このうち、好適に使用される塩として、メタンスルホン酸塩を挙げることができ、当該メタンスルホン酸塩としてより好適には、メタンスルホン酸と化合物Iのモル比が1:1の塩が挙げられる。斯かるメタンスルホン酸塩は、単一の結晶、2以上の結晶多形の混合物、非晶質、又はこれらの混合物のいずれかの形態で存在し得る。
したがって、化合物I又はその薬学的に許容される塩は、アビラテロン又はその誘導体と併用して投与される場合において、優れた抗腫瘍効果、好ましくは腫瘍の再発や転移の抑制効果を発揮する抗腫瘍剤、より好ましくは骨転移を伴う去勢抵抗性前立腺癌を予防又は治療するための抗腫瘍剤として使用でき、あるいは去勢抵抗性前立腺癌の骨転移抑制剤として使用できる。
また、化合物I又はその薬学的に許容される塩は、アビラテロン又はその誘導体の抗腫瘍効果、特に骨転移を伴う去勢抵抗性前立腺癌に対する抗腫瘍効果を増強する抗腫瘍効果増強剤として使用できる。さらに、アビラテロン又はその誘導体は、化合物I又はその薬学的に許容される塩の抗腫瘍効果、特に骨転移を伴う去勢抵抗性前立腺癌に対する抗腫瘍効果を増強する抗腫瘍効果増強剤として使用できる。
また、腫瘍の発生臓器の種類も特に制限されず、例えば、腫瘍としては耳下腺癌等の頭頚部癌、食道癌、胃癌、十二指腸癌、結腸癌、直腸癌、肝臓癌、胆嚢・胆管癌、胆道癌、膵臓癌、肺癌、乳癌、卵巣癌、卵管癌、子宮頸癌、子宮体癌、腎癌、副腎癌、膀胱癌、前立腺癌、尿路上皮癌、精巣腫瘍、骨・軟部肉腫、血液癌、多発性骨髄腫、皮膚癌、脳腫瘍、及び中皮腫が挙げられる。好ましくは治療対象となる腫瘍は前立腺癌であり、より好ましくは、去勢抵抗性前立腺癌である。
癌の転移は、その原発巣や癌の特徴によって転移する部位が異なるが、本発明においては原発巣の癌としては、上記の癌のうち前立腺癌が挙げられ、より好ましくは去勢抵抗性前立腺癌である。また、この場合、原発巣から癌が転移する部位は主として骨であり、本願発明の抗腫瘍剤は、去勢抵抗性前立腺癌の骨転移の抑制に特に有用である。去勢抵抗性前立腺癌の治療において、去勢により原発巣となっている前立腺癌そのものを除去する、又は薬剤により去勢状態を維持することがある。この場合、転移した部位における抗腫瘍効果をもって、去勢抵抗性前立腺癌の抗腫瘍効果を評価する。
従って、本発明は、アビラテロン誘導体を併用することを特徴とする化合物I又は薬学的に許容される塩の腫瘍の転移抑制剤と言うことができ、好ましくは腫瘍の骨転移抑制剤であり、より好ましくは前立腺癌の骨転移抑制剤であり、より好ましくは去勢抵抗性前立腺癌の骨転移抑制剤である。
また、本発明における抗腫瘍効果増強剤は、一方の薬剤が抗腫瘍効果を持つ場合には、他方の薬剤の抗腫瘍作用を増強する。
本発明における式(I)で示される化合物又はその薬学的に許容される塩とアビラテロン又はその誘導体とが併用投与される場合、さらに他の抗腫瘍剤が投与されてもよい。当該他の抗腫瘍剤は、抗腫瘍作用を持つものであれば特に制限はない。
また、アビラテロン又はその誘導体、及び化合物I又はその薬学的に許容される塩は、それぞれ別個の製剤としても、又は配合剤としても提供することができる。さらに、アビラテロン又はその誘導体、及び化合物I又はその薬学的に許容される塩は、これらを単独で含む別々の製剤とするキット製剤としても提供することができる。
また、本発明において、化合物I又はその薬学的に許容される塩は、毎日投与しても、癌腫や副作用等により適宜休薬期間を設けてもよい。好ましくは、化合物I又はその薬学的に許容される塩を5日間投与して、2日間休薬する。

特許文献1に記載の方法によって得られた化合物Iを、特許文献5に記載の方法により化合物Iのメタンスルホン酸塩を得た。なお、製剤化については、特許文献6に記載の方法に準じて製造することができる。
市販又は文献公知の情報により、酢酸アビラテロンを得た。
骨転移を伴う去勢抵抗性前立腺癌患者を対象に化合物Iのメタンスルホン酸塩、及び酢酸アビラテロンを投与し、有効性及び副作用を評価した。ドセタキセル治療前の2次内分泌療法を受けている患者を対象に、1日1回化合物Iのメタンスルホン酸塩をフリー体換算で3用量(200,300,400 mg/body/day)を5日間投与し、2日間化合物Iのみを投与しないスケジュールを繰り返すよう投与した。なお、この期間は、いずれの用量群においても1日1回酢酸アビラテロン1000mgを毎日投与し、各用量群は6名で検討した。なお、各用量群の化合物Iの投与開始から21日目において化合物Iによる「対症療法により軽快又は回復が認められない継続するグレード2以上の副作用、又はグレード3以上の副作用(臨床検査値異常は除く)」を2名以上に認めた場合は、Steering Committeeにおいて当該用量群あるいはその用量を超える群での登録・評価の中止等を検討することとした。なお、この登録・評価の中止等の判断となる副作用には、アラニンアミノトランスフェラーゼ(ALT)増加、アスパラギン酸アミノトランスフェラーゼ(AST)増加、血中クレアチンキナーゼ(CK)増加などの臨床検査値異常は含まれていない。また,各用量群の評価完了後、Steering Committeeにおいて副作用及び有効性評価のための症例追加の可否を判断し、忍容性が確認された用量で本コホート全体の評価例数が24名を超えない範囲で症例追加を可能とした。そのため、各用量群の評価症例数は6例と少数であることから、有効性及び、副作用、投与継続性の観点から、300mg/body/dayで6例の追加評価を行った。
さらに、図2に各用量群の化合物Iとアビラテロンを投与できた日数を示した。即ち、投与日数が長いほど、抗腫瘍効果が期待できる上、より安全に投与できる用量であることを示す。また、「継続中」の記載は、本願の明細書等の作成時に当該併用試験が継続中であることを示すが、この時点における各用量群の平均投与日数を算出したところ、200mg用量群では211日、300mg用量群では158日、400mg用量群では87日であった。これより、いずれの用量群における平均投与日数が50日を超え、特に200mg及び300mg用量群の平均投与日数が100日を超えたことが示された。
さらに、図2で示す結果が得られた時点から約1年経過した時点における、各用量群の化合物Iとアビラテロンを投与できた日数を図3に示した。即ち、投与日数が長いほど、抗腫瘍効果が期待できる上、より安全に投与できる用量であることを示す。また、「継続中」の記載は、本願の明細書等の作成時に当該併用試験が継続中であることを示すが、この時点における各用量群の平均投与日数を算出したところ、200mg用量群では225日、300mg用量群では225日、400mg用量群では87日であった。これより、いずれの用量群における平均投与日数が50日を超え、特に200mg及び300mg用量群の平均投与日数が200日を超えたことが示された。
このようにして、化合物Iとアビラテロン誘導体の併用により抗腫瘍効果が発揮できることが示された。

- アビラテロン又はその誘導体が酢酸アビラテロンである、請求項1又は2に記載の抗腫瘍剤。
- ヒトに投与される、請求項1~3に記載の抗腫瘍剤。
- 去勢抵抗性前立腺癌を治療するための、請求項1~4のいずれか1項に記載の抗腫瘍剤。
- 骨転移を伴う去勢抵抗性前立腺癌を治療するための、請求項1~5のいずれか1項に記載の抗腫瘍剤。
- 式(I)で示される化合物又はその薬学的に許容される塩を化合物Iのフリー体として1日1回200mg~400mgを投与することを特徴とする、請求項1~6のいずれか1項に記載の抗腫瘍剤。
- 式(I)で示される化合物又はその薬学的に許容される塩を化合物Iのフリー体として1日1回300mgを投与することを特徴とする、請求項1~6のいずれか1項に記載の抗腫瘍剤。
- 式(I)で示される化合物又はその薬学的に許容される塩を5日間投与し、その後2日間休薬を行うことを特徴とする、請求項1~8のいずれか1項に記載の抗腫瘍剤。
