본 발명은 대장암 진단에 정보를 제공하는 방법, 대장암 진단용 조성물 및 이를 포함하는 키트에 관한 것으로, 더욱 상세하게는 메틸화된 복수의 대장암 마커 유전자를 특이적으로 증폭하는 프라이머를 이용하여, 대장암 진단에 정보를 제공하는 방법, 대장암 진단용 조성물 및 이를 포함하는 키트에 관한 것이다.
포유류 세포의 게놈 DNA에는 A, C, G, T 외에 5번째 염기가 존재하며, 이는 시토신 환의 5번째 탄소에 메틸기가 붙은 5-메틸시토신(5-mC)이다. 5-mC는 항상 CG 다이뉴클레오타이드의 C에만 오며(5'-mCG-3'), 이러한 CG를 흔히 CpG라고 표시한다. CpG의 C는 대부분이 메틸기가 붙어서 메틸화되어 있다. 이러한 CpG의 메틸화는 알루(alu)나 전이인자(transposon)와 같이 게놈내에 반복되는 염기서열(repetitive sequence)이 발현되지 못하도록 억제하며, 포유류 세포에서 유전자 외 변화가 가장 흔히 나타나는 부위이다. 이러한 CpG의 5-mC는 자연히 탈아미노화(deamination)되어 T로 바뀌며, 이에 따라 포유류 게놈내 CpG는 정상적으로 나타나야 할 빈도(1/4 x 1/4 = 6.25%)보다 훨씬 낮은 1%의 빈도만을 나타낸다.
CpG 중에 예외적으로 밀집되어 나타나는 것들이 있으며, 이를 CpG 부위 (CpG 섬)라고 한다. CpG 부위는 길이가 0.2~3kb이고, C 및 G염기의 분포백분율이 50%를 넘으며, CpG의 분포백분율이 3.75%이상으로 높게 집중되어 나타나는 부위를 가리킨다. CpG 부위는 전체 인체 유전체에 약 45,000개가 나타나며, 특히 유전자의 발현을 조절하는 프로모터 부위에 집중되어 나타난다. 실제로 인체 유전자 중 약 절반을 차지하는 중요 유전자(housekeeping genes)의 프로모터에는 CpG 부위가 나타난다 (Cross, S. et al., Curr. Opin. Gene Develop., 5:309, 1995). 비정상적인 DNA 메틸화는 주로 해당 유전자의 5’ 발현조절 부위 (5’ regulatory region)에서 일어나 해당 유전자의 발현을 감소시키는 것으로 알려져 있다.
한편, 정상인의 체세포(somatic cell)에서는 이들 중요 유전자 프로모터 부위의 CpG 섬이 메틸화되어 있지 않으나, 발생 중에 발현되지 않도록 각인된(imprinted) 유전자와 비활성화(inactivation)된 X 염색체상의 유전자들은 메틸화되어 있다.
발암과정 중에는 프로모터 CpG 섬에 메틸화가 나타나며, 그 해당 유전자의 발현에 장애가 나타나게 된다. 특히, 세포주기나 고사를 조절하고, DNA를 복구하며 세포의 부착과 세포간 상호협조 작용에 관여하고, 침윤과 전이를 억제하는 종양억제 유전자들의 발현조절 부위 CpG 섬에 메틸화가 발생하는 경우, 이는 코딩서열의 돌연변이와 동일하게 이들 유전자의 발현과 기능을 차단하며, 그 결과 암의 발생과 진행이 촉진된다. 그 외에도 노화에 따라 CpG 섬에 부분적으로 메틸화가 나타나기도 한다.
종양 관련 유전자의 발현 조절부위 메틸화가 암의 중요한 지표이며, 따라서 이는 암의 진단과 조기진단, 발암위험의 예측, 암의 예후 예측, 치료 후 추적조사, 항암요법에 대한 반응 예측 등 다방면으로 이용될 수 있다는 것이다. 실제 혈액이나 객담, 침, 대변, 소변 등에서 종양관련 유전자의 프로모터 메틸화를 조사하여 각종 암 진료에 사용하려는 시도가 최근 활발하게 이루어지고 있다 (Ahlquist, D.A. et al., Gastroenterol., 119:1219, 2000).
이러한 기술적 배경하에서, 본 출원의 발명자들은 복수의 메틸화된 대장암 마커 유전자를 특이적으로 증폭하는 프라이머를 이용하여 대장암 마커 유전자의 메틸화를 높은 검출한도와 정확도로 검출할 수 있음을 확인하고, 본 발명을 완성하게 되었다.
발명의 요약

발명의 상세한 설명 및 구체적인 구현예
다른 식으로 정의되지 않는 한, 본 명세서에서 사용된 모든 기술적 및 과학적 용어들은 본 발명이 속하는 기술분야에서 숙련된 전문가에 의해서 통상적으로 이해되는 것과 동일한 의미를 갖는다. 일반적으로 본 명세서에서 사용된 명명법은 본 기술분야에서 잘 알려져 있고 통상적으로 사용되는 것이다.
본 출원의 발명자들은 복수의 메틸화된 대장암 마커 유전자를 특이적으로 증폭하는 프라이머를 통해, 단일 대장암 마커 유전자에 기반한 메틸화 검출도에 비해 메틸화 DNA 검출의 민감도 및 특이도가 향상될 수 있음을 확인하였다. 본 발명에 따른 구체적 실시예에서는 SDC2 유전자의 메틸화 검출에 비해, SDC2 유전자와 ADHFE1 유전자의 메틸화를 검출함으로써, 대장암 진단에 대한 민감도 및 특이도가 향상되어 대장암 진단에 유용성이 높음을 확인하였다.
따라서, 본 발명은 일 관점에서 다음의 단계를 포함하는 대장암 진단에 정보를 제공하는 방법에 관한 것이다: (a) 메틸화된 SDC2 (Syndecan 2) 유전자와 ADHFE1 (Alcohol Dehydrogenase Iron Containing 1) 유전자, 비메틸화된 SDC2 유전자와 ADHFE1 유전자를 서로 다르게 변형시키는 하나 이상의 시약으로 샘플을 처리하는 단계; 및 (b) 메틸화된 SDC2 유전자와 ADHFE1 유전자를 특이적으로 증폭하는 프라이머를 처리하는 단계.

검체로부터 분리된 핵산은 검체의 생물학적 시료에 의하여 얻어진다. 대장암이나 대장암의 진행 단계를 진단하고 싶다면, 스크랩이나 생검으로 대장 조직에서 핵산을 분리하여야 한다. 이러한 시료는 당해 분야에서 알려진 여러 의학적 과정에 의하여 얻어질 수 있다.
상기 검체로부터 얻어진 샘플의 핵산의 메틸화 정도는 대장 조직의 세포 성장성 이상이 없는 검체의 동일한 핵산 부분과 비교하여 측정한다. 하이퍼메틸화는 하나 이상의 핵산에서 메틸화된 대립유전자가 존재하는 것을 의미한다. 대장 조직의 세포 성장성 이상이 없는 검체는 동일한 핵산을 검사했을 때, 메틸화 대립유전자가 나타나지 않는다.
"정상" 세포는 비정상적 세포 형태 또는 세포학적 성질의 변화를 나타내지 않은 세포를 의미한다. "종양"세포는 암 세포를 의미하고, "비종양" 세포는 병증 조직의 일부이지만, 종양 부위는 아니라고 판단되는 세포를 의미한다.
본 발명에 따르면, 검체에서 분리된 하나 이상의 핵산의 메틸화 단계를 결정하여 검체의 대장 조직의 세포 성장성 이상을 조기 진단할 수 있다. 상기 하나 이상의 핵산의 메틸화 단계는 대장 조직의 세포 성장성 이상을 가지고 있지 않은 검체로부터 분리된 하나 이상의 핵산의 메틸화 상태와 비교하는 것을 특징으로 할 수 있다. 핵산은 CpG 섬과 같은 CpG-함유 핵산인 것이 바람직하다.
본 발명에 따르면, 검체로부터 분리된 하나 이상의 핵산의 메틸화를 결정하는 것을 포함하는 검체의 대장 조직의 세포 성장성 이상 소양을 진단할 수 있다. 상기 하나 이상의 핵산의 메틸화 단계는 대장 조직의 세포 성장성 이상에 대한 소양을 가지고 있지 않은 검체로부터 분리된 하나 이상의 핵산의 메틸화 상태와 비교하는 것을 특징으로 할 수 있다.
"소양"은 상기 세포 성장성 이상에 걸리기 쉬운 성질을 의미한다. 소양을 가진 검체는 아직은 세포 성장성 이상을 가지고 있지 않지만, 세포 성장성 이상이 존재하거나 존재할 경향이 증가된 검체를 말한다.
상기 타겟 DNA 중 CpG 메틸화의 존재가 질병의 지표일 수 있으며, 예를 들어 타겟 DNA의 프로모터, 5' 비번역 영역 및 인트론 중 어느 한 부위의 CpG 메틸화를 측정할 수 있다.
CpG-함유 유전자는 통상적으로 DNA이다. 그러나, 본 발명의 방법은 예를 들면, DNA 또는 DNA와 mRNA를 포함하는 RNA를 함유하는 시료를 적용할 수 있고, 여기서 DNA 또는 RNA는 단일가닥 또는 이중가닥일 수 있으며, 또는 DNA-RNA 하이브리드를 함유한 시료인 것을 특징으로 할 수 있다.
핵산 혼합물 또한 사용될 수 있다. 본 발명에서 사용되는 “다중”은 일종의 유전자 내 검출될 특이적인 핵산 서열 부위가 복수개인 경우와 하나의 튜브 (단일 반응기) 내 복수의 타겟 DNA를 포함하는 경우를 모두 포함하는 것이다. 검출될 특이적인 핵산 서열은 큰 분자의 분획일 수 있고, 처음부터 특이 서열이 전체 핵산 서열을 구성하는 분리된 분자 형태로 존재할 수 있다. 상기 핵산 서열은 순수한 형태로 존재하는 핵산일 필요는 없으며, 핵산은 전체 인간 DNA가 포함되어 있는 것과 같이 복잡한 혼합물 내의 적은 분획일 수도 있다.
구체적으로, 본 발명은 단일 반응기 내 샘플 중 복수의 타겟 DNA의 메틸화를 검출하기 위한 것으로, 상기 샘플은 복수의 다중 타겟 DNA를 포함할 수 있으며, 타겟 DNA는 대조군 유전자 뿐 아니라, 비정상적으로 메틸화되어 발현이 억제될 억제될 경우, 대장암의 발생 또는 진행에 영향을 주는 유전자라면 제한없이 이용 가능하다.
본 발명에 있어서, 상기 샘플은 인체로부터 유래한 것을 특징으로 할 수 있고, 예를 들어 상기 샘플은 대장암 조직, 세포, 대변, 소변, 혈액, 혈청 또는 플라즈마를 사용할 수 있다.
상기 메틸화된 DNA와 비메틸화된 DNA를 서로 다르게 변형시키는 하나 이상의 시약은 비메틸화된 사이토신 염기와 메틸화된 사이토신 염기를 구별할 수 있는 것이면 제한없이 사용 가능하나, 예를 들어 바이설파이트, 하이드로젠 설파이트, 다이설파이트 및 이들의 조합일 수 있으며, 이에 한정되는 것은 아니다. 구체적으로, 상기 시약에 의하여 메틸화된 사이토신 염기는 변환되지 않고, 비메틸화된 사이토신 염기는 우라실 또는 사이토신 이외의 염기로 변환될 수 있다.
본 발명에서 단계 (b)는 메틸화된 SDC2 유전자와 메틸화된 ADHFE1 유전자를 특이적으로 증폭하는 프라이머를 처리하는 단계이다.
상기 프라이머는 예를 들어, PCR을 위해 정방향 및 역방향 프라이머를 쌍으로 만들어서 동시에 사용할 수 있다. 또한 상기 프라이머들의 3’말단에는 반드시 CpG 부위의 메틸화된 “C”가 위치하도록 제작되어 메틸화 검출에 대한 특이성이 높은 특징을 가지게 된다. 상기 메틸화된 ADHFE1 유전자를 특이적으로 증폭하는 정방향 프라이머는 예를 들어, 서열번호 1 또는 4의 서열을 포함할 수 있다. 상기 역방향 프라이머는 예를 들어, 서열번호 2 또는 5의 서열을 포함할 수 있다. 상기 메틸화된 SDC2 유전자를 특이적으로 증폭하는 프라이머는 서열번호 10 또는 11의 서열을 포함할 수 있다.
본 발명은 상기 (b) 단계에서 프라이머에 의해 특이적으로 증폭된 메틸화 SDC2 유전자 및 메틸화 ADHFE1 유전자에 각각 상보적으로 혼성화할 수 있는 프로브를 처리하는 단계를 더 포함할 수 있다.
혼성화 반응에서, 엄격한 특정 수준을 달성하기 위하여 사용되는 조건은 혼성화 되는 핵산의 성질에 따라 다양하다. 예를 들면, 혼성화 되는 핵산 부위의 길이, 상동성 정도, 뉴클레오티드 서열 조성(예를 들면, GC/AT 조성비) 및 핵산 타입(예를 들면, RNA, DNA)등이 혼성화 조건을 선택하는데 고려된다. 추가적인 고려 조건은 핵산이 예를 들면, 필터 등에 고정화되어 있는지의 여부이다.
매우 엄격하게 진행되는 조건의 예를 들면 다음과 같다: 실온의 2X SSC/0.1% SDS(혼성화 조건); 실온의 0.2X SSC/0.1% SDS(엄격성이 낮은 조건); 42℃에서의 0.2X SSC/0.1% SDS(보통의 엄격성을 가지는 조건); 68℃에서 0.1X SSC(높은 엄격성을 가지는 조건). 세척 과정은 이들 중 한가지 조건을 사용하여 수행할 수 있고, 예를 들면 높은 엄격성을 가지는 조건, 또는 상기 조건을 각각 사용할 수 있으며, 상기 기재된 순서대로 각각 10~15분씩, 상기 기재된 조건을 전부 또는 일부 반복하여 수행할 수 있다. 그러나 상기에 기술한 바와 같이, 최적 조건은 포함된 특별한 혼성화 반응에 따라 다양하며, 실험을 통하여 결정할 수 있다. 일반적으로, 중요한 프로브의 혼성화에는 높은 엄격성을 가지는 조건이 사용된다.
상기 증폭된 메틸화 SDC2 유전자에 상보적으로 혼성화할 수 있는 프로브는 예를 들어, 서열번호 3 또는 6의 서열을 포함할 수 있다. 상기 증폭된 메틸화 ADHFE1 유전자에 상보적으로 혼성화할 수 있는 프로브는 서열번호 12의 서열을 포함할 수 있다.
경우에 따라서, 프로브는 검출할 수 있도록 표지되며, 예를 들면 방사선 동위원소, 형광 화합물, 바이오 발광 화합물, 화학 발광 화합물, 금속 킬레이트 또는 효소로 표지될 수 있다. 상기와 같은 프로브를 적당하게 표지하는 것은 당해 분야에서 널리 알려진 기술이며, 통상적인 방법을 통하여 수행할 수 있다.
상기 증폭 산물의 양은 형광신호에 의해 검출할 수 있다. 프로브가 결합된 증폭산물의 이중 나선 DNA에 결합하여 형광을 나타내는 시약(intercalator)을 사용하는 인터컬레이팅(Intercalating)법, 5' 말단은 형광물질, 3' 말단은 소광자(quencher)로 표지된 올리고뉴클레오티드를 사용하는 방법 등이 있다.
본원발명에 따른 증폭은 실-시간 정량 증폭 예를 들어 실시간 중합효소연쇄반응 (Real-Time PCR)을 통해 수행될 수 있으며, 실시간 중합효소연쇄반응에 있어 PCR 증폭 산물의 양은 형광 신호에 의해 검출할 수 있다. 실시간 중합효소연쇄반응이 진행되면서 증가하는 폴리뉴클레오티드 양에 따라 형광 신호의 세기가 증가하게 되고, 증폭 사이클 횟수에 따른 형광 신호 세기를 나타내는 증폭 프로파일(amplification profile) 곡선을 얻게 된다.
증폭 프로파일 곡선은 일반적으로 실질적인 폴리뉴클레오티드 양이 반영되지 않은 배경의 형광신호가 나타나는 베이스라인(baseline) 영역, 폴리뉴클레오티드 생성물량의 증가에 따른 형광신호 증가가 나타나는 지수적 영역(exponential region), 및 PCR 반응이 포화 상태에 이르러 형광신호 세기의 증가가 나타나지 않는 정체 상태 영역(plateau region)으로 나누어 진다.
통상적으로 베이스라인 영역에서 지수적 영역으로 넘어가는 지점, 즉 PCR 증폭 산물량이 형광으로 검출 가능한 양에 도달한 때의 형광신호 세기를 임계값(threshold)이라고 하고 증폭 프로파일 곡선에서 임계값에 대응되는 증폭 사이클 횟수를 임계 사이클(threshold cycle: Ct)값이라고 한다.
상기 Ct값을 측정하고 표준물질에 대한 Ct(threshold cycle)값을 기반으로 농도가 결정된 표준곡선을 분석하여, 증폭된 유전자의 농도를 확인함으로써, 메틸화 특이적 민감도 및/또는 특이도를 결정할 수 있다.
하나의 실시예에서, 상기 메틸화 검출은 PCR, 메틸화 특이 PCR (methylation specific PCR), 실시간 메틸화 특이 PCR (real time methylation specific PCR), 메틸화 DNA 특이적 결합 단백질을 이용한 PCR, 메틸화 DNA 특이적 결합 항체를 이용한 PCR, 정량 PCR, 유전자 칩, 시퀀싱, 시퀀싱 바이 신세시스 및 시퀀싱 바이 라이게이션으로 구성된 군에서 선택되는 방법에 의하여 수행될 수 있다.
(1) 메틸화 특이 PCR (methylation specific PCR): 상기 메틸화 특이 PCR로 검출하는 경우, 바이설파이트를 처리하면 5'-CpG'-3 부위의 시토신이 메틸화된 경우에는 그대로 시토신으로 남아 있고, 비메틸화된 경우에는 우라실로 변하게 된다. 따라서, 바이설파이트 처리 후 변환된 염기서열을 대상으로 5'-CpG-3' 염기서열이 존재하는 부위에 해당하는 프라이머를 제작할 수 있다. 프라이머를 이용하여 PCR을 하면 메틸화된 경우에는 메틸화된 염기서열에 해당되는 프라이머를 사용한 것에서 PCR 산물이 만들어지게 되고, 아가로즈겔 전기영동방법으로 메틸화 여부를 확인할 수 있다. 여기서, 상기 메틸화 검출 프로브는 TaqMan, Molecular Beacon, 자가 리포팅 (self-reporting) 기능을 가지거나 또는 에너지 전이 표지 기능을 가지는 프로브일 수 있으며, 이에 제한되는 것은 아니다.
(2) 실시간 메틸화 특이 PCR (real time methylation specific PCR): 상기 실시간 메틸화 특이 PCR은 메틸화 특이 PCR 방법을 실시간 측정방법으로 전환한 것으로, 지노믹 DNA에 바이설파이트를 처리한 후, 메틸화된 경우에 해당하는 PCR 프라이머를 디자인하고, 이들 프라이머를 이용하여 실시간 PCR을 수행하는 것이다. 이때, 증폭된 염기서열과 상보적인 TaqMan 프로브를 이용하여 검출하는 방법과 Sybergreen을 이용하여 검출하는 두 가지 방법이 있다. 따라서, 실시간 메틸화 특이 PCR은 메틸화된 DNA만을 선택적으로 정량 분석할 수 있다. 이때, in vitro 메틸화 DNA 샘플을 이용하여 표준곡선을 작성하고, 표준화를 위하여 염기 서열 내에 5'-CpG-3' 서열이 없는 유전자를 음성 대조군으로 함께 증폭하여 메틸화 정도를 정량 분석할 수 있다.
(3) 메틸화 DNA 특이적 결합 단백질을 이용한 PCR 또는 정량 PCR 및 DNA 칩: 상기 메틸화 DNA 특이적 결합 단백질을 이용한 PCR 또는 DNA 칩 방법은 메틸화 DNA에만 특이적으로 결합하는 단백질을 DNA와 섞어주게 되면, 메틸화 DNA에만 특이적으로 단백질이 결합하기 때문에 메틸화 DNA만을 선택적으로 분리할 수 있다.
또한, 정량 PCR 방법으로도 메틸화 여부를 측정할 수 있으며, 메틸화 DNA 특이적 결합 단백질로 분리한 메틸화 DNA는 형광 염료로 표지하여 상보적인 프로브가 집적된 DNA칩에 하이브리디제이션시킴으로써 메틸화 여부를 측정할 수 있다.
(4) 차별적 메틸화의 검출-바이설파이트 시퀀싱 방법: 메틸화 CpG를 함유한 핵산을 검출하는 다른 방법은 핵산을 함유한 시료를 비메틸화 시토신을 변형시키는 제제와 접촉시키는 단계 및 CpG-특이적 올리고뉴클레오티드 프라이머를 사용하여 시료의 CpG-함유 핵산을 증폭시키는 단계를 포함한다. 여기서, 상기 올리고뉴클레오티드 프라이머는 변형된 메틸화 및 비메틸화 핵산을 구별하여 메틸화 핵산을 검출하는 것을 특징으로 할 수 있다. 상기 증폭 단계는 선택적이고, 바람직하지만 필수적인 것은 아니다. 상기 방법은 변형된(예를 들면, 화학적으로 변형된) 메틸화 및 비메틸화 DNA를 구별하는 PCR 반응에 의존하는 것이다.
(5) 바이설파이트 시퀀싱 방법: 메틸화 CpG를 함유한 핵산을 검출하는 다른 방법은 핵산을 함유한 시료를 비메틸화 시토신을 변형시키는 제제와 접촉시키는 단계 및 메틸화-비의존적 (Methylation independent) 올리고뉴클레오티드 프라이머를 사용하여 시료의 CpG-함유 핵산을 증폭시키는 단계를 포함한다. 여기서, 상기 올리고뉴클레오티드 프라이머는 변형된 메틸화 및 비메틸화 핵산을 구별하지 않고 핵산을 증폭하는 것을 특징으로 할 수 있다. 상기 증폭된 산물을 시퀀싱 프라이머를 이용하여 Sanger 방법으로 시퀀싱하거나 차세대 시퀀싱 (next generation sequencing) 방법으로 메틸화 핵산의 검출을 위한 바이설파이트 (bisulfite) 시퀀싱과 연관하여 기재되어 있다.
(6) 여기서 차세대 시퀀싱 방법은 Sequencing by synthesis와 Sequencing by ligation 방법으로 하는 것을 특징으로 할 수 있다. 이 방법의 특징은 bacterial clone을 만드는 대신 단일 DNA 단편을 공간적으로 분리하여 in situ로 증폭하고 (clonal amplification), 시퀀싱을 해낸다는 것이다. 이때, 수십 만개의 단편을 동시에 읽어내기 때문에 매시브 페러럴 시퀀싱(massively parallel sequencing) 방법으로 불리기도 한다.
기본적으로는 시퀀싱 바이 신세시스(sequencing by synthesis) 방법이며, 모노 혹은 디뉴클레오티드를 순차적으로 붙여가면서 시그널을 얻는 방법을 사용하는데 파이로시퀀싱, ion torrent, Solexa 방법들이 여기에 해당한다.
시퀀싱 바이 신세시스에 기반하는 NGS 장비로는 로슈(Roche)사의 454 플랫폼, 일루미나(Illumina)사의 HiSeq 플랫폼, 라이프테크놀로지(Life Technology)사의 Ion PGM 플랫폼, 마지막으로 퍼시픽바이오사이언스(Pacific BioSciences)상의 PacBio 플랫폼이 있다. 454와 Ion PGM은 클로날증폭(clonal amplification)방법으로 emersion PCR을 사용하며, HiSeq은 브릿지 증폭(Bridge amplification)을 사용한다. 시퀀싱 바이 신세시스 방법은 한 개의 뉴클레오티드를 순차적으로 붙여가며 DNA를 합성시켜 나갈 때 발생되는 인산(phosphate), 수소이온, 혹은 미리 붙여 놓은 형광을 검출하여 서열을 읽어 나간다. 서열을 검출하는 방법에 있어, 454는 인산을 이용하는 파이로시퀀싱(pyroseqeuncing) 방법을 사용하며, Ion PGM은 수소이온 검출을 이용한다. HiSeq과 PacBio는 형광을 검출하여 서열을 해독한다.
시퀀싱 바이 라이게이션(Sequencing by ligation)은 DNA ligase를 이용하는 시퀀싱 기술로 DNA 염기서열에 존재하는 특정위치의 뉴클레오티드를 확인하는 기술이다. 대부분의 시퀀싱 기술이 중합효소를 사용하는 것과 달리 중합효소를 사용하지 않으며 DNA ligase가 미스매치 서열을 ligation하지 않는 특징을 이용한다. SOLiD 시스템이 여기에 해당한다. 이 기법에서는 간격을 두면서 두 개씩 염기를 읽는데, 프라이머 리셋(primer reset)을 통해 독립적으로 다섯 번을 반복하기 때문에, 최종적으로는 각 염기를 두 번씩 중복하여 읽어서 정확도를 높인다.
시퀀싱 바이 라이게이션(Sequencing by ligation)의 경우, 16개 조합으로 만들어진 디뉴클레오티드 프라이머 셋트 중, 해당 염기서열에 대응되는 디뉴클레오티드 프라이머가 순차적으로 라이게이션되며, 이 라이게이션들의 조합을 최종적으로 분석하여 해당 DNA의 염기서열이 완성된다.
여기서 차세대 시퀀싱 방법은 시퀀싱 바이 신테시스 (Sequencing by synthesis) 또는 시퀀싱 바이 라이게이션 (Sequencing by ligation) 방법으로 하는 것을 특징으로 할 수 있다. 여기서 메틸화 DNA 특이적 결합 단백질은 MBD2bt에 제한되지 않으며, 항체는 5'-메틸-시토신 (5'-methyl-cytosine) 항체로 이에 제한되지 않는다.
본 발명에서 사용되는 프라이머와 관련하여, 상기 단계 (a)에 따라 시약 예를 들어, 바이설파이트를 처리하면 5'-CpG'-3 부위의 시토신이 메틸화된 경우에는 그대로 시토신으로 남아 있고, 비메틸화된 경우에는 우라실로 변하게 된다. 따라서, 시약 예를 들어, 바이설파이트 처리 후 변환된 염기서열을 대상으로 5'-CpG-3' 염기서열이 존재하는 부위에 해당하는 프라이머를 제작할 수 있다.
상기 프라이머는 증폭될 유전자 로커스의 각 가닥과 "대체적으로" 상보성을 가지도록 제작될 수 있다. 이는 중합반응을 수행하는 조건에서 프라이머가 대응하는 핵산 가닥과 하이브리다이제이션 되기에 충분한 상보성을 가지는 것을 의미한다.
본 발명은 다른 관점에서, 메틸화된 Syndecan 2 (SDC2) 유전자와 ADHFE1 유전자, 비메틸화된 SDC2 유전자와 ADHFE1 유전자를 서로 다르게 변형시키는 하나 이상의 시약; 및 상기 메틸화된 SDC2 유전자와 메틸화된 ADHFE1 유전자를 특이적으로 증폭하는 프라이머를 포함하는 대장암 진단용 조성물에 관한 것이다.
본 발명에 따른 조성물에 포함되는 구성은 앞서 설명된 구성과 중첩되므로, 이에 대한 설명은 동일하게 적용된다.
본 발명은 또 다른 관점에서, 상기 조성물을 포함하는 타겟 DNA의 메틸화 검출용 키트에 관한 것이다.
하나의 실시예에서, 상기 키트는 샘플을 담는 구획된 캐리어 수단, 시약을 포함하는 용기, 메틸화 SDC2 유전자 및 메틸화 ADHFE1 유전자 각각을 프라이머를 포함하는 용기를 포함할 수 있다. 경우에 따라서, 상기 메틸화 SDC2 유전자 및 메틸화 ADHFE1 유전자 증폭 산물 각각을 검출하기 위한 프로브를 포함하는 용기를 추가로 포함할 수 있다.
상기 캐리어 수단은 병, 튜브와 같은 하나 이상의 용기를 함유하기에 적합하고, 각 용기는 본 발명의 방법에 사용되는 독립적 구성요소들을 함유한다. 본 발명의 명세서에서, 당해 분야의 통상의 지식을 가진 자는 용기 중의 필요한 제제를 손쉽게 분배할 수 있다.
이하, 실시예를 통하여 본 발명을 더욱 상세히 설명하고자 한다. 이들 실시예는 오로지 본 발명을 예시하기 위한 것으로, 본 발명의 범위가 이들 실시예에 의해 제한되는 것으로 해석되지 않는 것은 당업계에서 통상의 지식을 가진 자에게 있어서 자명할 것이다.
실시예 1. 대장암 특이적 메틸화 유전자 ADHFE1 선별
본 발명자들은 대장암에서 특이적으로 메틸화된 유전자들을 찾기 위하여 12명의 대장암 수술 환자의 암 조직 및 이와 연접하는 정상조직 DNA를 이용한 유전체 수준 메틸화 분석 (MeDIA-CpG microarray)을 수행하여 32종의 대장암에서 메틸화 된 유전자들을 보고한 바 있다 (Oh et al., Journal of Molecular Diagnostics, 2013;15(4):498-507: Supplemental Table 1). 이들 32종의 유전자들 중 대장암 조직에서 메틸화가 많이 된 ADHFE1 (NM_144650, Alcohol dehydrogenase, iron containing, 1)을 대장암 특이적 메틸화 유전자로 선별하였다 (도 1).
실시예 2. ADHFE1 메틸화 검출 프라이머 및 프로브 검증
ADHFE1 유전자의 메틸화를 검출하기 위한 프라이머를 제작하기 위하여 바이설파이트로 변환된 ADHFE1 유전자 서열을 대상으로 MethPrimer (http://www.urogene.org/cgi-bin/methprimer/methprimer.cgi)를 사용하여 메틸화 특이적 프라이머와 프로브를 설계하였다 (표 1).
설계된 프라이머의 ADHFE1 유전자의 메틸화 특이적 검출 능력을 확인하기 위하여 메틸화 및 비메틸화 control DNA (EpiTect PCR control DNA set (Qiagen, cat. no. 59695)를 사용하여 quantitative methylation-specific real time PCR (qMSP) 방법으로 검증하였다. qMSP는 AB 7500 Fast (Thermo Fischer) 장비를 이용하였다. 총 30 ㎕ PCR 반응 용액 (바이설파이트로 변환된 주형 사람 DNA, 20 ng/2 ㎕; 5X AptaTaq DNA Master (Roche Diagnostics), 6 ㎕; 메틸화 특이적 PCR 프라이머 (IDT, USA), 각 2 ㎕ (2.5 pmole/㎕), TaqMan probe (IDT, USA), 각 2 ㎕ (2.5 pmole/㎕); D.W. 14 ㎕)을 준비하고 PCR 조건은 95oC, 5분 처리한 후 95oC에서 15초, 65oC에서 1분으로 총 40회 수행하였다. PCR 산물의 증폭여부를 실시간으로 확인하였다. 내부 대조 유전자로는 COL2A1 유전자를 함께 증폭하였다 (도 2).
도 2에 보는 것처럼 메틸화 DNA에서는 ADHFE1 (Set 1)과 대조유전자 COL2A1이 공히 증폭되었으나, 비메틸화 DNA에서는 ADHFE1 (Set 1) 유전자는 증폭되지 않고 대조유전자인 COL2A1만 증폭되는 것을 확인함으로써, ADHFE1 유전자에 대한 set 1 메틸화 특이적인 프라이머들이 정상적으로 작동하는 것을 확인하였다.
도 3은 메틸화 DNA에서는 ADHFE1 (Set 2)과 대조유전자 COL2A1이 공히 증폭되었으나, 비메틸화 DNA에서는 ADHFE1 (Set 2) 유전자는 증폭되지 않고 대조유전자인 COL2A1만 증폭되는 것을 확인함으로써, ADHFE1 유전자에 대한 Set 2 메틸화 특이적인 프라이머들이 정상적으로 작동하는 것을 확인하였다.
실시예 3. 분변 DNA에서 SDC2와 ADHFE1 유전자 메틸화 조합을 통한 대장암 진단 성능 증가
ADHFE1 유전자를 기존의 대장암 조기진단용 메틸화 바이오마커인 SDC2 유전자 (대한민국 특허 등록 제10-1142131호)의 메틸화와 조합하였을 때, 대장암 진단능력이 개선되는지를 확인하기 위하여 대장내시경 정상인 (24명, 연세의료원 체크업 센터)과 대장암 환자 33명 (연세의료원 대장항문외과)을 대상으로 SDC2 유전자의 메틸화를 측정하였다 (도 4). 이들 검체들은 사전에 SDC2 메틸화를 측정한 검체들로서 대장내시경 정상인의 경우, 24명중 1명에서 SDC2 메틸화 양성 (4.2%)이 관찰되었으며, 대장암 환자의 경우 18명 (54.5%)만이 SDC2 메틸화가 양성인 검체들을 사용하였다.
이들 검체에서 메틸화 측정은 실시예 2에 기재된 quantitative methylation-specific real time PCR (qMSP) 방법을 사용하였다. qMSP는 AB 7500 Fast (Thermo Fischer) 장비를 이용하였으며, 바이설파이트 변환은 EZ Methylation Gold Kit (Catalog No. D5006, Zymo Research, USA)을 사용하였다. 분변 DNA 대상 SDC2와 ADHFE1 유전자의 메틸화 측정은 총 30 ㎕ PCR 반응 용액 (바이설파이트로 변환된 분변 DNA, 2.0 ug/10 ㎕; 5X AptaTaq DNA Master (Roche Diagnostics), 6 ㎕; 메틸화 특이적 PCR 프라이머 (IDT, USA), 각 2 ㎕ (2.5 pmole/㎕), TaqMan probe (IDT, USA), 각 2 ㎕ (2.5 pmole/㎕); D.W. 6 ㎕)을 준비하였고, 내부 대조 유전자로는 COL2A1 유전자를 함께 증폭하였다. PCR 조건은 95oC, 5분 처리한 후 95oC에서 15초, 65oC에서 1분으로 총 40회 수행하였다. PCR 산물의 증폭여부를 실시간으로 확인하였다 (도 3). 메틸화 및 비메틸화 control DNA (EpiTect PCR control DNA set (Qiagen, cat. no. 59695)를 control로 사용하여 함께 시험하였다.
ADHFE1 유전자 (Set 2)의 메틸화를 대장내시경 정상인 대변에서 측정하였을 때, 1명에서만 메틸화 양성을 나타내었고 (특이도 95.8%), 대장암 환자에서는 26명에서 메틸화 양성 (민감도 78.8%)을 나타내어 대장암 검출에 유효성이 있음을 확인하였다. 또한 기존에 알려진 SDC2 유전자 메틸화에 ADHFE1 유전자 (Set 2) 메틸화를 조합하였을 때, 대장내시경 정상인에서 메틸화 양성빈도는 1명이 추가되어 총 2명으로 특이도가 91.7%로 우수하여 특이도의 유의한 손상은 없었다. 대장암 환자에서의 메틸화 양성빈도는 9명이 추가되어 총 27명으로 민감도가 81.8%로 유의하게 증가하였다 (도 4, P < 0.033, Fisher’s exact test).
본 발명은 복수의 메틸화된 대장암 마커 유전자를 특이적으로 증폭하는 프라이머를 통해 메틸화를 높은 검출 민감도로 검출함으로써, 단일 마커 유전자로 검출하는 방법보다, 장암 검출능력이 개선정확하고 빠르게 대장암 진단이 가능하게 되어, 유용하다.
이상으로 본 발명 내용의 특정한 부분을 상세히 기술하였는 바, 당업계의 통상의 지식을 가진 자에게 있어서 이러한 구체적 기술은 단지 바람직한 실시 양태일 뿐이며, 이에 의해 본 발명의 범위가 제한되는 것이 아닌 점은 명백할 것이다. 따라서, 본 발명의 실질적인 범위는 첨부된 청구항들과 그것들의 등가물에 의하여 정의된다고 할 것이다.
