlimab、Balstilimab、CS1003、Serplulimab、BAT-1306、AK105、AK103、BI 754091、LZM009、CMAB819、Sym021、Geptanolimab、SSI-361、JY034、HX008、ISU106、Budigalimab、Prolgolimab、Sasanlimab、CX-188、CetrelimabまたはZimberelimabである、請求項19または請求項20に記載の剤。
- 抗PD-1抗体がNivolumabである、請求項21に記載の剤。
- Nivolumabとして1回480mgを4週間間隔で静脈内投与されることを特徴とする、請求項22に記載の剤。
