Abstract
本発明に係る医薬は、IL-34を発現する腫瘍を治療および/または予防するための医薬であって、エストロゲン受容体シグナル阻害剤と、殺細胞性抗がん剤または免疫チェックポイント阻害剤と、を有効成分として含有する。
Description
本発明は、IL-34を発現する腫瘍を治療および/または予防するための医薬に関する。
骨髄由来抑制細胞(myeloid-derived suppressor cell;MDSC)は、骨髄由来の免疫抑制細胞であり、腫瘍微小環境の構築に関与している。MDSCは、分化、形態および機能において不均質(heterogeneous)な細胞集団であり、少なくとも単球系骨髄由来抑制細胞(monocytic MDSC;M-MDSC)と多形核細胞系骨髄由来抑制細胞(polymorphonuclear MDSC;PMN-MDSC)の2つのサブセットに分けられる。MDSCは、免疫抑制および血管新生に関与していると報告されている(非特許文献1、2参照)。エストロゲン受容体(ER)阻害剤は、MDSCに作用し、腫瘍微小環境に影響を及ぼすと報告されている(非特許文献3参照)。
一方、トリプルネガティブ乳がん(triple negative breast cancer;TNBC)は、エストロゲン受容体(ER)、プロゲステロン受容体(PgR)およびヒト上皮増殖因子受容体2型(HER2)の発現を伴わない悪性腫瘍である。TNBCは、他のタイプの乳がんと比較して、より高い再発率、再発後の急速な進行を示し、予後が不良である。このため、TNBCに対する新たな治療法が望まれている。最近、本発明者らによって、TNBCではインターロイキン-34(IL-34)が高発現していると報告されている(非特許文献4参照)。

本発明者は、TNBCにおけるIL-34の作用を解析したところ、IL-34はMDSCの分化に関与していることを突き止めた。さらに、本発明者は、IL-34による上記の作用は、ERシグナルも必要であることも突き止めた。そして、本発明者は、これらの知見からIL-34を発現している腫瘍の新たな治療法を見出せるのではないかと考えた。
本発明の目的は、IL-34を発現する腫瘍を治療および/または予防するための医薬を提供することである。また、本発明の別の目的は、IL-34を発現する腫瘍の治療方法を提供することである。
本発明者は、上記課題を解決するべく鋭意研究を行った結果、IL-34を発現する腫瘍において、ERシグナルを抑制することで、IL-34によるMDSCを介した免疫の抑制などを解除できることを見出し、さらに検討を加えて本発明を完成させた。
すなわち、本発明は、以下のIL-34を発現している腫瘍を治療および/または予防するための医薬、ならびにIL-34を発現している腫瘍の治療方法に関する。

本発明によれば、IL-34を発現している腫瘍を治療および/または予防するための医薬、ならびにIL-34を発現している腫瘍の治療方法を提供することができる。
本発明に係る医薬は、IL-34を発現する腫瘍を治療および/または予防するための医薬であって、エストロゲン受容体(ER)シグナル阻害剤を有効成分として含有し、その他任意の成分を含むことができる。ERシグナル阻害剤は、殺細胞性抗がん剤および/または免疫チェックポイント阻害剤と併用投与される。したがって、本発明に係る医薬は、ERシグナル阻害剤に加えて、さらに免疫チェックポイント阻害剤および/または殺細胞性抗がん剤を有効成分として含有してもよい。
すなわち、本発明の第1の態様は、ERシグナル阻害剤と、殺細胞性抗がん剤または免疫チェックポイント阻害剤と、を有効成分として含有する、IL-34を発現する腫瘍を治療および/または予防するための医薬に関する。また、本発明の第2の態様は、免疫チェックポイント阻害剤または殺細胞性抗がん剤と併用投与されるための、ERシグナル阻害剤を有効成分として含有する、IL-34を発現している腫瘍を治療および/または予防するための医薬に関する。また、本発明の第3の態様は、腫瘍がIL-34を発現しているか否かを調べる工程と、前記腫瘍がIL-34を発現している場合に、ERシグナル阻害剤と、殺細胞性抗がん剤または免疫チェックポイント阻害剤とを患者に投与する工程と、を有する、IL-34を発現する腫瘍の治療方法に関する。また、本発明の第4の態様は、IL-34を発現している腫瘍を治療および/または予防するための医薬の製造のための、ERシグナル阻害剤と、殺細胞性抗がん剤または免疫チェックポイント阻害剤との使用に関する。
たとえば、本発明の一実施の形態に係る医薬は、ERシグナル阻害剤および殺細胞性抗がん剤を有効成分として含有する。また、本発明の別の実施の形態に係る医薬は、ERシグナル阻害剤および免疫チェックポイント阻害剤を有効成分として含有する。また、本発明のさらに別の実施の形態に係る医薬は、ERシグナル阻害剤、殺細胞性抗がん剤および免疫チェックポイント阻害剤を有効成分として含有する。
以下、特に断らない限り、本発明の詳細は、IL-34などの生体高分子および分子生物学的機構を含め、ヒトを代表例として説明される。
(エストロゲン受容体シグナル阻害剤)
本発明に係る医薬は、エストロゲン受容体(ER)シグナル阻害剤を含有する。ERシグナル阻害剤は、メカニズムにかかわらず、ERを介するシグナルを遮断または抑制する物質である。ERシグナル阻害剤の種類は、特に限定されず、治療対象の腫瘍に応じて公知のものから適宜選択されうる。たとえば、ERシグナル阻害剤は、エストロゲン受容体(ER)阻害剤またはエストロゲン産生阻害剤である。
本発明に係る医薬は、エストロゲン受容体(ER)シグナル阻害剤を含有する。ERシグナル阻害剤は、メカニズムにかかわらず、ERを介するシグナルを遮断または抑制する物質である。ERシグナル阻害剤の種類は、特に限定されず、治療対象の腫瘍に応じて公知のものから適宜選択されうる。たとえば、ERシグナル阻害剤は、エストロゲン受容体(ER)阻害剤またはエストロゲン産生阻害剤である。
ER阻害剤は、メカニズムにかかわらず、エストロゲンのERへの結合を妨害または阻害してERシグナルを遮断または抑制する物質である。ER阻害剤の種類は、特に限定されず、治療対象の腫瘍に応じて公知のものから適宜選択されうる。ER阻害剤の例には、メチルピペリジノピラゾール、フルベストラント、タモキシフェン、アムセネストラント、ジレデストラント、カミゼストラント、エラケストラント、リントデストラント、ラロキシフェン、ヨードキシフェン、LY353381、LY117081、トレミフェン、4-[7-(2,2-ジメチル-1-オキソプロポキシ-4-メチル-2-[4-[2-(1-ピペリジニル)エトキシ]フェニル]-2H-1-ベンゾピラン-3-イル]-フェニル-2,2-ジメチルプロパノアート、4,4’-ジヒドロキシベンゾフェノン-2,4-ジニトロフェニル-ヒドラゾン、SH646などが含まれる。本発明において好ましいER阻害剤は、フルベストラント、タモキシフェン、アムセネストラント、ジレデストラント、カミゼストラント、エラケストラント、リントデストラントなどのエストロゲン受容体発現低下薬(Selective estrogen receptor downregulator;SERD)である。ER阻害剤は、1種類を単独で使用してもよいが、2種以上の併用がより好ましい。
エストロゲン産生阻害剤は、メカニズムにかかわらず、ERリガンドであるエストロゲンの産生を停止または抑制する物質である。エストロゲン産生阻害剤の種類は、特に限定されず、治療対象の腫瘍に応じて公知のものから適宜選択されうる。エストロゲン産生阻害剤の例には、アナストロゾールやエキセメスタン、レトロゾールなどのアロマターゼ阻害剤、ゴセレリンやリュープロレリンなどのLH-RHアゴニスト製剤などが含まれる。エストロゲン産生阻害剤は、1種類を単独で使用してもよいが、2種以上の併用がより好ましい。
なお、ERシグナルを抑制する方法としては、上記のようにER阻害剤またはエストロゲン産生阻害剤を用いる方法の他に、他の物質(例えば抗体)をERリガンドに結合させてERリガンドがERと結合するのを阻害する方法も考えられる。この場合は、他の物質がERシグナル阻害剤として機能する。

免疫チェックポイント阻害剤は、自己に対する免疫応答や過剰な免疫応答を抑制する作用を持つ免疫チェックポイント分子と称される生体分子を標的とし、その分子の発現または生理活性を阻害または抑制する物質である。免疫チェックポイント阻害剤の種類は、特に限定されず、治療対象の腫瘍に応じて公知のものから適宜選択されうる。免疫チェックポイント分子としては、PD-L1/PD-L2およびその受容体であるPD-1、CD80/CD86およびその受容体であるCTLA-4、ガレクチン-9およびその受容体であるTIM-3、HVEMおよびその受容体であるBTLAなどが知られている。本発明においては、PD-1、PD-L1、PD-L2、CTLA-4、CD80およびCD86からなる群から選択される少なくとも1つを阻害の標的とすることが好ましい。免疫チェックポイント阻害剤の例には、免疫チェックポイント分子の生理活性を阻害する低分子化合物、免疫チェックポイント分子に特異的に結合する抗体、免疫チェックポイント分子の発現を抑制する阻害性核酸などが含まれる。本発明において好ましい免疫チェックポイント阻害剤は、免疫チェックポイント分子に対する特異抗体、好ましくは抗PD-1抗体、抗PD-L1抗体、抗PD-L2抗体、抗CTLA-4抗体などである。これらの典型例は、ニボルマブ(抗PD-1抗体、商品名:オプジーボ)、イピリムマブ(抗CTLA-4抗体、商品名:ヤーボイ)、ペムブロリズマブ(抗PD-1抗体、商品名:キイトルーダ)などの臨床で既に使用されている抗体である。免疫チェックポイント阻害剤は、1種類を単独で使用してもよいが、2種以上の併用がより好ましい。

(殺細胞性抗がん剤)
殺細胞性抗がん剤は、細胞中のDNAや微小管などを標的とし、がん細胞を殺傷したり増殖を抑制したりする物質である。殺細胞性抗がん剤の種類は、特に限定されず、治療対象の腫瘍に応じて公知のものから適宜選択されうる。殺細胞性抗がん剤の例には、アルキル化剤、代謝拮抗剤、プラチナ製剤、トポイソメラーゼ阻害剤、微小管作用抗癌剤、抗がん性抗生物質が含まれる。アルキル化剤の例には、イホスファミド、カルボコン、シクロホスファミド、ダカルバジン、チオテパ、テモゾロミド、ニムスチン、ブスルファン、プロカルバジン、メルファラン、ラニムスチンなどが含まれる。代謝拮抗剤の例には、エノシタビン、カペシタビン、カルモフール、クラドリビン、ゲムシタビン、シタラビン、シタラビンオクホスファート、テガフール、テガフール・ウラシル、テガフール・ギメラシル・オテラシルカリウム、ドキシフルリジン、ネララビン、ヒドロキシカルバミド、フルオロウラシル、フルダラビン、ペメトレキセド、ペントスタチン、メルカプトプリン、メトトレキサートなどが含まれる。プラチナ製剤の例には、オキサリプラチン、カルボプラチン、シスプラチン、ネダプラチンなどが含まれる。トポイソメラーゼ阻害剤の例には、イリノテカン、エトポシド、ノギテカンなどが含まれる。微小管作用抗癌剤の例には、エリブリン、ドセタキセル、ノギテカン、パクリタキセル、ビノレルビン、ビンクリスチン、ビンデシン、ビンブラスチンなどが含まれる。抗がん性抗生物質の例には、アクチノマイシンD、アクラルビシン、アムルビシン、イダルビシン、エピルビシン、ジノスタチンスチマラマー、ダウノルビシン、ドキソルビシン、ピラルビシン、ブレオマイシン、ペプロマイシン、マイトマイシンC、ミトキサントロン、リポソーマルドキソルビシンなどが含まれる。殺細胞性抗がん剤は、1種類を単独で使用してもよいが、2種以上の併用がより好ましい。

(医薬または医薬組成物)
本発明に係る医薬は、上記のERシグナル阻害剤(例えば、ER阻害剤またはエストロゲン産生阻害剤)を、あるいはERシグナル阻害剤と殺細胞性抗がん剤および/または免疫チェックポイント阻害剤との組み合わせを有効成分として含有し、IL-34を発現する腫瘍の治療および/または予防のための医薬として使用することができる。ここで「有効成分として含有する」とは、有効量のERシグナル阻害剤またはこれを含む組み合わせを含有することを意味する。
本発明に係る医薬は、上記のERシグナル阻害剤(例えば、ER阻害剤またはエストロゲン産生阻害剤)を、あるいはERシグナル阻害剤と殺細胞性抗がん剤および/または免疫チェックポイント阻害剤との組み合わせを有効成分として含有し、IL-34を発現する腫瘍の治療および/または予防のための医薬として使用することができる。ここで「有効成分として含有する」とは、有効量のERシグナル阻害剤またはこれを含む組み合わせを含有することを意味する。
本発明において、ERシグナル阻害剤の有効量は、殺細胞性抗がん剤または免疫チェックポイント阻害剤と組み合わされたときに、殺細胞性抗がん剤または免疫チェックポイント阻害剤が単独で用いられた場合に発揮する腫瘍の治療および/または予防の効果を上回る強さの効果を発揮する量を意味する。ERシグナル阻害剤の有効量は、ERシグナル阻害剤および組み合わされる殺細胞性抗がん剤または免疫チェックポイント阻害剤の種類、用法、対象者の年齢、性別、体重、腫瘍の種類などに応じて適宜決定される。
本発明において、殺細胞性抗がん剤の量は、ERシグナル阻害剤と組み合わされたときに腫瘍を治療および/または予防することができる量であればよい。通常は、殺細胞性抗がん剤の量は、単独で用いられる場合の量と同等であるか、またはこれより少ない。単独で用いられる場合の量と同等の量の殺細胞性抗がん剤を用いることで、腫瘍に対するより強力な効果が発揮され、より短期間に、または単独で用いても効果が確認できなかった対象者において、腫瘍を治療および/または予防することができる。また、単独で用いられる場合の量よりも少ない量の殺細胞性抗がん剤を用いることは、腫瘍に対する効果を維持しながら、殺細胞性抗がん剤の量を減らすことができるという利点を有する。
本発明において、殺細胞性抗がん剤は、1種類を単独で使用してもよく、2種以上を組み合わせて使用してもよい。「組み合わせて用いる」とは、腫瘍の治療および/または予防が望まれる対象者に対して、2種以上の殺細胞性抗がん剤を一緒にまたは別々に、同時にまたは逐次的に投与することを意味する。
本発明において、免疫チェックポイント阻害剤の量は、ERシグナル阻害剤と組み合わされたときに腫瘍を治療および/または予防することができる量であればよい。通常は、免疫チェックポイント阻害剤の量は、単独で用いられる場合の量と同等であるか、またはこれより少ない。単独で用いられる場合の量と同等の量の免疫チェックポイント阻害剤を用いることで、腫瘍に対するより強力な効果が発揮され、より短期間に、または単独で用いても効果が確認できなかった対象者において、腫瘍を治療および/または予防することができる。また、単独で用いられる場合の量よりも少ない量の免疫チェックポイント阻害剤を用いることは、腫瘍に対する効果を維持しながら、免疫チェックポイント阻害剤の量を減らすことができるという利点を有する。
本発明において、免疫チェックポイント阻害剤は、1種類を単独で使用してもよく、2種以上を組み合わせて使用してもよい。「組み合わせて用いる」とは、腫瘍の治療および/または予防が望まれる対象者に対して、2種以上の免疫チェックポイント阻害剤を一緒にまたは別々に、同時にまたは逐次的に投与することを意味する。
本明細書において用いられる用語「治療」は、疾患の治癒、一時的寛解などを目的とする医学的に許容される全てのタイプの治療的介入を包含する。また、用語「予防」は、疾患の罹患または発症の防止もしくは抑制を目的とする医学的に許容される全てのタイプの予防的介入を包含する。すなわち、IL-34を発現する腫瘍の治療および/または予防とは、IL-34を発現する腫瘍の進行の遅延または停止、病変の退縮または消失、発症の予防または再発の防止などを含む、種々の目的の医学的に許容される介入を包含する。
本発明に係る医薬は、IL-34を発現する腫瘍に罹患した、または罹患するおそれのある対象、例えばマウス、ラット、ハムスターおよびモルモットを含むげっ歯類、ヒト、チンパンジーおよびアカゲザルを含む霊長類、ブタ、ウシ、ヤギ、ウマおよびヒツジを含む家畜、イヌおよびネコを含む愛玩動物といった哺乳動物に投与される。好ましい対象は、ヒトである。
本発明に係る医薬により治療および/または予防することができる腫瘍は、IL-34を発現する腫瘍であれば特に限定されない。ここでIL-34を発現する腫瘍とは、IL-34が実質的に発現している腫瘍であり、例えばRT-PCRや免疫学的検出法などの分子生物学的手法によってIL-34の発現を検出することができる腫瘍である。IL-34を発現する腫瘍の例には、トリプルネガティブ乳がんなどの乳がん、大腸がん、肺がん、胃がん、食道がん、腎がん、膀胱がん、卵巣がん、子宮頸がん、子宮体がん、頭頚部がん、悪性黒色腫などが含まれる。本発明に係る医薬はERシグナル阻害剤を含むが、IL-34を発現する腫瘍(腫瘍細胞)は、エストロゲン受容体(ER)陰性であってもよい。その理由は、ERシグナル阻害剤は、腫瘍細胞ではなく他の細胞(例えばMDSC)に作用しているからと考えられるが、これに限定されない。ここでER陰性の腫瘍とは、ERが実質的に発現していない腫瘍であり、例えばRT-PCRや免疫学的検出法などの分子生物学的手法によってERの発現を検出することができないか、またはその発現量が僅かである腫瘍である。たとえば、トリプルネガティブ乳がんは、ER陰性である。また、本発明に係る医薬により治療および/または予防することができる腫瘍は、任意のステージのがんでありうる。
本発明に係る医薬は、上記の有効成分に加えて、有効成分以外の薬物または緩衝剤、抗酸化剤、保存剤、タンパク質、親水性ポリマー、アミノ酸、キレート化剤、非イオン性界面活性剤、賦形剤、安定化剤、担体などの薬学的に許容される成分と医薬組成物を形成しまたは製剤化して、使用することができる。薬学的に許容される成分は、当業者において周知であり、当業者が通常の実施能力の範囲内で、例えば第十八改正日本薬局方その他の規格書に記載された成分から製剤の形態に応じて適宜選択されうる。
本発明に係る医薬またはこれを含む医薬組成物の剤形は、特に限定されず、標的部位や腫瘍の種類などに応じて適宜選択されうる。剤形は、一般に非経口製剤であることが好ましく、例えば注射剤、経皮剤、経腸剤、点滴剤等であることができる。また、本発明に係る医薬の投与経路も、特に限定されない。非経口製剤である場合は、例えば血管内投与(好ましくは静脈内投与)、腹腔内投与、腸管内投与、皮下投与、標的部位への局所投与等を行うことができる。好ましい実施形態の一つにおいて、本発明に係る医薬は、静脈内投与または局所投与により生体に投与される。また、本発明に係る医薬またはこれを含む医薬組成物の投与量は、用法、対象者の年齢、性別、体重、腫瘍の種類などに応じて適宜選択される。
前述のとおり、本発明の第1の態様および第4の態様の医薬またはこれを含む医薬組成物は、ERシグナル阻害剤と殺細胞性抗がん剤および/または免疫チェックポイント阻害剤との組み合わせを有効成分として含有する。第1の態様および第4の態様の医薬は、ERシグナル阻害剤と殺細胞性抗がん剤および/または免疫チェックポイント阻害剤とを一緒にまたは別々に、同時にまたは逐次的に、対象者に投与することを目的として調製される医薬であればよく、それぞれ独立に製剤化されたERシグナル阻害剤と殺細胞性抗がん剤および/または免疫チェックポイント阻害剤とを含む併用剤、あるいは製剤化可能である限りERシグナル阻害剤と殺細胞性抗がん剤および/または免疫チェックポイント阻害剤とを含む合剤であってもよい。
また、本発明の第2の態様の医薬またはこれを含む医薬組成物は、殺細胞性抗がん剤または免疫チェックポイント阻害剤と組み合わせて用いるためのものである。第2の態様の医薬は、殺細胞性抗がん剤および/または免疫チェックポイント阻害剤と組み合わせて用いることが意図される限り、実際に組み合わせられる前の状態の医薬であってよく、すなわち組み合わせ医薬の製造に用いられる医薬、組み合わせて用いるために製造または販売されている医薬などであってもよい。また、ここで「殺細胞性抗がん剤または免疫チェックポイント阻害剤と組み合わせて用いる」とは、腫瘍の治療および/または予防が望まれる対象者に対して、殺細胞性抗がん剤または免疫チェックポイント阻害剤と一緒にまたは別々に、同時にまたは逐次的に投与することを意味する。
(腫瘍の治療方法)
本発明は、腫瘍がIL-34を発現しているか否かを調べる工程と、前記腫瘍がIL-34を発現している場合に、ERシグナル阻害剤(例えば、ER阻害剤またはエストロゲン産生阻害剤)と、殺細胞性抗がん剤または免疫チェックポイント阻害剤とを患者に投与する工程と、を有する、IL-34を発現する腫瘍の治療方法を別の態様として提供する。
本発明は、腫瘍がIL-34を発現しているか否かを調べる工程と、前記腫瘍がIL-34を発現している場合に、ERシグナル阻害剤(例えば、ER阻害剤またはエストロゲン産生阻害剤)と、殺細胞性抗がん剤または免疫チェックポイント阻害剤とを患者に投与する工程と、を有する、IL-34を発現する腫瘍の治療方法を別の態様として提供する。
腫瘍がIL-34を発現しているか否かを調べる方法は、特に限定されない。前述のとおり、腫瘍がIL-34を発現しているか否かは、例えばRT-PCRや免疫学的検出法などの分子生物学的手法によって検出することができる。
また、ERシグナル阻害剤と、殺細胞性抗がん剤または免疫チェックポイント阻害剤との投与経路も、特に限定されない。前述のとおり、非経口製剤である場合は、例えば血管内投与(好ましくは静脈内投与)、腹腔内投与、腸管内投与、皮下投与、標的部位への局所投与等を行うことができる。
(IL-34を発現する腫瘍におけるERシグナル阻害剤の効果)
以下、IL-34を発現する腫瘍におけるERシグナル阻害剤の推察される効果について説明するが、本発明に係る医薬の効果はこれに限定されない。

図1は、IL-34を発現する腫瘍(例えばトリプルネガティブ乳がん)において抗腫瘍免疫が抑制され、かつ抗腫瘍剤の効果が低減するメカニズムを説明するための模式図である。
図1に示されるように、腫瘍細胞により産生されたIL-34は、骨髄由来前駆細胞に作用し、骨髄由来抑制細胞(MDSC)のバランスを変化させる。より具体的には、IL-34は、単球系骨髄由来抑制細胞(M-MDSC)を増加させ、多形核細胞系骨髄由来抑制細胞(PMN-MDSC)を減少させる(図5A~図6参照)。M-MDSCは、制御性T細胞(regulatory T cell;Treg)を増加させ(図7A~図8B参照)、その結果として腫瘍内を免疫抑制状態とする。
また、IL-34は、MDSCからの血管内皮増殖因子(VEGF)の産生を抑制する。その結果として、腫瘍内の血管新生が抑制され(図9Aおよび図9B参照)、殺細胞性抗がん剤や免疫チェックポイント阻害剤などが腫瘍内に到達しにくくなる。
以上のように、IL-34は、MDSCを介して免疫抑制状態となるように腫瘍微小環境を変化させると考えられる。
図2は、IL-34を発現する腫瘍(例えばトリプルネガティブ乳がん)において、ERシグナル阻害剤が上記のIL-34の作用を解除するメカニズムを説明するための模式図である。
図2に示されるように、ERシグナル阻害剤(例えば、ER阻害剤またはエストロゲン産生阻害剤)は、IL-34によるMDSCへの作用を阻害することで、上記のIL-34による腫瘍微小環境の変化を抑制する。すなわち、ERシグナル阻害剤は、IL-34によるMDSCのバランスの変化を阻害し、M-MDSCを減少させ、PMN-MDSCを増加させる(図10A、図10Bおよび図12B参照)。その結果、Tregを減少させ、腫瘍内の免疫抑制状態を解除する(図15参照)。また、ERシグナル阻害剤は、腫瘍内の血管新生の抑制も解除しうる。
以上のように、ERシグナル阻害剤は、IL-34によるMDSCへの作用を阻害することで、上記のIL-34による腫瘍微小環境の変化を抑制すると考えられる。別の言い方をすれば、ERシグナル阻害剤は、IL-34による腫瘍微小環境の免疫抑制的な変化を改善する医薬となりうる。そして、ERシグナル阻害剤を使用することで、IL-34を発現する腫瘍における抗腫瘍剤(殺細胞性抗がん剤または免疫チェックポイント阻害剤)の抗腫瘍効果が高まる(図12A、図18A~図21B、図23Aおよび図23B参照)。
上記のERシグナル阻害剤と類似の効果は、IL-34に対する阻害抗体によっても得られると考えられる。しかしながら、ERシグナル阻害剤(例えば、ER阻害剤またはエストロゲン産生阻害剤)は既に臨床で使用されているのに対し、ヒトIL-34に対する阻害抗体は樹立されていない現在、ヒトにおいてIL-34の作用を阻害する代替法としてERシグナル阻害剤を用いることは有用である。
以下の実施例によって、本発明をさらに詳細に説明するが、本発明はこれらの実施例により限定されない。
[実施例1]
実施例1では、トリプルネガティブ乳がん(TNBC)に対するERシグナル阻害剤(ER阻害剤)としてのメチルピペリジノピラゾール(MPP)の結果を示す。TNBCは、ER陰性である。

1.TNBCにおけるIL-34の発現
図3Aは、各タイプのヒト乳がんにおけるIL-34の発現量の統計解析の結果を示すグラフである。上から、トリプルネガティブ乳がん(TNBC)、HER2陽性乳がん(HER2)、ルミナルA乳がん(Luminal A)、ルミナルB乳がん(Luminal B)の結果を示している。図3Bは、TNBC患者の中でIL-34の発現量が多い(上位50%)群と少ない(下位50%)群の全生存率のカプランマイヤープロットである。表1は、TNBC患者の10年生存率に関するコックス比例ハザード分析の結果を示す。これらの図および表は、本発明者らが発表した非特許文献4に記載されているものであり、詳細は非特許文献4を参照されたい。

図3Aに示されるように、IL-34は、乳がんの中でもTNBCにおいて特徴的に高発現していた。また、図3Bおよび表1に示されるように、TNBCにおけるIL-34の発現は、予後不良と有意に相関していた。
2.IL-34を発現する腫瘍におけるMDSCの変化
マウスのTNBC細胞株4T1を準備した。また、4T1細胞株からIL-34遺伝子のノックアウト細胞株を作製した(非特許文献4参照)。

図4Aは、4T1細胞株(Ctrl 4T1)およびIL-34ノックアウト4T1細胞株(Il34KO 4T1)の写真である。図4Bは、2つの細胞株の培養液中のIL-34の濃度を示すグラフである。このグラフに示されるように、4T1細胞株ではIL-34が発現していたのに対し、ノックアウト細胞株ではIL-34が発現していなかった。このグラフも、非特許文献4に記載されているものであり、詳細は非特許文献4を参照されたい。また、いずれの細胞株においても、エストロゲン受容体(ER)は発現していなかった。

3.IL-34を含む培養系におけるMDSCの変化
メスのBALB/cマウスの大腿骨から骨髄細胞を分離し、赤血球溶解バッファーを用いて赤血球を除去した。1×106個の骨髄細胞を、10%FBS、100U/mLペニシリン、100μg/mLストレプトマイシン、50ng/mL組換えGM-CSF(BioLegend)、50ng/mL組換えIL-34(BioLegend)を添加した2mLのRPMI-1640培地中で、37℃、5%CO2の環境下において培養した。比較のため、組換えIL-34を添加しない点以外は同じ条件で、1×106個の骨髄細胞を培養した。培養開始から3日目に細胞を回収し、上記と同じ手順でフローサイトメトリーで解析した。

6.ER阻害剤(MPP)投与によるMDSCの変化
1×105個の4T1細胞またはIL-34ノックアウト4T1細胞をメスのBALB/cマウスの右脇腹に皮下接種した。これらの細胞に由来する腫瘍の大きさをノギスで週3回測定し、腫瘍の大きさが5mmに達したときにER阻害剤であるメチルピペリジノピラゾール(MPP)(1.0mg/kg)を腹腔内投与した。細胞を接種してから14日後に、これらの細胞に由来する腫瘍をマウスから回収した。細胞単離試薬を用いて腫瘍から細胞を単離した。回収した細胞のCD11b、CD45、Ly6CおよびLy6Gを蛍光色素で標識された抗体を用いて染色した。染色後の細胞についてフローサイトメトリーで解析した。

図12Aに示されるように、MPPを投与することで、IL-34を発現する腫瘍内においてもPTXの治療効果が復活した。また、図12Bに示されるように、MPPを投与することで、IL-34を発現する腫瘍内においてM-MDSCが減少し、PMN-MDSCが増加した。また、ここでは結果を示さないが、MPPを投与したマウスの4T1細胞に由来する腫瘍では、腫瘍内部においても血管が新生されているのが確認された。これらのことから、MPPを投与することで、IL-34を発現する腫瘍に対する抗がん剤の効果を復活できることが示唆される。
[実施例2]
実施例2では、トリプルネガティブ乳がん(TNBC)に対するERシグナル阻害剤(ER阻害剤)としてのフルベストラント(FUL)の結果を示す。前述のとおり、TNBCは、ER陰性である。

1.培養系における腫瘍細胞に対するER阻害剤(FUL)の効果
5×103個の4T1細胞またはIL-34ノックアウト4T1細胞を、フルベストラント(FUL)を含むまたは含まない、10%FBS、100U/mLペニシリン、100μg/mLストレプトマイシンを添加した200μLのRPMI-1640培地に播種し、37℃、5%CO2の環境下において3日間培養して、MTT試験を行った。培地には最終濃度が0.1μM、1μMまたは10μMとなるようにFULを、あるいはコントロールとしてDMSOを添加した。
5×103個の4T1細胞またはIL-34ノックアウト4T1細胞を、フルベストラント(FUL)を含むまたは含まない、10%FBS、100U/mLペニシリン、100μg/mLストレプトマイシンを添加した200μLのRPMI-1640培地に播種し、37℃、5%CO2の環境下において3日間培養して、MTT試験を行った。培地には最終濃度が0.1μM、1μMまたは10μMとなるようにFULを、あるいはコントロールとしてDMSOを添加した。
図13Aは、4T1細胞に対するMTT試験の結果を示すグラフである。図13Bは、ノックアウト細胞に対するMTT試験の結果を示すグラフである。N.S.はp>0.05を示している。
図13Aおよび図13Bに示されるように、FULは腫瘍細胞そのものには有意な効果を示さなかった。このことから、ER阻害剤は、腫瘍細胞そのものではなく、腫瘍微小環境を構成する他の細胞に対して作用することが示唆される。
2.ER阻害剤(FUL)投与による腫瘍の変化
1×105個の4T1細胞またはIL-34ノックアウト4T1細胞をメスのBALB/cマウスの右脇腹に皮下接種した。細胞を接種してから5日目から3日に1回、ER阻害剤であるフルベストラント(FUL)(1.25mg)を皮下に投与した。また、腫瘍の大きさをノギスで週3回測定した。細胞を接種してから14日後に、これらの細胞に由来する腫瘍をマウスから回収した。細胞単離試薬を用いて腫瘍から細胞を単離した。回収した細胞のCD3、CD8、CD11b、CD45、CD69、CD206、Ly6CおよびLy6Gを蛍光色素で標識された抗体を用いて染色した。染色後の細胞についてフローサイトメトリーで解析した。

4.免疫チェックポイント阻害剤とER阻害剤(FUL)の併用投与による腫瘍の大きさの変化
1×105個の4T1細胞をメスのBALB/cマウスの右脇腹に皮下接種した。細胞を接種してから5日目から、免疫チェックポイント阻害剤である抗CTLA-4抗体(αCTLA-4)(250μg/3日)のみ、免疫チェックポイント阻害剤であるαCTLA-4(250μg/3日)と抗PD-1抗体(αPD-1)(250μg/3日)との組み合わせ、αCTLA-4(250μg/3日)とER阻害剤であるフルベストラント(FUL)(1.25mg/3日)の組み合わせ、またはαCTLA-4(250μg/3日)とαPD-1(250μg/3日)とFUL(1.25mg/3日)の組み合わせを投与した。また、腫瘍の大きさをノギスで週3回測定した。細胞を接種してから14日後に、これらの細胞に由来する腫瘍をマウスから回収した。

図20Aおよび図20Bに示されるように、αPD-1およびαCTLA-4のみを投与した場合に比べて、さらにFULを併用投与することで腫瘍の増大が顕著に抑制された。
これらのことから、FULを投与することで、IL-34を発現している腫瘍に対する免疫チェックポイント阻害剤の効果を復活または促進できることが示唆される。
5.抗がん剤と免疫チェックポイント阻害剤とER阻害剤(FUL)の併用投与による腫瘍の大きさの変化
1×105個の4T1細胞をメスのBALB/cマウスの右脇腹に皮下接種した。細胞を接種してから5日目から、抗がん剤であるパクリタキセル(PTX)(1.3mg/kg/日)とER阻害剤であるフルベストラント(FUL)(1.25mg/3日)の組み合わせ、免疫チェックポイント阻害剤である抗PD-1抗体(αPD-1)(250μg/3日)および抗CTLA-4抗体(αCTLA-4)(250μg/3日)とPTX(1.3mg/kg/日)との組み合わせ、またはαPD-1(250μg/3日)およびαCTLA-4(250μg/3日)とPTX(1.3mg/kg/日)とFUL(1.25mg/3日)との組み合わせ、を投与した。また、腫瘍の大きさをノギスで週3回測定した。細胞を接種してから14日後に、これらの細胞に由来する腫瘍をマウスから回収した。

[実施例3]
実施例3では、大腸がんに対するERシグナル阻害剤(ER阻害剤)としてのフルベストラント(FUL)の結果を示す。

1.培養系における腫瘍細胞に対するER阻害剤(FUL)の効果
マウスの大腸がんCT26細胞株を準備した。CT26細胞株ではIL-34が発現しているが、エストロゲン受容体(ER)は発現していない。

5×103個のCT26細胞を、フルベストラント(FUL)を含むまたは含まない、10%FBS、100U/mLペニシリン、100μg/mLストレプトマイシンを添加した200μLのRPMI-1640培地に播種し、37℃、5%CO2の環境下において3日間培養して、MTT試験を行った。培地には最終濃度が0.1μM、1μMまたは10μMとなるようにFULを、あるいはコントロールとしてDMSOを添加した。
図22は、CT26細胞に対するMTT試験の結果を示すグラフである。N.S.はp>0.05を示している。
図22に示されるように、FULは腫瘍細胞そのものには有意な効果を示さなかった。このことから、ER阻害剤は、腫瘍細胞そのものではなく、腫瘍微小環境を構成する他の細胞に対して作用することが示唆される。
2.免疫チェックポイント阻害剤とER阻害剤(FUL)の併用投与による腫瘍の大きさの変化
1×105個のCT26細胞をメスのBALB/cマウスの右脇腹に皮下接種した。細胞を接種してから5日目から、免疫チェックポイント阻害剤である抗CTLA-4抗体(αCTLA-4)(250μg/3日)のみ、またはαCTLA-4(250μg/3日)とER阻害剤であるフルベストラント(FUL)(1.25mg/3日)の組み合わせを投与した。また、腫瘍の大きさをノギスで週3回測定した。細胞を接種してから14日後に、これらの細胞に由来する腫瘍をマウスから回収した。

図23Aおよび図23Bに示されるように、IL-34を発現している腫瘍では、αCTLA-4の単独投与に有意な効果が見られなかった。一方、FULを投与することで、αCTLA-4の治療効果が復活した。これらのことから、FULを投与することで、IL-34を発現している腫瘍に対する免疫チェックポイント阻害剤の効果を復活または促進できることが示唆される。

図24Aおよび図24Bに示されるように、IL-34を過剰発現させた腫瘍では、αPD-1による抗腫瘍効果が見られなかった。一方、IL-34を欠損させた腫瘍では、αPD-1よる抗腫瘍効果が見られた。これらのことから、CT26細胞に由来する腫瘍においても、IL-34の発現により抗腫瘍免疫が抑制されていることが示唆される。
[実施例4]
実施例4では、トリプルネガティブ乳がん(TNBC)に対する、ERシグナル阻害剤(ER阻害剤)としてのフルベストラント(FUL)、タモキシフェン(TAM)およびトレミフェン(TOR)の結果、ならびにERシグナル阻害剤(エストロゲン産生阻害剤)としてのアナストロゾール(ANZ)の結果を示す。
実施例4では、トリプルネガティブ乳がん(TNBC)に対する、ERシグナル阻害剤(ER阻害剤)としてのフルベストラント(FUL)、タモキシフェン(TAM)およびトレミフェン(TOR)の結果、ならびにERシグナル阻害剤(エストロゲン産生阻害剤)としてのアナストロゾール(ANZ)の結果を示す。
1.免疫チェックポイント阻害剤と、ER阻害剤(FUL、TAM、TOR)またはエストロゲン産生阻害剤(TAM)との併用投与による腫瘍の大きさの変化
1×105個の4T1細胞をメスのBALB/cマウスの乳腺に皮下接種した。細胞を接種してから5日目から、免疫チェックポイント阻害剤、ER阻害剤(FUL、TAM、TOR)および/またはエストロゲン産生阻害剤(TAM)を投与した。また、腫瘍の大きさをノギスで週3回測定した。

フルベストラント(FUL)については、細胞を接種してから5日目から、免疫チェックポイント阻害剤である抗CTLA-4抗体(αCTLA-4)(250μg/3日)のみ、ER阻害剤であるフルベストラント(FUL)(1.25mg/3日)のみ、またはαCTLA-4(250μg/3日)とFUL(1.25mg/3日)の組み合わせを投与した。
タモキシフェン(TAM)については、細胞を接種してから5日目から、ER阻害剤であるタモキシフェン(TAM)(100μg/1日)のみ、またはαCTLA-4(250μg/3日)とTAM(100μg/1日)の組み合わせを投与した。
トレミフェン(TOR)については、細胞を接種してから5日目から、ER阻害剤であるトレミフェン(TOR)(400μg/1日)のみ、またはαCTLA-4(250μg/3日)とTOR(400μg/1日)の組み合わせを投与した。
アナストロゾール(ANZ)については、細胞を接種してから5日目から、エストロゲン産生阻害剤であるアナストロゾール(ANZ)(50μg/1日)のみ、またはαCTLA-4(250μg/3日)とANZ(50μg/1日)の組み合わせを投与した。

図25Aおよび図25Bに示されるように、IL-34を発現している腫瘍では、αCTLA-4の単独投与に有意な効果が見られなかった。一方、図25A~図28Bに示されるように、FUL、TAM、TORまたはANZを投与することで、αCTLA-4の治療効果が復活した。
これらのことから、FUL、TAM、TORまたはANZを投与することで、IL-34を発現している腫瘍に対する免疫チェックポイント阻害剤の効果を復活または促進できることが示唆される。
本出願は、2022年4月25日出願の特願2022-71785に基づく優先権を主張する。当該出願明細書および図面に記載された内容は、すべて本願明細書に援用される。
本発明に係る医薬は、IL-34を発現する腫瘍の治療または予防に有用である。
Claims (19)
- エストロゲン受容体シグナル阻害剤と、殺細胞性抗がん剤または免疫チェックポイント阻害剤と、を有効成分として含有する、IL-34を発現する腫瘍を治療および/または予防するための医薬。
- 前記エストロゲン受容体シグナル阻害剤および前記殺細胞性抗がん剤を有効成分として含有する、請求項1に記載の医薬。
- 前記エストロゲン受容体シグナル阻害剤および前記免疫チェックポイント阻害剤を有効成分として含有する、請求項1に記載の医薬。
- 前記エストロゲン受容体シグナル阻害剤、前記殺細胞性抗がん剤および前記免疫チェックポイント阻害剤を有効成分として含有する、請求項1に記載の医薬。
- 前記腫瘍は、エストロゲン受容体陰性の腫瘍である、請求項1に記載の医薬。
- 前記腫瘍は、トリプルネガティブ乳がんである、請求項5に記載の医薬。
- 前記エストロゲン受容体シグナル阻害剤は、エストロゲン受容体阻害剤である、請求項1~6のいずれか一項に記載の医薬。
- 前記エストロゲン受容体阻害剤は、メチルピペリジノピラゾール、フルベストラント、タモキシフェンおよびトレミフェンからなる群から選択される少なくとも1つである、請求項7に記載の医薬。
- 前記エストロゲン受容体シグナル阻害剤は、エストロゲン産生阻害剤である、請求項1~6のいずれか一項に記載の医薬。
- 前記エストロゲン産生阻害剤は、アナストロゾールである、請求項9に記載の医薬。
- 前記殺細胞性抗がん剤は、パクリタキセルである、請求項1に記載の医薬。
- 前記免疫チェックポイント阻害剤は、抗CTLA-4抗体および抗PD-1抗体からなる群から選択される少なくとも1つである、請求項1に記載の医薬。
- 免疫チェックポイント阻害剤または殺細胞性抗がん剤と併用投与されるための、エストロゲン受容体シグナル阻害剤を有効成分として含有する、IL-34を発現している腫瘍を治療および/または予防するための医薬。
- 前記腫瘍は、エストロゲン受容体陰性の腫瘍である、請求項13に記載の医薬。
- 前記腫瘍は、トリプルネガティブ乳がんである、請求項14に記載の医薬。
- 前記エストロゲン受容体シグナル阻害剤は、エストロゲン受容体阻害剤である、請求項13~15のいずれか一項に記載の医薬。
