Definitions
- the title of the invention is rivetHead immobilization device and method for three-dimensional examinations”.
- the invention relates to a device and method of immobilization of the head of the patient intended for the correct sampling of information during three-dimensional examinations for various operations and analysis of the condition of patients using mobile means of inspection.
- a headrest device for positioning the head of a surgery patient or during other medical operations was designed, in which the posterior part of the patient's head is placed [1] or another device in which the patient's face is placed, where the face of the patient is covered in a high proportion by the support. [2] In both cases the forward and backward flexion movements are not limited by the support.
- the problem solved by the invention is the immobilization of the patient's head and the elimination of even small amplitude movements to obtain accurate and systematic three- dimensional reconstructions of soft tissues and hard tissue surfaces from the patient's head area.
- the device and immobilization method of some areas of interest of the head in relation to the reference system of the three-dimensional ultrasonic scanner removes the above disadvantages because: a) for preparing the patient before acquiring information he is introduced and immobilized in an assembly of rods, joints and moving parts completed with a custom mouth guard; b) for the systematic taking over of the information, the transducer for taking over the sonographic image is fixed on a robotic arm in a well determined spatial relation to the immobilization assembly.
- the head immobilization device removes the above disadvantages since the immobilization is achieved by using a side support by two pre positioned rods, as well as by using in the posterior area of the head, the headrest of a dental chair, as well as by applying a fixing element directly at the hard tissue structures, at teeth level, called custom mouth guard and which is a component part of this immobilization device having a roll for the elimination of flexion movements and antero-posterior extension of the head, including low amplitude ones.
- the device is coupled at hard tissues level, namely teeth level, for the reduction of the reduced amplitude movements, during scanning, compared to the resilience and flexibility of other tissues like forehead skin; b) the area of interest of the face is uncovered, ready for three-dimensional imaging examinations; c) manufacturing the mechanical head immobilisation device is low cost compared to other known procedures or devices, like optical spatial positioning systems used in image guided surgery.
- FIG. 1 and FIG. 2 An example of the invention is presented below in FIG. 1 and FIG. 2:
- Fig. 1 the component parts of the head immobilization device in axonometric representation.
- Fig. 2 the head immobilization device applied during 3D image acquiring procedure.
- the head immobilization device consists (Fig. 1) of a custom mouth guard 0, a customized piece, specific to each patient, manufactured based on the jaw impression where the custom guard will be applied.
- the impressions will be registered classically, by the use of impression materials or, by the use of an intraoral optical scanner, a connecting element of the custom mouth guard with the rest of the head immobilization device of the patient O’ which is an extension of the custom mouth guard.
- Lockable articulation elements a, b, c, d, respectively e, positioned at the ends of the component rods give the possibility of sliding and rotating for two other adjacent parts / rods of the assembly, rods 4, respectively 5 which have the possibility of sliding towards the middle or side at the level of another rod, 6.
- Rods 4, 5, respectively 6 have the role of fixing the patient's head by supporting its lateral parts by forming a subassembly, framing the patient's head.
- a connecting element that ensures the connection between the splint and the rest of the immobilization device gives the possibility to adjust the length of the segment formed by the rod 9 and the splint element O' by sliding them, as the sliding connection element allows length adjustment of this segment and then ensures the immobilization of the custom mouth guard because it is a lockable element.
- the subassembly formed by the parts 1, 2, 3, 4 and 5 respectively by means of the articulation elements d and e can be rotated by at least 180 ° around the axis of the rod 6.
- the assembly formed by the parts 1, 2 and 3 can slide at the level of the parts 4, to position the custom mouth guard vertically, moving it cranio-caudally near the patient's oral cavity.
- a subassembly consisting of parts 8 and 9, respectively, supports the custom mouth guard and has the possibility of lateral sliding by means of the articulation element a and the possibility of adjusting the inclination of the rod 9 by means of a ball-type articulation element h.
- the patient will be placed in lying position in the dental chair and his head is supported by the headrest 15.
- the head immobilization device is tilted back to allow the patient to position his head at the dental chair headrest level.
- the tilting back and forth of the entire head immobilization assembly is done by rotating it around the rod 6 using the lockable joints d and e.
- the rods 4 and 5 will be moved to the side when applying the assembly to allow the framing of to the patient's head.
- the assembly will be brought into the scanning position, by framing the patient's head.
- Rods 4 and 5 will be positioned by sliding preauricular and meet the skin of the face. After reaching this position, the joints d and e will be locked. At this point the rods 4, 5, 6 will form a headdocking assembly and will be in the scanning position.
- the custom mouth guard support assembly consisting of rods 1, 2, 3, 8 and 9, will have locked the component joints a, b, c, f, g, h. At a later stage, this assembly is slid in the cranio- caudal direction through the sliding joints b and c until the splint is in front of the patient's oral cavity.
- the custom mouth guard will be positioned at the teeth occlusal surfaces level due to an antero-posterior inclination through the sliding joints a and i and the movable joints g, f and the ball-type joint, h.
- the three-dimensional scan will be performed by using a three- dimensional scanner consisting of an articulated arm 10 as a spatial positioning reading system in spatial coordinates for the movements of an ultrasound transducer 13 during scanning.
- probe 11 level is mechanically coupled through coupling piece 12 a medical imaging acquiring device such as a two-dimensional ultrasound transducer 13.
- the articulated arm 10 can be replaced with any positioning reading system, having its reference system supported by the stand 7.
- the linear two-dimensional transducer or any kind of two-dimensional ultrasound probe 13 will acquire ultrasound bidimensional images corresponding to the two-dimensional scanning plane at the examined area level for three-dimensional ultrasound reconstructions purposes.
