Abstract
Description
ウベニメクス((2S)-2-[(2S,3R)-3-アミノ-2-ヒドロキシ-4-フェニルブタノイルアミノ]-4-メチルペンタン酸/ベスタチン(登録商標))は、マクロファージ、T細胞、NK細胞等を非特異的に活性化、及び/又はアミノペプチダーゼ類を介して免疫担当細胞表面に結合し抗腫瘍免疫能を活性化することにより、抗腫瘍作用を発現する薬剤であり「成人急性非リンパ性白血病に対する完全寛解導入後の維持強化化学療法剤との併用による生存期間の延長。」にて臨床で用いられている(非特許文献1)。
[1] 免疫チェックポイント阻害剤である抗PD-1抗体、抗PD-L1抗体及び/又は抗CTLA-4抗体と組み合わせて投与するための、ウベニメクスを有効成分とする抗腫瘍剤。
[2] ウベニメクスの用量が、単回投与の際の血中濃度時間曲線下面積(AUC)が3.00~50.00μg・hr/mLとなる用量である、[1]に記載の抗腫瘍剤。
[3] ウベニメクスが、連日投与の用法である、[1]又は[2]に記載の抗腫瘍剤。
[5] ウベニメクスの用量が、単回投与の際のAUCが3.00~50.00μg・hr/mLとなる用量である、[4]に記載の抗腫瘍剤。
[6] 免疫チェックポイント阻害剤である抗PD-1抗体、抗PD-L1抗体及び/又は抗CTLA-4抗体と組み合わせて投与することにより、抗腫瘍性サイトカイン産生のための、ウベニメクスを有効成分とする医薬。
[7] ウベニメクスの用量が、単回投与の際のAUCが3.00~50.00μg・hr/mLとなる用量である、[6]に記載の医薬。
[8] 免疫チェックポイント阻害剤である抗PD-1抗体、抗PD-L1抗体及び/又は抗CTLA-4抗体と組み合わせて投与することにより、腫瘍浸潤性リンパ球を増加させるための、ウベニメクスを有効成分とする医薬。

[10] 免疫チェックポイント阻害剤である抗PD-1抗体、抗PD-L1抗体及び/又は抗CTLA-4抗体と組み合わせて投与することにより、小細胞肺癌、非小細胞肺癌、食道癌、頭頚部癌、脳腫瘍、結腸・直腸癌、結腸腺癌、胃癌、乳癌、肝細胞癌、膵臓癌、胆道癌、腎臓癌、前立腺癌、膀胱癌、卵巣癌、子宮頸癌、甲状腺癌およびメラノーマからなる群から選択される悪性腫瘍の治療に用いられる、ウベニメクスを有効成分とする医薬。
[11] ウベニメクスの用量が、単回投与の際の血中濃度時間曲線下面積(AUC)が3.00~50.00μg・hr/mLとなる用量である、[10]に記載の医薬。
[12] ウベニメクスが、連日投与の用法である、[10]又は[11]に記載の医薬。
ウベニメクスは、(2S)-2-[(2S,3R)-3-アミノ-2-ヒドロキシ-4-フェニルブタノイルアミノ]-4-メチルペンタン酸であり、アミノペプチダーゼ阻害活性を有するジペプチド化合物である。本発明において、ウベニメクスは塩フリー体であっても、塩酸塩、リン酸塩等の薬学的に許容される塩として用いても良い。ウベニメクスは、「ベスタチン(登録商標)カプセル」として10mg製剤及び30mg製剤が臨床に提供されており、これを用いても良い。
ウベニメクスの用法は、経口的に服用しても良く、静脈内投与等の非経口的な経路にて用いても良い。経口投与が好ましい。また、ウベニメクスは連日投与で用いることが好ましく、治療サイクル期間に亘り、経口的に連日投与にて用いることが好ましい。
抗PD-1抗体、抗PD-L1抗体、抗CTLA-4抗体は、多くの抗腫瘍剤が上市もしくは開発中であり、これらを適用しても良い。
抗PD-1抗体としてはニボルマブ(オプジーボ(登録商標))及びペムブロリズマブ(キイトルーダ(登録商標))が挙げられる。また、ピジリズマブ(CT-011)、PDR-001、JS001、STI-A1110、AMP-224、AMP-514(MEDI0680)等を用いても良い。
抗PD-L1抗体としては、アテゾリズマブ(テセントリク(登録商標))、デュルバルマブ(イミフィンジ(登録商標))、アベルマブ(バベンチオ(登録商標))が挙げられる。また、BMS-936559(MDX1105)、LY3300054等を用いても良い。
抗CTLA-4抗体としては、イピリムマブ(ヤーボイ(登録商標))が挙げられる。また、トレメリムマブ(CP-675206)等を用いても良い。
本発明において、ウベニメクスと併用するICIは抗PD-1抗体及び/又は抗PD-L1が好ましい。ウベニメクスと抗PD-1抗体の併用がより好ましい。
本発明は、ウベニメクスとICIの組み合わせ投与が、それぞれの単剤使用と比較して抗腫瘍性免疫を増強させることを見出し、抗腫瘍免疫増強剤としての新たな用途を提供することも含む。
抗腫瘍性及び炎症性サイトカイン等の産生誘導には、ウベニメクスの用量は、医薬的に有効量で用いればよい。好ましくは単回投与の際のAUCが3.00μg・hr/mLより大きく50.00μg・hr/mLより小さい範囲となる用量で適用する。また、好ましくはCmaxが2.0μg/mLより大きくなる用量で適用する。より好ましくはAUCが5.00μg・hr/mLより大きく50.00μg・hr/mLより小さい範囲となる用量であり、10.00μg・hr/mLより大きく50.00μg・hr/mLより小さい範囲となる用量が殊更好ましい。
腫瘍浸潤性リンパ球の誘導には、ウベニメクスの用量は、医薬的に有効量で用いればよい。好ましくは単回投与の際のAUCが3.00μg・hr/mLより大きく50.00μg・hr/mLより小さい範囲となる用量で適用する。また、好ましくはCmaxが2.0μg/mLより大きくなる用量で適用する。より好ましくはAUCが5.00μg・hr/mLより大きく50.00μg・hr/mLより小さい範囲となる用量であり、10.00μg・hr/mLより大きく50.00μg・hr/mLより小さい範囲となる用量が殊更好ましい。
Tregの誘導抑制には、ウベニメクスの用量は、医薬的に有効量で用いればよい。好ましくは単回投与の際のAUCが3.00μg・hr/mLより大きく50.00μg・hr/mLより小さい範囲となる用量で適用する。また、好ましくはCmaxが2.0μg/mLより大きくなる用量で適用する。より好ましくはAUCが5.00μg・hr/mLより大きく50.00μg・hr/mLより小さい範囲となる用量であり、10.00μg・hr/mLより大きく50.00μg・hr/mLより小さい範囲となる用量が殊更好ましい。
また、エルロチニブ、ラパチニブ、ゲフィチニブ、アファチニブ、オシメルチニブ、アキシチニブ、スニチニブ、ソラフェニブ、レゴラフェニブ、レンバチニブ、アレクチニブ、クリゾチニブ、パゾパニブ等のチロシンキナーゼ阻害剤。ボルテゾミブ、カルフィルゾミブ、イキサゾミブ等のプロテアソーム阻害剤。パルボシクリブ等のサイクリン依存性キナーゼ阻害剤。エベロリムス、テムシロリムス等のmTOR阻害剤。トラスツズマブ、セツキシマブ、ネシツムマブ、ベバシズマブ、パニツムマブ、ラムシルマブ等の抗成長因子(受容体)抗体。他にも公知の抗腫瘍剤を用いても良い。
他の抗腫瘍剤は、医薬的に有効量にて公知の用法にて組み合わせて用いることが好ましい。
(1)腫瘍細胞
マウスメラノーマB16-F0は、ATCC(The American Type Culture Collection)から入手した。これを10%ウシ胎仔血清(Corning International、Inc.)及び100μg/mLカナマイシン硫酸塩を含むDMEM(Thermo Fisher Scientific Inc.)中で、CO2インキュベーター内(37℃、5%二酸化炭素下)で培養したものを用いた。
マウス結腸腺癌細胞株MC38は、GenOway S.A.から入手した。これを10%ウシ胎仔血清(Corning International、Inc.)、2mM L-glutamine(Thermo Fisher Scientific Inc.)、10mM HEPES(Thermo Fisher Scientific Inc.)、1mM Sodium pyruvate(富士フイルム和光純薬株式会社)、0.1mM MEM Non-Essential Amino Acids Solution(Thermo Fisher Scientific Inc.)、penicillin/streptomycin(富士フイルム和光純薬株式会社)を含むDMEM(Thermo Fisher Scientific Inc.)中で、CO2インキュベーター内(37℃、5%二酸化炭素下)で培養したものを用いた。
(2)被験物質
ウベニメクスは日本化薬株式会社より入手した。
抗マウスPD-1抗体(クローンJ43)及びアイソタイプコントロール抗体IgG(polyclonal Armenian hamster IgG)はBioXcell社より入手した。
ウベニメクスは注射用蒸留水(株式会社大塚製薬工場)に溶解して用いた。抗マウスPD-1抗体及びIgGはPBS(-)(Thermo Fisher Scientific Inc.)で希釈して用いた。
マウスメラノーマ細胞B16-F0の皮下移植モデルを用いて、ウベニメクス及び抗マウスPD-1抗体の併用投与による抗腫瘍効果を評価した。また、同時に、サイトカイン及び免疫系マーカーの変動を確認した。
雌性C57BL/6Nマウス(日本チャールス・リバー株式会社)の皮下に2×105個のB16-F0を移植して皮下移植モデルを作製した。
B16-F0の皮下移植モデルマウスに、ウベニメクスを5又は50mg/kgの用量で移植翌日から移植後14日まで1日1回、計14回経口投与した。更に、抗マウスPD-1抗体を移植後4、7、11及び14日に200μg/bodyの用量で腹腔内投与した。対照として抗マウスPD-1抗体のアイソタイプコントロールであるIgG抗体を抗マウスPD-1抗体と同一の条件で投与した。各投与群構成について表1にまとめた。
統計解析には、統計解析ソフトEXSUS(ver.10.0、株式会社CACクロア)を用いて対照群と各投与群、併用群とそれぞれの単剤群でWilcoxon検定を実施した。有意水準P<0.05で両側統計学的分析を行った。
各投与群における腫瘍増殖推移を図1に示した。移植後15日における対照群(IgG投与)に対する相対平均腫瘍体積(T/C)において、ウベニメクス5mg/kg及び50mg/kg投与群、並びに抗PD-1抗体投与群は対照群に対して腫瘍増殖抑制効果を示さなかったのに対し、ウベニメクスの高用量と抗PD-1抗体の併用群のT/Cは68%であり腫瘍増殖抑制効果を示した。ウベニメクスと抗PD-1抗体の併用は、それぞれの単独投与では認められなかった腫瘍増殖抑制効果を奏することが確認された。
実施例1.1の抗腫瘍効果を相対腫瘍体積の個別値で解析したところ、移植後15日のT/Cが100%未満を示した個体は、各単剤投与では群の半数例以下だったのに対して、ウベニメクス低用量と抗PD-1抗体との併用群及びウベニメクス高用量と抗PD-1抗体との併用群では、いずれも過半数となる8匹中5匹であった(図2)。
各投与群における血漿中の抗腫瘍性サイトカインとして、IFN-γ及びTNF-α濃度を、レビスMouse IFN-γ ELISA kit、レビスMouse TNF-α ELISA Kit(富士フイルムワコーシバヤギ株式会社)を用いて測定した。結果を図3及び図4に示した。
また、ウベニメクスと抗PD-1抗体との併用群は、対照群、ウベニメクス単剤の各投与量群及び抗PD-1抗体単独での投与群に比してTNF-αの産生増強効果を示した。ウベニメクス高用量と抗PD-1抗体との併用群では、対照群及びウベニメクス単剤の高用量群に比して有意に増加した(P値0.045、図4)。
IFN-γ及びTNF-αは、免疫系ネットワークを連鎖的に活性化して抗腫瘍効果に寄与する抗腫瘍性サイトカインとして知られている。ウベニメクスと抗PD-1抗体の併用群が腫瘍増殖抑制効果を示したことから、IFN-γ及びTNF-αは腫瘍増殖抑制に寄与している可能性が示唆された。
抗腫瘍効果を確認後、剖検により採取した腫瘍組織について、フローサイトメトリー法により腫瘍浸潤リンパ球の解析を行った。
イムノフェノタイピングは、抗マウスCD45抗体、抗マウスCD3抗体、抗マウスCD4抗体、抗マウスCD8抗体、抗マウス/ヒトCD11b抗体、抗マウスCD25抗体、抗マウス/ヒトFOXP3抗体、抗マウスCD16/32抗体(BioLegend)、Lysing Buffer(BD Biosciences)、True-Nuclear Transcription Factor Buffer Set(BioLegend)、Tumor Dissocioation Kit、mouse(Milteny biotec)を用いた。
腫瘍片をTumor Dissocioation Kitの酵素液中で細切し、gentleMACS Octo Dissociatorで機械的分散後、37℃で緩やかに振盪しながら40分間インキュベートした。インキュベート終了後、再度、機械的分散処理を行った。10%FBS含有RPMI1640を加え、70μmのセルストレーナーを通し、遠心処理後、細胞塊を10%FBS含有RPMI1640に再懸濁し、これを腫瘍片から分散調製した単細胞懸濁液とした。単細胞懸濁液の一部を分取し、Lysing Bufferで溶血処理、抗マウスCD16/32抗体でFc受容体のブロッキング処理後、細胞表面マーカーCD45、CD3、CD4、CD8、CD11bを染色し、フローサイトメーターLSRII(BD Biosciences)で測定した。
評価は、CTLであるCD8陽性T細胞(CD45+、CD3+、CD8+)を指標にした。CD8陽性T細胞率は、フローサイトメーター測定における単細胞懸濁液中の全生細胞画分のカウント数とCD8陽性T細胞のカウント数から算出したCD8陽性T細胞率を用いて、単細胞懸濁液中の総生細胞数の総CD8陽性T細胞数を算出し、生細胞10000個当たりのCD8陽性T細胞数として算出した。各投与群におけるCD8陽性T細胞数の結果を図5に示した。
抗腫瘍効果を確認後、剖検により採取した血液についてフローサイトメトリー法により末梢血中リンパ球の解析を行った
末梢血をLysing Bufferで溶血処理し、末梢血単核細胞(PBMC)とした後、抗マウスCD16/32抗体でFc受容体をブロッキングし、細胞表面マーカーCD45、CD3、CD4、CD8、CD25を染色した。PBMCを固定、透過処理し、核内因子FOXP3を染色し、フローサイトメーターLSRII(BD Biosciences)で測定した。
評価は、CD45+、CD3+、CD4+、CD25+、FOXP3+で同定されるTregを指標とした。CD45陽性細胞中のTregの存在比率を算出し、総白血球数の値から末梢血1μLあたりのTreg数を算出した。各投与群におけるTreg数の結果を図7に示した。
以上から、ウベニメクスと抗PD-1抗体の組み合わせ投与は、ウベニメクスが有するリンパ球活性化作用を基に、腫瘍中CTL数の増加、抗腫瘍性サイトカイン産生増加、腫瘍免疫寛容状態の解除など、複数の腫瘍免疫系ネットワークを活性化して抗腫瘍効果を発揮できることが示された。
絶食後の9週齢雌性C57BL/6Nマウス(日本チャールス・リバー株式会社)に、ウベニメクスの5mg/kg又は50mg/kgを各群9匹ずつ単回経口投与した。採血時点は投与後5、10、30分、1、3、6、8、12及び24時間の9時点とし、投与後5分から6時間まではイソフルラン麻酔下で頸静脈より0.1mL採血し、投与後8時間以降はイソフルラン麻酔下で後大静脈より全採血した。各時点n=3を確保するため、1個体から3時点採血し、3匹1組で動物を入れ替えるスパース採血とした。得られた血液から血漿を調製後、血漿中ウベニメクス濃度を液体クロマトグラフ質量分析計(LC/MS/MS)で測定した。各投与量における薬物動態パラメータを表2に示す。
マウス結腸腺癌細胞MC38の皮下移植モデルを用いて、ウベニメクス及び抗マウスPD-1抗体の併用投与による抗腫瘍効果を評価した。
雌性C57BL/6Nマウス(日本チャールス・リバー株式会社)の皮下に1×105個のMC38を移植して皮下移植モデルを作製した。MC38の皮下移植モデルマウスに、ウベニメクスを0.5又は5mg/kgの用量で移植翌日から移植後24日まで1日1回、計24回経口投与した。更に、抗マウスPD-1抗体を移植後7、11、14、18、21及び24日に50μg/bodyの用量で腹腔内投与した。対照として抗マウスPD-1抗体のアイソタイプコントロールであるIgG抗体を抗マウスPD-1抗体と同一の条件で投与した。各投与群構成について表3にまとめた。
ウベニメクスの用量依存的な抗腫瘍効果が確認された。また、ウベニメクス5mg/kgと抗PD-1抗体の併用群では、単剤よりも強い効果が認められた(対照群に対するP値:0.002、ウベニメクス5mg/kg群に対するP値:0.015、ウベニメクス0.5mg/kgと抗PD-1抗体の併用群に対するP値:0.039、図9)。
マウス結腸腺癌細胞MC38の皮下移植モデルを用いて、ウベニメクス及び抗マウスPD-1抗体の併用投与による血漿中サイトカイン推移を評価した。
雌性C57BL/6Nマウス(日本チャールス・リバー株式会社)の皮下に1×105個のMC38を移植して皮下移植モデルを作製した。MC38の皮下移植モデルマウスに、ウベニメクスを5mg/kgの用量で移植翌日から移植後24日まで1日1回、計24回経口投与した。更に、抗マウスPD-1抗体を移植後7、11、14、18、21及び24日に50μg/bodyの用量で腹腔内投与した。対照として抗マウスPD-1抗体のアイソタイプコントロールであるIgG抗体を抗マウスPD-1抗体と同一の条件で投与した。各投与群構成について表4にまとめた。
Claims (12)
- 免疫チェックポイント阻害剤である抗PD-1抗体、抗PD-L1抗体及び/又は抗CTLA-4抗体と組み合わせて投与するための、ウベニメクスを有効成分とする抗腫瘍剤。
- ウベニメクスの用量が、単回投与の際の血中濃度時間曲線下面積(AUC)が3.00~50.00μg・hr/mLとなる用量である、請求項1に記載の抗腫瘍剤。
- ウベニメクスが、連日投与の用法である、請求項1又は2に記載の抗腫瘍剤。
- ウベニメクスと組み合わせて投与するための、免疫チェックポイント阻害剤である抗PD-1抗体、抗PD-L1抗体及び抗CTLA-4抗体からなる群から選択される1種を有効成分とする抗腫瘍剤。
- ウベニメクスの用量が、単回投与の際の血中濃度時間曲線下面積(AUC)が3.00~50.00μg・hr/mLとなる用量である、請求項4に記載の抗腫瘍剤。
- 免疫チェックポイント阻害剤である抗PD-1抗体、抗PD-L1抗体及び/又は抗CTLA-4抗体と組み合わせて投与することにより、抗腫瘍性サイトカイン産生を亢進させるための、ウベニメクスを有効成分とする医薬。
- ウベニメクスの用量が、単回投与の際の血中濃度時間曲線下面積(AUC)が3.00~50.00μg・hr/mLとなる用量である、請求項6に記載の医薬。
- 免疫チェックポイント阻害剤である抗PD-1抗体、抗PD-L1抗体及び/又は抗CTLA-4抗体と組み合わせて投与することにより、腫瘍浸潤性リンパ球を増加させるための、ウベニメクスを有効成分とする医薬。
- ウベニメクスの用量が、単回投与の際の血中濃度時間曲線下面積(AUC)が3.00~50.00μg・hr/mLとなる用量である、請求項8に記載の医薬。
- 免疫チェックポイント阻害剤である抗PD-1抗体、抗PD-L1抗体及び/又は抗CTLA-4抗体と組み合わせて投与することにより、小細胞肺癌、非小細胞肺癌、食道癌、頭頚部癌、脳腫瘍、結腸・直腸癌、結腸腺癌、胃癌、乳癌、肝細胞癌、膵臓癌、胆道癌、腎臓癌、前立腺癌、膀胱癌、卵巣癌、子宮頸癌、甲状腺癌およびメラノーマからなる群から選択される悪性腫瘍の治療に用いられる、ウベニメクスを有効成分とする医薬。
