コロナウイルス感染症の新たな予防又は治療薬を提供する。 本発明は、エストロゲン受容体と複合体を形成し得る物質を含む、コロナウイルスの感染を予防又は治療するための医薬組成物に関する。

本発明は、コロナウイルス感染症を予防又は治療するための医薬組成物に関する。
ヒトに感染するコロナウイルスとしては、ヒトに日常的に感染する4種類のコロナウイルス(Human Coronavirus:HCoV)(HCoV-229E、HCoV-OC43、HCoV-NL63及びHCoV-HKU1)、重症急性呼吸器症候群コロナウイルス(Severe acute respiratory syndrome:SARS-CoV)、中東呼吸器症候群コロナウイルス(Middle East respiratory syndrome:MERS-CoV)などが知られている(非特許文献1)。
新型コロナウイルス感染症(COVID-19)は、2019年12月、中華人民共和国湖北省武漢市において確認された、新型コロナウイルスであるSARS-CoV-2がヒト細胞内に侵入することにより発症する感染症である。2021年10月現在、感染者数(死亡者数)は世界で約2.19億人(約455万人)を超え、200カ国以上に広がっている。主な症状は、発熱、咳、咳以外の急性呼吸器症状・肺炎で、重症化すると、過剰な免疫反応に基づく重篤な臓器障害「サイトカインストーム症候群」や重度の呼吸不全「急性呼吸窮迫症候群(ARDS)」が引き起こされ死に至ることがある。
SARS-CoV-2は新興ウイルスであるため、国内外とも承認薬が存在しなかったが、世界的にCOVID-19が急速に拡大しパンデミック化したことを受けて、世界中で精力的に治療薬又は予防薬の開発が進められた。
COVID-19予防薬としては、ワクチンの開発が進んでおり、例えば、不活化ワクチン、組換えタンパク質ワクチン、ペプチドワクチン、メッセンジャーRNA(mRNA)ワクチン、DNAワクチン、ウイルスベクターワクチンなど様々な種類が挙げられるが、特にmRNAワクチンの接種が世界的に進められている。しかし、ワクチンの効果は、抗原として選択した抗原部位の存在に依存するため、ワクチンの抗原部位によっては変異体に対する効果が低くなるなどの問題がある。
COVID-19治療薬としては、モルヌピラビルなどの新薬の他、可及的速やかに上市させる目的で既承認薬である抗ウイルス薬、例えばレムデシビル、ネルフィナビル、ファビピラビル等を転用する治療候補薬の開発が進められ、薬事承認されるに至った薬もある。日本では2021年10月現在、レムデシビル、バリシチニブ、カシリビマブ、イムデビマブ、ソトロビマブがCOVID-19治療薬として承認されている(非特許文献2)が、実際に投与される例は少ない状況にある。
また、COVID-19の種々の症状に対する治療薬候補としては、ステロイドであるデキサメタゾン、シクレソニド、プロテアーゼ阻害薬であるナファモスタット、カモスタット、駆虫薬であるイベルメクチン、ヤヌスキナーゼ(JAK)阻害剤であるバリシチニブなどが期待されている。

しかしながら、いずれの予防薬及び/又は治療薬も未だコロナウイルス感染症、特にCOVID-19に対して十分な効果があることが確認されるに至っておらず、新たな予防薬及び/又は治療薬の開発が希求されている。
従って、本発明の課題は、コロナウイルス感染症、特にCOVID-19の新たな予防及び/又は治療薬を提供することにある。

そこで本発明者らは、既存の承認薬の中から新たなコロナウイルス、特にSARS-CoV-2に対する予防及び/又は治療薬を見出すべく種々検討した結果、エストロゲン受容体と複合体を形成し得る物質が、抗コロナウイルス活性作用、及びコロナウイルス感染による死亡に対する生存率の回復作用を有し、コロナウイルス感染症、特にCOVID-19の予防及び/又は治療薬として有用であることを見出し、本発明を完成した。

本発明の医薬組成物は、抗コロナウイルス活性作用、特に抗SARS-CoV-2活性作用;及びコロナウイルス感染、特にSARS-CoV-2感染による死亡に対する生存率の回復作用を有し、コロナウイルス感染症、特にCOVID-19の予防及び/又は治療薬として有用である。
一実施態様において、本発明は、エストロゲン受容体と複合体を形成し得る物質を含む、コロナウイルス感染症を予防又は治療するための医薬組成物である。
本発明の「エストロゲン受容体と複合体を形成し得る物質」は、エストロゲン受容体と複合体を形成し得る物質であれば任意の物質であってよく、例えば、低分子化合物、ペプチド、アミノ酸、タンパク質、核酸、脂質、糖などが含まれる。
なお、「エストロゲン受容体と複合体を形成し得る」とは、エストロゲン受容体と一時的に結合して一時的に複合体を形成し得ること、及びエストロゲン受容体と持続的に結合して持続的に複合体を形成し得ることの両方をいうことができる。
本明細書において、低分子化合物とは、分子量が1000以下の化合物をいう。本明細書において、ペプチドとは、2~49個のアミノ酸がペプチド結合した分子をいい、タンパク質とは、50個以上のアミノ酸がペプチド結合した分子をいい、それぞれ修飾アミノ酸を含む場合も含まれる。本明細書において、核酸とは、一本鎖又は二本鎖のDNA又はRNAをいい、二本鎖にはホモ二本鎖もヘテロ二本鎖も含み、直鎖状も環状も含み、修飾塩基を含む核酸も含む。本明細書において、脂質とは、生物から単離され得る無極性溶媒に可溶な物質をいい、例えば、脂肪酸、ワックス、ステロール、脂溶性ビタミン、アシルグリセロール、リン脂質などが含まれる。本明細書において、糖とは、ケトン基又はアルデヒド基を含み、かつ2以上のヒドロキシル基を有する化合物をいう。
好ましい実施態様において、エストロゲン受容体と複合体を形成し得る物質は、低分子化合物である。
本発明の「エストロゲン受容体と複合体を形成し得る物質」は、エストロゲン受容体と複合体を形成し得る物質であれば任意の物質であってよく、例えば、低分子化合物、ペプチド、アミノ酸、タンパク質、核酸、脂質、糖などが含まれる。

好ましい実施態様において、エストロゲン受容体と複合体を形成し得る物質は、(i)後掲の一般式(1)で表される化合物、又は薬学的に許容されるその塩、溶媒和物、水和物、若しくはプロドラッグ;(ii)後掲の一般式(2)で表される化合物、又は薬学的に許容されるその塩、溶媒和物、水和物、若しくはプロドラッグ;又は(iii)前記(i)及び(ii)の両方を含む。
「エストロゲン受容体と複合体を形成し得る物質」の好ましい態様としては:ステロイド骨格を有する女性ホルモン、例えば、エストロン、エストラジオール、エストリオールなど;選択的エストロゲン受容体モジュレーター(SERM)、例えば、バゼドキシフェン、ラロキシフェンなど;イソフラボン、例えば、ゲニステインなど;タモキシフェン;及び、これらの誘導体、例えば、エストラムスチン、エストラムスチンリン酸エステル、エストラムスチンリン酸エステルナトリウム水和物、エストロムスチンなど;が挙げられ、当該誘導体を形成し得る基としては、例えば、ナイトロジェンマスタード基、酸基(例えば、リン酸基など)などが挙げられる。
好ましい実施態様において、エストロゲン受容体と複合体を形成し得る物質は、(i)後掲の一般式(1)で表される化合物、又は薬学的に許容されるその塩、溶媒和物、水和物、若しくはプロドラッグ;(ii)後掲の一般式(2)で表される化合物、又は薬学的に許容されるその塩、溶媒和物、水和物、若しくはプロドラッグ;又は(iii)前記(i)及び(ii)の両方を含む。
「エストロゲン受容体と複合体を形成し得る物質」の好ましい態様としては:ステロイド骨格を有する女性ホルモン、例えば、エストロン、エストラジオール、エストリオールなど;選択的エストロゲン受容体モジュレーター(SERM)、例えば、バゼドキシフェン、ラロキシフェンなど;イソフラボン、例えば、ゲニステインなど;タモキシフェン;及び、これらの誘導体、例えば、エストラムスチン、エストラムスチンリン酸エステル、エストラムスチンリン酸エステルナトリウム水和物、エストロムスチンなど;が挙げられ、当該誘導体を形成し得る基としては、例えば、ナイトロジェンマスタード基、酸基(例えば、リン酸基など)などが挙げられる。
「エストロゲン受容体と複合体を形成し得る物質」は、エストロゲン受容体と複合体を形成する限り、エストロゲン受容体のアゴニストでもアンタゴニストでもあってよく、又はエストロゲン受容体の活性とは関係無く単にエストロゲン受容体に結合し得るリガンドであってもよい。好ましい実施態様において、エストロゲン受容体と複合体を形成し得る物質は、エストロゲン受容体のアゴニストである。
また、エストロゲン受容体と複合体を形成し得る物質は、エストロゲン受容体自体の発現又は活性を制御し得る物質であってよい。好ましい実施態様において、エストロゲン受容体と複合体を形成し得る物質は、エストロゲン受容体の発現又は活性を増加させ、本発明の効果の向上に寄与する物質である。好ましい実施態様において、エストロゲン受容体と複合体を形成し得る物質は、ステロイド骨格を有する女性ホルモンとナイトロジェンマスタードとが化学結合した構造を有することにより、エストロゲン受容体の発現又は活性を増加させるシグナルを長期化させて、本発明の効果の向上に寄与する。好ましい実施態様において、エストロゲン受容体と複合体を形成し得る物質は、SERMであることにより、エストロゲン受容体の発現又は活性を増加させ、本発明の効果の向上に寄与する。好ましい実施態様において、エストロゲン受容体と複合体を形成し得る物質は、ステロイド骨格を有する女性ホルモンとナイトロジェンマスタードとが化学結合した構造を有する化合物とSERMとを併用することにより相乗的にエストロゲン受容体の発現又は活性を増加させ、本発明の効果の向上に寄与する。
また、エストロゲン受容体と複合体を形成し得る物質は、エストロゲン受容体自体の発現又は活性を制御し得る物質であってよい。好ましい実施態様において、エストロゲン受容体と複合体を形成し得る物質は、エストロゲン受容体の発現又は活性を増加させ、本発明の効果の向上に寄与する物質である。好ましい実施態様において、エストロゲン受容体と複合体を形成し得る物質は、ステロイド骨格を有する女性ホルモンとナイトロジェンマスタードとが化学結合した構造を有することにより、エストロゲン受容体の発現又は活性を増加させるシグナルを長期化させて、本発明の効果の向上に寄与する。好ましい実施態様において、エストロゲン受容体と複合体を形成し得る物質は、SERMであることにより、エストロゲン受容体の発現又は活性を増加させ、本発明の効果の向上に寄与する。好ましい実施態様において、エストロゲン受容体と複合体を形成し得る物質は、ステロイド骨格を有する女性ホルモンとナイトロジェンマスタードとが化学結合した構造を有する化合物とSERMとを併用することにより相乗的にエストロゲン受容体の発現又は活性を増加させ、本発明の効果の向上に寄与する。
「エストロゲン受容体(ERともいう)」は、典型的には、リガンド(本発明のエストロゲン受容体と複合体を形成し得る物質を含む)が結合することにより活性化され、核内に移行して、遺伝子の転写を制御する転写因子として機能する。エストロゲン受容体には、典型的には、ERα及びERβのアイソフォームが存在し、αα型若しくはββ型のホモ二量体、又はαβ型のヘテロ二量体を形成することが知られている。本発明のエストロゲン受容体は、単量体又は二量体の両方が含まれ、より具体的には、ERα単量体又はERβ単量体、並びにαα型又はββ型のホモ二量体、及びαβ型のヘテロ二量体の全ての態様が含まれる。
「エストロゲン受容体と複合体を形成し得る物質」は、エストロゲン受容体の単量体であるERα又はERβと複合体を形成し得る物質であってよく、あるいはエストロゲン受容体のαα型若しくはββ型のホモ二量体又はαβ型のヘテロ二量体と複合体を形成し得る物質であってよい。好ましい実施態様において、エストロゲン受容体と複合体を形成し得る物質は、エストロゲン受容体と結合することにより、(i)エストロゲン受容体の核内移行を促進し、(ii)エストロゲン受容体の転写活性を促進し、及び/又は(iii)エストロゲン受容体の発現又は活性を正に制御し得る。
「エストロゲン受容体と複合体を形成し得る物質」は、エストロゲン受容体の単量体であるERα又はERβと複合体を形成し得る物質であってよく、あるいはエストロゲン受容体のαα型若しくはββ型のホモ二量体又はαβ型のヘテロ二量体と複合体を形成し得る物質であってよい。好ましい実施態様において、エストロゲン受容体と複合体を形成し得る物質は、エストロゲン受容体と結合することにより、(i)エストロゲン受容体の核内移行を促進し、(ii)エストロゲン受容体の転写活性を促進し、及び/又は(iii)エストロゲン受容体の発現又は活性を正に制御し得る。
エストロゲン受容体と複合体を形成し得る物質がエストロゲン受容体と複合体を形成し得るか否かは、当該技術分野において公知の任意の方法を用いて調べることができるが、例えば、分子間相互作用解析装置(例えば、Biacore(登録商標))を用いた相互作用解析;抗エストロゲン受容体抗体を用いたプルダウンアッセイ及び結合物質に対する質量分析、FT-IR法による分析、又はGC-MS法による分析;抗エストロゲン受容体抗体及び標的分子に対する抗体を用いたサンドイッチELISAアッセイ又はウエスタンブロッティング;標的分子を結合させたカラム又はタグ付き標的分子を用いた結合アッセイ;などを用いて、当該物質がエストロゲン受容体と結合するか否かを決定することができる。当該物質がエストロゲン受容体と結合したことは、例えば、当該解析の検出限界内で検出された場合には結合する、当該解析の検出限界外にある場合に結合しない、と評価することができる。
「コロナウイルス」には、本出願時において国際ウイルス分類委員会(ICTV)によって分類されている全てのコロナウイルスが含まれ、ヒトに感染する代表的なコロナウイルスであるHCoV-229E、HCoV-OC43、HCoV-NL63及びHCoV-HKU1、SARS-CoV(SARS-CoV-1及びSARS-CoV-2を含む)、及びMERS-CoVなども含まれる。
本発明の予防及び/又は治療薬が対象とするコロナウイルスとしては、SARSコロナウイルス又はMERSコロナウイルスが好ましく、SARS-CoV-1及びSARS-CoV-2を含むSARSコロナウイルスがより好ましく、特に新型コロナウイルス(SARS-CoV-2)が更により好ましい。
「SARS-CoV-2」には、野生型に加えて、全ての変異型が含まれる。例えば、SARS-CoV-2変異型には、N501Y、E484K、L452R、E484Q、delH69V70、N501Y、E484K、K417N、K417T、N439K、D614G、A222V、Y453F、P681H、Q27stop、delY144、A570D、T716I、S982A、A1708D、A701V、L242_244L、D80A、L18F、R246I、D215Gなどを含む変異の1つ又は任意の2つ以上の組み合わせを含む変異型が含まれる。また、SARS-CoV-2変異型には、イギリス型、南アフリカ型、ブラジル型、インド型、日本型(関西型、東京型など)、由来不明などの地域に関連する名称が含まれる変異型、及びアルファ(α)、ベータ(β)、ガンマ(γ)、デルタ(δ)、イータ(η)、イオタ(ι)、カッパ(κ)、ラムダ(λ)、イプシロン(ε)、ゼータ(ζ)、シータ(θ)などのギリシャ文字を使用する変異型も含まれる。
本発明の予防及び/又は治療薬が対象とするコロナウイルスとしては、SARSコロナウイルス又はMERSコロナウイルスが好ましく、SARS-CoV-1及びSARS-CoV-2を含むSARSコロナウイルスがより好ましく、特に新型コロナウイルス(SARS-CoV-2)が更により好ましい。

「コロナウイルス感染症」とは、上記コロナウイルスにより引き起こされる感染症をいう。コロナウイルス感染症は、コロナウイルスが感染する任意の動物におけるコロナウイルス感染症であってよく、哺乳動物におけるコロナウイルス感染症が好ましく、霊長類におけるコロナウイルス感染症がより好ましく、ヒトにおけるコロナウイルス感染症が更により好ましい。
好ましい実施態様において、コロナウイルス感染症は、SARSコロナウイルス又はMERSコロナウイルスにより引き起こされる感染症であり、特にSARS-CoV-2により引き起こされる感染症であるCOVID-19が好ましい。

前記一般式(1)は、ステロイド骨格を有する女性ホルモンとナイトロジェンマスタードとを化学結合させた構造を有し、好ましくは、エストラジオール、エストロン又はそれらの誘導体とナイトロジェンマスタードとを化学結合させた構造を有する。
前記一般式(1)中のR1はヒドロキシ基又は酸残基を示すか、R2と一緒になって酸素原子を形成する。R1がR2と一緒になって酸素原子を形成する場合、R1及びR2と隣接する炭素原子とで、ケトンを形成する。

脂肪酸残基としては、アセトキシ基、プロピオニルオキシ基、ブチリルオキシ基、ピバロイルオキシ基などの炭素数1~24の脂肪酸残基、酒石酸残基、クエン酸残基、コハク酸残基などが挙げられる。有機スルホン酸残基としては、メタンスルホン酸残基、エタンスルホン酸残基、プロパンスルホン酸残基などの炭素数1~8のアルカンスルホン酸残基、トルエンスルホン酸残基などの芳香族スルホン酸残基が挙げられる。有機硫酸残基としては、アルキル硫酸残基が挙げられる。有機リン酸残基としては、アルキルリン酸残基などが挙げられる。
脂肪酸残基としては、アセトキシ基、プロピオニルオキシ基、ブチリルオキシ基、ピバロイルオキシ基などの炭素数1~24の脂肪酸残基、酒石酸残基、クエン酸残基、コハク酸残基などが挙げられる。有機スルホン酸残基としては、メタンスルホン酸残基、エタンスルホン酸残基、プロパンスルホン酸残基などの炭素数1~8のアルカンスルホン酸残基、トルエンスルホン酸残基などの芳香族スルホン酸残基が挙げられる。有機硫酸残基としては、アルキル硫酸残基が挙げられる。有機リン酸残基としては、アルキルリン酸残基などが挙げられる。
R2は、水素原子又はエチニル基を示すか、R1と一緒になって酸素原子を形成する。R2がR1と一緒になって酸素原子を形成する場合、前記のようにR1及びR2と隣接する炭素原子とで、ケトンを形成する。
R2は、水素原子を示すか、R1と一緒になって酸素原子を形成するのが好ましい。

好ましいR1としては、ヒドロキシ基、脂肪酸残基、有機スルホン酸残基、リン酸基が挙げられ、より好ましくはヒドロキシ基、脂肪酸残基又はリン酸基であり、更に好ましくはヒドロキシ基又はリン酸基である。また、R1がR2と一緒になって酸素原子を形成する場合も好ましい。
R3は水素原子、ヒドロキシ基又はアシルオキシ基を示す。
アシルオキシ基としては、アセトキシ基、プロピオニルオキシ基、ブチリルオキシ基、ピバロイルオキシ基などの炭素数1~24のアルカノイルオキシ基が挙げられ、炭素数1~8のアルカノイルオキシ基がより好ましい。
アシルオキシ基としては、アセトキシ基、プロピオニルオキシ基、ブチリルオキシ基、ピバロイルオキシ基などの炭素数1~24のアルカノイルオキシ基が挙げられ、炭素数1~8のアルカノイルオキシ基がより好ましい。
R4は水素原子又はハロゲノアルキル基を示し、R5は水素原子又はハロゲノアルキル基を示す。ここでハロゲノアルキル基としては、クロロアルキル基、ブロモアルキル基、フルオロアルキル基、ヨードアルキル基が挙げられ、クロロアルキル基がより好ましい。
ハロゲノアルキル基としては、ハロゲノC1-6アルキル基が好ましく、ハロゲノC1-4アルキル基がより好ましく、ハロゲノC2アルキル基が更により好ましい。
ハロゲノアルキル基の具体例としては、クロロメチル基、クロロエチル基、3-クロロプロピル基、2-クロロプロピル基、4-クロロブチル基などが挙げられ、クロロエチル基が更に好ましい。
また、R4及びR5がハロゲノアルキル基である場合がより好ましく、R4及びR5がハロゲノC1-4アルキル基である場合がより好ましく、R4及びR5がハロゲノエチル基である場合が更に好ましく、R4及びR5がクロロエチル基である場合が更に好ましい。
ハロゲノアルキル基としては、ハロゲノC1-6アルキル基が好ましく、ハロゲノC1-4アルキル基がより好ましく、ハロゲノC2アルキル基が更により好ましい。
ハロゲノアルキル基の具体例としては、クロロメチル基、クロロエチル基、3-クロロプロピル基、2-クロロプロピル基、4-クロロブチル基などが挙げられ、クロロエチル基が更に好ましい。
また、R4及びR5がハロゲノアルキル基である場合がより好ましく、R4及びR5がハロゲノC1-4アルキル基である場合がより好ましく、R4及びR5がハロゲノエチル基である場合が更に好ましく、R4及びR5がクロロエチル基である場合が更に好ましい。
nは0~2の整数を示し、0又は1が好ましく、0がより好ましい。
一般式(1)中のR4及びR5が、ハロゲノアルキル基である化合物が好ましく、ハロゲノエチル基である化合物がより好ましく、クロロエチル基である化合物が更により好ましい。一般式(1)中のR4及びR5がハロゲノアルキル基であり、nが0の整数を示す化合物が好ましく、一般式(1)中のR4及びR5がハロゲノエチル基であり、nが0の整数を示す化合物がより好ましく、一般式(1)中のR4及びR5がクロロエチル基であり、nが0の整数を示す化合物が更により好ましい。

一般式(1)中のR1とR2が一緒になって酸素原子を形成し、R3が水素原子であり、R4及びR5がハロゲノアルキル基であり、nが0の整数を示す化合物が好ましい。また、一般式(1)中のR1とR2が一緒になって酸素原子を形成し、R3が水素原子であり、R4及びR5がハロゲノエチル基であり、nが0の整数を示す化合物がより好ましい。更に、一般式(1)中のR1とR2が一緒になって酸素原子を形成し、R3が水素原子であり、R4及びR5がクロロエチル基であり、nが0の整数を示す化合物が好ましい。
一般式(1)中のR1が酸残基であり、R2及びR3が水素原子である化合物が好ましい。また、一般式(1)中のR1がリン酸基であり、R2及びR3が水素原子であり、R4及びR5がハロゲノアルキル基であり、nが0の整数を示す化合物が好ましい。更に、一般式(1)中のR1がリン酸基であり、R2及びR3が水素原子であり、R4及びR5がハロゲノエチル基であり、nが0の整数を示す化合物が好ましい。更に、一般式(1)中のR1がリン酸基であり、R2及びR3が水素原子であり、R4及びR5がクロロエチル基であり、nが0の整数を示す化合物がより好ましい。

一般式(1)中のR1とR2が一緒になって酸素原子を形成し、R3が水素原子であり、R4及びR5がハロゲノアルキル基であり、nが0の整数を示す化合物が好ましい。また、一般式(1)中のR1とR2が一緒になって酸素原子を形成し、R3が水素原子であり、R4及びR5がハロゲノエチル基であり、nが0の整数を示す化合物がより好ましい。更に、一般式(1)中のR1とR2が一緒になって酸素原子を形成し、R3が水素原子であり、R4及びR5がクロロエチル基であり、nが0の整数を示す化合物が好ましい。
一般式(1)中のR1が酸残基であり、R2及びR3が水素原子である化合物が好ましい。また、一般式(1)中のR1がリン酸基であり、R2及びR3が水素原子であり、R4及びR5がハロゲノアルキル基であり、nが0の整数を示す化合物が好ましい。更に、一般式(1)中のR1がリン酸基であり、R2及びR3が水素原子であり、R4及びR5がハロゲノエチル基であり、nが0の整数を示す化合物が好ましい。更に、一般式(1)中のR1がリン酸基であり、R2及びR3が水素原子であり、R4及びR5がクロロエチル基であり、nが0の整数を示す化合物がより好ましい。
一般式(1)で表される化合物の塩としては、薬学的に許容される塩であれば特に限定されないが、塩酸塩、硫酸塩、硝酸塩などの鉱酸塩、リン酸塩、炭酸塩など、酢酸塩、コハク酸塩、酒石酸塩などの有機酸塩など、ナトリウム塩、カリウム塩、カルシウム塩などの金属塩などが挙げられる。
一般式(1)で表される化合物又はその塩の溶媒和物、水和物としては、エタノール和物、水和物などが挙げられる。また、プロドラッグとしては、R1が前記酸残基の場合のほか、ステロイド骨格のいずれかの水素原子が他の置換基に置換された場合などが挙げられる。
一般式(1)で表される化合物又はその塩の好ましい具体例としては、下記式(1-1)で表されるエストラムスチンリン酸エステル又はそのナトリウム水和物、下記式(1-2)で表されるエストラムスチン、及び下記式(1-3)で表されるエストロムスチンが挙げられる。
エストラムスチンリン酸エステルナトリウム水和物は、日本においては、販売名:エストラサイト(登録商標)カプセル156.7mg(販売会社:日本新薬)として、及び米国等の外国においては、販売名:EMCYT(登録商標)(Estramustine phosphate sodium capsule) (販売会社:Pfizer Inc.)として市販されている。
これらの一般式(1)で表される化合物の特定の態様は、前立腺癌治療剤として用いられているが、コロナウイルス、特にSARS-CoV-2に対する作用については、全く知られていない。
本発明の一般式(1)の化合物の態様には、ステロイド骨格を有する女性ホルモンが含まれるが、ステロイド骨格を有する女性ホルモンは、エストロゲン受容体に結合し、当該複合体が核内に移行して特定の遺伝子(群)の転写を制御することができる。特定の理論に拘束されず、本発明との関係において、ナイトロジェンマスタードは、ステロイド骨格を有する女性ホルモンとエストロゲン受容体との結合を安定化し、本発明の一般式(1)の化合物とエストロゲン受容体とから構成される複合体が長期にわたって作用することにより、本発明の医薬組成物の抗コロナウイルス活性作用、特に抗SARS-CoV-2活性作用が発揮されると考えられる。
本発明の一般式(1)の化合物の態様には、ステロイド骨格を有する女性ホルモンが含まれることから、性差によって効果の高さが異なる可能性があり得ると考えられる。
また、一般式(1)の化合物の態様のうち少なくともエストラムスチンについては、エストラムスチン結合タンパク質(EMBP)と結合し、及び/又はマイクロチューブルの構築阻害により、本発明の医薬組成物の抗コロナウイルス活性作用、特に抗SARS-CoV-2活性作用を発揮する可能性もあり得る。
本発明の一般式(1)の化合物の態様には、ステロイド骨格を有する女性ホルモンが含まれることから、性差によって効果の高さが異なる可能性があり得ると考えられる。
また、一般式(1)の化合物の態様のうち少なくともエストラムスチンについては、エストラムスチン結合タンパク質(EMBP)と結合し、及び/又はマイクロチューブルの構築阻害により、本発明の医薬組成物の抗コロナウイルス活性作用、特に抗SARS-CoV-2活性作用を発揮する可能性もあり得る。
別の実施態様において、本発明のコロナウイルスの感染を予防又は治療するための医薬組成物の有効成分は、下記一般式(2)
(式中、Xは炭素原子、硫黄原子又は窒素原子を示し;Yは窒素原子又は炭素原子を示し;R6は水素原子又はアルキル基を示し;R7は水素原子又は酸素原子を示し;mは1~3の整数を示し;lは4~7の整数を示し;破線部分は二重結合があり得ることを示す)
で表される化合物、又は薬学的に許容されるその塩、溶媒和物、水和物、若しくはプロドラッグである。

当該一般式(2)で表される化合物、又は薬学的に許容されるその塩、溶媒和物、水和物、若しくはプロドラッグは、単独でもコロナウイルスの感染を予防又は治療するための医薬組成物の有効成分として有用であるが、他のエストロゲン受容体と複合体を形成し得る物質と併用するのがより好ましく、前記一般式(1)で表される化合物、又は薬学的に許容されるその塩、溶媒和物、水和物、若しくはプロドラッグと併用するのが更に好ましい。
一般式(2)において、アルキル基としては、炭素数1~6のアルキル基が好ましく、炭素数1~4のアルキル基がより好ましく、メチル基、エチル基、n-プロピル基、イソプロピル基が更に好ましく、メチル基がより更に好ましい。ここで、アルキル基には、直鎖又は分岐鎖のアルキル基が含まれる。
Xは炭素原子、硫黄原子又は窒素原子を示し、Yが窒素原子又は炭素原子を示す。ここで、Xが炭素原子又は窒素原子であり、Yが窒素原子であり、R6が水素原子又は炭素数1-6のアルキル基である場合が好ましく;Xが炭素原子であり、Yが窒素原子であり、R6が水素原子又は炭素数1-6のアルキル基である場合がより好ましい。この態様の場合、R6としては、炭素数1~6のアルキル基が好ましく、炭素数1~4のアルキル基がより好ましく、メチル基が更に好ましい。
また、別の態様として、Xが硫黄原子であり、Yが炭素原子であり、R6は存在しない場合も好ましい。

R7は水素原子又は酸素原子を示す。R7が酸素原子を示す場合、R7は隣接する炭素原子と一緒になってケトン基を形成する。

lは、4~7の整数を示すが、4~6がより好ましく、5又は6が更に好ましい。
mは、1~3の整数を示すが、1又は2がより好ましく、2が更に好ましい。

一般式(2)で表される化合物の塩としては、薬学的に許容される塩であれば特に限定されないが、塩酸塩、硫酸塩、硝酸塩などの鉱酸塩、リン酸塩、炭酸塩など、酢酸塩、コハク酸塩、酒石酸塩などの有機酸塩などが挙げられる。
一般式(2)で表される化合物又はその塩の溶媒和物、水和物としては、エタノール和物、水和物などが挙げられる。また、プロドラッグとしては、一般式(2)中のヒドロキシ基に種々の酸がエステル結合した場合などが挙げられる。
好ましい実施態様において、エストロゲン受容体と複合体を形成し得る物質は、下記式(2-1)で表されるバゼドキシフェン又はその酢酸塩である。
好ましい実施態様において、エストロゲン受容体と複合体を形成し得る物質は、下記式(2-2)で表されるラロキシフェン又はその塩酸塩である。
これらの一般式(2)で表される化合物の態様には、バゼドキシフェン及びラロキシフェンなどのSERMが含まれるが、SERMが単独で、又はSERMが一般式(1)で表される化合物と併用されることでコロナウイルス感染症、特にCOVID-19に対する作用については、ほとんど知られていない。
本発明の一般式(2)の化合物の態様には、SERMが含まれるが、SERMはエストロゲン受容体に結合することができ、当該複合体が核内に移行して特定の遺伝子(群)の転写を制御することができ、エストロゲン受容体の長期的な転写活性の調節に関与することができる。よって、特定の理論に拘束されず、本発明との関係において、本発明の一般式(2)の化合物は、エストロゲン受容体と複合体を形成することにより、本発明の抗コロナウイルス活性作用、特に抗SARS-CoV-2活性作用が発揮されると考えられる。
後記実施例に示すように、本発明のエストロゲン受容体と複合体を形成し得る物質の態様である(i)一般式(1)で表される化合物、又は薬学的に許容されるその塩、溶媒和物、水和物、若しくはプロドラッグ、特にエストラムスチン、エストラムスチンリン酸エステルナトリウム水和物、及びエストロムスチン、;(ii)一般式(2)で表される化合物、又は薬学的に許容されるその塩、溶媒和物、水和物、若しくはプロドラッグ、特にバゼドキシフェン及びラロキシフェン;並びに(iii)(i)及び(ii)の併用は、抗SARS-CoV-2活性作用、及びSARS-CoV-2感染による死亡に対する生存率の回復作用を有し、コロナウイルス感染症、特にCOVID-19の予防又は治療薬として有用であることを本願明細書において実証している。
ここで、「抗SARS-CoV-2活性作用」とは、(1)SARS-CoV-2が細胞内へ侵入することの阻害作用;(2)SARS-CoV-2の細胞内複製、SARS-CoV-2遺伝子の発現、及び/又はSARS-CoV-2構成要素アセンブリの阻害作用;((1)及び/又は(2)の結果として)SARS-CoV-2の増殖及び/又は細胞外放出の抑制作用;並びに((1)~(3)の結果として)SARS-CoV-2感染による細胞傷害の抑制作用のいずれか又はこれらの任意の組み合わせをいう。
また、「SARS-CoV-2感染による死亡に対する生存率の回復作用」とは、同種生物においてはSARS-CoV-2感染による致死が引き起こされる条件(例えば、ウイルス力価、ウイルス数など)において、本発明の医薬組成物又はエストロゲン受容体と複合体を形成し得る物質を当該生物に投与することにより生存率が回復する作用をいう。
ここで、「抗SARS-CoV-2活性作用」とは、(1)SARS-CoV-2が細胞内へ侵入することの阻害作用;(2)SARS-CoV-2の細胞内複製、SARS-CoV-2遺伝子の発現、及び/又はSARS-CoV-2構成要素アセンブリの阻害作用;((1)及び/又は(2)の結果として)SARS-CoV-2の増殖及び/又は細胞外放出の抑制作用;並びに((1)~(3)の結果として)SARS-CoV-2感染による細胞傷害の抑制作用のいずれか又はこれらの任意の組み合わせをいう。
また、「SARS-CoV-2感染による死亡に対する生存率の回復作用」とは、同種生物においてはSARS-CoV-2感染による致死が引き起こされる条件(例えば、ウイルス力価、ウイルス数など)において、本発明の医薬組成物又はエストロゲン受容体と複合体を形成し得る物質を当該生物に投与することにより生存率が回復する作用をいう。
本願明細書及び実施例においてSARS-CoV-2について実証された結果及びそれによる本発明の効果は、ウイルス分類学上の類似性、並びに類縁ウイルスとのゲノム及び遺伝子の相同性に基づき、SARS-CoV-2以外のコロナウイルスにも同様に適用できると考える。
従って、本発明に係る「抗コロナウイルス活性作用」とは、(1)コロナウイルスが細胞内へ侵入することの阻害作用、(2)コロナウイルスゲノムの細胞内複製、コロナウイルス遺伝子の発現、及び/又はコロナウイルス構成要素アセンブリの阻害作用、並びに((1)及び/又は(2)の結果として)コロナウイルスの増殖及び/又は細胞外放出の抑制作用のいずれか又はこれらの任意の組み合わせをいう。
また、「コロナウイルス感染による死亡に対する生存率の回復作用」とは、同種生物においてはコロナウイルス感染による致死が引き起こされる条件(例えば、ウイルス力価、ウイルス数など)において、本発明の医薬組成物又はエストロゲン受容体と複合体を形成し得る物質を当該生物に投与することにより生存率が回復する作用をいう。
従って、本発明に係る「抗コロナウイルス活性作用」とは、(1)コロナウイルスが細胞内へ侵入することの阻害作用、(2)コロナウイルスゲノムの細胞内複製、コロナウイルス遺伝子の発現、及び/又はコロナウイルス構成要素アセンブリの阻害作用、並びに((1)及び/又は(2)の結果として)コロナウイルスの増殖及び/又は細胞外放出の抑制作用のいずれか又はこれらの任意の組み合わせをいう。
また、「コロナウイルス感染による死亡に対する生存率の回復作用」とは、同種生物においてはコロナウイルス感染による致死が引き起こされる条件(例えば、ウイルス力価、ウイルス数など)において、本発明の医薬組成物又はエストロゲン受容体と複合体を形成し得る物質を当該生物に投与することにより生存率が回復する作用をいう。
本発明の医薬組成物は、前記エストロゲン受容体と複合体を形成し得る物質の1種又は2種以上を含有すればよいが、これに薬学的に許容される担体を配合して種々の投与形態の医薬組成物とするのが好ましい。
このような医薬組成物の投与形態としては、注射剤、経口剤(錠剤、顆粒剤、散剤、カプセル剤等)、吸入剤、点鼻剤、軟膏剤、クリーム剤、貼付剤、坐剤等が挙げられ、注射剤、経口剤、又は吸入剤が特に好ましい。
このような医薬組成物の投与形態としては、注射剤、経口剤(錠剤、顆粒剤、散剤、カプセル剤等)、吸入剤、点鼻剤、軟膏剤、クリーム剤、貼付剤、坐剤等が挙げられ、注射剤、経口剤、又は吸入剤が特に好ましい。
これらの医薬組成物の形態とするために用いられる薬学的に許容される担体としては、例えば、溶解補助剤、賦形剤、結合剤、滑沢剤、崩壊剤、着色剤、矯味剤、防腐剤、界面活性剤などが挙げられる。
より具体的には、水、エタノール、プロピレングリコール、ポリエチレングリコール、乳糖、ブドウ糖、D-マンニトール、澱粉、結晶セルロース、炭酸カルシウム、カオリン、デンプン、ゼラチン、ヒドロキシプロピルセルロース、ヒドロキシプロピルメチルセルロース、ポリビニルピロリドン、エタノール、カルボキシメチルセルロース、カルボキシメチルセルロースカルシウム塩、ステアリン酸マグネシウム、タルク、アセチルセルロース、白糖、酸化チタン、安息香酸、パラオキシ安息香酸エステル、デヒドロ酢酸ナトリウム、アラビアゴム、トラガント、メチルセルロース、卵黄、非イオン界面活性剤、白糖、単シロップ、クエン酸、蒸留水、エタノール、グリセリン、プロピレングリコール、マクロゴール、リン酸-水素ナトリウム、リン酸二水素ナトリウム、リン酸ナトリウム、ブドウ糖、塩化ナトリウム、フェノール、チメロサール、パラオキシ安息香酸エステル、亜硫酸水素ナトリウム、カルメロース、ヒプロメロース、ラウリル硫酸ナトリウム、無水ケイ酸(好ましくは、軽質無水ケイ酸)、アスコルビン酸、カルナウバロウ、酸化鉄(好ましくは三二酸化鉄)、デンプングリコール酸Na、部分アルファ化デンプン、マクロゴール6000、クロスポビドン、ポビドン、ポリソルベート80等が挙げられる。
経口剤とする場合には、賦形剤、結合剤、滑沢剤、崩壊剤、着色剤、矯味剤などが用いられ、常法によって製造できる。経口剤としては、錠剤、顆粒剤、散剤、カプセル剤などが好ましい。
注射剤とする場合には、水、溶解補助剤などが用いられる。注射剤としては、静脈内投与剤、皮下注射剤、筋肉内投与剤が好ましく、凍結乾燥製剤、粉末充填剤の形態が挙げられる。
吸入剤の形態としては、吸入粉末剤、吸入液剤、吸入エアゾール剤が挙げられるが、特に限定されるものではない。吸入粉末剤は、吸入量が一定となるように調製した粉末状の医薬組成物をエアゾールとして吸入する製剤である。ここで、粉末状医薬組成物には、糖や糖アルコールなどを添加剤として用いることもできる。吸入粉末剤の具体例としては、ドライパウダー吸入器(Dry Powder Inhaler)を挙げることができる。吸入液剤は、ネブライザ等により吸入する液状の吸入製剤である。前記有効成分を適当な溶剤を用いて、溶解又は懸濁し、溶液又は懸濁液として調製すればよい。調製時、等張化剤やpH調節剤等を添加することができる。吸入液剤に用いるデバイスはネブライザとして通常用いられるものを使用することができる。
点鼻剤は、通常、液状の医薬組成物をミスト状に噴霧又は蒸発して用いることができる。また、例えば賦形剤等と混合して、ゲル状、粉状、粒状等の固形に加工されたものであってもよい。点鼻剤の医薬組成物においては、薬学的に許容しうる担体を含んでもよい。薬学的に許容しうる担体としては、賦形剤、結合剤、崩壊剤、流動化剤、滑沢剤、コーティング剤、懸濁化剤、着色剤、甘味剤または界面活性剤等を用いることができる。
より具体的には、水、エタノール、プロピレングリコール、ポリエチレングリコール、乳糖、ブドウ糖、D-マンニトール、澱粉、結晶セルロース、炭酸カルシウム、カオリン、デンプン、ゼラチン、ヒドロキシプロピルセルロース、ヒドロキシプロピルメチルセルロース、ポリビニルピロリドン、エタノール、カルボキシメチルセルロース、カルボキシメチルセルロースカルシウム塩、ステアリン酸マグネシウム、タルク、アセチルセルロース、白糖、酸化チタン、安息香酸、パラオキシ安息香酸エステル、デヒドロ酢酸ナトリウム、アラビアゴム、トラガント、メチルセルロース、卵黄、非イオン界面活性剤、白糖、単シロップ、クエン酸、蒸留水、エタノール、グリセリン、プロピレングリコール、マクロゴール、リン酸-水素ナトリウム、リン酸二水素ナトリウム、リン酸ナトリウム、ブドウ糖、塩化ナトリウム、フェノール、チメロサール、パラオキシ安息香酸エステル、亜硫酸水素ナトリウム、カルメロース、ヒプロメロース、ラウリル硫酸ナトリウム、無水ケイ酸(好ましくは、軽質無水ケイ酸)、アスコルビン酸、カルナウバロウ、酸化鉄(好ましくは三二酸化鉄)、デンプングリコール酸Na、部分アルファ化デンプン、マクロゴール6000、クロスポビドン、ポビドン、ポリソルベート80等が挙げられる。
経口剤とする場合には、賦形剤、結合剤、滑沢剤、崩壊剤、着色剤、矯味剤などが用いられ、常法によって製造できる。経口剤としては、錠剤、顆粒剤、散剤、カプセル剤などが好ましい。
注射剤とする場合には、水、溶解補助剤などが用いられる。注射剤としては、静脈内投与剤、皮下注射剤、筋肉内投与剤が好ましく、凍結乾燥製剤、粉末充填剤の形態が挙げられる。

本発明の医薬組成物は、一般式(1)の化合物を50mg~250mg含有する製剤とすることができ、特にエストラムスチン、エストロムスチン又はエストラムスチンリン酸エステルを含有する医薬組成物としては、既存薬の組成を好ましく使用することができ、これらの有効成分を50mg~250mg含有するカプセル剤とすることができる。典型的には、1カプセル中、エストラムスチンリン酸エステルナトリウム水和物を156.7mg(エストラムスチンリン酸エステルとして140mg)含有することが好ましい。
本発明の医薬組成物は、一般式(2)の化合物を5mg~120mg含有する製剤とすることができ、特にバゼドキシフェン又はラロキシフェンを含有する医薬組成物としては、既存薬の組成を好ましく使用することができ、これらの有効成分を5mg~120mg含有するカプセル剤とすることができる。典型的には、1カプセル中、バゼドキシフェン酢酸塩を22.6mg(バゼドキシフェンとして20mg)、又はラロキシフェン塩酸塩を60mg含有することが好ましい。
本発明の医薬組成物は、一般式(1)の化合物を50mg~250mg及び一般式(2)の化合物を5mg~120mg含有する製剤とすることができる。ここで、一般式(1)の化合物としては、エストラムスチン、エストロムスチン又はエストラムスチンリン酸エステルのいずれかであることが好ましく、一般式(2)の化合物としては、バゼドキシフェン又はラロキシフェンであることが好ましい。より好ましくは、本発明の医薬組成物は、一般式(1)の化合物としては、エストラムスチン、又はエストラムスチンリン酸エステルのいずれかを含有し、一般式(2)の化合物としては、バゼドキシフェンを含有する。
本発明の医薬組成物は、一般式(2)の化合物を5mg~120mg含有する製剤とすることができ、特にバゼドキシフェン又はラロキシフェンを含有する医薬組成物としては、既存薬の組成を好ましく使用することができ、これらの有効成分を5mg~120mg含有するカプセル剤とすることができる。典型的には、1カプセル中、バゼドキシフェン酢酸塩を22.6mg(バゼドキシフェンとして20mg)、又はラロキシフェン塩酸塩を60mg含有することが好ましい。
本発明の医薬組成物は、一般式(1)の化合物を50mg~250mg及び一般式(2)の化合物を5mg~120mg含有する製剤とすることができる。ここで、一般式(1)の化合物としては、エストラムスチン、エストロムスチン又はエストラムスチンリン酸エステルのいずれかであることが好ましく、一般式(2)の化合物としては、バゼドキシフェン又はラロキシフェンであることが好ましい。より好ましくは、本発明の医薬組成物は、一般式(1)の化合物としては、エストラムスチン、又はエストラムスチンリン酸エステルのいずれかを含有し、一般式(2)の化合物としては、バゼドキシフェンを含有する。
本発明の医薬組成物中における本発明の有効成分の含有量は、有効成分、投与形態によって大きく変動し、特に限定されるものではないが、通常は、組成物全量に対して0.01~100質量%、好ましくは1~100質量%である。好ましくは、本発明の医薬組成物中における本発明の有効成分の含有量は、承認された態様と同じであってよく、又は異なっていてもよい。
有効成分は、経口剤の場合には、通常一投与単位当たり、5質量%~70質量%程度、より好ましくは35質量%~60質量%程度含有するのが好ましい。

本発明の医薬組成物の投与量は、有効成分、投与する患者の症状、年齢、投与方法によって異なるが、前記有効成分量として、成人に対して1日あたり10mg~1000mgであるのが好ましく、成人に対して1回あたり10mg~500mgであるのがより好ましい。
一般式(1)の場合、本発明の医薬組成物の有効成分量として、成人に対して1日あたり200mg~1000mgであるのが好ましく、成人に対して1回あたり100mg~500mgであるのが好ましい。特定の実施態様において、一般式(1)の場合、本発明の医薬組成物の有効成分量として、成人に対して1日あたり626.8mgであるのが好ましく、成人に対して1回あたり313.4mgであるのが好ましい。
一般式(2)の場合、本発明の医薬組成物の有効成分量として、成人に対して1日あたり10mg~100mgであるのが好ましく、成人に対して1回あたり10mg~80mgであるのが好ましい。特定の実施態様において、一般式(2)の場合、本発明の医薬組成物の有効成分量として、成人に対して1日1回投与で20mgであるのが好ましく、別の特定の実施態様において、一般式(2)の場合、本発明の医薬組成物の有効成分量として、成人に対して1日1回投与で60mgであるのが好ましい。
本発明の医薬組成物において、一般式(1)の化合物及び一般式(2)の化合物の両方が含まれる場合、一般式(1)の有効成分量と、一般式(2)の有効成分量とは、上記記載の任意の組み合わせであってよい。
またこの投与量は1日に1~4回に分けて投与することもでき、1日1回又は2回の経口投与がより好ましい。

本発明の医薬組成物において、一般式(1)の化合物及び一般式(2)の化合物の両方が含まれる場合、一般式(1)の有効成分量と、一般式(2)の有効成分量とは、上記記載の任意の組み合わせであってよい。
またこの投与量は1日に1~4回に分けて投与することもでき、1日1回又は2回の経口投与がより好ましい。
本発明の医薬組成物は、コロナウイルスの感染前、感染時、及び感染後の任意の時間に投与でき、投与回数も性差、年齢、重篤度などに応じて適宜増減できる。本発明のエストロゲン受容体と複合体を形成し得る物質の態様には、一般式(1)のようにステロイド骨格を有する女性ホルモンや一般式(2)のようにSERMを含み、エストロゲン受容体に作用する可能性が考えられるため、本発明の医薬組成物の投与に関する諸事項、例えば投与部位、投与量、投与回数、投与時間などは、コロナウイルス感染者の性別、年齢及び/又は重篤度などにより必要に応じて適宜変更することができる。
本発明の投与経路は、1つであってよく、又は別の投与経路を組み合わせてもよい。投与経路は、経口、経静脈、経鼻、経肺などを含む任意の投与経路であってよい。

好ましい実施態様において、本発明の医薬組成物を投与するタイミングとしては、例えば以下が挙げられる:
(i)コロナウイルス感染の数日前、例えば、7日より前の任意の日、7日前、6日前、5日前、4日前、3日前、2日前、1日前;
(ii)コロナウイルス感染の数時間前、例えば、12時間より前の任意の時間、12時間前、6時間前、3時間前、若しくは1時間前、又は12時間前から1時間前の任意の時間;
(iii)コロナウイルスの感染時(好ましくは、コロナウイルス感染前後1時間を含む);
(iv)コロナウイルス感染の数日後、例えば、7日より後の任意の日、7日後、6日後、5日後、4日後、3日後、2日後、1日後;
(v)コロナウイルス感染の数時間後、例えば、12時間より後の任意の時間、12時間後、6時間後、3時間後、若しくは1時間後、又は12時間後から1時間後の任意の時間。
本発明の医薬組成物は、上記タイミングのいずれかで1回投与することができ、又は任意のタイミングの組み合わせで複数回に分けて投与できる。
(i)コロナウイルス感染の数日前、例えば、7日より前の任意の日、7日前、6日前、5日前、4日前、3日前、2日前、1日前;
(ii)コロナウイルス感染の数時間前、例えば、12時間より前の任意の時間、12時間前、6時間前、3時間前、若しくは1時間前、又は12時間前から1時間前の任意の時間;
(iii)コロナウイルスの感染時(好ましくは、コロナウイルス感染前後1時間を含む);
(iv)コロナウイルス感染の数日後、例えば、7日より後の任意の日、7日後、6日後、5日後、4日後、3日後、2日後、1日後;
(v)コロナウイルス感染の数時間後、例えば、12時間より後の任意の時間、12時間後、6時間後、3時間後、若しくは1時間後、又は12時間後から1時間後の任意の時間。
本発明の医薬組成物は、上記タイミングのいずれかで1回投与することができ、又は任意のタイミングの組み合わせで複数回に分けて投与できる。
本発明の好ましい別の実施態様は、コロナウイルス感染症、特にCOVID-19を予防又は治療するための、前記エストロゲン受容体と複合体を形成し得る物質である。
本発明の好ましい別の実施態様は、コロナウイルス感染症、特にCOVID-19を予防又は治療用の医薬組成物を製造するための、前記エストロゲン受容体と複合体を形成し得る物質の使用である。
本発明の好ましい別の実施態様は、前記エストロゲン受容体と複合体を形成し得る物質を投与することを特徴とする、コロナウイルス感染症、特にCOVID-19を予防又は治療する方法である。
本発明の好ましい別の実施態様は、コロナウイルス感染症、特にCOVID-19を予防又は治療用の医薬組成物を製造するための、前記エストロゲン受容体と複合体を形成し得る物質の使用である。

次に実施例を挙げて本発明を更に詳細に説明するが、本発明はこれらの実施例に限定されるものではない。
実施例1(抗SARS-CoV-2薬の探索)
1.実験方法
SARS-CoV-2感染細胞の生存率を指標として、抗SARS-CoV-2薬のスクリーニングを実施した。
使用ライブラリ:LTTバイオファーマ既承認薬ライブラリ(株式会社LTTバイオファーマから入手した)
使用細胞:VeroE6-TMPRSS2アフリカミドリザル腎由来細胞(医薬基盤・健康・栄養研究所から入手した)
ウイルス株:SARS-CoV-2 NIID株(国立感染症研究所より入手した)
陰性コントロール:化合物未処理
サンプル群:既承認薬ライブラリ(10μM)添加群
評価:VeroE6-TMPRSS2細胞は、37℃、5%CO2存在下で、DMEM(高グルコース)に非働化ウシ胎児血清(FBS)を10%となるように添加した培地を用いて培養した。VeroE6-TMPRSS2細胞を30,000細胞/ウェルで96ウェルプレートに播種し、細胞が接着後に1,500TCID50価のSARS-CoV-2を各化合物と共に添加した。48時間の培養後、WST-8アッセイ(Dojindoより入手)にて感染細胞の生存率を評価した。化合物非添加群の感染細胞では約30%の生存率であるのに対し、生存率が80%以上を示す既承認薬ライブラリ化合物を選別した。非感染細胞において、20%以上の増殖抑制を示して細胞毒性があると判断される化合物は選別外とした。

2.実験結果
図1にスクリーニングの結果を示す。SARS-CoV-2感染に対し、80%以上の生存率を示す既承認薬ライブラリ化合物を選別した。その結果、エストラムスチンリン酸エステル(Estramustine phosphate)、バゼドキシフェン酢酸塩(Bazedoxifene acetate)、及びラロキシフェン塩酸塩(Raloxifene hydrochloride)が同定された(図1)。
これらの化合物について、エストラムスチンリン酸エステルにおけるステロイド骨格を有する女性ホルモン部分、並びにSERMであるバゼドキシフェン及びラロキシフェンは、エストロゲン受容体に結合して、当該複合体が核内に移行して特定の遺伝子(群)を制御するということが知られ、共通の特徴を有する。
エストラムスチンリン酸エステルは、アルキル化剤としても知られ、マイクロチューブルの構築阻害により殺細胞作用を示すことも知られているが、SARS-CoV-2に対する効果は知られていない。
図1にスクリーニングの結果を示す。SARS-CoV-2感染に対し、80%以上の生存率を示す既承認薬ライブラリ化合物を選別した。その結果、エストラムスチンリン酸エステル(Estramustine phosphate)、バゼドキシフェン酢酸塩(Bazedoxifene acetate)、及びラロキシフェン塩酸塩(Raloxifene hydrochloride)が同定された(図1)。
これらの化合物について、エストラムスチンリン酸エステルにおけるステロイド骨格を有する女性ホルモン部分、並びにSERMであるバゼドキシフェン及びラロキシフェンは、エストロゲン受容体に結合して、当該複合体が核内に移行して特定の遺伝子(群)を制御するということが知られ、共通の特徴を有する。
エストラムスチンリン酸エステルは、アルキル化剤としても知られ、マイクロチューブルの構築阻害により殺細胞作用を示すことも知られているが、SARS-CoV-2に対する効果は知られていない。
実施例2(本発明の化合物の単剤又は併用によるin vitro抗SARS-CoV-2効果の評価)
1.実験方法
SARS-CoV-2感染細胞から放出される子孫ウイルス量を指標として、抗SARS-CoV-2活性を評価した。
Estramustine phosphate:Estramustine phosphate(Sodium salt)はCayman Chemicalより購入し、滅菌水に溶解して10mMの溶液を調製した。
Bazedoxifene acetate:Bazedoxifene acetateはMedChemExpressより購入し、DMSOに溶解して10mMの溶液を調製した。
Remdesivir:RemdesivirはSelleckより購入し、DMSOに溶解して10mMの溶液を調製した。
使用細胞:VeroE6-TMPRSS2アフリカミドリザル腎由来細胞(医薬基盤・健康・栄養研究所から入手した)
ウイルス株:SARS-CoV-2 NIID株(国立感染症研究所より入手した)

逆転写反応:cDNA合成キット(TOYOBO社;ReverTraAce)とN gene Reverseプライマーを用いて逆転写反応を行った。
定量PCR法:定量PCRキット(Thermofisher社;SYBR Green Real-Time PCR Master Mixes)とN gene Forwardプライマー及びN gene Reverseプライマーを用いて行った。
ウイルスRNA精製:培養上清からNucleoSpin RNA Virus精製キット(TaKaRa社)を用いてウイルスRNAを精製した。
評価:VeroE6-TMPRSS2細胞は、37℃、5%CO2存在下で、DMEM(高グルコース)に非働化ウシ胎児血清(FBS)を10%となるように添加した培地を用いて培養した。VeroE6-TMPRSS2細胞を2×105細胞/ウェルで12ウェルプレートに播種し、細胞が接着後に2×104TCID50価のSARS-CoV-2を各濃度のRemdesivir、Estramustine phosphate及びBazedoxifene acetateと共に添加した。1時間のウイルス吸着後、DMEM培地で細胞を洗浄し、未吸着のウイルスを除去後、各濃度の各化合物を含む1mlのDMEM培地を添加し、37℃、5%CO2存在下で24時間培養した。培養上清は15,000rpm、3分間の遠心後に回収し、ウイルスRNAを精製した。精製したウイルスRNAは、逆転転写反応後、SARS-CoV-2のN遺伝子に特異的なプライマーを用いて、定量PCR法を行った。化合物非添加群を100%として、各化合物添加群のウイルス産生量を評価した。

評価:VeroE6-TMPRSS2細胞は、37℃、5%CO2存在下で、DMEM(高グルコース)に非働化ウシ胎児血清(FBS)を10%となるように添加した培地を用いて培養した。VeroE6-TMPRSS2細胞を2×105細胞/ウェルで12ウェルプレートに播種し、細胞が接着後に2×104TCID50価のSARS-CoV-2を各濃度のRemdesivir、Estramustine phosphate及びBazedoxifene acetateと共に添加した。1時間のウイルス吸着後、DMEM培地で細胞を洗浄し、未吸着のウイルスを除去後、各濃度の各化合物を含む1mlのDMEM培地を添加し、37℃、5%CO2存在下で24時間培養した。培養上清は15,000rpm、3分間の遠心後に回収し、ウイルスRNAを精製した。精製したウイルスRNAは、逆転転写反応後、SARS-CoV-2のN遺伝子に特異的なプライマーを用いて、定量PCR法を行った。化合物非添加群を100%として、各化合物添加群のウイルス産生量を評価した。
2.実験結果
図2にEstramustine phosphate及びBazedoxifene acetateの単独投与、もしくは併用投与による抗SARS-CoV-2活性を示す。単独投与では、Estramustine phosphate及びBazedoxifene acetateは、濃度依存的に抗SARS-CoV-2活性を示すとともに最終的にはSARS-CoV-2産生の完全な抑制を示した。これらの単独投与におけるIC99値は、それぞれ約3μMであった。
一方、Estramustine phosphate及びBazedoxifene acetateの併用投与でも、濃度依存的に抗SARS-CoV-2活性を示すとともに最終的にはSARS-CoV-2産生の完全な抑制を示した。この併用におけるIC99値は0.3~1μMまで低下した。また、Combination indexは0.9(CI=1,additive;CI<1,synergism;CI>1,antagonism)であることから、Estramustine phosphate及びBazedoxifene acetateの併用は、相乗的に抗ウイルス活性が促進されることが観察された。
また、参考として、Remdesivirの抗SARS-CoV-2効果についてもパネルAに示す。この結果の比較から、本発明の化合物又はその併用は、COVID-19既承認薬であるRemdesivirと同等以上の抗SARS-CoV-2効果を有することが示された。
図2にEstramustine phosphate及びBazedoxifene acetateの単独投与、もしくは併用投与による抗SARS-CoV-2活性を示す。単独投与では、Estramustine phosphate及びBazedoxifene acetateは、濃度依存的に抗SARS-CoV-2活性を示すとともに最終的にはSARS-CoV-2産生の完全な抑制を示した。これらの単独投与におけるIC99値は、それぞれ約3μMであった。

実施例3(エストラムスチンリン酸エステルの作用機序の解析)
1.実験方法
SARS-CoV-2感染細胞でのスパイクタンパク質の発現量をウェスタンブロット法により検出し、エストラムスチンリン酸エステルの作用機構を評価した。
エストラムスチンリン酸エステル:エストラムスチンリン酸エステル(ナトリウム塩)はCayman Chemicalより購入しより購入し、滅菌水に溶解して10mMの溶液を調製した。
使用細胞:VeroE6-TMPRSS2アフリカミドリザル腎由来細胞(医薬基盤・健康・栄養研究所から入手した)
ウイルス株:SARS-CoV-2 NIID株(国立感染症研究所より入手した)、A型インフルエンザウイルス(筑波大学で保有)

評価:VeroE6-TMPRSS2細胞は、37℃、5%CO2存在下で、DMEM(高グルコース)に非働化ウシ胎児血清(FBS)を10%となるように添加した培地を用いて培養した。VeroE6-TMPRSS2細胞を2×105細胞/ウェルで12ウェルプレートに播種し、細胞が接着後に2×105TCID50価のSARS-CoV-2、若しくは2×105PFUのA型インフルエンザウイルスを各濃度のエストラムスチンリン酸エステルと共に添加した。1時間のウイルス吸着後、DMEM培地で細胞を洗浄し、未吸着のウイルスを除去後、各濃度のエストラムスチンリン酸エステルを含む1mLのDMEM培地を添加し、37℃、5%CO2存在下で24時間培養した。エストラムスチンリン酸エステルの作用機構を理解するため、ウイルス吸着時のみ、エストラムスチンリン酸エステルを添加したサンプル、及びウイルス吸着後のみ、エストラムスチンリン酸エステルを添加したサンプルも調製した。感染細胞を回収後、細胞抽出液を調製して7.5%SDS-PAGEで展開後、各抗体を用いてウェスタンブロット法を行った。

2.実験結果
図3にエストラムスチンリン酸エステルによるウイルスタンパク質発現阻害活性を示す。10μMのエストラムスチンリン酸エステルを添加することで、SARS-CoV-2のスパイクタンパク質発現量は顕著に低下するのに対し、A型インフルエンザウイルスのNPタンパク質の発現量は低下しなかった。よって、エストラムスチンリン酸エステルの抗ウイルス活性はSARS-CoV-2に特異的であることが示された。
また、エストラムスチンリン酸エステルは、ウイルス吸着時だけでなく、ウイルス吸着後から添加することでも、顕著な抗SARS-CoV-2活性を示した。よって、エストラムスチンリン酸エステルは、SARS-CoV-2の細胞への侵入後からウイルスタンパク質発現までのいずれかの過程も抑制することが示された。

実施例4(エストラムスチンリン酸エステル関連化合物の抗SARS-CoV-2活性の評価)
1.実験方法
エストラムスチンリン酸エステルは細胞内で脱リン酸化されエストラムスチンに変換される。一方、17β-ヒドロキシステロイドデヒドロゲナーゼにより、酸化されることでエストロムスチンに代謝される。また、エストラムスチンとエストロムスチンはナイトロゲンマスタード基が加水分解されることで、それぞれエストラジオールとエストロンに変換される。そこで、SARS-CoV-2感染細胞から放出される子孫ウイルス量を指標として、これらエストラムスチンリン酸エステル関連化合物の抗SARS-CoV-2活性を評価した。
エストラムスチンリン酸エステル:エストラムスチンリン酸エステル(ナトリウム塩)はCayman Chemicalより購入し、滅菌水に溶解して10 mMの溶液を調製した。
エストラムスチン、エストラジオール、エストロン:エストラムスチンはBLD Pharmatech、エストロンとエストラジオールはWakoより購入し、DMSOに溶解して10mMの溶液を調製した。

ウイルスRNA精製:培養上清からNucleoSpin RNA Virus精製キット(TaKaRa社)を用いてウイルスRNAを精製した。
評価:VeroE6-TMPRSS2細胞は、37℃、5%CO2存在下で、DMEM(高グルコース)に非働化ウシ胎児血清(FBS)を10%となるように添加した培地を用いて培養した。VeroE6-TMPRSS2細胞を2×105細胞/ウェルで12ウェルプレートに播種し、細胞が接着後に2×104TCID50価のSARS-CoV-2を各濃度の化合物と共に添加した。1時間のウイルス吸着後、DMEM培地で細胞を洗浄し、未吸着のウイルスを除去後、各濃度の化合物を含む1mLのDMEM培地を添加し、37℃、5%CO2存在下で24時間培養した。培養上清は15,000rpm、3分間の遠心後に回収し、ウイルスRNAを精製した。精製したウイルスRNAは、逆転転写反応後、SARS-CoV-2のN遺伝子に特異的なプライマーを用いて、定量PCR法を行った。化合物非添加群を100%として、各濃度の化合物添加群のウイルス産生量を評価した。

2.実験結果
図3にエストラムスチンリン酸エステル関連化合物による抗SARS-CoV-2活性を示す。エストラムスチンリン酸エステルと比較して、エストラムスチンでは高い抗SARS-CoV-2活性が観察された。一方、エストロムスチンでは活性は低下したものの、抗SARS-CoV-2活性は観察されており、ステロイド骨格上の水酸基は必須ではないことが示唆された。また、ナイトロゲンマスタード基が加水分解されたエストラジオール及びエストロンでは、抗ウイルス活性が観察されず、エストラムスチンの抗SARS-CoV-2活性にはナイトロゲンマスタード基が必須であることが明らかになった。
他方、既承認薬ライブラリには、ナイトロジェンマスタード又はその類似構造を含む化合物として、シクロホスファミド水和物、イホスファミド、メルファラン、及びベンダムスチン塩酸塩が含まれていたが、実施例1のスクリーニングではヒットしなかった。これは、本発明の構造に係るナイトロジェンマスタード及びステロイド女性ホルモンの両方の構造が存在することが、本発明の抗SARS-CoV-2活性に重要な寄与を果たしていることを示すと考えられる。
図3にエストラムスチンリン酸エステル関連化合物による抗SARS-CoV-2活性を示す。エストラムスチンリン酸エステルと比較して、エストラムスチンでは高い抗SARS-CoV-2活性が観察された。一方、エストロムスチンでは活性は低下したものの、抗SARS-CoV-2活性は観察されており、ステロイド骨格上の水酸基は必須ではないことが示唆された。また、ナイトロゲンマスタード基が加水分解されたエストラジオール及びエストロンでは、抗ウイルス活性が観察されず、エストラムスチンの抗SARS-CoV-2活性にはナイトロゲンマスタード基が必須であることが明らかになった。
他方、既承認薬ライブラリには、ナイトロジェンマスタード又はその類似構造を含む化合物として、シクロホスファミド水和物、イホスファミド、メルファラン、及びベンダムスチン塩酸塩が含まれていたが、実施例1のスクリーニングではヒットしなかった。これは、本発明の構造に係るナイトロジェンマスタード及びステロイド女性ホルモンの両方の構造が存在することが、本発明の抗SARS-CoV-2活性に重要な寄与を果たしていることを示すと考えられる。
実施例5(本発明の化合物によるin vivo抗SARS-CoV-2効果の評価)
1.実験方法
ヒトACE2トランスジェニックマウスにSARS-CoV-2を感染させ、生存率を評価した。
Estramustine phosphate:Estramustine phosphate(Sodium salt)はCayman Chemicalより購入し、滅菌水に溶解して10mMの溶液を調製後、PBSに希釈して鼻腔投与した。
Bazedoxifene acetate:Bazedoxifene acetateはMedChemExpressより購入し、DMSOに溶解して10mMの溶液を調製後、コーンオイルで希釈して腹腔投与した。
使用マウス:C57BL/6系統hACE2トランスジェニックマウス(医薬基盤・健康・栄養研究所から入手した)
ウイルス株:SARS-CoV-2 NIID株(国立感染症研究所より入手した)
陰性コントロール:化合物未処理
サンプル群:Estramustine phosphateを0.2mg/35μl/headで鼻腔投与、もしくはBazedoxifene acetateを3mg/200μl/kgで腹腔投与した。
評価:500TCID50/headのSARS-CoV-2をペントバルビタール麻酔下のマウスに鼻腔接種した。投与当日から感染5日後まで、1回/日でEstramustine phosphate及びBazedoxifene acetateを投与した。
1.実験方法
ヒトACE2トランスジェニックマウスにSARS-CoV-2を感染させ、生存率を評価した。
Estramustine phosphate:Estramustine phosphate(Sodium salt)はCayman Chemicalより購入し、滅菌水に溶解して10mMの溶液を調製後、PBSに希釈して鼻腔投与した。
Bazedoxifene acetate:Bazedoxifene acetateはMedChemExpressより購入し、DMSOに溶解して10mMの溶液を調製後、コーンオイルで希釈して腹腔投与した。
使用マウス:C57BL/6系統hACE2トランスジェニックマウス(医薬基盤・健康・栄養研究所から入手した)

図5にEstramustine phosphate投与によるマウスの生存率解析結果を示す。SARS-CoV-2接種5日後に全頭死亡するマウスインビボ系において、Estramustine phosphate単剤を投与することで、顕著にマウスの生存率が改善された(80%程度回復)。また、Estramustine phosphateをBazedoxifene acetateと併用投与することにより、より完全な生存率の改善が見られた(100%回復)。よって、Estramustine phosphateは、in vivoでも抗SARS-CoV-2活性があることが示唆され、Bazedoxifene acetateによりEstramustine phosphateのその抗コロナウイルス効果、特に抗SARS-CoV-2効果は増強される可能性がある。
また、図6に非感染マウスでの体重測定結果を示す。麻酔投与により、一時的な体重減少が観察されるが、顕著なものではなく、Estramustine phosphate及びBazedoxifene acetate投与による強い毒性は観察されていない。

一方、EMP及びBZA添加時は、血清存在下でもEFP遺伝子の発現促進を観察することができた。従って、EMP及びBZAは、血清の存在にかかわらず、エストロゲン受容体の発現又は活性の増加の持続性が長い、又はその作用が強いことが示唆された。

- 前記コロナウイルス感染症が、SARS-CoV2(severe acute respiratory syndrome coronavirus 2)による感染症であるCOVID-19である、請求項1~20のいずれか1項記載の医薬組成物。
- コロナウイルス感染症を予防又は治療するための、エストロゲン受容体と複合体を形成し得る物質。
- コロナウイルス感染症を予防又は治療用の医薬組成物を製造するための、エストロゲン受容体と複合体を形成し得る物質の使用。
- エストロゲン受容体と複合体を形成し得る物質を投与することを特徴とする、コロナウイルス感染症を予防又は治療する方法。
