Abstract
Description
特許文献1には、患者に関係する情報を受け取り、この情報を基に薬剤調合済み、すなわち、できあがっている医薬品の混合を制御する個別化された医薬品の生産のためのシステム及び方法が開示されている。
まず、図1~図12を参照して、本発明の第1実施形態について説明する。
<システム構成>
図1は、第1実施形態に係る医薬品製造支援システムの構成例を示す図であり、図2は、本実施形態に係る配送車の構成例を示す図である。
医薬品製造支援システムZは、中央管理装置(管理装置)1と、医薬品製造工場20に設置されているプラント制御装置2と、配送車8に設置されているナビゲーション装置(経路探索装置)3と、温度管理装置4とを有している。
中央管理装置1は、患者の検査結果を基に生成された分析データ91を取得する。また、中央管理装置1は、取得した分析データ91を基に、医薬品製造工場20での医薬品製造プラントを制御するためのプラント制御情報92を生成する。また、中央管理装置1は、配送車8における医薬品の保管温度に関する情報である管理温度情報93や、交通状況に関する情報である交通情報94や、製造した医薬品の保管場所に関する情報である保管場所情報95を生成する。そして、中央管理装置1は、生成したプラント制御情報92を医薬品製造工場20のプラント制御装置2へ送信したり、配送車8のナビゲーション装置3へ交通情報94等を送信したり、温度管理装置4へ管理温度情報93を送信したりする。
ナビゲーション装置3は、医薬品を配送するための配送車8に設置されており、中央管理装置1から取得した交通情報94や、保管場所情報95に基づいて、最適な配送ルートを検索する。
温度管理装置4は、医薬品を配送するための配送車8に設置されており、中央管理装置1から取得した管理温度情報93に基づいて、配送車8における医薬品保管庫の温度を制御する。
まず、病院10等の検査機関で患者が検査を受けると、病院10は検査結果を分析データ91としてDB(Data Base)管理会社30に送信する。DB管理会社30では、図示しないDB等に分析データ91を保存するとともに、中央管理装置1に分析データ91を送信する。
前記したように、中央管理装置1は、取得した分析データ91を基に、プラント制御情報92や、管理温度情報93や、保管場所情報95を生成する。また、前記したように、中央管理装置1は、交通情報94も生成する。
そして、中央管理装置1は、参照結果を基に適正な医薬品を選択し、この医薬品を製造するために必要な医薬品製造プラントの制御情報であるプラント制御情報92を生成する。
プラント制御情報92には、医薬品製造プラントにおける温度、pH、溶存酸素、栄養濃度等の情報が格納されている。さらに、中央管理装置1は、分析データ91に含まれる、患者に投与が必要な日時、量を基に、優先順位を算出し、この優先順位に基づいて、医薬品が製造されるように医薬品製造のタイムスケジュールを生成する。そして、中央管理装置1は、生成したタイムスケジュールに関する情報をプラント制御情報92に付加する。
次に、中央管理装置1は、生成したプラント制御情報92を医薬品製造工場20のプラント制御装置2に送信するとともに、管理温度情報93を配送車8に設置されている温度管理装置4に送信する。
医薬品製造工場20に設置されているプラント制御装置2では、中央管理装置1から通信網を介してプラント制御情報92を受信すると、受信したプラント制御情報92を基に医薬品製造プロセス制御を行う。ただし、分析データ91は個人情報を含むため、中央管理装置1から外部にデータが送信される際、中央管理装置1の情報変換部117によって、個人情報を含まない形式にデータが変更される。医薬品製造工場では医薬品ごとに適切な製造プロセスが異なる場合があり、詳細は後記する。
また、中央管理装置1は、分析データ91と、医薬品の管理温度、割れ物、医薬品の使用期限等といった輸送条件や、配送車8に搭載されている医薬品保管庫の種類等を基に、配送車8の配車を決定する。そして、中央管理装置1は、配車情報96を医薬品製造工場20や、製薬会社40等に設置されている配車情報表示装置5(図7)へ送信する。
医薬品を輸送する際には適切な温度管理が必要となる。
本実施形態における医薬品製造支援システムZでは、製造される医薬品の出荷予定日、出荷量等がわかるため、出荷予定の医薬品の輸送環境を把握することができる。輸送環境に応じて準備する配送車8も異なるので、中央管理装置1によって手配が行われる。つまり、配送情報による配送車8の配車が行われる。医薬品の輸送温度管理は、室温、冷所、冷蔵、冷凍の4つに区分される。室温は30度以下のため、国内の多くの地域では、夏場の冷房は必須である。また、冬場には、凍結の恐れがあるため、暖房を用いて温める必要がある。医薬品に対する温度管理は、物流現場の労働環境にも影響を及ぼす。
実際の温度管理は、エアコン(エアーコンディショナ)を一定温度に設定しておくだけでなく、配送車8に備えられている温度計により測定されるのが望ましい。このとき、温度の記録計は、紙に記載するアナログ式であっても構わないし、デジタル温度計を用いて、常時、温度をモニタリングしても構わない。
また、医薬品の物流業務では、医薬品という製品の特性上、安定的な供給網の維持が不可欠である。つまり、大震災等が生じても、敏速かつ安定的な供給体制の構築が必要とされる。本実施形態に係る医薬品製造支援システムZは、交通情報94センタから取得した道路状況の情報等を基に、ナビゲーション装置3が最適な配送ルートを検索、特定する。これにより、本実施形態に係る医薬品製造支援システムZは、大震災時等の災害時において、災害状況を把握し、輸送可能な流通経路を導きだすことができる。
<中央管理装置>
図3は、本実施形態に係る中央管理装置の構成例を示す機能ブロック図である。
中央管理装置1は、メモリ110、記憶装置120、NIC(Network Interface Card)等の送受信装置130、CPU(Central Processing Unit)140等を有している。
メモリ110には、記憶装置120に格納されているプログラムが展開され、CPU140によって実行されることで、処理部111及び処理部111を構成する情報取得部112、情報生成部113、配車検索部114、保管場所検索部115、送信処理部116、情報変換部117を有している。
情報取得部112は、分析データ91等を外部から取得する。
情報生成部113は、取得した分析データ91等を基に、プラント制御情報92を生成したり、管理温度情報93や交通情報94等を生成したりする。
配車検索部114は、分析データ91を基に配車を検索し、決定する。
保管場所検索部115は、分析データ91を基に空いている倉庫等の保管場所を検索する。
送信処理部116は、プラント制御情報92や、管理温度情報93や、交通情報94や、保管場所情報95を、プラント制御装置2や、ナビゲーション装置3や、温度管理装置4に送信する。
情報変換部117は、外部へ送信する情報を、患者の個人情報を参照不可能な情報へ変換する。具体的には、外部へ送信する情報から患者の個人情報を削除する。
図4は、本実施形態に係るプラント制御装置の構成例を示す機能ブロック図である。
プラント制御装置2は、メモリ210、NIC等の送受信装置230、CPU240等を有している。また、プラント制御装置2には、培養槽における糖鎖濃度と、培養槽における環境情報とが対応付けられている培養環境情報等が格納されているデータベース220が接続されている。
メモリ210には、データベース220に格納されているプログラムが展開され、CPU240によって実行されることで、処理部211及び処理部211を構成する情報取得部212、糖鎖モニタリング部213、培養環境制御部214及び細胞株構築部215を有している。
情報取得部212は、プラント制御情報92等を中央管理装置1から取得する。
糖鎖モニタリング部213は、培養槽において生成された抗体タンパクに含まれる糖鎖の濃度(糖鎖濃度)を監視している。また、糖鎖モニタリング部213は、糖鎖濃度の監視結果をキーとして、データベース220に格納されている培養環境情報を参照し、この培養環境を培養環境制御部214に送る。
培養環境制御部214は、糖鎖モニタリング部213から送られた培養環境情報等に基づいて、培養槽の環境を制御する。
細胞株構築部215は、後記する高薬効産生細胞株の生成を制御する。
図5は、本実施形態に係るナビゲーション装置の構成例を示す機能ブロック図である。
ナビゲーション装置3は、メモリ310、記憶装置320、NIC等の送受信装置330、CPU340等を有している。
メモリ310には、記憶装置320に格納されているプログラムが展開され、CPU340によって実行されることで、処理部311及び処理部311を構成する情報取得部312、経路検索部313を有している。
情報取得部312は、交通情報94や、保管場所情報95等を中央管理装置1から取得する。
経路検索部313は、中央管理装置1から取得した保管場所情報95や、交通情報94等を基に、目的地までの経路検索を行う。
図6は、本実施形態に係る温度管理装置の構成例を示す機能ブロック図である。
温度管理装置4は、メモリ410、記憶装置420、NIC等の送受信装置430、CPU440等を有している。
メモリ410には、記憶装置420に格納されているプログラムが展開され、CPU440によって実行されることで、処理部411及び処理部411を構成する情報取得部412、温度制御部413を有している。
情報取得部412は、管理温度情報93等を中央管理装置1から取得する。
温度制御部413は、中央管理装置1から取得した管理温度情報93を基に、配送車8に備えられている医薬品保管庫(不図示)の温度を制御する。
図7は、本実施形態に係る配車情報表示装置の構成例を示す機能ブロック図である。
配車情報表示装置5は、メモリ510、記憶装置520、NIC等の送受信装置530、CPU540、ディスプレイ等の表示装置550等を有している。
メモリ510には、記憶装置520に格納されているプログラムが展開され、CPU540によって実行されることで、処理部511及び処理部511を構成する情報取得部512、表示処理部513を有している。
情報取得部512は、送受信装置530を介して配車情報96を中央管理装置1から取得する。
表示処理部513は、中央管理装置1から取得した配車情報96の内容を表示装置550に表示することで、ユーザに配車情報96の内容を提示する。
ユーザは表示装置550に表示された配車情報96の内容に従って、配送車8を配車する。
図8は、本実施形態に係る医薬品製造管理システムの動作手順を示す図である。適宜、図1~図6を参照する。
まず、中央管理装置1の情報取得部112が、DB管理会社30から検査の分析データ91を取得する(S101)。
そして、中央管理装置1の情報生成部113は、取得した分析データ91を基に、プラント制御情報92を生成する(S102)。
次に、中央管理装置1の情報変換部117が生成したプラント制御情報92から患者の個人情報を削除する情報変換を行う(S103)。
また、中央管理装置1の送信処理部116は、情報変換されたプラント制御情報92(情報)をプラント制御装置2に送信する(S104)。
そして、プラント制御装置2の培養環境制御部214や、糖鎖モニタリング部213が、プラント制御情報92に従って、医薬品製造プラントの各装置(培養槽等)を制御するプラント制御を行う(S202)。プラント制御については後記する。
また、中央管理装置1の情報取得部112は交通センタ(不図示)から交通情報94を取得し、情報生成部113が取得した交通情報94を、ナビゲーション装置3へ送る交通情報94とする。
さらに、中央管理装置1の情報生成部113は、分析データ91を基に、医薬品を配送する際における保管温度の情報である管理温度情報93を生成する。
また、配車検索部114は、取得した分析データ91と、予め記憶装置120に格納されている輸送条件とを基に、配送車8の配車を検索し、決定する。ここで、輸送条件とは、医薬品の管理温度、割れ物、医薬品の使用期限等である。そして、情報生成部113は、決定した配送車8の配車に関する情報を、配車情報96とする。配車情報96には、使用する配送車8の識別番号等が格納されている。
このようにして、中央管理装置1の情報生成部113は、保管場所情報95、交通情報94、管理温度情報93、配車情報96といった各種情報を生成する(S111)。
その後、中央管理装置1の情報変換部117は、生成した保管場所情報95、交通情報94、管理温度情報93、配車情報96から患者の個人情報を削除する情報変換を行う(S112)。
そして、中央管理装置1の送信処理部116は、配送車8のナビゲーション装置3に、交通情報94、保管場所情報95といった情報を送信する。また、中央管理装置1の送信処理部116は、温度管理装置4に管理温度情報93といった情報を送信する。さらに、中央管理装置1の送信処理部116は、医薬品製造工場20や、製薬会社40に配車情報96といった情報を送信する。このようにして、情報送信部116は、各種情報を送信する(S113)。
なお、中央管理装置1から送信された配車情報96は、配車情報表示装置5に入力され、配車情報表示装置5の表示処理部513が表示装置550に配車情報96の内容を表示するが、この処理は図8では省略している。
個別化医療における医薬品製造では、より高い品質の医薬品の製造が要求される。個別化医療のための分子標的薬として最も期待されている抗体タンパクは、一般的に動物細胞を培養することで製造されることが多い。このような抗体タンパクは、培養される動物細胞内で糖鎖と呼ばれる分子の修飾が行われることにより医薬品の薬効、品質が変化する。
医薬品製造工場20では、まず、高薬効産生細胞が構築され、そこに抗体医薬遺伝子が導入される。ここで、高薬効産生細胞は、本実施形態に係る医薬品の生成に必要な糖鎖修飾を促進する遺伝子が組み込まれた動物細胞である。以下、高薬効産生細胞を細胞株と称することがある。
抗体医薬遺伝子は、生成対象となる医薬品を生成する遺伝子である。
医薬品製造工場20(図4)に設置されている医薬品製造プラントによる工程は、通常、図10に示すように培養槽等による培養工程や、精製装置610による精製工程等で構成されている。個別化医療向けの医薬品製造では細胞株と培養プロセスが密に連携した関係にあるため、培養槽601~604や、精製装置610の他に、消耗品としての細胞株と、その細胞株に適合した培養プロセス装置が含まれる。
図10に示すように、プラント制御装置2は、培養槽におけるタンク601~604等の制御を行っている。
本実施形態に用いられる医薬品に含まれる抗体タンパク701は、その一部に糖鎖702が結合した構造となっている。
このような抗体タンパク701は、細胞株711に抗体タンパク701を生成する制御責任遺伝子である抗体医薬遺伝子712が導入されることで生成される。
このような抗体医薬遺伝子712の同定は患者の表現型に対して、表現型‐遺伝型解析によって行われる。
抗体医薬遺伝子712による医薬品の生成には、遺伝子組換えによる細胞株内での糖鎖修飾反応の効率化が重要である。まずは、表現型-遺伝型解析による抗体医薬遺伝子712の同定が細胞株構築部215によって行われ、その後、細胞株構築部215によって構築された細胞株711に対して、抗体医薬遺伝子712をターゲットとした遺伝子組み換えをユーザが行うことで、抗体医薬遺伝子712の導入が行われる。
このような制御を行うことで、ばらつきの少ない、均一な糖鎖修飾が可能となり、均質な医薬品の製造が可能となる。
図12は、抗体医薬品における糖鎖修飾の反応経路の一例を示す図である。
図12において、A1~A21処理は以下の処理を意味している。ここで、プラント制御装置2は、図12に示すA1~A21処理のうち、四角で囲まれた処理の制御を行う。
(遺伝子制御)
(A1)GntI-/- Lec1 Mutant
細胞内のGntI遺伝子とLec1遺伝子とがノックアウトされる。これにより高マンノース型糖鎖が抗体タンパクに付加される。抗体依存性細胞傷害(ADCC)活性が抑制されたり、半減期が短くなったりする。
(A2)β(1,4)-GalT+
細胞内のβ(1,4)-GalT遺伝子がノックインされる。N-アセチルグルコサミン(GlcNAc)の末端が削減される。
(A3)α(2,3)‐Sialyltransferase+
細胞内のα(2,3)‐Sialyltransferase(シアル酸転移酵素)遺伝子がノックインされる。糖鎖へのシアル酸付加が行われる。
(A4)CMP-sialic acid transporter +
細胞内のCMP-sialic acid transporter遺伝子がノックインされ、シアル酸が増大される。
(A5)Gnt III+ Mutant
細胞内のGnt III遺伝子がノックインされる。N-アセチルグルコサミン(GlcNAc)が二分され、抗体依存性細胞傷害(ADCC)活性が増大される。
(A6)Fut8-/- Mutant
細胞内のFut8遺伝子がノックアウトされ、フコシル化が行われる。
(A7)antisense RNA sialidase
細胞内のantisense RNA sialidaseがノックインされ、細胞外シアル化酵素活性が抑制される。
(A8)Sialidase activity
細胞外シアリダーゼの増大によりシアル化が抑制される。
(A9)低濃度グルコース又は低濃度グルタミン(1mM未満)の添加。
(A10)低溶存酸素濃度(10%未満)変化。
(A11)pH濃度変化(pH6.8-7.8)。
(A12)マンガンの添加。
(A13)酪酸ナトリウムの添加。

前記したように、特許文献1に記載の技術は、この情報を基に薬剤調合済み、すなわち、できあがっている医薬品の混合を制御するため、製造される医薬品の多様性に限界がある。
これに対し、本実施形態に係る医薬品製造支援システムZは、医薬品を生成する細胞株の培養環境の制御を行うプラント制御装置2を備えて医薬品を製造する医薬品製造工場20におけるプラント制御装置2と、外部から入力された情報を基に、プラント制御装置2を遠隔制御する中央管理装置1と、を、有している。このようにすることで、本実施形態に係る医薬品製造支援システムZは、多様な医薬品の製造を行うことができる。
さらに、本実施形態に係る医薬品製造支援システムZにおいて、中央管理装置1は、外部へ情報を送信する際、外部から前記患者の個人情報を参照できないよう、前記情報の変換を行う。このようにすることで、患者の個人情報を守ることができる。
また、本実施形態に係る医薬品製造支援システムZにおいて、中央管理装置1は、医薬品を配送する際における医薬品の保管温度に関する情報である管理温度情報93を送信し、送信された管理温度情報93を基に、医薬品の保管温度を制御する温度制御装置を医薬品製造支援システムZは有している。このようにすることで、人手をかけることなく医薬品を適切な温度で保管することが可能となる。
図13は、第2実施形態に係る医薬品製造支援システムの構成例を示す図である。
なお、図13において、図1と同様の構成要素については、同一の符号を付して説明を省略する。
図13に示す医薬品製造支援システムZaでは、薬局50に中央管理装置1が設置されている。つまり、DB管理会社30から薬局50の薬剤師に直接分析データ91が送られる。なお、DB管理会社30から送られる分析データ91は、個人情報が参照されない形式となっていることが望ましい。
その後の処理は、第1実施形態と同様である。
図14は、第3実施形態に係る医薬品製造支援システムの構成例を示す図である。
なお、図14において、図1と同様の構成要素については、同一の符号を付して説明を省略する。
図14に示す医薬品製造支援システムZbでは、図1における製薬会社40が省略されており、医薬品製造工場20から直接薬局50へ、製造された医薬品が配送されている。つまり、途中の卸業者(製薬会社40)が介入せず、医薬品製造工場20から薬剤師を経由して患者に医薬品が処方される。
また、各実施形態において、制御線や情報線は説明上必要と考えられるものを示しており、製品上必ずしもすべての制御線や情報線を示しているとは限らない。実際には、ほとんどすべての構成が相互に接続されていると考えてよい。
