Abstract
본 발명은 IL-2, 이의 발현 촉진제 또는 활성화제를 유효성분으로 포함하는 암 질환 예방 또는 치료용 조성물에 관한 것으로, 본 발명에서는 암세포에 IL-2를 처리 시, 암세포 유래 엑소좀 분비가 억제되고, T 세포의 면역 회피를 유도하는 PD-L1의 발현이 억제되어 항암 효과가 증진되는 것을 확인하였으며, IL-2가 세포독성 T 세포를 활성화시켜 면역 항암 효과를 나타내는 종래 기술과는 달리, IL-2가 암세포에 직접적으로 항암 효과를 나타내는 것을 확인한 바, IL-2, 이의 발현 촉진제 또는 활성화제는 암 질환 예방 또는 치료용 조성물 등으로 유용하게 활용될 수 있다.
Description
본 발명은 엑소좀 PD-L1의 발현에 대한 억제제를 유효성분으로 포함하는 항암 효과 증진용 조성물로, 구체적으로 IL-2가 암세포의 엑소좀 PD-L1의 발현을 억제하는 새로운 기전을 이용한 항암 효과 증진제에 관한 것이다.
흑색종은 전 세계적으로 발병률이 지속적으로 증가하는 공격적인 피부암이다. 이 질환은 전 이율, 치사 및 재발 수준이 높은 두 성별의 백인 인구에 주로 영향을 미친다. 지난 몇 년간 FDA (미국 식품 의약품 안전청)와 EMA (유럽 의약품 청)에서 여러 요법을 승인했다. 종양의 특징에 따라, 치료제는 외과적 절제술, 화학 요법, 방사선 요법, 광 역학 요법 (PDT), 면역 요법 또는 표적 요법 일 수 있다. 베무라페닙 및 다브라페닙과 같은 BRAF 단백질 억제제, 또는 트라메티닙으로 대표되는 MEK 억제제로 구성되는 표적 요법, 및 니볼루맙, 이필리무맙, 펨브롤리주맙과 같은 면역 체크 포인트 억제제의 승인이 도입되었다. 또한 사이토 카인 IL-2는 암 요법에서 숙주의 면역 조절을 위한 단일 작용제로서 광범위하게 사용되어왔다.

한편, 흑색종의 치료로는 수술 외에도 면역치료(인터페론), 항암치료, 방사선 치료와 최근 도입 단계에 있는 표적치료가 있으며, 최근 각광받고 있는 면역요법이 있다. 면역요법은 면역반응이 흑색종의 발병에 관련이 있다는 사실을 근거로 전이성 흑색종 치료에 이용되는데 현재까지 미국식품의약군(FDA)에서 승인을 받은 치료로는 인터페론-알파(3기 치료용)와 인터루킨-2(4기 치료용)가 있다.
항암요법제로는 다카바진(DTIC)과 시스플라틴 등이 단독 또는 복합요법으로 사용되고 있으며, 최근들어 ‘표적치료’ 또는 ‘세포치료’로 알려진 새로운 치료제들이 개발되고 있어 희망을 주고 있으며 속속 FDA 승인을 얻어 점차 사용되는 추세이지만 국내에서는 현재 임상시험 또는 도입 직전 단계이다.
본 발명은 흑색종의 4기 치료제로 사용되고 있는 IL-2가 흑색종 세포에 있어 MAPK signaling 의존적으로 세포외 소포체 분비 및 PD-L1 발현을 억제시켜 보다 효과적인 항암치료가 가능함을 제시한다.
본 발명은 엑소좀 PD-L1의 발현에 대한 억제제를 유효성분으로 포함하는 항암 효과 증진용 조성물로, 구체적으로 IL-2가 암세포의 엑소좀 PD-L1의 발현을 억제하는 새로운 기전을 이용한 항암 효과 증진제에 관한 것으로, IL-2를 암세포에 처리하였을 때에 암세포 유래의 엑소좀 분비가 억제되고, T 세포의 면역 회피를 유도하는 PD-L1의 발현이 억제되는 항암효과가 증진되는 것을 확인함으로써, IL-2, 이의 발현 촉진제 또는 활성화제를 엑소좀 PD-L1의 발현에 대한 억제제로써 암 질환의 예방 또는 치료를 증진할 수 있는 항암 효과 증진용 제제로 제공하는 것이다.
본 발명은 엑소좀 PD-L1의 발현에 대한 억제제를 유효성분으로 포함하는 항암 효과 증진용 조성물을 제공한다.
또한, 본 발명은 상기 항암 효과 증진용 조성물을 유효성분으로 포함하는 약학적 조성물을 제공한다.

본 발명의 약학 조성물은 각각 통상의 방법에 따라 산제, 과립제, 정제, 캡슐제, 현탁액, 에멀젼, 시럽, 에어로졸 등의 경구형 제형, 외용제, 좌제 또는 멸균 주사용액의 형태로 제형화하여 사용할 수 있다. 상세하게는 제형화할 경우 통상 사용하는 충진제, 중량제, 결합제, 습윤제, 붕해제, 계면활성제 등의 희석제 또는 부형제를 사용하여 조제될 수 있다. 경구투여를 위한 고형 제제로는 정제, 환제, 산제, 과립제, 캡슐제 등을 포함하나, 이에 한정되는 것은 아니다. 이러한 고형 제제는 상기 유효성분 외에 적어도 하나 이상의 부형제, 예를 들면, 전분, 칼슘 카보네이트, 수크로오스, 락토오스, 젤라틴 등을 섞어 조제될 수 있다. 또한, 단순한 부형제 이외에 마그네슘 스테아레이트, 탈크 같은 윤활제들도 사용될 수 있다. 경구를 위한 액상물, 리퀴드 파라핀 이외에 여러 가지 부형제, 예를 들면 습윤제, 감미제, 방향제, 보존제 등을 첨가하여 조제될 수 있다. 비경구 투여를 위한 제제는 멸균된 수용액, 비수성 용제, 현탁제, 유제, 동결건조 제제 및 과제를 포함한다. 비수성 용제 및 현탁제로는 프로필렌글리콜, 폴리에틸렌글리콜, 올리브 오일과 같은 식물성 오일, 에틸올레이트와 같은 주사 가능한 에스테르 등이 사용될 수 있다. 좌제의 기제로는 위텝솔, 마크로솔, 트윈 61, 카카오지, 라우린지, 글리세로젤라틴 등이 사용될 수 있다.
본 발명의 약학 조성물의 적합한 투여량은 환자의 상태 및 체중, 질병의 정도, 약물형태, 시간에 따라 다르지만, 당 업자에 의해 적절하게 선택될 수 있는 바, 상기 조성물의 일일 투여량은 바람직하게는 0.001 mg/kg 내지 50 mg/kg이며, 필요에 따라 일일 1회 내지 수회로 나누어 투여할 수 있다.
또한, 본 발명은 암 질환이 의심되는 대상체로부터 분리된 시료에 후보 약물을 처리하는 제1단계; 상기 후보 약물이 처리된 시료에서 IL-2의 발현 또는 활성 수준을 측정하는 제2단계; 및 대조구 시료와 비교하여 상기 IL-2의 발현 또는 활성 수준을 증가시키는 후보 약물을 선별하는 제3단계; 를 포함하는 암세포 유래의 엑소좀 분비 및 PD-L1 발현에 대한 억제제 스크리닝 방법을 제공한다.
상기 제2단계의 IL-2의 발현 또는 활성 수준은 역전사 중합효소 연쇄반응(reverse transcription-polymerase chain reaction, RT-PCR), 효소면역분석법(enzyme-linked immunosorbent assay, ELISA), 방사능면역분석(radioimmunoassay), 면역조직화학(immunohistochemistry), 마이크로어레이(microarray), 웨스턴 블랏(western blotting) 및 유세포 분석법(FACS)으로 이루어지는 군에서 선택된 어느 하나 이상으로 측정할 수 있으나, 이에 제한되는 것은 아님을 명시한다.
또한, 본 발명은 IL-2, 이의 발현 촉진제 또는 활성화제를 대상체에 투여하는 단계를 포함하는 암세포 유래 엑소좀 분비 및 PD-L1 발현 억제 방법을 제공한다.
이하에서는 실시예를 통하여 본 발명을 더욱 상세히 설명하고자 한다. 이들 실시예는 오로지 본 발명을 보다 구체적으로 설명하기 위한 것으로, 본 발명의 요지에 따라 본 발명의 범위가 이들 실시예에 의해 제한되지 않는다는 것은 당업계에서 통상의 지식을 가진 자에 있어서 자명할 것이다.
실시예 1: 실시간 중합효소 연쇄반응을 이용한 흑색종 세포의 PD-L1 발현 분석
흑색종 세포와 IL-2 전처리 된 흑색종 세포로부터 mRNA (mRNA extraction kit, # 9767; TaKaRa)를 추출한 뒤, nanodrop (DS-11 Series Spectrophotometer; DeNovix)을 이용하여 mRNA의 농도를 측정하였다. 이후 100 ng의 mRNA로 cDNA (SuperScript III First-Strand Synthesis System, Ref. 18080-051; invitrogen)를 합성한 후, 실시간 중합효소 연쇄반응을 수행하여 mouse PD-L1(프라이머 세트: Forward: AGT TCC CAA AAC ATG AGG AT(서열번호 1), Reverse: CAC GTA CAA GTC CTT TGG AG(서열번호 2))와 human PD-L1(프라이머 세트: Forward: GGT CAT CCC AGA ACT ACC TC(서열번호 3), Reverse: ACG GAA GAT GAA TGT CAG TG(서열번호 4))의 유전자 발현 수준을 분석하였다.
그 결과, 도 1(A), 도 2(A)와 같이, 대조군 대비 IL-2 전처리된 흑색종 세포에서 PD-L1의 유전자 발현이 약 50% 감소하는 것을 확인하였다.
실시예 2: 엑소좀 정제
대조군 흑색종세포를 약 2 X 106 세포/웰 밀도로 접종하고, 우태아혈청이 첨가되지 않은 DMEM 배지에서 24 시간 배양하였다. 이후 엑소좀을 분리하기 위해, 각 세포에서 얻은 각 상등액을 300 x g, 2500 x g, 10,000 x g로 연속 원심분리하였다. 이어서, 상등액을 0.2 μm 주사기 필터로 여과하고, 120,000 x g에서 원심 분리하였다. 엑소좀 펠릿을 PBS에 재현탁하고 다시 120,000 x g에서 원심 분리하였다. 정제된 엑소좀 펠릿은 다음 실험을 위해 PBS 또는 1X 세포 용해 완충액에 재현탁하였다.
실시예 3: 정제된 엑소좀의 특성 분석
전자현미경을 사용하여 정제된 엑소좀의 특성을 확인하기 위해, PBS에 현탁한 정제된 엑소좀을 포름바 탄소 코팅 니켈 그리드 (formvar carbon-coated nickel grids) 위에 떨어뜨렸다. 2% 우라닐 아세테이트 (uranyl acetate)로 염색한 후, 그리드를 공기 건조시키고 HF-3300 (Hitachi) 투과 전자현미경을 사용하여 직경 구조를 가시화하였다.
그 결과, 도 3(A)과 같이, 50~150nm 크기의 엑소좀을 확인할 수 있었다.
실시예 4: 나노입자 추적을 이용한 흑색종 세포 유래 엑소좀 분석
흑색종 세포(B16F10: 5 X 104 세포)에 IL-2를 10 μg/ml 농도로 처리하고 48시간 동안 배양한 후, 배양 상층액으로부터 유래되는 엑소좀의 크기 및 농도를 fast video capture 및 입자 추적 소프트웨어가 장착된 Nanosight LM10(Malvern Instruments)을 사용하여 측정하였다.
그 결과, 도 3(B)와 같이, 대조군 대비 IL-2를 직접 처리한 흑색종 세포에서 엑소좀의 수가 감소하는 것을 나노입자 추적 분석으로 확인하였다.
실시예 5: 웨스턴 블랏을 이용한 흑색종 세포 유래 엑소좀 분석
상기 흑색 종 세포 유래 엑소좀 수의 감소를 재검증하기 위해, 엑소좀 분비 및 발현에 관여하는 Rab 단백질의 발현 수준을 웨스턴 블랏으로 측정하였다. 세포질 또는 엑소좀 단백질을 SDS-PAGE로 분리하고, 니트로셀룰로오스 막으로 이동시켰다. 이후, 각각의 일차 항체(ab18211; abcam/Rab5, ab50533; abcam/Rab7, ab55667; abcam/Rab 27a, # 4970; Cell signaling/β-actin, ab10931; abcam/PD-L1)와 배양하고, HRP가 접합된 이차 항체로 배양하였다. Enhanced chemiluminescence(ECL) 검출 시약(# 34095; Thermo Scientific, # RPN2209; GE Healthcare)을 사용하여 이미지를 시각화하고 ECL 하이퍼 필름(AGFA, Morstel)을 사용하여 정량화하였다.
그 결과, 도 5(C)와 같이, 대조군 대비 IL-2를 처리한 흑색 종 세포에서 Rab 27a의 단백질 발현이 현저하게 감소하는 것을 확인하였다.
더불어, 흑색종에서 IL-2의 효능을 평가하기 위해, 암세포의 면역회피에 가장 중요하다고 알려져 있는 PD-L1(programmed death ligand 1) 단백질의 발현을 측정하였다.
그 결과, 도 1(B), 도 2(B)와 같이, 대조군 대비 IL-2를 처리한 흑색종 세포에서 PD-L1의 단백질 발현이 현저하게 감소하는 것을 확인하였다. 이는 도 2(D)와 같이 면역염색법을 통해 재검증하였다.
다음으로 IL-2에 의한 흑색종 유래 exosomal PD-L1 발현 조절 여부를 평가하기 위하여 웨스턴 블랏과 유세포 분석법을 수행하였다.
그 결과, 도 4(A)와 같이, 대조군 대비 IL-2를 처리한 흑색종에서 exosomal PD-L1의 발현이 현저하게 감소하는 것을 웨스턴 블랏으로 확인하였으며, 도 4(B)와 같이 유세포 분석법을 이용하여 재검증하여 확인하였다.
실시예 6: 공동 배양 분석
IL-2에 의한 흑색종 세포의 PD-L1 감소와 더불어 세포독성 T 세포의 활성 증가에 의한 항암 효과를 증명하기 위해, 도 8(A) 및 9(A)와 같이, IL-2를 처리한 흑색종 세포(B16F10)와 세포독성 T 세포를 함께 공동 배양한 뒤 흑색종 세포의 생존율을 확인하였다. 세포 발광 유전자를 가진 흑색종 세포(B16F10-luc-g5)를 사용하였으며, IVIS spectrum(PerkinElmer)을 이용하여 분석하였다.
96 웰 플레이트에 흑색종 세포를 약 5 X 103 세포/웰 밀도로 접종하고 대조군으로 IL-2를 전처리한 흑색종 세포를 사용하였다. 배양 48시간 후, 세포를 상이한 비율의 CTLL-2 세포(1:1 내지 1:10)와 함께 배양하였다. 배양 종료 시, MTS 시약을 각 웰에 첨가하고 37℃에서 2 시간 동안 배양하였다. 각 웰의 상층액을 포함하는 MTS를 제거한 후, 마이크로플레이트 리더기를 사용하여 490 nm에서 흡광도를 측정하였다.
그 결과, 도 8(B)및 9(B)와 같이, 세포독성 T 세포와 IL-2를 전처리한 흑색종 세포의 공동 배양군에서 흑색종 세포의 생존율이 현저하게 감소하는 것을 확인하였다.
실시예 7: 흑색종 이식 마우스 모델에서 IL-2의 PD-L1발현 감소분석
B16F10 마우스 흑색종 세포 1 X 106을 balb/c 마우스 우측 옆구리에 피하주사하였다. 실험 시작일로부터 10일 후 6일간 매일 PBS, rhIL-2(50,000 IU/마리)를 암 조직에 직접 주사하였다. 이후 공학용 디지털 캘리퍼를 이용하여 2일에 한 번씩 마우스의 암 조직(장축*단축2*0.52)을 측정하여 비교하였다.
그 결과, 도 10과 같이, IL-2를 주입한 마우스의 암 성장은 PBS 대조군과 비슷한 크기로 성장하였다. 이후 마우스에서 적출되어진 암 조직의 무게 분석과 같은 결과를 보였다(도 10).
실시예 8: 흑색종 이식 마우스 모델에서 IL-2의 암 중 PD-L1발현 조절에 대한 분석
상기 실험의 마우스에서 암 조직을 적출한 뒤, 액체질소를 이용하여 급속냉각 후, RBC lysis buffer를 통해 조직 내 적혈구를 제거한 후, 550 x g에서 원심분리하여 세포을 얻었다. 이후 상등액을 300 x g, 2,500 x g, 10,000 x g로 연속 원심분리하였다. 이어서, 분리된 세포를 이용해 웨스턴 블랏 및 면역화학염색법을 이용해 PD-L1발현을 확인하였다.
그 결과 도 11와 같이, IL-2를 주입한 마우스의 PD-L1 발현양이 대조군에 비해 유의있게 감소됨을 확인하였다.
이상으로 본 발명의 특정한 부분을 상세히 기술한 바, 당업계의 통상의 지식을 가진 자에게 있어서 이러한 구체적인 기술은 단지 바람직한 구현 예일 뿐이며, 이에 본 발명의 범위가 제한되는 것이 아닌 점은 명백하다. 따라서, 본 발명의 실질적인 범위는 첨부된 청구항과 그의 등가물에 의하여 정의된다고 할 것이다.
본 발명의 범위는 후술하는 특허청구범위에 의하여 나타내어지며, 특허청구범위의 의미 및 범위 그리고 그 균등 개념으로부터 도출되는 모든 변경 또는 변형된 형태가 본 발명의 범위에 포함되는 것으로 해석되어야 한다.
Claims (10)
- 엑소좀 PD-L1의 발현에 대한 억제제를 유효성분으로 포함하는 항암 효과 증진용 조성물.
- 제1항에 있어서, 상기 억제제는 IL-2, 이의 발현 촉진제 또는 활성화제인 것을 특징으로 하는 항암 효과 증진용 조성물.
- 제2항에 있어서, 상기 IL-2 발현 촉진제 또는 활성화제는 IL-2 단백질에 특이적으로 결합하는 화합물, 펩타이드, 앱타머 및 항체로 이루어진 군에서 선택된 어느 하나인 것을 특징으로 하는 항암 효과 증진용 조성물.
- 제2항에 있어서, 상기 IL-2, 이의 발현 촉진제 또는 활성화제는 암세포 유래 엑소좀 분비 및 PD-L1의 발현을 감소시켜 면역 회피를 억제하고 항암 효과를 증대시키는 것을 특징으로 하는 항암 효과 증진용 조성물.
- 제1항에 있어서, 상기 암은 흑색종, 피부암, 폐암, 간암, 위암, 췌장암, 골암, 두부 또는 경부 암, 자궁암, 난소암, 유방암, 나팔관암종, 자궁내막암종, 자궁경부암종, 질암종, 음문암종, 호킨스씨병, 식도암, 소장암, 대장암, 결장암, 직장암, 항문부근암, 내분비선암, 갑상선암, 부갑상선암, 부신암, 연조직 육종, 요도암, 음경암, 전립선암, 만성 또는 급성 백혈병, 림프구 림프종, 방광암, 신장 또는 수뇨관암, 신장세포 암종, 신장골반 암종, 중추신경계 종양, 1차 중추신경계 림프종, 척수 종양, 뇌간신경교종 및 뇌하수체 선종으로 이루어진 군에서 선택된 것을 특징으로 하는 항암 효과 증진용 조성물.
- 제1항에 있어서, 상기 항암 효과 증진용 조성물은 하나 이상의 항암제와 병용 투여되는 것을 특징으로 하는 항암 효과 증진용 조성물.
- 제1항 내지 제6항 중 어느 한 항의 항암 효과 증진용 조성물을 유효성분으로 포함하는 약학적 조성물.
- 암 질환이 의심되는 대상체로부터 분리된 시료에 후보 약물을 처리하는 제1단계;상기 후보 약물이 처리된 시료에서 IL-2의 발현 또는 활성 수준을 측정하는 제2단계; 및대조구 시료와 비교하여 상기 IL-2의 발현 또는 활성 수준을 증가시키는 후보 약물을 선별하는 제3단계;를 포함하는 암세포 유래의 엑소좀 분비 및 PD-L1 발현에 대한 억제제 스크리닝 방법.
- 제8항에 있어서, 상기 제2단계의 IL-2의 발현 또는 활성 수준은 역전사 중합효소 연쇄반응(reverse transcription-polymerase chain reaction, RT-PCR), 효소면역분석법(enzyme-linked immunosorbent assay, ELISA), 방사능면역분석(radioimmunoassay), 면역조직화학(immunohistochemistry), 마이크로어레이(microarray), 웨스턴 블랏(western blotting) 및 유세포 분석법(FACS)으로 이루어지는 군에서 선택된 어느 하나 이상으로 측정하는 것을 특징으로 하는 암세포 유래 엑소좀 분비 및 PD-L1 발현 억제제 스크리닝 방법.
- IL-2, 이의 발현 촉진제 또는 활성화제를 대상체에 투여하는 단계를 포함하는 암세포 유래의 엑소좀 분비 및 PD-L1 발현을 억제하는 방법.
