Abstract

치료 약물 모니터링(TDM)은 개별 환자의 적정 약물 치료법을 위해 지속적으로 약물 반응을 관찰하면서 피드백을 제공하는 일련의 활동을 의미한다. 주로 약물농도 추이를 모니터링하여 유효 혈중 농도를 유지하면서 동시에 약물 과민반응을 최소화함으로써 환자 개인별 치료 효과를 극대화하는데 이용된다. 치료 약물 모니터링의 주요 대상이 되는 약물은 치료 농도 범위가 잘 알려진 몇몇 항생제와 항간질제, 강심제 등이 있으며, 연간 약 1만 건을 실시하고 있다. 적정 약물 요법을 위한 치료 약물 농도 모니터링은 치료 혈장 농도 범위(therapeutic range of plasma concentration)가 잘 알려져 있는 약물의 투여 시 측정된 약물 농도 데이터를 근거로 약물 용법을 조정함으로써 약동학적 개인차를 우회하는 것이 가능하다. 최근 체액 내 약물 및 이의 대사체를 측정하는 분석 화학의 발전과, 임상 약물 동태학(Clinical pharmacokinetics)의 도입 등으로 임상적으로 개개 환자에서의 치료적 약물 모니터링이 이미 보편화되고, 이러한 새로운 의료 분야의 연구 및 임상 응용의 필요성이 점차 증가하고 있다.
TDM이 경제적 관점에서도 유용한 서비스이지만, 약리학적으로도 충분히 그 근거가 있다고 할 수 있다. 동일 용량을 투여하고도 개인간의 반응효과에서 차이가 큰 것은 약동학적 변이와 약력학적 변이가 함께 포함되어 있기 때문인데, 동일 농도로 맞추어 주면 약동학적 변이는 제거되고, 약력학적 변이만 남기 때문에 더욱 정확하고 정밀하게 효과를 예측할 수 있다.
TDM은 약물의 용량, 용법의 적절성에 대한 유용한 정보를 제공하지만, 환자가 가진 질환의 심한 정도, 독성 발현 정도 등을 전반적으로 고려하여 위험에 비하여 이익이 상회한다고 판단할 때에는 TDM 결과 적용이 필요하다. 따라서 농도 및 투약 정보를 이용하여 그 개인의 약동학적 파라미터를 예측하고, 목표 농도를 이루기 위한 적정 용량, 용법을 결정하는 것이 유리하다.
생물학적 제제 중 TDM의 필요성이 대두되고 있는 항 TNF 약물 모니터링에 대해 추가 설명을 하자면, 항TNF 약물은 류마티스 관절염, 염증성 장질환 등의 만성 염증질환을 유발하는 주요 염증매개물질인 Tumor necrosis factor α(TNF- α)에 대한 항체로서 크론병에서 처음 사용된 이후 현재는 궤양성 대장염에서도 널리 사용되고 있다. 하지만, 항TNF 약물은 기존의 다른 약제에 비해 약가가 매우 비싸고, 결핵 등의 기회감염을 유발할 수 있다는 단점이 있으며, 약 1/3의 환자에서는 항TNF 약물에 치료 효과를 보이지 못하고 있으며, 나머지 환자들에게 치료를 지속하는 경우 그 효과가 떨어진다고 알려져 있다.
또한 크론병 환자에서 초기에 항 TNF 치료에 반응했던 환자들의 약 40%에서 반응 소실이 나타난다고 알려져 있다. 치료 효과의 감소 원인으로는, 혈중 항TNF약물의 농도 감소, 혈중 항TNF 약물에 대한 항체 형성이 잘 알려져 있으며 항TNF 약물을 먼저 사용하기 시작한 서양에서는 혈중 항TNF 약물의 농도와 혈중 항TNF 약물에 대한 항체 형성 유무를 측정하여 치료 도중 효과가 떨어졌을 때 용량 혹은 용법을 변경할지, 다른 약제로 변경해야 할지, 현재 치료를 지속할지 등을 결정하고 있다.
최근에 발표된 메타 분석에 따르면, 항TNF 약물에 대한 항체가 형성된 경우와 혈중 항TNF 약물의 농도가 낮은 경우, 치료 반응이 소실될 가능성이 높은 경향을 보였다. 2015년 발표된 TAXIT (The Trough Concentration Adapted Infliximab Treatment) 연구에서는 인플릭시맙 유지 요법으로 치료받는 염증성 장질환 환자에서 Trough level target (3~7 ㎍/mL)을 설정하여 대상자의 투약스케줄을 조정하거나 약물용량을 조절하였다. TAXIT 연구결과를 살펴보면 maintenance phase에서 clinically based dosing 군과 Concentration based dosing 군의 임상적 관해율의 차이는 없었으나, Optimization phase에서 크론병 환자의 경우에는 임상적 관해율이 65.1%에서 88.4%로 증가하였다(p=0.02).
또한 다른 연구에서는 IBD(Inflammatory Bowel Disease) 환자에서 인플릭시맙의 trough level을 6~10㎍/mL 유지했을 때, 내시경을 이용해 평가한 Mucosal healing rate가 80-90%로 나타났다. 이와 같이 해외에서는 이미 염증성 장질환 환자들에게 인플릭시맙을 투약하는 경우, TDM을 적극적으로 이용하여 그 검사 결과를 치료에 반영하고 있다. 그러나 국내에서 많은 임상 의사들이 염증성 장질환 환자의 인플릭시맙 치료에 있어서 TDM의 필요성을 제기하고 있으나, 비용적인 문제 및 검사를 하더라도 infliximab trough level 결과를 얻기까지의 시간 지연으로 인해 국내에서는 실제 진료 시에 TDM 검사 시행이 어려운 상태이다.
2. ADA(anti drug antibodies) 모니터링의 필요성
생물학적 제제의 안정하고 지속적으로 치료효과를 유지하기 위해 약물의 정량적 모니터링으로 약동학(pharmacokinetics) 및 안정성 평가가 필요하다. 그러나 생물학적 제제의 면역반응에 의해 유도된 ADA는 약물의 표적 결합을 중화하거나 제거율을 변경하고 혈액내 순환에서 약물농도를 측정하는 PK 분석을 방해하고 독성역학(TK) 분석에 상당한 영향을 미칠 수 있다.
안정성에 대한 우려는 면역 반응 평가의 필요성이 대두되는 주요 요인으로 작용한다. 즉 약물의 존재 하에서도 ADA를 충분히 검출할 수 있는 분석법 개발로 면역반응을 일으키는 피험자를 정확히 식별하는 것이 주요 관건이다. 이러한 이유로 증가된 약물 내성을 보여주는 ADA 분석법을 개발하려는 노력이 계속되고 있으며 다양한 접근 방식에 대한 관심이 증가하는 추세이다. ADA 분석법은 정량적 PK 분석법과 달리 실제 참조표준이 없으므로 면역반응을 정량화는 어렵다. 대신 약물 치료 경험이 없는 피험자로부터 얻은 샘플의 통계적 기반으로 양성 결과를 정의하고 반응 특이성을 확인하여 양성결과를 도출하는 방식으로 평가한다.
3. 현재 구현된 테스트 방법의 문제점
(ELISA 방법의 한계) 현재 대부분의 TDM 검사는 환자의 혈액을 채취하고, ELISA(Enzyme-linked immunosorbent assay)를 사용하여 농도를 정량적으로 측정한다. 이 방법은 96 웰 플레이트에서 특정 시간 동안 정확한 부피의 다양한 시약을 반응시키는 복잡한 과정을 필요로 한다. 대형 병원 중앙 연구실에서 고가의 장비를 사용하거나 전문가가 검사를 해야 하는 등의 문제로 검사는 오랜 시간이 걸리며, 가격이 비싸며, 결과가 실제 치료에 반영되기까지 3주 이상 소요된다. 또한 피험자는 약 투여전에 TDM 검사를 위해 추가적으로 병원방문을 해야 하는 불편함과 비용 부담이 증가하는 문제가 있다.
(ADA 검사법의 한계) ADA(anti drug antibodies)를 정확하게 정량하기 위해서는, 유리 형태(free form) 및 약물과의 결합 형태(bound form) 모두 측정하는 기술이 필요하다. 상용 제품 중 상당수는 유리 형태만을 측정하며, 일부 제품에서 총 ADA 측정을 하고 있으며, ELISA 방법을 사용하고 있다. 후자의 경우 시료의 전처리 단계가 여러 단계에 걸쳐 필요하고, 상당한 시간이 소요되어서 시험자간 수치 차이가 발생할 소지가 높고, 특히 시료의 수가 많을수록 교차 오염 및 시험 에러 발생 유발 위험도가 높다.
일반적으로 사용되는 ADA 검출 방법은 비오틴 표지 약물과 효소 표지 약물 사이의 multivalent ADA 브릿지 방법이다. 이 방법은 내인성 약물 간섭 (위음성 ADA) 및/또는 약물 표적 간섭(위양성 ADA)에 취약하다. 즉, 약물과 ADA의 면역복합체의 해리반응에 의존하여 ADA가 분석 시약에 의해 포획되도록 하는 방법이다. 그러나 면역복합체를 분리하는데 사용되는 전처리 단계는 생체 내 ADA 시나리오를 분명하게 대신할 수 있는 참조 표준이 없어서 정확하고 정밀하게 수행하기 어렵다. 더욱이 결합항체를 약물과 분리하고 ADA를 포획하는 과정에서 에피토프(eptiotpe)의 중첩(overlap)에 의한 약물의 직접적인 경쟁(direct competition)이나 에피토프가 매우 근접하여(proximal) 입체 장애(steric hindrance) 혹은 ADA의 avidity 나 affinity 로 인한 반응 저해로 인해 ADA 검출의 정확도에 문제가 발생될 수 있다. 이러한 이유로 산 또는 염기 해리, 3-party(3자, 타사) 결합 파트너 경쟁 억제 및/또는 간섭 요인 제거, 고체상 추출(SPEAD), ACE와 같은 분석법이 개발 혹은 고안되어져 왔다.
4. POCT 진단 방법 개발의 필요성
TDM을 통해 환자들마다 치료에 대한 반응이 달라 발생할 수 있는 치료효과의 저하나 부작용 뿐만 아니라 이로 인한 환자의 경제적 부담을 줄여서 적절한 시기에 적절한 치료를 하는 맞춤치료가 필요하다. TDM 검사에 있어서 기존의 효소 결합 면역 흡착법은 긴 검사시간, 고비용, 복잡함 등 때문에 맞춤 치료를 위한 적절한 방법이라 할 수 없다. 따라서 표준화되고 저렴하면서 진료 현장에서 빠르게 결과를 확인함으로써 진료 시 바로 의사 결정을 할 수 있는 검사 방법이 요구된다.
현장검사(point-of-care test, POCT)는 피검자(환자) 가까이에서 복잡한 실험실 장비가 없어도 검사 시료의 전처리 과정이 없이 다양한 검사를 쉽고 신속하게 시행하여 진단 및 치료에 이용할 수 있는 검사로서, 이로 인한 의료적, 혹은 경제적 효과를 최대화할 수 있다. 따라서 현장검사는 맞춤 치료를 위해 필요한 치료 약물 농도 감시를 현장에서 즉각 모니터링함으로써 맞춤 치료의 유효성을 극대화할 수 있다.
현장검사법을 토대로 한 ADA 검출 POCT 제품은 현재 1종(Promonitor Quick anti-IFX)이 개발되어 있다. 이 제품은 전혈 사용이 가능하나 정성 분석법이며, 약물 결합 항체(bound form)가 아닌 유리 항체(free form)만을 검사하는 방법이므로, 임상적 유용성이 극히 제한적이다. 즉, ADA는 혈중에 약물 결합형(bound form)과 유리형(free form)이 동시에 존재하고 있으며, 정확한 ADA의 검출은 약물 결합 항체와 유리 항체 모두(total form) 측정이 가능해야 한다. 또한 약물의 존재 하에서 면역복합체로부터 ADA 검출뿐만 아니라 약물에 대한 내성이 증가된 ADA 검사법에 기반한 POCT 제품은 전무한 상태이다.
따라서 본 발명은 정확하고, 간단하고, 신속한 방식으로 시료 내 존재하는 항 약물 항체를 정량화 할 수 있는 신속 검출 키트를 제공하는 것을 일 목적으로 한다.
또한 본 발명은 분석 물질이나 환자 시료에 존재하는 약물의 영향을 받지 않고, 간단한 방법으로 항 약물 항체의 수치를 정확히 측정할 수 있는 신속 검출 키트를 제공하는 것을 다른 목적으로 한다.
상기 목적을 달성하기 위한 본 발명은 항 약물 항체(anti-drug antibody)의 신속 측정 방법으로서, (a) 항 약물 항체와 약물의 복합체를 포함하는 생물학적 시료를 산과 접촉시켜 상기 항 약물 항체와 약물의 복합체를 해리시키는 단계; (b) 상기 복합체가 해리된 시료를, 신호 발생 수단이 결합된 약물 및 링커가 결합된 약물을 포함하는 건조 형태의 조성물과 접촉시켜 약물-항체-약물의 표지된 면역 복합체를 형성하는 단계; 및 (c) 상기 표지된 면역 복합체에서 발생하는 신호를 검출하는 단계를 포함하는 것을 일 측면으로 한다.
상기 건조 형태의 조성물은 추가로, 안정화제, 완충제 및 계면활성제를 포함할 수 있다.
상기 복합체가 해리된 시료와 상기 건조 형태의 조성물의 총 부피는 200㎕ 이하일 수 있다.
상기 표지된 면역 복합체에서 발생하는 신호를 검출하는 단계는, 표지된 면역 복합체를 링커가 고정된 테스트 라인을 포함하는 측방 유동 카트리지에 로딩하는 것을 포함할 수 있다.
본 발명은 정확하고, 간단하고, 빠른 방식으로 시료 내 존재하는 항 약물 항체를 정량화 할 수 있다. 본 발명은 분석 물질이나 환자 시료에 존재하는 약물의 영향을 받지 않고, 간단한 방법으로 항 약물 항체의 양을 정확히 측정한다. 본 발명은 약물의 효과적 치료와 관련될 수 있는 항 약물 항체에 관한 정확한 결정을 할 수 있어, 보다 정확한 치료에 도움이 된다.

본 발명은, (a) 항 약물 항체와 약물의 복합체를 포함하는 생물학적 시료를 산과 접촉시켜 상기 항 약물 항체와 약물의 복합체를 해리시키는 단계; (b) 상기 복합체가 해리된 시료를, 신호 발생 수단이 결합된 약물 및 링커가 결합된 약물을 포함하는 건조 형태의 조성물과 접촉시켜 약물-항체-약물의 표지된 면역 복합체를 형성하는 단계; 및 (c) 상기 표지된 면역 복합체에서 발생하는 신호를 검출하는 단계를 포함하는, 항 약물 항체(anti-drug antibody)의 신속 측정 방법을 제공한다.
본 발명에서 약물이란, 사람이나 다른 생물체에서 유래된 것을 원료 또는 재료로 제조한 의약품으로 특정 질병에 대한 예방, 치료, 개선, 지연 등의 효과를 갖는 생물학적 제제(biological agents)를 모두 포함한다. 본 발명에서의 약물의 예로서는, 종양 괴사 인자 알파(tumor necrosis factor α, TNF-α) 억제제, 예를 들면, 에타너셉트(Etanercept), 인플릭시맙(Infliximab), 아달리무맙(Adalimumab), 골리무맙(Golimumab), 세톨리주맙 페골(Certolizumab Pegol); B 세포 표면 단백질(CD20) 결합 단클론항체 예를 들면, 리툭시맙(Rituximab); 혈관내피성장인자(VEGF) 억제제, 예를 들면 베바시주맙(Bevacizumab); 인간 표피성장인자 수용체 type 2 (HER2) 억제제, 예를 들면, 트라스투주맙(Trastuzumab); 인테그린 α4β7 억제제, 예를 들면 베돌리주맙(Vedolizumab); 인터루킨 12 억제제, 예를 들면 우스테키누맙(Ustekinumab); 인터루킨 17A 억제제, 예를 들면 세쿠키누맙(Secukinumab); 인터루킨 6 억제제, 예를 들면 토실리주맙 (Tocilizumab); 인간 IgE 억제제, 예를 들면 오말리주맙(Omalizumab) 등을 들 수 있다.
본 발명에서 항 약물 항체(anti-drug antibody)란, 상기 나열된 약물에 대해 특이적으로 반응하는 모든 인간 항체를 포함한다. 본 발명에서의 항 약물 항체의 예로서는, 항-에타너셉트 항체, 항-인플릭시맙 항체, 항-아달리무맙 항체, 항-골리무맙 항체, 항 리툭시맙 항체, 항 베바시주맙 항체, 항 트라스투주맙 항체 등 상기 나열된 약물에 대한 항체를 모두 포함할 수 있다.
본 발명에서 생물학적 시료란, 사람으로부터 채취한 것으로, 액체상 또는 액체와 유사한 유동성 물질, 예를 들면 혈액, 혈구, 림프액, 타액, 소변, 땀, 간질 또는 세포내 체액, 또는 이로부터 추출된 물질 등을 들 수 있고, 혈액은 전혈, 혈장, 혈청이나 소정의 처리(예를 들면 응고 방지)가 이루어진 혈액, 혈장, 혈청 등이 이에 해당될 수 있으나, 이에 제한되지 않으며, 상기 생물학적 시료는 조작하거나 조작하지 않은 상태로 사용될 수 있다.
본 발명에서 링커란, 항체와 약물을 결합시킬 수 있는 화합물을 의미하며, 스트렙트아비딘, 뉴트라아비딘, 아비딘, 비오틴, 단백질 A/G, 글루타티온, 히스티딘 작용기를 포함하는 단백질 및 항 히스티딘 항체를 포함하는 군으로부터 선택된 어느 1종 이상일 수 있다.
본 발명에서 신호 발생 수단이란, 항원, 항체, 또는 단백질에 결합하여 신호를 발생시키는 수단으로서, HRP(Horseradish peroxidase), ALP(Alkaline phosphatase), GO(Glucose oxidase) 등의 효소 또는 효소 단백질; xanthene 유도체, cyanine 유도체, squaraine 유도체, naphthalene 유도체, coumarin 유도체, oxidazole 유도체, anthracene 유도체, arylmethine 유도체, dipyrromethene 유도체 등의 유기물 유도체 형광 염료; CdS/ZnS Quantum dot, CdSe Quantum dot, CdSe/ZnS Quantum dot, CdSSe/ZnS Quantum dot, CdTe/CdSe/ZnS Quantum dot, CdS Quantum dot, ZnCdSe/ZnS Quantum dot, CdTe Quantum dot, Eu, Tb 등의 무기물 유도체 형광 염료; 형광 염료 등이 착색된 라텍스, 폴리스타이렌 등의 미소입자; 및 Au, Ag, Fe, Pt, Co 또는 이들의 조합으로 구성된 나노입자를 포함할 수 있으나, 이에 제한되지 않는다.
본 발명에서 건조 형태의 조성물은, 신호 발생 수단이 결합된 약물 및 링커가 결합된 약물 외에 추가로, 첨가제로서 안정화제, 완충제 및 계면활성제를 포함할 수 있다. 상기 건조 형태의 조성물은, 동결 건조 또는 진공 건조에 의해 제조될 수 있으며, 분말 또는 과립 형태의 조성물을 포함한다.
상기 안정화제는 건조 형태의 조성물을 제조하는 동안 약물을 화학적 및/또는 물리적 분해로부터 보호하는 약학적으로 허용되는 부형제이다. 안정화제는 예를 들면, 사카라이드, 아미노산(예를 들어 L-아르기닌, L-글루탐산, L-이소루이신), 폴리올(예를 들어 만니톨, 솔비톨, 자일리톨, 덱스트란, 글리세롤, 아라비톨, 프로필렌 글리콜), 사이클로덱스트린(예를 들어 하이드록시프로필-β사이클로덱스트린, 설포부틸에틸-β사이클로덱스트린, β사이클로덱스트린), 폴리에틸렌글리콜(예를 들어 PEG 3000, PEG 3350, PEG 4000, PEG 6000, PEG 8000, PEG 10,000), 폴리비닐알코올, 폴리비닐피롤리돈, 알부민(예를 들어 인간 혈청 알부민(HSA), 소 혈청 알부민(BSA), 오브알부민(Ovalbumin)) 및 염(예를 들어 나트륨 클로라이드, 마그네슘 클로라이드, 칼슘 클로라이드)을 포함한다.
상기 사카라이드는 모노사카라이드 및 올리고사카라이드를 포함한다. 모노사카라이드는 산에 의해 가수분해가 불가능한 단량체성 탄수화물, 예컨대 단당류 및 이의 유도체, 예를 들어 아미노당이다. 모노사카라이드의 예는 글루코스, 락토오스, 프럭토스, 갈락토스, 만노스, 솔보스, 데옥시리보스 및 뉴리민산을 포함한다. 올리고사카라이드는 글리코시드 결합을 통해 연결된 하나 초과의 단량체성 사카라이드 단위로 이루어진 분지쇄 또는 직쇄 탄수화물이다. 올리고사카라이드 내의 단량체성 사카라이드 단위는 동일하거나 상이할 수 있다. 단량체성 사카라이드 단위의 개수에 따라, 올리고사아카이드는 다이-, 트라이-, 테트라- 및 펜타- 등의 사카라이드일 수 있다. 올리고사카라이드의 예는 수크로스, 트레할로스, 락토스, 말토스 및 라피노스를 포함한다.
상기 완충제는 약학 제제의 pH를 안정화시키는 약학적으로 허용되는 염이다. 완충제는 예를 들면, 시트레이트 염, 아세테이트 염, 히스티딘 염, 숙시네이트 염, 말레이트 염, 포스페이트 염 또는 락테이트 염이 사용될 수 있으며, 구체적으로는, Tris, HEPBS, HEPES, BIS-TRIS 프로판, phosphate, alkaline borate, Glycine, CAPSO, CHES, AMPSO, TABS, AMPD, TAPS, BICINE, Tricine, HEPPSO, POPSO, Gly-Gly를 포함한다.
상기 계면활성제는 약학적으로 허용되는 계면활성제이다. 계면활성제는, 예를 들면 폴리옥시에틸렌-솔비탄 지방산 에스테르(트윈(Tween)), 폴리에틸렌 알킬 에테르(브리즈(Brij)), 알킬페닐폴리옥시에틸렌 에테르(트라이톤 엑스(Triton X)), 폴리옥시에틸렌-폴리옥시프로필렌 공중합체(폴록사머(Poloxamer) 및 플루로닉(Pluronic)), 옥틸페녹시폴리에톡시에탄올(IGEPAL CA-630) 및 CHAPS를 포함한다. 바람직한 폴리옥시에틸렌-솔비탄 지방산 에스테르는 폴리솔베이트 20(폴리옥시에틸렌 솔비탄 모노라우레이트, 트윈 20) 및 폴리솔베이트 20(폴리옥시에틸렌 솔비탄 모노올리에이트, 트윈 80)이다. 바람직한 폴리에틸렌-폴리프로필렌 공중합체는 플루로닉 F68 또는 폴록사머 188으로 시판되는 것들이다. 바람직한 폴리옥시에틸렌 알킬 에테르는 브리즈(상표명)로 시판되는 것들이다. 바람직한 알킬페닐폴리옥시에틸렌 에테르는 트라이톤 엑스로 시판되고, p-tert-옥틸페녹시 폴리에톡시에탄올(트라이톤 엑스 100)이 바람직하다
본 발명은 치료 약물을 투여받는 환자에게 나타나는 항 약물 항체의 신속 정량 검출 방법을 제공한다. 본 발명은 면역 복합체의 면역학적 분석 방법을 쉽게 제공할 수 있는 측방 유동 면역 분석(Lateral flow assay)을 기반으로 한다. 본 발명은 샌드위치 유형(sandwich type) 또는 이중 항원 가교 면역 분석(double antigen bridging immunogenicity assay)을 사용하여 샘플 내 복합체의 면역학적 결정을 위한 방법을 제공한다.
도 1은 본 발명의 일 실시예에 따른 항 약물 항체 검출 분석의 개략도이다. 도 1에 도시된 바와 같이, 본 실시예에 따른 정량 검출 방법은 해리, 중화, 검출 단계로 구성된다. 도 2(a)는 본 발명의 일 실시예에 따른 측방 유동 카트리지에 시료를 인가하는 것을 도시하고, 도 2(b)와 도 2(c)는 검출된 형광 신호의 세기를 나타낸다. 측방 유동 카트리지(200)는 시료 패드(201), 컨트롤 라인(202), 테스트 라인(204), 흡수 패드(205), 다공성 멤브레인(206)을 포함한다.
1 단계: 해리 단계
약물을 투여한 환자로부터, 생물학적 시료를 채취한다. 도 1(a)에 도시된 바와 같이, 채취한 생물학적 시료에 존재하는 항 약물 항체는, 약물과 결합되지 않은 유리 형태(free form) 또는 약물과 결합된 약물 결합 형태(bound form)로 존재한다. 생물학적 시료 내 항 약물 항체의 정확한 수준을 측정하기 위해서는, 도 1(b)에 도시된 바와 같이 약물 결합 형태의 항 약물 항체를 유리 형태의 항 약물 항체로 해리시켜야 한다. 해리 단계에서는, 약물 결합 형태의 항 약물 항체를 해리시키기에 충분한 양의 산을 포함하는 해리 완충제와 생물학적 시료를 접촉시킨다. 여기서 산은 유기산 또는 무기산을 포함한다. 해리 단계는 10분 이하의 시간이 소요되며, 바람직하게는 5분 이하, 더욱 바람직하게는 2분 이하의 시간이 소요된다.
기존 ELISA 방법의 경우, 선호되는 시료 유형은 인간 혈장 또는 인간 혈청이고, 인간 전혈로 수행하면 일관되지 않은 결과를 얻을 수 있으나, 본 발명에서는 인간 혈장, 인간 혈청뿐만 아니라, 인간 전혈을 사용해도 일관된 결과를 얻을 수 있다.
2 단계: 중화 단계
1 단계에서 상기 복합체가 해리된 시료를, 신호 발생 수단이 결합된 약물 및 링커가 결합된 약물을 포함하는 건조 형태의 조성물과 접촉시켜 도 1(c)에 도시된 바와 같이 약물-항체-약물의 표지된 면역 복합체를 형성한다. 상기 건조 형태의 조성물은, 완충제, 신호 발생 수단이 결합된 약물 및 링커가 결합된 약물을 모두 혼합한 후 건조 형태의 조성물로 제조할 수도 있고, 각각 건조 형태의 조성물로 제조한 후 혼합할 수도 있다. 상기 완충제는 완충제의 용해도 및 단백질 안정성을 향상시키는 데 도움이 되는 첨가제를 추가로 포함할 수 있다. 중화 단계는 20분 이하의 시간이 소요되며, 바람직하게는 10분 이하, 더욱 바람직하게는 7분 이하의 시간이 소요된다.
기존의 일반적인 ELISA 방식에서는, 신호 발생 수단이 결합된 약물, 링커가 결합된 약물 및 완충제를 혼합하는 추가 작업 및 각각의 약물 및 완충제를 보관하는 용기가 추가적으로 더 필요했다. 이에 비해, 본 발명에 있어서는, 건조 형태의 조성물을 사용함으로써, 신호 발생 수단이 결합된 약물, 링커가 결합된 약물 및 완충제를 하나의 보관 용기에 함께 저장할 수 있게 되어, 작업 단계가 감소하여 전체 수행 시간이 줄어들 뿐만 아니라, 보관 용기가 적게 사용되는 이점이 있다.
또한, 본 발명에서는, 중화 단계에서의 소요 시간을 줄이기 위해, 신호 발생 수단으로서 나노 입자를 사용한다. 나노 입자는 표면적이 넓어서 많은 약물과 결합시킬 수 있고, 이로 인해 면역 반응성 및 민감도를 높일 수 있다. 또한 많은 약물과 결합하여 분자 크기가 커지고, 그 운동성(이동성)이 증가하여 반응 속도를 높일 수 있다.
본 발명은 또한, 종래 ELISA 방식의 경우 반응 또는 배양 부피가 300㎕ 이상인 반면, 본 발명의 경우 배양 부피를 200㎕ 이하로 줄이고 배양액의 농도를 높여 면역 반응 kinetics를 높임으로써 샘플 내 존재하는 타겟 약물 외의 다른 내제 약물의 간섭을 회피할 수 있도록 짧은 시간내에 중화 과정을 유도하였다. 상기 배양 부피는 200㎕ 이하, 바람직하게는 150㎕ 이하, 보다 바람직하게는 100㎕ 이하일 수 있다.
본 발명은 기존 방법과 달리, 중화 및 반응단계에서 분석 시약의 건조화를 통해 시약 간 혼합 단계를 줄이고, 분석 시약의 농도를 높이며 반응 시약 부피를 줄이고, 신호 발생 수단으로서 입자를 사용함으로써 반응 민감도 및 반응 속도를 향상시켜 중화 단계에서의 소요 시간을 최소화함으로써 궁극적인 목표인 샘플 내 존재하는 타겟 약물 외의 다른 내제 약물의 간섭 영향을 회피하였다.
3 단계: 검출 단계
2 단계에서 형성된 면역 복합체를 포함하는 시료를 도 1(d)에 도시된 바와 같이 측방 유동 카트리지에 투입한다. 측방 유동 카트리지는 공지의 것을 사용할 수 있다. 측방 유동 카트리지에 포함되는 링커가 고정된 테스트 라인에서 검출되는 신호를 기반으로, 미리 만들어진 수식을 사용하여 검량 곡선을 작성하여 정량 분석한다. 검출 단계는 20분 이하의 시간이 소요되며, 바람직하게는 15분 이하, 더욱 바람직하게는 12분 이하의 시간이 소요된다.
상기 일련의 단계는 사용자가 직접 피펫 등을 이용하여 각 단계에서 형성된 시료를 다음 단계로 옮길 수도 있고, 자동으로 시료를 다음 단계로 옮겨주는 장치 또는 도구를 사용할 수도 있다. 바람직하게는, 상기 일련의 단계는 바디텍메드의 AFIAS 시리즈를 이용하여 자동으로 수행될 수 있다.
본 발명은 기존 ELISA 방식에 비해 매우 간단하고 단계 수가 적다. 해리, 컨쥬게이트 제조, 중화, 여러 배양 단계, 검출 등 최소 5~6단계를 포함하는 기존 ELISA 방식과 다르게, 본 발명은 해리, 중화, 검출의 3단계를 사용하여 항 약물 항체의 양을 측정할 수 있다(도 3 참조). 상기와 같이, 본 발명은 운영 단계의 최소화로 인해 기존 ELISA 방식보다 훨씬 적은 시간이 소요된다. 작업 완료 및 결과 생성을 위해 기존 ELISA 방식은 3~12시간이 소요되지만, 본 발명은 0.3시간 미만이 소요된다(도 4 참조). 본 발명은 측방 유동 기술을 기반으로 하기 때문에 ELISA 방식에서 통상적으로 수반되는 워싱(washing) 작업이 필요하지 않다. 기존 ELISA 키트의 경우, 워싱 단계에서 재현 가능한 결과를 생성하기 위해 특별한 주의와 많은 시간이 필요하다. 또한, 결과 분석 및 정량화를 위해 기존의 방법은 특별한 계산이 필요하지만, 본 방법에서는 미리 만들어진 공식을 이용하여 정량화가 자동으로 수행될 수 있다.
이하, 첨부된 도면을 참조하여 본 발명의 실시예를 상세히 설명한다. 단, 하기 실시예는 본 발명을 예시하는 것일 뿐, 본 발명이 하기 실시예에 의해 한정되는 것은 아니다.
특히, 하기 실시예에서는 인플릭시맙 및 항 인플릭시맙을 사용하여 항 약물 항체 및 약물 면역 복합체를 검출하는 데 사용되는 세부 절차를 설명한다. 본 발명은 유사한 방식으로 다른 항 약물 항체도 검출할 수 있다. 따라서, 본 발명은 결코 하기 실시예에 제한되지 않는다.
실시예
1. 시약의 제조
1-1. 해리 완충제
해리 완충제는, pH 1.92의 0.1M 글라이신이다.
1-2. 중화 단계에서 사용하는 건조 형태의 조성물
10% Tween-20, 5% BSA, 1% poloxamer F68을 포함하는 1M Tris-HCl 완충제를 pH 8.5로 조절한다. 제조된 완충제에 500mM 아르기닌과 150mM MgCl2를 첨가하여 중화 완충제를 제조한다. 제조된 중화 완충제는 튜브에 30㎕ 씩 소량 분주하여 4일 동안 동결 건조시킨다.
50mg/mL NHS(n-hydroxysuccinimide)와 10mg/mL EDC(1-(3-dimethylaminopropyl)-3-ethylcarbodiimide hydrochloride)를 미리 DDW(double distilled water)에 녹인 후 각각 230㎕, 239㎕를 유로피움 나노 입자 100㎕가 포함된 MES(2-(N-morpholino)ethane Sulfonic acid)) 버퍼(pH 6.1)에 섞어 1mL의 혼합액을 제조한다. 이 혼합액을 30분간 상온에서 900rpm으로 혼합한 후 15분간 14,000rpm에서 원심 분리하여 상층액을 버린 후 MES를 첨가하고 초음파로 침전물을 분산시킨다. 0.5mg/mL의 인플릭시맙 항체 400㎕를 침전물이 분산된 용액에 혼합하여 항체가 나노 입자에 결합되도록 1시간 동안 실온에서 교반한다. 원심 분리 과정을 상기와 동일하게 3회 반복한 후 회수된 침전물에 1% BSA가 포함된 50mM Tris-HCl 완충제를 첨가하여 분산시켜 신호 발생 수단이 결합된 약물을 포함하는 시약을 제조한다.
인플릭시맙 항체에 0.1M NaHCO3를 녹인 후 적당량의 Biotin-NHS(N-hydroxysuccinimide) 에스테르를 첨가하여 반응시킨다. 반응이 완료된 용액은 크기 배제 크로마토그래피를 사용하여 링커가 결합된 약물을 포함하는 시약을 제조한다.
상기와 같이 제조된 신호 발생 수단이 결합된 약물을 포함하는 시약과 링커가 결합된 약물을 포함하는 시약 각각을 5% BSA, 3% Sucrose, 1% PEG, 0.1% PVA가 포함된 Tris-HCl 완충제에 희석한 후 과립 분주기와 동결 건조기를 이용하여 건조 형태의 과립을 제조한다.
제조된 건조 형태의 과립과 상기 중화 완충제를 건조시킨 조성물을 튜브에 넣고 혼합하여 건조 형태의 조성물을 제조한다.
2. 측방 유동 멤브레인의 제조
ChIgY를 함유하는 컨트롤 라인(202)과 스트렙트아비딘을 함유하는 테스트 라인(204)을 포함하는 측방 유동 멤브레인(206)을 제조한다.
3. 샘플의 제조
i) 참조 샘플(유리 형태)
농도 1mg/mL의 항 인플릭시맙 항체(Biorad)를 인간 혈장(건강한 사람의 것)으로 연속 희석하여 농도 0ng/mL, 10ng/mL, 50ng/mL 및 250ng/mL의 항 인플릭시맙 항체 용액을 제조했다.
ii) 참조 샘플 (복합체)
농도 1mg/mlL 항 인플릭시맙 항체(Biorad)를 인간 혈장(건강한 사람의 것)으로 연속 희석하여 농도 0ng/mL, 10ng/mL, 50ng/mL 및 250ng/mL의 2배 농도로 항 인플릭시맙 항체 용액을 제조한 후 100㎍/ml의 인플릭시맙과 함께 1:1 비율(v/v)로 혼합하여 50㎍/mL의 인플릭시맙이 포함된 0ng/mL, 10ng/mL, 50ng/mL 및 250ng/mL의 항 인플릭시맙 항체 용액을 제조한다.
4. 면역 분석
위 단계에서 준비한 해리 완충제을 카트리지 웰에 첨가하고, 건조 형태의 조성물이 포함된 튜브를 카트리지의 홀더에 삽입, 고정한다. 샘플을 샘플 웰에 첨가한 후 이후 절차는 자동으로 수행되었다.
(자동 수행 과정) 생물학적 시료 50㎕를 300㎕의 해리 완충제를 포함하는 해리 완충제 웰에 첨가하여 잘 혼합하고, 2분 동안 기다린다. 해리 반응이 완료된 후 해리된 시료의 150㎕를 다음 웰(건조 형태의 조성물을 포함하는 웰)로 옮겨 잘 혼합하고 5분 동안 기다린다. 인큐베이션 후 75㎕의 혼합물을 측방 유동 멤브레인(206)으로 이동시키고, 12분 후 테스트 라인(204)에서 검출되는 신호를 분석하여 최종 결과를 얻었고, 그 결과를 도 5 및 6에 나타낸다.
도 5는 본 발명의 일 실시예의 결과를 나타낸 것으로, 신호에 따른 항 약물 항체(항 인플릭시맙)의 농도(도 5(a) 참조)와 이를 나타낸 검량 곡선(도 5(b) 참조)이다. 도 6은 본 발명의 일 실시예의 결과를 나타낸 것으로, 항 약물 항체(항 인플릭시맙)의 유리 형태 또는 결합된 형태에 따른 리커버리(회수율)를 비교한 값(도 6(a) 참조)와 그래프(도 6(b) 참조)이다. 도 5 및 6에 나타낸 ratio는 테스트 라인(204)에서 검출된 신호의 세기를 컨트롤 라인(202)에서 검출된 신호의 세기로 나눈 값을 나타낸 것이다.
본 발명은 그 바람직한 실시예를 참조하여 구체적으로 도시되고 설명되었지만, 첨부된 청구 범위에 포함되는 본 발명의 범위를 벗어나지 않으면서 형태 및 세부 사항에서의 다양한 변화가 이루어질 수 있음이 통상의 기술자에 의해 이해될 것이다. 또한, 여기에 설명된 실시예들은 상호 배타적인 것이 아니며, 다양한 실시예들의 특징이 본 발명에 따라 전체적으로 또는 부분적으로 조합될 수 있다는 것을 이해해야 한다.
Claims (4)
- 하기의 단계를 포함하는, 항 약물 항체(anti-drug antibody)의 신속 검출 방법:(a) 항 약물 항체와 약물의 복합체를 포함하는 생물학적 시료를 산과 접촉시켜 상기 항 약물 항체와 약물의 복합체를 해리시키는 단계;(b) 상기 복합체가 해리된 시료를, 신호 발생 수단이 결합된 약물 및 링커가 결합된 약물을 포함하는 건조형태의 조성물과 접촉시켜 약물-항체-약물의 표지된 면역 복합체를 형성하는 단계;(c) 상기 표지된 면역 복합체에서 발생하는 신호를 검출하는 단계.
- 제1항에 있어서,상기 건조 형태의 조성물은 추가로, 안정화제, 완충제 및 계면활성제를 포함하는, 항 약물 항체의 신속 측정 방법.
- 제2항에 있어서,상기 복합체가 해리된 시료와 상기 건조 형태의 조성물의 총 부피는 200㎕ 이하인, 항 약물 항체의 신속 측정 방법.
- 제1항 내지 제3항 중 어느 한 항에 있어서,상기 표지된 면역 복합체에서 발생하는 신호를 검출하는 단계는, 표지된 면역 복합체를 링커가 고정된 테스트 라인을 포함하는 측방 유동 카트리지에 로딩하는 것을 포함하는, 항 약물 항체(anti-drug antibody)의 신속 측정 방법.
