Abstract
본 발명은 아젤록심 D를 유효성분으로 함유하는 방사선 민감성 증진용 약학 조성물, 항암 치료 보조용 조성물에 관한 것으로 상기 아젤록심 D는 암세포의 방사선 민감성을 증진시키고, 암세포에서 ATF4의 발현을 증가시켜 항암 치료의 효율을 높일 수 있다.
Description
본 발명은 아젤록심 D를 유효성분으로 함유하는 방사선 민감성 증진용 조성물에 관한 것으로 구체적으로는 암세포에 방사선 조사와 병용 처리하는 경우 방사선 민감제로 작용하여 암세포의 세포사멸을 촉진할 수 있는 방사선 민감성 증진용 조성물이다.
암의 치료 및 진단을 위한 많은 방법과 기술들이 개발되었으나 아직까지 암은 전 세계에 걸쳐 인간의 건강을 위협하는 심각한 질병이다. 암의 치료법은 크게 외과적 수술, 방사선 치료, 항암화학요법으로 나눌 수 있으며, 방사선 처리와 관련된 기술과 기기의 개발, 화학 요법의 부작용과 외과적 수술에 의한 통증 및 기능 소실 등에 대한 부담감 등으로 인해 방사선 치료를 받는 암환자의 수는 매년 증가하고 있다.
그러나 암세포의 방사선 내성 획득, 고선량 방사선 조사시 정상 조직의 손상 등은 방사선 치료의 치명적인 한계로서 이를 극복하기 위하여 방사선 민감성 증진제에 관한 연구가 이루어지고 있다. 현재까지 보고된 방사선 치료 민감제는 주로 항암제들로서, 예를 들어 탁솔(taxol)과 시스플라틴(cisplatin)이 유방암, 자궁암. 폐암, 위암, 대장암 등의 고형암에서 방사선 치료 민감제로 사용될 수 있다고 보고된 바 있다. 그러나 이들 방사선 치료 민감제들은 그 자체가 항암제로서 사용되는 물질로 높은 부작용을 가지므로 그 사용이 제한적이라는 문제점이 있다.
본 발명자들은 상기 문제점을 해결하기 위해 방사선 치료에 대한 민감성을 증진시킬 수 있는 물질을 연구하였고, 아젤라스 속의 해면으로부터 유래한 천연물인 아젤록심 D가 암세포의 방사선 민감성을 증가시키는 것을 확인하여 본 발명을 완성하였다.
본 발명의 목적은 항암치료를 위한 방사선 조사 전 또는 조사하는 동안에 투여하여 방사선 치료의 효과를 증진시킬 수 있는 방사선 민감성 증진용 약학 조성물을 제공하는 것이다.
상기 목적을 달성하기 위하여, 본 발명의 일 양상은 하기 화학식 1의 구조를 갖는 아젤록심 D 또는 이의 약학적으로 허용가능한 염을 유효성분으로 함유하는 방사선 민감성 증진용 약학 조성물을 제공한다.

상기 화학식 1의 아젤록심 D는 아젤라스(Agelas) 속의 해면으로부터 유래한 천연물로 상기 해면에서 빈번하게 발견되는 아젤라신 D(agelasine D)의 유도체이다. 아젤록심 D는 낮은 수준의 암세포 생장 억제 활성, 미생물 생장 억제 활성 등을 갖는 것으로 보고된바 있으나, 방사선 민감성 증진 효능에 대해서는 전혀 알려진 바가 없다. 아젤록심 D는 최초에 아젤라신 D의 7번 질소에 하이드록시기(-OH)가 치환된 옥심 화합물(하기 화학식 2)로 그 구조가 제안되었으나, 최근 합성 연구를 통해 아젤라신 D의 아데닌기 중 이미다졸 부분이 가수분해되어 개환된 형태인 화학식 1의 구조일 가능성이 큰 것으로 제시되었다. 본 발명자들은 아젤록심 D의 구조를 액체크로마토그래피-고분해능 질량분석으로 확인한 결과, 화학식 1의 구조를 갖는 것을 최종적으로 확인할 수 있었다.

본 발명에서, 상기 방사선 민감성 증진용 약학 조성물은 항암 치료시 방사선 조사와 병용하여 투여될 수 있다. 상기 암은 방사선 저항성 암일 수 있으나, 이에 제한되는 것은 아니며, 유방암, 폐암, 골암, 췌장암, 피부암, 구강암, 구강인두암, 자궁암, 난소암, 직장암, 위암, 자궁내막암종, 자궁경부암종, 질암종, 소장암, 갑상선암, 부갑상선암, 전립선암, 만성 또는 급성 백혈병, 림프구 림프종, 방광암, 신장암, 간암, 대장암 또는 뇌종양일 수 있다.
본 발명의 일 구체예에 따르면, 간암 세포주인 Hep3B 세포에 방사선 조사와 아젤록심 D를 병용 처리한 경우, 아젤록심 D를 단독으로 처리하거나 방사선만 조사한 실험군과 비교하여 암세포의 세포사멸(apoptosis)이 현저히 증가한다. 또한, 스트레스에 의한 세포사멸을 유도하는 유전자를 활성화시키는 ATF4의 발현 또한 병용 처리군에서 현저히 증가하므로 아젤록심 D는 암세포의 방사선 민감성 증가 용도로 유용하게 사용될 수 있다.
본 명세서에 사용된 용어, "약학적으로 허용가능한 염"은 인간에게 투여하기에 적합한 안전성 및 효능 프로파일을 갖는 염을 의미한다. 구체적으로는, 아젤록심 D의 제약상 허용되는 염으로서, 구체적인 형태로는 무기 산, 예컨대 염산, 질산, 인산, 황산, 브로민화수소산, 아이오딘화수소산, 아인산 및 이들의 혼합물로부터 유도된 염뿐만 아니라 유기산, 예컨대 지방족 모노- 및 디카르복실산, 페닐-치환된 알칸산, 히드록시 알칸산, 알칸디오산, 방향족 산, 및 지방족 및 방향족 술폰산으로부터 유도된 염을 포함할 수 있으나, 이에 제한되지는 않는다.
본 명세서에 사용된 용어, "방사선 민감성(radiation sensitivity)"은 방사선에 대한 생체의 민감성을 의미한다. 이에 관련된 주요 인자로는 표적의 크기(DNA 량), 세포 동태(세포분열의 주기), 세포의 상태(포자 등), 방사선 손상의 수복 등이 있다. 다세포생물계에서는, 일반적으로 세포분열이 활발한 세포, 예를 들면 발생 초기 태아의 모든 기관, 식물의 뿌리 끝이나 생장단, 생식세포, 조혈조직, 장상피, 면역기능세포 또는 암세포 등은 방사선에 고감수성이고, 세포분열이 활발하지 않은 세포, 예를 들면 성인의 신경이나 뇌의 대부분, 근육, 간, 성숙한 혈구 등은 방사선에 저항성을 나타내기도 한다. 그러나 방사선 감수성이 높은 조직에서도 여러 가지 이유로 인하여 방사선 저항성이 생겨날 수 있다.
본 명세서에 사용된 용어, "방사선 조사"는 이온화 방사선, 특히, 통상적으로 사용되는 선형 가속기(linear accelerators) 또는 방사핵종(radionuclides)에 의해 방사되는 감마 방사선 및 엑스선을 세포, 구체적으로 암세포에 쪼이는 것을 의미한다. 방사핵종에 의한 방사선 조사는 외부적 또는 내부적으로 이루어질 수 있으며, 방사선 조사량 및 방사선 조사량의 간헐성은 종양의 종류, 위치, 화학 요법 또는 방사선 요법에 대한 환자의 반응과 같은 일련의 변수들에 의해 달라질 수 있다.
본 발명에서, 방사선 조사를 위한 기술로는 근접치료, 방사선 핵종치료, 외부 빔 방사선 요법 (external beam radiation therapy), 온열치료 (냉동절제 치료 및 고열 치료 포함), 방사선 수술, 하전입자 방사선 치료 (charged-particle radiotherapy), 중성자 방사선 요법 및 광역동 치료 등을 포함할 수 있다.
방사선 조사는 국소 요법의 한 형태이므로, 일반적으로 부작용은 치료된 부위에 제한된다. 그러나 일부 암세포의 방사선 내성 획득, 고선량 방사선 치료시 정상 조직이 손상될 수 있는 점은 가장 큰 부작용이다. 본 발명에 따른 방사선 민감성 증가용 약학 조성물은 방사선 조사의 필요량을 감소시킴으로써 이러한 부작용을 줄일 수 있다. 또한, 방사선에 민감한 암세포뿐만 아니라, 방사선에 저항성이 있는 암세포에서도 방사선 민감도를 증가시킬 수 있다.
본 명세서에 사용된 용어, "병용하여 투여"는 다양한 종류의 암세포를 치료하는 항암 과정에서 방사선 조사와 함께 본 발명에 따른 방사선 민감성 증가용 약학 조성물을 투여하는 것을 의미한다. 구체적으로는, 간암, 폐암, 유방암, 대장암, 난소암, 두경부암, 뇌암 등의 고형암과 같은 암세포를 치료하는 항암 과정에서 방사선 조사 치료법과 병용 처리되는 것일 수 있다.
본 발명에서 용어, "투여"는 임의의 적절한 방법으로 개체에게 본 발명의 방사선 민감성 증가용 약학 조성물을 제공하는 것을 의미한다.
본 발명의 방사선 민감성 증가용 약학 조성물은 유효성분으로 아젤록심 D 또는 이의 약학적으로 허용가능한 염을 함유하는 것 이외에 약학 조성물의 제조에 통상적으로 사용하는 적절한 담체, 부형제 및 희석제를 추가로 포함할 수 있다. 또한, 통상의 방법에 따라 산제, 과립제, 정제, 캡슐제, 현탁액, 에멀젼, 시럽, 에어로졸 등의 경구형 제형, 외용제, 좌제 및 멸균 주사용액의 형태로 제형화하여 사용될 수 있다. 당해 기술 분야에 알려진 적합한 제제는 문헌(Remington's Pharmaceutical Science, 최근, Mack Publishing Company, Easton PA)에 개시되어 있는 것을 사용하는 것이 바람직하다.
본 발명의 약학 조성물에 포함될 수 있는 담체, 부형제 및 희석제로는 락토오스, 덱스트로오스, 수크로오스, 솔비톨, 만니톨, 자일리톨, 에리스리톨, 말티톨, 전분, 아카시아 고무, 알지네이트, 젤라틴, 칼슘 포스페이트, 칼슘 실리케이트, 셀룰로오스, 메틸 셀룰로오스, 미정질 셀룰로오스, 폴리비닐 피롤리돈, 물, 메틸히드록시 벤조에이트, 프로필히드록시 벤조에이트, 탈크, 마그네슘 스테아레이트 및 광물유 등이 있다. 상기 약학 조성물을 제제화할 경우에는 보통 사용하는 충진제, 증량제, 결합제, 습윤제, 붕해제, 계면활성제 등의 희석제 또는 부형제를 사용할 수 있다.
경구투여를 위한 고형제제에는 정제, 환제, 산제, 과립제, 캡슐제 등이 포함되며, 이러한 고형제제는 상기 조성물에 적어도 하나 이상의 부형제 예를 들면, 전분, 칼슘 카보네이트(calcium carbonate), 수크로오스, 락토오스, 젤라틴 등을 섞어 조제된다. 또한, 단순한 부형제 이외에 마그네슘 스테아레이트, 탈크 같은 윤활제들도 사용된다. 경구를 위한 액상 제제로는 현탁제, 내용액제, 유제, 시럽제 등이 해당되는데 흔히 사용되는 단순 희석제인 물, 리퀴드 파라핀 이외에 여러 가지 부형제, 예를 들면 습윤제, 감미제, 방향제, 보존제 등이 포함될 수 있다. 비경구 투여를 위한 제제에는 멸균된 수용액, 비수성용제, 현탁제, 유제, 동결건조 제제, 좌제가 포함된다. 비수성용제, 현탁제로는 프로필렌 글리콜 (propylene glycol), 폴리에틸렌 글리콜, 올리브 오일과 같은 식물성 기름, 에틸올 레이트와 같은 주사 가능한 에스테르 등이 사용될 수 있다. 좌제의 기제로는 위텝솔(witepsol), 마크로골, 트윈(tween) 61, 카카오지, 라우린지, 글리세로 젤라틴 등이 사용될 수 있다.
본 발명의 방사선 민감성 증가용 약학 조성물의 바람직한 투여량은 개체의 상태 및 체중, 질병의 정도, 약물형태, 투여경로 및 기간에 따라 변화될 수 있고, 당업자에 의해 적절하게 선택될 수 있다. 상기 조성물의 투여는 하루에 한번 투여할 수도 있고, 수회 나누어 투여할 수도 있다.
본 발명의 방사선 민감성 증가용 약학 조성물은 개체에 다양한 경로로 투여될 수 있다. 투여의 모든 방식은 예상될 수 있는데, 예를 들면, 경구, 직장 또는 정맥, 근육, 피하, 자궁내 경막 또는 뇌혈관 내(intracerebroventricular) 주사에 의해 투여될 수 있다.
또한, 상기 조성물은 약제학적으로 유효한 양으로 투여될 수 있다. 이는 질환의 종류, 중증도, 약물의 활성, 약물에 대한 환자의 민감도, 투여 시간, 투여 경로, 치료기간, 동시에 사용되는 약물 등에 따라 달라질 수 있다. 그러나, 바람직한 효과를 위해서, 본 발명에 따른 조성물에 포함되는 유효성분의 양은 0.0001 내지 100 ㎎/㎏, 구체적으로 0.001 내지 10 ㎎/㎏일 수 있다.
본 발명의 다른 양상은 하기 화학식 1의 구조를 갖는 아젤록심 D 또는 이의 약학적으로 허용가능한 염을 유효성분으로 함유하는 항암 치료 보조용 약학 조성물을 제공한다.

상기 항암 치료 보조용 약학 조성물은 방사선 민감성 증진용 약학 조성물의 유효성분인 아젤록심 D를 사용하므로 이 두 조성물 사이에 중복되는 내용은 명세서의 과도한 기재를 피하기 위하여 생략한다.
본 발명의 일 구체예에 따르면, 상기 아젤록심 D는 암세포의 방사선 민감성을 증진시키므로 방사선을 이용하는 항암 치료에 사용될 수 있고, 암세포에서 ATF4의 발현을 증가시켜 항암 치료의 효율을 높일 수 있으므로 항암 치료 보조 용도로 유용하게 사용될 수 있다.
본 발명의 또 다른 양상은 상기 화학식 1의 구조를 갖는 아젤록심 D 또는 이의 약학적으로 허용가능한 염을 유효성분으로 함유하는 항암 치료 보조용 식품 조성물을 제공한다.
상기 항암 치료 보조용 식품 조성물은 방사선 민감성 증진용 약학 조성물의 유효성분인 아젤록심 D를 사용하므로 이 두 조성물 사이에 중복되는 내용은 명세서의 과도한 기재를 피하기 위하여 생략한다.
본 발명의 일 구체예에 따르면, 상기 아젤록심 D는 해면에서 유래한 천연물로 암세포의 방사선 민감성을 증진시키고, 암세포에서 ATF4의 발현을 증가시켜 항암 치료의 효율을 높일 수 있으므로 항암 치료 보조용 식품 조성물로 유용하게 사용될 수 있다.
본 발명에서, 상기 식품 조성물은 분말, 과립, 정제, 캡슐, 시럽, 음료 또는 환의 형태로 제공될 수 있으며, 유효성분인 본 발명에 따른 아젤록심 D 이외에 다른 식품 또는 식품 첨가물과 함께 사용되고, 통상적인 방법에 따라 적절하게 사용될 수 있다. 유효성분의 혼합양은 그의 사용 목적 예를 들어 예방, 건강 또는 치료적 처치에 따라 적합하게 결정될 수 있다.

상기 식품 조성물의 종류에는 특별한 제한이 없고, 예로는 육류, 소세지, 빵, 쵸코렛, 캔디류, 스낵류, 과자류, 피자, 라면, 기타 면류, 껌류, 아이스크림류를 포함한 낙농제품, 각종 스프, 음료수, 차, 드링크제, 알콜 음료 및 비타민 복합제 등을 들 수 있다.
본 발명에 따른 아젤록심 D는 암세포의 방사선 민감성을 증진시키고, 암세포에서 ATF4의 발현을 증가시켜 항암 치료의 효율을 높일 수 있으므로 방사선 민감성 증진용 약학 조성물의 유효성분으로 유용하게 사용될 수 있다.

이하 하나 이상의 구체예를 실시예를 통하여 보다 상세하게 설명한다. 그러나, 이들 실시예는 하나 이상의 구체예를 예시적으로 설명하기 위한 것으로 본 발명의 범위가 이들 실시예에 한정되는 것은 아니다.
실시예 1: 아젤록심 D의 구조 확인
아젤록심 D(ageloxime D)는 애디포젠(Adipogen, 미국)사로부터 구입하여 수소 핵자기 공명 스페트럼(도 1), 탄소 핵자기 공명 스펙트럼(도 2), 액체크로마토그래피-고분해능 질량분석(도 3 내지 7)을 통해 화합물의 구조와 순도를 확인한 후 사용하였다.
그 결과, 도 3 내지 7에 제시된 고분해능 질량분석 결과를 통해 아젤록심 D의 m/z 값이 440.3357인 것을 확인하여 하기 화학식 1에 해당하는 구조의 M+H 이온에 해당하는 440.3389가 도출된 바, 아젤록심 D 의 구조가 최초에 제안된 화학식 2가 아닌 화학식 1인 것으로 확정할 수 있었다.

실시예 2: 아젤록심 D의 효과 확인_인 비트로(

)
2-1. 세포 생존율
간암 세포주인 Hep3B 세포를 6 웰 플레이트에 X선 흡수선량이 0 Gy, 4 Gy, 6 Gy, 8 Gy에 해당하는 조건에 각각 800, 2000, 4000, 6000 세포/웰의 농도로 접종하여 24시간 동안 전배양하였다. 이후 아젤록심 D를 2 ㎍/㎖ 농도로 처리하고 (대조군은 DMSO 처리) 3시간 뒤에 X선을 세포에 조사하였다. 2주 후 크리스탈 바이올렛(crystal violet)으로 세포를 염색하고, 4 Gy, 6 Gy, 8 Gy 조사군에서 각각 살아남은 콜로니의 비율을 방사선 비조사군 대비 상대적인 값으로 환산하여 세포의 방사선 민감도를 측정하였다. 방사선 조사는 환자 치료용 선형가속기(Varian 사 Clinac 6EX)를 사용하여 3.96 Gy/분의 속도로 해당 방사선량(dose)의 X선을 세포가 들어있는 플레이트에 직접 조사하였다.
실험 결과, 도 8에 나타낸 바와 같이 아젤록심 D 단독처리군과 비교하여 아젤록심 D+방사선 병용처리군에서 세포사멸이 현저히 많이 발생하는 것을 확인할 수 있었다. 본 결과는 아젤록심 D가 방사선 처리에 의한 암세포의 사멸을 촉진시킬 수 있음을 의미한다.
2-2. ATF4 발현량 확인
ATF4 (activating transcription factor 4)는 세포 내에서 산화적 스트레스, 소포체 스트레스를 비롯한 여러 스트레스 요인으로부터 세포를 보호하는 기작에 관여하는 중요한 인자로서, 스트레스에 의한 세포사멸(apoptosis)을 유도하는 유전자의 활성화와 연관 있는 것으로 알려져 있다. 따라서, 아젤록심 D가 ATF4의 발현에 영향을 미치는지 확인하였다.
상기 실시예 2-1과 동일한 조건으로 Hep3B 세포에 아젤록심 D를 단독으로 처리(1, 5 및 10 ㎍/㎖; 아젤록심 D 단독처리군)하거나 아젤록심 D 처리 후 방사선을 조사하는 병용 처리(2 ㎍/㎖+6 Gy; 아젤록심 D+방사선 병용처리군)를 수행하였다. 24시간 후 세포를 회수하여 웨스턴 블롯으로 ATF4의 발현 수준을 확인하였다.
확인 결과, 도 9에 나타낸 바와 같이 아젤록심 D 단독처리군에서 처리 농도의 증가에 따라 ATF4 발현이 증가하고 그 하위인 DDIT3 (DNA damage-inducible transcript 3) 수준도 함께 증가하는 것을 알 수 있었다. 또한 세포자가포식 마커인 LC3B(microtubule-associated protein light chain 3) 수준도 증가하였다.
또한, 아젤록심 D는 2 ㎍/㎖의 처리 농도에서 세포사멸을 유도하지 않으나 6 Gy의 X선 단독 처리는 세포사멸을 유도하는 것을 절단된 PAPR 단백질의 수준 증가를 통해 확인할 수 있었다. 아젤록심 D+방사선 병용 처리군에서 아젤록심 D 단독처리군 대비 세포사멸 유도 효과가 가장 현저한 것을 확인하였고, 이러한 현상은 아넥신 V 염색을 통한 유세포 분석으로도 확인할 수 있었다.
실시예 3: 아젤록심 D의 효과 확인_인 비보(
in vivo
)
6주령의 암컷 BALB/c 누드 생쥐를 구입하고, 무균 시설에서 12시간 명암 주기로 2주 간의 적응 기간을 거친 후 실험에 사용하였다. 상기 누드 생쥐의 오른쪽 다리에 Hep3B 세포를 5x10
6개 주입하여 자유식이 조건으로 사육하였다. 종양의 크기가 100 내지 200 ㎣에 도달했을 때 누드 생쥐를 무작위로 4개 그룹으로 나누고, 각 그룹에 해당하는 실험 조건으로 3주 동안 일주일에 3회 종양 크기의 변화를 관찰하였다: 대조군(Sham; 식염수 투여), 아젤록심 D 단독투여군(아젤록심 D; 1.25 ㎎/㎏, 주 3회 복강 주입), 방사선 단독조사군(RT; 3 Gy/일 x 3일 연속) 및 아젤록심 D+방사선 병용처리군(아젤록심 D+RT).
실험 결과, 도 10에 나타낸 바와 같이 대조군 및 다른 실험군과 비교하여 아젤록심 D+방사선 병용처리군에서 종양 크기가 가장 많이 감소한 것을 확인할 수 있었다.
지금까지의 실험 결과를 통하여 아젤록심 D가 암세포에 대한 방사선 조사의 효과를 증폭시키는 것을 알 수 있었으며, 방사선 조사 민감성 조성물의 유효성분으로 유용하게 사용될 수 있음을 확인하였다.
Claims (7)
- 제1항에 있어서, 상기 조성물은 암 치료시 방사선 조사와 병용하여 투여되는 것인 방사선 민감성 증진용 약학 조성물.
- 제1항에 있어서, 상기 조성물은 방사선 조사에 의한 암세포의 세포사멸을 유도하는 것인 방사선 민감성 증진용 약학 조성물.
- 제1항에 있어서, 상기 조성물은 암세포에서 활성 전사 인자 4 (activating transcription factor 4, ATF4)의 발현을 증가시키는 것인 방사선 민감성 증진용 약학 조성물.
- 제1항에 있어서, 상기 암은 유방암, 폐암, 골암, 췌장암, 피부암, 구강암, 구강인두암, 자궁암, 난소암, 직장암, 위암, 자궁내막암종, 자궁경부암종, 질암종, 소장암, 감상선암, 부갑상선암, 전립선암, 만성 또는 급성 백혈병, 림프구 림프중, 방광암, 신장암, 간암, 대장암 또는 뇌종양인 것인 방사선 민감성 증진용 약학 조성물.
